1. Alcohol and other substance use disorder recovery during pregnancy among patients with posttraumatic stress disorder symptoms: A qualitative study

Abstract

Background: About 5% of women are pregnant at substance use disorder (SUD) treatment entry, and pregnant women with SUD often belong to marginalized groups experiencing social, economic, and health care barriers associated with stigma from prenatal substance use. Pregnant women in SUD treatment have high rates of trauma and posttraumatic stress disorder (PTSD). This study sought to (1) examine the lived experiences of pregnant individuals with PTSD symptoms in SUD treatment and (2) understand the roles of systematic or contextual barriers to the pursuit of prenatal abstinence. Methods: We draw upon in-depth semi-structured interviews to examine relationships between SUD, psychological trauma/PTSD experience, social resources, and lived experiences among patients in prenatal SUD treatment with PTSD symptoms. Our sample was pregnant patients (N = 13) with prior DSM-5 Criterion A trauma and current PTSD symptoms enrolled in a comprehensive program integrating prenatal care, substance use counseling, medication for opioid use disorder and case management at three sites affiliated with an urban academic medical center in New Mexico. Results: Using thematic analysis, four main themes identified structural forces influencing alcohol and drug use: (a) lack of access or ability to obtain resources, (b) substance use to cope with negative affect, (c) social stigma, and (d) interpersonal relationships. Conclusions: Despite receiving high-quality integrated prenatal and SUD care, these pregnant patients with PTSD symptoms in SUD treatment still experienced substantial social and structural hurdles to achieving abstinence during pregnancy.

O Pará detém uma rica diversidade de ecossistemas. Contudo, é um dos Estados que mais contribuem para o desmatamento na Amazônia. Atualmente, 22% do Estado foram desflorestados. Uma nova política de ocupação está sendo estudada para a Amazônia, baseada na criação de novos Estados. A criação de novos Estados pode aumentar o desmatamento na Amazônia, especialmente em regiões onde a fronteira agropecuária e mineraria é intensa como no Pará. Este estudo compara as mudanças da representatividade das áreas protegidas e do desmatamento, considerando a proposta de divisão do Estado do Pará em três novos Estados. A criação dos novos Estados pode levar a uma diminuição ou mesmo eliminação de algumas unidades de conservação, o que terá como consequência direta o aumento do desmatamento. Outra consequência grave da criação de novos Estados será a extinção do Zoneamento Ecológico-Econômico do Pará, um importante instrumento de políticas públicas. A criação de novos Estados deve ser precedida de estudos que envolvam uma avaliação criteriosa dos impactos ambientais, sociais e econômicos. Umas das consequências mais graves se isso não for levado em consideração é a criação de um vácuo jurídico que será aproveitado para aumentar a pressão nos recursos naturais da Amazônia.The state of Pará has a rich diversity of ecosystems. However, it is one of the states that the most contribute with the deforestation of Amazonian. Actually, 22% of the state was deforested. Currently, a new policy of occupation is being studied for the Amazonian, based on the creation of new states. The creation of new states can increase deforestation in the Amazonian, especially in regions where the agricultural frontier and minerals activities are intense such as the state of Para. This study compares the changes of the representativeness of protected areas and deforestation, considering the proposed of division of the Para in three new states. The creation of new states can lead to a reduction or elimination of the protected areas resulting in the increase of the deforestation. Another serious consequence of the creation of new states will be the extinction of the Ecological-Economic Zoning of Para, an important instrument of public policy. The creation of new states must be preceded by studies involving the evaluation of the environmental, social and economic variables. One of the most serious consequences if it is not taken into consideration is the creation of a legal vacuum that will be used to increase the pressure on natural resources of the Amazon.

Abstract

Background Substance use disorder (SUD) during pregnancy requires efficacious interventions based on understanding the ebb and flow of risk and protective factors for substance use across time. To assess how these fluctuations are associated temporally with substance use, we used ecological momentary assessment (EMA) to evaluate substance use risk (posttraumatic stress disorder [PTSD] symptoms) and protective (prenatal fetal bonding) factors and their associations with prenatal substance use recorded in real time. Methods Pregnant women in SUD treatment (N = 33) with prior trauma exposure received smartphones with an EMA application that queried them thrice daily for 28 days about PTSD symptoms, prenatal bonding, and substance use. Results Nearly all (N = 32) provided EMA data resulting in 2049 EMA reports (74% compliance). Most participants reported tobacco (72%), alcohol (22%), heroin (41%), and/or other illicit drug (6%–31%) use at least once via EMA. There were moderate associations (average β = 0.23) between greater daily peak PTSD symptoms and substance use with significant effects on illicit drug (β = 0.37), cannabis (β = 0.35) and cigarette use (β = 0.24). Prenatal bonding subscales were modestly associated with substance use, with daily intensity of attachment low point associated with lower heroin (β=-0.34), but higher alcohol (β = 0.24) use. Quality of attachment low point was associated with higher cigarette use (β = 0.06). Conclusions Despite the SUD severity and social instability of this sample, we observed high rates of compliance. We found preliminary support suggesting daily PTSD symptoms as a risk factor and less consistent support for prenatal bonding as a protective factor for prenatal substance use.

Abstract

Background Although not routinely assessed, prenatal posttraumatic stress disorder (PTSD) is associated with poor maternal mental health and mother-infant bonding. Prenatal PTSD may also be associated with birth weight and gestational age outcomes, but this remains unclear. This systematic review and meta-analysis investigated the association of prenatal PTSD with risk of low birth weight (LBW) or preterm birth (PTB) (dichotomous medically-defined cut-offs) or with birth weight (BW) or gestational age (GA) (continuous variables). Methods A comprehensive literature search was conducted in Web of Science, MedLine, PubMed, and PsychInfo. Data were collected and processed according to Meta-analysis of Observational Studies in Epidemiology (MOOSE) guidelines. Study quality was assessed with the Newcastle-Ottowa Quality Assessment Scale. Pooled effect sizes were estimated with random-effects models (correlation for continuous and odds ratios for dichotomous outcomes). Results Sixteen studies with 51,470 participants (prenatal PTSD 8%) were included in 4 meta-analyses. Maternal prenatal PTSD was associated with higher risks of LBW (OR = 1.96; 95% CI, 1.26, 3.03; P = .003), PTB (OR = 1.42; 95% CI, 1.16, 1.73; P = .001), and reduced GA (r = -0.04; 95% CI, -0.06, -0.01; P = .002). Limitations Different designs across studies, variety of PTSD assessment practices, and a small pool of studies were noted. Conclusions Findings suggest prenatal PTSD presents increased risks of LBW, PTB, and reduced GA. Evidence of physical harm to neonates from prenatal PTSD provides a powerful rationale to increase prenatal PTSD screening and identify effective prenatal interventions to improve maternal and child outcomes.

Abstract

There are significant barriers in engaging pregnant and postpartum women that are considered high-risk (e.g., those experiencing substance use and/or substance use disorders (SUD)) into longitudinal research studies. To improve recruitment and retention of this population in studies spanning from the prenatal period to middle childhood, it is imperative to determine ways to improve key research engagement factors. The current manuscript uses a qualitative approach to determine important factors related to recruiting, enrolling, and retaining high-risk pregnant and postpartum women. The current sample included 41 high-risk women who participated in focus groups or individual interviews. All interviews were analyzed to identify broad themes related to engaging high-risk pregnant and parenting women in a 10-year longitudinal research project. Themes were organized into key engagement factors related to the following: (1) recruitment strategies, (2) enrollment, and (3) retention of high-risk pregnant and parenting women in longitudinal research studies. Results indicated recruitment strategies related to ideal recruitment locations, material, and who should share research study information with high-risk participants. Related to enrollment, key areas disclosed focused on enrollment decision-making, factors that create interest in joining a research project, and barriers to joining a longitudinal research study. With regard to retention, themes focused on supports needed to stay in research, barriers to staying in research, and best ways to stay in contact with high-risk participants. Overall, the current qualitative data provide preliminary data that enhance the understanding of a continuum of factors that impact engagement of high-risk pregnant and postpartum women in longitudinal research with current results indicating the need to prioritize recruitment, enrollment, and retention strategies in order to effectively engage vulnerable populations in research.

Abstract

A 2019 NM-based pilot research project funded by NIH (Grant # U54 MD004811 – UNM TREE Center) suggests that pregnant patients receiving substance use disorder treatment may greatly benefit from access to experienced doulas providing trauma-informed and addiction-informed support. The results from the pilot also suggest that both doulas and medical providers are eager to facilitate access to doula care for these pregnant individuals.

Abstract

Pregnant women with substance use disorder (SUD) comprise an underserved population with complex treatment needs, including complications from trauma histories and comorbid psychological disorders. Using ecological momentary assessment, we examined momentary fluctuations in posttraumatic stress disorder (PTSD) symptoms, prenatal bonding, and substance craving, among pregnant women in SUD treatment who had a history of trauma. We hypothesized that (a) PTSD symptoms and prenatal bonding would each be associated with substance craving and (b) PTSD symptoms would be negatively associated with prenatal bonding, and this would at least partially account for the association between PTSD symptoms and substance craving (i.e., indirect effect). Participants (n = 32) were on average 27.1 weeks pregnant (SD = 5.27), 27.8 years old (SD = 4.54), and predominantly Hispanic/Latina (66%). At the within subjects level, higher momentary ratings of PTSD symptoms were associated with lower quality (but not intensity of preoccupation) of prenatal bonding, which in turn was associated with greater craving. Lower quality of prenatal bonding partially mediated the positive association between PTSD symptoms and craving, which remained strong after accounting for prenatal bonding. Our results provide some preliminary support for considering interventions aimed at stabilizing or decreasing PTSD symptoms and stabilizing or increasing prenatal bonding to reduce substance craving and, thus, the risk of perinatal substance use among women with SUD and trauma histories. Abstract

Compared to infants born to mothers without PTSD, infants born to mothers with active PTSD develop poorer behavioral reactivity and emotional regulation. However, the association between perinatal maternal PTSD and infant neural activation remains largely unknown. This pilot study ( N = 14) examined the association between perinatal PTSD severity and infant frontal neural activity, as measured by MEG theta power during rest. Results indicated that resting left anterior temporal/frontal theta power was correlated with perinatal PTSD severity ( p = 0.004). These findings suggest delayed cortical maturation in infants whose mothers had higher perinatal PTSD severity and generate questions regarding perinatal PTSD severity and infant neurophysiological consequences.

Abstract

Research indicates that negative mood effects associated with anabolic-androgenic steroids (referred to as “steroids” here) are likely idiosyncratic, however, little is known about what factors might lead to higher risk for these effects. Research is lacking on the possible contribution of thermogenic/ergogenic drugs to this risk. As thermogenic/ergogenic drugs are stimulants with mood effects similar to those attributed to steroids, this study aimed to build on past research by disentangling the effects on mood from each drug class. An internet survey on current mood as well as steroid and stimulant use was completed by 122 male weightlifters aged 19–57. Current steroid users reported significantly less vigor (ES = 0.025, p = 0.021) than steroid abstainers. Current stimulant users reported significantly greater tension/anxiety (ES = 0.023, p = 0.019) than stimulant abstainers. Abstract

Attention impairments are common symptoms of posttraumatic stress disorder (PTSD); however, the nature of these impairments remains elusive. Attention impairment may arise as the result of either excessive response to task-irrelevant stimuli or reduced response to task-relevant information. To test the association between PTSD and response to task-relevant and task-irrelevant stimuli, we used a 3-tone novelty auditory oddball task (AOD). We hypothesized that participants with PTSD relative to trauma controls would have less response during novelty processing in the dorsolateral prefrontal cortex (dlPFC) and the anterior cingulate cortex, as well as less response in the dlPFC and the orbitofrontal cortex during target detection. Thirty-one male veterans completed a 3-tone novelty AOD task during functional magnetic resonance imaging. Compared to trauma controls, the PTSD group had reduced response during novelty processing in ventromedial prefrontal cortex, superior/middle frontal gyrus (dlPFC), supplementary motor area/caudate, and in posterior regions including bilateral posterior cingulate cortex. The current results suggest PTSD is associated with a pattern of reduced response to novel stimuli. A disturbed orienting response in these brain regions could theoretically underlie PTSD attention-related symptoms.

Abstract

During the last 10 years the European Medicines Agency (EMA) organized a number of workshops on modeling and simulation, working towards greater integration of modeling and simulation (M&S) in the development and regulatory assessment of medicines. In the 2011 EMA - European Federation of Pharmaceutical Industries and Associations (EFPIA) Workshop on Modelling and Simulation, European regulators agreed to the necessity to build expertise to be able to review M&S data provided by companies in their dossier. This led to the establishment of the EMA Modelling and Simulation Working Group (MSWG). Abstract

The prevalence of alcohol, tobacco, and other drug (ATOD) use among emergency department (ED) patients is high and many of these patients have unrecognized and unmet substance use treatment needs. Identification of patients in the ED with problem substance use is not routine at this time.We examined screening data, including standardized measures of ATOD use (HSI, AUDIT-C, DAST-10), from 14,866 ED patients in six hospitals across the United States. We expected younger age, male gender, higher triage acuity, and other substance use severity (ATOD) to be associated both with use versus abstinence and with severity of each substance use type. We used negative binomial hurdle models to examine the association between covariates and (1) substance use versus abstinence (logistic submodel) and with (2) severity among those who used substances (count submodel).Rates of use and problem use in our sample were similar to or higher than other ED samples. Younger patients and males were more likely to use ATOD, but the association of age and gender with severity varied across substances. Triage level was a poor predictor of substance use severity. Alcohol, tobacco, and drug use were significantly associated with using other substances and severity of other substance use.Better understanding of the demographic correlates of ATOD use and severity and the patterns of comorbidity among classes of substance can inform the design of optimal screening and brief intervention procedures addressing ATOD use among ED patients. Tobacco may be an especially useful predictor.

Abstract

Posttraumatic stress (PTSD) and alcohol use (AUD) disorders are associated with abnormal anterior cingulate cortex/ventromedial prefrontal cortex, thalamus, and amygdala function, yet microstructural white matter (WM) differences in executive-limbic tracts are likely also involved. Investigating WM in limbic-thalamo-cortical tracts, this study hypothesized (1) fractional anisotropy (FA) in dorsal cingulum, parahippocampal cingulum, and anterior corona radiata (ACR) would be lower in individuals with comorbid PTSD/AUD compared to in individuals with AUD-only and (2) that FA would be related to both AUD and PTSD severity. 22 combat veterans with comorbid PTSD/AUD or AUD-only completed DTI scans. ANCOVAs indicated lower FA in right (F(df= 1,19)=9.091, P=0.0071) and left (F(df= 1,19) = 10.375, P=0.0045) dorsal cingulum and right ACR (F(df= 1,19) = 18.914, P= 0.0003) for individuals with comorbid PTSD/AUD vs. individuals with AUD-only, even controlling for alcohol use. Multiple linear regressions revealed that FA in the right ACR was inversely related to PTSD severity (r= −0.683, P=0.004). FA was not significantly related to alcohol severity. Reduced WM integrity in limbic-thalamo-cortical tracts is implicated in PTSD, even in the presence of comorbid AUD. These findings suggest that diminished WM integrity in tracts important for top-down control may be an important anomaly in PTSD and/or comorbid PTSD/AUD.

Abstract

Summary Introduction: Temporal bone fracture is a common complication of high-energy cranial trauma. Labyrinth involvement is rare, but there is a risk of perilymphatic rupture that is often underes- timated on initial clinical examination due to the predominance of neurological and/or somatic symptoms. Case report: A patient presented with overlooked perilymphatic fistula, decompensated by hyperpressure effort due to poorly adapted management. Discussion: Following a review of the literature on post-traumatic pneumolabyrinth, overall management (from diagnosis to treatment, via prevention advice) was analyzed. A constructive critique of the patient's treatment was thus made. Conclusion: We argue for a systematic management protocol in cranial trauma with temporal bone fracture, comprising ENT examination, millimetric-scale cross-sectional imaging of the fracture site, and standardized counseling to prevent late complications. © 2013 Published by Elsevier Masson SAS.

Abstract

Tandem-L is a highly innovative SAR satellite mission for the global observation of dynamic processes on the Earth's surface with hitherto unknown quality and resolution. Thanks to its novel imaging techniques and its unprecedented acquisition capacity, Tandem-L will deliver urgently needed information for the solution of pressing scientific questions in the areas of the biosphere, geosphere, cryosphere and hydrosphere. The feasibility of Tandem-L has been analyzed and confirmed in the scope of a phase A study, which has been conducted in close cooperation between the German Aerospace Center (DLR) and the German space industry. This paper provides an overview of the Tandem-L mission concept and summarizes the actual development status.

Abstract

Objective: We describe the systematic approach to incidental findings (IFs) used at the Mind Research Network (MRN) where all MRI scans receive neuroradiologist interpretation and participants are provided results. Methods: From 2004 to 2011, 8,545 MRI scans were acquired by 45 researchers. As mandated by MRN′s external institutional review board, all structural sequences were evaluated by a clinical neuroradiologist who generated a report that included recommendations for referral if indicated. Investigators received a copy of their participants9 reports, which were also mailed to participants unless they specifically declined. To better understand the impact of the radiology review process, a financial analysis was completed in addition to a follow-up phone survey to characterize participant perceptions regarding receiving their MRI scan results. Results: The radiologist identified IFs in 34% of the 4,447 participants. Of those with IFs (n = 1,518), the radiologist recommended urgent or immediate referral for 2.5% and routine referral for 17%. For 80.5%, no referral was recommended. Estimated annual cost for this approach including support for the neuroradiologist, medical director, and ancillary staff is approximately $60,000 or $24/scan. The results of the retrospective phone survey showed that 92% of participants appreciated receiving their MRI report, and the majority stated it increased their likelihood of volunteering for future studies. Conclusions: Addressing IFs in a cost-effective and consistent manner is possible by adopting a policy that provides neuroradiology interpretation and offers participant assistance with clinical follow-up when necessary. Our experience suggests that an ethical, institution-wide approach to IFs can be implemented with minimal investigator burden.

Abstract

Resume Objectif Etudier les procedes d’ossiculoplastie utilisant les protheses en titane de type Kurz et evaluer les facteurs pronostiques des resultats fonctionnels. Methodes Une etude retrospective a ete realisee pour les patients ayant eu une ossiculoplastie par des protheses en titane de type Kurz durant la periode allant de 2006 a 2009 au sein du service ORL et chirurgie cervicofaciale de l’hopital Nord de Marseille. Resultats La population etudiee est de 70 patients, correspondant a 37 reconstructions ossiculaires par prothese partielle (PORP) et 33 par prothese totale (TORP). Le suivi moyen etait de neuf mois. Une comparaison des resultats audiometriques pre- et postoperatoires a ete realisee en se basant sur les frequences (0,5, 1, 2, et 3 kHz) selon les recommandations de l’Academie americaine d’otolaryngologie et chirurgie cervicofaciale. Un rinne audiometrique postoperatoire (ABG) ≤ 20 dB a ete obtenu chez 71,43 % des patients (86,49 % en cas de PORP et 54,55 % en cas de TORP). L’amelioration moyenne du rinne audiometrique (ABG) etait de 12,45 dB dans les otites cholesteatomateuses versus 13,41 dB dans les otites non cholesteatomateuses. Conclusion L’augmentation de la longueur des protheses d’ossiculoplastie en titane, surtout pour les protheses totales (TORP), peut ameliorer les resultats fonctionnels postoperatoires.

Abstract

Many women with sexual assault histories receive care in alcohol/other drug (AOD) treatment programs. Affected women frequently suffer from sexual dysfunction, leading investigators to suggest self-medication may be one path to AOD use disorders and relapse. This preliminary study examined sexual dysfunction and sexual assault in 71 women receiving treatment for addiction. Women with prior sexual assault scored higher than nonassaulted women on sexual dysfunction overall, a discrepancy accounted for by higher scores among assaulted women on sexual inhibition subscales. Sexual inhibition and sexual assault each predicted the use of alcohol or other drugs to increase sexual desire. These preliminary findings suggest sexually abused women may follow a different course into AOD-related problems than nonabused women, possibly including self-medication to relieve sexual inhibition.

La adenoidectomia y la amigdalectomia son intervenciones quirurgicas que se practican con frecuencia en los ninos. A menudo se realizan en una misma fase, aunque sus finalidades y sus indicaciones y postoperatorias son distintas. En la mayoria de los casos, la indicacion deriva de un proceso meditado y se adopta en comun con el medico de atencion primaria o el pediatra, que conoce mejor al nino. Las recomendaciones difundidas por los organismos sanitarios permiten precisar adecuadamente estas indicaciones. La informacion que se aporta a los padres constituye una fase fundamental que permite a menudo garantizar un postoperatorio mas tranquilo. Las complicaciones, en las que predominan las hemorragias, pueden ser graves, por lo que estas intervenciones deben planificarse con todo el rigor necesario y no considerarse como una cirugia menor o anodina. Por este motivo, una colocacion adecuada del nino, una anestesia general adaptada, una tecnica precisa con hemostasia reglada y una vigilancia postoperatoria excelente son los unicos elementos eficaces para prevenir las complicaciones. La prescripcion sistematica de analgesicos durante los primeros dias postoperatorios contribuye a asegurar un postoperatorio de mejor calidad.

Abstract

Long-term use of anabolic-androgenic steroids (AASs) is associated with both positive and negative effects. The authors examined possible mechanisms by which these effects contribute to AAS satisfaction and predict intentions for future AAS use. Five hundred male AAS users completed an interactive Web-based instrument assessing the psychological and physical effects of AAS use. Covariance structure modeling was used to evaluate both direct and indirect effects of AAS consequences on satisfaction with AASs and intentions for future AAS use. Results suggest that gain in muscle mass and psychological benefits from AAS use uniquely contributed to both AAS satisfaction and intentions for future use. Side effects from AAS use also uniquely contributed to AAS satisfaction, but ancillary drug use was found to partially mediate this relationship, suggesting that the satisfaction of experienced AAS users is enhanced by their mastery of side effects through the use of ancillary drugs. The final model explained 29% of the variance in intentions for future AAS use. Mechanisms for sustained AAS use and implications for intervention and prevention strategies are discussed.

Abstract

Item response theory (IRT) is supplanting classical test theory as the basis for measures development. This study demonstrated the utility of IRT for evaluating DSM-IV diagnostic criteria. Data on alcohol, cannabis, and cocaine symptoms from 372 adult clinical participants interviewed with the Composite International Diagnostic Interview--Expanded Substance Abuse Module (CIDI-SAM) were analyzed with Mplus (B. Muthen & L. Muthen, 1998) and MULTILOG (D. Thissen, 1991) software. Tolerance and legal problems criteria were dropped because of poor fit with a unidimensional model. Item response curves, test information curves, and testing of variously constrained models suggested that DSM-IV criteria in the CIDI-SAM discriminate between only impaired and less impaired cases and may not be useful to scale case severity. IRT can be used to study the construct validity of DSM-IV diagnoses and to identify diagnostic criteria with poor performance.

Summary • The expression of nodA and dctA genes of Rhizobium leguminosarum bv. viciae has been studied in mutant nodules of pea (Pisum sativum L.), blocked at the following developmental stages: infection thread development inside the nodule (Itn); infection droplet differentiation (Idd); bacteroid differentiation after endocytosis (Bad); and nodule persistence (Nop). • With the use of reporter fusions to these symbiotic bacterial genes it was shown that both nodA and dctA were expressed at all developmental stages, with a pattern similar to that of constitutive, symbiosis-unrelated genes. • As well as two constitutively expressed genes, both nodA and dctA genes seemed to be subjected to gradual downregulation in nodule bacteria, correlating with the stage of bacteroid differentiation reached. No such effect was observed for the symbiotic, oxygen-regulated fixN gene. The bacteroid development stage also appeared to be related to the ability of bacteria that have been subjected to endocytosis to resume free-living vegetative growth. • The results support the suggestion that bacteroid differentiation into a nitrogen-fixing, organelle-like form, is a gradual process involving several stages, each controlled by different plant genes.

Abstract

The role of leucine biosynthesis by Sinorhizobium meliloti in the establishment of nitrogen-fixing symbiosis with alfalfa ( Medicago sativa) was investigated. The leuA gene from S. meliloti, encoding alpha-isopropylmalate synthase, which catalyses the first specific step in the leucine biosynthetic pathway, was characterized. S. melilotiLeuA(-) mutants were Leu auxotrophs and lacked alpha-isopropylmalate synthase activity. In addition, leuA auxotrophs were unable to nodulate alfalfa. Alfalfa roots did not seem to secrete enough leucine to support growth of leucine auxotrophs in the rhizosphere. Thus, this growth limitation probably imposes the inability to initiate symbiosis. However, in addition to the leucine auxotrophy, leuA strains were impaired in activation of nodulation genes by the transcriptional activator NodD1 in response to the plant flavone luteolin. By contrast, nod gene activation by NodD3, which does not involve plant-derived inducers, was unaffected. Our results suggest that a leucine-related metabolic intermediate may be involved in activation of nodulation genes by NodD1 and luteolin. This kind of control could be of relevance as a way to link bacterial physiological status to the response to plant signals and initiation of symbiosis.

Abstract

The reactions of Zn(OAc)(2) with acetoacetanilide, methyl acetoacetate, o-acetoacetanisidide, and ethyl 2-methylacetoacetate thiosemicarbazones (HTSC(1), HTSC(2), HTSC(3), and HTSC(4), respectively) were explored in methanol. With HTSC(1), HTSC(2), and HTSC(3), following isolation of the corresponding zinc(II) thiosemicarbazonates [Zn(TSC(x))(2)] (x = 1, 2, 3), the mother liquors afforded pyrazolonate complexes [ZnL(1)(2)(H(2)O)] (HL(1) = 2,5-dihydro-3-methyl-5-oxo-1H-pyrazole-1-carbothioamide) that had been formed by cyclization of the corresponding TSC(-). The reaction of HTSC(4) with zinc(II) acetate gave only the pyrazolonate complex [ZnL(2)(2)(H(2)O)] (HL(2) = 2,5-dihydro-3,4-dimethyl-5-oxo-1H-pyrazole-1-carbothioamide). All compounds were studied by IR and NMR spectroscopy, and HTSC(3), [Zn(TSC(3))(2)] x DMSO, [ZnL(1)(2)(H(2)O)] x 2DMSO, and [ZnL(2)(2)(H(2)O)] x 2DMSO were also studied by X-ray diffractometry, giving a thorough picture of the cyclization process. In preliminary tests of the effects of HL(1) and [ZnL(1)(2)(H(2)O)] on rat paw inflammatory edema induced by carrageenan, HL(1) showed antiinflammatory activity.

Abstract

To present the option to use a Fascia Temporalis Superficialis pedicled flap for the implantation of a transcutaneous magnetic osseointegrated auditory implant in selected revision procedures. Osseointegrated auditory implants represent a reliable option for patients. Skin intolerance and aesthetic issue are two clear limitations of percutaneous system. Magnetic transcutaneous devices have been developed to overcome such drawbacks. The current new experience of such a device shows that they are well tolerated in primary cases. However, switching from a previous percutaneous system raises the question of skin tolerance. Hence, previous implanted abutments with skin penetration and skin reduction are more likely cause to skin intolerance and complication. We described here the option to use a Fascia Temporalis Superficialis flap to cover the implanted magnet. When switching from percutaneous osseointegrated auditory implant to transcutaneous system in case of high risk of skin intolerance, this procedure may provide sufficient tissue coverage to prevent skin intolerance, pain and delayed extrusion.

Abstract

The Diagtnostic and Statistical Manual of Mental Disorders (4th ed.; DSM-IV; American Psychiatric Association, 1994) classifies as dependent many cases of mild alcohol problems. DSM-IV diagnoses have modest relationships with predictive and some concurrent validators and often improperly sequence the onset of abuse versus dependence, perhaps due to insufficient emphasis on physiological features. Testing reliability, syndrome prevalence, syndrome sequencing, and concurrent and predictive validity, this study contrasted the DSM-IV with the Withdrawal-Gate Model (WGM), in which alcohol withdrawal is necessary and sufficient for the dependence diagnosis. Clinical samples of adults (baseline n = 318) and adolescents (baseline n = 214) meeting abuse or dependence were assessed for DSM-IV alcohol symptoms and external measures of problem severity and reinterviewed at 6 (adults) and 12 months (adults and adolescents). Among DSM-IV dependent cases, the WGM shifted 32% of adults and 80% of adolescents to the abuse category, making both categories more symptomatically severe, but had a negligible effect on the prevalence of total alcohol diagnoses. The WGM was more reliable than the DSM-IV and temporally sequenced abuse before dependence in a greater number of cases. The WGM was superior to the DSM-IV in concurrent and predictive validity on most measures. Future diagnostic systems may be more reliable and valid if they require evidence of withdrawal for substance dependence.

Abstract

This study examined the prevalence and descriptive psychopathology of pathological gambling in a heterogeneous treatment sample of 372 substance users. About 14% of male participants and 10% of female participants were identified as presumptive pathological gamblers (PGs) on the South Oaks Gambling Screen (SOGS). The authors contrasted 49 PGs with 323 participants who were not pathological gamblers (NPGs) on a host of variables measuring premorbid risk, pathological patterns of substance use, consequences of use, and psychiatric comorbidity. PGs showed more disturbance than NPGs on some measures of premorbid risk, pathological substance use, social consequences of use, and psychiatric comorbidity. Gambling status may be an important comorbid condition in addictions treatment settings and a significant covariate in research.

Abstract

The antro-choanal polyp is a rather common pathology in children.To discuss the diagnostic modalities, the risk factors, the differential diagnoses and treatment options.This study was performed in an ENT department of a 3rd referal center Fifteen patients, 12 males and 3 females (8 to 16 year old), were treated for an antro-choanal polyp (Killian's polyp) between 1997 and 2007.All patients presented a nasal obstruction. Other signs of discovery were a rhinorrhea, a maxillary pain or a pharyngeal mass. Two patients presented a recurrence after a surgery in another center Atopy did not seem to be a risk factor. The imaging evaluation showed a tissular process developed in the maxillary sinus and extending through the maxillary ostium, or an accessory ostium, toward the nose. All patients were treated by intranasal endoscopic surgery. The middle meatal antrostomy was systematic, associated with a inferior antrostomy in 3 cases, with an anterior ethmoidectomy in one case and with a vestibular approach (Caldwell-Luc procedure) in 4 cases. Two patients presented a recurrence which necessited a new surgery.The antro-choanal polyp must be suspected in front of nasal obstruction in children. The imaging evaluation eliminates the main differential diagnoses. The endo-nasal removal seems to be the best procedure.

Abstract

Appearance- and performance-enhancing drugs (APEDs) constitute a wide range of substances, including anabolic-androgenic steroids, nonsteroidal anabolics, and licit and illicit ergo/thermogenics. A great deal of heterogeneity exists in APED use patterns among weight-lifting men, and, consequently, little is known about how these patterns are related to side effect profiles or risk potential. In the current study, a sample of 400 adult men who were regular APED users completed an interactive Web-based instrument detailing information about APED use, side effects, and related indicators of risk. To explore the heterogeneity of APED use patterns, the authors subjected data on use patterns to (a) latent class analysis (LCA), (b) latent trait analysis (LTA), and (c) factor mixture analysis to determine the best model of APED use. Results indicated that a 4-class factor mixture model provided a better fit than LCA and LTA models. The authors also found that severity and latent class were uniquely associated with negative outcomes. Each of the 4 classes was associated with unique side effects, motivations, and participant use patterns. Implications for identifying pathological forms of APED use are discussed.

Abstract

To define clinical, histological and radiological features of sinonasal hemangiopericytoma (SNHPC) and to describe its therapeutic management.A case of SNHPC (47 year old man) of the left nasal cavity underwent endoscopic sinus surgery at our institution. This case is reported and discussed with a review of the literature.SNHPC is a low grade malignant vascular tumour. The intra-nasal location is very rare. This unusual tumour of the nasal cavity has a specific clinical behaviour and prognosis. Diagnosis is confirmed by histological studies with immunohistochemical methods. Vimentin and SMA markers are reported positive in 98% and 92% of cases, respectively. Work up includes MRI and CT scan in order to assess the tumour extent. Angiography is useful to assess the vascularization and preoperative embolization reduces the intra-operative bleeding of large tumours. The gold standard treatment is surgery and the aim is to perform a wide excision with tumour free margins. Surgery can be performed by endoscopic techniques in patients without extra-nasal extension. Radiotherapy is advised for cases of suspect or positive margins. Post-treatment follow up focuses on lifelong endoscopic surveillance. Metastases are rare but recurrences can occur many years postoperatively. The treatment of recurrence is also surgical.

Screening of different mutant traits with respect to nodule structure and function has shown that three of 20 legume plant symbiotic genes should be treated as crucial ones for the determination of the fate of bacteria inside the nodule. The identification of those genes in the model legume symbiotic systems and characterization of them at molecular level looks to be very promising for understanding the molecular mechanisms of their action.

Zinc(II) and cadmium(II) acetates were reacted in methanol under various experimental conditions with thiosemicarbazones derived from β-keto amides or β-keto esters (HTSC). Some of these reactions afforded thiosemicarbazonate complexes [M(TSC)2] with IR and NMR spectra compatible with N,S-coordination, but most gave complexes [ML2], where HL is a substituted 2,5-dihydro-5-oxo-1H-pyrazole-1-carbothioamide resulting from cyclization of the HTSC. Some of these pyrazolonates and two of the HL ligands were studied by X-ray diffractometry, and their structures are discussed. Surprisingly, the reactions of zinc(II) acetate with HTSC in 1 ∶ 1 mol ratio usually gave a third, previously unreported type of complex with a dideprotonated ligand, [Zn(L − H)], which was also formed when [ZnL2] and Zn(OAc)2 interacted at room temperature in 1 ∶ 1 mol ratio. These L − H complexes are highly insoluble in all common solvents, which hinders their characterization but suggests that they are polymeric in nature.

Abstract

Rhizobium etli type strain CFN42 contains six plasmids. We analyzed the distribution of genetic markers from some of these plasmids in bean-nodulating strains belonging to different species ( Rhizobium etli , Rhizobium gallicum , Rhizobium giardinii , Rhizobium leguminosarum , and Sinorhizobium fredii ). Our results indicate that independent of geographic origin, R. etli strains usually share not only the pSym plasmid but also other plasmids containing symbiosis-related genes, with a similar organization. In contrast, strains belonging to other bean-nodulating species seem to have acquired only the pSym plasmid from R. etli .

But de la presentation Une grande variete de pistons est disponible pour la chirurgie stapedienne dans l’otospongiose. Disponible depuis 2007, le piston Soft-CliP ® est une prothese stapedienne en titane medical. Avec ce nouveau design, son placement devrait etre facilite, et la biomecanique de la chaine ossiculaire mieux conservee. Nos objectifs etaient de comparer le piston Soft-CliP ® a un piston de reference dans la chirurgie d’otospongiose. Materiel et methodes Apres stapedotomie au skeeter Oto-tool ® , des pistons Soft-CliP ® ont ete places chez 11 patients, et des pistons teflon Causse ® chez 11 patients. La taille des pistons etait definie par une mesure peroperatoire. Nous avons evalue la facilite de placement, et les audiometries pre- et postoperatoires. Resultats Dans tous les cas, le piston a ete place, avec une duree moyenne de pose de 53,18 sec pour le Soft-Clip ® , et de 30,36 sec pour le piston teflon ( p = 0,060). Le rinne audiometrique moyen etait, dans le groupe Soft-CliP ® et le groupe de reference, de 26,6 dB et de 22,53 dB respectivement en preoperatoire ( p = 0,449), de 11,14 dB et de 6,93 dB a 2 mois de la chirurgie ( p = 0,465), de 11,14 dB et de 5 dB a 6 mois ( p = 0,506). Dans les 2 groupes, le rinne postoperatoire etait statistiquement inferieur au rinne preoperatoire (Soft-CliP ® : p = 0,004 a 2 mois, p = 0,021 a 6 mois ; piston teflon : p = 0,011 a 2 mois, p = 0,046 a 6 mois). Pour les 2 groupes, la perte auditive moyenne de la conduction osseuse postoperatoire etait non statistiquement differente a celle en preoperatoire. Conclusion La fixation du piston sur l’enclume devrait etre facilitee par le nouveau design du piston Soft-CliP ® , sans serrage necessaire ; mais le chirurgien doit acquerir une experience suffisante pour son placement du fait de la largeur plus grande de la tete du piston. Les resultats audiometriques sont satisfaisants, equivalent aux resultats obtenus avec le piston de reference. Des resultats a long terme sur une plus grande population seront necessaires.

Abstract

To evaluate the safety and effectiveness of gadodiamide-enhanced magnetic resonance (MR) angiography with single and triple doses in the assessment of abdominal arterial stenoses.One hundred five patients were included in the randomized, double-blind, phase III multicenter trial. Results of MR angiography with 0.1 mmol/kg and 0.3 mmol/kg doses of gadodiamide were compared with those of digital subtraction angiography (DSA) and according to dose.No serious adverse events were observed. The mean contrast index at the region proximal to the primary stenosis was significantly higher in the triple-dose group (P =.03). Mean 95% CI values for the difference in depicted degree of stenosis between DSA and postcontrast MR angiography improved from -3.4% +/- 4.7 (SD) in the single-dose group to -1.2% +/- 4.7 in the triple-dose group. Mean values for overall image quality on the visual analogue scale improved with the triple dose (P =.02). Confidence in diagnosis was high at postcontrast MR angiography in 88% and 96% of cases in the single- and triple-dose groups, respectively.Gadodiamide-enhanced MR angiography performed with single and triple doses is safe and effective for assessing major abdominal arterial stenoses. Although high agreement between MR angiography and DSA was achieved with both doses, triple-dose MR angiography was superior in the evaluations of image quality, degree of arterial stenoses, and confidence in diagnosis.
