1. Human dental pulp stem cells produce mineralized matrix in 2D and 3D cultures

Abstract

The aim of this study was to characterize the in vitro osteogenic differentiation of dental pulp stem cells (DPSCs) in 2D cultures and 3D biomaterials. DPSCs, separated from dental pulp by enzymatic digestion, and isolated by magnetic cell sorting were differentiated toward osteogenic lineage on 2D surface by using an osteogenic medium. During the differentiation process, DPSCs express specific bone proteins like Runx-2, Osx, OPN and OCN with a sequential expression, analogous to those occurring during osteoblast differentiation, and produce extracellular calcium deposits. In order to differentiate cells in a 3D space that mimes the physiological environment, DPSCs were cultured in two distinct bioscaffolds, MatrigelTM and Collagen sponge. With the addition of a third dimension, osteogenic differentiation and mineralized extracellular matrix production significantly improved. In particular, in MatrigelTM DPSCs differentiated with osteoblast/osteocyte characteristics and connected by gap junction, and therefore formed calcified nodules with a 3D intercellular network. Furthermore, DPSCs differentiated in collagen sponge actively secrete human type I collagen micro-fibrils and form calcified matrix containing trabecular-like structures. These neo-formed DPSCs-scaffold devices may be used in regenerative surgical applications in order to resolve pathologies and traumas characterized by critical size bone defects.

Abstract

Rates of physical inactivity continue to rise in the United States. With this work, we tested the efficacy of a strategy affecting the scope of visual attention designed to promote walking as a form of exercise. Specifically, we examined the influence of narrowed attention on the frequency (Studies 1a, 1b, and 3) and efficiency (Studies 2 and 4) of physical activity in general (Studies 1 and 2) and within exercise bouts measured across multiple days (Studies 3 and 4). We provide convergent evidence by investigating both individual differences in (Studies 1 and 2) and experimentally manipulated patterns of visual attention orienting (Studies 3 and 4). We discuss implications of attentional strategies for self-regulation and fitness.

Abstract

Only few medium and high-strength aluminium alloys can be processed by Laser Powder Bed Fusion without forming solidification cracks. This constraint limits the diffusion of this technology in many industrial fields, including aerospace and motorsport sectors. In this study, a novel high-strength aluminium alloy for Laser Powder Bed Fusion was designed and its solidification behavior, microstructure and mechanical performance were investigated. The results were compared with those achieved by processing the high-strength A20X alloy processed with the same technology. The alloy was designed based on the chemical composition of the widely used 2618 Al alloy, a conventional high strength Al–Cu–Mg alloy for high temperature applications. The chemical composition of the 2618 alloy was modified by adding Ti and B, which form TiB2 particles that act as nuclei for the solidification of primary α-Al grains. The resulting microstructure made of equiaxed grains revealed resistant to hot cracking. A20X and 2618-modified alloys produced with optimized parameters featured relative densities higher than 99,7% and crack-free microstructures. The A20X-T6 showed yield strength and ultimate tensile strength of 428 MPa and 485 MPa, respectively, while the modified 2618-T6 revealed upper yield strength and ultimate tensile strength of 370 MPa and 468 MPa, respectively. The two alloys also showed a remarkably high strength at 150 °C and 250 °C, exceeding the typical strength values of the 2618 alloy produced by forging.

7. Adverse Outcome of Early Recurrent Ischemic Stroke Secondary to Atrial Fibrillation after Repeated Systemic Thrombolysis

Abstract

Background. Recurrent ischemic stroke is associated with adverse neurological outcome in patients with atrial fibrillation. There is very scarce information regarding the neurological outcome of atrial fibrillation patients undergoing repeated systemic thrombolysis after early recurrent ischemic stroke. Clinical Case and Discussion. We describe a case of a 76-year-old woman with known paroxysmal atrial fibrillation who was admitted because of an acute right middle cerebral artery ischemic stroke and who underwent repeated systemic thrombolysis within 110 hours. The patient underwent systemic thrombolysis after the first ischemic stroke with almost complete neurological recovery. On the fourth day after treatment, an acute left middle cerebral artery ischemic stroke was diagnosed and she was treated with full-dose intravenous recombinant tissue plasminogen activator. A hemorrhagic transformation of the left middle cerebral artery infarction was noted on follow-up cranial computed tomographic scans. The patient did not recover from the second cerebrovascular event and died 25 days after admission. Conclusion. To the best of our knowledge, this is the second case reporting the adverse neurological outcome of a patient with diagnosis of atrial fibrillation undergoing repeated systemic thrombolysis after early recurrent ischemic stroke. Our report represents a contribution to the scarce available evidence suggesting that repeated systemic thrombolysis for recurrent ischemic stroke should be avoided.

Abstract

In this research, steel alloys based on the Fe-Cr-Mo, Fe-Cr-Mn and Fe-Cr-Mo-Mn-Ni systems have been designed, produced by different atomisation techniques, and processed by laser powder bed fusion (L-PBF) to investigate their microstructural and mechanical behaviour. Both gas atomisation and water atomisation were considered for powder preparation. The resulting different flowability of powders, hence a different densification behaviour during processing, could be compensated by tuning the L-PBF parameters and by the application of a post treatment to improve flowability of the water atomised powders. In agreement with thermodynamic calculations, small-size oxide-based nonmetallic inclusions of the type SiO2, MnO-SiO2, Cr2O3-SiO2 were found within the steel matrix and on the fracture surfaces of the water atomised L-PBF alloys, featuring higher amounts of oxygen than the gas-atomised steels. Analyses on microstructure and hardness of the hardenable as-built steels suggested that during laser processing, the multilayer L-PBF structure undergoes an in-situ tempering treatment. Furthermore, the mechanical properties of the L-PBF steels could be widely tuned depending on the post-thermal treatment conditions.

Abstract

Evolution in consumer electronics has brought the need for high-current laser diode drivers (LDDs) compatible with integrated systems on chip (SOCs). Traditional solutions device-based are migrating to integrated architectures granting easy digital control, compliance to the voltage rails and fast switching times. The introduction of the digital control directly implies the need of digital-to-analog converter (DAC) structures for the modulation of the laser current and the chosen solution has to match the analog performances required for the particular application. This paper presents a current-mode DAC architecture for LDDs together with a 9-bit implementation which targets a rise time of 1.5 ns for a zero-to-full-current (51 mA) switching condition. A 0.105-mm2 prototype has been manufactured in a 130-nm BiCMOS technology, and the experimental measurements are reported.

Abstract

This study carries out an experimental characterization of lattice structures that are based on cubic cells fabricated through selective laser melting (SLM) and electron beam melting (EBM). The lattice failure under compressive load is studied as a function of the process typology, material properties, and dimensional parameters of the unit cell. The bulk material is first characterized to evaluate the process stability. Three main failure modes of the lattice are identified, depending on the response of ductile/brittle material and the direction of crack propagation. The relationship between lattice geometrical parameters and mechanical strength is observed. The results of the modeling and experiments are suitable to validate the design of lightweight components built with AM processes. The structural performances related to geometrical features, material properties and technological constraints are well explained for further applications in structural design. The equivalent Young’s module of lattice samples with different cell size has been measured and compared with numerical simulations based on the homogenization method.

Abstract

Chronic constipation is a common cause of morbidity in the elderly and institutionalized population. It can be associated with significant morbidity and socioeconomical burden. Chronic resistance constipation can rarely be associated with megarectum. Herein, we present the case of a patient with physical and mental disability that presented with refractory constipation associated with extreme stool burden and a massive megarectum. We discuss chronic constipation in the elderly population, its etiologies and diagnostic work-up including surgical options. The management of chronic constipation with megarectum should be on a case-by-case basis.

Abstract

Lattice structures fabricated with micromelting of metal powders are promising solutions for lightweight applications. Additive manufacturing processes as selective laser melting are largely used to build bulk components, but the influence of laser settings on lattice struts morphology is not jet fully investigated. Previous studies demonstrate the effect of laser speed and layers thickness on the material density and lattice struts dimensions. In this paper, the effects of the laser volume energy density associated with the process setup parameters are analyzed in relation to the dimensional accuracy of lattice struts. The statistical approach based on design of experiments used in this paper allows getting appreciable reduction of the average errors of struts dimensions (from 48% to 16% and from 22% to 7% in horizontal and vertical orientations, respectively).

Abstract

Selective laser melting fabricated materials regularly exhibit excellent static strength, thanks to the very high thermal gradient that characterizes the process; however, their performance under dynamic loading is somehow restricted due to the limited ductility, tensile residual stresses, the defect density and the inadequate surface morphology in the as built configuration. Currently mechanical or electrochemical surface polishing steps combined with various heat treatments are integrated into the production lines to respectively address these issues. However, these methods are reported to occasionally lead to inconsistent results apart from the burden of additional costs. Herein, we applied various post treatments including shot peening, sand blasting and heat treatment to evaluate their individual and synergetic effect to tackle the aforementioned challenges. Physical, microstructural and mechanical properties of SLM fabricated AlSi10Mg specimens were investigated with special focus on fatigue strength. The results highlight that appropriate post treatments can significantly enhance the fatigue performance of SLM specimens resulting in characteristics that are comparable and even better than conventionally manufactured material. Surface treatments, in particular, were found to be efficient in significantly enhancing the fatigue strength, while eliminating the necessity of polishing steps that are currently applied on as built material. 14. Morpho-functional changes in human tendon tissue

Abstract

Insertion tissue biopsies of right arm common extensor tendons from 11 patients with chronic lateral epicondylitis were processed for light and electron microscopy. The subjects were aged between 38 and 54 years (only one was 25). The specimens showed a variety of structural changes such as biochemical and spatial alteration of collagen, hyaline degeneration, loss of tenocytes, fibrocartilage metaplasia, calcifying processes, neovascularization and vessel wall modifications. Tissue alterations were evident in limited zones of the tendon fibrocartilage in which the surgical resection was generally visible. The areas where the degenerative processes were localized, were restricted and in spatial contiguity with morphologically normal ones. The observed cases presented histological and electron microscopic findings that characterize lateral epicondylitis as a degenerative phenomenon involving all tendon components.

Abstract

Laser powder bed fusion (LPBF) as an additive manufacturing technology offers high potential to fabricate parts with complex geometries layer-by-layer. However, these parts have inhomogeneous microstructure and very poor surface quality in their as-built condition. The presence of high surface irregularities especially in the downskin surfaces is a challenging issue that can directly influence their mechanical performance especially under fatigue loading conditions. Hence, applying post-treatments to modulate these imperfections can play a critical role. In this study, the individual and hybrid effects of different post-treatments including T6 heat treatment and shot peening on microstructure, mechanical properties and fatigue behaviour of LPBF V-notched AlSi10Mg specimens were investigated. Two different shot peening processes were applied on both as-built and heat treated specimens using steel and ceramic shots with different Almen intensity, shot diameter and shot hardness. The specimens were comprehensively characterized in terms of microstructural features, surface morphology and surface roughness. Mechanical properties including microhardness and residual stresses were measured and fatigue behaviour of the specimens was determined using a stair-case method; fracture surfaces were also critically analyzed. The results of the analysis performed both on the smooth section and the notched section (including notch root, up and down skin areas) indicated the importance of the choice of shot peening parameters with respect to the target geometry and its material properties. In this case, the shot peening treatment with smaller media and lower intensity was more efficient in terms of surface modification and homogenization especially in the downskin surfaces leading to higher fatigue strength. The significant finding of this study is that by pairing the heat treatment and shot peening, the effect of the presence of the notch can be masked obtaining almost the same fatigue strength for the notched specimens as the un-notched counterparts.

Abstract

Background Ongoing development and expansion of trauma centers in the United States necessitates empirical analysis of the effect of investment in such resources on population-level health outcomes. Methods Multiple linear regressions were performed to predict state-level trauma-related mortality among adults and the elderly across 50 US states in 2010. The number of trauma centers per capita in each state and the percentage of each state’s population living within 45-min of a trauma center served as the key independent variables and injury-related mortality served as the dependent variable. All analyses were stratified by age (adult versus elderly; elderly ≥ 65 years old) and were performed in SPSS. Results The proportion of a population with geographic proximity to a trauma center demonstrates a consistent inverse linear relationship to injury-related mortality. The relationship reliably retains its significance in models including demographic covariates. Interestingly, access to Levels I and II trauma centers demonstrates a stronger correlation with mortality than was observed with Level III centers. Conclusion Trauma center access is associated with reduced trauma-related mortality among both adults and the elderly as measured by state reported mortality rates. Ongoing efforts to designate and verify new trauma centers, particularly in poorly-served ‘trauma deserts’, could lead to lower mortality for large populations.

Abstract

Ti6Al4V specimens were produced using electron beam melting (EBM) with the goal of studying the effects of the scanning strategies on the surface morphology, microstructure, and mechanical properties. As the factory condition consists in the use of different parameters for scanning the hatch and the contour, additional processing conditions were investigated using the hatch and contour strategies separately, oriented from the center to the border, and vice versa. Due to material anisotropy, the samples were fabricated in both the vertical and planar directions. This work demonstrates that the scanning strategies affect several features of EBMed TiAl4V samples, like the relative density of the specimens, the surface morphology, and the microstructure and mechanical properties. In details, pure contour can increase the relative density, due to the use of pulses instead of continuous electron beam emission, while the hatch scanning produces smoother and more uniform surface morphology. As different thermal histories can be associated with the variation of the scanning strategy, the microstructure can be varied in terms of grain growth. Finally, the tensile behavior of the specimens, evaluated in both the principal orientations, can be correlated to the relative density and surface irregularity.

Abstract

Background:Omalizumab may modulate airway remodeling in severe asthma. Using forced expiratory volume in 1 second (FEV1) as a surrogate of airway remodeling, we aimed to investigate if an omalizumab add-on in severe allergic asthma may lead to a persistent reversal of airway obstruction and to evaluate the potential biomarkers of airway obstruction reversibility.Methods:Data were collected before (T0) and after omalizumab add-on for 1 year (T1, 32 patients), 2 years (T2, 26 patients) and 4 years (T4, 13 patients). All patients had baseline FEV1below 80 % predicted (60.5 ± 12.5 %). After omalizumab, 18 patients showed FEV1normalization (reversible airway obstruction; RAO+) already at T1 (88.7 ± 14.9 %, p < 0.0001) that persisted up to T4 (83.2 ± 7.9, p < 0.01), while 14 patients (RAO−) had FEV1persistently decreased, from T1 (65.2 ± 8.4%, p < 0.05) up to T4 (61.4 ± 6.2%, not significant). Both groups had significant improvement of symptoms and exacerbations after omalizumab at T1, which persisted up to T4. The comparison between pretreatment characteristics of the two groups showed that RAO+ patients, had higher values of circulating eosinophils, exhaled nitric oxide (FENO), prevalence of rhinitis and nasal polyps, need of oral corticosteroids, shorter asthma duration, higher FEV1and response to albuterol test. The optimal cut-off points predicting FEV1normalization after omalizumab add-on were 30.5 ppb for FENO and 305 cells/µl for eosinophils.Conclusions:This study suggests that omalizumab add-on contributes to the persistent reversal of airway obstruction in a consistent number of patients with severe allergic asthma, and this beneficial effect is predicted by elevated pretreatment FENO and circulating eosinophils.

Abstract

This study investigated the effects of the ‘as-built’ condition on the fatigue properties of an AlSi 10 Mg aluminium alloy manufactured by Selective Laser Melting (SLM) in different orientations. The results showed a moderate correlation between defects/surface features at the fracture origin and the roughness measurements. On the other hand, a fatigue strength analysis based on the size of features at fracture origin and residual stress just below the surface proved to be successful. Although considering the synergistic effect of different variables increased the scatter in fatigue life data points, a fatigue strength model including short crack effect and residual stresses made it possible to estimate the EIFS close to the real critical features. Results for life prediction models based on EIFS showed that 87% of the estimated number of cycles to failure are within a factor of 0.5–2 with respect to the experimental number of cycles to failure.

Abstract

The exponential increase in cases of type 2 diabetes mellitus (T2DM), particularly in developing countries, is combined with the progressive failure of healthcare facilities to provide adequate assistance. The role of Therapeutic Education (TE) in the treatment of T2DM has long been certified. However, despite efforts to implement educational interventions, these have proved to be largely insufficient in meeting current needs. Diabetology – like modern medicine – has therefore been looking into digitization as a possible solution. The increasingly widespread diffusion of the Network and the means for accessing it have in fact triggered a full-fledged race to computerization, a growth that is not always consistent. TE could not get away from these new circumstances. The digital world offers opportunities for reviewing and updating TE without denying or replacing its recent and prestigious past. This new way of doing TE, however, requires attention and new skills to be acquired and tested extensively. Abstract

Background and aims: Recently, the albuminocentric view of diabetic kidney disease (DKD) in type 2 diabetes (T2DM) has been changing. Therefore, the relationship between diabetic retinopathy (DR) and chronic kidney disease (CKD) has to be addressed according to this new clinical presentation of DKD. The aim of this study was to evaluate, in a real-world setting, the correlation DR–DKD in T2DM. Methods and results: A total of 2068 type 2 diabetic patients enrolled in a multicenter cross-sectional study were investigated. Albuminuric subjects were largely prevalent among subjects with DR (p = 0.019). In the whole study population, no difference in albumin excretion rate (AER) was observed between presence/absence of DR; instead, AER was significantly higher among patients with glomerular filtration rate (eGFR) < 60 mL/min/1.73 m2 (CKD) (p = 0.009), above all in those with CKD and AER ≥0.03 g/24 h (p = 0.005). Multivariate analysis confirmed that eGFR (O.R. 0.976; 95% C.I.: 0.960–1.028; p < 0.001) and AER (O.R. 1.249; 95% C.I. 1.001–1.619; p = 0.004) were independently associated with DR and HDL–cholesterol (O.R.: 1.042; 95% C.I.: 1.011–1.120; p = 0.014). Abstract

Despite additive manufacturing processes are already widely used in several industrial applications, there are few materials that are specifically designed and optimized for these technologies. Currently, only few Al alloys are available on the market and employed for 3D printing of structural parts. In particular, Si–Mg bearing alloys are the most common Al alloys for additive manufacturing, featuring high processability but moderate mechanical properties. By this work, we studied the effect of Si addition on the hot cracking susceptibility of a high strength Al–Zn–Mg–Cu alloy. A preliminary activity has been carried out by blending Al–Zn–Mg–Cu and Al–Si–Mg powders and analysing their microstructure and properties achieved after selective laser melting. Eventually a new Al–Zn–Si–Mg–Cu alloy has been designed, produced as powder alloy by gas atomization and tested. The microstructure and phase transformations of the new alloy has been investigated by synchrotron X-ray diffraction, differential scanning calorimetry and microscope analysis. The Al–Zn–Si–Mg–Cu alloy processed by selective laser melting featured a relative density of 99.8%, no hot cracks were noticed within the investigated microstructures. The ability of the new alloy to respond to aging starting from both as built and solution annealed conditions has been also evaluated. A good response to direct aging (directly from as built condition) was demonstrated, featuring yield strength and ultimate tensile strength of 402 and 449 MPa, respectively, and hardness of 174 HV after optimized aging at 165 °C for 2 h.

Abstract

Summary Background The Global Burden of Disease, Injuries, and Risk Factor study 2013 (GBD 2013) is the first of a series of annual updates of the GBD. Risk factor quantification, particularly of modifiable risk factors, can help to identify emerging threats to population health and opportunities for prevention. The GBD 2013 provides a timely opportunity to update the comparative risk assessment with new data for exposure, relative risks, and evidence on the appropriate counterfactual risk distribution. Methods Attributable deaths, years of life lost, years lived with disability, and disability-adjusted life-years (DALYs) have been estimated for 79 risks or clusters of risks using the GBD 2010 methods. Risk–outcome pairs meeting explicit evidence criteria were assessed for 188 countries for the period 1990–2013 by age and sex using three inputs: risk exposure, relative risks, and the theoretical minimum risk exposure level (TMREL). Risks are organised into a hierarchy with blocks of behavioural, environmental and occupational, and metabolic risks at the first level of the hierarchy. The next level in the hierarchy includes nine clusters of related risks and two individual risks, with more detail provided at levels 3 and 4 of the hierarchy. Compared with GBD 2010, six new risk factors have been added: handwashing practices, occupational exposure to trichloroethylene, childhood wasting, childhood stunting, unsafe sex, and low glomerular filtration rate. For most risks, data for exposure were synthesised with a Bayesian metaregression method, DisMod-MR 2.0, or spatial-temporal Gaussian process regression. Relative risks were based on meta-regressions of published cohort and intervention studies. Attributable burden for clusters of risks and all risks combined took into account evidence on the mediation of some risks such as high body-mass index (BMI) through other risks such as high systolic blood pressure and high cholesterol. Findings All risks combined account for 57·2% (95% uncertainty interval [UI] 55·8–58·5) of deaths and 41·6% (40·1–43·0) of DALYs. Risks quantified account for 87·9% (86·5−89·3) of cardiovascular disease DALYs, ranging to a low of 0% for neonatal disorders and neglected tropical diseases and malaria. Abstract

This paper presents the design of an energy harvesting interface for multiple piezoelectric elements. The latter is targeted to be integrated in an air spring to convert the mechanical energy in electrical one usable to power a wireless sensor node. Modelling of the energy harvesting electronic circuit has been performed to optimize the management of the multiple signals provided by the piezoelectric elements in function of a variable electrical load. An experimental activity has been conducted to validate the circuit design and the tests results demonstrate a good agreement with the numerical ones, confirming the effectiveness of the proposed interface.

Abstract

Recent advances in the field of cold spray have put forward the potential of this deposition technique to be used as a non-thermal additive manufacturing process with significantly high deposition rates. In this study, we use the additive manufacturing potential of cold spray for fabrication of freestanding three-dimensional Inconel 718 samples, which is a challenging material for cold spray due to its high hardness and limited deformability. Additionally, we fabricated samples with similar geometry using one of the most common additive manufacturing methods, i.e. selective laser melting. Microstructural characteristics, distribution of residual stresses, porosity and structural integrity of the cold spray deposited samples were compared with those obtained by selective laser melting before and after different heat treatments. The results of the first time study of axial fatigue strength of cold spray deposited freeform samples indicate the notable efficiency of cold spray for fabrication of freestanding objects for structural components, with similar characteristics to those obtained from laser based additive manufacturing technique and even comparable to bulk material properties. The low working temperature of the cold spray method, suggests it as a promising additive manufacturing technique with a high potential to address many challenges regarding laser based approaches.

Abstract

Nasal cytology is an easy, cheap, non-invasive and point-of-care method to assess nasal inflammation and disease-specific cellular features. By means of nasal cytology, it is possible to distinguish between different inflammatory patterns that are typically associated with specific diseases (ie, allergic and non-allergic rhinitis). Its use is particularly relevant when other clinical information, such as signs, symptoms, time-course and allergic sensitizations, is not enough to recognize which of the different rhinitis phenotypes is involved; for example, it is only by means of nasal cytology that it is possible to distinguish, among the non-allergic rhinitis, those characterized by eosinophilic (NARES), mast cellular (NARMA), mixed eosinophilic-mast cellular (NARESMA) or neutrophilic (NARNE) inflammation. Despite its clinical usefulness, cheapness, non-invasiveness and easiness, nasal cytology is still underused and this is at least partially due to the fact that, as far as now, there is not a consensus or an official recommendation on its methodological issues. We here review the scientific literature about nasal cytology, giving recommendations on how to perform and interpret nasal cytology.

Abstract

Introduction: PROXIMA study evaluated the effect of omalizumab in severe allergic asthma (SAA) patients. In the context of this study, a sub-study on proteomic analysis of asthma biomarkers was conducted on biological samples of SAA patients. Aim and objectives: The aim of this sub-study was to characterize patients with SAA, exploring asthma-related proteins in biologic samples as possible biomarkers and to assess whether these biomarkers correlated with the achievement and maintenance of disease control. Methods: Proteomic analysis was performed using chromatography and mass spectrometry applied on exosomes extracted from urine samples of patients at baseline (T0) and 12 months (T12) after omalizumab. Results: The protein profiles of samples from patients before treatment allowed a stratification of patients in two main groups characterized by a different clinical trend after treatment (Table). The same procedure was applied also to clinical parameters and the segregation of patients resulted in good agreement with that obtained by proteomics analysis. Finally, some proteins, such as Galectin 3-binding protein (Gal-3BP), resulted very consistent for predicting the patient behavior to omalizumab treatment. Conclusions: Overall the data suggest that molecular stratification correlates with those obtained by means of clinical parameters and thus proteomics profiles of urine samples from SAA patients could be useful for predicting clinical response to treatment.

Abstract

Heart failure (HF) and atrial fibrillation (AF) have been associated with a higher risk of Alzheimer's disease (AD). Whether HF and AF are related to AD by enhancing AD neuropathological changes is unknown.We applied network analyses and multiple logistic regression models to assess the association between HF and AF with severity of AD neuropathology in patients from the National Alzheimer's Coordinating Center database with primary neuropathological diagnosis of AD.We included 1593 patients, of whom 129 had HF and 250 had AF. HF and AF patients were older and had milder AD pathology. In the network analyses, HF and AF were associated with milder AD neuropathology. In the regression analyses, age (odds ratio [OR] 0.94, 95% confidence interval [CI] 0.93-0.95 per 1-year increase in age, P .001) and the interaction term HF × AF (OR 0.61, 95% CI 0.40-0.91, P = .014) were inversely related to severe AD pathology, whereas APOE ε4 genotype showed a direct association (OR 1.68, 95% CI 1.31-2.16). Vascular neuropathology was more frequent in patient with HF and AF patients than in those without.HF and AF had milder AD neuropathology. Patients with milder AD lived longer and had more exposure to vascular risk factors. HF and AF patients showed a higher frequency of vascular neuropathology, which could have contributed to lower the threshold for clinically evident dementia.

Abstract

IMPORTANCE: Gait performance is affected by neurodegeneration in aging and has the potential to be used as a clinical marker for progression from mild cognitive impairment (MCI) to dementia. A dual-task gait test evaluating the cognitive-motor interface may predict dementia progression in older adults with MCI. OBJECTIVE: To determine whether a dual-task gait test is associated with incident dementia in MCI. DESIGN, SETTING, AND PARTICIPANTS: The Gait and Brain Study is an ongoing prospective cohort study of community-dwelling older adults that enrolled 112 older adults with MCI. Participants were followed up for 6 years, with biannual visits including neurologic, cognitive, and gait assessments. Data were collected from July 2007 to March 2016. MAIN OUTCOMES AND MEASURES: Incident all-cause dementiawas the main outcome measure, and single- and dual-task gait velocity and dual-task gait costs were the independent variables. A neuropsychological test battery was used to assess cognition. Gait velocity was recorded under single-task and 3 separate dual-task conditions using an electronic walkway. Dual-task gait cost was defined as the percentage change between single- and dual-task gait velocities: ([single-task gait velocity - dual-task gait velocity]/single-task gait velocity) × 100. Cox proportional hazard models were used to estimate the association between risk of progression to dementia and the independent variables, adjusted for age, sex, education, comorbidities, and cognition. Abstract

Insufficient physical activity contributes to rising obesity rates. We tested one social cognitive strategy aimed at increasing physical activity in the environment. Specifically, we tested whether attentional narrowing can shift people’s perceptual representations of the environment and improve exercise behavior. Participants who adopted a narrow focus of attention, compared to participants who looked around the environment as they naturally would, perceived a target as physically closer (Studies 1, 2). In addition, narrowed attention reduced the time required to walk to a finish line and increased subjective ease of physical task performance, two markers of improved exercise (Study 2). We discuss implications of attentional strategies for perception and action in regards to health and fitness.

Abstract

Background and Purpose Ascertaining stroke severity and predicting risk of in-hospital mortality is crucial to advise patients and families about medical decisions. We developed and tested the validity of a new stroke score, the 6S Score (Stroke Severity Score based on Six Signs and Symptoms), for quantifying ischemic stroke severity and predicting in-hospital mortality. Methods We prospectively assessed 210 consecutive acute ischemic stroke patients. The cohort was further divided into a derivation (n = 120) and a validation (n = 90) sample. From a total of 10 stroke signs and symptoms, we selected those with likelihood ratio’s P < 0.005. We tested the validity of the score for predicting in-hospital mortality by using receiver operating characteristic curves. We used a scatterplot and the Spearman’s test to evaluate the correlation between the 6S Score and the National Institutes of Health Stroke Scale as a marker of stroke severity. We used principal component and exploratory factor analyses for assessing qualitative aspects of the 6S Score. Results The C statistic for in-hospital mortality was 0.82 for the 6S Score and 0.86 for the National Institutes of health Stroke Scale, respectively, with no significant differences between each other (P = 0.79). The correlation between both scores was strong (Spearman’s rho 0.68, P < 0.001). The factor analyses showed a good balance between left/right hemispheres and anterior/posterior circulations. Conclusions The 6S Score may constitute a tool for easily assessing stroke severity and predicting stroke mortality. Further research is needed for further assessing its external validity.

Abstract

We hypothesized that a longer duration of antibiotic treatment for intra-abdominal infections (IAI) would be associated with an increased risk of extra-abdominal infections (EAI) and high mortality.We reviewed all IAI occurring in a single institution between 1997 and 2010. The IAI were divided into two groups consisting of those with a subsequent EAI and those without; the data for each group were analyzed. Patients with EAI following IAI were matched in a 1:2 ratio with patients who did not develop EAI on the basis of their Acute Physiology and Chronic Health Evaluation (APACHE II) score±1 point. Statistical analyses were done with the Student t-test, χ(2) analysis, Wilcoxon rank sum test, and multi-variable analysis.We identified 2,552 IAI, of which 549 (21.5%) were followed by EAI. Those IAI that were followed by EAI were associated with a longer initial duration of antimicrobial therapy than were IAI without subsequent EAI (median 14 d [inter-quartile range (IQR) 10-22 d], vs. 10 d [IQR 6-15 d], respectively, p0.01), a higher APACHE II score (16.6±0.3 vs. 11.2±0.2 points, p0.01), and higher in-hospital mortality (17.1% vs. 5.4%, p0.01). The rate of EAI following IAI in patients treated initially with antibiotics for 0-7 d was 13.3%, vs. 25.1% in patients treated initially for7 d (p0.01). A successful match was made of 469 patients with subsequent EAI to 938 patients without subsequent EAI, resulting in a mean APACHE II score of 15.2 for each group. After matching, IAI followed by EAI were associated with a longer duration of initial antimicrobial therapy than were IAI without subsequent EAI (median 14 d [9-22 d], vs. 11 d [7-16 d], respectively, p0.01), and with a higher in-hospital mortality (14.9% vs. 9.0%, respectively, p0.01). Logistic regression showed that days of antimicrobial therapy for IAI was an independent predictor of subsequent EAI (p0.001).A longer duration of antibiotic therapy for IAI is associated with an increased risk of subsequent EAI and increased mortality.

Abstract

Poststroke atrial fibrillation (AF) represents up to 1 of 4 overall AF cases in acute ischemic stroke. Current guidelines recommend oral anticoagulation for every ischemic stroke patient in whom AF is diagnosed. However, in some cases, AF detected after acute ischemic stroke may be short-lasting and perhaps a nonrecurrent autonomic and inflammatory epiphenomena of stroke. The autonomic regulation of cardiac rhythm constitutes an integrated relay system. The highest level of control is exerted by the cerebral cortex, particularly the insula. The onset of AF may be associated with an imbalance of sympathetic and parasympathetic activity, a common consequence of insular infarctions. This autonomic imbalance and an interruption in the cerebral regulation of the intrinsic cardiac autonomic system constitute the most likely mechanisms responsible for the autonomic pathway. The role of inflammation in the genesis of AF within the first few days after ischemic stroke may occur through inflammatory mediators stimulating the intrinsic autonomic system and by direct damage to atrial myocardium. To what extent poststroke AF is the cause or a consequence remains uncertain.

Abstract

Though people often believe their visual experiences reflect the objective state of the surrounding world, a wealth of recent evidence suggests that perceptions are systematically biased. We draw from contemporary research and supplement with real world anecdotes to suggest that two aspects of perception are subject to influence by states internal to perceivers themselves. We focus on the biases that arise from two difficult tasks faced by the visual system. First, visual information is often unclear and ambiguous; yet visual input must be categorized quickly and efficiently. Second, people can focus on only a subset of perceptual information at once, and as a result visual attention is necessarily directed at a subset of the surrounding world. We document ways in which expectations, desires, and fears help to resolve perceptual ambiguity and direct attention. Finally, we discuss the functional nature of visual biases and note potential consequences in important domains.

Background: The infected abdomen poses substantial challenges to surgeons, and often, both temporary and definitive closure techniques are required. We reviewed the options available to close the abdominal wall defect encountered frequently during and after the management of complicated intra-abdominal infections. Methods: A comprehensive review was performed of the techniques and literature on abdominal closure in the setting of intra-abdominal infection. Results: Temporary abdominal closure options include the Wittmann Patch, Bogota bag, vacuum-assisted closure (VAC), the AbThera™ device, and synthetic or biologic mesh. Definitive reconstruction has been described with mesh, components separation, and autologous tissue transfer. Conclusion: Reconstructing the infected abdomen, both temporarily and definitively, can be accomplished with various techniques, each of which is associated with unique advantages and disadvantages. Abstract

A role in pulmonary immunity has been ascribed to Natural Killer (NK) cells and several in vitro studies have shown a corticosteroid-induced inhibition of NK cells mediated cytotoxicity. Several clinical trials on chronic obstructive pulmonary disease (COPD) have suggested a relationship between COPD treatment and occurrence of respiratory infections. Aims of our study were to investigate if real life COPD treatment affects peripheral blood NK cells total count and their receptors expression and to assess if different doses of formoterol and budesonide, administered alone or in combination, are able to modulate the surface expression of activating (NKp30, NKp44, NKp46 and NKG2D) and inhibitory (KIR2DL2/L3, KIR3DL1 and NKG2A) receptors on peripheral blood NK cells of COPD patients. Moreover, we evaluated the potential effect of treatment with budesonide and/or formoterol on IFN-γ secretion in vitro. NK cells were isolated from peripheral blood of 7 healthy volunteers, 9 chronic bronchitis (CB) and 11 COPD patients. Total NK cells count and activating and inhibitory receptors expression were evaluated. NK cells were cultured for 20 h in 96-well plates with IL-2 (100 IU/ml) + IL-12 (2.5 ng/ml), with or without budesonide (Bud; 1 and 0.01 μM) and formoterol (For; 30 and 0.3 nM) alone or in combination. Cells were analyzed by flow cytometry and IFN-γ was measured in cell supernatants by ELISA test. No difference between real life treated COPD, CB and healthy subjects was found concerning NK total count and NK cell receptors expression. When cells were stimulated over night with cytokines and treated with drugs, only NKG2D receptor was modulated. Its expression was significantly downregulated by budesonide alone and in combination with formoterol in COPD patients. IFN-γ production induced by stimulation with IL-2 + IL-12 was decreased in a highly significant way (p < 0.01) by all treatments in all groups. Even if in vitro experiments with budesonide, alone or in combination with formoterol, showed a modulation of NKG2D receptor expression and IFN-γ production, our ex vivo results show that real life LABA and ICS treatment does not influence peripheral NK cells count and their receptors phenotype.

Abstract

Polyspecific organic cation transporters (OCTs) in human cell membranes are involved in the uptake, distribution and excretion of cationic compounds. Although their relevance to drug disposition in the liver, small intestine and kidney has been investigated previously, less is known about the influence of these transporters on the pharmacokinetics and pharmacodynamics of inhaled drugs. Drugs that are commonly administered by inhalation for the treatment of respiratory diseases, such as glucocorticoids and cationic β(2)-agonists, might interact with several of these transporters, which are strongly expressed on the surfaces of airway epithelial cells. We evaluated the expression of OCT3 and measured the in vitro uptake of the short-acting β(2)-agonist salbutamol (SALB), alone or in combination with corticosterone (CS) and beclomethasone dipropionate (BDP), by bronchial smooth muscle cells. Our results showed that these cells express the OCT3 transporter and that SALB enters the cell in a transporter-independent fashion. Moreover, CS and BDP have different activities on SALB transport inside the cell. CS increases SALB transport and BDP decreases SALB transport, although neither of these effects are statistically significant. A better understanding of these mechanisms might lead to the improved treatment of airway diseases.

Abstract

Atrial fibrillation (AF) is the major cause of cardioembolic stroke. It often remains occult when asymptomatic and paroxysmal. We hypothesized that the detection of AF after acute ischemic stroke (AIS) or transient ischemic attack (TIA) could be improved by using continuous cardiac monitoring (CCM) immediately after admission. We sought to determine the detection rate of AF by immediate in-hospital CCM after cryptogenic and noncryptogenic AIS or TIA in patients without a previous diagnosis of AF. We retrospectively studied a cohort of 155 patients with cryptogenic and noncryptogenic AIS or TIA without known AF. We compared the detection rates of newly diagnosed AF (NDAF) in patients admitted to areas with CCM and those never admitted to these areas. We developed a multiple logistic regression model for identifying predictors of NDAF. We characterized NDAF episodes and analyzed how the availability of CCM data changed secondary prevention strategies. We detected NDAF in 21 patients (13.5%). Diagnostic rates of NDAF in patients who underwent CCM and those who did not undergo CCM were 18.2% and 2.2%, respectively (P = .005). The median time from admission to recognition of NDAF was 2.0 days. Most NDAFs were paroxysmal (95.2%) and lasted less than 1 hour (85.7%). Diabetes mellitus and infarct size were predictors of NDAF. Detection of NDAF prompted the initiation of anticoagulation therapy in 8.2% of the patients admitted to areas with CCM availability. Our findings suggest that immediate and prolonged CCM significantly improves the detection of NDAF after cryptogenic and noncryptogenic AIS or TIA, and that diabetes mellitus and infarct size are significantly associated with NDAF.

Abstract

Severe persistent asthma causes a substantial morbidity and mortality burden and is frequently not well controlled, despite intensive guideline-based therapy. The unique monoclonal antibody approved for patients with severe allergic asthma is omalizumab: a recombinant humanised murine against IgE antibodies. The aim of the present study is to investigate the effect of long-term anti-IgE on the thickening of the reticular basement membrane (RBM) and eosinophil infiltration in bronchial biopsies from patients with severe persistent allergic asthma. Biopsies were obtained from 11 patients with severe persistent allergic asthma before and after (12 months) treatment with omalizumab. RBM thickness and eosinophils were measured by using light microscope image analysis. A significant mean reduction in RBM thickness and eosinophil infiltration were measured after one-year omalizumab treatment. No correlation between eosinophil reduction and RBM thickness reduction was found. No correlation between each of the previous two parameters and clinical parameters was detected. In conclusion, our study showed that a substantial proportion of severe asthmatics reduced the original bronchial RBM thickness and eosinophil infiltration after one-year treatment with anti-IgE, thus emphasizing the possible role of omalizumab in affecting airway remodeling in severe persistent allergic asthma.

Abstract

Isolation is defined as the separation of persons with communicable diseases from those who are healthy. This public health practice, along with quarantine, is used to limit the transmission of infectious diseases and provides the foundation of current-day cohorting.Review of the pertinent English-language literature.Mass isolation developed during the medieval Black Death outbreaks in order to protect ports from the transmission of epidemics. In the mid-1800s, infectious disease hospitals were opened. It now is clear that isolation and cohorting of patients and staff interrupts the transmission of disease. Over the next century, with the discovery of penicillin and vaccines against many infectious agents, the contagious disease hospitals began to close. Today, we find smaller outbreaks of microorganisms that have acquired substantial resistance to antimicrobial agents. In the resource-limited hospital, a dedicated area or region of a unit may suffice to separate affected from unaffected patients.Quarantine, or cohorting when patients are infected with the same pathogen, interrupts the spread of infections, just as the contagious disease hospitals did during the epidemics of the 18th and 19th centuries.

La enfermedad aterosclerótica asintomática de la arteria carótida interna extracraneal alcanza una prevalencia de hasta el 12.5%. La angioplastia carotídea todavía no ha demostrado ser lo suficientemente segura y eficaz para prevenir el ACV isquémico en estos pacientes. Estudios aleatorizados demostraron que la endarterectomía carotídea es superior al tratamiento médico en cuanto a reducción del riesgo de ACV isquémico si es realizada por equipos con tasas de complicaciones (ACV o muerte) menores que 3%. Sin embargo, los pacientes evaluados en estos estudios comenzaron a reclutarse hace más de 25 años, cuando la utilización de antiagregantes plaquetarios era menor que la actual, el tratamiento de la hipertensión arterial era menos efectivo y todavía no se usaban estatinas como componentes fundamentales de los esquemas de prevención vascular. La optimización de la calidad del tratamiento médico en las últimas décadas ha llevado a una significativa reducción del riesgo de ACV en pacientes no intervenidos quirúrgicamente. En base a estas observaciones y con la excepción de casos específicos, el tratamiento médico es la opción terapéutica de elección en pacientes con enfermedad aterosclerótica carotídea extracraneal asintomática.The reported prevalence of asymptomatic atherosclerotic disease of the extracranial internal carotid artery is up to 12.5%. Carotid angioplasty has not yet proven safe and effective enough to prevent ischemic stroke in these patients. Randomized studies showed that carotid endarterectomy is superior to medical therapy in reducing the risk of ischemic stroke when performed by surgical teams with complication rates (stroke or death) of less than 3%. However, recruitment of these patients began more than 25 years ago, when the use of antiplatelet agents was lower than today, the treatment of hypertension was less effective than currently, and statins were not considered as key components of vascular prevention strategies. Optimizing the quality of medical treatment in recent decades has led to a significant reduction in stroke risk in patients not undergoing surgery. Based on these observations and with the exception of specific cases, medical therapy is the treatment of choice for patients with asymptomatic atherosclerotic disease of the extracranial carotid arteries.

Abstract

Background Information regarding predisposing factors, frequency, and prognostic implications of new onset atrial fibrillation (NOAF) after carotid endarterectomy (CEA) is scarce. We assessed the frequency, risk factors, and the prognostic impact of NOAF after CEA. Methods We assessed every patient undergoing CEA (n = 186) at our academic hospital between 2006 and 2009. Patients underwent continuous electrocardiographic monitoring during surgery and during the rest of hospital stay. We performed univariate and multivariate analyses for identifying variables associated with NOAF and for individualizing variables related to four perioperative adverse outcome measures: a) ischemic stroke; b) ischemic stroke and myocardial infarction, c) ischemic stroke and death, and d) ischemic stroke, myocardial infarction, and death. Results The study cohort comprised 186 patients. Overall, NOAF was detected in 7 cases (3.8%). The only variable associated with NOAF was intraoperative hypotension (OR 9.6, 95% CI 1.9–47.4, P = .006). There were no perioperative deaths. NOAF was associated with perioperative ischemic stroke and with the combined outcome of ischemic stroke and myocardial infarction. Conclusions We found a low frequency of NOAF after CEA. Intraoperative hypotension was associated to a higher risk of NOAF. In turn, NOAF was related to adverse postoperative outcome. Further research is needed to clarify the pathophysiological relation between intraoperative hypotension, NOAF, and adverse CEA outcome.
