1. Effect of Charge-balancing Species on Sm3+ Incorporation in Calcium Vanadinite

Abstract

Apatites are often seen as good potential candidates for the immobilization of halide-rich wastes. In particular, phosphate apatites have received much attention in recent years, however, their synthesis often produces complicated multi-phase systems, with a number of secondary phases forming [1.2]. Calcium vanadinite (Ca5(VO4)3Cl) demonstrates a much simpler phase system, with only a single Ca2V2O7 secondary phase which can easily be retarded by the addition of excess CaCl2. However, when doping with SmCl3 (as an inactive analogue for AnCl3) the Sm forms a wakefieldite (SmVO4) phase rather than being immobilized within the vanadinite, a result of having to form an energetically unfavourable Ca vacancy in order for the lattice to remain neutral overall. It has been postulated that charge-balancing the lattice via co-substitution of a monovalent cation will be less disfavoured and therefore help stabilise formation of a (Ca5-2xSmxAx)(VO4)3Cl solid solution (A = monovalent cation). This has been investigated using a combined modelling and experimental approach. Static lattice calculations performed using Li+, Na+ and K+ as charge-balancing species have shown the energy cost to be less than half that of charge-balancing via formation of a Ca vacancy. As a result, solid state synthesis of (Ca5-2xSmxLix)(VO4)3Cl, (Ca5−2xSmxNax)(VO4)3Cl and (Ca5-2xSmxKx)(VO4)3Cl solid solutions have been trialled, and analysis of the resulting products has shown a significant reduction in both the SmVO4 and Ca2V2O7 secondary phases across all dopant levels.

Abstract

Sodalite (Na8[AlSiO4]6Cl2), a naturally occurring Cl-containing mineral, has long been regarded as a potential immobilization matrix for the chloride salt wastes arising from pyrochemical reprocessing operations, as it allows for the conditioning of the waste salt as a whole without the need for any pre-treatment. Here the consolidation and densification of Sm-doped sodalite (as an analogue for AnCl3) has been investigated with the aim of producing fully dense (i.e. > 95 % t.d.) ceramic monoliths via conventional cold-press-and-sinter techniques at temperatures of < 1000 °C. Microstructural analysis of pressed and sintered sodalite powders under these conditions is shown to produce poorly sintered, porous, inhomogeneous pellets. However, by the addition of a sodium aluminophosphate glass sintering aid, fully dense Sm-sodalite ceramic monoliths can successfully be produced by sintering at temperatures as low as 800 °C.

Abstract

Calcium chlorosilicate (Ca3(SiO4)Cl2) is seen as a potential host phase for the immobilization of Cl-rich wastes arising from pyrochemical reprocessing, a waste stream often containing a mix of both di- and trivalent cations. Substitution of trivalent cations into the lattice requires some form of charge compensation to ensure the lattice remains charge neutral overall. Whilst previous work has only examined this through the formation of Ca vacancies, this study investigates the feasibility of charge-balancing via the substitution of a monovalent cation onto the Ca sites of the lattice. To that end, a series of static lattice calculations were performed to determine the site selectivity of monovalent cations of differing size when substituted onto the Ca sites of the calcium chlorosilicate lattice and the solution energies for the overall substitution processes compared with those for charge compensation via vacancy formation. In all cases the monovalent charge-balancing species shows a clear preference for substitution onto the Ca1 site in the calcium chlorosilicate lattice. The solution energy of the substitution process increases with the increasing ionic radii of both the mono- and trivalent species as the steric stresses associated with substitution of larger cations than the Ca2+ host increase. As such, only charge-balancing using Li+, Na+ or K+ is more favourable than via formation of a Ca vacancy.

4. Molten Salt Synthesis of Zirconolite Polytypes

Abstract

Zirconolite (CaZrTi2O7), a durable and compositionally flexible titanate ceramic for the immobilization of separated actinides, is currently the UK’s preferred candidate phase for the immobilization of plutonium dioxide arising from aqueous reprocessing. Here, its suitability as a waste-form for actinide chlorides arising from pyrochemical reprocessing is investigated through synthesis via a molten salt mediated reaction using a number of different salt eutectics (MgCl2:NaCl, CaCl2:NaCl and KCl:NaCl). It is found that the effectiveness of the molten salt synthesis of zirconolite is governed by the solubility of ZrO2 in the salt medium used; the synthesis proceeding via the formation of a perovskite (CaTiO3) intermediate which then reacts with ZrO2 to form zirconolite via a solution-diffusion mechanism. Most notably, in the KCl:NaCl eutectic different zirconolite polytypes are formed at different synthesis temperatures, with zirconolite-3T forming at 900 °C, giving way to zirconolite-2M at 1200 °C.

Apatites are often seen as good potential candidates for the immobilization of halide-rich wastes and, in particular, chlorapatite (Ca5(PO4)3Cl) has received much attention in recent years. However, synthesis of chlorapatite waste-forms can produce a complicated multi-phase system, with a number of secondary phases forming, including β-TCP (Ca3(PO4)2), spodiosite (Ca2(PO4)Cl) and pyrophosphate (Ca2P2O7), many of which require elevated temperatures and extended calcinations times to reduce. Calcium vanadinite (Ca5(VO4)3Cl) demonstrates a much simpler phase system, with calcination at 750 °C yielding Ca5(VO4)3Cl together a small quantity of a Ca2V2O7secondary phase, the formation of which can be retarded by the addition of excess CaCl2. Characterization of compositions doped with SmCl3as an inactive analogue for AnCl3show the Cl to be immobilized in the vanadinite whilst the Sm forms a wakefieldite (SmVO4) phase.

Abstract

Single-phase calcium chlorosilicate and sodalite, two potential ceramic waste-forms for the immobilisation of CaCl2-based pyroprocessing wastes, have been fabricated at temperatures below the volatilisation point of CaCl2. Solid solutions doped with Sm3+ as an inactive analogue for trivalent actinides have been fabricated and characterised. XRD analysis shows both phases will successfully accommodate Sm3+, with the sodalite in particular remaining single-phase. Fabrication of Sm-doped calcium chlorosilicate in air results in the formation of SmOCl and Ca(Si2O5) secondary phases, however, calcination in an inert atmosphere is shown to successfully retard the formation of SmOCl allowing for higher levels of doping.

Abstract

The value of patient-centered outcomes research is increasingly being recognized as a means to assess the impact of the disease and therapy on the trajectory of an illness, in patients with solid tumor malignancies, including tumors of the central nervous system (CNS). Hypothesis-based research projects are integral to improving our knowledge of the impact of these tumors and their treatment on patients, mandating that survival measures be considered in the context of function and symptom burden. Comprehensive assessment requires integration of research that can accurately measure, predict risk, and explore the biologic basis of both symptoms and toxicity, with the goals of incorporating these parameters into treatment assessment and creating symptom treatment or prevention paradigms. Because of recognition of the importance of this aspect of clinical research, the Outcomes Section of the NOB has been developed with four areas of focus: (1) The development, establishing the utility and assessing the innovation of clinical outcomes assessments (COAs) for use in the primary brain tumor population; (2) Creating mechanisms and methodologies for incorporating clinical outcomes assessment (COAs) into brain tumor clinical trials; (3) Utilizing advanced data management, epidemiologic and informatics technologies, to identify clinical, genomic, and pathway-based predictors and mechanisms of toxicity; and (4) Optimizing patient outcomes by exploring new avenues of patient education and outreach to apply results from outcomes measures. Integrating research across disciplines and incorporation of bench-bedside-bench research is an essential component of the Outcomes Section, designed to provide a model of patient oriented clinical-translational research that may be useful in brain tumor as well as other areas of cancer research. An overview of the program including current projects and future directions will be provided.

Abstract

Screw dislocations in bcc metals display non-planar cores at zero temperature which result in high lattice friction and thermally-activated strain rate behavior. In bcc W, electronic structure molecular statics calculations reveal a compact, non-degenerate core with an associated Peierls stress between 1.7 and 2.8 GPa. However, a full picture of the dynamic behavior of dislocations can only be gained by using more efficient atomistic simulations based on semiempirical interatomic potentials. In this paper we assess the suitability of five different potentials in terms of static properties relevant to screw dislocations in pure W. Moreover, we perform molecular dynamics simulations of stress-assisted glide using all five potentials to study the dynamic behavior of screw dislocations under shear stress. Dislocations are seen to display thermally-activated motion in most of the applied stress range, with a gradual transition to a viscous damping regime at high stresses. We find that one potential predicts a core transformation from compact to dissociated at finite temperature that affects the energetics of kink-pair production and impacts the mechanism of motion. We conclude that a modified embedded-atom potential achieves the best compromise in terms of static and dynamic screw dislocation properties, although at an expense of about ten-fold compared to central potentials.

Abstract

Intrathecal administration of aluminum (AI) salts to susceptible species causes prominent accumulations of neurofilaments (NFs) in neurons of the CNS. Involved nerve cells display abnormal phosphorylation of perikaryal NFs, impaired axonal transport of NFs, and reduced levels of mRNA for NF proteins. Further understanding of the pathogenesis of AI toxicity has been limited by difficulties inherent in the available in vivo systems. For this reason, we have developed a model to study the effects of AI on cultured sensory neurons. Explant cultures of rat dorsal root ganglia (DRG) were exposed to 1 mM aluminum lactate for 1 d, 3 d, or 7 d and then examined morphologically. Accumulations of NFs were noted as early as 1 d after exposure, and prominent masses of NFs were seen at 3 and 7 d. Northern analysis of mRNA extracted from the cultured ganglia showed that high, medium, and low molecular weight NF protein mRNA levels were markedly reduced compared to control values by 1 d of exposure. Class II beta- tubulin mRNA was also moderately decreased. Reversibility of toxicity was assessed by removing the aluminum lactate from the medium after a 3 d exposure and examining the cultures 1 week later. The perikaryal masses of NFs dispersed and the levels of mRNA coding for the NF proteins and class II beta-tubulin increased. The neurotoxic effects of AI on cultured DRG recapitulates the effects of intrathecal administration of AI on animals; this model produces similar changes in neuronal morphology with neurofilamentous masses and similar modifications of NF gene expression.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract

Metastatic tumors are by far the most common cause of brain cancer, with an incidence rate higher than all other types of primary brain tumors combined. Cancer cells can spread to the brain from other sites via the blood supply, lymphatic system, or direct extension. Neurologic symptoms, which are dependent on tumor location, often include headache, weakness, or cognitive deficits. Although the standard of care is evolving, treatment often includes some combination of surgery and radiation therapy. Newer treatments, including radiosurgery and novel chemotherapy approaches, currently are being investigated. These treatment advances have markedly altered the prognosis for patients with brain metastases. Consequently, the status of the systemic components of the malignancy often may be the determinant of outcome.

To provide a review of glial neoplasms including histologic classification, signs and symptoms, diagnostic evaluation, treatment options, and nursing care.Articles, books, proceedings of national meetings.Glial neoplasms vary according to prognosis and treatment options based on histologic classifications. Prediction of deficits and potential complications of disease and treatment can prepare the patient and healthcare provider in management. Treatment options include surgery, radiation therapy, and chemotherapy or combinations of these options. New treatment regimes currently are being investigated.Despite advances in the diagnosis and treatment of malignant gliomas, the outlook remains poor. As new combination treatment regimens become available, nursing care will significantly affect outcome and quality of life.Goals of nursing care include prevention and education related to tumor- and treatment-related side effects, providing support and coordinating referrals to appropriate services during the disease process, and helping the patient and family cope with this devastating disease.

26. Management of seizures in the adult patient with cancer

Abstract

Seizures are a frequent neurologic complication of cancer. Seizures may develop as a consequence of direct extension of cancer into the brain or surrounding meninges, an underlying metabolic imbalance from treatment or the tumor, or the neurotoxic effects of cancer therapy. Emergency treatment is essential for status epilepticus and seizures that occur frequently to prevent permanent neurologic sequelae. In all cases, a diagnostic evaluation is required to discover the underlying cause of the seizures. Often, an easily reversible process will be uncovered, and the complete resolution of the seizure disorder can occur. In other circumstances, determining the cause of the seizure may allow early diagnosis and treatment of brain lesions or may disclose neurotoxicity, thereby modifying future treatment. This article reviews the causes, diagnostic evaluation, and treatment of seizures in adult patients with cancer, providing an overview of the medical and nursing aspects of patient management.

28. Analgesic decision-making skills of nurses

Abstract

Administration of analgesics is a common nursing intervention for providing pain relief to patients with cancer. A sample of 177 nurses participated in a study to examine their decision-making skills when analgesic orders were changed. Nurses were given four clinical vignettes and reference materials to assist them in responding to the vignettes. Twenty-six percent of 708 responses provided by the nurses were answered correctly. Twenty-nine percent of the sample answered all four vignettes incorrectly. The majority (44%) answered only one vignette correctly. No statistically significant differences were found between correct answers and type of work setting, academic preparation, number of patients with cancer cared for per week, number of years employed as a nurse, recent clinical experience with the analgesics used in the vignettes, or the use of reference materials. These findings raise concerns that patients requiring analgesics may not receive an amount that adequately relieves their pain or avoids unnecessary toxicity.

Abstract

The mechanism of methotrexate (MTX)-induced neurotoxicity was investigated using cerebellar explant cultures from fetal rats. After 3 weeks of growth, myelinated cultures were treated with MTX at 1 microM, lysolecithin at 1 mg/dl, or unaltered nutrient medium. Myelin sheaths devoid of axons were observed by histological and electron microscopic preparations after 2 weeks of MTX exposure. After 5 weeks, cultures were almost entirely devoid of myelin sheaths. Myelin basic protein in the media removed from the cultures showed an increase in concentration after 3 weeks of MTX exposure and was significantly greater than control after 5 weeks of exposure. 2',3'-Cyclic nucleotide 3'-phosphohydrolase activity, a measure of oligodendroglial function, was not significantly different in the MTX group compared to controls. Lysolecithin-treated cultures showed widespread destruction and an early increase in myelin basic protein release into the medium. These data indicate that, in the cerebellar explant cultures, MTX is primarily a neuronal toxin, and the demyelination is a consequence of axonal loss and is not related to a change in oligodendroglial cell function. These findings provide new insight into the pathogenesis of MTX-induced neurotoxicity.

Abstract

We sought to determine if verapamil induces frequency-dependent prolongation of atrioventricular nodal conduction in 10 consecutive patients studied in the electrophysiology laboratory. We used a maintenance infusion of verapamil designed to produce plasma concentrations of verapamil in the "therapeutic" range and that did not alter heart rate or blood pressure significantly. Frequency-dependent prolongation of atrioventricular nodal conduction (AH interval) was demonstrated in all 10 patients (p less than .001), and no change in HV conduction time with decreasing cycle length was noted in any patients while receiving verapamil. Two patterns of use-dependent response were seen. In four patients frequency-dependent prolongation of the delta(AH) interval [delta(AH) = AHverapamil - AHcontrol at a given cycle length] was seen with each decrement in pacing cycle length. In six patients frequency-dependent prolongation of the delta(AH) interval was not manifest until the fifth to eighth pacing cycle length tested. There was no association between the pattern observed and the initial heart rate or AH interval. After an abrupt change in pacing cycle length, the kinetics of delta (AH) interval prolongation were rapid; equilibrium was achieved by five to eight pulses in all patients. There was no correlation between the magnitude of prolongation of the AH interval noted at a particular cycle length and the concentration of verapamil during the maintenance infusion. These results indicate that verapamil causes use-dependent prolongation of atrioventricular nodal conduction in man.

Abstract

Background: To illustrate challenges of imaging interpretation in patients with oligodendroglioma seen at a referral center and evaluate interrater reliability. Methods: Two neuro-oncologists reviewed diagnostic preradiation MRIs of oligodendroglioma patients; interrater reliability was calculated with the kappa coefficient (k). A neuroradiologist measured presurgical apparent diffusion coefficient (ADC), if available. Results: Extensive enhancement was noted in four of 58 patients, k = 0.7; necrosis in seven of 58, k = 0.61; calcification in seven of 17, k = 1.0; diffusion restriction in two of 39 patients, k = 1.0 (all only in grade 3). ADC values with receiver operator characteristic analysis for area under the curve were 0.473, not significantly different from the null hypothesis (p = 0.14). Conclusions: Extensive enhancement, necrosis and calcification correlated with grade 3 oligodendroglioma in our sample. However, interrater variability is an important limitation when assessing radiographic features, supporting the need for standardization of imaging protocols and their interpretation.
