Abstract: This article presents the results of a study of gold samples obtained by students during a practical field trip in the summer of 2014. Placer gold samples retrieved from four rivers in southeast Siberia (Khakassia and Tyva territories) by panning and sluicing were described and analyzed compositionally by Scanning Electron Microscopy. There is evidence from grain flattening and morphology for significant variations in gold transport distance, both within and between sample locations. The composition and texture of gold is compared to similar studies in the Yukon, and it is inferred that most of the placer gold in the region originated from orogenic lode sources. This orogenic gold is of Devonian to Carboniferous age. There is also evidence for a contribution from igneous intrusion-related bedrock gold, which is supported by the presence of granite, granodiorite and sienite intrusions of Devonian age. There is scope for further study, since relatively few grains were analyzed here. In addition, if compositional data of the prospective primary gold deposits can be obtained, there would be potential for more precise determination of provenance.

Abstract: Abstract Background Parkinson’s disease (PD) affects approximately 145,519 people in the UK. Speech impairments are common with a reported prevalence of 68%, which increase physical and mental demands during conversation, reliance on family and/or carers, and the likelihood of social withdrawal reducing quality of life. In the UK, two approaches to Speech and Language Therapy (SLT) intervention are commonly available: National Health Service (NHS) SLT or Lee Silverman Voice Treatment (LSVT LOUD®). NHS SLT is tailored to the individuals’ needs per local practice typically consisting of six to eight weekly sessions; LSVT LOUD® comprises 16 sessions of individual treatment with home-based practice over 4 weeks. The evidence-base for their effectiveness is inconclusive. Methods/design PD COMM is a phase III, multicentre, three-arm, unblinded, randomised controlled trial. Five hundred and forty-six people with idiopathic PD, reporting speech or voice problems will be enrolled. We will exclude those with a diagnosis of dementia, laryngeal pathology or those who have received SLT for speech problems in the previous 2 years. Following informed consent and completion of baseline assessments, participants will be randomised in a 1:1:1 ratio to no-intervention control, NHS SLT or LSVT LOUD® via a central computer-generated programme, using a minimisation procedure with a random element, to ensure allocation concealment. Participants randomised to the intervention groups will start treatment within 4 (NHS SLT) or 7 (LSVT LOUD®) weeks of randomisation. Primary outcome: Voice Handicap Index (VHI) total score at 3 months. Secondary outcomes include: VHI subscales, Parkinson’s Disease Questionnaire-39; Questionnaire on Acquired Speech Disorders; EuroQol-5D-5 L; ICECAP-O; resource utilisation; adverse events and carer quality of life. Mixed-methods process and health economic evaluations will take place alongside the trial. Assessments will be completed before randomisation and at 3, 6 and 12 months after randomisation. The trial started in December 2015 and will run for 77 months. Recruitment will take place in approximately 42 sites around the UK. Discussion The trial will test the hypothesis that SLT is effective for the treatment of speech or voice problems in people with PD compared to no SLT. It will further test whether NHS SLT or LSVT LOUD® provide greater benefit and determine the cost-effectiveness of both interventions. Abstract: В статье представлены результаты изучения проб россыпного золота, отобранных студентами во время полевой практики летом 2014 г. на четырёх реках Юго-Восточной Сибири с помощью лотков и винтового шлюза, описанных и проанализированных методами электронной микроскопии. Нами установлено, что особенности морфологии зерен золота свидетельствуют о значительном отличии в расстояниях их транспортировки как в пробах с одного места отбора, так и между пробами с разных мест. Состав и текстура золота сравнимы с наблюденными при подобных исследованиях на Юконе. Это позволило предположить, что большая часть россыпного золота в местах отбора проб связана с источниками в коренных золотосодержащих породах эндогенного происхождения девонско-каменноугольного возраста. Присутствие многочисленных интрузий гранитов, гранодиоритов и сиенитов является подтверждением того, что источники россыпного золота могут быть связаны также с коренным золотом интрузивов девонского возраста. Остаются широкие возможности для дальнейших исследований, т. к. в результате работ изучено очень небольшое количество зерен. Следует отметить, что если бы была возможность изучения состава коренного золота территорий, то это позволило бы более точно определить расположение источников россыпного золота из отобранных проб., This article presents the results of a study of gold samples obtained by students during a practical field trip in the summer of 2014. 7. SELECTIVE ENDOSCOPY IN MANAGEMENT OF INGESTED FOREIGN BODIES OF THE UPPER GASTROINTESTINAL TRACT: IS IT SAFE?

Abstract: During a four-year period, 308 patients presented following ingestion of foreign bodies. Ingestion was accidental in 272 cases (88.3%) and deliberate in the remainder. Symptoms at presentation included dysphagia, odynophagia, nausea and vomiting, chest pain and pharyngeal discomfort. Sixty-eight patients were asymptomatic. A policy of expectant management and selective endoscopy was employed. Following initial assessment 202 patients (65.6%) were discharged without treatment, 30 (9.7%) of whom were later reviewed as outpatients and did not require admission. Forty-nine patients (16%) were admitted for treatment; 27 had oesophagoscopy, five bronchoscopy and two had foreign body extraction with direct laryngoscopy. In nine patients who were endoscoped, no foreign body was identified. Twenty-seven others were referred to the otorhinolaryngology service in another hospital. There were no deaths in the group and morbidity was 1.2%. We conclude that a policy of selective endoscopy is safe and effective in the management of patients following ingestion of foreign bodies.

Abstract: Summary Of splenectomies performed in the Cork Regional Hospital over an 11 year period, ten were undertaken primarily for diagnostic purposes. A definitive histological diagnosis was established in nine patients, seven of whom had lymphoma, two with Hodgkin's disease and five with non-Hodgkin's lymphoma. The weight of the excised spleen in all patients with lymphoma exceeded 1 kg; in all those with a diagnosis other than lymphoma, the spleen weighed less than 1 kg. A majority of patients also had symptomatic improvement from reversal of hypersplenism and from relief of the mechanical pressure effects of an enlarged spleen. Operative mortality was zero. Diagnostic splenectomy is a worthwhile procedure. Most patients will have lymphoma.

12. A modified technique of delayed primary closure using a povidone iodine wick: Influence on wound healing in an experimental model

Abstract: We report a modified technique of delayed primary wound closure using a wound wick. A rodent model was used to determine its influence on healing. After loose primary closure of two paravertebral incisions on the dorsum of male rats, a 10 cm length of ribbon gauze, soaked in either saline or 1% povidone-iodine, was inserted into the right sided wound. Wicks were soaked daily with the same solution until removal on the third post-operative day. The inclusion of a wick soaked in saline had an early transient effect on healing. A more prolonged impairment of healing was demonstrated in povidone-iodine wicked wounds and their respective controls. These data suggest that povidone-iodine impaired healing not only in the wicked wound, but also inhibited healing in the adjacent unwicked wound which was not directly exposed to the antiseptic. The observed delay in healing in povidone-iodine wicked wounds must however be balanced against its beneficial effect on wound infection rates. We suggest that further evaluation of this method of closure in contaminated wounds is warranted.

Abstract: Deliberate and recurrent foreign body ingestion is a common problem among institutionalised patients. We review our experience with 36 cases of deliberate foreign body ingestion by prisoners or psychiatric patients, thirty of whom were institutionalised at the time of ingestion. Symptoms were frequently severe in the prison inmate group but, in contrast, psychiatric patients presented with few, if any, symptoms. A majority of objects pass spontaneously or remain in situ without complication. Twenty-four patients were discharged following initial evaluation and without specific treatment. Eight of these were reviewed electively and discharged within one week. Twelve patients were admitted for observation, seven of whom were discharged within 48 hrs. Upper gastrointestinal endoscopy was performed in four patients and an intragastric foreign body identified in two cases. Laparotomy was performed in two cases for unresolving mechanical intestinal obstruction. Management should be conservative when possible, with surgery indicated only for complications.

Abstract: Mesenteric panniculitis is a recently described, non-specific inflammatory disorder of the intestinal mesentery. The majority of cases reported concern the mesentery of the small intestine. We report a case of mesenteric panniculitis of the colon in a patient with Crohn’s disease and review 22 reported cases of mesenteric panniculitis of the colon.

Abstract: Peritoneal adhesions cause much long-term postoperative morbidity. This study evaluates the efficacy of polyethylene glycol (PEG) 4000 in reducing adhesion reformation after lysis. Adhesions were induced, by abrasion, in 111 Sprague-Dawley rats at a first laparotomy. At a second operation, 10 days later, these adhesions were graded and lysed, after which the animals received one of the following solutions intra-peritoneally: 5 per cent PEG 4000 (n = 21), 25 per cent PEG 4000 (n = 23), 32 per cent dextran 70 (n = 22) or isotonic saline (n = 25), or were left as an untreated control group (n = 20). When the reformed adhesions were graded after a further 10 days 5 per cent PEG 4000 was found to be the only solution that inhibited adhesion reformation. The adhesions that reformed in the other four test groups were significantly worse than when they were first graded (P ≦0·033 for all groups). Therefore 5 per cent PEG 4000 may be useful in clinical practice for the reduction of adhesion formation after lysis.

Abstract: Intraperitoneal (I.P.) administration of chemotherapeutic agents, in particular, cisplatin and bleomycin is successfully used in the treatment of ovarian and gastrointestinal malignancies. Their use however, is limited by the rapid development of adhesions. The vinca alkaloid group of chemotherapeutic agents has been shown to impair the acute inflammatory reaction, which plays an important role in adhesion formation, thereby providing the rationale for addition of vindesine to an I.P. regimen containing cisplatin and bleomycin. A rat model was used to study the effects of various I.P. chemotherapeutic regimens on adhesion formation. Four groups were studied (30 rats/Group). Regimens used included Group A (Saline), Group B (Vindesine 0.1 mg/kg), Group C (Cisplatin 5 mg/kg and Bleomycin 0.2 mg/kg), and Group D (Cisplatin 5 mg/kg, Bleomycin 0.2 mg/kg and Vindesine 0.1 mg/kg). In order to grade adhesion formation, animals were sacrificed on days 2, 14 and 28, with the severity of adhesions graded on a scale of I-IV in order of increasing severity. Adhesion formation in Group A (Saline) and Group B (Vindesine) was similar. The addition of Vindesine to Group C (Cisplatin and Bleomycin) resulted in a significant reduction in adhesion formation (p less than 0.01). The results of this experimental study suggest that the local toxicity of I.P. chemotherapy may be reduced by the inclusion of a vinca alkaloid.

24. A quantitative study of articular repair in the guinea pig

Abstract: This study examined the healing of articular defects, with and without carbon fiber implants, and the response of repair tissue to its interim removal in guinea pigs of different ages. These were investigated after the induction of full thickness articular cartilage defects in the patellar groove of skeletally mature and immature guinea pigs. To indicate its capacity for replacement after attrition, repair tissue occurring in untreated (control) and carbon fiber treated (experimental) defects was ablated after 8 weeks, and the animals were sacrificed after an additional 8 weeks. The repair tissue was studied quantitatively at gross and microscopic levels and qualitatively using scanning and transmission electron microscopic study. The principal findings were as follows. The initial formation of repair tissue was variable, but it occurred in most cases. Secondary repair tissue formation consistently occurred after excision. Age did not influence the degree of repair. Carbon fiber implants of the type used impaired healing of small full thickness articular cartilage defects, compared with no treatment. Microscopically, repair tissue contains five main cell types, each with a characteristic surrounding matrix. Intermediate forms also are found. These, together with four of the five main types comprise a morphologic continuum and fit readily into a proposed developmental sequence that may stem from the fibroblast.

Abstract: This article describes a 22 year experience of a general surgical unit in the treatment of infantile hypertrophic pyloric stenosis (IHPS). The hospital course of 229 IHPS patients is reviewed. The male:female ratio was 3.6:1, median age 6 weeks (range 2-26 weeks) with a positive family history in 8.3%. The diagnosis of IHPS was established clinically by palpation of a "pyloric tumour" during a pre operative test meal/clinical examination in 92.6%; in the remainder, the diagnosis was made radiologically. Ramstedt's pyloromyotomy was performed within 5 days of admission in 74% of patients and within 10 days of admission in 89%. The median post-operative hospital stay was 10 days (range 3-60 days). Wound morbidity occurred in 10.0% wound infection (7.3%) and wound dehiscence (2.6%). However, wound morbidity was reduced in the second half of the series, partly by greater utililisation of non-absorbable suture in place of chromic catgut for wound closure. Mucosal penetration was suspected in 14.8% of cases. Repeat pyloromyotomy was necessary in 1.3%. One baby died (0.4%)- this was in the early part of the series and was directly attributable to fluid and electrolyte disorder. We conclude that Ramstedt's pyloromyotomy for infantile hypertrophic pyloric stenosis can be performed with acceptable morbidity and minimal mortality in a general surgical unit.

26. An exploratory study of the pattern of consent for autopsy in a regional hospital setting

Abstract: A prospective study of the pattern of responses to requests for autopsy in a general surgical unit was performed. Information on the characteristics of the deceased, of the requestee and of the requester was documented in the case of 66 patients who died while in hospital. Permission to perform autopsy was not requested in 39 out of 66 cases and this was the most frequent contributory factor to the low rate of autopsy. Once a decision to grant or refuse autopsy is made by relatives of the deceased, the decision is unlikely to be reversed. Permission to perform autopsy was more likely to be sought when the deceased was male than when deceased was female. The relatives of patients who had recently undergone surgery were more likely to refuse permission for autopsy than were those of patients who had not had recent surgery.

Abstract: During an 8-year period, 104 patients (19 males, 85 females) underwent surgery for a solitary thyroid nodule. There were 19 (18.3%) malignant nodules and 85 (81.7%) benign nodules. Malignancy occurred in 3 (15.8%) males and 16 (18.8%) females. Pre-operative scanning was performed prior to referral in 59 patients (52 had ultrasound, 41 had isotope scanning; the majority had both). Of the scanned patients, 12 (20%) had a malignant nodule while 7 (15.5%) of the 45 patients who did not have scanning had a malignant nodule. In patients who had an ultrasound scan, malignancy was found in 5 (23.8%) of the 21 solid nodules and 7 (22.7%) of the 31 cystic (or solid/cystic) nodules. In patients who had thyroid isotope scanning, malignancy was found in 6 (17.5%) of the 34 cold (non-functioning) nodules and 3 (43%) of the 7 warm or hot (functioning) nodules. Ultrasound and isotope scanning may be misleading and neither help to differentiate benign from malignant thyroid nodules which require surgical excision.

Abstract: The increased risk of sepsis in patients following splenectomy has been well documented. Fear of overwhelming post-splenectomy sepsis (OPSI) has resulted in a generalized trend towards splenic salvage among surgeons. However, splenorrhaphy and attempts at splenic salvage may of themselves predispose to significant morbidity, sometimes more serious than increased susceptibility to infection associated with splenectomy. This study aims to assess the risk of splenectomy and subsequent asplenia. We reviewed 246 patients who underwent splenectomy over a 16 year period. Indications for splenectomy were considered under the following headings: haematological (N = 116), trauma (N = 69), visceral carcinoma (N = 28), incidental (N = 13) and miscellaneous (N = 20). There were 28 deaths in the series, primarily among those in the intra-abdominal carcinoma (13) and multiple trauma (13) groups. Two deaths were recorded among patients undergoing elective splenectomy for benign disease. Thrombo-embolic complications were recorded in nine patients; respiratory tract infection in 36 patients and intra-abdominal abscess in two patients. Two cases of post-splenectomy pneumococcal septicaemia were documented, neither of which was fatal. While not an entirely benign procedure, splenectomy can be performed relatively safely, especially when performed for benign disease in an adult population.

Abstract: In an attempt to identify patients at risk of breast cancer, 9E 10 c-myc monoclonal antibody was used to detect the oncogene products in 64 breast tissue biopsies. There were 44 specimens of benign breast disorders: 21 from patients who developed subsequent breast cancer, and 23 from patients who did not. The remainder were malignant tumor biopsies from those who developed breast cancer. C-myc over-expression was identified in 12 (60%) of the breast carcinomas; it was also noted in 13 (62%) of the benign biopsies from patients who developed subsequent breast carcinoma. Expression was observed in only 3 (13%) controls—patients who did not develop cancer. A prospective study is now warranted to determine the clinical usefulness of our findings. © 1993 Wiley-Liss, Inc.

Abstract: Parathyroidectomy is an established surgical treatment for symptomatic hyperparathyroidism. In this regional hospital experience, 50 patients who had undergone a parathyroidectomy were looked at. We examined clinical indications and noted trends for hypertension, neuromuscular symptomatology and psychiatric complaints to occur commonly as presenting symptoms. Operative results and histology results are examined and correlated. Particular note is made of any postoperative complications.

Abstract: This study investigates the ultrastructure of articular tissue generated on osteochondral defects in skeletally immature rabbits from free reversed periosteal allografts after 50 and 100 days of post-operative intermittent active motion. Tissue samples prepared for transmission electron microscopy were compared with normal cartilage and periosteum in terms of cell morphology and the pattern of intercellular collagen deposition. Well-differentiated tissue demonstrated many ultrastructural features of normal articular cartilage while poorly differentiated samples contained cells and intercellular collagen profiles which were somewhat similar to those observed in periosteum.

Abstract: Primary hyperhidrosis, although lacking a precise definition and of unknown aetiology, disrupts professional and social life and may lead to emotional problems. A variety of treatment methods are used to control or reduce the profuse sweating which involves mainly the palms, soles and axillae. The simplest method, the application of topical agents, is usually attempted first for axillary and plantar sweating. Iontophoresis may provide relief especially in patients with plantar or palmar involvement. In severe cases operative intervention is necessary. Excision of sweat glands is successful in patients with axillary hyperhidrosis but the role of suction-assisted removal of axillary sweat glands remains to be determined. Sympathectomy remains the standard by which other treatments must be judged. For upper thoracic sympathectomy a variety of surgical approaches are used with satisfactory relief of hyperhidrosis. Complications related to the surgical approach, such as Horner's syndrome, brachial plexus injuries, pneumothorax and painful scars may occur, while following sympathectomy compensatory hyperhidrosis is usual and hyperhidrosis may recur. Plantar hyperhidrosis which may be exacerbated or ameliorated by upper thoracic sympathectomy and which fails to respond to non-operative intervention is relieved by lumbar sympathectomy.

Abstract: Marlex™ (polypropylene) mesh was used to replace the abdominal wall in massive incisional herniation in 50 patients. Seventeen unsuccessful attempts at primary repair had previously been made on nine patients. Patient follow-up ranged from 6 to 120 months (mean = 45 months). There was minor hernia recurrence in four (8 per cent) patients. In each instance it was due to partial detachment of the peripheral fixation of the patch. Complications were: wound infection, four (8 per cent); wound seroma, two (4 per cent); wound haematoma, one (2 per cent); and wound sinus, six (12 per cent). Complications did not necessitate removal of the patch in any case. Inadequate peripheral attachment of the patch has been the only cause of hernia recurrence and should be avoidable.

Abstract: Laves phase (Cr2X) strengthened Cr solid solution [Cr(X)]-based alloys are under development for a variety of high-temperature structural applications. Among the most promising alloys are castings based on the Cr(Ta)-Cr2Ta eutectic, which exhibit room-temperature fracture toughness in the 10-12 MPa√m range and creep rupture lives in excess of 1000 h at 1000° (138 MPa) in air. These alloys exhibit superior high-temperature oxidation resistance to previously developed Cr(Nb)-Cr2 Nb based alloys despite the general similarities of Ta and Nb. In order to optimize high-temperature oxidation resistance an understanding of why the Cr(Ta)-Cr2Ta alloys are more oxidation resistant is needed. Detailed EPMA characterization of the oxidized scaleswas initiated as a first steptowards gaining this understanding.Fig. 1 shows a backscattered electron image of cast and heat treated and heat treated Cr-10Nb at.% after a 6 cycle, 120 h exposure at 1100°C in humid air. Most of the coupon was converted to a complex mixture of Cr-, Nb-, and Cr+ Nb- oxides.

44. Oxidation Behavior of Cr2N, CrNbN, and CrTaN Phase Mixtures Formed on Nitrided Cr and Laves-Reinforced Cr Alloys.

Abstract: A series of single-phase Cr(X), single phase Cr 2X Laves phase, and two-phase Cr(X) + Cr2 X alloys (X = Nb or Ta) were thermally nitrided for 24 hr at 1100°C in N2--4H2 and then oxidized for 2 hr at 1100°C in air. The Cr(X) phase nitrided to form Cr2N while the Cr2X phases nitrided to form a complex local mixture of Cr2N/Cr and CrNbN/CrTaN, Cr3Nb3N/Cr3Ta3N phases depending on the depth in the nitrided zone. The Ta only slightly increased the isothermal oxidation rate of nitrided Cr(Ta) and Cr2Ta-reinforced Cr alloys, compared with nitrided, unalloyed Cr. Further, the nitrided two-phase alloys Cr--9.5Ta and Cr--20Ta exhibited improved Cr2O3 scale adherence relative to nitrided unalloyed Cr and Cr--1Ta. In contrast, Nb was detrimental to the oxidation resistance of the nitrided Cr(Nb) and Cr2Nb-reinforced Cr alloys, resulting in the formation of nonprotective Cr--Nb oxides rather than continuous Cr2O3. A phenomenological explanation for these effects based on phase chemistry and microstructural distribution is presented. Implications of these results for understanding the oxidation behavior of developmental high-temperature, Laves-strengthened Cr alloys, as well as possible applications as oxidation and wear-resistant coatings are discussed. Abstract: An analysis of 500 consecutive operations on the biliary tract for benign disease, performed over a 5 year period is presented. Twenty-two per cent of patients had common duct exploration, and the incidence of choledochotomy increased with advancing age. The overall incidence of choledocholithiasis was 15.2% and calculi in the common duct were found more frequently in the elderly. The use of operative cholangiography increased over the 5 year study period, but was not associated with any change in the incidence of common bile duct exploration or choledocholithiasis. Four patients had re-exploration for retained calculus (0.8%). The overall operative mortality was 0.6%.

Abstract: A PROSPECTIVE study was designed to investigate the relationship between post-operative hypoxaemia and duration of anaesthesia. The Po2 was monitored pre-operatively, 4 hours postoperatively, and daily thereafter for 2 days in 33 female patients undergoing elective biliary tract surgery. A significantly greater fall in Po2 occurred in these patients who had prolonged anaesthetics suggesting that this group of patients should be carefully monitored during the post-operative period and oxygen administered if necessary.

Abstract: Forty-seven patients with chronic duodenal ulceration have had a pylorus-preserving gastrectomy carried out. The mean reduction in maximal acid output after the procedure was 65 per cent. Pyloric sphincter function appeared normal postoperatively and gastric emptying time was slow without stasis. Postoperative dumping was absent and there has been no evidence of recurrence. In spite of retention of a cuff of antrum circulating gastrin levels are greatly reduced. The results appear to justify the use of the procedure for uncomplicated duodenal ulcer.

50. Lollard Sources of ‘The Pore Caitif’

Abstract: The extraordinary productivity of John Wycliffe and his followers is evident in the number of their extensive projects that despite ecclesiastical condemnation survived to modern times: the large corpus of Wycliffe's own Latin treatises and the grand-scale undertakings he inspired including the translations of the Bible, the Glossed Gospels, the vernacular sermon cycle, the multiple versions of the Floretum, and the numerous Lollard tracts. Fourteenth- and fifteenth-century writers in England would inevitably have been aware of these Wycliffite resources, and the present study will indicate that the compiler of the orthodox collection of Middle English tracts known as The Pore Caitif(PC), ca. 1395–1402, used several of the great Lollard reference works in assembling his materials. In four sections below, evidence is presented that links The Pore Caitif tothe Glossed Gospels, an Early Version of the Wycliffite Bible (EV), the Lollard translation of the pseudo-Augustinian De salutaribus documentis, and possibly the Floretum.
