Aims The results of previous randomised controlled trials suggest that radiation oncologists should consider the presence of neuropathic pain when they prescribe dose fractionations for painful bone metastases. Although validated screening tools for neuropathic pain features are currently available, the prevalence of such features among patients with painful bone metastases is still poorly understood. The purpose of this study was to estimate the prevalence of neuropathic pain features among patients who received palliative radiotherapy for painful bone metastases. Materials and methods We conducted a cohort survey of consecutive patients who received palliative radiotherapy for painful bone metastases at St Luke's International Hospital between 2013 and 2014. Patients were prospectively assessed before radiotherapy using the validated screening questionnaire to identify neuropathic pain components in Japanese patients. Pain with neuropathic features was prospectively defined using the total score of the seven-item questionnaire and a cut-off score ≥9. The pain response was assessed 2 months after the start of radiotherapy according to the criteria defined by the International Bone Metastases Consensus Working Party. Results Eighty-seven patients were assessed. Twenty-four per cent of patients (95% confidence interval: 16–35%) were diagnosed as having pain with neuropathic features. On multivariate analysis, no significant correlations were seen between neuropathic pain features and patient characteristics. Sixty-four patients (74%) were assessable 2 months after the start of radiotherapy. Overall response rates were 59% (95% confidence interval: 33–82%) in patients with neuropathic features and 55% (95% confidence interval: 40–70%) in those without such features. Conclusions A considerable proportion of the patients were proven to have bone pain with neuropathic features. Further investigations are warranted to validate symptom assessment tools in cooperation with pain distribution and image findings, and to clarify if the presence of neuropathic pain affects the response to palliative radiotherapy.

An AC–DC method has been proposed for simultaneous measurements of Hall effect and magnetoresistance effect in solid and liquid state of Ga and Hg metals. In low magnetic field Hall signal in solid state is proportional to magnetic field B, while in liquid state Hall signal is affected by magnetoresistance effect. It has been found that magnetoresistance has a B2 dependence on magnetic field and affects the Hall signal. In high magnetic field, the Hall effect in liquid state is affected by a very large magnetoresistance effect compared in solid state. The magnetoresistance effect in liquid state is higher than solid state.

There are few papers comparing FDG and MET uptake in miscellaneous brain tumors. In the present study, we demonstrated both PET studies of five patients with untreated brain tumors (gemistocytic astrocytoma, tanycytic ependymoma, astroblastoma, gliomatosis cerebri and medulloblastoma). The results show more marked MET uptake into the tumors compared with FDG. As MET PET has an advantage for FDG in delineation of tumor margins, it will be a more promising method to evaluate biological behavior and for instructing surgical procedure or radiotherapy in various brain tumors.

Background The mammalian mandible develops around Meckel's cartilage and other secondary cartilages, including the dentary. There have already been many studies of the development of the rat mandible that have employed histological serial sections. However, no previous investigators have captured the three-dimensional features of the developmental process. Methods In this study, the technique of double staining with alizarin red S and alcian blue was employed directly on whole body specimens to investigate the three-dimensional development of the rat mandible. Results and Conclusions We found that the molar socket obstructs the developing mandible, causing it to emerge medial to Meckel's cartilage. Our results also indicate that the secondary mandibular cartilages may contribute to supplementary growth in response to local factors. Anat. Rec. 249:233-239, 1997. © 1997 Wiley-Liss, Inc.

We demonstrate a new type of fiber optic pH sensor, which is the application of evanescent-wave spectroscopic technique. A methyl red (MR)-doped-poly(methyl methacrylate) (PMMA) film that coated as part of cladding does function as a pH sensor probe. In this system MR doped in PMMA is used as indicator dye for pH measurement. The absorption spectrum shift in wavelength of indicator dye enables us to get the pH value. The sensor probe is immersed in water solution containing a small proportion of acetic acid over the wide pH range of 5.0 to 7.0. The chemical interaction between MR in sensor probe and hydrogen ion in the water solution causes a change in the dipole moment of MR, that is, the absorption spectrum macroscopically. The evanescent-wave spectroscopic technique provides the measurement of the absorption spectrum shift over a broad range of visible wavelength. The result of experiment was that MR absorption spectrum shifted by 40 nm every increase of 1.0 in pH. The small change in the pH value can be sensed as a large wavelength shift of pH indicator absorption spectrum.

In this paper, a sol–gel derived coating U-shaped optical fiber sensors for measurement of critical micelle concentration of surfactants has been studied. The doped sol–gel cladding fiber was used to construct an active cladding U-shaped optical fiber CMC measurement sensor. The porous sol–gel film was deposited a 1000 μm core plastic clad silica fiber after removing the cladding of the fiber at the middle portion nearly 6 cm and bending in U-shaped design. CMC detection is based on an adsorption effect in sample solution of sodium dodecylbenzenesulfonate. Three different types of particles size of sol–gel porous cladding were tested in our lab. Test results show that the sensitivity of the small particle size porous sol–gel cladding U-shaped optical fiber in sensing region is higher than that of large particle size sol–gel cladding based U-shaped.

In this paper the critical micelle concentration (CMC) in surfactant solutions was detected by using an optical fiber. CMC detection is based on an adsorption effect in sample solution. The incident beam passing through the sensing region of the fiber reflected at the interface between the fiber core and the solution with repeating reflections. The change in adsorption condition leads to an effective change in the refractive index at the surrounding surface of the core. The output signal suddenly increases at the CMC due to the reflectivity change caused by adsorption of surfactant molecules onto the surface of the fiber. Plastic clad silica (PCS) fiber with a uniform sensing region was used to measure the CMC point in surfactant solution. It has been found that sensitivity of the sensor depends on the length of the sensing region, the launching condition of the light source and the optical fiber material. The higher the sensing length, the higher the sensitivity of the sensor. The U-shaped sensing region also tried to find the CMC point, and the experimental results indicate that the U-shaped sensing region is more sensitive than the uniform sensing region. Using this method we are able to detect the CMC of the surfactant solution very accurately and in real time.

Bi[sub 2+[ital x]]Sr[sub 2[minus][ital x]]CuO[sub [ital y]] (0.10[le][ital x][le]0.40) polycrystalline samples have been studied by x-ray diffraction, resistivity and Hall-coefficient measurements, x-ray and ultraviolet photoemission spectroscopy, and inverse-photoemission spectroscopy (IPES). The lattice constant of the [ital c] axis decreases and that of the [ital a]([ital b]) axis increases as a function of [ital x]. The results of Hall-coefficient measurements indicate that hole concentration is reduced as a function of [ital x], which is consistent with the results of oxygen-concentration measurements. We find no energy shifts of the core levels, valence bands, and conduction bands for the samples, which is completely different from the results for the Bi[sub 2]Sr[sub 2]Ca[sub 1[minus][ital x]]Y[sub [ital x]]Cu[sub 2]O[sub [ital y]] system. The results of IPES and resistivity measurements show that the metal-semiconductor transition occurs at about [ital x]=0.35. The states created by hole doping move to the Fermi level. The change in the electronic states is discussed.

In order to refine our adjuvant chemotherapy against locally advanced breast cancer, an anticancer drug sensitivity test, using the MTT assay was conducted with biopsy specimens of human breast cancer obtained prior to therapy. Dose-dependent sensitivity was observed with all tested agents: cisplatinum, mitomycin C, methotrexate and doxorubicin. The sensitivity of the premenopausal specimens to the effects of mitomycin C and doxorubicin varied widely. When selecting chemotherapeutic agents to combat breast cancer, this variation in drug sensitivity based on menopausal status and patient age should be considered.

The properties of Bi{sub 2}Sr{sub 2}CaCu{sub 2}O{sub {ital y}} annealed at 200--650 {degree}C in vacuum (10{sup {minus}4}Pa) are studied by x-ray powder diffraction, ac magnetization, and Hall measurement. The amount of oxygen loss, {Delta}{ital y}, was obtained by an integration of the oxygen partial pressure measured by a quadrupole mass spectrometer. The {ital T}{sub {ital c}} increases from 65 to 92 K, which is accompanied by a small amount of oxygen loss (on the order of 10{sup {minus}3}). The {ital a} and {ital c} axes are extended by the oxygen loss. The change of the hole concentration estimated by Hall measurement is of the same order of magnitude as that for the other superconducting oxides against the change of {ital T}{sub {ital c}}, however, it is larger by about two orders of magnitude than that expected from the oxygen loss. This discrepancy suggests charge redistribution in the structure.

17. New concept phase shifting masks based on projection lens design

For the purpose of shortening turn around time (TAT) and costs of phase-shifting mask (PSM) fabrication, we proposed a new concept of PSM composed of an opaque pattern substrate and a phase shifter substrate. It was considered that this PSM has the disadvantage of high wave-front aberration. In order to solve this problem, we have designed a high NA (0.75), very low aberration (0.0017 /spl lambda/) reduction projection lens system. Based on this design, we found that wave-front aberration of phase shifter substrate (0.09 inches thickness) between the mask and projection lens was reduced from 0.3314 /spl lambda/ to 0.0089 /spl lambda/ by projection lens system optimization of mask and imaging positions. We measured the intensity profiles of this prototype PSM using AMIS (Zeiss optical simulation microscope) and checked the intensity profiles of this PSM were better than that of the conventional PSM.

18. A new exposure method using cover glass

We present a new exposure method to achieve super resolution of a projection lens. The idea is based on a cover glass inserted between an optical lens and image plane. We have designed a projection lens system using a cover glass. It has found that the resolution of the projection pattern can be improved by use of a cover glass under the condition that the practical size of the projection lens is restricted. We have also set up a proto-type exposure evaluation system of reduction projection lens. It has been found that projection image of mask pattern was improved using a cover glass inserted between a projection lens and its image plane. We have also found that projection image of mask pattern was much improved by use of both phase shifting mask and a cover glass inserted between a projection lens and its image plane.

19. A new simultaneous method of Hall and magnetoresistance measurements at low and high magnetic field on liquid and amorphous metals, and semiconductors

The Hall signals of Ga and Hg metals at liquid and solid states obtained through a new simultaneous method have been examined and compared with those of amorphous metals and solid state semiconductors of Si and InSb single crystals. Liquid metals of Hg and Ga of 5 N pure, para and ferromagnetic films of amorphous iron, and single crystals of Si and InSb were used as samples. Liquid metals were enclosed in the cell made of Macor or Pyrex glass, in which ditch is 200 and 400 /spl mu/m deep. The temperature was monitored and controlled within 0.5/spl deg/C by a thermocouple buried at the bottom of the cell. DC magnetic field of low field less than 0.3 Tesla is applied with a lab electromagnet, a superconductor magnet for high magnetic field up to 9 Tesla. Liquid metals such as Hg and Ga show the Hall effect and negligible magnetoresistance effect in the solid state, but the magnetoresistance effect in the liquid state appears much larger than in the solid state and badly affects the Hall effect.. It is also found that there is the magnetoresistance effect even in Si single crystal although the effect cannot normally be observed in low magnetic field less than 1 Tesla. For the ferromagnetic materials, the Hall signal is very much affected by the magnetoresistance.

A new generation type of vinca alkaloid, vinorelbine (VNR), is a promising agent for the treatment of breast cancer patients. As a first line treatment for metastatic breast cancer, combination chemotherapy including anthracyclines plus VNR has demonstrated a high response rate and tolerability. In addition, other VNR containing regimens, such as 5-fluorouracil plus VNR, produced a 64% response rate. This evidence suggests VNR-containing combination chemotherapy may be a promising first-line treatment for breast cancer patients. In Japan, VNR is not registered, but a late phase II study has been completed. An approximately 30% response rate was obtained with its use as a monotherapy. In addition, a phase I/II study of combination VNR plus AC (adriamycin + cyclophosphamide) has been conducted. At the present time, the results are being analyzed.

A trial was designed to examine the combination of UFT and mitomycin (Mutamycin) plus tamoxifen (Nolvadex) as postoperative adjuvant therapy in the treatment of patients with stage II, estrogen receptor (ER)-positive primary breast cancer. Mitomycin was administered intravenously at 13 mg/m2 on the day of surgery. Patients judged to be ER-positive were randomly allocated to either group A, which received oral tamoxifen 20 mg/day 14 days after surgery for 2 years, or group B, receiving oral UFT 400 mg/day plus tamoxifen 20 mg/day. A total of 219 patients were enrolled in group A, of which 213 (97.3%) were determined to be eligible; 225 patients enrolled in group B and 223 (99.1%) were eligible. The 5-year survival rates were 93.0% for group A and 95.4% for group B, with no significant difference between groups. The 5-year relapse-free survival rates were 83.1% for group A and 90.7% for group B, a significant advantage (P = .020) for the UFT plus tamoxifen group. Combination therapy with mitomycin, tamoxifen, and UFT proved to be an effective postoperative chemoendocrine therapy for stage II, ER-positive breast cancer.

Liver metastasis from breast cancer has a poor prognosis. While there are some reports of good response rates of hepatic metastasis from breast cancer by hepatic intra-arterial infusion chemotherapy, no phase I study including pharmacokinetic analysis has been reported. We performed a phase I/II study of intra-arterial infusion chemotherapy using adriamycin and 5-fluorouracil to find the maximum tolerated dose and response rate in patients with advanced or recurrent breast cancer.A hepatic arterial catheter with an access port was inserted into the proper hepatic artery. Patients received 30 mg/m2 adriamycin on days 1 and 8 and 100 mg/m2 5-fluorouracil at level 1, 200 mg/m2 at level 2,300 mg/m2 at level 3 and 400 mg/m2 at level 4 continuously from day 1 through day 14 every 28 days. At least two cycles were required before evaluation. Twenty-eight patients were entered into this study and 26 patients were evaluable. Seventeen patients had hepatic metastasis only, although nine patients had additional metastasis to other sites.Dose-limiting toxicity of thrombocytopenia and neurotoxicity occurred at level 4. Leukocytopenia (ECOG grade 3-4) was observed in five (19%), thrombocytopenia in three (12%) and anemia in two (8%) patients. There were 11 catheter-related complications which were not dose dependent. Seven out of 13 evaluable patients (54%) responded at level 3. The median duration of response was 5.8 months (range, 1-23+) and median survival was 25.3 months (range, 6.2-54.7+).Hepatic arterial infusion therapy appears to be safe and effective but catheter-related complications must be overcome before starting a phase III trial.

The mammalian mandible develops around Meckel's cartilage and other secondary cartilages, including the dentary. There have already been many studies of the development of the rat mandible that have employed histological serial sections. However, no previous investigators have captured the three-dimensional features of the developmental process.In this study, the technique of double staining with alizarin red S and alcian blue was employed directly on whole body specimens to investigate the three-dimensional development of the rat mandible.We found that the molar socket obstructs the developing mandible, causing it to emerge medial to Meckel's cartilage. Our results also indicate that the secondary mandibular cartilages may contribute to supplementary growth in response to local factors.

A patient with pulmonary pseudolymphoma whose chest X-ray shadows could be observed for over five years is reported. A 73-year-old man was admitted to our hospital in March 1993, because of abnormal shadows on a chest X-ray film. There was a solitary mass in the left upper lung field and infiltrate in the right middle and lower lung fields. These shadows had been observed on a chest X-ray film in 1988, and had been gradually growing for more than five years. Transbronchial lung biopsy (TBLB) of the left upper lobe mass resulted in a histological diagnosis of pulmonary pseudolymphoma. The shadows showed no change during the next nine months after his discharge. These findings are suggestive of the natural history of pulmonary pseudolymphoma. It seems that the process involved in this case was benign rather than malignant.

27. [Treatment results of small cell lung cancer: referring to the clinical value of BAI combined with radiotherapy]

Results of 65 cases of small cell lung cancer (SCLC) treated from Jan. 1981 to May 1991 were reviewed. There were 58 male and 7 female patients and their age was ranging from 37 to 86 (mean 65). There were 41 limited disease (LD) and 24 extensive disease (ED) cases. According to TNM (UICC 1987) staging system, there were 2 cases of stage I, 4 of stage II, 9 of stage III A, 28 of stage III B and 22 of stage IV. Among 65 cases, 60 cases received radiotherapy and 55 cases of them received radiotherapy for primary site. There were 29 cases received radiotherapy combined with BAI (bronchial artery infusion) and 20 cases received systemic chemotherapy. On survival, the 2-year survival rate was 26% and MST was 13 months in LD patients (n = 41). No 2-year survivors were seen in ED patients and MST was 10 months. Tumor response of primary site was as follows. In systemic chemotherapy group, CR 35%, PR 59%, NC 6% and PD 0% before radiotherapy and CR 59% after radiotherapy were obtained. In BAI group (including BAI + systemic chemotherapy), CR 6% and PR 88% before radiotherapy in BAI group. BAI did not seem to improve response rate compared to systemic chemotherapy. On survival, BAI group did not show significant better survival compared to BAI (-) group in LD cases (n = 31). In responders (evaluable LD cases, n = 24), the MSTs were 25 months in CR cases and 13 months in PR cases. No 2-year survivors were seen in PR cases.(ABSTRACT TRUNCATED AT 250 WORDS)

Comparative clinical trials among 3 regimens for patients with advanced or recurrent breast cancer were performed as a multi-institutional joint study. Arm-I of 3 regimens consisted of a 3-day consecutive administration of adriamycin (ADM) at 10 mg/body every 4 weeks, a daily oral administration of cyclophosphamide (CPA), 5-FU and tamoxifen (TAM) at 100 mg, 200 mg, and 20 mg, respectively, and a once-a-week intramuscular or subcutaneous injection of OK-432 (OK) gradually increased from the initial dose of 1 KE to the maintenance dose of 5 KE. Arm-II contained methotrexate (MTX) at 10 mg/body for 3 consecutive days every 4 weeks in place of ADM in Arm-I. Arm-III contained neither ADM nor MTX. Of 69 cases registered, 52 were eligible, leaving 6 non-eligible and 11 incomplete cases. The results of the overall evaluation of 48 complete cases other than 4, in which the tumor was not exactly measured, were as follows. Arm-I resulted in 2 CR and 3 PR out of 19 cases, the response rate being 23.6% (5/19). Arm-II resulted in 1 CR out of 15, the response rate being 6.7% (1/15). In Arm-III, no response cases were found. No significant difference was observed among three treatment groups (p = 0.055), but in Arm-I, the response rate was higher than in the other treatment groups, suggesting that there is a probability of useful combined use of ADM.

We developed a modified transcatheter arterial infusion method using anticancer agents to treat hepatic malignancies; intermittent injections of iodized oil, lipiodol, containing adriamycin or epirubicin during the arterial infusion of cisplatin (75-200 mg/body) in order to achieve a higher concentration and longer retention of these anticancer agents in the tumor tissue. Fourteen patients with hepatocellular carcinoma (HCC) and five patients with metastatic liver cancer were treated with this "pile-up" arterial infusion therapy by anticancer agents without gelatin sponge TAE. In HCC patients, 50% or greater reduction in tumor size was obtained in 7 of 14 patients (50%). Serum AFP levels decreased by more than 75% in 6 of 7 patients in whom pretreatment serum levels of AFP were more than 200 ng/ml. The one-year and two-year survival rates were estimated at 55% and 27.5%, respectively, by the Kaplan-Meier method. Significant reduction in tumor size was not observed in 5 cases with metastatic liver cancer. Concerning the adverse effects, alimentary symptoms and fever were noted for a few days in many cases, but they were temporary and tolerable in almost all of the patients. Severe adverse changes in laboratory data were not observed. Thus this "pile-up" infusion therapy of anticancer agents without TAE may be a useful therapy for HCC.

Adrenal imaging was performed using magnetic resonance (MR) was in 100 patients who had no clinical or biochemical evidence of adrenal abnormality and in 19 patients with 24 adrenal lesions (adenoma in 5, hyperplasia in 2, metastasis in 5, (lung cancer in 1, hepatoma in 4) adrenal cancer in 1, pheochromocytoma in 3, neuroblastoma in 3). Normal adrenal glands showed intermediate intensity between muscle and liver, and were detected in over 90% of cases on T1-weighted images (T1-weighted SE, inversion recovery). Adenomas and hyperplasias had the same intensity as normal glands. Medullary masses showed extreme hyperintensity on T2-weighted images and could be differentiated from cortical masses. Neuroblastomas were detected as hyperintense tumors with intratumoral hemorrhage and necrosis on T2-weighted MR images. Metastatic adrenal tumors from lung cancer were hyperintense on T2-weighted images, while metastasis from hepatoma showed low intensity on the same pulse sequence. In diagnosing adrenal metastasis, we must compare and contrast the tumor intensity and structure with those of the primary lesions. MR is considered a useful modality in characterizing adrenal tissue.

The total dosage and therapeutic value of Cyclophosphamide (CPA) administered as adjuvant chemotherapy for breast cancer were studied. The subjects were those under 65 years of age who were diagnosed as Stage II breast cancer and underwent curative operation at any of 10 national hospitals from May, 1978 to April, 1981. CPA at dose of 100mg was administered orally every day. Comparison was made between the long-term administration group (L) of patients receiving a total of 25 g/m2 over 18 months and the short-term administration group (S) receiving a total of 6 g/m2 over 6 months, in terms of survival rate (SV) and disease-free survival rate (DFV). Cases selected as the subjects were randomly allotted to the two groups by means of the envelope method. The number of cases registered was 316. Of them, 308 cases were eligible for the study. Thirty-eight cases (23.6%) in L and 15 cases (10.2%) in S dropped out due to side effects and other reasons. No difference was observed between the two groups in background factors such as age, menopausal status, type of operation, histological diameter of tumor, histological type and metastasis to lymph nodes. Results of a 10-year follow-up revealed no difference in SV and DFV between the two groups. Excluding the drop-out cases, however, DFV was significantly high in L. There was no difference in DFV between the two group in terms of menopausal status. DFV was significantly high in L, if one includes cases with metastasis to lymph nodes. DFV was high in L, even when a group with no more than 3 metastatic lymph nodes and the group with not less than 4 were included. It is of little value to administer CPA to all Stage II cases for a long period of time. CPA may be indicated for cases showing metastasis to lymph nodes.

34. Comparisons of interval breast cancers with other breast cancers detected through mass screening and in outpatient clinics in Japan

In a nation-wide collaborative study on mass screening for breast cancer, we collected 152 cases of interval breast cancer diagnosed at 35 hospitals or clinics distributed throughout Japan. The definition of interval breast cancer used in the present study is "breast cancer cases which were diagnosed as having 'no malignant findings' in a previous screening for breast cancer but subsequently diagnosed as 'breast cancer' at a hospital or medical clinic within two years of the previous screening." The clinical stages and prognoses of these interval cancer were analyzed and compared with those of other breast cancers detected through mass screening and in outpatient clinics. In the clinical staging of interval breast cancer, Tis (non infiltrating cancer) accounted for only 2.1%, compared to 8.0% in cases detected through mass screening. At stage I 43.4% were interval breast cancers compared to 32.9% breast cancers detected through mass screening and 25.4% diagnosed in outpatient clinics. The stage differences between interval breast cancers and breast cancers detected through mass screening were not statistically significant. Five-year survival rates were 85.6% for interval breast cancers, 91.7% for breast cancers detected through mass screening and 84.7% for breast cancers diagnosed in outpatient clinics. Ten-year survival rates were 75.9, 80.5 and 78.1%, respectively, suggesting the interval breast cancer cases to show a similar prognosis to that of breast cancer cases diagnosed in outpatient clinics. The differences in five- and 10-year survival rates among the three groups were not statistically significant. From the present study we were not able to confirm the general belief of interval cancer being more aggressive in nature and showing a poorer prognosis than cancer detected through periodic screening. The reasons for this are discussed.

The property of pluronic F127 (PLF) as a base for an anticancer agent, mitomycin C (MMC) was evaluated. A weighed amount of PLF and MMC were slowly added to cold water and mixed completely. The solution was warmed to 37 degrees C and MMC containing PLF gel was formed. The cumulative amount of MMC which was released from the gel during a 3-hour period was calculated. 5.5 percent of MMC was released from PLF-MMC gel which contained 20 or 25% PLF and 3.8% MMC from PLF-MMC gel containing 30% PLF. Intraperitoneal (i.p.) administration of large amount of PLF gel alone in rabbits or mouse revealed severe hepatorenal toxicity, and acute i.p. administered LD50 of PLF was supposed to be between 1.7 g and 5.0 g/kg body weight in mouse. Further evaluation of gradual releasing property of PLF is needed.

36. [Urinary and sexual dysfunction after rectal cancer operation]

Using a questionnaire, the urinary function of 68 patients and the sexual function of 81 patients were evaluated after rectal cancer operation. The patients with carcinoma of the rectum suffered from severe damage to the urinary system following Miles' operation (Miles) which included 85.7% of the male and 42.9% female. On the other hand, 31.8% male and 33.3% female suffered following anterior resection or pull through operation (AR). Following Miles 64.3% male and 71.4% female had developed urinary incontinence while 36.4% male and 33.3% female developed the same following AR. These results indicate that dysfunction of the urinary voiding system was more common in the males while the females experienced urinary incontinence. This difference may be due to the severity of the surgical intervention to the bladder neck and partly to the anatomical difference of pelvic floor and of urethra. The recovery was poor in the patients who had developed disorder of the urinary voiding system and unfortunately 84% patients following Miles and 75% patients after AR could not return to normal voiding even five years after operation. Urinary incontinence persisted after Miles and after AR in 77.8% and 31% patient respectively. Sexual activity was remarkably reduced following Miles. The males lost the power of erection and ejaculation, the females suffered due to the existence of artificial colostomy. In order to maintain the normal physiological activity of the urogenital system following operation, it is important to avoid any damage preferably to the bladder neck and urethra in case of males and to avoid artificial stoma in females.

To establish the effective mass screening projects for the breast cancer, our study group cosponsered by the Welfare Ministry was organized by 11 regional groups. From 1968 to 1986, 643, 513 women at the initial screening and 719, 189 women at the subsequent screening were examined by physical examination. Out of them, abnormalities were present in 24, 864 women (3.9%) and 23,880 (3.3%), respectively. A detection of cancer was made in 833 women (detection rate 1.3%) and 428 women (0.6%), respectively. As the examinees increased from year to year, the subsequent examinees were over the initial ones. It was suggested that most of examinees had been fixed year by year and the detection rate of initial screening had been higher than that of the subsequent one. Then more initial examinees are expected to be increased. Women 30 to 59 years of age were mostly screened, but women aged 60 or more should be increased. Of women with abnormal physical findings, women over 90% took the second examination. But more effective, economical and safer screening instruments are expected to be developed and be used at the first examination.

Seventy-four breast cancer patients, who had undergone outpatient excisional biopsy followed by radical mastectomy, were divided into three groups according to the types of intramammary cancer residuals: no cancer residuals--45 patients, noninvasive cancer residuals--15, and invasive cancer residuals--14. The invasive cancer residual group included nine patients with axillary metastasis and showed a significantly higher rate of five-year recurrence (8/14) than the other two groups. In some of the patients with invasive cancer residuals, the lesions were more advanced and aggressive by nature. Histopathologically, invasive cancer residuals closely correlated to the nondesmoplastic infiltrating tumor border, and noninvasive cancer residuals to intraductal cancerous spread. Most intramammary cancer residuals were found close to the wounds made by biopsy.

Thirty-three anus-preserved rectal cancer patients with anterior resection or low anterior resection were asked about their postoperative defecation conditions with questionnaires after a more than one and half year postoperative period. Beforehand the residual anorectum length was defined by subtracting the length of the specimen AW and 1.5 cm as suturing width from the length of the anal verge through the anal tumor margin during romanoscopic examination. Involuntary soiling frequently occurred in the cases with a less than 9 cm residual anorectum, in the female cases and in the cases associated with lateral lymph node dissection. But these factors have no definite relation to the time of disappearance of involuntary soiling. Half of the cases were normalized in 2 years, and 80% in 3 years. Frequent bowel movement occurred in a high percentage of cases, especially in the cases with short residual anorectum or with lateral lymph node dissection. Frequency of defecation decreased from 8-9/day up to 6 months later, to 5/day up to one year later, and 2-3/day up to 6 years later. Improvement of the above-mentioned condition was slightly delayed in the less than 6 cm residual anorectum group and in females, but not in the lateral lymph node dissection group. Anastomosis insufficiency caused a delay in recovery from involuntary soiling.

A new method of plotting the impurity profile is described. It makes efficient use of the advantage of an operational amplifier and the nonlinearity of the junction capacitance in a semiconductor. When two small ac signals are applied to the inverting input terminal of the operational amplifier, intermodulation arises due to the nonlinearity of the junction capacitance inserted in the feedback loop, at the time the reverse bias voltage to the junction can be supplied from the noninverting input terminal; then the signals concerning both the depletion layer width x and the impurity concentration N (x) arise at the output terminal of the operational amplifier. By detecting each of the amplified input signals and the difference‐frequency ones corresponding to x and N (x), respectively, the relation of x−N (x) can be plotted easily, at the same time, in a short time, and at a low cost.

Hall measurements of liquid metals, using two-frequency, ac-dc and simultaneous methods are described. The Hall effect has been measured in Hg and Ga metals, in both solid and liquid states. The magnetoresistance and Hall effects have also been measured in an InSb single crystal, which exhibited magnetoresistance even in low magnetic field, and in Si , which did not exhibit magnetoresistance in low magnetic field. In order to investigate the magnetic field dependence of the observed galvanomagnetic effects for solid and liquid state metals, and for semiconductors, Hall measurements in high magnetic field, up to ±9 T, were also performed.

Purpose: Radiation injury is one of the most important problems after stereotactic radiosurgery (SRS) and radiotherapy (SRT). However differential diagnosis between radiation injury and tumor recurrence is difficult. We evaluate availability of 11C-Methionine positron emission tomography (Met-PET) in differential diagnosis after SRS and SRT in the patient with brain metastasis. Material and Method: Between January 2008 and May 2012, 42 patients who had expanding lesion of brain metastasis after stereotactic radiosurgery and radiotherapy were engaged on this study. Primary lesions of brain metastasis were lung cancer in 28 cases, breast cancer in 7cases, ovarian cancer in 3 cases and others in 4 cases. Gamma knife radiaosurgery and Cyberknife radiotherapy were performed in 30 cases and 12 cases, respectively. Met-PET images were obtained as a static scans of 10 minutes performed 20 minutes after injection of 11C-methionine. On Met-PET scans, the portion of the tumor with the highest accumulation was selected as the region of interest (ROI), tumor-versus-normal ratio (TN ratio) was defined as the ratio of the standardized uptake value (SUV) in the tumor (T), divided by SUV in normal gray matter (N). Met-PET scan accuracy was evaluated by histological assessment from removed tissue (12 cases) or, in cases without surgery or biopsy, by the subsequent MR findings (30 cases). Result: Met-PET images were obtained 459 days after SRS or SRT. Maximum TN ratio was 1.16 in the cases with radiation necrosis (25 cases) and 1.74 in the cases with tumor recurrence (14 cases). The cut-off value of SUV was about 1.5. Subsequent Met-PET was performed in the patients who had around 1.5 in the SUV and could not be evaluated differential diagnosis from initial Met-PET only. Maximum TN ratio elevated from 1.38 to 1.69 in the cases with recurrence. However, in the cases with radiation necrosis, it did not changed from 1.16 to 1.26. Conclusion: Met-PET proved useful for differential diagnosis for radiation injury and tumor recurrence. - Abstract
Background: Behavioural and psychological symptoms of dementia (BPSD) negatively impact the quality of life of patients with dementia and their surrounding caregivers. Therefore, identifying modifiable factors to prevent BPSD is crucial. This cross-sectional study investigated the association between hearing impairment (HI), social relationships, and BPSD., Methods: A survey involving 179 patients who attended a memory clinic was conducted between July 2023 and March 2024. Pure-tone audiometry and questionnaire interviews were conducted, while medical records were reviewed. HI was defined as an average hearing level ≥ 40 dB, measured by pure-tone audiometry using the better-hearing ear. BPSD presence and severity were assessed using the BPSD25Q-based questionnaire. Multiple regression analysis was used for calculating partial regression coefficients to assess the association between HI, social relationship indicators, and BPSD presence and severity, adjusting for confounders., Results: Among the 144 participants included in the analysis, the mean age was 82.7 years, and 96 (66.7%) were female individuals. Using multiple regression analysis, individuals with HI, whether living with others (β = 1.49, P = 0.038) or living alone (β = 2.23, P = 0.044) had a higher presence of BPSD, compared with those without HI who live with others. Additionally, those with HI who regularly engaged in conversations (β = 1.51, P = 0.027) or did not participate in social interactions (β = 2.02, P = 0.020) also had a higher presence of BPSD, compared with individuals without HI in similar circumstances. Similarly, those with HI and living alone (β = 4.54, P = 0.033) and lacking social interaction (β = 3.89, P = 0.020) experienced increased BPSD severity., Conclusion: Our findings indicated that living alone and lacking social interaction with having HI were linked to both the presence and severity of BPSD. Additionally, increased verbal communication through conversations with cohabiting family members and others in patients with HI was associated with a higher presence of BPSD. - Abstract
Background and Aims: There was no previous trial comparing aspirin monotherapy with a P2Y12 inhibitor monotherapy following short dual antiplatelet therapy after percutaneous coronary intervention with drug-eluting stents., Methods: In the STOPDAPT-3, patients with acute coronary syndrome or high bleeding risk (HBR) were randomly assigned to either 1-month dual antiplatelet therapy with aspirin and prasugrel followed by aspirin monotherapy (aspirin group) or 1-month prasugrel monotherapy followed by clopidogrel monotherapy (clopidogrel group). This secondary analysis compared aspirin monotherapy with clopidogrel monotherapy by the 30-day landmark analysis. The co-primary endpoints were the cardiovascular endpoint defined as a composite of cardiovascular death, myocardial infarction, definite stent thrombosis, or ischaemic stroke and the bleeding endpoint defined as Bleeding Academic Research Consortium 3 or 5., Results: Of the 6002 assigned patients, 5833 patients (aspirin group: N = 2920 and clopidogrel group: N = 2913) were included in the 30-day landmark analysis. Median age was 73 (interquartile range 64-80) years, women 23.4%, acute coronary syndrome 74.6%, and high bleeding risk 54.1%. The assigned monotherapy was continued at 1 year in 87.5% and 87.2% in the aspirin and clopidogrel groups, respectively. The incidence rates beyond 30 days and up to 1 year were similar between the aspirin and clopidogrel groups for both cardiovascular endpoint [4.5 and 4.5 per 100 person-year, hazard ratio 1.00 (95% confidence interval .77-1.30), P = .97], and bleeding endpoint [2.0 and 1.9, hazard ratio 1.02 (95% confidence interval .69-1.52), P = .92]., Conclusions: Aspirin monotherapy compared with clopidogrel monotherapy was associated with similar cardiovascular and bleeding outcomes beyond 1 month and up to 1 year after percutaneous coronary intervention with drug-eluting stents (STOPDAPT-3 ClinicalTrials.gov number, NCT04609111)., (© The Author(s) 2024. - Abstract
Aim: This cohort study aimed to assess weight loss associated with new long-term care insurance (LTCI) certifications over a 9-year period, accounting for the competing risk of death., Methods: We analyzed data from 3749 Japanese individuals aged ≥65 years in Kami Town, Hyogo Prefecture, Japan. Weight loss was assessed using the Kihon Checklist during the baseline survey. Data regarding LTCI certifications were collected until March 2022. Cox proportional hazards models were used to calculate adjusted hazard ratios (HRs) of 9-year LTCI certification because of weight loss, adjusted for confounding factors. To exclude the effect of competing risks, Fine-Gray regression was used to estimate subdistribution HRs. Subgroup analyses were carried out after the examination of potential interactions between subjective cognitive function, body mass index categories and weight loss., Results: The incidence rate of new LTCI certifications was 5.16 per 100 person-years overall - broken down into 7.02 for those with weight loss and 4.97 for those without. The adjusted HR for weight loss to new LTCI certifications was 1.35 (95% CI 1.15-1.59). Considering mortality as a competing risk, the adjusted subdistribution HR was 1.37 (95% CI 1.16-1.61). Conversely, no interaction was observed between weight loss and subjective cognitive function or body mass index categories., Conclusions: Excluding the effect of mortality, weight loss was identified as a risk factor for new LTCI certifications. - Abstract
Background: Primary percutaneous coronary intervention (PCI) for ST-segment-elevation myocardial infarction (STEMI) complicated by cardiogenic shock (CS) may reduce the risk of subsequent cardiovascular events but remains challenging. The study aim was to evaluate the clinical characteristics and long-term outcomes of patients undergoing primary PCI for STEMI with CS., Methods: We conducted an observational cohort study of patients with STEMI who underwent primary PCI between April 2004 and December 2017 at Juntendo University Shizuoka Hospital. The primary outcome was cardiovascular death (CVD) during the median 3-year follow-up. We performed a landmark analysis for the incidence of CVD from 0 day to 1 year and from 1 to 10 years., Results: Among the 1758 STEMI patients in the cohort, 212 (12.1 %) patients with CS showed significantly higher 30-day CVD rate on admission than those without (26.4 % vs 2.9 %). Landmark Kaplan-Meier analysis showed that CVD from day 0 to year 1 was significantly higher in the patients with CS (log-rank p < 0.0001). Multivariate Cox regression analysis showed that CS was significantly associated with higher cardiovascular mortality (adjusted hazard ratio, 11.8; 95%confidence intervals, 7.78-18.1; p < 0.0001), but the mortality rates from 1 to 10 years were comparable (log-rank p = 0.68)., Conclusion: The cardiovascular 1-year mortality rate for patients with STEMI was higher for those with CS on admission than without, but the mortality rates of >1 year were comparable. - Abstract
The current guidelines for acute coronary syndrome (ACS) discourage the use of anticoagulation after percutaneous coronary intervention (PCI) without specific indications, although the recommendation is not well supported by evidence. In this post hoc analysis of the ShorT and OPtimal Duration of Dual AntiPlatelet Therapy-3 (STOPDAPT-3) trial, 30-day outcomes were compared between the 2 groups with and without post-PCI heparin administration among patients with ACS who did not receive mechanical support devices. The co-primary end points were the bleeding end point, defined as the Bleeding Academic Research Consortium type 3 or 5 bleeding, and the cardiovascular end point, defined as a composite of cardiovascular death, myocardial infarction, definite stent thrombosis, or ischemic stroke. Among 4,088 patients with ACS, 2,339 patients (57.2%) received post-PCI heparin. The proportion of patients receiving post-PCI heparin was higher among those with ST-elevation myocardial infarction compared with others (72.3% and 38.8%, p <0.001), and among patients with intraprocedural adverse angiographic findings compared with those without (67.6% and 47.5%, p <0.001). Post-PCI heparin compared with no post-PCI heparin was associated with a significantly increased risk of the bleeding end point (4.75% and 2.52%, adjusted hazard ratio 1.69, 95% confidence interval 1.15 to 2.46, p = 0.007) and a numerically increased risk of the cardiovascular end point (3.16% and 1.72%, adjusted hazard ratio 1.56, 95% confidence interval 0.98 to 2.46, p = 0.06). Higher hourly dose or total doses of heparin were also associated with higher incidence of both bleeding and cardiovascular events within 30 days. In conclusion, post-PCI anticoagulation with unfractionated heparin was frequently implemented in patients with ACS. Post-PCI heparin use was associated with harm in terms of increased bleeding without the benefit of reducing cardiovascular events.
