1. Renal Trauma in the West of Ireland — A Regional Review

Abstract

There is a paucity of data regarding renal trauma. The majority of cases of renal trauma are amenable to conservative management. We sought to streamline the management of renal trauma in the west of Ireland. Patients presenting with a computerised tomogram–confirmed renal injury were assessed over 5 years. Patient demographics, injury details, initial emergency department management, definitive management, and follow-up were assessed. Renal trauma was graded in a blind fashion (I-V). Twenty-five patients were identified; male:female (23:2). The mean age was 26 years. The majority of renal traumas were managed conservatively (92%); 8% patients underwent nephrectomy. The common mechanisms of renal injuries were road traffic accidents (44%). The majority of cases of renal injuries occur as a result of blunt trauma and can be conservatively treated. Two nephrectomies (8%) were performed. We believe this study potentially can be beneficial as part of an all-Ireland trauma database to improve patient outcome.

2. PRELIMINARY OBSERVATIONS ON EFFECTS OF FAT CONTENT AND DEGREE OF FAT EMULSIFICATION ON THE STRUCTURE-FUNCTIONAL RELATIONSHIP OF CHEDDAR-TYPE CHEESE

Abstract

Cheddar type cheeses of different fat contents were produced and denoted: full-fat (FFC), 306 g/kg; half-fat (HFC), 174 g/kg; and low fat (LFC, 13 g/kg). Full-fat Cheddar cheese (FFCH) was also prepared from milk which had been homogenized at first and second stage pressures of 25 and 5 MPa, respectively. The cheeses were held at 4C for 30 days and at 7C for the remainder of the 190-day ripening period. Reducing the fat level from 174 to 13 g/kg resulted in decreases in contents of moisture in nonfat substance and pH 4.6 soluble N as a percentage of total N (pH4.6SN), and increases in the contents of moisture, protein and intact casein. Homogenization of cheesemilk resulted in a slight increase in moisture content and an increase in pH4.6SN. Confocal laser scanning microscopy revealed that the extent of fat globule clumping and coalescence in both the unheated and heated (to 95C) cheeses decreased with homogenization of the cheesemilk and with fat reduction. Homogenization of the cheesemilk and reducing the fat content of the cheese resulted in a decrease in the flowability and stretchability of the melted cheese. Dynamic measurement of the viscoelastic changes on heating the cheese from 20 to 90C showed that reduction of fat content resulted in a decrease in the magnitude of the phase angle, δ, at temperatures >50C. At temperatures 45-50C, with δ max typically decreasing from 65-70° in the FFC to 35° in the FFCH.

Abstract

The compositional and functional properties of commercial retail and/or wholesale samples (n = 8) of low-moisture mozzarella, cheddar and analogue (pizza) cheeses were compared. Inter-and intravariety differences were evident with intravariety differences in composition being relatively large for the analogue cheese. Cheddar had the lowest mean pH and level of expressible serum and the highest mean levels of proteolysis, expressible fat, and serum calcium and nitrogen (p < 0.05). Compared to mozzarella, the analogue cheeses had significantly lower (p < 0.05) mean levels of total protein and serum calcium, higher levels of total calcium and higher cheese pH. The mean stretchability of the melted mozzarella cheese was significantly higher than that of the melted cheddar or analogue cheeses. The melted cheddar had the highest mean flowability and lowest mean apparent viscosity (p < 0.05). The mean flowability and apparent viscosity of the analogue cheese were numerically lower and higher, respectively, than those of mozzarella.

Milk was collected from three spring-calving herds, on different daily herbage allowances (DHA) of perennial rye-grass (16, 20 or 24 kg dry matter (DM)/cow for a 17 week period. Abstract

One hundred consecutive parotid sialograms were reviewed to assess the clinical usefulness of the technique. In 54 per cent of patients significant information was provided by the sialogram, and in 22 per cent the diagnosis was made on sialographic appearances alone or the findings altered subsequent management. The highest proportion of useful investigations was found in those presenting with bilateral parotid swelling. No significant information was obtained in patients with an isolated lump in the parotid region.

Abstract

We have reviewed the surgical treatment of 12 consecutive patients with advanced carcinoma of the parotid gland at this hospital. Ten patients received immediate preoperative radiotherapy to a mean total dose (TD) of 4000 cGy (rad), range 2600–6500 cGy. Two patients with recurrent carcinoma had previously been treated with local radiotherapy (TD 6500 cGy) at 1 year and 2 years respectively prior to referral. A radical parotidectomy with block dissection of the neck was performed on all 12 patients. After a follow-up period ranging from 6 months to 8 years, 6 patients have remained disease free including 3 patients who have died from unrelated causes within this period. Three patients have developed local recurrent disease (interval 9 months–3 years), 3 further patients with residual microscopic disease have been observed postoperatively (follow-up interval 6 months–2 years) without developing overt local recurrence.

Abstract

Fifty-three consecutive, unselected patients with invasive bladder cancer, Stage T2 to T3, treated by radical radiotherapy have been reviewed. Cystectomy was reserved for patients with significant worsening of disease during treatment, histologically confirmed persistent or recurrent invasive tumour after treatment, or patients with intolerable symptoms due to radiation cystitis. In 64% of our patients a favourable tumour response to radiotherapy was seen, while a further 31% showed disease progression either during or on completion of radiotherapy. Cystectomy was performed on 22% of patients, mainly for radiation cystitis, and was not associated with a significant operative mortality rate. The crude 5-year survival rate was 42%. We conclude that radical radiotherapy is as effective as other forms of treatment for invasive bladder cancer, but that there remains a need to identify those bladder tumours destined to respond poorly to radiotherapy at an earlier stage.

11. Surgical excision of first cleft branchial fistulae

Abstract

Three patients were studied who had fistulae in the neck derived from the first branchial cleft. Evidence is presented to show that although these fistulae usually pass superficial to the facial nerve they may also pass deep to one or both main divisions of the nerve. We conclude that a formal superficial conservative parotidectomy with full exposure of the facial nerve is the safest operative course when excising these fistulae.

Abstract

Eighty patients with prostatic obstruction were entered into a double-blind parallel study of prazosin versus placebo. There were 25 withdrawals or exclusions, leaving 55 patients for analysis. Mean maximum flow rates increased significantly more in patients treated with prazosin than in those treated with placebo (P less than 0.005), but there was no significant reduction in maximum voiding pressure. The mean number of voids, recorded on diary cards, was reduced from an initial 10.0/24 h by 2.1 in the final week, a significantly greater reduction than in the placebo group (P less than 0.01). However, there were no statistically significant changes in the filling cystometrograms. When patients were classified as responders or non-responders in terms of bladder filling, urine flow, bladder emptying, weekly average of voids/24 h and nocturia, the proportion of patients responding to prazosin was significantly greater in all categories except bladder filling and emptying. It was concluded that prazosin at a dose of 2 mg bd is a safe and effective treatment for prostatic obstruction and may be used in patients awaiting surgery and those who are unfit for operation.
