Abstract Background Noncommunicable diseases (NCDs) are public health threats globally and recognized impediments to socioeconomic development. This study aimed to identify the prevalence and clustering of NCDs risk factors among Bangladeshi men and women aged 20–59 years using nationally representative data. Methods This study was conducted in 82 rural, nonslum urban, and slum clusters across all eight administrative divisions of Bangladesh using multistage cluster sampling. A total of 4917 men and 4905 women aged 20–59 years were included in the study. Descriptive analyses were performed to report the prevalence and distribution of behavioral and clinical risk factors. Multivariable binary logistic regression was performed to identify factors associated with the coexistence of three or more NCD risk factors. Results The prevalence of tobacco use (any form), insufficient physical activity, inadequate fruit and vegetable consumption, overweight and obesity, and central obesity were 38.3%, 13.6%, 87.1%, 42.3%, and 36.0%, respectively. Furthermore, 21.9% and 4.9% participants had hypertension and self‐reported diabetes, respectively. Regarding the clustering of risk factors, 37.1% men and 50.8% women had at least three NCD risk factors. Only 3.0% men and 1.8% women reported no NCD risk factors. Age, place of residence, education, and wealth status were associated with the presence of at least three risk factors for both sexes. Conclusion Since a large proportion of Bangladeshi 20–59 years old population had multiple risk factors, population‐based programs with multisectoral approaches are essential to reduce NCDs among Bangladeshi women and men.

A numerical study of the unsteady mixed convection heat transfer characteristics of an Ag–water nanofluid confined within a square shape lid-driven cavity has been carried out. The Galerkin weighted residual of the finite element method has been employed to investigate the effects of the periodicity of sinusoidal boundary condition for a wide range of Grashof numbers (Gr) (105 to 107) with the parametric variation of sinusoidal even and odd frequency, N, from 1 to 6 at different instants (for τ = 0.1 and 1). It has been observed that both the Grashof number and the sinusoidal even and odd frequency have a significant influence on the streamlines and isotherms inside the cavity. The heat transfer rate enhanced by 90% from the heated surface as the Grashof number (Gr) increased from 105 to 107 at sinusoidal frequency N = 1 and τ = 1.

The development of a country is strongly influenced by a higher smoking rate among the young population. Understanding knowledge and attitudes regarding smoking among young people lies at the heart of effective tobacco control policies. Bangladesh is a large, low-income country in South Asia with a high rate of youth smoking. As yet, however, little attention has been paid to knowledge and attitudes among rural youth. Face-to-face interviews were conducted with 385 youth males from the rural Mymensingh district of Bangladesh aged 15 to 24 were interviewed face-to-face. Univariate distribution (i.e., frequency analysis), bivariate analysis (i.e., Chi-square tests), and multivariate analysis (i.e., binary logistic regression model) were performed to determine the influence of knowledge and attitudes on youth male smoking. The percentage of youth smoking is 40.3 with knowledge and attitudes revealed as the more influential factors of smoking. For example, the chance of being a smoker with a satisfactory knowledge score is significantly lower than their counterparts (Odds ratio [OR]: 0.47 [0.30, 0.70]). Similarly, respondents with satisfactory attitudes scores have a significantly lower chance (OR: 0.40 [0.26, 0.61]) to be a smoker than those having an unsatisfactory score. This study exposes the influences of knowledge and attitudes among the youth toward smoking in rural parts of the Mymensingh district of Bangladesh. The findings and recommendations of this study will be more helpful to the authorities in revising the existing smoking policies accordingly to reduce and prevent youth smoking for the betterment of the country.

Abstract Background Several strategies and policies are being implemented in Bangladesh to address the healthcare needs of people with disabilities, who comprise about 10% of the country’s total population. However, these measures are not adequate to provide accessible or friendly healthcare to the people with disabilities. This study aimed to explore the disability-friendliness of healthcare facilities, and the challenges of people with disabilities in terms of access to 1) information and communication, 2) access to infrastructure, and 3) providers’ capacity in Bangladesh. Methods An explanatory sequential mixed-method study was conducted, including a cross-sectional survey of healthcare facilities, followed by structured-interview with people with disabilities and healthcare managers, and qualitative interviews among people with disabilities or their caregivers, healthcare providers (HCPs), policymakers, and community leaders. Data were collected from 150 public healthcare (primary-to-tertiary) facilities and from 300 people with disabilities in 16 districts across Bangladesh between January-December 2019. An observational checklist and structured questionnaires were used to assess the situation of healthcare facilities, and literature-guided guidelines were used for qualitative interviews. During analysis, the disability-friendliness of healthcare facilities were quantified through a scoring system, and thematic analysis of qualitative data was performed to identify the challenges of implementing disability-friendly healthcare (DFHC). Results The score for providing DFHC was low across all the four objectives in the healthcare facilities. The highest score (mean percentage) was observed in the infrastructure domain: 29.3 ± 20.5, followed by communication: 18.2 ± 4.8, and information: 14.6 ± 6.22, and the lowest (0.93 ± 7.1) score was for capacity of the HCPs to provide DFHC. Mean percentage scores for access to 13 infrastructure points were low, and extremely low scores were found in areas such as access to elevators (5.6 ± 5.0), ticket counters (7.3 ± 17.7) and toilets (10.6 ± 9.3). Furthermore, about 59.1% of people with disabilities expressed dissatisfaction regarding access to information and communication. The majority (98.2%) recommended that training of HCPs can improve the situation. Conclusion This study revealed that most of the public health facilities in Bangladesh were not disability-friendly. Findings can inform development of a national disability-friendly policy with implementation guidelines.

Background: The largest contiguous tract of tidal halophytic mangroves in Bangladesh is about 10,000 km2. It has a wide range of physiochemical and nutritional qualities, as well as ecological health. Methods: To understand the hazardous and heavy metal concentrations, ten heavy metals concentrations were examined in both water and sediment. Indices for pollution and contamination, such as Modified Hazard Quotient (mHQ), Degree of Contamination (Cd), Geo-Accumulation Index (Igeo), Mean Effect Range Median Quotient (M-ERM-Q), Nemerow's Pollution Index (NPI), Heavy Metal Evaluation Index (HEI) give a more accurate diagnostic result for water and sediment in the Sundarbans mangroves. The Water Quality Index (WQI) and Mangrove Quality Index (MQI) are also evaluated for a final statement. Results: The nutrient concentrations average of NO3, PO4, NH3, and SiO2 are found 0.59 ± 0.05, 0.09 ± 0.01, 0.23 ± 0.02 and 1.72 ± 0.17 (mg/L), respectively in water. Pb, Cd, Hg, As, Cr, Mn, Fe, Ni, Cu, and Zn have average concentrations of 19.1 ± 1.62, 1.1 ± 0.11, 0.48 ± 0.04, 1.19 ± 0.11, 83.27 ± 8.3, 35.45 ± 2.95, 282.73 ± 30.82, 33.85 ± 2.95, 6.09 ± 0.54, and 172.18 ± 17.15 (µg/L) in surface water and 19.57 ± 1.25, 0.16 ± 0.005, 0.026 ± 0.0009, 1.48 ± 0.08, 36.38 ± 4.31, 456.66 ± 76.76, 42.36 ± 2.57, 53.81 ± 1.93, 20.67 ± 1.24, and 43.60 ± 6.09 (mg/kg) for sediments. According to the heavy metals analysis indices, we discovered very little contamination. The WQI indicates that the water quality in the Sundarbans Mangroves is generally protected but occasionally deteriorates. The MQI categorizes the Sundarbans as a moderate mangrove, falling into category 3. Conclusions: Despite being controlled the heavy metal pollution is getting worse as the Mongla port's economy grows. A more seasonal and in-depth approach to this topic can be achieved by collecting more samples over a year. The deltaic Sundarbans mangrove ecosystem is currently in moderate to an acceptable condition, with low or non-significant pollution levels and fair water quality, according to this study, except for certain constraints.

Abstract Reuse of wastewater for vegetable cultivation is becoming popular in order to augment the inadequate irrigation supplies and meet the growing demands of ground water for agriculture and industries production in different regions of the world. This study was investigated to optimize different stages of textile dyeing wastewater (TDW) for irrigation focusing on their effect on growth, yield and physiochemical attributes of tomato, plant nutrient use, heavy metals enrichment and pollution load of the irrigated soil. Textile wastewater were collected from the seven stages of (second wash after scouring and bleaching T2; enzyme treated water T3; second wash after bath drain T4; neutralization treatment T5; second wash after soaping T6; fixing treatment water T7; mixed effluent T8) of a dyeing process for physiochemical characterization and evaluation their irrigation feasibility for tomato cultivation in compare with the ground water (T1). The pot experiment consists of eight irrigation treatments was laid out following a completely randomized block design with three replications. Results showed the presence of plant nutrients and heavy metals in all the studied samples where T8 (mixed effluent) exceeded the limit of agricultural standard for almost all physiological parameters such as TDS, TSS, EC, BOD, COD affording the highest value. Objectives Insufficient physical activity (IPA) is a crucial risk factor for non-communicable diseases (NCDs). The elderly population has a higher likelihood of suffering from NCDs. We aimed to estimate the prevalence of and factors associated with IPA among the elderly people in Bangladesh.Methods We analysed data from the Bangladesh Food Security and Nutrition Surveillance round 2018–2019, collected from 82 rural, non-slum urban and slum clusters selected using multistage cluster sampling. IPA was defined as 10 years of education, inadequate fruits and vegetable consumption, self-reported asthma and higher waist circumference among men; and higher household income and self-reported diabetes among women were associated with a higher prevalence of IPA.Conclusions IPA is highly prevalent among Bangladeshi elderly men and women. Sedentary behaviour, inadequate fruits and vegetable consumption and higher waist circumference were the modifiable factors of IPA. Evidence from this study can guide the development of appropriate interventions to promote healthy ageing in Bangladesh.

Malnutrition among adolescents is often associated with inadequate dietary diversity (DD). We aimed to explore the prevalence of inadequate DD and its socio-economic determinants among adolescent girls and boys in Bangladesh. A cross-sectional survey was conducted during the 2018–19 round of national nutrition surveillance in Bangladesh. Univariate and multivariable logistic regression was performed to identify the determinants of inadequate DD among adolescent girls and boys separately. This population-based survey covered eighty-two rural, non-slum urban and slum clusters from all divisions of Bangladesh. A total of 4865 adolescent girls and 4907 adolescent boys were interviewed. The overall prevalence of inadequate DD was higher among girls (55⋅4 %) than the boys (50⋅6 %). Moreover, compared to boys, the prevalence of inadequate DD was higher among the girls for almost all socio-economic categories. Poor educational attainment, poor maternal education, female-headed household, household food insecurity and poor household wealth were associated with increased chances of having inadequate DD in both sexes. In conclusion, more than half of the Bangladeshi adolescent girls and boys consumed an inadequately diversified diet. The socio-economic determinants of inadequate DD should be addressed through context-specific multisectoral interventions.

Objective We aimed to estimate the gender-specific prevalence and associated factors of hypertension among elderly people in Bangladesh.Design and method We analysed data from the food security and nutrition surveillance round 2018–2019. The multistage cluster sampling method was used to select the study population. Hypertension was defined as systolic blood pressure ≥140 mm Hg and/or diastolic blood pressure ≥90 mm Hg and/or having a history of hypertension. We carried out the descriptive analysis, bivariate and multivariable logistic regression to report the weighted prevalence of hypertension as well as crude and adjusted ORs with 95% CI. A p value10 years increased odds of hypertension (1.83 (1.38, 2.44)) among males.Conclusion In Bangladesh, half of the elderly persons were hypertensive, with a higher prevalence in females. In both sexes, odds of hypertension was higher among persons with older age (≥70 years), insufficient physical activity, higher waist circumference and self-reported diabetes. The Ministry of Health of Bangladesh should consider these findings while designing and implementing health programmes for elderly population.

The World Health Organization (WHO) has recently developed a non-laboratory based cardiovascular disease (CVD) risk chart considering the parameters age, sex, current smoking status, systolic blood pressure, and body mass index. Using the chart, we estimated the 10-years CVD risk among the Bangladeshi population aged 40-74 years. We analyzed data from a nationally representative survey conducted in 2018-19. The survey enrolled participants from 82 clusters (57 rural, 15 non-slum urban, and 10 slums) selected by multistage cluster sampling. Using the non-laboratory-based CVD risk chart of the World Health Organization (WHO), we categorized the participants into 5 risk groups: very low ( = 30%) risk. We performed descriptive analyses to report the distribution of CVD risk and carried out univariable and multivariable logistic regression to identify factors associated with elevated CVD risk (> = 10% CVD risk). Of the 7,381 participants, 46.0% were female. The median age (IQR) was 59.0 (48.0-64.7) years. Overall, the prevalence of very low, low, moderate, high, and very high CVD risk was 34.7%, 37.8%, 25.9%, 1.6%, and 0.1%, respectively. Elevated CVD risk (> = 10%) was associated with poor education, currently unmarried, insufficient physical inactivity, smokeless tobacco use, and self-reported diabetes in both sexes, higher household income, and higher sedentary time among males, and slum-dwelling and non-Muslim religions among females. One in every four Bangladeshi adults had elevated levels of CVD risk, and males are at higher risk of occurring CVD events. Non-laboratory-based risk prediction charts can be effectively used in low resource settings. The government of Bangladesh and other developing countries should train the primary health care workers on the use of WHO non-laboratory-based CVD risk charts, especially in settings where laboratory tests are not available.

BackgroundSmoking cigarette/bidi, is a serious health threat, causes preventable premature morbidity and mortality. Higher prevalence of smoking among the youth hampers a country's development, as the youth are the main drivers of socio-economic development. An effective understanding of factors associated with youth smoking is precious to prevent youth smoking. This study aims to identify the determinants of smoking cigarette/bidi among the youth male of the rural areas of Mymensingh district in Bangladesh.MethodsThe primary data from the project "Knowledge, awareness and practices among youth smokers in Trishal Upazila under Mymensingh district: A micro-survey study", funded by the Research and Extension Center, Jatiya Kabi Kazi Nazrul Islam University, Bangladesh was utilized in the current study. The data consists of 385 youth males aged 15-24 years who were interviewed face-to-face from the rural areas of Mymensingh district in Bangladesh. Univariate distribution, chi-square tests, and binary logistic regression model were employed to identify the factors associated with smoking cigarette/bidi among the youth male.ResultsThe prevalence of smoking cigarette/bidi among the youth male is 40.3% [95% CI: 35.0%-45.0%]. Age, occupation, monthly income, family's monthly income, cigarette/bidi smoking status of father, brother and close friends, and knowledge about harmfulness of smoking are revealed as the determinants of cigarette/bidi smoking. For instance, the odds of being smoker increases with the increase in age (Odds ratio [OR]: 1.33 [1.17-1.51]). Business owner is less likely (OR: 0.15 [0.03-0.68]) to smoke than the day labourer. Having smoker fathers (OR: 2.51 [1.39-4.53]), smoker brothers (OR: 2.88 [1.39-5.96]), smoker friends (OR: 9.85 [5.85-1.27]) are significantly associated with smoking cigarette/bidi.ConclusionAs the first study, it provides the determinants of cigarette/bidi smoking among youth male of the rural areas of Mymensingh district in Bangladesh. Relevant authorities are suggested to consider the study's findings and recommendations to revise the existing smoking policies so that smoking among youth can be prevented for future development of the country.

Abstract Background Major depressive disorder (MDD) is a mixed disorder with the highly irregular course, inconsistent response to treatment and has no well-known mechanism for the pathophysiology. Major causes of depression are genetic, neurobiological, and environmental. However, over the past few years, altered serum levels of macro-minerals (MM) and trace elements (TE) have been recognized as major causative factors to the pathogenesis of many mental disorders. The purpose of this study was to determine the serum levels of MM (calcium and magnesium) and TE (copper, iron, manganese, selenium, and zinc) in MDD patients and find out their associations with depression risk. Methods This prospective case-control study recruited 247 patients and 248 healthy volunteers matched by age and sex. The serum levels of MM and TE were analyzed by atomic absorption spectroscopy (AAS). Statistical analysis was performed with independent sample t-tests and Pearson’s correlation test. Abstract Background Severe acute malnutrition (SAM) in children is the most serious form of malnutrition and is associated with very high rates of morbidity and mortality. For sustainable SAM management, United Nations recommends integration of community based management of acute malnutrition (CMAM) into the health system. The objective of the study was to assess the preparedness of the health system to implement CMAM in Bangladesh. Methods The assessment was undertaken during January to May 2014 by conducting document review, key informant interviews, and direct observation. A total of 38 key informant interviews were conducted among government policy makers and program managers (n = 4), nutrition experts (n = 2), health and nutrition implementing partners (n = 2), development partner (n = 1), government health system staff (n = 5), government front line field workers (n = 22), and community members (n = 2). The assessment was based on: workforce, service delivery, financing, governance, information system, medical supplies, and the broad socio-political context. Results The government of Bangladesh has developed inpatient and outpatient guidelines for the management of SAM. There are cadres of community health workers of government and non-government actors who can be adequately trained to conduct CMAM. Inpatient management of SAM is available in 288 facilities across the country. However, only 2.7% doctors and 3.3% auxiliary staff are trained on facility based management of SAM. In functional facilities, uninterrupted supply of medicines and therapeutic diet are not available. There is resistance and disagreement among nutrition stakeholders regarding import or local production of ready-to-use therapeutic food (RUTF). Nutrition coordination is fragile and there is no functional supra-ministerial coordination platform for multi-sectoral and multi-stakeholder nutrition. Conclusion There is an enabling environment for CMAM intervention in Bangladesh although health system strengthening is needed considering the barriers that have been identified. Training of facility based health staff, government community workers, and ensuring uninterrupted supply of medicines and logistics to the functional facilities should be the immediate priorities. Availability of ready-to-use therapeutic food (RUTF) is a critical component of CMAM and government should promote in-country production of RUTF for effective integration of CMAM into the health system in Bangladesh.

The western part of East/Japan Sea (WES) is an important area for understanding climate change processes and interactions between atmospheric and oceanic conditions. We analyzed the trends in recent oceanic conditions in the WES after the recent climate regime shift (CRS) that occurred in the late 1990s in the North Pacific. We explored the most important climate factors that affect oceanic conditions and determined their responses to changes in climate change. In the CRS that occurred in the late 1980s, changes in oceanic conditions in the WES were influenced by intensity changes in climate factors, and, in the late 1990s, it was by spatial changes in climate factors. The latitudinal shift of the Aleutian low (AL) pressure influences recent changes in oceanic and atmospheric conditions in the WES. The intensity of the Kuroshio Current and the sea level pressure in the Kuroshio extension region associated with the latitudinal shift of the AL pressure affects the volume of transport of the warm and saline water mass that flows into the WES and its atmospheric conditions. In addition, the fluctuations in the oceanic conditions of the WES affect various regions and depth layers differently, and these variations are evident even within the WES.

The title compound, C14H13BrO3 [systematic name: rac-(9S,10R)-9-bromo-10-hydroxy-8,8-dimethyl-9,10-dihydro-2H,8H-pyrano[2,3-f]chromen-2-one], is a substituted pyranocoumarin, obtained by bromination of seselin [8,8-dimethyl-2H,8H-pyrano[2,3-f]chromen-2-one], which was isolated from the Indian herb Trachyspermum stictocarpum (Aajmod). The pyrano ring has a distorted half-chair conformation and its mean plane is inclined to the coumarin mean plane by 1.6 (2)°. In the crystal, molecules are linked by pairs of O—H...O hydrogen bonds, forming inversion dimers with an R22(16) ring motif. The dimers stack along the a-axis direction and are linked by offset π–π interactions, forming columns [intercentroid distance = 3.514 (4) Å].

We report here a detailed experimental investigation on noncovalent functionalization of single-walled carbon nanotubes (SWNTs) with four different metallo-octaethylporphyrins (MOEPs). It has been found that the identity of the center metal of MOEP strongly influences the solubilization of SWNTs. MnOEPs and ZnOEPs successfully extracted SWNTs in methanol, as confirmed by absorption spectroscopy, while CoOEPs and CuOEPs were not able to extract SWNTs at all. Atomic force microscopy (AFM) studies revealed that large SWNTs bundles could be exfoliated into either individual SWNTs or very small bundles by complexation with ZnOEP molecules. As for enrichment of SWNTs, ZnOEPs and MnOEPs show similar diameter discrimination ability toward 76-CoMoCAT, providing the extracted SWNTs with relatively large diameters.

Bangladesh is a country with a high burden of micronutrient malnutrition. Stunting affects 41 % of children aged under 5 years. Zn is one of the key micronutrients that is associated with stunting. The present study, as part of the national micronutrient survey 2011–2012, revealed for the first time the nationally representative prevalence of Zn deficiency and determined the associations of the condition. A cross-sectional ‘nationwide’ survey was conducted in pre-school-age children (6–59 months; PSAC) and non-pregnant non-lactating women (15–49 years; NPNLW). Multistage random sampling was done in 150 clusters; fifty in each of the rural, urban and slum strata. Data were analysed on 662 PSAC and 1073 NPNLW.
