Abstract

Summary: Background: The COVID-19 pandemic heightened risks for individuals with hematological malignancies due to compromised immune systems, leading to more severe outcomes and increased mortality. While interventions like vaccines, targeted antivirals, and monoclonal antibodies have been effective for the general population, their benefits for these patients may not be as pronounced. Methods: The EPICOVIDEHA registry (National Clinical Trials Identifier, NCT04733729) gathers COVID-19 data from hematological malignancy patients since the pandemic's start worldwide. It spans various global locations, allowing comprehensive analysis over the first three years (2020–2022). Findings: The EPICOVIDEHA registry collected data from January 2020 to December 2022, involving 8767 COVID-19 cases in hematological malignancy patients from 152 centers across 41 countries, with 42% being female. Over this period, there was a significant reduction in critical infections and an overall decrease in mortality from 29% to 4%. However, hospitalization, particularly in the ICU, remained associated with higher mortality rates. Factors contributing to increased mortality included age, multiple comorbidities, active malignancy at COVID-19 onset, pulmonary symptoms, and hospitalization. On the positive side, vaccination with one to two doses or three or more doses, as well as encountering COVID-19 in 2022, were associated with improved survival. Interpretation: Patients with hematological malignancies still face elevated risks, despite reductions in critical infections and overall mortality rates over time. Hospitalization, especially in ICUs, remains a significant concern. The study underscores the importance of vaccination and the timing of COVID-19 exposure in 2022 for enhanced survival in this patient group. Ongoing monitoring and targeted interventions are essential to support this vulnerable population, emphasizing the critical role of timely diagnosis and prompt treatment in preventing severe COVID-19 cases. Abstract

Objectives: Elderly patients with hematologic malignancies face the highest risk of severe COVID-19 outcomes. The infection's impact on different age groups remains unstudied in detail. Methods: We analyzed elderly patients (age groups: 65-70, 71-75, 76-80, and >80 years old) with hematologic malignancies included in the EPICOVIDEHA registry between January 2020 and July 2022. Univariable and multivariable Cox regression models were conducted to identify factors influencing death in COVID-19 patients with hematological malignancy. Results: The study included data from 3,603 elderly patients (aged 65 or older) with hematological malignancy, with a majority being male (58.1%) and a significant proportion having comorbidities. The patients were divided into four age groups, and the analysis assessed COVID-19 outcomes, vaccination status, and other variables in relation to age and pandemic waves. The 90-day survival rate for patients with COVID-19 was 71.2%, with significant differences between groups. The pandemic waves had varying impacts, with the first wave affecting patients over 80 years old, the second being more severe in 65-70, and the third being the least severe in all age groups. Factors contributing to 90-day mortality included age, comorbidities, lymphopenia, active malignancy, acute leukemia, less than three vaccine doses, severe COVID-19, and using only corticosteroids as treatment. Conclusion: These data underscore the heterogeneity of elderly hematological patients, highlight the different impacts of COVID-19 waves and the pivotal importance of vaccination, and may help in planning future healthcare efforts.

Abstract

This paper details the development and evaluation of the enhanced future FLows and Groundwater (eFLaG) dataset of nationally consistent hydrological projections for the UK, based on the latest UK Climate Projections (UKCP18). The projections are derived from a range of hydrological models. For river flows, multiple models (Grid-to-Grid, PDM (Probability Distributed Model) and GR (Génie Rural; both four- and six-parameter versions, GR4J and GR6J)) are used to provide an indication of hydrological model uncertainty. For groundwater, two models are used, a groundwater level model (AquiMod) and a groundwater recharge model (ZOODRM: zooming object-oriented distributed-recharge model). A 12-member ensemble of transient projections of present and future (up to 2080) daily river flows, groundwater levels and groundwater recharge was produced using bias-corrected data from the UKCP18 regional (12 km) climate ensemble. Projections are provided for 200 river catchments, 54 groundwater level boreholes and 558 groundwater bodies, all sampling across the diverse hydrological and geological conditions of the UK. An evaluation was carried out to appraise the quality of hydrological model simulations against observations and also to appraise the reliability of hydrological models driven by the regional climate model (RCM) ensemble in terms of their capacity to reproduce hydrological regimes in the current period. The dataset was originally conceived as a prototype climate service for drought planning for the UK water sector and so has been developed with drought, low river flow and low groundwater level applications as the primary objectives. The evaluation metrics show that river flows and groundwater levels are, for the majority of catchments and boreholes, well simulated across the flow and level regime, meaning that the eFLaG dataset could be applied to a wider range of water resources research and management contexts, pending a full evaluation for the designated purpose. Only a single climate model and one emissions scenario are used, so any applications should ideally contextualise the outcomes with other climate model–scenario combinations. Abstract

Saving energy is as important as energy production. In systems like cars, trains, elevators, and machines that require frequent start/stop operation, a significant amount of kinetic energy is wasted during braking. It may be necessary to employ a sophisticated breaking mechanism to catch waste kinetic energy for later usage. Several criteria influence the proposed braking mechanism. A hydraulic test bench is developed and built to investigate how much kinetic energy can be absorbed and stored during braking using a hydraulic circuit. The test bench’s driving power is a variable voltage and variable frequency electric motor that drives a flywheel that constitutes the majority of the kinetic energy with speeds ranging from 876 to 2975 rpm. A positive displacement gear pump, flow control valve, non-return valve, on/off valve, direction control valve, and an accumulator are the major components of the hydraulic circuit. The use of an automated contactor and a magnetic clutch allows for alternate operations between the mechanical system and the hydraulic brake circuit. The entire system is theoretically modeled, and a MATLAB code is created to investigate the impact of various parameters on the braking mechanism’s performance. Four groups of experiments were conducted to investigate the features of the driving and braking systems, as well as the influence of various factors on the amount of energy that may be collected during braking. The results indicate that 18% to 55% of the kinetic energy may be absorbed and transformed into hydraulic power stored in the accumulator. The amount of hydraulic power gained is governed by the operating speed and restriction ratio (hydraulic system resistance). At certain rotational speeds and restriction ratios, maximum gain hydraulic power could well be produced. Not only can braking energy regeneration improve system efficiency, but it also provides environmental benefits. Alexandria, Egypt is used as a case study to investigate the economic and environmental benefits of employing this technology in cars.

Abstract

Study region: The study uses 78 groundwater head time series across 10 European countries with various geological and hydrological settings. Study focus: The estimation of groundwater recharge using time series analysis and lumped modelling based on groundwater head time series is a low-cost and practical method. However, lumped recharge estimation models based on groundwater level variations are uncertain, and successful applications are known to depend on both climate and hydrogeological setting. Here, we assess the suitability of three different models to estimate recharge (Metran - Transfer Function-Noise model, AquiMod - groundwater level driven hydrological model, and GARDÉNIA - lumped catchment model). New hydrological insights: Results showed that while all three models generally did well during the modelling of groundwater heads, the resulting recharge estimations from the models were different. The analysis showed that the transfer-noise modelling of groundwater heads with recharge and evapotranspiration in Metran is not generally applicable for recharge estimation. The addition of physical information in AquiMod improved the recharge estimations, but the reliability was still limited without control of the water balance due to non-uniqueness. By adding discharge information to the modelling, GARDÉNIA can provide more reliable recharge values. Thus, recharge estimation from groundwater head time series without water balance information must be considered uncertain with low precision, but applicability can be improved when including knowledge of the local system.

Abstract

Two field experiments were conducted in 2018/2019 and 2019/2020 seasons, at Ras Sidr Experimental Station in south SinaiAffiliated to Desert Research Center, Mataria, Cairo Egypt in cooperation with the Department of Horticulture, Faculty of Agriculture, Benha University, to study the effect of biogas fertilizer and natural extracts on vegetative characteristics and essential oil yield of Stevia rebaudiana L. plant. Results showed that biogas fertilizer along with different natural extracts exerted a significant influence on the growth characteristics and essential oil content of stevia. The highest values in most of the parameters in the first cut were found from T1 (recommended dose of chemical fertilizer) combining with (M4), (M5) or (M2). On the other hand, the second cut took the same line as the first cut but with T3 (50% NPK + biogas slurry at 5000 kg h-1) as (T3×M4) or (T3×M2) as drench addition registered the highest values of parameters mention before in most cases. Conclusively, the use of half (50%) of the recommended dose of chemical fertilizer with the equivalent of the recommended nitrogen content and replaced it with biogas fertilizer led to reducing the problem of the use of chemical fertilizers on the health and economic damages, in addition to the use of both extracts of Azolla and garlic as a drench addition with organic fertilization and half the dose of chemical fertilizer.

Abstract

Two field experiments were conducted in 2019 and 2020 seasons at Desert Research Center, Mataria, Cairo, Egypt, and its experimental farm at Ras Sudr Experimental Station in South Sinai, in cooperation with the Department of Horticulture, Faculty of Agriculture Benha University, to study the effect of biogas slurry fertilizer and natural extracts on biomass, chemical constituents, and quality of Stevia rebaudiana L. plant. Results showed that biogas fertilizer along with the different natural extracts exerted a significant influence on yield, chemical constituents, stevioside and rebaudioside A., of stevia. The highest values in most of the parameters i.e. shoot fresh and dry weights biomass, leaves fresh and dry weights biomass, N, P, K and pigments in the first cut by recommended dose of chemical fertilizer (T1) combining with garlic as drench addition, Yucca extract as a foliar spray or Azolla extract as drench addition (M4, M5 or M2,respectively). On the other hand, the second cut took the same line as the first cut but with 50% NPK + 5000 kg h-1 (T3) as either garlic extract or Azolla extract as drench addition (T3×M4 or T3×M2) as registered the highest values of parameters mention before in most cases. As for the highest values of stevioside and total stevioside content were recorded by 50% NPK + 3000 kg h-1 combining with Azolla aqueous extract as drench addition (T4×M2). Rebaudioside A., registered by combination of the recommended dose of biogas fertilizer and yucca aqueous extract as a foliar spray (T2×M5). Conclusively, the use of half (50%) of the recommended dose of chemical fertilizer with its equivalent of the recommended nitrogen dose and replaced with biogas fertilizer, in addition to the use of both extracts of Azolla and garlic as a drench addition, led to reducing nitrate accumulation as a problem of the use of chemical fertilizers on the human health and economic damages.

9. Thermal study of clay bricks reinforced by sisal-fibers used in construction in south of Morocco

Abstract

Clay is a local construction material used in a traditional building, it has many environmental, social and cultural benefits. The present research study focuses on improving the thermophysical properties of clay extracted from Erfoud city in the south-east of Morocco by incorporating natural additives such as sisal fibers. Several samples were prepared and their thermophysical properties were characterized experimentally using the steady state hot plate and the flash methods to estimate the thermal conductivity and the thermal diffusivity, respectively. The results showed a significant interest in the future use of this composite material in the construction buildings in order to ensure thermal comfort and reduce greenhouse gas emissions (CO2). The dynamic thermal simulation was performed of the traditional building Ksar Lamaadid in Erfoud city in order to get energy savings without affecting indoor comfort levels according to outside temperatures. 11. Quantification of the exhaust emissions from seagoing ships in Suez Canal waterway

Abstract

This study aimed to quantify the exhaust emissions from seagoing ships and evaluate the relevance of shipping as an air polluter in Suez Canal area. Suez Canal and Port Said authority's databases and engine manufactures are used to collect technical information about seagoing ships passing the Canal. To overcome the lake of data, correlations have been developed for each ship category to predict the power of the auxiliary engines as a function of the power of the main engines.In this study, exhaust pollutants are estimated based on two models developed by the maritime industry. Also, ships emissions are predicted for each ship category at different modes of operation. Then, the predicted emissions are correlated and expressed as a function of the numbers of ships passing the canal and their net tonnage.The results indicated that the average emissions from seagoing ships passing Suez Canal were evaluated to be 8.16 million tons annually with a potential increase in the future. Also, container ships are the main source of air pollution amongst other ship categories and mainly responsible about 48–59% of the air pollution. Abstract

The performance of induction magnetometers, in terms of resolution, depends both on the induction sensor and the electronic circuit. To investigate accurately the sensor noise sources, an induction sensor, made of a ferrite ferromagnetic core, is combined with a dedicated low voltage and current noise preamplifier, designed in CMOS 0.35 μm technology. A modelling of the contribution of the ferromagnetic core to the noise through the complex permeability formalism is performed. Its comparison with experimental measurements highlight another possible source for the dominating noise near the resonance.

Abstract

Studying Gaussian beam is a method to investigate laser beam propagation and ABCD matrix is a fast and simple method to simulate Gaussian beam propagation in different mediums. Of the ABCD matrices studied so far, reflection and refraction matrices at various surfaces have attracted a lot of researches. However in previous work the incident beam and the principle axis of surface are in parallel. As an extension to those investigations, a general scheme that the incident beam is oblique is discussed here and the full analysis of the reflection and refraction of a Gaussian beam at the surface of a tilted concave/convex elliptic paraboloid surface is addressed. Based on the optical phase matching, analytic mathematical equations are derived for the spot size and the wavefront radius of a beam. Expressions are converted into the ABCD matrices, which are more convenient and practical to use. Finally, a practical case is analyzed by applying the obtained formulas. This analysis is important since paraboloid surfaces in optics or terahertz waves are used as mirrors or lenses.

Abstract

Small bowel lymphomas of the extranodal type occur in the young and are characteristically associated with malabsorption syndrome. We present the case of an elderly in whom there was no malabsorption and the duodenal tumor was a gastric type marginal zone B cell lymphoma also known as gastric mucosa-associated lymphoid tissue (MALT) lymphoma. A 73-year-old woman presented to the emergency room with 2 weeks of general weakness, recurrent vomiting containing food particles and abdominal distension. She had been diagnosed with diabetic gastroparesis 4 years prior. CT of the abdomen and pelvis was suggestive of gastric outlet obstruction but no evidence of pancreatic or duodenal mass. Endoscopy and biopsy of the tumor obstructing the distal first part of the duodenum confirmed a gastric marginal MALT lymphoma. The patient’s symptoms improved with radiotherapy. Gastric MALT lymphoma, an extranodal lymphoma primarily described in the stomach, can also present in the small bowel and is not associated with malabsorption.
