A myocardial infarction can cause irreversible damage to the heart muscle. A promising approach for the treatment of myocardial infarction and prevention of severe complications is the application of cardiac patches or epicardial restraint devices. The challenge for the fabrication of cardiac patches is the replication of the fibrillar structure of the myocardium, in particular its anisotropy and local elasticity. In this study, we developed a chitosan–gelatin–guar gum-based biomaterial ink that was fabricated using 3D printing to create patterned anisotropic membranes. The experimental results were then used to develop a numerical model able to predict the elastic properties of additional geometries with tunable elasticity that could easily match the mechanical properties of the heart tissue (particularly the myocardium).

nvestigations on adverse biological effects of nanoparticles (NP) are performed usually either in vivo on rodents or in vitro under submerged conditions where NP are in suspension into cell culture media. However, sedimentation of NP in vitro is a continuous process and to assess the exact deposited cellular dose remains difficult, as the cellular internal dose is a function of time. Moreover, the cellular responses to NP under submerged culture conditions or by exposing rodents by nose-only to NP aerosols might differ from those observed at physiological settings at the air-liquid interface (ALI). Rat alveolar NR8383 macrophages were exposed to aerosols at the air-liquid interface. We studied TiO2 NM105, a mixture of anatase and rutile. NR8383 cells were exposed to a single dose of 3.0 cm2/cm2 of TiO2 aerosol. Following RNA extraction, transcriptome allowing full coverage of the rat genome was performed, and differentially expressed genes were retrieved. Their products were analyzed for functions and interaction with String DB. Only 126 genes were differentially expressed and 98 were recognized by String DB and give us the gene expression signature of exposed rat alveolar NR8383 macrophages. Among them, 13 display relationships at a high confidence level and the ten most differentially expressed compared to unexposed cells were: Chac1, Ccl4, Zfp668, Fam129b, Nab2, Txnip, Id1, Cdc42ep3, Dusp6 and Myc, ranked from the most overexpressed to the most under-expressed. Some of them were previously described as over or under-expressed in NP exposed cell systems. We validated in our laboratory an easy-to-use device and a physiological relevant paradigm for studying the effects of cell exposure to TiO2. Ccl4 gene expression seems to be a positive marker of exposure evidenced as well as in vivo or in both in vitro conditions.

Obesity is linked with insulin resistance and is characterized by excessive accumulation of adipose tissue due to chronic energy imbalance. Increasing thermogenic brown and beige adipose tissue futile cycling may be an important strategy to increase energy expenditure in obesity, however, brown adipose tissue metabolic activity is lower with obesity. Herein, we report that the exposure of mice to thermoneutrality promotes the infiltration of white adipose tissue with mast cells that are highly enriched with tryptophan hydroxylase 1 (Tph1), the rate limiting enzyme regulating peripheral serotonin synthesis. Engraftment of mast cell-deficient mice with Tph1−/− mast cells or selective mast cell deletion of Tph1 enhances uncoupling protein 1 (Ucp1) expression in white adipose tissue and protects mice from developing obesity and insulin resistance. These data suggest that therapies aimed at inhibiting mast cell Tph1 may represent a therapeutic approach for the treatment of obesity and type 2 diabetes., Serotonin inhibits adipose tissue thermogenesis. Here the authors show that obese mice housed in thermoneutrality have increased mast cell serotonin synthesis, and that inhibiting this pathway through deletion of mast cell Tph1 increases white adipose tissue browning and protects against diet-induced obesity, insulin resistance and liver steatosis.

Elevated liver de novo lipogenesis contributes to non-alcoholic steatohepatitis (NASH) and can be inhibited by targeting acetyl-CoA carboxylase (ACC). However, hypertriglyceridemia limits the use of pharmacological ACC inhibitors as a monotherapy. ATP-citrate lyase (ACLY) generates acetyl-CoA and oxaloacetate from citrate, but whether inhibition is effective for treating NASH is unknown. Here, we characterize a new mouse model that replicates many of the pathological and molecular drivers of NASH and find that genetically inhibiting ACLY in hepatocytes reduces liver malonyl-CoA, oxaloacetate, steatosis, and ballooning as well as blood glucose, triglycerides, and cholesterol. Pharmacological inhibition of ACLY mirrors genetic inhibition but has additional positive effects on hepatic stellate cells, liver inflammation, and fibrosis. Mendelian randomization of human variants that mimic reductions in ACLY also associate with lower circulating triglycerides and biomarkers of NASH. These data indicate that inhibiting liver ACLY may be an effective approach for treatment of NASH and dyslipidemia.

There are many studies concerning titanium dioxide (TiO2) nanoparticles (NP) toxicity. Nevertheless, there are few publications comparing in vitro and in vivo exposure, and even less comparing air–liquid interface exposure (ALI) with other in vitro and in vivo exposures. The identification and validation of common markers under different exposure conditions are relevant for the development of smart and quick nanotoxicity tests. In this work, cell viability was assessed in vitro by WST-1 and LDH assays after the exposure of NR8383 cells to TiO2 NP sample. To evaluate in vitro gene expression profile, NR8383 cells were exposed to TiO2 NP during 4 h at 3 cm2 of TiO2 NP/cm2 of cells or 19 μg/mL, in two settings—submerged cultures and ALI. For the in vivo study, Fischer 344 rats were exposed by inhalation to a nanostructured aerosol at a concentration of 10 mg/m3, 6 h/day, 5 days/week for 4 weeks. This was followed immediately by gene expression analysis. The results showed a low cytotoxic potential of TiO2 NP on NR8383 cells. Despite the absence of toxicity at the doses studied, the different exposures to TiO2 NP induce 18 common differentially expressed genes (DEG) which are involved in mitosis regulation, cell proliferation and apoptosis and inflammation transport of membrane proteins. Among these genes, we noticed the upregulation of Ccl4, Osm, Ccl7 and Bcl3 genes which could be suggested as early response biomarkers after exposure to TiO2 NP. On the other hand, the comparison of the three models helped us to validate the alternative ones, namely submerged and ALI approaches.

International audience; Human gastroenteritis viruses are amid the major causes of disease worldwide, responsible for more than 2 million deaths per year. Human noroviruses play a leading role in the gastroenteritis outbreaks and the continuous emergence of new strains contributes to the significant morbidity and mortality. Many aspects of the viral entry and infection process remain unclear, including the major response of the host cell to the virus, which is the trigger of several programmed cell death related mechanisms. In this review, we assessed apoptosis and autophagy at various stages in the infection process to provide better understanding of the viral–host interaction. This brings us closer to fully understanding how noroviruses work, thus allowing the development of specific antiviral therapies.

Functionalized multi-walled carbon nanotubes (MWCNT) have become the focus of increased research interest, particularly in their application as tools in different areas, such as the biomedical field. Despite the benefits associated with functionalization of MWCNT, particularly in overcoming issues relating to solubility, several studies have demonstrated that these functionalized nanoparticles display different toxicity profiles. For this study, we aim to compare NR8383 cells responses to three well-characterized MWCNT with varying functional groups. This study employed cytotoxicity assays, transcriptomics and proteomics to assess their toxicity using NR8383 rat alveolar macrophages as an in vitro model. The study findings indicated that all MWCNT altered ribosomal protein translation, cytoskeleton arrangement and induced pro-inflammatory response. Only functionalized MWCNT alter mTOR signaling pathway in conjunction with increased Lamtor gene expression. Furthermore, the type of functionalization was also important, with cationic MWCNT activating the transcription factor EB and inducing autophagy while the anionic MWCNT altering eukaryotic translation initiation factor 4 (EIF4) and phosphoprotein 70 ribosomal protein S6 kinase (p70S6K) signaling pathway as well as upregulation Tlr2 gene expression. This study proposes that MWCNT toxicity mechanisms are functionalization dependent and provides evidence that inflammatory response is a key event of carbon nanotubes toxicity.

Resume Objectifs La survenue d’une thrombose de la valve ou des cusps apres TAVI est mal connue, alors qu’elle impacte negativement la durabilite de la prothese et que son incidence est plus frequente qu’initialement suppose. Epidemiologie L’incidence de thromboses infracliniques survenant entre le premier et le troisieme mois apres implantation de TAVI est comprise entre 10 et 15 %. Toutes les valves implantees sont concernees. Diagnostic La reprise de symptomes ou une augmentation des gradients transvalvulaires sont les deux modes d’expressions classiques de thrombose. Les facteurs favorisants sont nombreux : l’âge avance des patients, l’armature metallique des protheses, l’expansion parfois incomplete de la prothese, son apposition incomplete, la creation de zones de stagnation de sang. Certains facteurs de risques sont rapportes : les valves larges, l’alteration de la FEVG, les valve in valve, le sexe masculin, les sinus de grand diametre, l’absence de traitement anticoagulant. Traitement Le traitement post-TAVI reste empirique et retient 1 a 6 mois de monotherapie ou de bitherapie anti-agregante, prolonge d’un traitement par aspirine a vie. En cas de thrombose averee, une anticoagulation orale par AVKs ou par anticoagulants oraux directs permet a 3 mois une restauration du jeu valvulaire normal, avec un delai moyen de reduction du gradient transvalvulaire de 14 jours.

Screening of myocardial ischemia refers to the use of one or more diagnostic tests for coronary heart disease with a dual objective of appropriateness and promptness. In women, as compared to men, the accuracy of the different tests is worse. Thus, to overcome this sex-related penalty, we must define a diagnosis strategy based on risk stratification, enabling the identification of patients requiring invasive investigations. This review discusses various non-invasive diagnostic tests focusing on a female-specific approach and defines the use of numerous diagnostic tests with respect to both risk stratification and symptoms.

Routine manual thrombectomy during primary percutaneous coronary intervention might be intuitively justified. While older registers reported contradictory results, interventional cardiologists remained interested in using such devices during the mechanical treatment of acute myocardial infarction. The first studies were congruent to demonstrate a significant relationship between thromboaspiration and significant improvement of ST-segment elevation, lower distal embolization, despite TAPAS was the only to significantly reduce the mortality. Later studies were unable to confirm these promising data, avoiding routine manual thrombectomy prior to primary angioplasty to decrease cardiovascular mortality, recurrent myocardial infarction, cardiogenic shock or severe heart failure. Moreover, thrombectomy was associated with an increased rate of stroke. Should thrombectomy therefore be conclusively overlooked? It is presumably required to define which patient is eligible for thrombectomy, to define how to perfectly perform manual thrombectomy, to specify how to gently move towards the thrombus, the optimal pharmacological environment, the number of aspirations and the criterion to stop or to repeat aspiration. Indeed, while thrombectomy is nowadays scientifically downgraded, it remains of potential interest in numerous interventional cardiologists.

We demonstrate edge-emitting exciton-polariton (polariton) laser operation from 5 to 300 K and polariton amplifiers based on polariton modes within ZnO waveguides. The guided mode dispersion below and above the lasing threshold is directly measured using gratings placed on top of the sample, fully demonstrating the polaritonic nature of the lasing modes. The threshold is found to be smaller than that expected for radiative polaritons in planar ZnO microcavities below 150 K and comparable above. These results open up broad perspectives for guided polaritonics by enabling easier and more straightforward implementation of polariton integrated circuits that exploit fast propagating polaritons, and, possibly, topological protection., Edge-Emitting Polariton lasers: A step closer to two-dimensional all-optical devices A type of laser that can propagate along tiny, two-dimensional horizontal cavities in semiconductor materials at room temperature could be used in novel all-optical devices. Polaritons are quasi-particles created from the resonant coupling of light (photons) and matter (electron-hole pairs, or ‘excitons’) in tiny enclosed spaces, such as the microcavities fabricated from semiconductor materials. Polariton lasers could enable researchers to build energy-efficient, compact all-optical devices. Guillaume Malpuech at the Institut Pascal in Clermont-Ferrand, France, and co-workers have designed a new type of polariton laser and amplifier operating at room temperature. Their design does not require the use of a microcavity but is based on the use of the polariton guided modes of a ZnO layer. This horizontal, guided geometry is appealing because it is easy to fabricate and to integrate in two-dimensional optical circuits.

Viral respiratory infections are common in children, most of which are due to a virus. They can lead to serious infections, and these children may require treatment in a pediatric intensive care unit (PICU). This prospective study evaluated the epidemiology of respiratory viruses and associated illnesses among children hospitalized in a PICU during the three winter months of 2012-2013.All the children admitted to the PICU, University Hospital of Caen, France, were included. Nasal swabs were collected and specimens were tested using a single real-time PCR (polymerase chain reaction).Of the 105 patients admitted to the PICU during the study period, 84 fulfilled the inclusion criteria. The "respiratory group" included 37 patients with respiratory symptoms at admission while the "nonrespiratory group" included 47 patients with no respiratory symptoms. The 84 nasal swabs collected included 54 that were considered positive (64.3%) and 70 viruses were detected. The most commonly detected virus was RSV (n=28; 40.0% positive samples), followed by HRV (n=24; 34.3%). Viruses were more frequently detected in the respiratory (86.5%) than in the nonrespiratory (42.6%) group (P0.001). Statistical analysis by subgroups revealed that RSV infections were significantly more frequent in the respiratory group (54.1%) than in the nonrespiratory group (6.4%) (P0.001). There was no difference for HRV (32.4% and 27.7%) or for the other viruses. No difference in duration of hospitalization or duration of mechanical ventilation was demonstrated depending on the virus detected.The use of the very sensitive multiplex PCR technique increased virus detection rates in both symptomatic and asymptomatic subjects.We have confirmed the frequency of RSV infections in a PICU and found that many patients without respiratory symptoms have respiratory infections caused by viruses. The impact of these infections on patient outcome should now be analyzed in order to demonstrate the role played by respiratory viruses.

Transcatheter aortic valve or leaflets thrombosis are mainly misapprehended. It negatively impacts the long-term efficiency of such prosthesis. Moreover, its incidence is presumably higher than previously described.Recently reported subclinical leaflet thrombosis, occurring between first to third months after implantation, is about 10to 15%. All prosthesis are concerned by potential thrombosis.Transcatheter aortic valve thrombosis is usually detected on the basis of increased transvalvular pressure gradients or symptoms at routine follow-up. Main causes of post-TAVI thrombosis are: elderly patients, incomplete TAVI expansion, incomplete TAVI apposition to the aortic wall, areas of diminished blood flow and stagnation around the prosthesis. Main risk-factors of thrombosis are: large valves, patients with ejection fraction lower than 35%, valve-in-valve implantation, males, patients with large sinus of Valsalva, patients without anticoagulants.Post-TAVI anti thrombotic regimen remains empirical. It is based on aspirin alone or dual antiplatelet therapy for 1-6months, followed by aspirin for life. Symptomatic patients with thrombus-based valve dysfunction are to be treated by oral anticoagulation (i.e., VKA or new oral anticoagulants NOAC). This treatment allows the restoration of normal leaflet motion in patients with a median time of 14 days.

Widespread antibiotic use in clinical medicine and the livestock industry has contributed to the global spread of multidrug-resistant (MDR) bacterial pathogens, includingAcinetobacter baumannii. We report on a method used to produce a personalized bacteriophage-based therapeutic treatment for a 68-year-old diabetic patient with necrotizing pancreatitis complicated by an MDRA. baumanniiinfection. Despite multiple antibiotic courses and efforts at percutaneous drainage of a pancreatic pseudocyst, the patient deteriorated over a 4-month period. In the absence of effective antibiotics, two laboratories identified nine different bacteriophages with lytic activity for anA. baumanniiisolate from the patient. Administration of these bacteriophages intravenously and percutaneously into the abscess cavities was associated with reversal of the patient's downward clinical trajectory, clearance of theA. baumanniiinfection, and a return to health. The outcome of this case suggests that the methods described here for the production of bacteriophage therapeutics could be applied to similar cases and that more concerted efforts to investigate the use of therapeutic bacteriophages for MDR bacterial infections are warranted.

Fire blight, caused by enterobacterium Erwinia amylovora, is the most important bacterial disease affecting pome fruit. It can cause devastating economic losses and is reliably controlled only by the application of antibiotics, which are banned in many European countries due to environmental and sustainability issues and consumers’ mistrust. One solution could be the utilization of fire blight resistant cultivars in apple production. In 2003, we started an approach at Dresden-Pillnitz to detect different mechanisms conferring resistance to fire blight aimed at their com-bination in new cultivars. Four segregating populations were established to map QTLs for fire blight resistance. The donors used were three wild species accessions of Malus baccata, M. fusca, M. ×robusta 5 and the Pillnitz cultivar ‘Rewena’. The susceptible parent in each case was Idared. Grafted scions of each progeny were inoculated with E. amylovora strain Ea222_JKI. Average percent lesion length (PLL) of all progenies was determined. Genetic linkage maps were established using DArT- SCAR-, SNP-, and SSR-markers. Whereas in Rewena no QTL could be determined, major QTLs were detected in M. baccata on linkage group 12, in M. fusca on linkage group 10, and in M. ×robusta 5 on linkage group 3 explaining up to around 50, 85 and 85% of the phenotypic variance, respectively. The fact that all resistance QTLs are located on different linkage groups enhances the chance that different mechanisms are acting in the donors. Additionally, trees of the Idared by M. ×robusta 5 population were planted in an orchard and flowers were inoculated for phenotyping. The QTL on linkage group 3 could be con-firmed after mapping.

UNLABELLED • PREMISE OF THE STUDY Given the worldwide economic importance of maize endosperm, it is surprising that its development is not the most comprehensively studied of the cereals. We present detailed morphometric and cytological descriptions of endosperm development in the maize inbred line B73, for which the genome has been sequenced, and compare its growth with four diverse Nested Association Mapping (NAM) founder lines.• METHODS The first 12 d of B73 endosperm development were described using semithin sections of plastic-embedded kernels and confocal microscopy. Longitudinal sections were used to compare endosperm length, thickness, and area.• KEY RESULTS Morphometric comparison between Arizona- and Michigan-grown B73 showed a common pattern. Early endosperm development was divided into four stages: coenocytic, cellularization through alveolation, cellularization through partitioning, and differentiation. We observed tightly synchronous nuclear divisions in the coenocyte, elucidated that the onset of cellularization was coincident with endosperm size, and identified a previously undefined cell type (basal intermediate zone, BIZ). NAM founders with small mature kernels had larger endosperms (0-6 d after pollination) than lines with large mature kernels.• CONCLUSIONS Our B73-specific model of early endosperm growth links developmental events to relative endosperm size, while accounting for diverse growing conditions. Maize endosperm cellularizes through alveolation, then random partitioning of the central vacuole. This unique cellularization feature of maize contrasts with the smaller endosperms of Arabidopsis, barley, and rice that strictly cellularize through repeated alveolation. NAM analysis revealed differences in endosperm size during early development, which potentially relates to differences in timing of cellularization across diverse lines of maize.

Aims This article shows the effect of nanosecond pulsed electric field (nsPEF) on Escherichia coli, which could imply a durable change in protein expressions and then impacted the phenotype of surviving bacteria that might lead to increase pathogenicity. Methods and Results The effects of nsPEF on E. coli viability and membrane permeabilization were investigated. One log10 reduction in bacterial counts was achieved at field strength of 107 V m−1 with a train of 500 successive pulses of 60 × 10−9 s. Incubation of germs after treatment with propidium iodide showed that membrane permeabilization was reversible. Possible protein changes of surviving bacteria were checked to assess potential phenotypical changes using two-dimensional electrophoresis. In our study, after 40 generations, only UniProt #P39187 was up-regulated with P ≤ 0·05 compared with the control and corresponded to the uncharacterized protein YtfJ. Antibiograms were used to check whether or not the pattern of cultivable bacteria after nsPEF deliveries changed. Conclusions The results tend to show that nsPEFs are able to inactivate bacteria and have probably no serious impact in E. coli protein patterns. Significance and Impact of the Study The use of nsPEF is a safe promising new nonthermal method for bacterial inactivation in the food processing and environmental industry.

We report a case of bilateral spontaneous uterine rupture of an unscarred uterus occured in a primigravida at 32 weeks to take care in our department after in utero transfert. Uterine rupture occurs mainly on scarred uterus during labor. This is an unfrequent but serious complication involving fetal-maternal prognosis in the absence of immediate care. We are conducting a review about spontaneous uterine rupture of unscarred uterus, before and during labor.

Resume Introduction La syphilis est une maladie en resurgence depuis une dizaine d’annees. La neurosyphilis peut apparaitre a n’importe quel stade d’infection. Son diagnostic est rendu difficile par l’existence de formes trompeuses. Nous en decrivons un exemple. Observation Une femme de 56 ans presentait une symptomatologie evocatrice de maladie de Horton associee a une pseudopolyarthrite rhizomelique dans un contexte de prise prolongee d’ibuprofene. On notait egalement des myodesopsies, un exantheme a type de syphilides papuleuses et de roseole, ainsi qu’un syndrome inflammatoire biologique. Une ponction lombaire objectivait une meningite lymphocytaire avec Treponema Pallidum Haemagglutination Assay (TPHA) positif dans le liquide cephalo-rachidien, confirmant le diagnostic de neurosyphilis. De plus, l’examen ophtalmologique objectivait une nevrite optique avec atteinte papillaire d’origine syphilitique. L’evolution etait favorable sous un traitement de penicilline G par voie intraveineuse poursuivi pendant trois semaines. Discussion Une symptomatologie evocatrice de maladie de Horton et de pseudopolyarthrite rhizomelique peut reveler une syphilis, qui reste classiquement une « grande simulatrice ».

South Africa has the highest prevalence of Fetal Alcohol Spectrum Disorders (FASDs) in the world. The purpose of this study was to identify high risk factors associated with drinking alcohol prior to pregnancy recognition in 24 neighborhoods in the Cape Flats outside Cape Town, South Africa. An interviewer assessed risk among 619 pregnant Black/African women between the ages of 18 and 41 years. Logistic regression analyses explored factors associated with drinking alcohol post conception but prior to pregnancy recognition. Forced multiple logistic regression analysis revealed that drinking prior to pregnancy recognition was associated with being younger, single, having better living conditions, smoking, having a longer gestation prior to pregnancy recognition, having a greater number of sexual partners, and a higher incidence of intimate partner violence. Depressive symptoms tended to be higher among alcohol users. These risk factors were consistent with other research on the characteristics of South African women having children with a diagnosis of Fetal Alcohol Spectrum Disorders and/or of non pregnant women at high risk for an alcohol-exposed pregnancy. These findings highlight the need for women of child-bearing age to be routinely screened for alcohol use and its associated risk factors. Intervention efforts could be integrated into health initiatives already present in South Africa including the prevention and treatment of HIV/AIDS, tuberculosis, and malnutrition. Preconception care is particularly important since pregnancy recognition often occurs several weeks to months following conception and could be implemented by South African community health workers. These endeavors should facilitate national goals of healthier pregnancies and the elimination of FASDs in South Africa.

Exploratory surveys were conducted in Europe to locate potential biological control agents of Vincetoxicum nigrum and Vincetoxicum rossicum and to collect known specialist herbivores of Vincetoxicum hirundinaria (Abrostola asclepiadis, Chrysolina a. asclepiadis, Eumolpus asclepiadeus and Euphranta connexa). Development of the most abundant herbivores found attacking leaves, roots, and developing seeds was evaluated in the laboratory on three Vincetoxicum spp. Field surveys revealed that the highest diversity of herbivores was associated with the abundant and widespread V. hirundinaria. No new herbivores were reported from V. hirundinaria or V. nigrum, but the leaf-feeding noctuid Hypena opulenta was recorded for the first time attacking V. rossicum and V. scandens in Ukraine. Based upon larval survival and development and adult fecundity, the leaf feeders A. asclepiadis, C. a. asclepiadis, and H. opulenta, perform better on V. nigrum and V. rossicum than on V. hirundinaria. Larval performance of the root feeder E. asclepiadeus followed a similar pattern, but adult fecundity of this insect did not vary among host plant species. Immature development time of the pre-dispersal seed feeder E. connexa is similar among hosts, but larvae grow larger on V. nigrum and V. hirundinaria than on V. rossicum. All herbivores are promising biological control agents of Vincetoxicum in North America and we have prioritized host range testing with H. opulenta and E. asclepiadeus.

Two major antigenic subgroups (designated A and B) have been described for human respiratory syncytial virus (HRSV). Between and within the two main subgroups, there is antigenic variation in the attachment protein G. The variability of the G protein is known to be located in two hypervariable regions of the ectodomain. Most investigators have studied the gene segment coding the C-terminal end of the protein, and little is known about the N-terminal variable region. In the present study, the genetic variability of HRSV subgroup B was evaluated by nucleotide sequencing of the N-terminal region of the G gene of 52 Tunisian isolates. Tunisian subgroup B isolates clustered into two main lineages designated arbitrarily as Tu-GB1 and Tu-GB2. Three distinct subtypes were identified within genotype Tu-GB2. The inter- and intragenotype nucleotide variability ranged from 4 to 8% and from 0 to 4%, respectively. Overall divergence values of the G sequences were inferior or equal to 15% at the aminoacid level. Comparison of sequences among Tunisian HRSV strains and viruses isolated in other geographical areas during different epidemics demonstrated close similarity to strains from Kenya, Belgium, the UK, Qatar, Canada and South Korea.

Routine manual thrombectomy during primary percutaneous coronary intervention might be intuitively justified. While older registers reported contradictory results, interventional cardiologists remained interested in using such devices during the mechanical treatment of acute myocardial infarction. The first studies were congruent to demonstrate a significant relationship between thromboaspiration and significant improvement of ST-segment elevation, lower distal embolization, despite TAPAS was the only to significantly reduce the mortality. Later studies were unable to confirm these promising data, avoiding routine manual thrombectomy prior to primary angioplasty to decrease cardiovascular mortality, recurrent myocardial infarction, cardiogenic shock or severe heart failure. Moreover, thrombectomy was associated with an increased rate of stroke. Should thrombectomy therefore be conclusively overlooked? It is presumably required to define which patient is eligible for thrombectomy, to define how to perfectly perform manual thrombectomy, to specify how to gently move towards the thrombus, the optimal pharmacological environment, the number of aspirations and the criterion to stop or to repeat aspiration. Indeed, while thrombectomy is nowadays scientifically downgraded, it remains of potential interest in numerous interventional cardiologists.

Past studies of the effect of kidney function on intracerebral hemorrhage (ICH) outcomes have yielded inconsistent findings. Although the second, main phase of the Intensive Blood Pressure Reduction in Acute Cerebral Hemorrhage Trial (INTERACT2) suggests the effectiveness of early intensive blood pressure (BP) lowering in improving functional recovery after ICH, the balance of potential benefits and harms of this treatment in those with decreased kidney function remains uncertain. Study Design: Secondary analysis of INTERACT2, which randomly assigned patients with ICH with elevated systolic BP (SBP) to intensive (target SBP 90, 60-90, and Outcomes: The effect of admission eGFR on the primary outcome of death or major disability at 90 days (defined as modified Rankin Scale scores of 3-6) was analyzed using a multivariable logistic regression model. Potential effect modification of intensive BP lowering treatment by admission eGFR was assessed by interaction terms. Results: Of 2,623 included participants, 912 (35%) and 280 (11%) had mildly and moderately/severely decreased eGFRs, respectively. Patients with moderately/severely decreased eGFRs had the greatest risk for death or major disability at 90 days (adjusted OR, 1.82; 95% CI, 1.28-2.61). Effects of early intensive BP lowering were consistent across different eGFRs (P = 0.5 for homogeneity). Limitations: Generalizability issues arising from a clinical trial population. Conclusions: Decreased eGFR predicts poor outcome in acute ICH. Early intensive BP lowering provides similar treatment effects in patients with ICH with decreased eGFRs. Objective The aim of this study was to evaluate the effect of hydroxychloroquine (HCQ) on fetal preterm delivery and intrauterine growth restriction (IUGR) in a cohort of pregnant women with systemic lupus erythematosus (SLE). Methods Over an 11-year period (January 1, 2001 to December 31, 2011), all women with SLE and admitted to deliver after 22 weeks of gestation to Bordeaux University Hospital (France), were retrospectively enrolled in the present study. The population was then split into two groups based on the treatment they received: HCQ exposed (HCQ+) versus HCQ non-exposed (HCQ−) group. Results 118 pregnancies were included, 41 in the HCQ+ group and 77 in the HCQ− group. The rate of adverse fetal outcome was significantly lower in the HCQ+ group ( p = 0.001), particularly in terms of preterm delivery, 15.8 % versus 44.2 % ( p = 0.006), and IUGR, 10.5 % versus 28.6 % ( p = 0.03). No adverse outcomes were reported in the HCQ+ group. Conclusion HCQ reduces neonatal morbidity in women with SLE by significantly decreasing the rate of prematurity and intrauterine growth restriction.

South Africa has the highest prevalence of Fetal Alcohol Spectrum Disorders (FASD) in the world yet many women have no access to clinic care or to physicians in their communities. The shortage of physicians trained in the diagnosis of FASD is even more severe. Thus there is a need to train community workers to assist in the delivery of health care.This study reports on the effectiveness of training community workers to screen for a possible diagnosis of a FASD.Community workers in Cape Town, South Africa were trained to screen for FASD in 139, 18-month-old toddlers with prenatal alcohol exposure (PAE). Children were assessed according to the salient characteristics of individuals with PAE using height, weight, head circumference (OFC), philtrum, and lip measurements according to criteria set forth by the Institute of Medicine. Screen-positive children were referred for diagnostic assessment to a pediatrician reliably trained in the diagnosis of FASD.Of the screen-positive children, 93% received an FASD diagnosis suggesting that the screening procedure was highly sensitive. Diagnoses included 15% with fetal alcohol syndrome (FAS), 23% with Partial FAS, and 62% with Alcohol Related Neurodevelopmental Disorder (ARND, provisional).The use of community workers to screen for FASD represents a promising approach to effective diagnosis of children affected by PAE in areas lacking adequate medical resources.

Objective: To assess the efficacy and safety of the dopamine agonist cabergoline in the treatment of patients with idiopathic restless legs syndrome (CATOR study). Methods: Patients with moderate to severe restless legs syndrome (RLS) were randomly assigned to cabergoline (single evening dose: 2 mg) or placebo and treated for 5 weeks in a double-blind, multicenter polysomnography (PSG) trial. The primary efficacy measures were the periodic leg movements during sleep arousal index (PLMS-AI) and sleep efficiency. These and further PSG variables were monitored by centrally evaluated PSG. Severity of RLS was assessed using the International RLS Study Group Severity Scale (IRLS), the RLS-6 scales, the Sleep Questionnaire Form A (SF-A; quality of sleep), and the Quality of Life for RLS questionnaire. Results: Forty-three patients were treated and 40 patients were evaluated with PSG (age 56 ± 10 years, 73% women). Cabergoline was superior to placebo in terms of the PLMS-AI (−17.7 ± 16.4 vs −4.5 ± 20.0 placebo; p = 0.0024), sleep efficiency (+6.2 ± 13.9% vs +3.3 ± 11.7%; p = 0.0443), PLMS index ( p = 0.0014), PLM index ( p = 0.0012), and total sleep time ( p = 0.0443). Improvements in IRLS total score (−23.7 ± 11.2 vs −7.9 ± 11.0 placebo; p = 0.0002), RLS-6 severity scales during the night ( p = 0.0010) and during the day ( p = 0.0018), Clinical Global Impressions severity item ( p = 0.0003), sleep quality ( p = 0.0180), SF-A sleep quality ( p = 0.0371), and QoL-RLS ( p = 0.0247) were larger in patients treated with cabergoline compared with the placebo group. Adverse events were only mild and well-known side effects of dopamine agonists. Conclusion: Single-evening cabergoline is an efficacious and well-tolerated short-term therapy for sensorimotor symptoms of restless legs syndrome and associated sleep disturbances.

I study, by Monte Carlo simulations, the features of a model of catalytic reaction in which the dynamics of the slowest step of the reaction process is specified by a time random walk (TRW). This TRW aims at reproducing the actual dynamics of an adsorbed atom escaping out of a potential well.

Local bureaucracy is the administrative apparatus of local political jurisdictions that exists for the primary purposes of carrying out state and federal policy objectives and for planning and providing local public services. The goals of local bureaucratic actors, and thus the organizational behavior of local bureaucracies, are often driven by considerations of improving efficiency, effectiveness, and responsiveness in local service delivery. In principal, these goals are held to be equally important by professional public managers, yet the complex and diverse demands of the local electorate combined with the increasingly networked context of local bureaucracy dictate that public managers will confront tradeoffs in the pursuit of these objectives. This chapter examines evidence and debates related to the three key themes of efficiency, responsiveness, and effectiveness in the local bureaucracy literature. In summarizing the state of knowledge in local bureaucracy research related to these three themes, this chapter highlights a number of unanswered questions and promising theoretical developments that might serve to guide future research.

OBJECTIVE: To evaluate the clinical performance of the Glucometer Elite XL Diabetes Care System in neonatal settings using a multicenter study RESEARCH DESIGN AND METHODS: A total of 388 blood specimens from 333 neonates were included in the study. A capillary or arterial sample was analyzed for determination of glucose with the Glucometer Elite XL system by an attending trained nurse. Through the same sampling site, a specimen was collected and sent to the laboratory for measurement of plasma glucose, bilirubin, and hematocrit. RESULTS: The regression analysis between the results of the Glucometer Elite XL system and comparative methods resulted in the following: Glucometer Elite XL meter = 1.01 x laboratory method + 0.02 mmol/l (n = 388). For the 1.1-4.0 mmol/l plasma glucose range, the regression was Glucometer Elite XL meter = 1.07 x laboratory method + 0.12 mmol/l (n = 150). A difference plot indicated a mean bias of 0.04 mmol/l (95% CI -0.01 to 0.10). No relationship was found between meter glucose biases and hematocrit levels (r = 0.10, P = 0.14). Although a statistically significant correlation existed between bilirubin levels and the glucose meter biases (r = 0.14, P = 0.005), the predicted mean biases were of little clinical significance. CONCLUSIONS: The Glucometer Elite XL system showed a good performance when used in neonatal settings.

We describe an implemented system which reads amounts on French checks images. This system is made of two modules which recognize independently the courtesy and the legal amount. The first one is based on a segmentation-by-recognition approach while the second uses hidden Markov models of words. The outputs of these two modules are combined into a system decision. A high reliability is achieved through this combination since the errors made by the recognition modules should be mostly uncorrelated.
