2. Leisure time physical activity is associated with improved HDL functionality in high cardiovascular risk individuals: a cohort study

Abstract

© The European Society of Cardiology 2020. Aims: Physical activity has consistently been shown to improve cardiovascular health and high-density lipoprotein-cholesterol levels. However, only small and heterogeneous studies have investigated the effect of exercise on high-density lipoprotein functions. Our aim is to evaluate, in the largest observational study to date, the association between leisure time physical activity and a range of high-density lipoprotein functional traits. Methods: The study sample consisted of 296 Spanish adults at high cardiovascular risk. Usual leisure time physical activity and eight measures of high-density lipoprotein functionality were averaged over two measurements, one year apart. Multivariable linear regression models were used to explore the association between leisure time physical activity (exposure) and each high-density lipoprotein functional trait (outcome), adjusted for cardiovascular risk factors. Results: Higher levels of leisure time physical activity were positively and linearly associated with average levels over one year of plasma high-density lipoprotein-cholesterol and apolipoprotein A-I, paraoxonase-1 antioxidant activity, high-density lipoprotein capacity to esterify cholesterol and cholesterol efflux capacity in individuals free of type 2 diabetes only. The increased cholesterol esterification index with increasing leisure time physical activity reached a plateau at around 300 metabolic equivalents.min/day. In individuals with diabetes, the relationship with cholesteryl ester transfer protein followed a U-shape, with a decreased cholesteryl ester transfer protein activity from 0 to 300 metabolic equivalents.min/day, but increasing from there onwards. Our aim is to assess whether following a Mediterranean Diet (MedDiet) decreases the risk of initiating antithrombotic therapies and the cardiovascular risk associated with its use in older individuals at high cardiovascular risk. We evaluate whether participants of the PREvención con DIeta MEDiterránea (PREDIMED) study allocated to a MedDiet enriched in extra-virgin olive oil or nuts (versus a low-fat control intervention) disclose differences in the risk of initiation of: (1) vitamin K epoxide reductase inhibitors (acenocumarol/warfarin; n = 6772); (2) acetylsalicylic acid as antiplatelet agent (n = 5662); and (3) other antiplatelet drugs (cilostazol/clopidogrel/dipyridamole/ditazol/ticlopidine/triflusal; n = 6768). We also assess whether MedDiet modifies the association between the antithrombotic drug baseline use and incident cardiovascular events. The MedDiet intervention enriched with extra-virgin olive oil decreased the risk of initiating the use of vitamin K epoxide reductase inhibitors relative to control diet (HR: 0.68 [0.46–0.998]). Their use was also more strongly associated with an increased risk of cardiovascular disease in participants not allocated to MedDiet interventions (HRcontrol diet: 4.22 [1.92–9.30], HRMedDiets: 1.71 [0.83–3.52], p-interaction = 0.052). In conclusion, in an older population at high cardiovascular risk, following a MedDiet decreases the initiation of antithrombotic therapies and the risk of suffering major cardiovascular events among users of vitamin K epoxide reductase inhibitors.

Abstract

Objective: To characterize meal patterns across ten European countries participating in the European Prospective Investigation into Cancer and Nutrition (EPIC) calibration study. Design: Cross-sectional study utilizing dietary data collected through a standardized 24 h diet recall during 1995-2000. Eleven predefined intake occasions across a 24 h period were assessed during the interview. In the present descriptive report, meal patterns were analysed in terms of daily number of intake occasions, the proportion reporting each intake occasion and the energy contributions from each intake occasion. Setting: Twenty-seven centres across ten European countries. Subjects: Women (64 %) and men (36 %) aged 35-74 years (n 36 020). Results: Pronounced differences in meal patterns emerged both across centres within the same country and across different countries, with a trend for fewer intake occasions per day in Mediterranean countries compared with central and northern Europe. Differences were also found for daily energy intake provided by lunch, with 38-43% for women and 41-45% for men within Mediterranean countries compared with 16-27% for women and 20-26% for men in central and northern European countries. Likewise, a south-north gradient was found for daily energy intake from snacks, with 13-20% (women) and 10-17% (men) in Mediterranean countries compared with 24-34% (women) and 23-35% (men) in central/northern Europe. Conclusions: We found distinct differences in meal patterns with marked diversity for intake frequency and lunch and snack consumption between Mediterranean and central/northern European countries. Monitoring of meal patterns across various cultures and populations could provide critical context to the research efforts to characterize relationships between dietary intake and health.

L'objectif de cette etude est de decrire l'association entre le statut ponderal au moment de l'initiation de la dialyse (ou la variation d'indice de masse corporelle, IMC) et l'acces a la transplantation renale chez des patients en IRT. L'objectif secondaire et d'analyser l'association entre l'IMC et la mortalite. Materiels et Methodes 19 524 patients en IRT et dialyses en France issus du Reseau Epidemiologie et Information en Nephrologie (REIN) ont ete inclus entre 2002 and 2011. L'IMC etait collecte a l'inclusion et tous les ans lors du suivi. Resultats Les moyennes±SD de l'âge et de l'IMC etaient de 54,9±11,9 ans et 26,2±6,1kg/m 2 , respectivement. La mediane de suivi etait de 20,3 mois durant laquelle 6 634 patients ont ete transplantes. 63 % des patients etaient des hommes, 23 % avaient debute la dialyse avec un IMC≥30kg/m 2 ; enfin, 1/3 etaient diabetiques. Les patients avec les IMC les plus eleves etaient plus âges, diabetiques, avaient une insuffisance cardiaque congestive et etaient hemodialyses ( vs dialyse peritoneale). Un IMC>32kg/m 2 a l'initiation de la dialyse etait associe a une moindre chance d'etre transplante, avec un risque croissant avec l'elevation de l'IMC au-dessus de ce seuil. Cher les sujets obeses, pour chaque elevation d'1kg/m 2 durant le suivi, les chances d'acces a la greffe renale etaient diminuees de 8 a 10 %. Pour un IMC entre 23-38kg/m 2 la mortalite etait diminuee. Si l'IMC etait>38kg/m 2 , la mortalite augmentait mais seulement de l'ordre d'1 % pour 1kg/m 2 . Conclusions Les patients obeses en IRT et dialyses ont une moindre chance d'acces a la greffe renale en France, en comparaison a ceux de corpulence standard.
