1. Primary and Community Care Transformation in Post-COVID Era: Nationwide General Practitioner Survey

Abstract

Introduction: The health emergency caused by COVID-19 has led to substantial changes in the usual working system of primary healthcare centers and in relations with users. The Catalan Society of Family and Community Medicine designed a survey that aimed to collect the opinions and facilitate the participation of its partners on what the future work model of general practitioners (GPs) should look like post-COVID-19. Methodology: Online survey of Family and Community Medicine members consisting of filiation data, 22 Likert-type multiple-choice questions grouped in five thematic axes, and a free text question. Results: The number of respondents to the questionnaire was 1051 (22.6% of all members): 83.2% said they spent excessive time on bureaucratic tasks; 91.8% were against call center systems; 66% believed that home care is the responsibility of every family doctor; 77.5% supported continuity of care as a fundamental value of patient-centered care; and >90% defended the contracting of complementary tests and first hospital visits from primary healthcare (PHC). Conclusions: The survey responses describe a strong consensus on the identity and competencies of the GP and on the needs of and the threats to the PHC system. The demand for an increase in health resources, greater professional leadership, elimination of bureaucracy, an increase in the number of health professionals, and greater management autonomy, are the axes towards which a new era in PHC should be directed.

Abstract

A novel instrumented baffle surrounding the suspended end mirror in the input mode cleaner cavity of the Virgo interferometer was installed in spring 2021. Since then, the device has been regularly operated in the experiment and the obtained results indicate a good agreement with simulations of the stray light inside the optical cavity. The baffle will operate in the upcoming O4 observation run, serving as a demonstrator of the technology designed to instrument the baffles in front of the main mirrors in time for O5. In this paper we present a detailed description of the baffle design, including mechanics, front-end electronics, data acquisition, as well as optical and vacuum tests, calibration and installation procedures, and performance results., Comment: 12 pages, 21 figures, 3 tables, to be submitted to PRD

Abstract

Abstract Background Brassicas (Brassicaceae) are recognized as excellent sources of nutrients and bioactive compounds. Among these, wild turnip (Brassica rapa L.), holds significant promising nutritional properties owed to its abundant glucosinolates and phenolic compounds. To enhance its potential values, the application of elicitors is crucial and good strategy prompting an enrichment in the concentration of phytochemicals, as well established in other relevant Brassicas, such as broccoli. While the responses triggered by certain elicitors such as salicylic acid, methyl jasmonate, or chitosan are widely documented, little is known about the impact of electrolyzed water, an economically viable elicitor. Through elicitation strategies, the aim of this work was to unravel insights into enhancing the phytochemical content of wild turnip sprouts for potential use as healthy food, comparing with well-studied broccoli as control of the experiments. Results Our findings revealed that wild turnip exhibited a notable higher glucosinolate (GSL) contents (487–712 mg 100 g−1 D.W.), than in broccoli sprouts. Furthermore, the use of electrolyzed water (2 vol.) boosted the accumulation of glucosinolates with significant increase up to twofolds the content. Specifically, treatments with salicylic acid (250 μM) and electrolyzed water (2 vol.) favored the significant increase of mainly aliphatic GSL (progoitrin, PRO; gluconapin, GNA; glucobrassicin, GBN). On the other hand, natural antioxidants such as of the characteristic acylated cyanidins present in wild turnip sprouts were not affected by the elicitor treatments, indicative of higher tolerance to oxidative stress in wild turnip. Conclusions These observations underlined the potential of using electrolyzed water in wild turnips as elicitor for GSL-enriched food ingredients. Further studies will be necessary to align with the broader goal of evaluating abiotic and biotic factors affecting the phytochemical composition in mature organs not only in germinating seeds and sprouts, for agricultural performance for quality and healthy foods purposes. Abstract

A new instrumented baffle was installed in Spring 2021 at Virgo surrounding the suspended mirror in the input mode cleaner triangular cavity. It serves as a demonstrator of the technology designed to instrument the baffles in the main arms in the near future. We present, for the first time, results on the measured scattered light distribution inside the cavity as determined by the new device using data collected between May and July 2021, with Virgo in commissioning phase and operating with an input laser power in the cavity of 28.5~W. Abstract

Physical review / D 103(5), 054504 (1-48) (2021). doi:10.1103/PhysRevD.103.054504, Systems with the quantum numbers of up to 12 charged and neutral pseudoscalar mesons, as well as one-, two-, and three-nucleon systems, are studied using dynamical lattice quantum chromodynamics and quantum electrodynamics (QCD+QED) calculations and effective field theory. QED effects on hadronic interactions are determined by comparing systems of charged and neutral hadrons after tuning the quark masses to remove strong isospin breaking effects. A nonrelativistic effective field theory, which perturbatively includes finite-volume Coulomb effects, is analyzed for systems of multiple charged hadrons and found to accurately reproduce the lattice QCD+QED results. QED effects on charged multihadron systems beyond Coulomb photon exchange are determined by comparing the two- and three-body interaction parameters extracted from the lattice QCD+QED results for charged and neutral multihadron systems., Published by Inst., Woodbury, NY

Abstract

The PAU (Physics of the Accelerating Universe) Survey goal is to obtain photometric redshifts (photo-z) and Spectral Energy Distribution (SED) of astronomical objects with a resolution roughly one order of magnitude better than current broad band photometric surveys. To accomplish this, a new large field of view camera (PAUCam) has been designed, built, commissioned and is now operated at the William Herschel Telescope (WHT). With the current WHT Prime Focus corrector, the camera covers ~1-degree diameter Field of View (FoV), of which, only the inner ~40 arcmin diameter are unvignetted. The focal plane consists of a mosaic of 18 2k$x4k Hamamatsu fully depleted CCDs, with high quantum efficiency up to 1 micrometers in wavelength. To maximize the detector coverage within the FoV, filters are placed in front of the CCDs inside the camera cryostat (made out of carbon fiber) using a challenging movable tray system. The camera uses a set of 40 narrow band filters ranging from ~4500 to ~8500 Angstroms complemented with six standard broad-band filters, ugrizY. The PAU Survey aims to cover roughly 100 square degrees over fields with existing deep photometry and galaxy shapes to obtain accurate photometric redshifts for galaxies down to i_AB~22.5, detecting also galaxies down to i_AB~24 with less precision in redshift. With this data set we will be able to measure intrinsic alignments, galaxy clustering and perform galaxy evolution studies in a new range of densities and redshifts. Abstract

The Dark Energy Spectroscopic Instrument (DESI) is under construction to measure the expansion history of the universe using the baryon acoustic oscillations technique. The spectra of 35 million galaxies and quasars over 14,000 square degrees will be measured during a 5-year survey. A new prime focus corrector for the Mayall telescope at Kitt Peak National Observatory will deliver light to 5,000 individually targeted fiber-fed robotic positioners. The fibers in turn feed ten broadband multi-object spectrographs. We describe the ProtoDESI experiment, that was installed and commissioned on the 4-m Mayall telescope from August 14 to September 30, 2016. ProtoDESI was an on-sky technology demonstration with the goal to reduce technical risks associated with aligning optical fibers with targets using robotic fiber positioners and maintaining the stability required to operate DESI. The ProtoDESI prime focus instrument, consisting of three fiber positioners, illuminated fiducials, and a guide camera, was installed behind the existing Mosaic corrector on the Mayall telescope. A Fiber View Camera was mounted in the Cassegrain cage of the telescope and provided feedback metrology for positioning the fibers. ProtoDESI also provided a platform for early integration of hardware with the DESI Instrument Control System that controls the subsystems, provides communication with the Telescope Control System, and collects instrument telemetry data. Lacking a spectrograph, ProtoDESI monitored the output of the fibers using a Fiber Photometry Camera mounted on the prime focus instrument. Abstract

The PAUCam is an optical camera with a wide field of view of 1 deg x 1 deg and up to 46 narrow and broad band filters. The camera is already installed on the William Herschel Telescope (WHT) in the Canary Islands, Spain and successfully commissioned during the first period of 2015. The paper presents the main results from the readout electronics commissioning tests and include an overview of the whole readout electronics system, its configuration and current performance.

Cannabis use and age at onset of psychosis: further evidence of interaction with COMT Val158Met polymorphism. Objective: To examine, in a sample of young psychiatric patients, (n = 157, mean age 17.01 years (SD = 3.6)) whether i) age at first cannabis use and age at emergence of psychiatric disorders are related and ii) such a relationship is modulated by the Val158Met polymorphism in the COMT gene. Method: Cannabis use profiles and COMT Val158Met genotypes were obtained from 80 inpatients with schizophrenia-spectrum disorders and 77 inpatients with other non-psychotic disorders. Results: First, age at first cannabis use correlates with age at onset in both schizophrenia-spectrum and other psychiatric disorder groups: those who started using cannabis earlier had an earlier age at onset of psychiatric disorders. Second, the distribution of the Val158Met genotypes was not different either between diagnosis groups or between cannabis users and non-users. Third, an interaction between Val158Met genotypes and cannabis use was observed specifically on age at emergence of psychotic disorders, with Val/Val genotype carriers showing an earlier age at onset than Met carriers. Conclusion: Our results suggest the importance of brain maturation timing in which exposure to cannabis occurs. The COMT Val158Met genotype seems to modulate the association between cannabis and age at onset of psychotic disorders. These results are consistent with previous studies.

Abstract

The MAGIC telescopes are two Imaging Atmospheric Cherenkov Telescopes (IACTs) located on the Canary island of La Palma. The telescopes are designed to measure Cherenkov light from air showers initiated by gamma rays in the energy regime from around 50 GeV to more than 50 TeV. The two telescopes were built in 2004 and 2009, respectively, with different cameras, triggers and readout systems. In the years 2011-2012 the MAGIC collaboration undertook a major upgrade to make the stereoscopic system uniform, improving its overall performance and easing its maintenance. In particular, the camera, the receivers and the trigger of the first telescope were replaced and the readout of the two telescopes was upgraded. This paper (Part I) describes the details of the upgrade as well as the basic performance parameters of MAGIC such as raw data treatment, dead time of the system, linearity in the electronic chain and sources of noise. Abstract

The Cherenkov Telescope Array (CTA) is a new observatory for very high-energy (VHE) gamma rays. CTA has ambitions science goals, for which it is necessary to achieve full-sky coverage, to improve the sensitivity by about an order of magnitude, to span about four decades of energy, from a few tens of GeV to above 100 TeV with enhanced angular and energy resolutions over existing VHE gamma-ray observatories. An international collaboration has formed with more than 1000 members from 27 countries in Europe, Asia, Africa and North and South America. In 2010 the CTA Consortium completed a Design Study and started a three-year Preparatory Phase which leads to production readiness of CTA in 2014. Abstract

The Physics of Accelerating Universe Camera (PAUCam) is a new camera for dark energy studies that will be installed in the William Herschel telescope. The main characteristic of the camera is the capacity for high precision photometric redshift measurement. The camera is composed of eighteen Hamamatsu Photonics CCDs providing a wide field of view covering a diameter of one degree. Unlike the common five optical filters of other similar surveys, PAUCam has forty optical narrow band filters which will provide higher resolution in photometric redshifts. In this paper a general description of the electronics of the camera and its status is presented.

Abstract

The PAUCam is an optical camera with an array of 18 CCDs (Hamamatsu Photonics K.K.) and up to 45 narrow and broad band filters. The camera will be installed on the William Herschel Telescope (WHT) in the Canary Islands, Spain. In order to fulfill with the specifications for the camera readout system, it was necessary to test the different readout electronics subsystems individually before to integrate the final readout work package, which is composed of 4 MONSOON (NOAO) front-ends, 6 fan out boards (MIX), each one driving up to 5 CCDs signals and a pre-amplification stage (PREAMP) located inside the cryostat. To get the subsystems integration, it was built a small camera prototype using the same technology as used in the main camera: a carbon fiber cryostat refrigerated by a cryotiger cooling system but with capacity to allocate just 2 CCDs, which were readout and re-characterized to measure the electronics performance as conversion factor or gain, readout noise, stability, linearity, etc. while the cross-talk was measured by using a spot-light. The aim of this paper is to review the whole process of assembly, integration and test (AIT) of the readout electronics work package and present the main results to demonstrate the viability of the proposed systems to be use with the PAUCam camera.

Abstract

To examine, in a sample of young psychiatric patients, (n = 157, mean age 17.01 years (SD = 3.6)) whether i) age at first cannabis use and age at emergence of psychiatric disorders are related and ii) such a relationship is modulated by the Val158Met polymorphism in the COMT gene.Cannabis use profiles and COMT Val158Met genotypes were obtained from 80 inpatients with schizophrenia-spectrum disorders and 77 inpatients with other non-psychotic disorders.First, age at first cannabis use correlates with age at onset in both schizophrenia-spectrum and other psychiatric disorder groups: those who started using cannabis earlier had an earlier age at onset of psychiatric disorders. Second, the distribution of the Val158Met genotypes was not different either between diagnosis groups or between cannabis users and non-users. Third, an interaction between Val158Met genotypes and cannabis use was observed specifically on age at emergence of psychotic disorders, with Val/Val genotype carriers showing an earlier age at onset than Met carriers.Our results suggest the importance of brain maturation timing in which exposure to cannabis occurs. The COMT Val158Met genotype seems to modulate the association between cannabis and age at onset of psychotic disorders. These results are consistent with previous studies.

Abstract

Abstract Aims Mobile Airways Sentinel NetworK (MASK) belongs to the Fondation Partenariale MACVIA-LR of Montpellier, France and aims to provide an active and healthy life to rhinitis sufferers and to those with asthma multimorbidity across the life cycle, whatever their gender or socio-economic status, in order to reduce health and social inequities incurred by the disease and to improve the digital transformation of health and care. The ultimate goal is to change the management strategy in chronic diseases. Methods MASK implements ICT technologies for individualized and predictive medicine to develop novel care pathways by a multi-disciplinary group centred around the patients. Stakeholders Include patients, health care professionals (pharmacists and physicians), authorities, patient’s associations, private and public sectors. Results MASK is deployed in 23 countries and 17 languages. 26,000 users have registered. EU grants (2018) MASK is participating in EU projects (POLLAR: impact of air POLLution in Asthma and Rhinitis, EIT Health, DigitalHealthEurope, Euriphi and Vigour). Lessons learnt (i) Adherence to treatment is the major problem of allergic disease, (ii) Self-management strategies should be considerably expanded (behavioural), (iii) Change management is essential in allergic diseases, (iv) Education strategies should be reconsidered using a patient-centred approach and (v) Lessons learnt for allergic diseases can be expanded to chronic diseases.

Abstract

Resumen: Introducción: La información existente sobre el impacto de la gripe en la población infantil española es escasa. El presente trabajo pretende aumentar este conocimiento estudiando aspectos clave como la incidencia de hospitalización, clínica, comorbilidades y el estado vacunal en los niños hospitalizados. Métodos: Estudio retrospectivo, observacional, por revisión de historias clínicas, en menores de 15 años hospitalizados por gripe adquirida en la comunidad, confirmada microbiológicamente, durante 2 temporadas gripales (2014-2015 y 2015-2016). El estudio se realizó en 10 hospitales de 6 ciudades, que atienden aproximadamente al 12% de la población infantil española. Resultados: Fueron hospitalizados 907 niños con diagnóstico principal de gripe (447 50%) de los casos. Una gran parte de las formas graves de gripe en población infantil podrían ser evitada si se cumplieran las indicaciones actuales de vacunación. Abstract

Abstract mHealth, such as apps running on consumer smart devices is becoming increasingly popular and has the potential to profoundly affect healthcare and health outcomes. However, it may be disruptive and results achieved are not always reaching the goals. Allergic Rhinitis and its Impact on Asthma (ARIA) has evolved from a guideline using the best evidence-based approach to care pathways suited to real-life using mobile technology in allergic rhinitis (AR) and asthma multimorbidity. Patients largely use over-the-counter medications dispensed in pharmacies. Shared decision making centered around the patient and based on self-management should be the norm. Mobile Airways Sentinel networK (MASK), the Phase 3 ARIA initiative, is based on the freely available MASK app (the Allergy Diary, Android and iOS platforms). MASK is available in 16 languages and deployed in 23 countries. The present paper provides an overview of the methods used in MASK and the key results obtained to date. These include a novel phenotypic characterization of the patients, confirmation of the impact of allergic rhinitis on work productivity and treatment patterns in real life. Most patients appear to self-medicate, are often non-adherent and do not follow guidelines. Moreover, the Allergy Diary is able to distinguish between AR medications. The potential usefulness of MASK will be further explored by POLLAR (Impact of Air Pollution on Asthma and Rhinitis), a new Horizon 2020 project using the Allergy Diary.

Abstract

Abstract A Good Practice is a practice that works well, produces good results, and is recommended as a model. MACVIA-ARIA Sentinel Network (MASK), the new Allergic Rhinitis and its Impact on Asthma (ARIA) initiative, is an example of a Good Practice focusing on the implementation of multi-sectoral care pathways using emerging technologies with real life data in rhinitis and asthma multi-morbidity. The European Union Joint Action on Chronic Diseases and Promoting Healthy Ageing across the Life Cycle (JA-CHRODIS) has developed a checklist of 28 items for the evaluation of Good Practices. SUNFRAIL (Reference Sites Network for Prevention and Care of Frailty and Chronic Conditions in community dwelling persons of EU Countries), a European Union project, assessed whether MASK is in line with the 28 items of JA-CHRODIS. A short summary was proposed for each item and 18 experts, all members of ARIA and SUNFRAIL from 12 countries, assessed the 28 items using a Survey Monkey-based questionnaire. A visual analogue scale (VAS) from 0 (strongly disagree) to 100 (strongly agree) was used. Agreement equal or over 75% was observed for 14 items (50%). MASK is following the JA-CHRODIS recommendations for the evaluation of Good Practices.

Abstract

Ground-based gamma-ray astronomy has had a major breakthrough with the impressive results obtained using systems of imaging atmospheric Cherenkov telescopes. Ground-based gamma-ray astronomy has a huge potential in astrophysics, particle physics and cosmology. CTA is an international initiative to build the next generation instrument, with a factor of 5-10 improvement in sensitivity in the 100 GeV-10 TeV range and the extension to energies well below 100 GeV and above 100 TeV. CTA will consist of two arrays (one in the north, one in the south) for full sky coverage and will be operated as open observatory. The design of CTA is based on currently available technology. Abstract

With the development of the Imaging Atmospheric Cherenkov Technique (IACT), Gamma-ray astronomy has become one of the most interesting and productive fields of astrophysics. Current IACT telescope arrays (MAGIC, H.E.S.S, VERITAS) use photomultiplier tubes (PMTs) to detect the optical/near-UV Cherenkov radiation emitted due to the interaction of gamma rays with the atmosphere. For the next generation of IACT experiments, the possibility of replacing the PMTs with Silicon photomultipliers (SiPMs) is being studied. Among the main drawbacks of SiPMs are their limited active area (leading to an increase in the cost and complexity of the camera readout) and their sensitivity to unwanted wavelengths. Here we propose a novel method to build a relatively low-cost pixel consisting of a SiPM attached to a PMMA disc doped with a wavelength shifter. This pixel collects light over a much larger area than a single standard SiPM and improves sensitivity to near-UV light while simultaneously rejecting background. We describe the design of a detector that could also have applications in other fields where detection area and cost are crucial. Abstract

The Dark Energy Spectroscopic Instrument (DESI) is under construction to measure the expansion history of the universe using the baryon acoustic oscillations technique. The spectra of 35 million galaxies and quasars over 14,000 square degrees will be measured during a 5-year survey. A new prime focus corrector for the Mayall telescope at Kitt Peak National Observatory will deliver light to 5,000 individually targeted fiber-fed robotic positioners. The fibers in turn feed ten broadband multi-object spectrographs. We describe the ProtoDESI experiment, that was installed and commissioned on the 4-m Mayall telescope from 2016 August 14 to September 30. ProtoDESI was an on-sky technology demonstration with the goal to reduce technical risks associated with aligning optical fibers with targets using robotic fiber positioners and maintaining the stability required to operate DESI. The ProtoDESI prime focus instrument, consisting of three fiber positioners, illuminated fiducials, and a guide camera, was installed behind the existing Mosaic corrector on the Mayall telescope. A fiber view camera was mounted in the Cassegrain cage of the telescope and provided feedback metrology for positioning the fibers. ProtoDESI also provided a platform for early integration of hardware with the DESI Instrument Control System that controls the subsystems, provides communication with the Telescope Control System, and collects instrument telemetry data. Lacking a spectrograph, ProtoDESI monitored the output of the fibers using a fiber photometry camera mounted on the prime focus instrument. ProtoDESI was successful in acquiring targets with the robotically positioned fibers and demonstrated that the DESI guiding requirements can be met. [ABSTRACT FROM AUTHOR]

Abstract

Human activities provide wildlife with highly abundant and predictable food subsidies, which can affect population dynamics and have wide-ranging ecological impacts. A key ecological question is how species adapt their foraging behaviour to capitalize on these new feeding opportunities. We investigate habitat use by Audouin's Gulls (Ichthyaetus audouinii) off the Western Mediterranean Sea, an opportunistic seabird that has recently expanded to diverse breeding colonies subjected to varying degrees of human influence. By combining GPS tracking, remote sensing, and GIS, we assessed the resource selection and habitat preferences of gulls from five colonies across their breeding latitudinal range, including interactions with industrial fisheries. Overall, the use of terrestrial habitats was slightly higher (57 % of total positions) compared to the marine environment (42 %), with individuals preferentially feeding on urban and related areas or fishing ports. However, habitat utilization varied among studied colonies, likely in response to contrasting food availability and accessibility of human related food resources on land (e.g., agriculture and livestock areas, landfills or rice fields). At sea, individuals largely distributed over highly productive and persistent marine areas with intense fishing pressure. Individuals also adapted their daily activity patterns to match food availability: gulls preferentially feed on the marine environment during the night, while the use of terrestrial habitats increases during daylight hours. Individuals' daily activity patterns also matched that for the two main fishing gears operating in the area: diurnal trawlers and nocturnal purse-seiners. Our findings offer perspectives on the reliance of opportunistic seabird species on anthropogenic food subsidies and inform on potential implications for the conservation and management of these under changes in fishing policies (EU discard ban). Abstract

Fetal anomalies among young women and adolescents pose major public health concerns. This systematic review aims to elucidate the relationship between young maternal age and the incidence of fetal abnormalities. According to the systematic review and meta-analysis PRISMA protocol, cohort, cross-sectional and case-control studies were scrutinized to include 80,393,450 participants across diverse regions. The meta-analysis utilized Odds Ratios (OR) as the effect measure, adopting a random-effects model. The screening process involved 157 selected and verified manuscripts, which ultimately resulted in the final inclusion of 20 studied in the meta-analysis. The criterion for young maternal age was the age of ≤ 20 years. The meta-analysis revealed a pooled OR of 0.93 (95% CI: 0.82-1.05, p = 0.252), indicating no statistically significant association between young maternal age (≤ 20 years) and fetal anomalies. However, considerable heterogeneity (I² = 96.21%) was noted, prompting the use of a random-effects model to derive the reported results. The meta-analysis did not find statistically significant differences in the occurrence of congenital anomalies in fetuses of younger women than in overall population. Although due to significant heterogeneity of the analyzed studies, and a risk of bias, caution should be exercised when interpreting the results, further investigation may be warranted to understand the relationship between maternal age and risk of fetal anomalies. Nevertheless, the study shows significant differences, which diminish in collective analysis, suggests that factors beyond age may be influential. 38. Third Trimester Stillbirth Associated With Hamartoma of Mature Cardiac Myocytes (HMCM).

Abstract

Fetal primary cardiac tumors (FPCTs) are very rare. The majority of them correspond to cardiac rhabdomyomas, followed by other benign neoplasms or hamartomas. We describe the case of a third trimester female stillborn with an incidental autopsy finding of Hamartoma of Mature Cardiac Myocytes (HMCM) , a rare benign cardiac tumor previously unreported in the fetal or neonatal period. The intrauterine demise occurred at 32 + 6 weeks gestation after an uneventful pregnancy. The fetal autopsy revealed a structurally normal heart with a small subendocardial nodule just below the membranous septum. Microscopically, the nodule was well-demarcated from the surrounding penetrating bundle of the conduction axis and the adjacent left ventricular myocardium and consisted of disorganized mature cardiac myocytes in a haphazard arrangement with patchy mild interstitial fibrosis, consistent with HMCM. 39. Acute fetal cardiovascular adaptation to artificial placenta in sheep model.

Abstract

Objective: To describe the acute cardiovascular adaptation of the fetus after connection to an artificial placenta (AP) in a sheep model, using ultrasound and invasive and non-invasive hemodynamic assessment., Methods: This was an experimental study of 12 fetal sheep that were transferred to an AP system, consisting of a pumpless circuit with umbilical cord connection, at 109-117 days' gestation. The study was designed to collect in-utero and postcannulation measurements in all the animals. The first six consecutive fetuses were fitted with intravascular catheters and perivascular probes to obtain invasive physiological data, including arterial and venous intravascular pressures and perivascular blood flows, with measurements taken in utero and at 5 and 30 min after cannulation. These experiments were designed with a survival goal of 1-3 h. The second set of six fetuses were not fitted with catheters, and experiments were aimed at 3-24 h of survival. Echocardiographic assessment of cardiac anatomy and function, as well as measurements of blood flow and pre- and postmembrane pressures recorded by circuit sensors in the AP system, were available for most of the fetuses. These data were acquired in utero and at 30 and 180 min after cannulation., Results: Compared with in-utero conditions, the pulsatility index at 30 and 180 min after connection to the AP system was reduced in the umbilical artery (median, 1.36 (interquartile range (IQR), 1.06-1.50) vs 0.38 (IQR, 0.31-0.50) vs 0.36 (IQR, 0.29-0.41); P < 0.001 for extreme timepoints) and the ductus venosus (median, 0.50 (IQR, 0.41-0.67) vs 0.29 (IQR, 0.22-0.33) vs 0.36 (IQR, 0.22-0.41); P = 0.011 for extreme timepoints), whereas umbilical venous peak velocity increased (median, 20 cm/s (IQR, 18-22 cm/s) vs 39 cm/s (IQR, 31-43 cm/s) vs 43 cm/s (IQR, 34-54 cm/s); P < 0.001 for extreme timepoints) and flow became more pulsatile. Intravascular monitoring showed that arterial and venous pressures increased transiently after connection, with median values for mean arterial pressure at baseline, 5 min and 30 min of 43 mmHg (IQR, 35-54 mmHg), 72 mmHg (IQR, 61-77 mmHg) and 58 mmHg (IQR, 50-64 mmHg), respectively (P = 0.02 for baseline vs 5 min). Echocardiography showed a similar transient elevation of fetal heart rate at 30 and 180 min after connection compared with in utero (median, 145 bpm (IQR, 142-156 bpm) vs 188 bpm (IQR, 171-209 bpm) vs 175 bpm (IQR, 165-190 bpm); P = 0.001 for extreme timepoints). Fetal cardiac structure and function were mainly preserved; median values for right fractional area change were 36% (IQR, 34-41%) in utero, 38% (IQR, 30-40%) at 30 min and 37% (IQR, 33-40%) at 180 min (P = 0.807 for extreme timepoints)., Conclusions: Connection to an AP system resulted in a transient fetal hemodynamic response that tended to normalize over hours. In this short-term evaluation, cardiac structure and function were preserved. Abstract

The time evolution of multi-neutrino entanglement and correlations are studied in two-flavor collective neutrino oscillations, relevant for dense neutrino environments, building upon previous works. Specifically, simulations performed of systems with up to 12 neutrinos using Quantinuum's H1-1 20 qubit trapped-ion quantum computer are used to compute n-tangles, and two- and three-body correlations, probing beyond mean-field descriptions. n-tangle rescalings are found to converge for large system sizes, signaling the presence of genuine multi-neutrino entanglement.

41. Lactoferrin/sialic acid prevents adverse effects of intrauterine growth restriction on neurite length: investigations in an in vitro rabbit neurosphere model.

Abstract

Introduction: Intrauterine growth restriction (IUGR) is a well-known cause of impaired neurodevelopment during life. In this study, we aimed to characterize alterations in neuronal development underlying IUGR and discover strategies to ameliorate adverse neurodevelopment effects by using a recently established rabbit in vitro neurosphere culture., Methods: IUGR was surgically induced in pregnant rabbits by ligation of placental vessels in one uterine horn, while the contralateral horn remained unaffected for normal growth (control). At this time point, rabbits were randomly assigned to receive either no treatment, docosahexaenoic acid (DHA), melatonin (MEL), or lactoferrin (LF) until c-section. Neurospheres consisting of neural progenitor cells were obtained from control and IUGR pup's whole brain and comparatively analyzed for the ability to differentiate into neurons, extend neurite length, and form dendritic branching or pre-synapses. We established for the very first time a protocol to cultivate control and IUGR rabbit neurospheres not only for 5 days but under long-term conditions up to 14 days under differentiation conditions. Additionally, an in vitro evaluation of these therapies was evaluated by exposing neurospheres from non-treated rabbits to DHA, MEL, and SA (sialic acid, which is the major lactoferrin compound) and by assessing the ability to differentiate neurons, extend neurite length, and form dendritic branching or pre-synapses., Results: We revealed that IUGR significantly increased the neurite length after 5 days of cultivation in vitro, a result in good agreement with previous in vivo findings in IUGR rabbits presenting more complex dendritic arborization of neurons in the frontal cortex. MEL, DHA, and SA decreased the IUGR-induced length of primary dendrites in vitro , however, only SA was able to reduce the total neurite length to control level in IUGR neurospheres. After prenatal in vivo administration of SAs parent compound LF with subsequent evaluation in vitro , LF was able to prevent abnormal neurite extension., Discussion: We established for the first time the maintenance of the rabbit neurosphere culture for 14 days under differentiation conditions with increasing complexity of neuronal length and branching up to pre-synaptic formation. 42. An Artificial Placenta Experimental System in Sheep: Critical Issues for Successful Transition and Survival up to One Week.

Abstract

Objective: To describe the development of an artificial placenta (AP) system in sheep with learning curve and main bottlenecks to allow survival up to one week., Methods: A total of 28 fetal sheep were transferred to an AP system at 110-115 days of gestation. The survival goal in the AP system was increased progressively in three consecutive study groups: 1-3 h (n = 8), 4-24 h (n = 10) and 48-168 h (n = 10). Duration of cannulation procedure, technical complications, pH, lactate, extracorporeal circulation (EC) circuit flows, fetal heart rate, and outcomes across experiments were compared., Results: There was a progressive reduction in cannulation complications (75%, 50% and 0%, p = 0.004), improvement in initial pH (7.20 ± 0.06, 7.31 ± 0.04 and 7.33 ± 0.02, p = 0.161), and increment in the rate of experiments reaching survival goal (25%, 70% and 80%, p = 0.045). In the first two groups, cannulation accidents, air bubbles in the extracorporeal circuit, and thrombotic complications were the most common cause of AP system failure., Conclusions: Achieving a reproducible experimental setting for an AP system is extremely challenging, time- and effort-consuming, and requires a highly multidisciplinary team. As a result of the learning curve, we achieved reproducible transition and survival up to 7 days. Extended survival requires improving instrumentation with custom-designed devices.

43. Response of a single European burn center to Centelles mass casualty burn disaster: enzymatic debridement utility.

Abstract

Introduction: Mass burn casualty disasters present with a big challenge due to the complex multidisciplinary management of severely burned patients and the limited capacity of the specialized centers. Literature is scarce, and so is the management of these disasters with enzymatic debridement (ED)., Methods: Retrospective observational analysis of nine patients admitted to the Vall d'Hebron University Hospital Burn Center (Barcelona, Spain), as a consequence of a bell tower explosion on December 30, 2019. The patients with intermediate-deep second-degree burns, either in circumferential or affecting highly functional areas as hands, feet or face, were included in the ED group. Continuous variables are expressed as mode and standard deviation and quantitative ones as percentages., Results: Fourteen people were injured after the explosion of gunpowder-containing bags in a bell tower during a cultural celebration. Nine casualties (6 men and 3 women) suffered burn injuries that required assessment and admission in our Burn Center. The mean age was 44.33 years (range 19-61 years), with burns covering a mean total body surface area (TBSA) of 15% (range 5-48% TBSA). One patient required invasive mechanical ventilation and intensive care management. Seven patients required ED, with an average debrided TBSA of 6.1% (range 3-10% TBSA). Seven out of 9 patients required at least one surgery. The average hospital stay was 23.33 days (range 2-53 days). No escharotomy was required and no patient died., Conclusions: This experience brought out the weak and strong features of our center when facing a situation that implies an important care stress. It can be useful for other Burn Centers in similar situations in the future. Abstract

Following a multi-disciplinary approach integrating information from several experimental models we have collected new evidence supporting, expanding and redesigning the AOP "Disrupted laminin/int-β1 interaction leading to decreased cognitive function". Investigations in vitro in rabbit and rat neurospheres and in vivo in mice exposed to EGCG (epigallocatechin-gallate) during neurodevelopment are combined with in vitro evaluations in neural progenitor cells overexpressing int-β1 and literature information from int-β1 deficiency models. We have discovered for the first time that neural progenitor cells from intrauterine growth restricted (IUGR) animals overexpress int-β1 at gene and protein level and due to this change in prenatal brain programming they respond differently than control neurospheres to the exposure of EGCG, a compound triggering neural progenitor cell migration alterations. We have also identified that EGCG developmental exposure has deleterious effects on neuronal branching and arborization in vitro and in vivo. Abstract

The rabbit model is gaining importance in the field of neurodevelopmental evaluation due to its higher similarity to humans in terms of brain development and maturation than rodents. In this publication, we detailed 14 protocols covering toxicological relevant endpoints for the assessment of neurodevelopmental adverse effects in the rabbit species. These protocols include both in vitro and in vivo techniques, which also cover different evaluation time-points, the neonatal period, and long-term examinations at postnatal days (PNDs) 50-70. Specifically, the protocols (P) included are as follows: neurosphere preparation (GD30/PND0; P2) and neurosphere assay (P3), behavioral ontogeny (PND1; P4), brain obtaining and brain weight measurement at two different ages: PND1 (P5) and PND70 (P12), neurohistopathological evaluations after immersion fixation for neurons, astrocytes, oligodendrocytes and microglia (PND1; P6-9) or perfusion fixation (PND70; P12), motor activity (P11, open field), memory and sensory function (P11, object recognition test), learning (P10, Skinner box), and histological evaluation of plasticity (P13 and P14) through dendritic spines and perineuronal nets. The expected control values and their variabilities are presented together with the information on how to troubleshoot the most common issues related to each protocol. 46. Docosahexaenoic Acid and Melatonin Prevent Impaired Oligodendrogenesis Induced by Intrauterine Growth Restriction (IUGR).

Abstract

In this study, our aims were to characterize oligodendrogenesis alterations in fetuses with intrauterine growth restriction (IUGR) and to find therapeutic strategies to prevent/treat them using a novel rabbit in vitro neurosphere culture. IUGR was surgically induced in one uterine horn of pregnant rabbits, while the contralateral horn served as a control. Neural progenitor cells (NPCs) were obtained from pup's whole brain and cultured as neurospheres mimicking the basic processes of brain development including migration and cell differentiation. Five substances, chosen based on evidence provided in the literature, were screened in vitro in neurospheres from untreated rabbits: Docosahexaenoic acid (DHA), melatonin (MEL), zinc, 3,3',5-Triiodo-L-thyronine (T3), and lactoferrin (LF) or its metabolite sialic acid (SA). DHA, MEL and LF were further selected for in vivo administration and subsequent evaluation in the Neurosphere Assay. In the IUGR culture, we observed a significantly reduced percentage of oligodendrocytes (OLs) which correlated with clinical findings indicating white matter injury in IUGR infants. We identified DHA and MEL as the most effective therapies. In all cases, our in vitro rabbit neurosphere assay predicted the outcome of the in vivo administration of the therapies and confirmed the reliability of the model, making it a powerful and consistent tool to select new neuroprotective therapies.

47. Fetal ischemia monitoring with in vivo implanted electrochemical multiparametric microsensors.

Abstract

Under intrauterine growth restriction (IUGR), abnormal attainment of the nutrients and oxygen by the fetus restricts the normal evolution of the prenatal causing in many cases high morbidity being one of the top-ten causes of neonatal death. The current gold standards in hospitals to detect this relevant problem is the clinical observation by echography, cardiotocography and Doppler. These qualitative techniques are not conclusive and requires risky invasive fetal scalp blood testing and/or amniocentesis. We developed micro-implantable multiparametric electrochemical sensors for measuring ischemia in real time in fetal tissue and vascular. This implantable technology is designed to continuous monitoring for an early detection of ischemia to avoid potential fetal injury. Two miniaturized electrochemical sensors were developed based on oxygen and pH detection. The sensors were optimized in vitro under controlled concentration, to assess the selectivity and sensitivity required. The sensors were then validated in vivo in the ewe fetus model, by means of their insertion in the muscle leg and inside the iliac artery of the fetus. Ischemia was achieved by gradually obstructing the umbilical cord to regulate the amount of blood reaching the fetus. An important challenge in fetal monitoring is the detection of low levels of oxygen and pH changes under ischemic conditions, requiring high sensitivity sensors. Significant differences were observed in both; pH and pO 2 sensors under changes from normoxia to hypoxia states in the fetus tissue and vascular with both sensors. 48. Miniaturized Electrochemical Sensors to Monitor Fetal Hypoxia and Acidosis in a Pregnant Sheep Model.

Abstract

Perinatal asphyxia is a major cause of severe brain damage and death. For its prenatal identification, Doppler ultrasound has been used as a surrogate marker of fetal hypoxia. However, Doppler evaluation cannot be performed continuously. We have evaluated the performance of a miniaturized multiparametric sensor aiming to evaluate tissular oxygen and pH changes continuously in an umbilical cord occlusion (UCO) sheep model. The electrochemical sensors were inserted in fetal hindlimb skeletal muscle and electrochemical signals were recorded. Fetal hemodynamic changes and metabolic status were also monitored during the experiment. Additionally, histological assessment of the tissue surrounding the sensors was performed. Both electrochemical sensors detected the pO 2 and pH changes induced by the UCO and these changes were correlated with hemodynamic parameters as well as with pH and oxygen content in the blood. Finally, histological assessment revealed no signs of alteration on the same day of insertion. This study provides the first evidence showing the application of miniaturized multiparametric electrochemical sensors detecting changes in oxygen and pH in skeletal muscular tissue in a fetal sheep model.

Abstract

Intrauterine growth restriction affects up to 10% of all pregnancies, leading to fetal programming with detrimental consequences for lifelong health. However, no therapeutic strategies have so far been effective to ameliorate these consequences. Our previous study has demonstrated that a single dose of nutrients administered into the amniotic cavity, bypassing the often dysfunctional placenta via intra-amniotic administration, improved survival at birth but not birthweight in an intrauterine growth restriction rabbit model. The aim of this study was to further develop an effective strategy for intra-amniotic fetal therapy in an animal model. Intrauterine growth restriction was induced by selective ligation of uteroplacental vessels on one uterine horn of pregnant rabbits at gestational day 25, and fetuses were delivered by cesarean section on GD30. During the five days of intrauterine growth restriction development, three different methods of intra-amniotic administration were used: continuous intra-amniotic infusion by osmotic pump, multiple intra-amniotic injections, and single fetal intraperitoneal injection. Technical feasibility, capability to systematically reach the fetus, and survival and birthweight of the derived offspring were evaluated for each technique. Continuous intra-amniotic infusion by osmotic pump was not feasible owing to the high occurrence of catheter displacement and amnion rupture, while methods using two intra-amniotic injections and one fetal intraperitoneal injection were technically feasible but compromised fetal survival. Taking into account all the numerous factors affecting intra-amniotic fetal therapy in the intrauterine growth restriction rabbit model, we conclude that an optimal therapeutic strategy with low technical failure and positive fetal impact on both survival and birthweight still needs to be found.

50. Lattice QCD Constraints on the Parton Distribution Functions of ^{3}He.

Abstract

The fraction of the longitudinal momentum of ^{3}He that is carried by the isovector combination of u and d quarks is determined using lattice QCD for the first time. The ratio of this combination to that in the constituent nucleons is found to be consistent with unity at the few-percent level from calculations with quark masses corresponding to m&#95;{π}∼800  MeV. With a naive extrapolation to the physical quark masses, this constraint is consistent with, and more precise than, determinations from global nuclear parton distribution function fits through the nnnpdf framework. It is thus concretely demonstrated that lattice QCD calculations of light nuclei have imminent potential to enable more precise determinations of the u and d parton distributions in light nuclei and to reveal the QCD origins of the EMC effect.
