2. Discrepancy between subjective perception and objective cognitive performance in attention assessment within the winners project for cancer survivors. a case report

Introduction Paediatric cancer survivors have a risk for neuropsychological impairment due to the disease and the treatment received. These affections have been neglected in the follow-up of these patients. It is important to identify the most valid outcomes in the evaluation of neurocognitive sequelae in childhood cancer survivors. Objectives This work aims to compare the results obtained between subjective perception of caregivers and objective cognitive performance based on validated attention tests. Methods In a randomized controlled and unblinded trial to demonstrate the benefit of video games on different neurocognitive areas in cancer survivors, we studied attention functioning before and after the intervention program. The attention deficit subscale from the Behavior Assessment System for Children 3rd edition (BASC-3), self- and parent-reported versions, and the Continuous Performance Test, 3rd edition (CPT 3) will be used as outcomes (z scores: mean = 0, S.D. = 1). Results We observed an improvement in attention after intervention using the CPT-3 (omissions z = 1.2; hit reaction time z = 3.4; hit reaction time block change z = 1.2 versus hit reaction time z = 3.6 without other atipycal z scores after intervention), changing the attentional pattern from “ADHD” to “slowed”. However, in the parent-reported version of the BASC-3, a worsening in the attention subscale is observed (z = 0.3 pre-intervention vs z = 1.0 post-intervention) while the self-reported version of the patient didn’t show any significant changes (z = 1.4 pre-intervention vs z = 1.1 post-intervention). Conclusions It is essential to use objective tests to measure neurocognitive sequelae in these patients. Subjective surveys can provide additional information, but not substitute the above. Introduction Children who have undergone an oncological process and have received treatment with chemotherapy or radiotherapy on the central nervous system may have significant neurocognitive sequelae. Some video games have shown neurocognitive benefits in people with impairments in different areas, such as attention or memory. Objectives This work aims to demonstrate the benefit of a video game-based training program to improve the neurocognitive profile in a child survivor of cancer. Methods The patient is a 9-year-old female who was diagnosed with acute lymphoblastic leukemia at the age of 4 years. She received routine treatment of this disease by chemotherapy, including high-dose chemotherapy (with blood-brain barrier crossing) and intrathecal chemotherapy. She is currently 3 years after the end of treatment. The Continuous Performance Test 3 (CPT-3) (sustained attention/vigilance) was administered before and after a multifaceted training program consisting of playing 3 video games for 12 weeks, as follows: a brain-training game (4 days per week, 7-12 minutes per day), a skill-training game (2 days per week, 10 minutes per day) and an exergaming game (2 days per week, 10 minutes per day). Results Prior to intervention, the patient had 3 atypical z-scores on the CPT-3 (z scores: mean = 0, S.D. = 1), with a pattern compatible with ADHD (omissions z = 1.2; hit reaction time z = 3.4; hit reaction time block change z = 1.2). After intervention, she had only an atypical z-score (hit reaction time z = 3.6), with a pattern compatible with slowing, without ADHD. Conclusions The neuropsychological evaluation of this patient showed an improvement in his attentional pattern on the CPT-3 after the video game-based training. Disclosure of Interest None Declared

Introduction Childhood cancer survivors have neurocognitive sequelae that in most survivor follow-up programs are underdiagnosed and for which there is usually no treatment plan. Video games have demonstrated various psychological and neurocognitive benefits in different subpopulations, such as patients with organic neurological deficits or children with ADHD. However, few studies have been carried out using video games-based interventions in the paediatric oncology population. Objectives The aim of this work is to present the WINNERS study protocol, the objectives of which are to diagnose the neurological and cognitive sequelae in child cancer survivors, and to demonstrate the benefit in these areas of a training program based on video games. Methods A randomized controlled and unblinded trial is presented. Fifty-six patients aged 8 to 17 years stratified into two age groups (8-12 and 13-17) who had received any of the following treatments 1 to 6 years before the enrolment will be selected: high-dose chemotherapy with blood-brain barrier crossing, intrathecal or intraventricular chemotherapy, CNS radiotherapy or hematopoietic stem cell transplantation. A neuropsychological evaluation will be performed consisting of a battery of neuropsychological tests to assess parameters such as attention, memory, visuospatial ability or speed of response, as well as a neuroimaging evaluation by structural and functional magnetic resonance imaging. The evaluation will be repeated 3 months and 6 months after the enrolment. Patients will be randomized to a treatment group or to a recycled waiting group. Intervention will consist on a 12-week training at home using 3 video games: a brain training game, an exergaming game and a skill training game. Results According to the hypotheses of this study, it is expected that the proposed program of videogame-based interventions will improve neurocognitive and other wellbeing parameters in the intervention group. Conclusions This study aims to improve the quality of care for patients who have survived a cancer disease by detecting sequelae that have so far been poorly attended, and by proposing a gamification-based intervention program that is effective and attractive for this population. Introduction Alpha-pyrrolidinohexanophenone (α-PHP) is a synthetic cathinone with uneven distribution throughout the world. Its use is not uniformly regulated and its distribution is legal in some European countries. Easily accessible and available through different websites. Synthetic cathinones inhibit monoamine transporters which include dopamine, norepinephrine, and serotonin, resulting in increased neurotransmitter synaptic concentration. Ways of administration show wide range regarding latency period. Onset and appearance of symptoms as well as their duration and intensity may fluctuate. A decreasing order of latency (oral, inhaled, sublingual and intravenous) has been reported. α-PHP can result in the appearance of psychiatric symptoms, include among others, intoxication with sensory perception disturbances and α-PHP -induced psychotic episodes. Objectives The aim of our study was to assess the epidemiology, clinical and legal features regarding Alpha-pyrrolidinohexanophenone (α-PHP). Methods Review the current bibliography to upgrade the existing knowledge. -Present assorted cases with diverse clinical features. All cases include variability through psychopathological interview, symptoms assessment and treatment response according to rating scales (PANSS, YMRS). -Evaluate different treatment administration ways during acute phase and after hospital discharge. Results Differences were observed after hospitalization in the response using diverse rating scales. We used antipsychotics to treat intoxication with sensory perception disturbances and α-PHP -induced psychotic episodes. α-PHP had a negative impact on the quality of life of the patients. Conclusions α-PHP is a synthetic cathinone with potential risk to mental health and life of users. It is mandatory to implement common legislation all through the European Union to prevent its use and possible implications on population’s mental health. Disclosure No significant relationships.

The methodology presented in this paper produces a circular economy jobs (CE jobs) measure. Using jobs as a proxy indicator, these measure gives cities a robust number to indicate progress toward the circular economy and is designed to serve as a first step in developing a circular economy strategy. The CE jobs measure tracks the inputs and outputs of goods in a city's “boundaries” through the material import dependency of the city's economic sectors. At the same time, tracking and assessing the circularity of the local jobs in these economic sectors will also provide city leaders with an indication of which sectors circularity is happening and could potentially happen. This paper also concludes that the process of coming to the CE jobs has two parts, the first more relevant to the local government and the second better influenced by the national government. Both need to come together for a truly circular local economy to happen.

Visual strength grading is widely used to estimate mechanical properties of structural timber. National visual grades are allocated to strength classes according to European standard EN1912. The recent discussion about the proper function of visual strength grading standards and the assignment of strength classes shows the need for further research in this field. Spanish-sourced radiata, Scots, Salzmann and maritime pine timber samples were visually graded in wet and dry condition according to the Spanish UNE56544 (2011) and German DIN4074-1 (2012) standards. Rejection was far higher dry due to distortion (warping) parameters. However, this rejection could be significantly mitigated by adopting a higher twist limit (2 mm / 25 mm width). UNE 56544 is more suitable for visually grading these species because it was specifically designed for them and a Spanish source. However, both standards underestimated the Scots pine rejection pieces which mechanical properties fulfill the MEG and S10 grades.

This paper presents a geological map and cross-section of the Falcón Basin based both on published and unpublished work and on new data collected in the northern and southern basin margins. The geological map covers an area of 4600 km2 at 1:100,000 scale. The cross-section is oriented NNW-SSE, traversing perpendicular to the main structures. In general, the structure of the study area results from the inversion of a graben (Oligocene-early Miocene back-arc basin), that started in the middle Miocene due to the convergence between the Caribbean and South American plates. The map, the cross-section and the observations made in the field have been used to generate a tectonostratigraphic reconstruction of the Falcón Basin. The Oligocene-early Miocene sedimentary succession mapped and described is relevant to the hydrocarbon exploration in the Caribbean and in the Gulf of Venezuela, where new hydrocarbon resources have recently been discovered (i.e. Perla gas field).

Thin coastal dykes typically found in developing countries may suddenly collapse due to rapid land subsidence, material ageing, sea-level rise, high wave attack, earthquakes, landslides, or a collision with vessels. Such a failure could trigger dam-break tsunami-type flooding, or “dyke-break-induced tsunami”, a possibility which has so far been overlooked in the field of coastal disaster science and management. To analyse the potential consequences of one such flooding event caused by a dyke failure, a hydrodynamic model was constructed based on the authors' field surveys of a vulnerable coastal location in Jakarta, Indonesia. In a 2 m land subsidence scenario – which is expected to take place in the study area after only about 10–20 years – the model results show that the floodwaters rapidly rise to a height of nearly 3 m, resembling the flooding pattern of earthquake-induced tsunamis. The depth–velocity product criterion suggests that many of the narrow pedestrian paths behind the dyke could experience strong flows, which are far greater than the safe limits that would allow pedestrian evacuation. A couple of alternative scenarios were also considered to investigate how such flood impacts could be mitigated by creating a mangrove belt in front of the dyke as an additional safety measure. The dyke-break-induced tsunamis, which in many areas are far more likely than regular earthquake tsunamis, cannot be overlooked and thus should be considered in disaster management and urban planning along the coasts of many developing countries.

10. The effect of wanes on the bending strength of solid timber beams

A theoretical analysis is conducted to determine the effect of wanes on the bearing capacity of timber in light of the reduction they entail in cross-sectional area compared to fully rectangular sections. The concomitant loss of bending strength is likewise evaluated. Parallel tests are run on 84 samples —51 with wanes— of large section, Scotch (Pinus sylvestris L.) and Corsican (Pinus pinaster Ait.) pine beams taken from old buildings. The bending strength, modulus of elasticity and density are found in accordance with European standard EN 408. The mechanical properties of the specimens with and without wanes are found to be very similar, and no statistically significant differences are recorded. The effect of the decline in section may be offset by greater continuity of surface fibers and the section shape effect. One practical consequence of this finding is that (within certain limits) wanes may be disregarded when grading large section timber beams in existing structures, where this flaw is very common; this in turn would reduce the high percentage of rejected beams imposed by the present visual grading standards.

La región frontorbitaria tiene una especial significación en el contexto del envejecimiento facial. La literatura cuenta con numerosa bibliografía que trata sobre este problema. Se considera como una unidad diagnóstico-terapéutica en los últimos tiempos y la evolución de la cirugía hace que se vaya hacia soluciones cada vez más fisiológicas y mínimamente invasivas. Nuestro trabajo queremos que sea un paso más en esa dirección. Sobre una muestra de 25 pacientes en los que se trató la región frontorbitaraia con diferentes procedimientos, y que además mostraban un descenso de la altura de las cejas, se presenta una técnica de suspensión frontorbitaria con mínima incisión, apoyada en una malla de polipropileno anclada en el músculo frontal y galea en el extremo inferior y en el periostio y la galea en su extremo superior en el plano supraperióstico y sin resección cutánea. Se emplea un sistema de medición de resultados basado en puntos del esqueleto facial. Los resultados han sido satisfactorios subjetiva y objetivamente, y comparables a los de otras técnicas más complejas y de mayor coste económico. Las complicaciones desarrolladas han sido leves y pasajeras. Se valora porqué se decidió utilizar la malla de polipropileno, así como porqué se eligió un gramaje concreto. También se analizan las características que aportan el material y su uso en esta técnica como comportamiento ante la infección, tolerancia tisular, retracción cicatricial y adhesividad. Asímismo se justifica el plano de disección y colocación elegido, y se valora el sistema de medición para el control objetivo de los resultados. La aportación de este trabajo consiste en desarrollar una alternativa dentro de las técnicas mínimamente invasivas para le reposición de la región frontorbitaria, sin resección de piel y con buenos resultados en la corrección de las arrugas frontales, comparables a los de otras técnicas de mayor complejidad y elevado coste económico.The fronto-orbital region is of particular significance in the context of facial ageing. There is a considerable amount of research on the nature of this problem. This area has recently come to be considered as a therapeutic- diagnostic unit and surgical developments mean that focus is switching more towards less invasive physiological solutions. Our study aims to take another step in this direction. Taking a sample of 25 patients, all of them undergoing different procedures on the fronto-orbital region and with a lowering of the eyebrows, a suspension technique was introduced with minimum incisions supported by a polypropylene mesh attached to the frontal muscle and galea in the inferior extreme and to the periosteum and galea in the superior extreme in the supra-periosteum plane and without skin removal. A measuring system based on points of the facial skeleton was used. The results were subjectively and objectively satisfactory compared to those from more complicated and expensive techniques. Complications were of a slight and temporary nature. This section carries out an assessment of the reasons for selecting a polypropylene mesh and specific grammage. In addition, the properties of the material are analysed and its use in this context of preventing infection, tissue tolerance, scar reduction and adhesiveness. Justification of the site of dissection and the choice of positioning, and an assessment of the course of medication given in order to produce a set of controlled and objective results, will also be carried out. The aim of this work is to develop an alternative within minimally invasive techniques for the restructuring of the fronto-orbital region without skin removal and with good results in the correction of facial wrinkles compared to other more complicated and expensive techniques.

12. Evaluation of the load carrying capacity of large cross section coniferous timber in standing structures

A total of 85 large section timber pieces (Pinus sylvestris L. and Pinus pinaster Alt.) found in a number of old buildings were visually analyzed and graded pursuant to Spanish standard UNE 56544 and German standard 4074. The object was to formulate a non-destructive method to reliably and effectively determine the mechanical properties of existing timber structures with large cross sections. A new system is proposed based on the chief visual grading parameters and consisting in a single grade; the percentage of rejections with this system is low. In this regard, a specific strength class is established for large cross section members in existing coniferous wood structures, namely F14/E9/D380 (MOR of 14 N/mm2, MOE of 9 kN/mm2 and characteristic density of 380 kg/m3). The use of ultrasonic velocity is proposed to define the next higher strength class - F16/E10/D380, to which timber with velocities of 4,900 m/s or over would be assigned.

Durante el periodo 1978-1994 se han descrito cerca de 2.000 especies animales nuevas para la ciencia en territorio ibérico-balear. Se presenta como apéndice un listado completo de las especies (1978-1993), ordenadas taxonómicamente, así como de sus referencias bibliográficas. Como tendencias generales en este proceso de inventario de la biodiversidad se aprecia un incremento moderado y sostenido en el número de taxones descritos, junto a una cada vez mayor contribución de los autores españoles. Es cada vez mayor el número de especies publicadas en revistas que aparecen en el Science Citation Index, así como el uso del idioma inglés. La mayoría de los phyla, clases u órdenes muestran gran variación en la cantidad de especies descritas cada año, dado el pequeño número absoluto de publicaciones. Los insectos son claramente el colectivo más estudiado, pero se aprecia una disminución en su importancia relativa, asociada al incremento de estudios en grupos poco conocidos como los nematodos.

La cocaína posee un potente efecto simpaticomimético, produciendo vasoconstricción coronaria e infarto miocárdico. Presentamos un inusual caso de insuficiencia mitral postinfarto tras consumo de cocaína inhalada. Material y métodos: Varón de 26 años que acude a urgencias por cuadro de opresión retrosternal y disnea aguda tras inhalación de cocaína que progresa rápidamente hacia edema pulmonar y parada cardiorrespiratoria (PCR), requiriendo 15 min de reanimación cardiopulmonar (RCP) avanzada. El electrocardiograma (ECG) evidenciaba elevación posterolateral del ST, y la ecocardiografía transesofágica (ETE) mostró una insuficiencia mitral grave por rotura parcial del músculo posterolateral con basculación de la cabeza del mismo, generando un prolapso de velo anterior e hipocinesia lateral de ventrículo izquierdo (VI). El paciente fue intervenido de manera emergente previa inserción de balón de contrapulsación. Durante la preparación del campo quirúrgico presentó una fibrilación ventricular (FV) rebelde a cardioversión externa (CVE), procediéndose a la esternotomía y canulación para circulación extracorpórea (CEC) con masaje cardíaco simultáneo. Tras confirmación de la imposibilidad de conservación del músculo papilar posteromedial, se resecó el velo anterior mitral y se implantó una prótesis mecánica St. Jude número 27, conservando el velo posterior. Resultados: La salida de CEC fue dificultosa por la presencia de fallo biventricular grave, presentando un cuadro de shock cardiogénico que derivó en fallo multiorgánico en el postoperatorio inicial. Finalmente, el paciente fallece al séptimo día postoperatorio en situación de muerte encefálica. Conclusiones: La insuficiencia mitral aguda postinfarto por rotura del músculo papilar tras el consumo de cocaína es excepcional. La cirugía en estas condiciones conlleva un riesgo muy elevado.

La implantación percutánea de prótesis aórtica es una opción prometedora para los pacientes de riesgo quirúrgico prohibitivo. Presentamos el caso de una cirugía de emergencia por implante fallido de implante valvular aórtico transcatéter (TAVI) por insuficiencia periprotésica grave. Varón de 85 años sometido a implantación de TAVI CoreValve por estenosis aórtica degenerativa, descartándose previamente para cirugía. Tras el despliegue de la endoválvula se objetivó una fuga periprotésica muy grave que no disminuyó tras inflados seriados con balón y que producía gran compromiso hemodinámico. Dada la mala evolución inmediata con tratamiento conservador se optó por tratamiento quirúrgico urgente. En quirófano se observó la presencia de calcificación muy grave y rígida en comisura intercoronariana que condicionaba un gap amplio a dicho nivel. Se explantó la endoprótesis y se sustituyó la válvula aórtica por una bioprótesis Carpentier-Edwards. Resultados: La salida de circulación extracorpórea (CEC) se produjo sin incidencias, mejorando ostensiblemente la hemodinámica del paciente. No obstante, la evolución ulterior fue desfavorable, produciéndose la muerte por fallo multiorgánico precoz (renal, hepático y pulmonar), probablemente por la situación de bajo gasto preoperatorio. Conclusiones: La implantación de TAVI está demostrando ser una buena opción para el tratamiento de la estenosis aórtica grave no susceptible de sustitución quirúrgica. Sin embargo, está terapia aún está lejos de estar libre de complicaciones que requieran cirugía urgente de rescate.

Presentamos un caso de paciente mujer de 38 años con antecedentes de coartación aórtica típica leve, que consulta por clínica de disfagia lusoria, objetivándose masa pulsátil en la fosa supraclavicular izquierda en la exploración física. Se practica angiotomografía computarizada que muestra aneurisma sacular localizado en arco aórtico, cuyo origen se encuentra entre los orígenes de las arterias carótida y subclavia izquierdas y se extiende hasta la zona coartada, con diámetro máximo de 8 cm. La arteria carótida izquierda es hipoplásica y la subclavia derecha es aberrante, naciendo de la aorta descendente y cruzando la línea media hasta alcanzar MSD. Es intervenida quirúrgicamente a través de esternotomía media y toracotomía anterolateral izquierda. Tras establecer bypass femorofemoral izquierdo se clampa arco aórtico entre el origen de ambas carótidas y distalmente a la coartación, resecando el aneurisma que incluye los orígenes de ambas arterias subclavias, interponiendo a continuación injerto vascular tubular de dacrón de 20 mm de diámetro, reinsertando en el mismo la arteria subclavia izquierda. No se reimplantó la arteria subclavia derecha. El control de la presión arterial se estableció mediante sendas cánulas en aorta ascendente y arteria femoral derecha. En el postoperatorio se produjo trombosis venosa del eje subclavio izquierdo secundario verosímilmente a la sección iatrógena del tronco venoso innominado y que se resolvió satisfactoriamente con tratamiento convencional. Se muestran las imágenes de la técnica quirúrgica y la idoneidad de la corrección quirúrgica con la angiotomografía computarizada postoperatorio.

Los efectos pleiotrópicos de las estatinas convierten a este grupo farmacológico en un coadyuvante ideal para la optimización de los procedimientos de revascularización y regeneración tisular. El objetivo de este estudio es identificar sus potenciales beneficios sobre el endotelio vascular, determinando los efectos que la atorvastatina a altas concentraciones genera en la biodinámica endotelial, utilizando un modelo experimental de activación endotelial mediante ácido lisofosfatídico (LPA). Material y métodos: El ensayo se realizó sobre células endoteliales de la vena del cordón umbilical (HUVEC) in vitro, utilizando los siguientes grupos experimentales: LPA: Células activadas con LPA. ATS: Células tratadas con atorvastatina 1 μiM. LPA + ATS: Células activadas con LPA y posteriormente tratadas con atorvastatina 1 μM. ATS + LPA: Células tratadas con atorvastatina 1 μM y posteriormente activadas con LPA. Control: El análisis de viabilidad se realizó mediante microscopia electrónica analítica, y los patrones de expresión génica se obtuvieron mediante microarrays de oligonucleótidos. Resultados: El tratamiento con atorvastatina 1 uM induce un perfil de expresión génica tendente al mantenimiento de la homeostasis cardiovascular. Las células endoteliales preactivadas con LPA y posteriormente tratadas con atorvastatina 1 μM mantienen niveles óptimos de viabilidad celular. Por el contrario, las células pretratadas con atorvastatina 1 μM y posteriormente activadas con LPA y aquellas tratadas exclusivamente con atorvastatina evidencian una importante pérdida de viabilidad celular. Conclusiones: el tratamiento con atorvastatina a altas concentraciones ejerce diferentes efectos sobre la viabilidad, proliferación y funcionalidad celular dependiendo del estado basal de las células endoteliales, siendo las células previamente activadas las que presentan niveles óptimos de viabilidad y funcionalidad.

La extracción de los trombos pulmonares en el tromboembolismo pulmonar masivo antes de 1953 (operación de Trendelenburg) cosechó fracaso tras fracaso. Desde el advenimiento de la circulación extracorpórea (CEC) el porcentaje de éxito de esta operación mejoró notablemente, no habiendo dejado de aumentar hasta la actualidad, existiendo series que reflejan porcentajes de hasta el 94% de supervivencia inmediata, con 86 y 83% de supervivencia actuarial a 1 y 3 años. Presentamos un caso de paciente varón de 35 años sin enfermedad previa ni antecedentes familiares de enfermedad tromboembólica. Consulta por insuficiencia respiratoria, siendo diagnosticado por tomografía computarizada de tromboembolismo en AP izquierda, la cual ocluye. El ECO-Doppler de MMII descarta trombosis venosa profunda (TVP). Una nueva tomografía computarizada informa de persistencia de la obstrucción de AP izquierda y sus ramas, que parece haber aumentado. En ecocardiografía transtorácica (ETT) dilatación de cavidades derechas con función de ventrículo derecho (VD) normal y presión sistólica de la arteria pulmonar (PSAP) de 70 mmHg. Los estudios para investigar trombofilia fueron negativos. Tras unos días empeora su situación, produciéndose dos paradas cardiorrespiratorias, de las que es reanimado y tratado inmediatamente con TNK. La tomografía computarizada muestra la existencia de nuevo episodio de tromboembolismo pulmonar que afecta a la AP derecha gravemente. Es intervenido quirúrgicamente bajo CEC sin parada circulatoria, extrayéndose molde trombótico de tronco y ambas ramas de la AP. La evolución postoperatoria fue favorable. En tomografía computarizada de control se observan limpios el tronco y las ramas de la AP, con cierto compromiso en ramas subsegmentarias de la AP izquierda. Se muestran imágenes de la intervención y tomografía computarizada pre y posquirúrgicas.

Mostrar la eficacia y reproducibilidad de la «septoplastia ventricular de ampliación» descrita por De León (Konno modificado) en la resolución de la obstrucción subaórtica de la miocardiopatía hipertrófica obstructiva (MHO). Material y métodos: Presentamos cuatro casos de MHO. Edad 8-59 años, sexo tres mujeres (dos de ellas madre e hija) y un varón, clínica de angina y disnea en los tres adultos y presíncopes en la niña. El ecocardiograma mostró en todos la morfología típica de hipertrofia septal asimétrica alta, con gradientes máximo instantáneo entre 65-120 mmHg. En los cuatro había SAM e insuficiencia mitral. Fueron intervenidos mediante creación de comunicación interventricular (CIV) subaórtica y cierre de la misma con parche de ampliación y subsiguiente ampliación del tracto de salida de ventrículo derecho (VD) (Konno modificado). Resultados: Los cuatro pacientes sobrevivieron a la intervención. El valor medio del gradiente máximo instantáneo preoperatorio (93 mmHg) se redujo un 91 %, pasando a 8 mmHg. La insuficiencia mitral pasó a grado ligero de forma inmediata en tres casos. No hubo complicaciones reseñables salvo la necesidad de un marcapasos definitivo por bloqueo auriculoventricular (AV) completo. Los parámetros hemodinámicos permanecen estables en todos los casos. Los cuatro pacientes pasaron a grado funcional I, con seguimiento medio de 54 meses. Conclusiones: Esta técnica supone una alternativa a la cirugía convencional de la MHO, cambiando la resección muscular (Morrow) por la ampliación del tracto de salida de ventrículo izquierdo (VI) en la zona hipertrofiada. En nuestro caso se ha mostrado eficaz y segura, y ha proporcionado resultados estables en el tiempo.

The aim of this research is to evaluate the influence of the moisture content of wood on stress wave velocity, as a nondestructive technique for estimating the mechanical properties of gross cross-section Spanish Scots pine (Pinus sylvestris) lumber for structural use. 26 100 x 150 x 3000 mm pieces from Segovia, Spain, were tested with different moisture content values, from 35,5 to 9%. Measurements of longitudinal stress wave velocity were carried out during the natural drying process by using three commercial portable devices based on ultrasonic, acoustic and vibrational techniques. The results can be summarized as three percentage points of decreasing velocity per percentage point of increased wood moisture content in the range from 11,8 to 21,4%: 0,48% for ultrasonic, 0,50% for acoustic wave and 0,65% for longitudinal vibration.

The emergence of different organizational cultures inside companies has been found to impact their overall success. It is the set of beliefs, values, and customs present in a workplace that influences the behavior of its employees. Meanwhile, job satisfaction is the employees’ contentment level with respect to different aspects of their job. Hence, this study employed a descriptive-correlational research design to investigate organizational culture's influence on insurance agents' job satisfaction. To gather the essential data and meet the objectives of this study, the Organizational Culture Assessment Instrument and Job Satisfaction Survey were distributed to the respondents. The data gathered were then computed and analyzed using regression analysis, revealing that organizational culture significantly influences satisfaction among insurance agents. These results were analyzed and interpreted, with implications and recommendations provided for better understanding and application for the stakeholders of this study.

To study the prevalence and the type of childhood trauma (CT) in a first-episode psychosis (FEP) cohort and in a healthy control (HC) sample. To study which clinical and sociodemographic variables in the onset of the FEP are related to having suffered some traumatic experience in childhood.100 FEP patients and 94 HC participated in the study. The Childhood Traumatic Questionnaire (CTQ) was used to evaluate CT. The Positive and Negative Syndrome Scale (PANSS), the Personal and Social Performance (PSP), the Suicide Risk Scale of Plutchik (SRSP), and the Perceived Stress Scale (PSS) were also administered.61% of FEP patients and 17% of HC reported having experienced some kind of CT. FEP showed more CT than controls in all subscales, except in sexual abuse. The most frequent CT was emotional abuse. For the FEP group, younger age, more years of education, have a first-degree family history, more positive and negative symptoms, more perceived stress and more personal and social functioning were the variables more influenced by having suffered some kind of CT.There is a high prevalence of CT in FEP patients. Having a first-degree family history of mental illness, more positive symptoms, and more perception of stress at the time of hospital admission were related to having suffered CT. More research is needed to find out the best way to detect CT and its role in psychosis to be able to implement interventions to improve the evolution of these patients.

Background: Perfluoroalkyl substances (PFAS) are man-made fluorinated chemicals, widely used in various types of consumer products, resulting in their omnipresence in human populations. The aim of this study was to describe current PFAS levels in European teenagers and to investigate the determinants of serum/plasma con-centrations in this specific age group.Methods: PFAS concentrations were determined in serum or plasma samples from 1957 teenagers (12-18 years) from 9 European countries as part of the HBM4EU aligned studies (2014-2021). Questionnaire data were post -harmonized by each study and quality checked centrally. Only PFAS with an overall quantification frequency of at least 60% (PFOS, PFOA, PFHxS and PFNA) were included in the analyses. Sociodemographic and lifestyle factors were analysed together with food consumption frequencies to identify determinants of PFAS exposure. The variables study, sex and the highest educational level of household were included as fixed factors in the multivariable linear regression models for all PFAS and each dietary variable was added to the fixed model one by one and for each PFAS separately.Results: The European exposure values for PFAS were reported as geometric means with 95% confidence intervals (CI): PFOS [2.13 mu g/L (1.63-2.78)], PFOA ([0.97 mu g/L (0.75-1.26)]), PFNA [0.30 mu g/L (0.19-0.45)] and PFHxS [0.41 mu g/L (0.33-0.52)]. The estimated geometric mean exposure levels were significantly higher in the North and West versus the South and East of Europe. Boys had significantly higher concentrations of the four PFAS compared to girls and significantly higher PFASs concentrations were found in teenagers from households with a higher education level. Consumption of seafood and fish at least 2 times per week was significantly associated with 21% (95% CI: 12-31%) increase in PFOS concentrations and 20% (95% CI: 10-31%) increase in PFNA concentrations as compared to less frequent consumption of seafood and fish. The same trend was observed for PFOA and PFHxS but not statistically significant. Consumption of eggs at least 2 times per week was associated with 11% (95% CI: 2-22%) and 14% (95% CI: 2-27%) increase in PFOS and PFNA concentrations, respectively, as compared to less frequent consumption of eggs. Significantly higher PFOS concentrations were observed for participants consuming offal (14% (95% CI: 3-26%)), the same trend was observed for the other PFAS but not statistically significant. Local food consumption at least 2 times per week was associated with 40% (95% CI: 19-64%) increase in PFOS levels as compared to those consuming local food less frequently.Conclusion: This work provides information about current levels of PFAS in European teenagers and potential dietary sources of exposure to PFAS in European teenagers. These results can be of use for targeted monitoring of PFAS in food. Articles published in international and national journals have rich potentials for learning sources for a greater understanding and productivity to conduct novel and essential research since their publication has been scholarly processed through the hands of journal editors. Readers can learn the research methods from the models found in journal articles. This paper elaborates the research methods used in studies on Listening skills using social media and other technological tools. The elaboration is formulated from the research methods used by journal article writers worldwide. The articles are taken as the corpus of the study and analyzed using grounded analysis. Among them are case study, ethnography research, phenomenology, grounded theory, narrative, content analysis, document analysis, descriptive qualitative, pre-experimental research, quasi-experimental research, true experimental research, correlational research, ex-post facto, survey research, classroom action research, collaboration action research, and mixed-methods. However, due to the limited space, the paper will only describe five popular methods in teaching listening skills utilizing social media and technological tools for the last ten years include experimental research, classroom action research, survey research, case study, and descriptive qualitative research. The description of each method is supported by examples found in international and national journal articles published worldwide.

35. ¿Existe concordancia entre vejiga hiperactiva e hiperactividad del detrusor en varones con síntomas predominantemente de llenado derivados a Unidades de Urología Funcional y Urodinámica?

Resumen Objetivo Estudiar el grado de concordancia diagnostica entre vejiga hiperactiva (VH) e hiperactividad del detrusor (HD) en varones con sintomas del tracto urinario inferior (STUI) predominantemente de llenado, y el perfil clinico y urodinamico segun presencia de HD y grado de obstruccion del tracto urinario inferior (OTUI). Material y metodos Estudio epidemiologico, transversal, multicentrico y nacional. Se cumplimentaron: diario miccional de 3 dias (DM3d), International Prostate Symptom Score (IPSS) y Cuestionario de Autoevaluacion del Control de la Vejiga (CACV). Se determino el volumen prostatico (Vp) por ecografia y se realizo estudio urodinamico (EUD). Se analizo la prevalencia de VH y HD y la concordancia (kappa). Se realizo un analisis descriptivo de caracteristicas clinicas y urodinamicas; posteriormente se comparo su frecuencia segun presencia de HD y OTUI. Resultados Se evaluaron 445 pacientes con edad media ± DE de 54,8 ± 9,9 anos. Segun el DM3d, un 89,8% presentaba frecuencia miccional aumentada, un 87,9% nicturia, un 72,8% urgencia y un 31,9% incontinencia urinaria de urgencia (IUU). Un 36,8% tenia OTUI. El 54,5% presentaba VH y HD. La concordancia diagnostica entre HD y VH fue baja (κ = 0,1702). Mas pacientes con HD que sin ella presentaron urgencia (DM3d y CACV; p Conclusiones En pacientes varones de 18 a 65 anos con STUI predominantemente de llenado derivados a unidades especializadas, aproximadamente la mitad tienen coexistencia de VH y HD y un tercio tenia obstruccion. Hay baja concordancia diagnostica entre VH y HD.

Although linearly combining multiple variables can provide adequate diagnostic performance, certain algorithms have the limitation of being computationally demanding when the number of variables is sufficiently high. Liu et al. proposed the min–max approach that linearly combines the minimum and maximum values of biomarkers, which is computationally tractable and has been shown to be optimal in certain scenarios. We developed the Min–Max–Median/IQR algorithm under Youden index optimisation which, although more computationally intensive, is still approachable and includes more information. The aim of this work is to compare the performance of these algorithms with well-known Machine Learning algorithms, namely logistic regression and XGBoost, which have proven to be efficient in various fields of applications, particularly in the health sector. This comparison is performed on a wide range of different scenarios of simulated symmetric or asymmetric data, as well as on real clinical diagnosis data sets. The results provide useful information for binary classification problems of better algorithms in terms of performance depending on the scenario.

Xanthan gum is a polysaccharide that is widely used as a thickening agent in numerous food, cosmetic, and technical applications. Therefore, the knowledge of the molecular interplay that builds up and stabilizes water-binding networks is crucial for the optimization of xanthan thickening performance. Using atomic force microscopy, rheometry, and inductively coupled plasma optical emission spectroscopy, we show a clear correlation between xanthan thickening properties and the ability to form characteristic secondary structures as well as the valence and amount of cations coordinated at the polysaccharide side chain. Based on these findings and the Debye–Hückel theory, we derive an ion-interaction model in which divalent cations mediate bridging of adjacent single polymer strands due to chelate-like coordination building stable secondary structures. We furthermore demonstrate in a cation exchange assay that xanthan secondary structures can be modified in a directed and reversible manner, which, in turn, alters its thickening properties.

39. Prevalence and diagnostic significance of novel 12-lead ECG patterns following COVID-19 infection in elite soccer players

Background Identification of athletes with cardiac inflammation following COVID-19 can prevent exercise fatalities. The efficacy of pre and post COVID-19 infection electrocardiograms (ECGs) for detecting athletes with myopericarditis has never been reported. We aimed to assess the prevalence and diagnostic significance of novel 12-lead ECG patterns following COVID-19 infection in elite soccer players. Methods We conducted a multicentre study over a 2-year period involving 5 centres and 34 clubs and compared pre COVID and post COVID ECG changes in 455 consecutive athletes. ECGs were reported in accordance with the International recommendations for ECG interpretation in athletes. The following patterns were considered abnormal if they were not detected on the pre COVID-19 infection ECG: (a) biphasic T-waves; (b) reduction in T-wave amplitude by 50% in contiguous leads; (c) ST-segment depression; (d) J-point and ST-segment elevation >0.2 mV in the precordial leads and >0.1 mV in the limb leads; (e) tall T-waves ≥1.0 mV (f) low QRS-amplitude in >3 limb leads and (g) complete right bundle branch block. Athletes exhibiting novel ECG changes underwent cardiovascular magnetic resonance (CMR) scans. One club mandated CMR scans for all 28 (6%) athletes, despite the absence of cardiac symptoms or ECG changes. Results Athletes were aged 22±5 years (89% male and 57% white). 65 (14%) athletes reported cardiac symptoms. The mean duration of illness was 3±4 days. The post COVID ECG was performed 14±16 days following a positive PCR. 440 (97%) athletes had an unchanged post COVID-19 ECG. Of these, 3 (0.6%) had cardiac symptoms and CMRs resulted in a diagnosis of pericarditis. 15 (3%) athletes demonstrated novel ECG changes following COVID-19 infection. Among athletes who demonstrated novel ECG changes, 10 (67%) reported cardiac symptoms. 13 (87%) athletes with novel ECG changes were diagnosed with inflammatory cardiac sequelae; pericarditis (n=6), healed myocarditis (n=3), definitive myocarditis (n=2), and possible/probable myocarditis (n=2). The overall prevalence of inflammatory cardiac sequelae based on novel ECG changes was 2.8%. None of the 28 (6%) athletes, who underwent a CMR, in the absence of cardiac symptoms or novel ECG changes revealed any abnormalities. Athletes revealing novel ECG changes, had a higher prevalence of cardiac symptoms (67% v 12% p Conclusions 3% of elite soccer players demonstrated novel ECG changes post COVID-19 infection, of which almost 90% were diagnosed with cardiac inflammation during subsequent investigation. Most athletes with novel ECG changes exhibited cardiac symptoms. Novel ECGs changes contributed to a diagnosis of cardiac inflammation in 20% of athletes without cardiac symptoms. Unlike colorectal cancer (CRC), few studies have explored the predictive value of genetic risk scores (GRS) in the development of colorectal adenomas (CRA), either alone or in combination with other demographic and clinical factors. In this study, genomic DNA from 613 Spanish Caucasian patients with CRA and 829 polyp-free individuals was genotyped for 88 single-nucleotide polymorphisms (SNPs) associated with CRC risk using the MassArray™ (Sequenom) platform. After applying a multivariate logistic regression model, five SNPs were selected to calculate the GRS. Regression models adjusted by sex, age, family history of CRC, chronic use of NSAIDs, low-dose ASA, and consumption of tobacco were built in order to study the association between GRS and CRA risk. We evaluated the discriminatory capacity using the area under the receiver operating characteristic curve (AUC). The interactions between demographic information and GRS were also analyzed. Significant associations between high GRS values and risk of CRA for analyzed models were observed. In particular, patients with higher GRS values had 2.3–2.6-fold increase in risk of CRA compared to patients with middle values. Combining sex and age with the GRS significantly increased the discriminatory accuracy of the univariate model with GRS alone. The best model achieved an AUC value of 0.665 (95% CI: 0.63–0.69). The GRS showed a different behavior depending on sex and age. Our findings showed that, besides sex and age, GRS is an important risk factor for development of CRA and may be useful for CRC risk stratification and adaptation of screening programs.

Resumen Introduccion y objetivos Un porcentaje no despreciable de pacientes incluidos en programas de vigilancia activa (VA) para el cancer de prostata (CaP) de bajo y muy bajo riesgo son reclasificados en la biopsia confirmatoria o desarrollan progresion de la enfermedad durante el seguimiento. Nuestro objetivo es evaluar el papel del PCA3 y el SelectMDx, de manera individual y combinada, para predecir la progresion patologica (PP) en un programa habitual de VA. Materiales y metodos Estudio prospectivo y observacional que incluyo 86 pacientes inscritos en un protocolo de VA desde 2009 hasta 2019, con resultados de PCA3 y SelectMDx previos al diagnostico de CaP o durante su periodo de confirmacion. Se realizaron analisis univariantes y multivariantes para la correlacion de las puntuaciones de PCA3 y SelectMDx, asi como de las variables clinicopatologicas con la supervivencia libre de progresion patologica (SLPP). Se definieron los puntos de corte mas fiables para ambos biomarcadores en el contexto de VA. Resultados SelectMDx mostro diferencias estadisticamente significativas en relacion con la SLPP (HR: 1,035; IC 95%: 1,012-1,057) (p = 0,002) con un indice C de 0,670 (IC 95%: 0,529-0,810) y un AUC de 0,714 (IC 95%: 0,603-0,825) a 5 anos. En nuestra serie, el punto de corte mas fiable para el SelectMDx fue 5, con una sensibilidad y una especificidad para la PP del 69,8 y del 67,4%, respectivamente. El punto de corte del test PCA3 fue de 65, con una sensibilidad y una especificidad para la PP del 51,16 y del 74,42%, respectivamente. La combinacion de ambos biomarcadores no mejoro la prediccion de la PP, con un indice C de 0,630 (IC 95%: 0,455-0,805). Conclusiones En el contexto del CaP de bajo o muy bajo riesgo, SelectMDx > 5 predijo una supervivencia libre de PP de 5 anos con una capacidad de discriminacion moderada, superando al PCA3. La combinacion de ambos no mejoro los resultados.

The main objective is to study the predictive capacity of intrapartum total fetal reperfusion (fetal resilience) by itself or in combination with other parameters as a predictor of neonatal acidemia.A retrospective case-control study was carried out at the Miguel Servet University Hospital (Zaragoza, Spain) on a cohort of 5694 pregnant women between June 2017 and October 2018. Maternal, perinatal, and cardiotocographic records were collected. Two reviewers blindly described the monitors with the American College of Obstetricians and Gynecologists (ACOG) categorizations and parameters and the non-ACOG parameters. Neonatal acidemia was defined as pH7.10. The parameters analyzed to predict acidemia were evaluated using the sensitivity for specificity 90% value, and the area under the receiver operating characteristic curve.We recorded 192 infants with acidemia, corresponding to a global acidemia rate of 3.4%. Of these, 72 were excluded for lack of criteria, leaving 120 patients with arterial acidemia included in the study and 258 in the control group. The sensitivity (specificity 90%) of detection of acidemia was 42% for the ACOG III categorization (AUC, 0.524: 95% CI, 0.470-0.578), 24% for fetal reperfusion (AUC, 0.704: 95% CI, 0.649-0.759), 27% for total area of decelerations (AUC, 0.717: 95% CI, 0.664-0.771) and 50% for the multivariate model built from total reperfusion time (AUC, 0.826: 95% CI, 0.783-0.869). The total reperfusion time corresponding to a false negative rate of 10% is 23.75 min, with 28% of fetuses above this time. The AUC and sensitivity for a false negative rate of 10% are equivalent for deceleration area and time of reperfusion (The total reperfusion time (fetal resilience) and total deceleration area are non-ACOG parameters with a good predictive ability for neonatal acidemia, higher than the ACOG III classification and without statistical differences between them. The discrimination ability of total reperfusion time can be improved using a multivariate model. As a cutoff for its use we suggest 23.75 min in 30 min corresponding to an acidemic classification rate of 90%. New parameters in combination with other maternal, obstetrics, or fetal variables, are required for the interpretation of fetal well-being.

Obesity is the result of interactions between genes and environmental factors. Since monogenic etiology is only known in some obesity-related genes, a genetic risk score (GRS) could be useful to determine the genetic predisposition to obesity. Therefore, the aim of our study was to build a GRS able to predict genetic predisposition to overweight and obesity in European adolescents. A total of 1069 adolescents (51.3% female), aged 11–19 years participating in the Healthy Lifestyle in Europe by Nutrition in Adolescence (HELENA) cross-sectional study were genotyped. The sample was divided in non-overweight (non-OW) and overweight/obesity (OW/OB). From 611 single nucleotide polymorphisms (SNP) available, a first screening of 104 SNPs univariately associated with obesity (p < 0.20) was established selecting 21 significant SNPs (p < 0.05) in the multivariate model. Unweighted GRS (uGRS) was calculated by summing the number of risk alleles and weighted GRS (wGRS) by multiplying the risk alleles to each estimated coefficient. The area under curve (AUC) was calculated in uGRS (0.723) and wGRS (0.734) using tenfold internal cross-validation. Both uGRS and wGRS were significantly associated with body mass index (BMI) (p < .001). Gaspar Gómez de Toledo, with his Tercera parte de la tragicomedia de Celestina, intends not only to continue the previous work, the Segunda Comedia de Celestina, by Feliciano de Silva, and thus enter the celestinesque cycle that began with the publication of Celestina, but also to become the author with whom the cycle is definitively closed. Consequently he concludes unfinished episodes in Silva’s work to definitively end with the old pimp Celestina resurrected by Silva. This intention to close a cycle is motivated by the desire for fame, in order to pass into posterity as the author who culminated the adventures of Fernando de Rojas’ procurer and, therefore, to enjoy the success that the public granted to the two works that preceded him.

espanolObjetivo. Conocer las caracteristicas y factores de riesgo de infeccion por Clostridioides difficile (ICD) en un hospital de convalecencia es clave para mejorar su manejo. Material y metodos. Estudio retrospectivo con 37 pacientes, durante 43 meses. Describimos variables demograficas, datos clinicos, tiempo hasta el diagnostico, el tratamiento y la evolucion. Resultados. Analisis de 46 episodios (37 pacientes, edad media=82,2 anos). 77,8% tenian dependencia absoluta, 41,7% enfermedad renal cronica, 64,9% habian recibido antibioticos en los 3 meses previos, 40,5% recibian antibioticos en el momento del diagnostico. Fue el primer episodio en 78,4%, y la primera recidiva en 21,6%. En el 89,2% se comenzo tratamiento en las primeras 24 horas tras el diagnostico, mayoritariamente metronidazol. El 83,3% se recuperaron. 3 pacientes murieron por ICD. El diagnostico figuraba en el informe de alta en 91,1%. Conclusiones. El tratamiento antibiotico previo, un alto grado de dependencia, y el fracaso renal, fueron los factores de riesgo principales. Hay margen de mejora en el manejo de ICD en nuestro hospital. EnglishObjective. The aim of the study was to know the characteristics and risk factors of Clostridioides difficile infection (CDI) in a long-term hospital is key to improve its management. Material and methods. Retrospective study with 37 patients, along 43 months. We describe demographic variables, clinical data, time to diagnosis, treatment, and evolution. Results. Analysis of 46 episodes (37 patients, mean age=82.2 years). 77.8% were absolutely dependent, 41.7% had chronic kidney disease, 64.9% had received antibiotics in the previous three months, 40.5% received antibiotics at diagnosis. It was the first episode in 78.4%, and first recurrence in 21.6%. Therapy was started in the first 24 hours after diagnosis in 89.2%, mostly metronidazole. 83.3% recovered, 3 patients died from CDI, diagnosis was registered in the discharge report in 91.1%. Conclusions. Previous antibiotic therapy, high grade of dependency and renal failure were the main risk factors. There is room for improvement in CDI management at our hospital.

Introduction Telemedicine provides remote clinical support through technology tools. It can facilitate medical care delivery while reducing unnecessary office visits. The COVID-19 outbreak has caused an abrupt change in our daily urological practice, where teleconsultations play a crucial role. Objective To provide practical recommendations for the effective use of technological tools in telemedicine. Materials and methods A literature search was conducted on Medline until April 2020. We selected the most relevant articles related to “telemedicine” and “smart working” that could provide valuable information. Results Telemedicine refers to the use of electronic information and telecommunication tools to provide remote clinical health care support. Smart working is a working approach that uses new or existing technologies to improve performance. Telemedicine is becoming a useful and fundamental tool during the COVID-19 pandemic and will be even more in the future. It is time for us to officially give telemedicine the place it deserves in clinical practice, and it is our responsibility to adapt and familiarize with all the tools and possible strategies for its optimal implementation. We must guarantee that the quality of care received by patients and perceived by them and their families is of the highest standard. Conclusions Telemedicine facilitates remote specialized urological clinical support and solves problems caused by limited patient mobility or transfer, reduces unnecessary visits to clinics and is useful to reduce the risk of COVID-19 viral transmission.
