VURD syndrome has been repeatedly described as unilateral reflux into a nonfunctioning renal moiety. This syndrome is considered a pop-off mechanism dissipating pressure in lower urinary tract obstruction: it may be found in association with other protective mechanisms occurring in utero, such as ascites and/or urinomas, and has been exclusively described in male patients. A premature female baby with signs and symptoms of outflow obstruction underwent diagnostic workup revealing congenital urethral hypoplasia with unilateral reflux into a dysplastic kidney. Obstetrical history was positive for early onset, serologically negative ascites without cardiomegaly, which required serial aspirations. Reconstructive surgery was carried out with good results: ascites and VURD syndrome were both deemed to be perinatal protective mechanism against excess pressure in the urinary tract. Although rare, lower urinary tract obstruction in the female can lead to the same protective mechanisms seen in male fetuses/newborns. VURD syndrome and ascites should be interpreted as such and require perinatal specialist counselling.

Objective. To present a case of isolated ascites secondary due to urogenital abnormalities (urogenital sinus) without any other prenatal ultrasound marker. Method. A 36-year-old woman with prenatal isolated ascites delivered a female baby, weighing 2.285 g; ascites was drained at birth and the baby underwent several episodes of urinary retention prior to undergoing X-ray investigations. Results. A voiding cystourethrogram revealed a short urogenital sinus: a vesicostomy was performed. A vaginoscopy revealed double vagina with a large posterior vagina. A posterior sagittal anorectal pull-through with genitoplasty was performed at 2 years old with 1-year follow-up. Conclusions. Though rare, a urogenital abnormality is to be suspected in fetal ascites cases with negative viral tests and no cardiac anomalies. The most common ultrasound marker of such abnormalities (fluid filled cavity) may be missing because of complete drainage of urine through the tubes into peritoneum.

The Neapolitan Yellow Tuff (hereafter NYT) is definitely the building material most used in the historical architecture of Naples (Southern Italy) since Greek times. The high heterogeneity of NYT, resulting from the concomitant occurrence of lithic fragments, pumices, crystals and glass cemented by crystalline or amorphous phases, the latter highly sensitive to acid environments, define a strong attitude to weathering. This research confirmed the inhomogeneous features of this volcanic formation, within the same deposit and sometimes within the same outcrop. Based on these premises, the investigation was carried out on four most representative lithofacies, according to their different textural and petrophysical features. Actually, mechanical features are governed by two concurring factors: a) the mineralogical composition; b) the texture of the rock. In the first case, the predominance of anhydrous phases (analcime + feldspar) over zeolites improves its mechanical strength. By contrast, if the mineralogical composition is constant (similar total wt.% zeolite content), the petrophysical parameters are strictly related to the texture of the rock.

The use of the integrated receiver in condensers for common automotive air conditioning – A/C – systems is widespread, because of its thermodynamic and operational advantages. Many studies have been already conducted on estimating the effect of the subcooling value. However, this study aims at determining the most important factors affecting the length of the refrigerant stable operating plateau and how the receiver filling is affected by geometrical and thermodynamic boundary conditions, by means of an experimental campaign built using design of experiments – DOE – techniques. Results demonstrate how the receiver diameter and the axial spacing between its inlet and outlet holes have the highest influence on the receiver operation. Finally, these results have been used to set up a numerical model able to accurately estimate the filling efficiency of the integrated receiver, in terms of volume of the operating plateau compared to the net available receiver volume.

Epididymo-orchitis (EO) is infrequently reported in anorectal malformation (ARM) cases. Therefore, it is difficult to assess its risk factors. A total of 110 male patients who were operated on for ARM at the same Institution over a period of 13 years were contacted. Association was assessed between EO and the following: spinal dysraphism (SD), symptomatic VUR (VUR), and bowel management (BM) requiring enemas. The data were analyzed with the Chi-square test. A total of 89 patients were contacted. Ten cases of EO were found, and all occurred in patients with recto-urethral (RU) fistula after reconstruction. The patients’ age at first episode ranged between 4 and 11 years. RU fistula patients experiencing EO (Group A, 10 patients) were compared with those without EO (Group B, 33 patients). VUR occurred in 9/10 cases in Group A and in 13/33 cases in group B (Chi-square 7.8658, p = 0.005038). SD was present in 4/10 cases in group A and in 13/33 cases in Group B (Chi-square 0.0434, p = 0.83491). A total of 8/10 cases in Group A and 12/33 cases in Group B were on BM (Chi-square 5.87, p = 0.0015). EO occurs in approximately in 20 % of male cases with ARM, and recto-urinary communication and should be considered the primary diagnosis in the presence of testicular pain. This could avoid unnecessary surgical exploration, and the family should be counseled about this subject

The Sabatini Volcanic District belongs to the Roman magmatic province of Central Italy, and the Tufo Giallo della Via Tiberina was one of the most voluminous pyroclastic flow-forming eruptions in this district. Post-depositional processes strongly affected this pyroclastic flow deposit leading to the crystallization of different authigenic phases (chabazite, phillipsite, feldspar). A field volcanological survey, along with a careful mineralogical characterization of a large amount of samples of the lithified facies, allowed us to reconstruct a type section primarily based on the amounts of the main authigenic phases. Chabazite always prevailed over phillipsite throughout the entire section, although in the innermost portions of the deposit, where temperatures remained high, chabazite mostly converted into a more stable phase such as an adularia-like phase. In addition to the zeolitization process, the fairly strong mechanical properties of this tuff can be also ascribed to the diffuse occurrence of microcrystalline calcite, which re-precipitated as a secondary phase after the dissolution of carbonaceous clasts.

Objective To determine whether urinary incontinence (UI) and lower urinary tract symptoms (LUTS) persist over years, patients treated for UI and LUTS in childhood were re-evaluated in adulthood. Materials and methods Forty-seven women (cases) treated in childhood for daytime UI/LUTS (group A) and nocturnal enuresis (group B) self-completed (average age: 24.89 ± 3.5 years) the International Consultation on Incontinence Questionnaire for Female with LUTS (ICIQ-FLUTS). ICIQ-FLUTS was self-administered to 111 healthy women (average age: 23 ± 5.1 years) from a nursing school as a control group. Data obtained from ICIQ-FLUTS and quality of life (QoL) score (0–10) were compared (Fisher's exact test) between patients and controls, and between group A (n = 28) and group B (n = 19). Results Prevalence of LUTS was higher in patients than in controls. The difference between patients and controls was statistically significant (p = 0.0001) for UI (34% vs. 7%) and feeling of incomplete bladder emptying (49% vs. 28%). QoL score was >5 in 59% of patients and 1% of controls (p = 0.0001). No significant differences were found between groups A and B. Conclusions UI and LUTS are confirmed in young women who suffered for the same condition in childhood. Longitudinal studies are needed to assess if these symptoms persist or are newly onset.

This paper presents the innovative design of an air cooled permanent magnet assisted synchronous reluctance machine (PMaSyRM) for automotive traction application. Key design features include low cost ferrite magnets in an optimized rotor geometry with high saliency ratio, low weight and sufficient mechanical strength as well as a tailored hairpin stator winding in order to meet the demands of an A-segment battery electric vehicle (BEV). Effective torque ripple reduction techniques are analyzed and a suitable combination is chosen to keep additional manufacturing measures as low as possible. Although the ferrite magnets exhibit low remanence, it is shown that their contribution to the electrical machine's performance is essential in the field weakening region. Efficiency optimized torque-speed-characteristics are identified, including additional losses of the inverter, showing an overall system efficiency of more than 94 %. Lastly, the results of no load measurements of a prototype are compared to the FEM simulation results, indicating the proposed design of a PMaSyRM as a cost-effective alternative to state-of-the-art permanent magnet synchronous machines (PMSM) for vehicle traction purposes.

Summary In humans, studies on the cellular immune response against Trichinella are scarce. Aim of this study was to characterize the cytokine profile of T cells specific for Trichinella britovi in trichinellosis patients. Peripheral blood mononuclear cells (PBMC) were obtained from five patients involved in a trichinellosis outbreak caused by T. britovi, which occurred in 2013 in Tuscany (Italy). All the patients resulted positive for Trichinella-specific IgG, IgE and presented eosinophilia. T cells were investigated for their proliferation to excretory/secretory antigens from Trichinella spiralis muscle larvae (TsES) and for their cytokine profile. A total of 284 CD4+ and 42 CD8+ T-cell clones were obtained from the TsES-specific T-cell lines from PBMC. All T-cell clones proliferated in response to mitogen. Of the 284 CD4+ T-cell clones generated from TsES-specific T-cell lines, 135 (47%) proliferated significantly to TsES; 26% CD8+ T-cell clones showed proliferation to TsES. In the series of the 135 TsES-specific CD4+ clones, 51% expressed a Th2 profile, 30% a Th0 and 19% Th1. In the series of the 11 TsES-specific CD8+ T-cell clones, 18% were Tc2, 45% Tc0 and 36% Tc1. In human trichinellosis, the cellular immune response is, during the chronic phase, mixed Th1/Th2.

This paper aims at testing the use of mixtures constituted by natural zeolitized products and SiC-bearing industrial wastes (sludge deriving from polishing of porcelain stoneware tiles, hereafter DPM) for the production of lightweight expanded aggregates as constituents of structural and/or thermo-insulating lightweight concretes. Two commercial products have been used as zeolite natural source: Cab70 (Yellow facies of Campanian Ignimbrite) and IZclino (Turkish clinoptilolite-rich epiclastite). Different amounts of a calcareous material (Pozzano limestones — hereafter CP) from the Sorrento peninsula (Naples — Italy) were also added to a Cab70–DPM mixture. All raw materials were characterized by means of mineralogical (XRPD) and chemical (XRF) analyses. All the products and mixtures were tested from a technological point of view by means of fusibility and firing tests in order to evaluate the expanding properties. It was evidenced that the expansion of the mixture was deeply depending on the occurrence of SiC in the industrial waste. The addition of CP (10 wt.%) to the mixtures accounts for an even increased expansion, though this is accompanied by a worsening of the mechanical features of the material. These results along with literature data allowed to select 3 mixtures (70% Cab70–30% DPM, 70% IZclino–30% DPM, 60% Cab70–30% DPM–10% CP) and each of them was used for the preparation of 5 l of lightweight aggregates afterward employed for the manufacture of lightweight concretes. It was remarked that natural zeolitized materials mixed with DPM (30 wt.%) can provide lightweight aggregates with densities ranging between 0.8 and 1.0 g/cm 3 suitable for the preparation of structural lightweight concretes. The addition to the mixture of CP (10 wt.%) produces less dense aggregates (0.6–0.7 g/cm 3 ) potentially useful for the manufacture of thermo insulating lightweight concretes.

Industrial minerals, particularly bentonites, have long been used in treatments to improve the stability and shelf life of white wines. We evaluated a new combination of rocks and minerals, including steam-treated bentonites and natural zeolites (chabazite and phillipsite), to greatly reduce the risk of protein and tartaric instability of wines. Detailed mineralogical, chemical and electrokinetic studies of these materials were conducted using powder X-ray diffraction (PXRD), X-ray fluorescence (XRF), microporosimetry, BET surface-area analysis and zeta-potential measurements. Several model wine solutions containing Bovine Serum Albumin (BSA) were prepared to evaluate the oenological performance of the rock/mineral combinations. UV-VIS spectrophotometry and ion chromatography were used to evaluate the degree of wine stabilization from the protein and tartaric point of view.The experimental results showed that steam treatment modifies both the microporosity and external surface area of the bentonite. These changes in surface area, along with creation of hydrophobic surfaces, significantly modified the behaviour of the steam-treated bentonites, requiring an increase in the amount of material necessary to bring the protein content to required levels. An important benefit derived from the use of steam-treated bentonites is that the pre-mixing with water before addition to wine is not necessary, as the material is readily dispersed. Finally, the addition of natural zeolites effectively decreased the potassium content, thereby improving the tartaric stability of white wines. In addition, this procedure results in minimal waste, as the bentonite-zeolite mixture can be reused as soil amendments in agriculture.

Aim of this research is an evaluation of the zeolitized facies of Campanian Ignimbrite as raw material for the production of lightweight expanded aggregates (LEA). A commercial product (Cab70) was used as reference for this rock. Tests were carried out on “pure” samples or mixed with an industrial waste mud (DPM — Dried Polishing Mud) deriving from a porcelain stoneware tile polishing process. This mud contains SiC, a phase known to act as bloating enhancer. Raw materials were subjected to mineralogical (XRPD–RIR method) and chemical (XRF) analyses. Bloating upon heating was measured (Leitz heating microscope) on Cab70, DPM and on three mixtures having the following Cab70/DPM ratios: 85/15 (Mix1); 70/30 (Mix2); 50/50 (Mix3). LEA production was assessed both in static (muffle kiln) and in dynamic (rotating kiln) conditions by firing pellets, 3–8 mm in size, between 1220–1380 °C. The unit weight of the single particle was determined on fired and unfired products. All materials evidenced a decreasing density with temperature whereas the same parameter decreases with DPM increasing content. Cab70 LEAs never dropped below 0.90 g/cm3, even at the highest temperature, whereas DPM LEAs range between 0.86 g/cm3 (at 1260 °C) and 0.46 g/cm3 (at 1380 °C). A direct but not linear relationship between DPM content and density was evidenced. Among the three mixtures, Mix2 showed the lowest DPM content able to produce the strongest density reduction. To get LEAs with the same density the rotating kiln required temperatures generally 60–80 °C higher than those of the muffle kiln. This difference should be related to the different thermal insulation of the two systems during firing. Two sets of LEAs were massively produced in dynamic conditions from Cab70 (at 1380 °C) and Mix2 (at 1300 °C). These latter showed better technical performances in terms of unit weight of the single particle (0.81 vs. 0.98 g/cm3), bulk unit weight (460 vs. 565 kg/m3), water absorption coefficients (1.4 vs. 5.5% after 24 h) and compressive strength of the particles (2.9 vs. 0.6 MPa). Differences in pore shape, dimension, abundance and spatial distribution were observed by SEM between the two sets. The technical features of these LEAs were comparable to some expanded clays, with similar grain size, commercialised in Italy. These results are worth interesting as they reveal new perspectives of application both for a rock characterized by a low exploitation cost and widely available in central-southern Italy, as well as for an industrial waste produced in high amount but not recycled at all.

The combination of erythromycin and zinc in the topical antibiotic therapy of acne has been proved useful to reduce the development of antibiotic-resistant strains. An Italian zeolite-rich rock, containing 66 wt.% of clinoptilolite, has been used to prepare an active carrier for erythromycin. The zeolite has been conditioned in zinc form by a set of cation exchange processes, then erythromycin has been adsorbed onto the micronized rock by vacuum drying. After 6 month ageing, almost 95% of the loaded drug was found, indicating good stability of the Zn-carrier-erythromycin system. Minimum inhibitory concentration (M.I.C.) assay on Propionibacterium acnes showed that the process of adsorption of erythromycin does not affect its antimicrobial activity. Both drug and zinc release were evaluated from the carrier powder: the erythromycin release was more than 80% in the first hour and independent on hydrodynamic conditions (United States Pharmacopoeia dissolution apparatus or Franz cell). The release of zinc was fast, and more dependent on the hydrodynamic conditions. The influence of formulating the carrier by dispersion in two anhydrous bases, an absorption petrolatum base (HP) and a water soluble polyethylenglycol base (PEG), was evaluated. In both cases erythromycin and zinc showed a release quantitatively similar to that of the powder, although the release rate was slightly lower from the more hydrophobic HP base.

A clinoptilolite-rich rock was evaluated as inorganic Zn2+ releasing carrier for antibiotic erythromycin. The perspective is its use in the topical treatment of acne, a diffused skin pathology, given the efficacy of zinc-erythromycin combination against resistant Propionibacterium strains. The tested rock is an ash-rich epiclastite collected in Northern–Central part of Sardinia island (Italy). ICP chemical analyses of the bulk rock evidenced a composition compatible with topical applications. A 66 wt.% of clinoptilolite content was determined by means of XRD analysis (reference intensity ratio [RIR] technique). EDS chemical analyses of zeolite crystals were performed on polished thin section. The CEC of the rock is 1.45±0.08 meq/g. Using a specific exchange method, the material was previously Na-conditioned then Zn-conditioned. A substantially complete Zn-form was obtained, as demonstrated by AAS analyses. A back-exchange reaction toward Na-form was performed in the same conditions (65 °C in 1 M NaCl solution): zinc release was fast and almost complete (94%). Zn-conditioned powder was then micronized to achieve a volume/surface ratio suitable for a topical therapy. After micronization, the specific surface area, determined by BET gas adsorption, was 30.2 m2/g, and 92% of the powdered rock was lower than 30 μm in size (measured by a Coulter Counter apparatus); the so-called “volume-surface diameter” was 6.48 μm, compatible with the intended topical application. Zn2+ release was measured on micronized rock at 37 °C both in physiologic solution as in 0.05 M KH2PO4/Na2HPO4 buffer. Also in these conditions, a prompt and significant zinc release was recorded: after 30 min, 68% and 60%, respectively. Erythromycin was charged onto the micronized material using a solvent evaporation method. HPLC determinations showed that 85% of the drug contacted with the carrier was loaded. The simultaneous release of zinc and erythromycin were evaluated in phosphate buffer. Eighty-two percent of the loaded antibiotic was released after 30 min. Zinc exchange is substantially unaffected by the contemporary drug release. The request to file an international patent for this pharmaceutical application has been accepted by the European International Preliminary Examination Office.

OBJECTIVE To evaluate 10 years of experience, and thus define the occurrence and causes, of neurogenic lower urinary tract dysfunction in children with pelvic neoplasms treated by surgery. PATIENTS AND METHODS From 1991 to 2000, 33 children were operated by the same surgeons for pelvic neoplasms; 11 were analysed, comprising four each with sacrococcygeal teratoma (ST) and ganglioneuroma, and one each with yolk sac tumour (YST), neuroblastoma and myofibroblastic bladder sarcoma (MBS). The other patients were not assessed because eight had died or were in severe progression, three were treated by bladder substitution and the others were lost to follow-up or refused a urological evaluation. All 11 children were evaluated at ≥ 6 months after surgery with a questionnaire about bowel and voiding habits, a neurological and orthopaedic assessment, a noninvasive urodynamic study, renal ultrasonography and spinal and pelvic magnetic resonance imaging (MRI). All patients with signs of bladder dysfunction were evaluated by a pressure-flow study. The results were analysed for surgical approach and anatomical involvement, i.e. group A, extensive surgery for complete tumour excision in the sacral area (ST and YST); group B, surgery for tumour resection in the paraspinal ganglia area (neuroblastoma and ganglioneuroma); and group C, bladder tumour with partial bladder resection (MBS). RESULTS Eight patients had signs or symptoms related to bladder sphincter dysfunction. One child refused the invasive urodynamic evaluation, leaving seven for analysis (two each ST and ganglioneuroma, one each YST, neuroblastoma and MBS). The urodynamic findings were normal in three children. On spinal and pelvic MRI a presacral lipoma with syringomyelia was discovered in one child with ST. Eight children had bladder dysfunction and two had no neurogenic damage (which was only in sacral tumours); in one child it was related to an upper motor neurone lesion from spinal dysraphism and in the other to a lower motor neurone lesion from surgical injury to the splanchnic nerves. Patients operated for paraspinal tumours had more bladder dysfunction but no signs of neurogenic damage, as did the patient with partial bladder resection. However in Group B, there may have been a transient or incomplete nerve injury in one patient. CONCLUSIONS Deficits of parasympathetic, sympathetic and somatic innervation of the bladder and the urethra may occur in children after surgery for pelvic neoplasms, related to minor or major surgical trauma. In ST, a tethered cord may be associated with mixed neurogenic damage. Knowledge of bladder dysfunction in anorectal malformations, spinal dysraphism, etc. and the clinical protocol used in these patients also seemed to be useful for understanding the development of voiding dysfunction in patients with neoplasm.

Aerodynamic noise prediction is a major challenge in computational aeroacoustics due to the complexity of phenomena involved such as turbulence and laminar to turbulent transition. Accurate numerical methodologies, capable to provide reliable predictions in a reasonable computational time, are of large interest for the industrial design of Heating, Ventilation and Air-Conditioning (HVAC) systems. The objective of the present research work is to benchmark different CFD/CAA simulation setup (i.e. mesh topologies, boundary conditions) for predicting the broadband noise generated by low speed axial fans to develop guidelines for reliable and computationally affordable simulation. In previous works the authors investigated the capabilities of the Zonal LES technique coupled with the Ffowcs Williams-Hawkings acoustic analogy for the prediction of the noise generated by an unducted low speed axial fan. The results showed a good agreement with aerodynamic and aeroacoustic experimental data. Despite the achievements obtained so far, the high physical and numerical complexity of the problem calls for further investigations. The latest developments, presented here, focus on the impact of the mesh topology and the inflow turbulence on the far field noise prediction. Two computational meshes with different topology are investigated: an unstructured-hybrid mesh, which can be generated with fast and highly automated methods, and a structured-hybrid mesh, which allows better control of the volume mesh around the blade. Both meshes are designed to adequately resolve the boundary layer, providing LES driven values of y+, x+ and z+ on the blade surface for the operating condition considered. Two different levels of inflow turbulence are studied, one representing an ideal turbulence-free unbounded environment, and one mimicking the experimental measurements environment. All the aerodynamic and aeroacoustic simulation results presented are benchmarked with experimental data acquired by the authors.

The wild boar is an important source of trichinellosis for people in European countries as a large number of hunted animals escape veterinary control. In November 2012, uncooked sausages made with meat from wild boar were consumed by 38 persons in a village of the Lucca province (Tuscany region, Italy). Of them, 34 were serologically positive, 32 developed clinical signs and symptoms of trichinellosis, and two were asymptomatic. Trichinella britovi larvae were detected in vacuum-packed sausages made with the same batch of sausages consumed raw which had been prepared with meat from wild boar hunted in the Lucca province. As no case of trichinellosis had been reported in this region during the last 20 years, the regional public health authority considered the risk for this zoonosis to be negligible and put in place a surveillance programme on Trichinella spp. in indicator animals (mainly foxes and including wild boar for private consumption), by testing only a percentage of heads. The experience from this outbreak shows that the definition of a region with a negligible risk for Trichinella infection is not applicable to wild boar and stresses the need to test all Trichinella-susceptible wild animals intended for human consumption and to implement risk communication to consumers and hunters.

The role of bony pelvic anomalies in bladder exstrophy is long established and has generated many papers addressing walking problems. Biomechanical studies and kinematic gait analysis were performed on very young children.A direct kinetic gait evaluation has never been performed, nor has the effect of pelvis dimorphism on the upper body been studied.Controlled experimental study.Outpatients were studied at the time of periodic follow up.Nineteen patients with bladder exstrophy, age 14±8 years, and twenty-five healthy control participants, age 15±8 years, were enrolled in the present gait analysis study.Clinical evaluation and standard gait analysis were performed.Gait analysis deviations between exstrophy patients and controls and between patients that received pelvic osteotomy (OT--6 patients) and those that did not (no-OT--13 patients) were analyzed. Bladder exstrophy significantly affects kinematics and kinetics of trunk, spine, pelvis, knee and foot; in particular: in OT, trunk retroversion, pelvic retroversion and rotation, hip adduction angle and moment, knee flexion and its maximum power during loading response increased, whereas in no-OT, spine angle, pelvic posterior tilt, hip extension, and the external rotation of the foot progression angle increased. All the kinetics parameters analyzed in the study showed lower values in the patient group than in controls.. Walking in patients with bladder exstrophy is accomplished by retroversion of the pelvis and deviations mainly in the spine angle in no-OT and in knee flexion in OT.Gait analysis was shown to be an effective tool for the detection of walking deviations that should be identified early, prompting rehabilitative treatment in order to prevent spine and knee diseases.

The first urodynamic study was undertaken at 1–4 years of age in 15 boys and at 5–13 years in 15. They were re-evaluated at least 3 years later; 15 patients underwent the first and last urodynamic study, respectively, at a mean age of 2.8 and 7.7 years (group A), 10 boys at 6.2 and 8.8 years (group B) and five at 9.4 and 15.2 years (group C). In 10 boys aged > 5 years the first and last pressure-flow studies (PFS) were analysed using an advanced analysis (PFA) to better identify hypocontractility. Results Bladder dysfunction was found in 21 of 30 (70%) boys at the first evaluation and in 18 (60%) at the last. In 25 boys the urodynamic pattern changed. Of the 15 boys in group A, 10 of 12 who had hypercontractility changed to normal (seven), low compliance (one) or hypocontractility (two), and two remained stable; two of the remaining three with normal urodynamic studies changed to hypocontractility, while one was unchanged. Among the 10 boys in group B, six with hypercontractility changed to normal (three) or hypocontractility (three); two with normal urodynamic findings and one with low compliance changed to hypocontractility. Of the five boys in group C who showed severe hypocontractility after puberty, three had a normal pattern, one low compliance and one hypercontractility before puberty. The PFA showed a ‘weak’ detrusor in four of the seven boys who were considered normal on standard PFS. At the urodynamic follow-up, the PFA pattern changed to a ‘weak’ detrusor in four boys who had a normal (two) or strong (two) detrusor at the first evaluation. Conclusions Bladder dysfunction in boys with PUV changes during childhood and through adolescence. The urodynamic pattern of hypercontractility generally found soon after valve ablation gradually changes to hypocontractility in many boys and this pattern seems to be the rule after puberty. The evidence from this series supports the hypothesis that long-term detrusor hyperactivity in boys with PUV leads to detrusor failure, but a longitudinal 15-year follow-up from birth to puberty is needed to validate this concept.

Recent findings of clinoptilolite-bearing volcanoclastic deposits in northern Sardinia promoted a study aimed at evaluating the cation exchange properties of this zeolite in view of a possible future utilization of the parent rock. Isotherms of NH + 4 , Cu2+, Zn2+, Cd2+ and Pb2+ exchange for Na+ at 25°C and 0.1 total normality were obtained and the related thermodynamic quantities Ka and ΔG0 computed. The maximal exchange level was in any case less than 100% for each cation investigated. Good selectivities were recorded for NH + 4 and Pb2+, whereas clinoptilolite proved to be unselective for Cu2+, Zn2+ and Cd2+. The cation selectivity sequence NH + 4 >Pb 2+ >Na + >Cd 2+ >Cu 2+ ≅Zn 2+ was obtained. The results are discussed in the light of literature data, and contrasting performances of various clinoptilolites were ascribed to different cationic compositions of the minerals, due to different minerogenetic conditions, which were proved to affect both cation exchange capacity and selectivity. This led to the conclusion that any practical utilization of a clinoptilolite-rich rock as cation exchanger should be preceded by basic studies on representative samples from the deposit under examination.

The understanding of pathophysiology of obstructed uropathy has been facilitated by animal models with partial ureteric obstruction. Some studies on partially obstructed adult rats have drawn attention to a biphasic pattern of obstructive uropathy: an initial 'destructive' phase and a 'steady' phase in which renal deterioration no longer occurs and in which relief of obstruction would be of no advantage. We aimed to verify if this pattern applies also to younger (weanling) rats with more immature kidneys, resembling those of the human fetus. We measured the NAG-values in the urine samples of partially obstructed animals at different intervals of obstruction and in those of controls. The biphasic pattern proved to be the same as in adult rats as was previously documented, but the turning point occurred earlier (between 10 and 15 days of obstruction). Furthermore, there is evidence of low level values of N-Acetyl-Glucosaminidase (NAG) in the early phase of obstruction (5 days), demonstrating that the increase of tubular enzyme is not due to the operation itself. There is evidence that, if the 'destructive' phase can be precisely identified by biochemical studies, this could help identifying those subjects who could benefit from relief of obstruction.

The objective of this study was to review the published literature on aetiology and treatment of nocturnal enuresis, with the aim of providing a treatment strategy which is easy for the patient and their family to follow. Results from European studies conducted over the last 15 y were included in this review. It can be concluded from the results of these studies that enuresis is the cause and not the result of a psychiatric disorder. However, there is still considerable variation in success rates, from 28 to 90%. It is of vital importance that a caring approach from the doctor and a positive family and patient attitude are present for successful treatment. The first choice of treatment should be the one most acceptable to the family, e.g. alarm, desmopressin and combination treatment.

In order to integrate electric vehicles into urban mobility it is necessary to assess the usability and efficiency of hybrid, plug-in and battery electric vehicles. Despite the progress that has been made in this field over the last decade many issues still need to be addressed. These include the limited range of electric vehicles, life cycle assessment of their parts and components, integration with the electricity grid as well as long-term sustainability of Li-ion batteries. This paper analyses real urban driving data collected with GPS devices installed in vehicles. These devices enable to map typical urban mobility needs and driving patterns in detail. The data was collected in May 2011 in the Italian province of Modena. The analysis involved approximately 16,000 vehicles out of more than 50,000 surveyed. The travel data was used to quantify the capabilities of different types of electric vehicles and their ability to meet modern urban mobility needs. This paper presents the model, the input parameters and the assumptions adopted for the analysis. It examines the relevance of different recharging scenarios against real-life mobility data. The results provide a new insight into the potential electrification of urban transport and development of recharging infrastructure.

Adrenoleukodystrophy is an x-linked genetic disorder characterized by the breakdown of the myelin sheath surrounding nerve cells in the brain and progressive dysfunction of the adrenal gland with 6 possible phenotypes. Lower urinary tract symptoms affect the quality of life of these patients but only sporadic cases have been previously studied by urodynamics. We report to our knowledge the first series in the literature looking at the prevalent pattern.A total of 14 children and adults 8 to 53 years old (average age 30) with different phenotypes were referred for evaluation. Of the patients 6 had adrenomyeloneuropathy without cerebral involvement (C-) 3 had the adult cerebral form, 3 had the childhood cerebral form, 1 had cerebral involvement (C+) and 1 was asymptomatic. All patients were submitted to a screening protocol, consisting of the self-administered short form questionnaire International Conference on Incontinence Questionnaire-Short Form (ICIQ-SF) and a 3-day voiding diary. Patients were divided in group 1-no urinary symptoms, ICIQ-SF score 0 and no further investigation (3); group 2-frequency/urgency, ICIQ-SF score 1 to 5, uroflowmetry and post-voiding residual urine (4); and group 3-voiding symptoms and incontinence, ICIQ-SF score greater than 5, standard urodynamic examination (7).A neurogenic overactive bladder was the prevalent urodynamic finding (6 of 7 patients), while hypocontractility was found in 1 case.Vesicosphincteric dysfunction can be explained by spinal and/or cerebral involvement in the different neurological forms of adrenoleukodystrophy. Attempts at treatment with anticholinergics and neuromodulation in 4 of our patients provided temporary benefit with clear subjective amelioration of symptoms and improved quality of life.

We evaluated pain tolerability and the preliminary results of percutaneous tibial nerve stimulation (PTNS) in children with unresponsive lower urinary tract symptoms (LUTS).A total of 23 children 4 to 17 years old with LUTS refractory to conventional treatment underwent PTNS at 12, 30-minute weekly sessions. Ten patients had idiopathic overactive bladder, 7 were in nonneurogenic urinary retention and 6 had neuropathic bladder. Ten children were carefully evaluated for pain during needle insertion and electrical stimulation using certain scoring systems, namely the faces pain rating scale, Children's Hospital of Eastern Ontario pain scale, visual analogue scale and Questionario Italiano del Dolore. Evaluation was done at the first, sixth and last sessions. An anxiety-depression test was administered. All 23 children underwent clinical and urodynamic evaluation before and after treatment.All except 1 patient completed treatment. An anxious-depressive trait was found in 7 of 10 children/parents on anxiety-depression testing. Regarding pain, the faces pain rating scale never showed the severe pain face, the Children's Hospital of Eastern Ontario scale showed signs of pain at the beginning of each stimulation but not at the end, and the visual analog scale generally showed a low score with a further decrease during the first (p = 0.05), sixth (p = 0.03) and twelfth (p = 0.02) sessions. The Questionario Italiano del Dolore score was significantly related to the affective component of pain (p = 0.002) and it decreased between the first and last sessions. The 10 children with overactive bladder had symptom improvement in 80%, incontinence was cured in 5 of 9 and urodynamics showed normalization of cystometric bladder capacity in 62.5% with no more unstable contractions in those who became continent. Symptoms improved in 71% of the children in urinary retention. One of 3 and 4 of 7 patients had incontinence and post-void residual urine cured, respectively. Urodynamics showed an improved detrusor pressure at maximum flow (p = 0.009) and flow rate (p = 0.005). Symptoms and urodynamics did not significantly change in the neuropathic bladder group.PTNS is safe, minimally painful and feasible in children. It seems helpful for treating refractive nonneurogenic LUTS.

Industrial minerals represent an important resource for the Italian economy, both in terms of exploitation and transformation, especially in those sectors for which Italy holds a leadership such as ceramics and glass. Among Italian regions Campania is one of the poorest of such kind of resources although some geological formations could be advantageously exploited. An important role is played by the deposits of sedimentary zeolites linked to the activity of different volcanic districts occurring on the Neapolitan territory such as the Campanian Ignimbrite, the most important volcanic episode of the Campi Flegrei (Southern Italy), which blankets a wide area of the Campanian region. The Campanian Ignimbrite has been thoroughly investigated from a volcanological and petrographic point of view. However, little attention was devoted to the attainment of those information useful to verify the potential of the deposit as well as the interpretation of post-depositional mineral-forming processes that have affected this deposit and have led to the formation of several facies, among which the most important are characterized by intense feldspathization (grey facies) and remarkable zeolitization (yellow facies). X-ray diffraction, X-ray fluorescence, inductively coupled plasma emission spectrometry and scanning electron microscopy methods were used to thoroughly characterize the entire set of samples collected from 31 outcrops. Data so far acquired enabled to define the role played by several parameters (i.e., temperature, alkaline and alkaline-earth cations, etc.) in influencing the zeolitization process and the consequent crystallization of phillipsite, chabazite, and analcime. Feldspathization appears to have been controlled mainly by the emplacement temperature of the deposits. The proposed genetic model involves emplacement of the pyroclastic flow in a single episode, producing a deposit with an upward-decreasing temperature gradient. In this model, the central portion had a temperature insufficient to cause significant feldspathization, and the upper portion of the formation was affected by water percolation while still hot, producing progressive zeolitization. Volcano-stratigraphical parameters suggest some constrains on the maximum temperatures affecting the central portion of WGI. Thermodynamic data on zeolites constrain the maximum temperatures of the LYT unit which likely decrease upwards in the unit up to ambient conditions in CPF.

Objective Bovine collagen has been successfully used for the endoscopic treatment of urinary incontinence (UI) and vesico-ureteric reflux (VUR) in children for more than 8 years, although its long-term efficacy has sometimes been questioned. Its failure is generally ascribed either to technical difficulties or to instability of collagen at the site of the implant. To assess the efficacy of the procedure we evaluated the children treated at this hospital between 1990 and 1993. Patients and methods A total of 156 children, aged 5 months to 13 years, were treated for VUR. Over the same period, 25 patients aged between 2 and 14 years (nine with neuropathic bladder and 16 with exstro-phy-epispadias complex who were still incontinent after bladder neck reconstruction) had periurethral or pericervical glutaraldehyde cross-linked bovine collagen injection for UI. Five more children with exstro-phy-epispadias complex (aged 1–3 years) underwent periurethral collagen injection to stimulate bladder enhancement and allow subsequent bladder neck reconstruction. Results In children treated for VUR, a single injection proved successful in 72.2% of cases (127 ureters); a second collagen injection raised the success rate to 81%. Continence improved in all nine neuropathic bladders and in 10 of 16 children with exstrophy-epispadias complex treated for UI after bladder neck reconstruction. In four of the five exstrophy-epispadias complex patients who were treated to stimulate bladder enhancement, bladder capacity increased by 25%. Conclusions Endoscopic treatment of VUR seems to be a valid alternative to open surgery, even though concerns remain about the long-term efficacy of collagen implantation. An important distinction should be made between early and late failure of the procedure. Early failure, which we define as persistence of reflux, is usually due to incorrect technique or technical difficulties. Late failure, or recurrence of reflux, which has previously been attributed to the biodegradability of collagen, seems to be due to the displacement of the injected collagen. Micturition itself or high bladder pressure (such as detrusor instability) could be responsible for the displacement of the injected collagen medially and distally, where it can no longer support the submucosal ureteric tunnel. In the treatment of urinary incontinence, both the implant technique and the choice of the site of injection seem to have a considerable effect on the results. In our experience, endoscopic collagen injection is effective in the treatment of both urinary incontinence and VUR in paediatric patients. Accurate selection of patients and technical adjustments and refinements are essential to obtain the best results.

Analysis of Quaternary sands from coastal Somalia indicates the potential for economically useful glass-sand components and heavy minerals. The sands occur in four distinct stratigraphic levels and form subparallel ridges. Four components are present in all sand specimens: calcium carbonate, quartz, feldspars, and green hornblende; mineral compositions vary along a sea- ward-landward progression. The red sands are characterized by silica dissolution-precipitation features and alteration to laterite products.

VURD syndrome has been repeatedly described as unilateral reflux into a nonfunctioning renal moiety. This syndrome is considered a pop-off mechanism dissipating pressure in lower urinary tract obstruction: it may be found in association with other protective mechanisms occurring in utero, such as ascites and/or urinomas, and has been exclusively described in male patients. A premature female baby with signs and symptoms of outflow obstruction underwent diagnostic workup revealing congenital urethral hypoplasia with unilateral reflux into a dysplastic kidney. Obstetrical history was positive for early onset, serologically negative ascites without cardiomegaly, which required serial aspirations. Reconstructive surgery was carried out with good results: ascites and VURD syndrome were both deemed to be perinatal protective mechanism against excess pressure in the urinary tract. Although rare, lower urinary tract obstruction in the female can lead to the same protective mechanisms seen in male fetuses/newborns. VURD syndrome and ascites should be interpreted as such and require perinatal specialist counselling.

In April 1991, a cross-sectional study of HIV, HBV and HCV markers among dialysis patients was carried out in 19 Italian units in order to evaluate the spread of these bloodborne infections among patients and to evaluate the potential risk for staff who care for them. A total of 2,180 patients were eligible and all consented to be tested. Of the 1,347 patients who had not been given hepatitis B vaccine, 67.9% had at least one marker of HBV infection; of these 9.2% were HBsAg carriers; conversely, the rates were 7.6 and 0.4%, respectively, among the 833 vaccinated patients. Antibodies against HCV were found in 501 patients (23%) by EIA C100-3; of these, 270 were tested by RIBA-100: 246 (91%) were reactive and 11 (4.1%) indeterminate. Five patients resulted anti-HIV positive [0.22%, 95% CI (Poisson distribution): 0.07-0.53] by EIA and Western blot techniques. Length of time on dialysis seems to correlate with higher prevalence of HBV and HCV infection markers, but (not) HIV. Overall, 608 (28%) patients were a potential source of infections for other patients and staff. This emphasizes the need for stricter adherence to infection control, barrier precautions and preventive behaviours with all patients.

In cases of failed exstrophic closure, total cystectomy is sometimes performed combined with external urinary diversion or ureterosigmoidostomy. Prolonged life expectation and the request for a high quality of life, characteristic of pediatric and juvenile age, make it mandatory to look for different surgical solutions.Among 44 patients (26 males and 18 females) born with “exstrophy-epispadias complex”, the creation of a new orthotopic urinary reservoir, according to the “Indiana Pouch” technique, was chosen in 2 children, 4 and 9 years of age respectively, who had previously undergone a total cystectomy elsewhere. The reservoir is made of a ceco-colic segment 35 cm long detubularized on one tenia and remodelled. Ureters are reimplanted with an antireflux technique on the second tenia. Continence is assured by the ileo-cecal valve opportunely reinforced.Both patients are now perfectly continent day and night and voiding is assured by transurethral intermittent catheterization.

Objective: Oligohydramnios (OA) is nowadays regarded as one of the best markers of renal function (RF) impairment in bladder outlet obstruction (BOO) detected in utero. As such, its onset is usually early and progressive because of decline in fetal urine production. A series of acute OA complicating pregnancies with BOO has never been reported. Methods: Over a 7-year period, 5 fetuses with in utero suspicion of BOO exhibited an abrupt decrease of amniotic fluid after the 30th week of gestation. Results: All fetuses were delivered by cesarean section: diagnosis was posterior urethral valves in 3 cases, urethral atresia in 1, and prune-belly syndrome in 1. Urologic work-up demonstrated a unilateral vesicoureteral reflux dysplasia (VURD syndrome) in all 5 fetuses. RF at 1 year was normal in 4 fetuses and impaired in 1. Conclusions: Besides obstetrical reasons, OA may also have acute onset occurring in the presence of anomalies of the urinary tract; although diagnosis is almost always BOO, functional and anatomical characteristics of the urinary tract are those of VURD syndrome with a non-functioning, refluxing renal unit. The associated acute OA/VURD syndrome may represent a milder expression of a pop-off mechanism advocated in this syndrome with a more favorable prognosis than progressive OA detected early in pregnancy.
