1. A Comparison of Adult and Pediatric Enhanced Recovery after Surgery Pathways: A Move for Standardization

Abstract

Background Enhanced recovery protocols (ERP) are a multimodal approach to standardize perioperative care. To substantiate the benefit of a pediatric-centered pathway, we compared outcomes of children treated with pediatric ERP (pERP) versus adult (aERP) pathways. We aimed to compare components of each pathway to create a new comprehensive pERP to reduce variation in care. Methods Retrospective study of children (≤18 y) undergoing elective colorectal surgery from August 2015 to April 2019 at a single institution managed with pERP versus aERP. Multivariable linear and logistic regression, adjusting for demographics and operation characteristics, were used to compare outcomes. Results Out of 100 hospitalizations (72 patients) were identified, including 37 treated with pERP. pERP patients were, on average, younger (13 versus 16 y), more likely to be ASA III (70% versus 30%), and more likely to receive regional (32% versus 3%) or neuraxial (35% versus 8%) anesthesia. Epidural use was an independent risk factor for longer length of stay (P = 0.000). After adjustment, pERP patients had similar LOS and time to oral intake, but shorter foley duration. pERP patients used significantly fewer opioids and were less likely to return to the operating room within 30 d. 30-d readmissions and ED visits were also lower, but this was not statistically significant. Conclusions At our institution, data from both ERPs contributed formation of a synthesized pathway and reflected the pERP approach to opioid utilization and the aERP approach to earlier enteral nutrition.

Abstract

Background We hypothesized that an enhanced recovery after surgery (ERAS) pathway for pediatric patients undergoing surgery for inflammatory bowel disease (IBD) would be beneficial. Methods This is a single institution retrospective comparative study comparing patients treated with an ERAS pathway to consecutive patients in a Preimplementation Cohort (PIC) with similar open and laparoscopic surgeries for IBD. The pathway emphasized minimal preoperative fasting, multimodal and regional analgesia, and early enteral nutrition after surgery. Primary endpoints were time to 120 mL of PO intake (POI), length of stay (LOS), opioid utilization, and 30-day surgical outcomes. Continuous and categorical variables were compared (p Results There were 23 PIC and 28 ERAS patients with similar demographic data and surgical and anesthetic approaches. ERAS patients experienced a significant increase in the use of regional anesthesia, faster time to POI, and a nonsignificant decrease in mean LOS. ERAS patients had decreased total and daily opioid use with similar complication rates. Conclusion This study demonstrates the effectiveness of a pediatric ERAS pathway for IBD patients requiring laparoscopic and (unique to this study) open surgery. The study demonstrates that opioid utilization and time to feeding can be positively impacted using ERAS pathways without negatively impacting outcomes. Type of study Retrospective comparative study. Abstract

Background Acute hyperkalemia (serum potassium ≥ 5.1 mEq/L) is often treated with a bolus of IV insulin. This treatment may result in iatrogenic hypoglycemia (glucose Objectives The aims of this study were to accurately determine the frequency of iatrogenic hypoglycemia following insulin treatment for hyperkalemia, and to develop an electronic health record (EHR) orderset to decrease the risk for iatrogenic hypoglycemia. Design This study was an observational, prospective study. Setting The setting for this study was a university hospital. Patients All nonobstetric adult inpatients in all acute and intensive care units were eligible. Intervention Implementation of a hyperkalemia orderset (Orderset 1.1) with glucose checks before and then one, two, four, and six hours after regular intravenous insulin administration. Based on the results from Orderset 1.1, Orderset 1.2 was developed and introduced to include weight-based dosing of insulin options, alerts identifying patients at higher risk of hypoglycemia, and tools to guide decision-making based on the preinsulin blood glucose level. Measurements Patient demographics, weight, diabetes history, potassium level, renal function, and glucose levels were recorded before, and then glucose levels were measured again at one, two, four, and six hours after insulin was administered. Results The iatrogenic hypoglycemia rate identified with mandatory glucose checks in Orderset 1.1 was 21%; 92% of these occurred within three hours posttreatment. Risk factors for hypoglycemia included decreased renal function (serum creatinine >2.5 mg/dl), a high dose of insulin (>0.14 units/kg), and re-treatment with blood glucose Conclusions The use of an EHR orderset for treating hyperkalemia may reduce the risk of iatrogenic hypoglycemia in patients receiving insulin while still adequately lowering their potassium.

6. Emergency airway management of tracheo-innominate fistulas in the pediatric population

Abstract

Tracheo-innominate fistula (TIF) is a rare but well recognized complication of tracheostomy placement, and it is universally fatal in the absence of surgical management. Adult algorithms for TIF management do not address the unique dilemmas that exist in the pediatric population with respect to uncuffed tracheostomy tubes (TT). We present a case that highlights the need to modify the adult algorithm when applying it in children. Parental informed consent has been obtained for publication of the following case. A seven-year-old 25 kg encephalopathic ventilatordependent patient with a 4.0 uncuffed TT presented with emergency tracheostomy bleeding, which resulted in an inability to ventilate and subsequent loss of airway. The attending anesthesiologist was summoned immediately to the patient’s bedside and elected to secure the airway by placing a 4.0 cuffed endotracheal tube (ETT) through the tracheostomy stoma. Profuse bleeding ensued within an hour, and the patient was transported immediately to the operating room for further management. The patient was tachycardic and hypertensive, with 95% oxygen saturation on 100% O2 and unobtainable end-tidal CO2 due to copious blood in the sampling line. An earnose-and-throat (ENT) surgeon performed rigid bronchoscopy, confirming the diagnosis of TIF, and partially blocked the bleeding fistula by applying anterior pressure against the sternum. With hyperinflation of the cuff of the ETT and application of direct pressure with the rigid bronchoscope, the bleeding decreased so central venous and arterial accesses could be secured. A sternotomy was then performed, allowing identification and resection of a 0.5 cm TIF. Postoperatively, the ETT was sutured in place via the tracheostomy stoma and replaced by an uncuffed TT on postoperative day nine. The patient was discharged home at baseline condition a few days later. Previous reports have typically described the existing adult management algorithm being completely appropriate in pediatric patients with a cuffed TT who were older than age ten. The adult algorithm involves TT cuff hyperinflation distal to the fistula. If hyperinflation proves unsuccessful, digital compression of the innominate artery against the sternum via the tracheostomy stoma should be attempted. This maneuver has been reported as being successful in up to 90% of cases. It is important to recognize that the paucity of prior pediatric reports may be attributed to the primary utilization of uncuffed TT and the rarity of TIF in the pediatric population younger than age eight. This is highlighted by the longstanding recommendation in major pediatric textbooks to place an uncuffed ETT in all patients younger than age eight. With increased cuffed TT utilization, one might posit that this pediatric complication may become more prevalent in the future. Therefore, despite the current rarity of pediatric TIF in patients with an uncuffed TT, we feel that this alternate algorithm may be critical to the survival of future patients. There are several major distinctions between the adult algorithm and our proposed algorithm for emergency management of pediatric patients with uncuffed trachestomies (Figure). Cuff hyperinflation, recommended as the first step in most adult algorithms, is an obvious impossibility in most pediatric patients with an uncuffed TT.
