1. Epipericardial fat necrosis – a rare cause of pleuritic chest pain: and review of the literature

Abstract

Epipericardial fat necrosis (EPFN) is an uncommon cause of chest pain. Typically manifesting as severe acute chest pain, EPFN can be mistaken for a serious disorder, such as pulmonary embolism or myocardial infarction. We report a case of EPFN, diagnosed tentatively based on clinical and radiographic findings, with documented resolution of the lesion on chest CT. According to our literature search, this is only the second case in which chest CT alone was used to both diagnose the lesion and track its resolution. It is the first documented case diagnosed and followed as such in North America.

Abstract

In Reply. — Pais addresses a very worrisome deficiency of clinical role models in the underfinanced, understaffed city hospital setting. The "rambo resident" fills in the void and sets the tone for more junior learners—a technologically focused, clinically immature, and psychosocially barren approach that neglects the patient as a person with "human" needs. The learner is basically taught to view the patient as a disease manifest by a set of laboratory results, not as a human being trying to cope with an illness. Pais' points are well taken and portray the worst case scenario of the problems that I addressed in my commentary. The take home message remains the same: patient care in general and patient-physician communication in particular will not improve until the most clinically adept and highly motivated bedside clinicians return to center stage as the role models for students and residents. In Reply . —Drs Kaplan, Stern, and Espenschade raise a very important issue, the appropriate utilization of part-time or volunteer clinical faculty who are in private practice as clinical role models. I did not address this in my commentary because my primary focus was the effectiveness of geographical full-time faculty as clinician teachers. The negative impact of the academic reward system on the quality of clinical teaching and on patient care was also a central concern. Clearly, the potential for part-time clinical faculty to serve as effective role models warrants careful consideration. However, there are some major stumbling blocks. One potential problem is the limited availability of private practitioners who are willing to serve as inpatient and/or outpatient attendings on a regular basis. Abstract

• The faculty, residents, and fellows of the Department of Medicine, University of North Carolina (UNC) School of Medicine, Chapel Hill, were surveyed about procedures that graduates of general internal medicine programs should be able to perform independently. More than 95% of the 177 respondents agreed that, of 71 procedures, all program graduates should be able to perform 13 without supervision. Our results are similar to those of studies at two other universities with geographically distant and philosophically different departments of medicine. The UNC faculty, fellows, and residents had significant differences of opinion on the need for training in 18 procedures. Residents tended to endorse training in the largest number of procedures, faculty the fewest, with fellows in between. The respondents' subspecialty affiliations did not influence their opinions on any of the procedural skills. ( Arch Intern Med 1984;144:1177-1179)

Abstract

The role of the attending physician is crucial to the education of house staff and students. Nevertheless, certain aspects of this role have not been the subject of serious discussion or scrutiny.1What are the attending physician's expectations as a teacher? Are they compatible with the needs of students and house staff? We will examine these questions as they relate to the attending physician's effectiveness as an educator. Several principles and behaviors that we describe, if implemented, should make the attending physician's experience more gratifying. These ideas developed from our personal observations as attending physicians and from the comments of fellow faculty members in the Department of Medicine, University of North Carolina School of Medicine, Chapel Hill, at scheduled evening conferences related to patient care and teaching.
