MoveCare develops and field tests an innovative multi-actor platform that supports the independent living of the elder at home by monitoring, assist and promoting activities to counteract decline and social exclusion. It is being developed under H2020 framework and it comprises 3 hierarchical layers: (1) A service layer provides monitoring and intervention. It endows objects of everyday use with advanced processing capabilities and integrates them in a distributed pervasive monitoring system to derive degradation indexes linked to decline. (2) A context-aware Virtual Caregiver, embodied into a service robot, is the core layer. It uses artificial intelligence and machine learning to propose to the elder a personalized mix of physical/cognitive/social activities as exer-games. It evaluates the elder status, detects risky conditions, sends alerts and assists in critical tasks, in therapy and diet adherence. (3) The users’ community strongly promotes socialization acting as a bridge towards the elders’ ecosystem: other elders, clinicians, caregivers and family. Introducción: aprender a dormir en la infancia tiene repercusiones relevantes en el desarrollo integral del niño. Como profesionales sanitarios, debemos ser capaces de proporcionar las directrices adecuadas para la adquisición de hábitos saludables en la necesidad de dormir de los más pequeños. El análisis de cada caso requiere una solución adaptada a cada familia para lograr el descanso nocturno. En nuestra sociedad actual, se utilizan diferentes métodos para la adquisición de buenos hábitos para dormir de forma saludable desde los primeros años de vida. Eduard Estivill, a través de su libro Duérmete, niño, y Carlos González, con Bésame mucho, han expuesto dos estilos diferentes sobre cómo crear correctos hábitos del sueño infantil. Material y métodos: en este trabajo se realiza una investigación cualitativa a través del estudio de dichas metodologías y del testimonio de dos familias. Se quiere profundizar en los beneficios y dificultades que han aportado ambos métodos en el aprendizaje del dormir de los más pequeños. Resultados: el conocimiento de ambos métodos nos enriquece, ya que incrementa las herramientas disponibles para solventar cualquier situación relacionada con los hábitos del sueño. Después de analizar las dos entrevistas, se comprueba que las dos metodologías fueron llevadas a término con éxito, al conseguir dormir a los pequeños. Conclusiones: lo más importante no es el método utilizado, sino que los padres, ante la dificultad de enseñar a dormir a sus hijos, pasan por diferentes etapas en las que tanto su salud como la de sus niños pueden estar en situación de riesgo, sobre todo cuando se trata del primer hijo y cada situación es un nuevo reto.

From extensive application over a number of years, it has been established that the nonlinear rotor aerodynamics of typical medium and large wind turbines exhibit an effectively global separability property, in other words the aerodynamic torque of the machine can be defined by two independent additive functions. Two versions of the separability of aerodynamic torque for variable speed wind turbines are investigated here; the separated function, related to wind speed, in the first version is only dependent on that variable and not rotor speed and in the second version is only dependent on tip speed ratio. Both provide very good approximations to the aerodynamic torque over extensive neighbourhoods of T0, at least from 0 to 2T0.

SUMMARY The Mulcahy Gabbro is an Archean, layered intrusion in the Wabigoon subprovince of north-western Ontario. Alternating field and thermal demagnetization of specimens from it have provided consistent within-site characteristic remanence (ChRM) directions in eight of 19 sites. Antipodal polarities of remanence are recovered and the overall in situ formation mean is D = 240", I = -51", k = 21. This direction is unlike any reported Archean direction from the Superior province of North America. The degree of anisotropy of low-field magnetic susceptibility is high with K,/K3 values up to 1.3. Cooling selected specimens in weak fields indicate that TRM is deflected towards the magnetic fabric plane. Accordingly the in situ ChRM was corrected for the effect of magnetic anisotropy. Two methods of correction give similar results. However, the resultant formation mean remains essentially unchanged as a consequence of sufficient variability in the attitudes of K,-K, planes. Although uncertainty in the initial dip renders the tilt-correction problematical, partial tilt-correction (36 per cent) results in a formational mean with better precision than the in situ formation mean. Furthermore partial untilting of remanence directions results in a positive reversal test with a,better classification than that obtained using in situ directions. This partially tilt-corrected result (D = 202", I = -49", k = 35) yields a pole (latitude = 62"N, longitude = 42"E) which is Archean in aspect.

Thrombopheresis procedures have been recently expanded with the development or different programmes. Taking into account that this reasonably safe procedure is not devoid of complications, it would be desirable to select those individuals with lower risk of suffering adverse side effects as donors. The thrombopheresis procedures performed in our hospital between 1986 and 1997 were analysed in order to establish the useful guidelines for such selection.All the thrombopheresis procedures performed in the Asturias Central Hospital blood bank in the 1986-1987 period were analysed. The first procedure per donor, along with all data referred to adverse effects appearing during thrombopheresis, were collected. Sex, age, body, weight, blood cells count (before and after thrombopheresis) and serum calcium levels (before and after thrombopheresis) were taken as variables with predictive value for adverse effects. With regard to the procedure, the model of cell separator, the duration of the procedure, the amount and type of anticoagulant solution and the prophylactic use of calcium ions were assessed.A total number of 1,024 thrombophereses were analysed. Some types of adverse effect were seen in 259 instances (25.3%). Of these, 70.3%, were mild, 29.3% moderate and 0.4% severe. The commonest adverse effect was perioral paraesthesia. Of the different variables studied, female sex and low weight acquired predictive value with respect to the occurrence of adverse effects. Prophylactic administration of calcium did not prevent the appearance of complications.The thrombopheresis procedures may present adverse side effects in a high percentage of cases, which, although mostly mild, require specialised personnel for identification and management. Males weighing over 70 kg are less prone to suffer such effects. Oral administration of calcium before the apheresis does not prevent the adverse reactions.

Transfusion is not devoid of adverse effects; therefore every transfusional procedure must be indicated correctly. With this in mind in 1991 transfusion guidelines were published by our Hospital Transfusion Committee. Four years later, a retrospective audit was performed.We have evaluated all blood component orders (order is a transfusion request) during one month, classifying them as appropriate or inappropriate according to the transfusion guidelines. The evaluation was performed 24 hours after transfusion. The results obtained were compared with the ones obtained before the published transfusion guidelines.481 transfusion orders were studied. 87 percent (420/481) were considered appropriate according to the transfusion guidelines. These 481 orders referred to the transfusion of 1178 units to 293 patients. 12 percent of the transfused units was considered as inappropriate. 16 percent of the patients (48/239) had at least on inappropriate transfusion procedure. The comparison with the data obtained in 1991 showed a significant increase of the appropriate transfusion orders (74 versus 87 percent). The use of fresh frozen plasma showed the greatest incidence in inappropriate use, mainly in haemorrhagic disorders without factor deficit. The patients with haemoglobin levels between 70 and 100 g/L without signs of hypoxia were the most important cause of inappropriate use of red cell concentrates. And the patients with platelet levels20 x 10(9)/L were the major cause of inappropriate use of platelet concentrates.Transfusion guidelines knowledge improves the use of blood components.

Antibody formation against red blood cells' antigen is a very important complication due to transfusions, and it can make the following transfusions difficult. To avoid this, it has been proposed giving identical red blood cells for the antigens more frequently involved in sensitizations. To evaluate this fact, we have accomplished a study with the transfused patients in our Hospital since 1990.10,308 transfused patients in the Hospital Nitra. Sra. de Covadonga (Oviedo), between January 1990 and March 1994, were studied. The patients have been included in two groups: The first one was constituted by 4,226 patients from the Haematology and the Nephrology Departments, who received red blood cells units compatible with ABO and CcDEe antigens. The second group was formed by the remainding patients transfused with red blood cells compatible only with ABO and D antigens.All 165 antibodies were detected in 132 patients, which means an incidence of 1.3 percent. In 63 cases, antibodies were present before the first transfusion. In the remaining patients, an allosentitization of 0.2 percent in group 1 and 1 percent in group 2 (p0.0001) was shown. This difference cannot be explained only for transfusing red blood cells with the same Rh phenotype in the group 1, because a lower immune response had persisted when we analyzed the other antibodies. More than 70 percent of antibodies appeared before the 10th transfusion.A lower sensitization exist in the patients of group 1. This seems to be caused by a state of immunosuppression for their disease or their treatments. However, in some patients, the risk of allosensitization persists, so we think it is a good practice to transfuse red blood cells without the most immunogenic antigens in haematological and nephrological patients who already have one antibody.

We report a case of a graft-versus-host disease (GVHD) in a patient with non Hodgkin Lymphoma who received multiple blood transfusion for anemia and thrombocytopenia. Although WBC-reduction filters were used, the patient developed a transfusion associated graft-versus-host disease. We do not recommend WBC-reduction filters to prevent postransfusional-GVHD.

Experience with a colonoscope with a distal segment that allows for the incorporation of dilating olives of the Eder Puestow type is presented. 15 patients with the diagnosis of benign stenosis of the colon or rectum who suffered of abdominal pain and constipation were treated. In 13 patients it was possible to eliminate the patients symptoms. There where no complications. The dilating colonoscope is a valid, alternative in the management of stenosis of the colon and rectum.
