Aim: assessment of the impact of biological therapy on risk factors for postoperative complications. Materials and methods: the study included 116 patients operated on for Crohn's disease. The mean age of the patients was 38.2 years, among them 65 men (56 %) and 51 women (44 %). The influence of various factors on complications in the postoperative period was analyzed using the IBM-SPSS software for Mac version 22.0 and Microsoft Excel 1997-2003. Results: 32.7 % of the patients received biological therapy, and postoperative complications developed in 31 % of the operated patients. There was no association between receiving biological therapy and the risk of postoperative complications. The risk factor was the formation of a stoma and surgery for emergency indications. Conclusion: an important task before surgical treatment for Crohn's disease is a thorough preoperative preparation and assessment of all risk factors for complications of surgical intervention by a multidisciplinary team.

Four-wire distribution electrical networks of 0.4 kV equipped with automated systems of electrical energy accounting are considered. The problem of identifying the resistances of the wires of the distribution network is solved on the basis of the effective values of voltages and currents, as well as of their phase shift angles obtained by the accounting system in the power supply node of the network and from its subscribers for the selected observation intervals. A brief analysis of the known methods and technologies used in this area of research is carried out. The importance of the formulated problem for applied problems, such as control and diagnostics of electrical energy losses, as well as the technical condition of the network, is noted. A method (algorithm) is proposed that allows determining unknown, unequal complex resistances of inter-subscriber sections of the distribution network. In these resistances, the reactive components are considered equal within the inter-subscriber section; the active components differ due to the influence of unequal flowing currents and/or weather factors. At the same time, data from two different network operating modes are required, which are selected based on the analysis of the dynamics of changes of supply currents and/or voltages by the accounting system device connected to the power supply node. Considering that the active resistances of the wires must remain unchanged, the mode that is used for calculations is the one that is before the change in power consumption in the network and the next one immediately (about 0.1 s) after it. An example of a calculation that demonstrates the reliability of the proposed equations of the method that has been developed on a simulated distribution network is given. The research results are focused on the improvement of automated accounting systems and the implementation of their new functions that elevate the reliability of distribution networks, as well as allowing for the rapid identification of non-technical losses of electrical energy.

Background: the most common surgical intervention for complicated forms of Crohn's disease is ileocecal resection. This operation is performed in a planned manner with the ineffectiveness of conservative therapy. Also, the operation is performed according to emergency indications with the development of intestinal obstruction or septic complications. In the vast majority of cases, it is possible to perform a laparoscopic approach, thereby reducing the risks of complications and accelerating the recovery of the patient in the postoperative period.Aim: evaluation of the results of laparoscopic ileocecal resection in patients with Crohn's diseaseMaterials and methods: the study included 46 patients (21 women, 25 men) who underwent laparoscopic ileocecal resection for the stricture form of Crohn's disease. 18 patients were operated on for emergency indications, 28 – for planned indications. The mean age was 37.6 year.Results: conversion to laparotomy was performed in 4 patients (8.7%). The mean operative time was 128.2 minutes. Complications that required reoperation were in 3 patients (6.5%). 4 patients (8.7%) had purulent-septic wound complications.Conclusion: laparoscopic ileocecal resection for stricture Crohn's disease is a simple, easily reproducible operation, both in planned surgical interventions and in emergency cases.

Dzungarian Alatau mountain is a unique place of agrobiodiversity concentration. The apple forests consisted of the red book species Malus sieversii (Ledeb.) M. Roem. in combination with Abies sibirica Ledeb. and Populus tremula L. form a pronounced altitudinal-climatic band within 1200-1600 m in Dzungarian Alatau. In this article the features of the floristic composition of M. sieversii communities in four gorges of the Zhongar-Alatau National Park (Pikhto-voye, Mushabay, Kok-Zhota and Krutoye) were revealed. Plant species characteristic of the apple forest belt are shown. In the studied communities, there are 138 species of higher vascular plants from 110 genera, 42 families. The leading families in terms of the number of species are Asteraceae, Apiaceae, Rosaceae, Poaceae, Lamiaceae. Generic coefficient was revealed as 79.7 %. The ecological and geographical analysis of the noted species indicates that the floristic richness of the communities is mainly due to the natural conditions of growth, the leading of which are the height, exposure, moisture content of the substrate, and the closeness of the crowns of the tree canopy. In addition, the degree of anthropogenic impact on communities is of great importance.

A thermochemical method has been applied and a new routine has been developed for the synthesis of a layered material from graphite-like carbon nitride (g-C3N4) using urea and melamine compounds. The production of the graphite-like carbon nitride has been proven by TEM, electron diffraction, x-ray diffraction, Raman spectroscopy, and elemental analysis. The electrochemical studies have shown that the obtained material can be a promising candidate for metal-free catalysts of oxygen electrodes for fuel cells and a base for the development of novel functional composite materials.

Simple chronic instability of the elbow joint is one of the urgent problems in modern traumatology due to the fact that the standard tactics of managing patients with traumatic injuries of the elbow joint due to dislocation, which amounts to its elimination, immobilization of the upper limb and subsequent development of movements in the joint, most often does not take into account damage to specific soft tissue structures. With untimely treatment for qualified help, with late or unsuccessful attempts to eliminate dislocation of the bones of the forearm, less often with chronic traumatization of the ligamentous apparatus of the elbow joint during some types of occupation, one of the most unpleasant complications may form, namely, chronic instability. This article discusses the main types of chronic instability of the elbow joint, analyzes approaches to the management of patients with simple chronic instability. Despite the not very high occurrence of this pathology, the effectiveness of modern surgical treatment options aimed at reconstruction of the ligamentous apparatus of the elbow joint, the percentage of unsatisfactory results of treatment of simple chronic recurrent and persistent instability of the elbow joint remains high.

The aim. To determine the definition of nutritional extinction and its characteristics in various phases of carcinogenesis, to show the role of nutritional counseling in the selection of personalized metabolic correction programs.Materials and methods. 107 patients with pancreatic head adenocarcinoma (T3 and T4). In addition to traditional methods, the nutritional status was assessed according to the parameters of the well-known personalized alimentary-volemic diagnosis and the main common metabolic syndromes (inflammatory, hypermetabolism-hypercatabolism, toxic-anemic, anorexia-cachexia).Results. The phases of nutritional extinction are identified, in accordance with which the original schemes of metabolic correction are developed. The effectiveness of the programs used, calculated according to the degree of well-being, was significantly higher in comparison with standard solutions. The feasibility of using this tactic was confirmed (in addition to traditional tests) by measuring the metabolism in lysosomes (according to the analysis of the level of cathepsin L using ELISA in the blood serum of the considered groups of patients).Conclusions. 1) When determining the tactics of nutritional treatment of cancer patients, it is advisable to distinguish the phases of nutritional extinction, on the basis of which to carry out a differentiated metabolic correction. 2) The role of nutritional counseling (along with consulting with a surgeon and an anesthesiologist) allows you to clarify the strategy of treatment of patients with oncological pathology, and in some cases, make adjustments to the choice of the nature of the surgical aid.

Unlike ion-exchange resins, inorganic sorbents possess high selectivity towards heavy metal ions and stability against ionizing radiation. However, sorption on these materials is rather slow. Moreover, sorption capacity strongly depends on the solution pH. In order to improve sorption properties of inorganic ion-exchangers, composites containing advanced carbon materials are obtained. Regularities of sorption of U(VI) compounds from low-concentrated aqueous solutions (up to 0.1 mmol dm–3 of uranium) on hydrated zirconium dioxide and zirconium hydrophosphate are considered. The sorbents were modified with partially unzipped multiwalled carbon nanotubes (PUMWCNTs). Sorption isotherms were obtained and analyzed. They obey Dubinin-Radushkevich model indicating sorption sites, a size of which is comparable with that of ions being sorbed. As found, the sorption mechanism is ion exchange. The effect of the solution pH on the sorption rate of U(VI) ions and capacity of inorganic ion-exchangers and their composites has been considered. Carbon additions increase sorption capacity of zirconium dioxide and zirconium hydrophosphate, when the initial pH of one-component solution is 3–4 and 5–7 respectively. Under these conditions, U(VI)-containing cations are removed from the solution practically completely. The rate of sorption obeys the model of chemical reaction of pseudo-second order, when uranium is removed from one-component solution. PUMWCNTs slow down sorption on zirconium dioxide and accelerate it on zirconium hydrophosphate. The dependence of the pseudo second order equation constants on the pH of U(VI) solutions was analyzed. The reaction of the first order occurs, when the solution contains also Ca2+ and Mg2+ ions. Regeneration was carried out using HNO3 and NaHCO3 solutions: the rate-determining stage of desorption is particle diffusion. It has been shown that one-component ZHP can be regenerated with a NaHCO3 solution practically completely. The most suitable solution for U(VI) desorption from ZHP-PUMWCNTs composite is a 1 M HNO3 solution.

THE PURPOSE. The process of o n-line calculation of electricity losses in three phase distribution electric networks (DEN) with a voltage of a 0,4 kV, where monitoring of electricity losses (commercial accounting) is carried out by an automated information measuring system of electrici ty monitoring and accounting (AIMS EMA) is considered. The problem of operative detecting of unauthorized power take -offs in the DEN under condition of unknown values of parameters of the network equivalent circuit, i.e. resistances of its interpersonal sections, which can change significantly over time because of external climatic factors (temperature, humidity, etc.) is being solved. METHODS. The procedure of the proposed calculation can be implemented by the available means of AIMS EMA. It is based on simultaneous measurements for the same observation interval of effective values of current and voltage, active and reactive powers at the beginning of the DEN and at each subscriber. In the course of periodic measurements of these mode parameters, the initial data used in the proposed calculation are formed. RESULTS. The analysis of the known methods of solving this problem is carried out, their disadvantages are shown and the new methodology, which is based on a preliminary operative calculation of reliable values of the resistances of interpersonal sections, provided that unauthorized power take -offs are possible in DEN is presented. While solving, the conditions of equality of resistances of the phase and neutral wires within the interpersonal section of th e DEN are used. The proposed method makes available the implementation of the operative calculation of technical losses in DEN and the identification of the commercial losses (unauthorized power take -offs) in it, as well as a detailed analysis of subscribe r data in order to detect the location (coordinate) and the amount of unaccounted electricity. CONCLUSIONS. The proposed methodology can be used in existing AIMS EMA without the introduction of additional measuring means (functions).

A ceramic Li conducting membrane which consist from Li0.56La0.33TiO3 and an ion-conducting membrane MF4-SK (Nafion type) have been investigated in solutions with different pH values. It was found that the MF4-SK and LiLaTiO3 membranes work effectively in a photoelectrochemical system for the hydrogen evolution. Is shown the fundamental possibility of creating an electrochemical cell with a membrane based on lanthanum-lithium titanates (LiLaTiO3). By using ceramic membrane pH in the cathode chamber of the cell were stable for 2-3 months. The ceramic membrane has an advantage over MF4-SK at a low current density and subject to a decrease in its resistivity (an increase in the area and a decrease in its thickness) to increase the current.

A method for producing graphene quantum dots (GQDs) by ultrasonic grinding of e1lectrochemically obtained partially unzipped multi-walled carbon nanotubes is proposed. As a result, photoluminescent GQDs with a blue glow have been obtained from partially unzipped multi-walled carbon nanotubes processed two hours by ultrasonic grinding (UZ-CNTs). Various physicochemical methods including photoluminescence spectroscopy have been used to study the electrochemically synthesized UZ-CNTs GQD. After ultrasonic grinding of electrochemically synthesized UZ-CNTs the GQDs were obtained as a result of the exfoliation from the stress-bent MWCNTs walls one- or two-layer cup-shaped graphene nanoparticles 1.5-3 nm in size. The obtained GQDs are promising materials as a new type of catalyst carriers for oxygen electrodes of fuel cells.

The problem of organization of monitoring and diagnosing of the technical state of a four-wire distribution electric network (DEN) by means of an automated information-measuring system of monitoring and accounting of electricity (AIMS EMA) consumed by subscribers is considered. As parameters characterizing the technical state of the DEN, the values of the active resistances of the lines (wires) in the network are taken. The indicated parameters can change significantly over time depending on the values of flowing currents, climatic factors of the external environment, as well as due to wear out of the wires. A technique of the operational identification of resistances of inter-subscriber DEN sections, which uses the data of the distribution network operating modes is proposed. Its implementation in AIMS EMA allows to monitor changes in values of their active resistances. A procedure of diagnosing lines in DEN is presented, it consists in analyzing the dynamics of active resistances values changes obtained at different points in time, and identifying the increase of the wear level of individual wires against the background of flowing currents and climatic factors. It is believed, that external influences lead to the same relative changes of parameters of all wires for the same interpersonal site. The wear diagnostic criterion is an increase of the active resistance values for individual wires with their unequal change relatively to the baseline values, which are determined during observations of the distribution network condition.

Population dynamics of the bream Abramis brama in lakes Peipsi and Pihkva for the period from 1980 to 2009 has been studied by means of cohort models. Bream stock increased steadily in Lake Pihkva until 1999 and in Lake Peipsi until 2004, however then a trend towards a decrease of stock down to the level of early 1980s has been detected. In the 2000s a decrease in the population average age has been registered; it could be explained by both an increase in fishing intensity and high recruitment rates of the late 1990s. Highly abundant bream generations emerged in both lakes simultaneously. In late 2000s low level of recruitment was noted in both lakes. The qualitative relationship between stock and recruitment is best described by Ricker’s curve. It has been shown that bream recruitment depends on water transparency, water temperature, average annual number of days with air temperature above 20°С, and average annual water level. The existing fishery pressure on the stock of bream is close to optimal. A further increase in fishing intensity in both lakes is impractical. The current level of equilibrium yield can be achieved with much lower fishing intensity.

The composites based on hydrated zirconium dioxide and zirconium hydrophosphate have been obtained. The materials contained 0.5–7% of graphene-like additions, which were obtained by chemical treatment of multiwalled carbon nanotubes (8–15 layers). The composites as well as their constituents were investigated with the XRD, FTIR, TEM, and porometric methods. When the content of carbon material is 2%, large granules (0.3–0.35 mm) are formed. GO was found to increase specific surface area of zirconium hydrophosphate and reduces it in the case of zirconium dioxide. Despite the small amount of GO additions, these composites show a growth of total sorption capacity of 12–14% in alkaline media. Sorption of cationic complexes of U(VI) under batch conditions was investigated. The composite based on zirconium hydrophosphate allows us to remove U(VI) from water practically completely in the presence of hardness ions; the highest regeneration degree is also achieved. Sorption kinetics is described by the model of pseudo-second order. The Dubinin–Raduchkevich model was applied to adsorption isotherms. As found, the mechanism of sorption is ion exchange that is accompanied by additional interaction of sorbed ions with functional groups.

Criteria for estimating the quality of coordinate measurements under the conditions of experimental pilot-scale construction based on the use of modern methods of construction of high-quality measurement systems and traditional methods of estimating the quality of measurements in metrology are considered. The duration of the measurements is proposed as an additional criterion of the quality of measurements. The role of technological inheritance in the analysis of sources of uncertainty of coordinate measurements is noted. It is shown that such characteristics of the form of controlled geometric elements as planeness, roundness, and cylindricity must be taken into account in order to achieve an objective estimate of the quality of coordinate measurements. It is established that decreasing the uncertainty of the initial data and the measurement conditions together with increasing the stability of already solved problems through a well-defined selection of the basic geometric elements (measurement base of the part) is one of the basic methods of reducing the uncertainty of coordinate measurements. A necessary condition for obtaining reliable quality indicators, such as quantitative estimation of conditions and management of the measurement process, is found. It is proposed that analytic dependences needed for quantitative analysis and optimization of the quality indicators of measurements, including measurement time, should be used to schedule and standardize the measurement time.

Treatment of iron deficiency conditions by nutritive support media is one of the debatable areas. In the analysis of nutritional status, nutritional risk and ferrumdeficiency parameters in patients with operated colorectal cancer in different stages, the possibility of correction of prelatent and latent iron deficiency with ferrum‑containing media of nutritional alimentation is shown. With the development of anemia, the use of с intravenous correction (preferably with drugs sorbed on a special matrix that allows the gland to be isolated in portions) is shown.

The first colproctrectomy with the formation of a pelvic intestinal reservoir was performed in 1970. Since then, the technique has been thoroughly tested and currently it is the gold standard in the surgical treatment of ulcerative colitis and familial adenomatous polyposis. This operation allows to improve the quality of patients’ life, reducing the need to form a permanent ileostomy, improving the psychomotional, social and physical rehabilitation of patients. However, this intervention is associated with a high risk of postoperative complications. Impairment of the the pelvic reservoir functioning, as well as the addition of infection, are terrible complications that significantly worsen the patient’s quality of life. Surgeons performing this type of surgery have to face a number of intraoperative and postoperative complications that require repeated interventions. The aim of the study is to improve the results of treatment of patients with the small bowel reservoir. A review and analysis of the literature is performed in this issue. Successful results of performing colproctectomy with the formation of a pelvic enteric reservoir depend on careful preoperative planning, experience of the surgeon, prevention of intra‑ and postoperative complications.

The photoelectrochemical properties of electrodes based on a nanocomposite consisting of TiO2 nanotubes and CdSe films modified with graphene oxide have been studied. It is shown that in such a nanocomposite, a good adhesion of the CdSe layer to the substrate is provided. In this nanocomposite, the cathodic dark leakage currents are lower, and the efficiency of photoelectrodes is higher. From studies of Raman spectra, in which 2LO and 3LO phonon lines were observed, the quality of the modified polycrystalline films of CdSe was concluded. The amount of graphene oxide in NT-TiO2/CdSe/GO composite was determined to be 1.16 mass%, which allows one to obtain optimal photosensitivity of electrodes. It was established that the introduction of graphene oxide into semiconductor electrodes leads to an increase in their photosensitivity which is associated with a decrease of the surface recombination rate. The efficiency of the investigated nanocomposites in the photoelectrochemical cell for hydrogen production was studied together with composites based on graphene materials.

Variational problems that arise in the course of minimization of the number of control points in coordinate measurements of the characteristics of form, orientation, and relative position of the geometric elements of articles is solved on the basis of analytic models of estimates of the errors under the condition of an upper bound on the value of the measurement error. The proposed methods of solution are illustrated by examples of planning of coordinate measurements of flatness, parallelism, and perpendicularity of planes, the distance between planes, the circularity parameters, and the coaxiality of cylinders.

Background. Fracture of the distal radius is the most common injures bones of the forearm. Multifragmentary articular injures have occupied a special place among all fractures of the distal radius This factor due to the severity of the injury wrist joint, young patients and difficult rehabilitation. Purpose of study: Improvement results surgical treatment unstable fractures of the distal radius. Patients and methods. All patients were depended into two groups: The first group consisted of 34 patients. The first stage of treatment include close reduction and external fixations. If after external fixation we have persisting fractures dislocation, we take conversion external fixation, open reduction and plate fixation. The second group were included 30 person. In this group we take open reduction and plate fixation. Results. 6 or 12 month after surgery in 1 group we have receiver 87% excellent results. In the the second group we have received 77% excellent results. In the first group 2 cases have slow fractures consolidation, infection complication not found. In the second group we have 2 complication: CRPS and infection. Conclusion. Results in two groups of patients are assessed as good and excellent. The results of the study showed that external fixation in first stage of treatment allows to achieved better restoration radiocarpal joint anatomy (except for type В fractures), and without secondaly displacement tendency. The last fact is very important for patient with delayed the second stage of treatment. This patients with multiple injuries, open fractures and presence of an infected wound and etc. The second stage of treatment consisted of open reduction and plate fixation. Achieved full anatomy restoration radiocarpal joint with possible early rehabilitation.

reversible photoelectrochemical system with using of accumulated hydrogen has been developed. Reversibility was ensured by introducing a third electrode based on Cu2S, onto which a metal hydride electrode based on MH alloys was discharged. The photoaccumulator made it possible to obtain a maximum current of 97 mA and Vo.c. ≈ 0.4 V, during the discharge by the MH electrode without illumination. A new feature of the cell design was construction in 3D. Almost all the details of the cell were printed on a 3D printer. It was found that the peak of current during cell illumination was several times higher than the maximum current obtained by the 2-electrode scheme. For consumers with uneven power consumption, such a system is equivalent to a solid state system with more power.

The paper considers the issue of calculating of the wires complex impedances and resistances of the re-grounding connections in a three-phase distribution network. It is based on measurement data of the effective values of voltages and currents, as well as phase shifts between them, performed simultaneously by the electricity metering system at the start of the network and at each of its subscribers. An algorithm (method) of solving the above problem is presented for the case when in the distribution network the resistances of the phase and neutral wires within each inter-subscriber section and of all re-grounding connections are the same. The proposed algorithm (method) is based on serial (iterative) calculation and verification of the equality of the current through the grounding connection of the electrical energy source and the total current of all re-grounding connections in the distribution network. The correctness of the calculated expressions of the proposed algorithm (method) is confirmed by the presented calculation example for the simulated distribution network. The results obtained can be useful in organizing diagnostics of the functional state of re-grounding connections and sections of the main line, as well as in calculations and monitoring of electricity losses performed by an automated metering system operating in a distribution network with re-grounding connections.

The work is devoted to the development of technology of computer-aided design of elastic elements of wind tunnel balances, used in the testing of scale models of prospective aircrafts in wind tunnels. Modern technologies of automated design are based on the application of methods of parametric and topological optimization of structures, as well as the use of finite element analysis of stress-strain state of structures. These computational methods are implemented in software packages for engineering analysis and design (CAD/CAE systems). The problem of optimization of elastic elements of multi-component load cells is formulated. The main aspects of application of parametric and topological optimization methods in relation to elastic element design are considered. The practical application of topological optimization methods is limited by the high complexity of the resulting geometry. There is a problem of interpretation of the problem solution results. At the same time, the elastic element geometry is determined primarily by technological constrains associated with used manufacturing technologies. It is shown that the introduction of technological constraints in the formulation of the topological optimization problem allows to obtain geometry that is suitable for practical application. An algorithm for solving the problem of optimization with technological constraints has been developed, based on the correlation analysis of two geometrical models, performed using mutual correlation functions, showing the degree of difference or similarity of these models. The algorithm is based on a combination of parametric and topological optimization methods, so it can be attributed to parametric-topological optimization methods. The proposed algorithm has been tested. It is shown that the results of algorithm using are adequate, can be easily interpreted and suitable for practical use.

One of the most important components of the quality of functioning of power systems is their oscillatory stability. In the conditions of the development and improvement of the reliability of power systems, the task of monitoring low-frequency oscillations (LFO) is becoming more and more urgent. The increasing availability of phasor measurement units (PMUs) and the ongoing implementation of the Wide Area Measurement Systems (WAMS) makes it possible to solve applied tasks at different levels of the power system. In addition to detecting emergency and abnormal modes, based on the processing of data from synchrophasor measurements, the search for sources of system destabilization can also be performed.

Restrictions of emissions for civil aircraft engines, on the one hand, and the need in increasing the engine efficiency, on the other hand, cause difficulties during development of low-emission combustors for such engines.

Partially unzipped multi-walled carbon nanotubes were used as an active layer of the oxygen electrodes. The obtaining of partially unzipped carbon nanotubes was confirmed by X-ray diffraction, transmission electron microscopy and Raman spectroscopy. An investigation of the electrocatalytic characteristics of such oxygen electrodes was carried out in a fuel half-cell with an alkaline electrolyte. Electrochemical studies have established that the obtained partially unzipped multi-walled carbon nanotubes are promising materials as a catalyst carrier for oxygen electrodes of fuel cells. The oxygen electrodes based on the electrochemically obtained carbon materials were stable for six months at a discharge current density of 200 mA cm−2.

The criteria of the coordinate measurements quality at pilot-experimental production based on contemporary methods of quality management system and traditional methods of the measurements quality in Metrology are considered. As an additional criterion for quality of measurements, their duration is proposed. Analyzing the problem of assessing the quality of measurements, the authors pay particular attention to the role of technological heredity in the analysis of the sources of uncertainty of coordinate measurements, including not only the process of manufacturing the part, but all stages of the development of design and technological documentation. Along with such criteria as the degree of confidence in the results of measurements; the accuracy, convergence, reproducibility and speed of the results must take into account the correctness of technical specification, and such characteristics of the shape of the geometric elements to be controlled, such as flatness, roundness, cylindrical. It is noted that one of the main methods to reduce the uncertainty of coordinate measurements is to reduce the uncertainty in the initial data and measurement conditions, as well as to increase the stability of the tasks due to the reasonable choice of the basic geometric elements (measuring bases) of the part. A prerequisite for obtaining reliable quality indicators is a quantitative assessment of the conditions and organization of the measurement process. To plan and normalize the time of measurements, the authors propose to use analytical formulas, on the basis of which it is possible to perform quantitative analysis and optimization of quality indicators, including the speed of measurements.
