Abstract

The first source catalog of the Large High Altitude Air Shower Observatory (LHAASO) reported the detection of a very high energy gamma-ray source, 1LHAASO J1219+2915. This Letter presents a further detailed study of the spectral and temporal behavior of this pointlike source. The best-fit position of the TeV source (R.A. = 185.°05 ± 0.°04, decl. = 29.°25 ± 0.°03) is compatible with NGC 4278 within ∼0.°03. Variation analysis shows an indication of variability on a timescale of a few months in the TeV band, which is consistent with low-frequency observations. Based on these observations, we report the detection of TeV γ -ray emissions from this low-luminosity active galactic nucleus. The observation by LHAASO's Water Cherenkov Detector Array during the active period has a significance level of 8.8 σ with a best-fit photon spectral index Γ = 2.56 ± 0.14 and a flux f _1–10 TeV = (7.0 ± 1.1 _sta ± 0.35 _syst ) × 10 ^−13 photons cm ^−2 s ^−1 , or approximately 5% of the Crab Nebula. The discovery of VHE gamma-ray emission from NGC 4278 indicates that compact, weak radio jets can efficiently accelerate particles and emit TeV photons.

Abstract

We present the first catalog of very-high-energy and ultra-high-energy gamma-ray sources detected by the Large High Altitude Air Shower Observatory. The catalog was compiled using 508 days of data collected by the Water Cherenkov Detector Array from 2021 March to 2022 September and 933 days of data recorded by the Kilometer Squared Array from 2020 January to 2022 September. This catalog represents the main result from the most sensitive large coverage gamma-ray survey of the sky above 1 TeV, covering decl. from −20° to 80°. In total, the catalog contains 90 sources with an extended size smaller than 2° and a significance of detection at >5 σ . Based on our source association criteria, 32 new TeV sources are proposed in this study. Among the 90 sources, 43 sources are detected with ultra-high energy ( E > 100 TeV) emission at >4 σ significance level. We provide the position, extension, and spectral characteristics of all the sources in this catalog.

3. Python-assisted biological knowledge acquisition method to trigger design inspiration

Abstract

Abstract Design inspiration comes from the continuous stimulation of external information and the continuous accumulation of knowledge. In order to obtain an ideal design inspiration from nature, researchers have proposed a large number of biological information retrieval and knowledge acquisition methods. But how to purposefully acquire valuable biological knowledge in order to effectively stimulate design inspiration and produce the novel and feasible designs idea is still an urgent problem to be solved. This paper proposes a method for acquiring valuable biological knowledge to efficiently stimulate inspiration and quickly conceive solutions in engineering design. First, keywords, such as the functional requirements and key components of design objects, are selected as the engineering terminologies. Next, biological keywords related to the engineering terminologies are searched from the biological dictionary and biology websites. Then in order to retrieve enough biological knowledge, these biological keywords are expanded manually and automatically respectively based on Thesaurus Webpage and WordNet database, and expanded keywords are filtered according to repeated words and different forms of the same words. Finally, in the biological knowledge base, biological keywords that had been filtered are used to obtain biological knowledge with Python web crawler programming. Through an example of application for ship equipment, the effectiveness of the method is verified.

Abstract

Background The COVID‐19 pandemic has disproportionately affected Black and Hispanic communities, whose stroke care has been previously shown to experience existing disparities. We sought to evaluate how these disparities in stroke care and in‐hospital mortality have been affected by the COVID‐19 pandemic. Methods In this retrospective observational cohort study, we evaluated stroke hospitalizations in the Society of Vascular and Interventional Neurology COVID‐19 registry. We compared stroke characteristics between non‐Hispanic White, non‐Hispanic Black, and Hispanic patients pre–COVID‐19 and post–COVID‐19 (March–July 2019 versus March–July 2020) and evaluated whether racial and ethnic differences present before the pandemic were exacerbated during the pandemic. Our primary outcome was in‐hospital mortality/discharge to hospice, and secondary outcomes were acute treatment use. Results Of the 4908 included patients, numerically fewer non‐Hispanic White and Hispanic patients were evaluated during COVID‐19. Abstract

Abstract The focal plane camera is the core component of the Wide Field-of-view Cherenkov/fluorescence Telescope Array (WFCTA) of the Large High-Altitude Air Shower Observatory (LHAASO). Because of the capability of working under moonlight without aging, silicon photomultipliers (SiPM) have been proven to be not only an alternative but also an improvement to conventional photomultiplier tubes (PMT) in this application. Eighteen SiPM-based cameras with square light funnels have been built for WFCTA. The telescopes have collected more than 100 million cosmic ray events and preliminary results indicate that these cameras are capable of working under moonlight. The characteristics of the light funnels and SiPMs pose challenges (e.g. dynamic range, dark count rate, assembly techniques). In this paper, we present the design features, manufacturing techniques and performances of these cameras. Finally, the test facilities, the test methods and results of SiPMs in the cameras are reported here.

6. Inpatient Teleneurology Follow-up Has Comparable Outcomes to In-Person Neurology Follow-up

Abstract

Background and ObjectivesCommunity emergency departments often transfer patients for lack of neurology coverage, potentially burdening patients and accepting facilities. Telestroke improves access to acute stroke care, but there is a lack of data on inpatient teleneurology and telestroke care.MethodsFrom our prospective telestroke registry, we retrospectively reviewed 3702 consecutive patients who were seen via telestroke between September 2015 and December 2018. Patients who required transfer after initial telestroke evaluation or who were kept at hospitals without consistent neurology coverage were excluded from analysis. We compared baseline demographics, clinical characteristics, and hospital outcomes in patients who were subsequently followed remotely by a teleneurology neurohospitalist and those followed in person by a neurohospitalist.ResultsThere were 447 (23%) patients followed by a teleneurology neurohospitalist and 1459 (77%) patients followed in person by a neurohospitalist. Both groups presented with similar stroke severity. In multivariate analysis, there were no significant differences in discharge disposition, stroke readmission rates, or 90-day modified Rankin Scale (mRS) scores. Length of stay was shorter with teleneurology follow-up. In the subgroup of patients who received tissue plasminogen activator, patients showed no differences in outcomes and had similar complication rates. Teleneurology follow-up resulted in a 3% transfer rate for higher level of care after admission. There remained no difference in outcomes in a subanalysis without Comprehensive Stroke Centers. A higher proportion of non-Hispanic Black patients and a lower proportion of Hispanic patients in the teleneurology follow-up group were possibly due to spoke location demographics.DiscussionTeleneurology follow-up resulted in comparable outcomes to in-person neurology follow-up, with few transfers after admission. For select neurology and ischemic stroke patients, teleneurology follow-up provides an alternative to transfer for hospitals lacking neurology coverage.

Abstract

Background and purpose Previous studies suggest that mechanisms and outcomes in patients with COVID-19-associated stroke differ from those in patients with non-COVID-19-associated strokes, but there is limited comparative evidence focusing on these populations. The aim of this study, therefore, was to determine if a significant association exists between COVID-19 status with revascularization and functional outcomes following thrombectomy for large vessel occlusion (LVO), after adjustment for potential confounding factors. Methods A cross-sectional, international multicenter retrospective study was conducted in consecutively admitted COVID-19 patients with concomitant acute LVO, compared to a control group without COVID-19. Data collected included age, gender, comorbidities, clinical characteristics, details of the involved vessels, procedural technique, and various outcomes. A multivariable-adjusted analysis was conducted. Results In this cohort of 697 patients with acute LVO, 302 had COVID-19 while 395 patients did not. There was a significant difference (p < 0.001) in the mean age (in years) and gender of patients, with younger patients and more males in the COVID-19 group. In terms of favorable revascularization (modified Thrombolysis in Cerebral Infarction [mTICI] grade 3), COVID-19 was associated with lower odds of complete revascularization (odds ratio 0.33, 95% confidence interval [CI] 0.23-0.48; p < 0.001), which persisted on multivariable modeling with adjustment for other predictors (adjusted odds ratio 0.30, 95% CI 0.12-0.77; p = 0.012). Moreover, endovascular complications, in-hospital mortality, and length of hospital stay were significantly higher among COVID-19 patients (p < 0.001). Conclusion COVID-19 was an independent predictor of incomplete revascularization and poor functional outcome in patients with stroke due to LVO. Furthermore, COVID-19 patients with LVO were more often younger and had higher morbidity/mortality rates.

Abstract

Observation of high energy and very high emission from Gamma Ray Bursts (GRBs) is crucial to study the gigantic explosion and the underline processes. With a large field-of-view and almost full duty cycle, the Water Cherenkov Detector Array (WCDA), a sub-array of the Large High Altitude Air Shower Observatory (LHAASO), is appropriate to monitor the very high energy emission from unpredictable transients such as GRBs. Nevertheless, the main issue for an extensive air shower array is the high energy threshold which limits the horizon of the detector. To address this issue a new trigger method is developed in this article to lower the energy threshold of WCDA for GRB observation. The proposed method significantly improves the detection efficiency of WCDA for gamma-rays around the GRB direction at 10-300 GeV. The sensitivity of the WCDA for GRB detection with the new trigger method is estimated. The achieved sensitivity of the quarter WCDA array above 10 GeV is comparable with that of Fermi-LAT. The data analysis process and corresponding fluence upper limit for GRB 190719C is presented as an example.

Abstract

The main scientific goal of LHAASO-WCDA is to survey gamma-ray sources with energy from 100 GeV to 30 TeV. To observe high-energy shower events, especially to measure the energy spectrum of cosmic rays from 100 TeV to 10 PeV, a dynamic range extension system (WCDA++) is designed to use a 1.5-inch PMT with a dynamic range of four orders of magnitude for each cell in WCDA-1. The dynamic range is extended by using these PMTs to measure the effective charge density in the core region of air shower events, which is an important parameter for identifying the composition of primary particles. The system has been running for more than one year. In this paper, the details of the design and performance of WCDA++ are presented.

Abstract

The kilometer square array (KM2A) of the large high altitude air shower observatory (LHAASO) aims at surveying the northern γ-ray sky at energies above 10 TeV with unprecedented sensitivity. γ-ray observations have long been one of the most powerful tools for dark matter searches, as, e.g., high-energy γ rays could be produced by the decays of heavy dark matter particles. In this Letter, we present the first dark matter analysis with LHAASO-KM2A, using the first 340 days of data from 1/2-KM2A and 230 days of data from 3/4-KM2A. Several regions of interest are used to search for a signal and account for the residual cosmic-ray background after γ/hadron separation. We find no excess of dark matter signals, and thus place some of the strongest γ-ray constraints on the lifetime of heavy dark matter particles with mass between 105 and 109 GeV. Our results with LHAASO are robust, and have important implications for dark matter interpretations of the diffuse astrophysical high-energy neutrino emission.

Abstract

Water Cherenkov detector array (WCDA) is one of the key parts of the Large High Altitude Air Shower Observatory (LHAASO), the construction of which was completed by the end of 2020. The WCDA covers a 78 000-m2 area and there exist 3120 large size photomultiplier tubes (PMTs) in three ponds: 8-in PMTs are used in WCDA pond No. 1 and 20-in PMTs are used in ponds No. 2 and No. 3. The front-end electronics (FEE) system based on multigain measurement technique is designed to achieve both high-precision time and charge measurements over a large dynamic range from single photon electron (S.P.E.) to 4000 P.E. (for water pond No. 1)/1800 P.E. (for water ponds No. 2 and No. 3). To achieve a high-quality clock distribution and phase alignment as well as mixed transmission of data, clock, and commands in one fiber over a long distance, an enhanced white rabbit (WR) technique is used. Testing of all the 350 FEE modules for the WCDA is presented in this article. Test results indicate that the charge resolution is better than 20% at S.P.E. and 1% at 1800/4000 P.E. and the time resolution is better than 300 ps root mean square (rms), which successfully meets the application requirement. All the FEE modules have been fabricated and installed for the LHAASO WCDA from 2018 to 2020, and the initial commissioning operation indicates that the FEEs function well.

Abstract

INTRODUCTION: We have demonstrated in a multicenter cohort that the COVID-19 pandemic has led to a delay in intravenous thrombolysis (IVT) among stroke patients. Whether this delay contributes to meaningful short-term outcome differences in these patients warranted further exploration. METHODS: We conducted a nested observational cohort study of adult acute ischemic stroke patients receiving IVT from 9 comprehensive stroke centers across 7 U.S states. Patients admitted prior to the COVID-19 pandemic (1/1/2019-02/29/2020) were compared to patients admitted during the early pandemic (3/1/2020-7/31/2020). Multivariable logistic regression was used to estimate the effect of IVT delay on discharge to hospice or death, with treatment delay on admission during COVID-19 included as an interaction term. RESULTS: Of the 676 thrombolysed patients, the median age was 70 (IQR 58-81) years, 313 were female (46.3%), and the median NIHSS was 8 (IQR 4-16). Longer treatment delays were observed during COVID-19 (median 46 vs 38 min, p = 0.01) and were associated with higher in-hospital death/hospice discharge irrespective of admission period (OR per hour 1.08, 95% CI 1.01-1.17, p = 0.03). This effect was strengthened after multivariable adjustment (aOR 1.15, 95% CI 1.07-1.24, p < 0.001). There was no interaction of treatment delay on admission during COVID-19 (pinteraction = 0.65). Every one-hour delay in IVT was also associated with 7% lower odds of being discharged to home or acute inpatient rehabilitation facility (aOR 0.93, 95% CI 0.89-0.97, p < 0.001). CONCLUSION: Treatment delays observed during the COVID-19 pandemic led to greater early mortality and hospice care, with a lower probability of discharge to home/rehabilitation facility. There was no effect modification of treatment delay on admission during the pandemic, indicating that treatment delay at any time contributes similarly to these short-term outcomes.

Abstract

BackgroundThe mechanisms and outcomes in coronavirus disease (COVID-19)-associated stroke are unique from those of non-COVID-19 stroke.ObjectiveTo describe the efficacy and outcomes of acute revascularization of large vessel occlusion (LVO) in the setting of COVID-19 in an international cohort.MethodsWe conducted an international multicenter retrospective study of consecutively admitted patients with COVID-19 with concomitant acute LVO across 50 comprehensive stroke centers. Our control group constituted historical controls of patients presenting with LVO and receiving a mechanical thrombectomy between January 2018 and December 2020.ResultsThe total cohort was 575 patients with acute LVO; 194 patients had COVID-19 while 381 patients did not. Patients in the COVID-19 group were younger (62.5 vs 71.2; P < .001) and lacked vascular risk factors (49, 25.3% vs 54, 14.2%; P = .001). Modified thrombolysis in cerebral infarction 3 revascularization was less common in the COVID-19 group (74, 39.2% vs 252, 67.2%; P < .001). Poor functional outcome at discharge (defined as modified Ranklin Scale 3-6) was more common in the COVID-19 group (150, 79.8% vs 132, 66.7%; P = .004). COVID-19 was independently associated with a lower likelihood of achieving modified thrombolysis in cerebral infarction 3 (odds ratio [OR]: 0.4, 95% CI: 0.2-0.7; P < .001) and unfavorable outcomes (OR: 2.5, 95% CI: 1.4-4.5; P = .002).ConclusionCOVID-19 was an independent predictor of incomplete revascularization and poor outcomes in patients with stroke due to LVO. Patients with COVID-19 with LVO were younger, had fewer cerebrovascular risk factors, and suffered from higher morbidity/mortality rates.

Abstract

The extension of the cosmic-ray spectrum beyond 1 petaelectronvolt (PeV; 1015 electronvolts) indicates the existence of the so-called PeVatrons—cosmic-ray factories that accelerate particles to PeV energies. We need to locate and identify such objects to find the origin of Galactic cosmic rays1. The principal signature of both electron and proton PeVatrons is ultrahigh-energy (exceeding 100 TeV) γ radiation. Evidence of the presence of a proton PeVatron has been found in the Galactic Centre, according to the detection of a hard-spectrum radiation extending to 0.04 PeV (ref. 2). Although γ-rays with energies slightly higher than 0.1 PeV have been reported from a few objects in the Galactic plane3–6, unbiased identification and in-depth exploration of PeVatrons requires detection of γ-rays with energies well above 0.1 PeV. Here we report the detection of more than 530 photons at energies above 100 teraelectronvolts and up to 1.4 PeV from 12 ultrahigh-energy γ-ray sources with a statistical significance greater than seven standard deviations. Despite having several potential counterparts in their proximity, including pulsar wind nebulae, supernova remnants and star-forming regions, the PeVatrons responsible for the ultrahigh-energy γ-rays have not yet been firmly localized and identified (except for the Crab Nebula), leaving open the origin of these extreme accelerators. Observations of γ-rays with energies up to 1.4 PeV find that 12 sources in the Galaxy are PeVatrons, one of which is the Crab Nebula.

Supplemental Digital Content is available in the text., Background and Purpose: The pandemic caused by the novel coronavirus disease 2019 (COVID-19) has led to an unprecedented paradigm shift in medical care. We sought to evaluate whether the COVID-19 pandemic may have contributed to delays in acute stroke management at comprehensive stroke centers. Methods: Pooled clinical data of consecutive adult stroke patients from 14 US comprehensive stroke centers (January 1, 2019, to July 31, 2020) were queried. The rate of thrombolysis for nontransferred patients within the Target: Stroke goal of 60 minutes was compared between patients admitted from March 1, 2019, and July 31, 2019 (pre–COVID-19), and March 1, 2020, to July 31, 2020 (COVID-19). The time from arrival to imaging and treatment with thrombolysis or thrombectomy, as continuous variables, were also assessed. Results: Of the 2955 patients who met inclusion criteria, 1491 were admitted during the pre–COVID-19 period and 1464 were admitted during COVID-19, 15% of whom underwent intravenous thrombolysis. Patients treated during COVID-19 were at lower odds of receiving thrombolysis within 60 minutes of arrival (odds ratio, 0.61 [95% CI, 0.38–0.98]; P=0.04), with a median delay in door-to-needle time of 4 minutes (P=0.03). Abstract

The coronavirus disease 2019 pandemic has changed the way we engage patient care, with a move toward telemedicine-based health care encounters. Teleneurology is now being rapidly embraced by neurologists in clinics and hospitals nationwide but for many, this paradigm of care is unfamiliar. Exposure to telemedicine in neurology training programs is scarce despite previous calls to expand teleneurology education. Programs that provide a teleneurology curriculum have demonstrated increased proficiency, accuracy, and post-training utilization among their trainees. With the current changes in health care, broad incorporation of teleneurology education in resident and fellow training after this pandemic dissipates will only serve to improve trainee preparedness for independent practice.

Abstract

Introduction: The AHA 2014 statement on palliative and end-of-life (EOL) care in stroke recommended patient- and family-centered care and goals of care discussions. Despite a recurrent stroke risk of up to 39% at 10 years, stroke survivors have low advance directive completion rates. We conducted a survey characterizing the advance care planning (ACP) attitudes in our population of stroke clinic patients. Methods: We developed a survey based on validated surveys assessing ACP behaviors and engagement. The survey was provided to all TIA, ischemic and hemorrhagic stroke patients in our clinic between February and August 2021. Non-English-speaking patients and those unable to express preferences were excluded. Results: Surveys were provided to 88 stroke survivors with a 68.2% response rate (N=60). Patient characteristics reported in Table 1. Patients were racially and ethnically diverse and 54.2% had greater than high school education. Patients were also fairly independent, most living at home (96.7%) and ambulatory (63.3%). Most patients completed the survey a median of 43 days after their first stroke (63.3%). Less than half (48.3%) of patients had designated a medical decision maker but many (63.3%) had had conversations with family/decision makers about preferences for future medical care if they were to become very sick or near the EOL. However, only 46.7% felt ready to talk with a doctor about these preferences. More than half (53.3%) of patients indicated that they would like their stroke doctor’s participation if/when they were to have such a conversation. Conclusions: In our study group of diverse, younger and more independent stroke survivors, we found that many had had ACP conversations with surrogate decision makers and more than half wanted their stroke doctor to participate in future discussions. More work is needed to assess the generalizability of our findings and how ACP can be improved for stroke survivors who are at high risk for recurrent events.

Abstract

Introduction : Post‐stroke delirium (PSD) is associated with increased mortality and worse long‐term functional outcomes. Patients who receive reperfusion therapies in the hospital are frequently kept from regular sleep‐wake cycles the first 24 hrs after treatment, and this disruption could lead to an increase in PSD. In this study, we evaluate the effect of PSD on immediate and long‐term outcomes in AIS patients receiving reperfusion therapies. Methods : Between September 2019 and June 2021, patients diagnosed with AIS within 48 hrs of stroke onset were prospectively evaluated for PSD using the Confusion Assessment Method (CAM)‐ICU daily for the first eight days of their hospital stay. Patients with severe stroke and expected mortality within the first month at the time of admission or with severe aphasia unable to follow commands were excluded. Reperfusion therapies were defined as any IV thrombolytic, IA thrombolytic, or mechanical thrombectomy (MT). The primary outcome was considered a 90‐day mRS score of 0–2. Results : Of 179 patients assessed with the CAM‐ICU, 89 (49.7%) had PSD. We identified 94 patients that had undergone one or both reperfusion therapies; 52 (55.3%) had delirium. Patients who received tPa had a higher risk for delirium (42 vs 29, p = 0.04), but no difference was observed with MT (Table 1). Patients with PSD had a longer hospital length of stay and a higher median admission NIHSS. Patients with delirium who received tPA were more likely to be discharged to inpatient rehabilitation facilities than home (p‐value 0.004, OR 10.1 95%CI 2.1,48). No significant difference was found in 90‐day mRs scores of 0–2 in those with or without PSD. Conclusions : AIS patients with PSD after reperfusion therapy had no significant difference in 90‐day good outcomes despite having longer hospital admissions and being less likely to be discharged home. Further evaluation into how reperfusion therapies convey protection to patients is necessary.

Abstract

Design inspiration comes from the continuous stimulation of external information and the continuous accumulation of knowledge. In order to obtain an ideal design inspiration from nature, researchers have proposed a large number of biological information retrieval and knowledge acquisition methods. But how to purposefully acquire valuable biological knowledge in order to effectively stimulate design inspiration and produce the novel and feasible designs idea is still an urgent problem to be solved. This paper proposes a method for acquiring valuable biological knowledge to efficiently stimulate inspiration and quickly conceive solutions in engineering design. First, keywords, such as the functional requirements and key components of design objects, are selected as the engineering terminologies. Next, biological keywords related to the engineering terminologies are searched from the biological dictionary and biology websites. Then in order to retrieve enough biological knowledge, these biological keywords are expanded manually and automatically respectively based on Thesaurus Webpage and WordNet database, and expanded keywords are filtered according to repeated words and different forms of the same words. Finally, in the biological knowledge base, biological keywords that had been filtered are used to obtain biological knowledge with Python web crawler programming. Through an example of application for ship equipment, the effectiveness of the method is verified.

Abstract

The spin-polarized bulk states in the centrosymmetric nodal-line semimetal HfSiS were observed by spin- and angle-resolved photoelectron spectroscopy. Combining with density functional theory calculations and the tight-binding model, the layer-locked spin states' nature of the observed $\mathrm{\ensuremath{\Lambda}}$-shaped bulk bands was revealed and the tight-binding model shows the importance of the next-nearest-neighbor term of spin-orbit coupling in the spin reversal of the bands along the $\overline{\mathrm{\ensuremath{\Gamma}}}\text{\ensuremath{-}}\overline{\mathit{M}}$ direction. Furthermore, the topological essence of attractive Dirac node arc states was confirmed.

Abstract

We report the discovery of a new unidentified extended $\gamma$-ray source in the Galactic plane named LHAASO J0341+5258 with a pre-trial significance of 8.2 standard deviations above 25 TeV. The best fit position is R.A.$=55.34^{\circ}\pm0.11^{\circ}$ and Dec$=52.97^{\circ}\pm0.07^{\circ}$. The angular size of LHAASO J0341+5258 is $0.29^\circ \pm 0.06^\circ_{stat} \pm0.02^\circ_{sys}$. The flux above 25 TeV is about $20\%$ of the flux of Crab Nebula. Although a power-law fit of the spectrum from 10 TeV to 200 TeV with the photon index $\alpha=2.98 \pm 0.19_{stat} \pm 0.02_{sys}$ is not excluded, the LHAASO data together with the flux upper limit at 10 GeV set by the Fermi LAT observation, indicate a noticeable steepening of an initially hard power-law spectrum %($\alpha \leq 1.75$) spectrum with a cutoff at $\approx 50$ TeV. We briefly discuss the origin of UHE gamma-rays. The lack of an energetic pulsar and a young SNR inside or in the vicinity of LHAASO J0341+5258 challenge, but do not exclude both the leptonic and hadronic scenarios of gamma-ray production., Comment: Accepted by APJL

Abstract

Recently, the LHAASO Collaboration published the detection of 12 ultrahigh-energy γ-ray sources above 100 TeV, with the highest energy photon reaching 1.4 PeV. The first detection of PeV γ rays from astrophysical sources may provide a very sensitive probe of the effect of the Lorentz invariance violation (LIV), which results in decay of high-energy γ rays in the superluminal scenario and hence a sharp cutoff of the energy spectrum. Two highest energy sources are studied in this work. No signature of the existence of the LIV is found in their energy spectra, and the lower limits on the LIV energy scale are derived. Our results show that the first-order LIV energy scale should be higher than about 10^{5} times the Planck scale M_{Pl} and that the second-order LIV scale is10^{-3}M_{Pl}. Both limits improve by at least one order of magnitude the previous results.

Abstract

Stress-related peptide corticotropin-releasing factor (CRF) and CRF-related peptides are distributed in the peripheral viscera such as the bladder. We investigated the contribution of psychological stress (PS) and CRF on bladder function. Male rats received sham stress (SS) or PS using a communication box method for 120 min every day for 7 days. One group of rats received the intraperitoneal CRF-R1 antagonist antalarmin for 7 days during stress exposure. Mean voided volume per micturition was significantly lower in PS rats compared to SS rats, which was antagonized by antalarmin treatment. Increases in plasma and bladder CRF, and mRNA expressions of bladder CRF, CRF-R1, and M2/3 muscarinic receptors, were found in PS rats. CRF did not influence bladder contraction in itself; however, stress increased the response of muscarinic contraction of bladder strips. These changes were antagonized by antalarmin treatment. In conclusion, PS reinforces M3 receptor-mediated contractions via CRF-R1, resulting in bladder storage dysfunction.

Abstract

Introduction: Our telestroke (TS) network instituted a regional transfer protocol (RTP) that allows for stroke patients in need of higher level of care to be pre-accepted and transferred to the nearest appropriate comprehensive stroke center (CSC). We studied the impact of the RTP on resource utilization and time metrics in patients transferred for evaluation of intra-arterial thrombectomy (IAT). Before the RTP, all potential IAT patients were transferred to one central CSC. After the RTP was initiated, the network had the capability to transfer to two additional CSCs within the same health system that are strategically located in the Houston area. Methods: We identified patients evaluated via TS in spoke emergency rooms that were subsequently transferred for IAT evaluation from 1/1/2016 to 12/31/2017 - one year prior and one year after the RTP. Baseline demographic characteristics, transfer and IAT metrics, and outcomes were compared for the two time periods. Results: Of 220 patients, 102 patients were transferred pre-RTP, and 120 were transferred to the three CSCs post-RTP. There were no significant differences in baseline characteristics, except fewer patients received tPA post-RTP (Table 1). In total, 30 patients (29%) pre-RTP and 42 patients (35%) post-RTP underwent IAT (p=0.38). Post-RTP, there was a trend toward faster travel times (median 40 vs 32 minutes, p=.07) and transfer initiation times to hub arrival times (median 109 vs 100.5 minutes, p=0.09). Door to groin puncture times were not statistically different between the two time periods. Post-RTP patients had a significantly shorter length of stay (median 6 vs 5 days, p=0.03). Conclusions: Regional transfer protocols can potentially help reduce transfer times and length of stay for stroke patients at CSCs that were initially seen by TS at community hospitals; however, larger sample size is needed to study its impact on other IAT-related metrics and clinical outcomes.

Abstract

We report the discovery of an extended very-high-energy (VHE) gamma-ray source around the location of the middle-aged (207.8 kyr) pulsar PSR J0622+3749 with the Large High-Altitude Air Shower Observatory (LHAASO). The source is detected with a significance of 8.2σ for E25 TeV assuming a Gaussian template. The best-fit location is (right ascension, declination) =(95.47°±0.11°,37.92°±0.09°), and the extension is 0.40°±0.07°. The energy spectrum can be described by a power-law spectrum with an index of -2.92±0.17_{stat}±0.02_{sys}. No clear extended multiwavelength counterpart of the LHAASO source has been found from the radio to sub-TeV bands. The LHAASO observations are consistent with the scenario that VHE electrons escaped from the pulsar, diffused in the interstellar medium, and scattered the interstellar radiation field. If interpreted as the pulsar halo scenario, the diffusion coefficient, inferred for electrons with median energies of ∼160 TeV, is consistent with those obtained from the extended halos around Geminga and Monogem and much smaller than that derived from cosmic ray secondaries. The LHAASO discovery of this source thus likely enriches the class of so-called pulsar halos and confirms that high-energy particles generally diffuse very slowly in the disturbed medium around pulsars.

Abstract

Summary Unconventional hydrocarbons have been considered as important complementary resources to conventional resources, which have been intensely investigated in recent years. Pore microstructure, especially of tight reservoirs, has significant effect on flow capacity, storage capability and recovery efficiency. A comprehensive understanding of pore microstructure is crucial for the prediction of transport property, and thus provides some basis for the evaluation of tight oil potential area. Three-dimensional features of pore network were studied by X-ray CT data analysis, where geometrical and topological parameters were calculated. On the basis of these quantitative analyses, numerical simulations were conducted for demonstrating transport capability.

Abstract

© 2020 World Stroke Organization., [Background]: Severe acute respiratory syndrome-coronavirus-2 (SARS-CoV-2) has been associated with a significant risk of thrombotic events in critically ill patients. [Aim]: To summarize the findings of a multinational observational cohort of patients with SARS-CoV-2 and cerebrovascular disease. [Methods]: Retrospective observational cohort of consecutive adults evaluated in the emergency department and/or admitted with coronavirus disease 2019 (COVID-19) across 31 hospitals in four countries (1 February 2020–16 June 2020). The primary outcome was the incidence rate of cerebrovascular events, inclusive of acute ischemic stroke, intracranial hemorrhages (ICH), and cortical vein and/or sinus thrombosis (CVST). [Results]: Of the 14,483 patients with laboratory-confirmed SARS-CoV-2, 172 were diagnosed with an acute cerebrovascular event (1.13% of cohort; 1130/100,000 patients, 95%CI 970–1320/100,000), 68/171 (40.5%) were female and 96/172 (55.8%) were between the ages 60 and 79 years. Of these, 156 had acute ischemic stroke (1.08%; 1080/100,000 95%CI 920–1260/100,000), 28 ICH (0.19%; 190/100,000 95%CI 130–280/100,000), and 3 with CVST (0.02%; 20/100,000, 95%CI 4–60/100,000). The in-hospital mortality rate for SARS-CoV-2-associated stroke was 38.1% and for ICH 58.3%. After adjusting for clustering by site and age, baseline stroke severity, and all predictors of in-hospital mortality found in univariate regression (p < 0.1: male sex, tobacco use, arrival by emergency medical services, lower platelet and lymphocyte counts, and intracranial occlusion), cryptogenic stroke mechanism (aOR 5.01, 95%CI 1.63–15.44, p < 0.01), older age (aOR 1.78, 95%CI 1.07–2.94, p ¼ 0.03), and lower lymphocyte count on admission (aOR 0.58, 95%CI 0.34–0.98, p ¼ 0.04) were the only independent predictors of mortality among patients with stroke and COVID-19. [Conclusions]: COVID-19 is associated with a small but significant risk of clinically relevant cerebrovascular events, particularly ischemic stroke. The mortality rate is high for COVID-19-associated cerebrovascular complications; therefore, aggressive monitoring and early intervention should be pursued to mitigate poor outcomes.

Abstract

We report the discovery of a UHE gamma-ray source, LHAASO J2108+5157, by analyzing the LHAASO-KM2A data of 308.33 live days. Significant excess of gamma-ray induced showers is observed in both energy bands of 25-100 TeV and $\gt$100 TeV with 9.5 sigma and 8.5 sigma, respectively. This source is not significantly favored as an extensive source with the angular extension smaller than the point-spread function of KM2A. The measured energy spectrum from 20 to 200 TeV can be approximately described by a power-law function with an index of -2.83$\pm$ 0.18stat. A harder spectrum is demanded at lower energies considering the flux upper limit set by Fermi-LAT observations. The position of the gamma-ray emission is correlated with a giant molecular cloud, which favors a hadronic origin. Abstract

Introduction: Community emergency departments (EDs) often transfer ischemic stroke (IS) patients for lack of neurology coverage, burdening patients and accepting facilities. Telestroke (TS) improves acute stroke care access, but data lack on inpatient teleneurology follow-up (TNF) care. We hypothesized that IS patients evaluated in the ED via TS, then admitted and managed by TNF, have similar outcomes to those seen by in-person neurology follow-up (IPF) after admission. Methods: In our spoke EDs, 4069 IS patients were seen via TS (9/2015-12/2018). Figure 1 demonstrates hospital stroke designations and patient triage patterns. Transfer was at ED discretion. We compared baseline demographics, clinical characteristics, and hospital outcomes in patients with TNF vs IPF. Results: There were 447 (23%) patients with TNF and 1459 (77%) IPF. Both groups presented with similar stroke severity (Table 1). In multivariate analysis, there were no significant differences in discharge disposition, stroke readmission rates, or 90-day mRS; length of stay was shorter with TNF. tPA-only patients showed no differences in outcomes and similar complication rates. TNF patients less likely received tPA or endovascular therapy. TNF resulted in a 3% transfer rate for higher level of care. There remained no difference in outcomes in a sub-analysis without CSCs. A higher proportion of non-Hispanic Black patients and lower proportion of Hispanic patients in the TNF group were possibly due to spoke demographics. Conclusion: TNF resulted in comparable outcomes to IPF and few transfers after admission. For select IS patients, TNF provides an alternative to transfer for hospitals lacking neurology coverage.

Abstract

Introduction: Recent studies support tPA for acute ischemic stroke (AIS) patients presenting beyond 4.5 hours from last known well (LKW), if established infarct is not evident on advanced imaging. Many community hospitals, where AIS patients may be managed via telestroke (TS), lack advanced imaging capability and hesitate to administer off-label tPA. In our TS network, physicians adhere to an extended window tPA (EW) protocol also used in the hub emergency room; eligibility includes NIHSS≤25, Methods: We identified 1,150 AIS patients who received tPA via TS (9/2015-12/2018). We compared baseline characteristics between patients who received EW (arrival >4.5 hrs) and those who received standard window tPA (SW, arrival ≤4.5 hrs). We explored clinical outcomes and describe incidence of adverse effects from tPA. Results: Forty patients received EW, with median ASPECTS of 9 (Q1-Q3: 9-10). Median LKW to arrival time was 491 mins with EW and 66 mins with SW (p Conclusions: Our data suggest that EW is safe via TS for select patients with favorable CT, in settings that may lack advanced imaging capability. A specified mismatch between NIHSS and acute ischemia on plain CT is not part of our EW protocol, however EW is more likely given for severe stroke in our TS network. Small sample size warrants further study on clinical outcomes.

Abstract

The Water Cherenkov Detector Array (WCDA) is a major component of the Large High Altitude Air Shower Array Observatory (LHAASO), a new generation cosmic-ray experiment with unprecedented sensitivity, currently under construction. The WCDA is aimed at the study of TeV $\gamma$-rays. In order to evaluate the prospects of searching for TeV $\gamma$-ray sources with the WCDA, we present in this paper a projection for the one-year sensitivity of the WCDA to TeV $\gamma$-ray sources from TeVCat (footnote: http://tevcat.uchicago.edu) using an all-sky approach. Out of 128 TeVCat sources observable to the WCDA up to a zenith angle of $45^\circ$, we estimate that 42 would be detectable for one year of observations at a median energy of 1 TeV. Abstract

The main scientific goal of the LHAASO WFCTA experiment is to measure the cosmic ray energy spectra and composition from 10 TeV to 1 EeV. Cherenkov photons in the extensive air shower measured by the SiPM camera of Cherenkov telescopes can be used to reconstruct the cosmic ray energy. The absolute calibration of the camera is a crucial step to achieve the accurate measurement of the cosmic ray energy spectrum. A multi-wavelength cylindrical illuminator based on LEDs is developed and mounted inside the telescope to calibrate and monitor the camera, and the illuminator’s stability is better than 0.5% under the temperature variation from -26 to 26 ° C. A portable probe with a single photoelectron resolution of 21.6% is developed. After calibration by National Institute of Metrology, China (NIM), the probe is taken to the LHAASO site to measure the absolute photon density of the cylindrical illuminator inside the telescope. Based on the illuminator with known photon density, the photon conversion factor of the camera can be calibrated, and the overall calibration uncertainty is less than 2.6%.

Abstract

Resistive Plate Chambers have largely been used in High Energy Physics and Cosmic Ray research. In view of using this detector for calorimetry applications it is important to know the maximum measurable particle density, or its intrinsic linearity limit, which is tightly related to the dimension of the discharge region. In this paper we report the results of measurements performed at the Beam Test Facility (INFN National Laboratory of Frascati, Italy) where the intrinsic linearity of bakelite RPCs operated in streamer mode has been tested at different impinging particle densities.

Abstract

An end-to-end calibration method, which is different from the traditional way, has been employed in WFCTA prototype experiment based on N 2 laser. As we known, the calculation of the expected and the observed light fluxes at the front of the aperture of the telescope are crucial in the such method. To calculate predicted light flux accurately, the propagation and scattering of photons in the atmosphere have been considered in detail. In order to get the detector effective areas, which will be used in the transformation of ADC signals to the observed light flux, a ray trace procedure, considering all details of detector response, has been performed for all incident direction of photons in field of view of the detector. By matching the observed and expected light flux at the entrance of the telescope, the overall calibration gains of two telescopes can be obtained. During the test run of the calibration, a overall calibration gain around G = 0 . 38 A D C c o u n t s ∕ p h o t o n and G = 0 . 36 A D C c o u n t s ∕ p h o t o n for WFCT01 and WFCT02 are given, respectively. Furthermore, in view of all uncertainty sources, systematic uncertainties for the calibration have also been estimated. They are around the level of ( − 7 . 3 % , + 7 . 8 % ) . The calibration results achieved by end-to-end method and the traditional way are in consistent at the level of 10%, which is within the range of uncertainties. Since the end-to-end and traditional calibration methods have their own advantages, the performance of the combination of these two calibration methods has also been discussed in the end.

Abstract

The first Water Cherenkov detector of the LHAASO experiment (WCDA-1) has been operating since April 2019. The data for the first year have been analyzed to test its performance by observing the Crab Nebula as a standard candle. The WCDA-1 achieves a sensitivity of 65 mCU per year, with a statistical threshold of 5 . To accomplish this, a 97.7% cosmic-ray background rejection rate around 1 TeV and 99.8% around 6 TeV with an approximate photon acceptance of 50% is achieved after applying an algorithm to separate gamma-induced showers. The angular resolution is measured using the Crab Nebula as a point source to be approximately 0.45° at 1 TeV and better than 0.2° above 6 TeV, with a pointing accuracy better than 0.05°. These values all match the design specifications. The energy resolution is found to be 33% for gamma rays around 6 TeV. The spectral energy distribution of the Crab Nebula in the range from 500 GeV to 15.8 TeV is measured and found to be in agreement with the results from other TeV gamma ray observatories.

Abstract

In 2011 ARGO-YBJ experiment has reported a work to study the absolute rigidity scale of the primary cosmic ray particles based on the Moon's shadow observation. Given the progress in high energy hadronic interaction models with LHC data, in cosmic ray chemical composition measurement and in experimental data accumulation, more updates can be researched. This paper aims to further disentangle the composition dependence in absolute-energy-scale calibration by using specific moon-shadow data which mainly is comprised of light component cosmic rays. Results show that, 17% energy scale error is estimated from 3 TeV to 50 TeV. To validate the performance of this technique, the light component cosmic ray spectrum in the same energy region is shown. Abstract

Background and Purpose— There is uncertainty among many emergency medicine physicians about the decision to give intravenous tPA (tissue-type plasminogen activator), which limits its use. A checklist approach has been suggested as a solution. We compared agreement on tPA treatment in suspected acute ischemic stroke patients between emergency medicine residents (EMRs) using a checklist and vascular neurology fellows (VNFs). Methods— Every suspected acute stroke patient brought to our comprehensive stroke center emergency room within 4.5 hours from symptom onset was prospectively evaluated simultaneously and independently by VNFs and EMRs. The latter used a tPA screening checklist, which included guideline exclusion criteria to help with their treatment decision. Agreement was determined using kappa (k) statistics. Results— Over 6 months, 60 patients were enrolled; 10% large vessel atherosclerosis, 18% cardioembolism, 12% small vessel, 12% cryptogenic, and 47% mimic. Forty-two percent were deemed tPA eligible by the EMR, 30% by the VNF, and 37% by the vascular neurology faculty. There were no complications in any tPA-treated patients. Agreement was substantial between EMR and VNF (κ=0.68 [95% CI, 0.49–0.87]) and between EMR and vascular neurology faculty (κ=0.69 [95% CI, 0.50–0.87]). Stroke mimics were the main cause of disagreement between EMR and VNF (κ=0.24 [95% CI, −0.15 to 0.63]) and between EMR and vascular neurology faculty (κ=0.35 [95% CI, −0.08 to 0.78]). Conclusions— Our data suggest that with the aid of a checklist, EMRs can accurately treat stroke patients with tPA. Areas for improvement include recognition of stroke mimics. Further studies are warranted to evaluate checklist-enhanced tPA treatment to allay emergency medicine physician uncertainty and expand the use of tPA.

Abstract

The Large High Altitude Air Shower Observatory (LHAASO) is built at an altitude of 4410 meters in Daocheng, Sichuan Province, China. As one of its major components, the Water Cherenkov Detector Array (WCDA) will focus on surveying the Northern sky for gamma-ray sources in a wide energy range. Because most of the triggered events are induced from hadronic cosmic-rays, suppressing a large number of background events is very important for LHAASO-WCDA. In this work, several sensitive parameters are chosen for the single parameter gamma/hadron discrimination based on topology characteristics of showers. Different methods of multivariate analysis are also used in this study. Results from simulation data show that multivariate analysis can improve the separation significantly compared to single traditional methods. Some preliminary results using the separation technique on experimental data are introduced.

Abstract

With high duty cycle and wide aperture, the Large High Altitude Air Shower Observatory - Water Cherenkov Detector Array(LHAASO-WCDA) can conduct an unbiased gamma-ray sky survey in the energy range from a few hundred GeV to 100 TeV. The sensitivity of WCDA is as high as a few percent of Crab units, which allow us to monitor the VHE variability of blazars. The LHAASO Collaboration has developed an online monitoring program to monitor the extragalactic VHE flare in the WCDA’s field of view. Once a flare exceeding the threshold is detected, an alert will be sent to other instruments automatically, and a follow-up multiwavelength observation could be carried out. In this paper, the current status of the monitor, the candidate sources, background estimation method, data reduction, testing running result are presented.

Abstract

The hadronic interaction of cosmic rays with solar atmosphere can produce high energy gamma-rays. The gamma-ray luminosity is correlated both with the flux of primary cosmic rays and the intensity of the solar magnetic field. The gamma-rays below 200 GeV have been observed by Fermi without any evident energy cutoff. The bright gamma-ray flux above 100 GeV has been detected only during solar minimum. The only available data in the TeV range come from the HAWC observations, however, outside the solar minimum. The ARGO-YBJ data set has been used to search for sub-TeV/TeV gamma-rays from the Sun during the solar minimum from 2008 to 2010, the same time period covered by the Fermi data. A suitable model containing the Sun shadow, solar disk emission, and inverse-Compton emission has been developed, and the chi-square minimization method was used to quantitatively estimate the disk gamma-ray signal. The result shows that no significant gamma-ray signal is detected and upper limits to the gamma-ray flux at 0.3-7 TeV are set at the 95% confidence level. In the low energy range these limits are consistent with the extrapolation of the Fermi-LAT measurements taken during solar minimum and are compatible with a softening of the gamma-ray spectrum below 1 TeV. They also provide an experimental upper bound to any solar disk emission at TeV energies. Models of dark matter annihilation via long-lived mediators predicting gamma-ray fluxes >10-7 GeV cm-2 s-1 below 1 TeV are ruled out by the ARGO-YBJ limits.

Eventual etiology was: 10% large vessel atherosclerosis, 18% cardioembolism, 12% small vessel, 12% cryptogenic, and 48% mimic. 25 (42%) were deemed tPA eligible by the EMR, 18 (30%) by the VNF, and 22 (37%) by the VN faculty. Agreement was substantial between EMR and VNF (κ=0.68, 95% CI 0.49 to 0.87) and between EMR and VN faculty (κ=0.69, 95% CI 0.50 to 0.87]). Age, NIHSS, premorbid mRS, gender and race had no effect on agreement. There were no complications in any tPA treated patients. Stroke mimics were the main cause of disagreement between EMR and VNF (κ=0.24, 95% CI -0.15 to 0.63) and between EMR and VN faculty (κ=0.35, 95% CI -0.08 to 0.78). Conclusions: Our data suggest that with the aid of a checklist, emergency medicine physicians can reliably treat acute stroke patients with tPA. Areas for improvement include recognition of stroke mimics. Further studies are warranted to evaluate checklist enhanced tPA treatment in order to allay emergency medicine physician uncertainty and expand the use of tPA.

Abstract

The LHAASO-WFCTA experiment, which aims to observe cosmic rays in the sub-EeV range using the fluorescence technique, uses a new generation of high-performance telescopes. To ensure that the experiment has excellent detection capability associated with the measurement of the energy spectrum, the primary composition of cosmic rays, and so on, an accurate geometrical reconstruction of air-shower events is fundamental. This paper describes the development and testing of geometrical reconstruction for stereo viewed events using the WFCTA (Wide Field of view Cherenkov/Fluorescence Telescope Array) detectors. Two approaches, which take full advantage of the WFCTA detectors, are investigated. One is the stereo-angular method, which uses the pointing of triggered SiPMs in the shower trajectory, and the other is the stereo-timing method, which uses the triggering time of the fired SiPMs. The results show that both methods have good geometrical resolution; the resolution of the stereo-timing method is slightly better than the stereo-angular method because the resolution of the latter is slightly limited by the shower track length.

Abstract

BackgroundUnprecedented workflow shifts during the coronavirus disease 2019 (COVID-19) pandemic have contributed to delays in acute care delivery, but whether it adversely affected endovascular thrombectomy metrics in acute large vessel occlusion (LVO) is unknown.MethodsWe performed a retrospective review of observational data from 14 comprehensive stroke centers in nine US states with acute LVO. EVT metrics were compared between March to July 2019 against March to July 2020 (primary analysis), and between state-specific pre-peak and peak COVID-19 months (secondary analysis), with multivariable adjustment.ResultsOf the 1364 patients included in the primary analysis (51% female, median NIHSS 14 [IQR 7–21], and 74% of whom underwent EVT), there was no difference in the primary outcome of door-to-puncture (DTP) time between the 2019 control period and the COVID-19 period (median 71 vs 67 min, P=0.10). After adjustment for variables associated with faster DTP, and clustering by site, there remained a trend toward shorter DTP during the pandemic (βadj=-73.2, 95% CI −153.8–7.4, Pp=0.07). There was no difference in DTP times according to local COVID-19 peaks vs pre-peak months in unadjusted or adjusted multivariable regression (βadj=-3.85, 95% CI −36.9–29.2, P=0.80). In this final multivariable model (secondary analysis), faster DTP times were significantly associated with transfer from an outside institution (βadj=-46.44, 95% CI −62.8 to – -30.0, Padj=-2.15, 95% CI −4.2to – -0.1, P=0.05).ConclusionsIn this multi-center study, there was no delay in EVT among patients treated for intracranial occlusion during the COVID-19 era compared with the pre-COVID era.
