1. Establishment of enzyme-linked immunosorbent assays for lipoprotein lipase with newly developed antibodies.

Abstract

We developed eight new antibodies against lipoprotein lipase (LPL), which included polyclonal antibodies raised against recombinant human LPL produced by transformant cells and two synthetic peptides corresponding to either amino (N)- or carboxy (C)-terminus of human LPL. With these antibodies, we established three effective sandwich enzyme-linked immunosorbent assays (ELISAs) for LPL, which enabled us to examine LPL mass not only in the postheparin plasma from human, rat, mouse, and guinea pig but also in the media and lysates of cultured cells. All of the developed antibodies showed high affinities for LPL, but their binding to LPL did not always influence the lipolytic activity of the enzyme. Interestingly, although the anti-C-terminus antibody should bind to a common epitope of human and mouse LPL, its binding selectively suppressed only human LPL activity. Because amino acid sequence surrounding the epitope is common to both LPLs, difference in the sequence outside the epitope will contribute to the selective suppression of LPL activity by the antibody. Our results also suggested that both termini of LPL would be exposed on the surface of the molecule because they were fully accessible to antibodies and that the N-terminus of LPL would be functionally less important because binding of the anti-N-terminus antibody did not affect human LPL activity. The ELISAs were further utilized to demonstrate the presence of C-terminus truncated LPL protein in the postheparin plasma of an LPL-deficient patient, to map an epitope of the anti-C-terminus antibody within residues 433-436, and to gain insight into the structure-function relationship of the LPL molecule.(ABSTRACT TRUNCATED AT 250 WORDS)

Abstract

A rapid detection method was developed for DNA polymorphisms in the human lipoprotein lipase (LPL) gene. The examined polymorphisms include an A-C transversion in the 5'-region of intron 3, a T-G transversion that occurs within a Hind III site of intron 8, and the previously described C-T transition that causes a Pvu II polymorphism in intron 6. Gene fragments encompassing each polymorphic site were amplified by the polymerase chain reaction (PCR) and digested with an appropriate restriction enzyme whose recognition site was either naturally affected by the polymorphism or artificially created with a mismatched PCR-primer. According to the digestion profiles, genotypes were unambiguously distinguished. With this method, respective allelic frequencies were determined for 50 or 70 normal subjects. The procedure will facilitate LPL genotyping in the large population.

Abstract

Many spatio-temporal systems can undergo instabilities, leading to the spontaneous formation of spatial structures (patterns). However, a range of cases exist for which the pattern itself is not directly visible because of technical or fundamental reasons. This is the case for the spontaneous formation of millimeter-scale patterns appearing inside relativistic electron bunches of accelerators. We demonstrate in this case how the study of responses to sine external perturbations can be used as a ‘probe’ to deduce the characteristic wavenumber of the pattern formation process. Experiments are performed in the UVSOR-II electron storage ring when the electron bunch is subjected to so-called microbunching instability, and the sine perturbations are provided by an external laser. The response is constituted of pulses of coherent synchrotron radiation, whose amplitude depends on the perturbation wavenumber. Experimental results on the dynamics are compared to numerical calculations obtained using a Vlasov–Fokker–Planck model.

4. Improved tritium retention modeling with reaction-diffusion code TMAP and bulk depth profiling capability

Abstract

ITER-grade tungsten (W) specimens were exposed to similar deuterium (D) plasma condition (ion flux density of 6.0 × 1021 D m − 2s−1, D ion fluence of 5.0 × 1025 D m − 2) at the surface temperature of 623 K. Thermal desorption spectroscopy was used to measure total D retention in one W specimen after D plasma exposure. Glow-discharge optical emission spectroscopy was used to measure D depth profiling from the other W specimen exposed to the similar condition, and deep D trapping up to 25 μm was observed. When the normalized D depth profile was used with a reaction-diffusion code TMAP7 to model experimental D desorption behavior, an excellent agreement to experimental results was achieved. The modeling results suggested that the predominate mechanism of the deep D trapping might be D trapping in intrinsic intergranular cracks in ITER-grade W. 6. Deuterium retention and blistering in tungsten foils

Abstract

To investigate deuterium retention and the onset of blistering, deuterium was implanted in cold rolled tungsten foils at fluences ranging from 3 ×1020 to 3 ×1022D/m2. Ion energies were 300eV and 2000eV in order to be below and above the tungsten theoretical damage energy threshold. While energy dependent phenomena were observed, blistering occurs regardless of ion energy. Both plastically and elastically deformed blisters were found, as manifest in before and after micrographs. The fraction of plastically deformed blisters did not saturate at the fluences used in these studies. However, the size of the largest blister that relaxed during TDS does saturate at ∼7µm. A simple conceptual model is presented, which proposes that the deuterium released from elastically deformed blisters appears at ∼600K in the thermal desorption spectra, which is consistent with large vacancy clusters. 7. Tritium decay helium-3 effects in tungsten

Abstract

Tritium (T) implanted by plasmas diffuses into bulk material, especially rapidly at elevated temperatures, and becomes trapped in neutron radiation-induced defects in materials that act as trapping sites for the tritium. The trapped tritium atoms will decay to produce helium-3 (3He) atoms at a half-life of 12.3 years. 3He has a large cross section for absorbing thermal neutrons, which after absorbing a neutron produces hydrogen (H) and tritium ions with a combined kinetic energy of 0.76 MeV through the 3He(n,H)T nuclear reaction. The purpose of this paper is to quantify the 3He produced in tungsten by tritium decay compared to the neutron-induced helium-4 (4He) produced in tungsten. This is important given the fact that helium in materials not only creates microstructural damage in the bulk of the material but alters surface morphology of the material effecting plasma-surface interaction process (e.g. material evolution, erosion and tritium behavior) of plasma-facing component materials. Effects of tritium decay 3He in tungsten are investigated here with a simple model that predicts quantity of 3He produced in a fusion DEMO FW based on a neutron energy spectrum found in literature. This study reveals that: (1) helium-3 concentration was equilibrated to ∼6% of initial/trapped tritium concentration, (2) tritium concentration remained approximately constant (94% of initial tritium concentration), and (3) displacement damage from 3He(n,H)T nuclear reaction became >1 dpa/year in DEMO FW. 9. Annealing effects on deuterium retention behavior in damaged tungsten

Abstract

Effects of annealing after/under iron (Fe) ion irradiation on deuterium (D) retention behavior in tungsten (W) were studied. The D2 TDS spectra as a function of heating temperature for 0.1dpa damaged W showed that the D retention was clearly decreased as the annealing temperature was increased. In particular, the desorption of D trapped by voids was largely reduced by annealing at 1173K. The TEM observation indicated that the size of dislocation loops was clearly grown, and its density was decreased by the annealing above 573K. After annealing at 1173K, almost all the dislocation loops were recovered. The results of positron annihilation spectroscopy suggested that the density of vacancy-type defects such as voids, was decreased as the annealing temperature was increased, while its size was increased, indicating that the D retention was reduced by the recovery of the voids. Furthermore, it was found that the desorption temperature of D trapped by the voids for damaged W above 0.3dpa was shifted toward higher temperature side. These results lead to a conclusion that the D retention behavior is controlled by defect density. The D retention in the samples annealed during irradiation was less than that annealed after irradiation. This result shows that defects would be quickly annihilated before stabilization by annealing during irradiation.

Abstract

This study aims to evaluate height accuracy of two datasets obtained by spaceborne optical instruments of a digital elevation data for a large-scale area. The digital surface model (DSM) was generated by the Panchromatic Remote-sensing Instrument for Stereo Mapping (PRISM) onboard the Advanced Land Observing Satellite (ALOS, nicknamed 'Daichi'), and the global digital elevation model (DEM) version 2 (GDEM-2) was derived from the Advanced Spaceborne Thermal Emission and Reflection Radiometer (ASTER) onboard NASA's TERRA satellite. The test site of this study was the entire country of Bhutan, which is located on the southern slopes of the eastern Himalayas. Bhutan is not a large country, covering about 330 km from east to west, and 170 km from north to south; however, it has large height variation from 200 m to more than 7,000 m. This therefore makes it very interesting for validating digital topographic information in terms of national scale generation as well as wide height range. Regarding the reference data, field surveys were conducted in 2010 and 2011, and collected ground control points by a global positioning system were used for evaluating precise height accuracies in point scale as check points (CPs), with a 3 arc-sec DEM created by the Shuttle Radar Topography Mission (SRTM-3) used to validate the wide region. The results confirmed a root mean square error of 8.1 m for PRISM DSM and 29.4 m for GDEM-2 by CPs.

Abstract

Spray pyrolysis is a method for preparing nanocomposite materials in the gas phase. Due to the ease with which the temperature in the furnace can be controlled, this method can produce more uniform nanoparticle sizes. This method can be used to synthesize ZnO nanoparticles. Additionally, due to the photocatalytic properties of the synthesized particles, they are used to decompose synthetic dye waste. Due to electron-hole recombination, ZnO’s photocatalytic properties are limited. A doping process with various metals was used to enhance the photocatalytic properties of ZnO nanoparticles. Ag is one of these metals. The addition of Ag metal to ZnO nanoparticles results in the formation of nanocomposites with preferable photocatalytic properties. However, the addition of Ag results in the aggregation of the obtained particles. As a result, the ZnO particles must be trapped in a matrix (for example, SiO2) to inhibit particle growth. This research aims to determine the effect of the SiO2 composition on the characteristics and photocatalytic properties of ZnO/Ag/SiO2 nanocomposite synthesized via spray pyrolysis. SEM (Scanning Electron Microscope) analysis was used to determine the morphology of the nanocomposites, XRD (X-ray Diffraction) analysis was used to determine the purity and degree of crystallization, FTIR (Fourier-transform Infrared) analysis was used to determine the functional groups, and UV-Vis Spectrophotometer analysis was used to determine the photocatalytic characteristics. According to the analysis results, the optimal photocatalytic activity was obtained at 1% SiO2, with a degradation rate of up to 47% of methylene blue and a rate constant (k) of 0.0086 L/min under UV light irradiation.

Abstract

Background & aims: Genome-wide association studies in primary biliary cholangitis (PBC) have failed to find X chromosome (chrX) variants associated with the disease. Here, we specifically explore the chrX contribution to PBC, a sexually dimorphic complex autoimmune disease. Methods: We performed a chrX-wide association study, including genotype data from 5 genome-wide association studies (from Italy, United Kingdom, Canada, China, and Japan; 5244 case patients and 11,875 control individuals). Results: Single-marker association analyses found approximately 100 loci displaying P < 5 × 10-4, with the most significant being a signal within the OTUD5 gene (rs3027490; P = 4.80 × 10-6; odds ratio [OR], 1.39; 95% confidence interval [CI], 1.028-1.88; Japanese cohort). Although the transethnic meta-analysis evidenced only a suggestive signal (rs2239452, mapping within the PIM2 gene; OR, 1.17; 95% CI, 1.09-1.26; P = 9.93 × 10-8), the population-specific meta-analysis showed a genome-wide significant locus in East Asian individuals pointing to the same region (rs7059064, mapping within the GRIPAP1 gene; P = 6.2 × 10-9; OR, 1.33; 95% CI, 1.21-1.46). Indeed, rs7059064 tags a unique linkage disequilibrium block including 7 genes: TIMM17B, PQBP1, PIM2, SLC35A2, OTUD5, KCND1, and GRIPAP1, as well as a superenhancer (GH0XJ048933 within OTUD5) targeting all these genes. GH0XJ048933 is also predicted to target FOXP3, the main T-regulatory cell lineage specification factor. Consistently, OTUD5 and FOXP3 RNA levels were up-regulated in PBC case patients (1.75- and 1.64-fold, respectively). Conclusions: This work represents the first comprehensive study, to our knowledge, of the chrX contribution to the genetics of an autoimmune liver disease and shows a novel PBC-related genome-wide significant locus.

Abstract

Objectives: Homologous recombination deficiency (HRD) is a well-known biomarker of PARP inhibitor sensitivity. The Cancer Genome Atlas (TCGA) has clarified that about 50% of high-grade serous ovarian cancer shows HRD. However, the frequency of HRD in Japanese patients with ovarian cancer remains unclear. The aim of JGOG3025 study (NCT03159572) is to identify the frequency of HR-associated gene mutations in ovarian cancer, especially non-serous histologic types. Methods: The JGOG3025 study is a collaborative prospective observational study involving 65 study sites throughout Japan. We recruited 996 patients who were clinically diagnosed with ovarian cancer before surgery from March 2017 to March 2019, and 711 patients were eligible for JGOG3025 criteria. After central pathological review, we used frozen tumor tissues to extract DNA and performed targeted sequencing for 51 HR-associated genes in 711 ovarian cancers (298 high-grade serous, 189 clear cell, 142 endometrioid, 15 mucinous, 4 low-grade serous, and 63 others). Results: To focus on pathogenic mutations of HR-associated genes which led to HRD, we used only loss-of-function (LOF) mutations (nonsense, readthrough, and frameshift) to perform subsequent analysis. At least one HR-associated gene was mutated in 105 high-grade serous (35.2%), 21 clear cell (11.1%), 36 endometrioid (25.4%), 1 low-grade serous (25.0%) carcinomas. Although 81.8% of ovarian cancers with BRCA1 LOF mutation were diagnosed as high-grade serous type, one case of mucinous carcinoma harbored BRCA1 c.1961delA. On the other hand, 71.4% of ovarian cancers harboring BRCA2 LOF mutation were high-grade serous type. Mutant allele frequencies of BRCA1 and RAD51D were apparently higher than that of other HR-associated genes, suggesting allelic imbalance. Conclusions: Our findings demonstrated that mutations in HR-associated genes were detected in not only high-grade serous but also non-serous histologic type ovarian cancers. To clarify the frequency of HRD in Japanese patients with ovarian cancer, we need to assess the pathogenicity of missense mutations and investigate the correlation between HR associated gene mutations and HRD score based on copy number assay.

Objectives: While malignant peritoneal cytology is not the component of the current cancer staging schema, the collection of a peritoneal cytologic sample at the time of hysterectomy-based surgical staging for endometrial cancer is highly endorsed by multiple societies and organizations. Suspicious peritoneal cytology refers to the result of peritoneal cytology testing that is insufficient in either quality or quantity for a definitive diagnosis of malignancy. This study examined characteristics and survival outcomes related to suspicious peritoneal cytology in endometrial cancer. Methods: A population-based retrospective study by querying the National Cancer Institute's Surveillance, Epidemiology, and End Results Program was conducted. A total of 41,229 women with stage I-III endometrial cancer who had peritoneal cytologic sampling at hysterectomy from 2010 to 2016 were examined. A Cox proportional hazard regression model and a competing risk analysis with Fine-Gray model were fitted to assess survival outcome related to suspicious peritoneal cytology. Results: Suspicious peritoneal cytology was seen in 702 (1.7%) cases. Abstract

Narrow-bandwidth photon beams in the x-ray and γ-ray energy ranges are expected to be applied in various fields. An energy recovery linac (ERL)-based laser Compton scattering (LCS) source employing a laser enhancement cavity can produce a high-flux and narrow-bandwidth photon beam. We conducted the first experiment of an ERL-based LCS source in combination with a laser enhancement cavity. We obtained LCS photons with an energy of 6.95±0.01 keV by colliding an electron beam of 20 MeV with a laser of 1064 nm wavelength. The photon flux at the interaction point was evaluated to be (2.6±0.1)×10^{7} photons/s with an average beam current of 58 μA and an average laser power of 10 kW. The energy bandwidth was evaluated to be 0.4% (rms) with an opening angle of 0.14 mrad. The technologies demonstrated in this experiment are applicable for future ERL-based LCS sources.

Abstract

PURPOSE The aim of this study was to investigate advantages of the left-handed ultrasonic shears technique in robotic gastrectomy for gastric cancer. METHODS We retrospectively analyzed 67 consecutive gastric cancer patients who underwent robotic gastrectomy. Fifty-six patients underwent gastrectomy with the left-handed ultrasonic shears technique (the left hand group), and 11 patients underwent surgery with the conventional approach (the conventional group). Intraoperative and postoperative outcomes were compared between the 2 groups. RESULTS Operative blood loss, morbidity, and mortality were similar between the 2 groups. We observed a trend toward a shorter operation time and higher number of retrieved lymph nodes in the left hand group compared with the conventional group. Abstract

Purpose Whereas fenestration at Fontan operation was reported to be effective for smooth initiation to Fontan circulation, subsequently developed aortopulmonary collateral arteries under the presence of remaining hypoxia were a matter of concern.So as our institutional policy, it is closed as much and early as possible (temporary fenestration). This study aimed to reveal long-term outcomes for patients at riskswho underwent Fontan operation with temporary fenestration. Methods Of 107 patients undergoing Fontan operation from 1995 to 2004, 79 patients (73.8%) were free from any risks (NR group) and 28 patients (26.2%) had risks for Fontan operation (R group). Hypoplastic left heart syndrome, heterotaxy syndrome with obstructive total anomalous of pulmonary venous connection, and individual cases with high preoperative pulmonary artery pressure (PAP), systemic ventricular end diastolic pressure (SVEDP) or low ejection fraction (SVEF) were defined as a risk for Fontan completion.Age at Fontan operation was older in R group (6.6 vs 2.3years-old, p = 0.0004). Preoperative PAP (12.8 vs 11.3mmHg, p = 0.03) and SVEDP (8.8 vs 7.1mmHg, p = 0.02) were higher and SVEF (53.7 vs 60.0%, p = 0.002) was lower in R group. Pulmonary vascular resistance (PVR) was not different. Fenestration was created in all patients in R group, then it was closed at the mean duration of 2.9 years after Fontan operation. Results Freedom from death or protein-losing enteropathy rate at 5, 10, and 15 years after Fontan operation were 97, 96, and 93% in NR group and 96, 93, and 86% in R group (log-rank: p = 0.20). Fifty-one patients (64.6%) in NR group and 17 patients (60.7%) in R group underwent serial catheter examinations at 5, 10, and 15 years after Fontan operation and exercise capacity testing at 15 years after Fontan operation (Figure). In R group, arterial oxygen saturation was lower and PVR was higher even after close of fenestration (Figure). In addition, 12 patients (71%) in R group developed significant veno-venous collaterals, which were defined by more than 3mm in diameter of collateral veins or a condition where pulmonary veins were contrasted in venography. On the contrary, cardiac index (CI), SVEDP, and PAP were not different and no significant difference on peak oxygen uptake and anaerobic threshold were observed between in 2 groups. Conclusions Although the mild cyanosis remained due to the development of veno-venous collaterals after the closure of temporary fenestration, CI, SVEDP and PAP well maintained in patients at risk for Fontan operation. Abstract

Background—The changes in cardiac function that occur after pericardiocentesis are unclear.Purpose—This study was performed to assess right ventricular (RV) and left ventricular (LV) function with echocardiography before and after pericardiocentesis. Method and Results—In total, 19 consecutive patients who underwent pericardiocentesis for more than moderate pericardial effusion were prospectively enrolled from August 2015 to October 2017. Comprehensive transthoracic echocardiography was performed before, immediately after (within 3 hours), and 1 day after pericardiocentesis to investigate the changes in RV and LV function. RV dysfunction is defined as meeting three of the four criteria: a TAPSE of −20%. The mean age of all patients was 72.6 ± 12.2 years. The changes of echocardiographic parameters related to RV function are shown in Table. After pericardiocentesis, RV inflow and outflow diameters increased and the parameters of RV function significantly decreased. These abnormal values or RV dysfunction remained at 1 day after pericardiocentesis. Conversely, no parameters of LV function parameters changed after pericardiocentesis. Of 19 patients, 13 patients showed RV dysfunction immediately after pericardiocentesis and 6 patients did not. RV free wall longitudinal strain before pericardiocentesis was higher in patients with post-procedural RV dysfunction (−18.9 ± 3.6%) than in those without (−28.4 ± 6.3%). ROC analysis revealed that a RV free wall longitudinal strain cut-off value of −23.0% had a sensitivity of 100% and a specificity of 83.3% for predicting the occurrence of RV dysfunction after pericardiocentesis (AUC = 0.910). Conclusions—The occurrence of RV dysfunction after pericardiocentesis should be given more attention. Pre-existing RV dysfunction maybe related to the occurrence of RV dysfunction after pericardiocentesis. Abstract

Background Condyloma acuminatum (CA) is a common sexually transmitted disease associated with human papilloma virus (HPV). CA occurring in the urethra is rare and has not been reported in male renal transplant recipients. In addition, despite immunosuppressive conditions and increased risk of HPV-related malignant neoplasms in transplant recipients, HPV testing in male transplant recipients has been uncommon. Here we report a case of urethral CA in a male deceased donor renal transplantation recipient and discuss the importance of HPV testing in male transplant recipients. Case Presentation A 33-year-old male deceased donor renal transplant recipient presented with miction pain 5 years after the transplantation. He reported repeated urinary tract infections with no sexual contact since the renal transplantation. Multiple papillary tumors in his penile urethra were detected by cystoscopy, and a biopsy sample was pathologically diagnosed with CA. Transurethral tumor resection was performed, and the tumors were completely resected. Additional HPV risk type screening with a urethral smear sample showed the prevalence of low-risk HPV. Although tacrolimus was switched to everolimus and imiquimod cream was administered, the tumors recurred 6 months after the resection, and a second resection was performed. No further recurrence has been observed for 1 year to date. Conclusion As the urethral CA was possibly related to immunosuppressive conditions and a risk for HPV-related malignant neoplasm, the case required careful diagnosis, including HPV risk type. The methodology of sampling for HPV testing in men has not been established. This case suggests the necessity for further discussion about HPV testing in male transplant recipients.

Abstract

High energy γ rays with several MeV produced by (n,γ) reactions can be a trouble for low background measurements in the underground laboratories such as double beta decay experiments. In the CANDLES project, which aimed to observe the neutrino-less double beta decay from 48Ca, γ rays caused by (n,γ) reactions were found to be the most significant background. The profile of the background was studied by measurements with a neutron source and a simulation with a validity check of neutron processes in Geant4. The observed spectrum of γ rays from (n,γ) reactions was well reproduced by the simulated spectra, which were originated from the surrounding rock and a detector tank made of stainless steel. The environmental neutron flux was derived by the observed event rate of γ rays from (n,γ) reactions using the simulation. The thermal and non-thermal neutron flux were found to be ( 1.3 ± 0.6 ) × 10 − 6 cm − 2 s − 1 and ( 1.1 ± 0.5 ) × 10 − 5 cm − 2 s − 1 , respectively. It is necessary to install an additional shield to reduce the background from (n,γ) reaction to the required level.

A brief background discussion. Nanoparticle albumin-bound paclitaxel (nab-PTX) has been developed under the concept of improved drug delivery. nab-PTX has been shown to significantly increase progression-free survival compared with solvent-based paclitaxel (PTX) in metastatic breast cancer. However, the long-term outcomes of nab-PTX induced peripheral neuropathy (nPIPN) have not yet been fully elucidated. Trial design: observational cohort study (UMIN20852) Primary objective: The long-term outcome of nPIPN Secondary objectives (1)The validity of the Neuropathic Pain Screening Questionnaire (Japan–Q, Ishikawa et al. Pain Research 2016 ) (2) To evaluate the effect of frozen gloves and elastic stockings for nPIPN nPIPN was assessed by the Japan-Q (J-Q) and Common Toxicity Criteria for Adverse Events ver 4.0 (CTC). The J-Q is an assessment tool for patient-reported pain severity, which consists of seven items, each rated on a five-point scale 0-4; grade 0 (no symptoms) to grade 4 (verysevere symptoms). Pricking pain, electric shock pain, burn like pain, tingling pain with numbness, allodynia, reduced or hyper sensitivity and hand-foot syndrome. Assessment was performed every day during chemotherapy, six months and one year after completion of nab-PTX therapy. Total of 28 points for each day and 588 points for each cycle, higher point correlates with severity. Frozen gloves during administration of nab-PTX and/or elastic stockings were optionally used. Statistical methods Kruskal Wallis test was used and p RESULTS: Between May 2013, and April 2016, 105 patients were enrolled from 7 hospitals. In 73 patients (69.5%) nab-PTX was administered as primary and in 32 patients (30.5%) as adjuvant therapy. Forty three (41.0%) patients received nab-PTX as first line and 62 (59.0%) received after anthracycline contained regimen. Trastuzumab was administered as combination therapy with nab-PTX for Her2 positive patients. Both frozen gloves and stockings were used in 21 patients (20.0%) and frozen gloves only were used in 21 patients (20.0%) according to patient preference. One hundred patients (95.2%) completed four courses, and overall relative dose intensity was 91.4%. During chemotherapy, J-Q scores go elevated from day 1 to day 5 and gradually declined throughout the rest of the cycle. Without using frozen gloves, there was a significant increase according to courses (1st: 45.0 ± 5.7, 2nd: 76.7 ± 8.6, 3rd: 94.3 ± 11.8, 4th: 95.4 ± 11.8). Using frozen gloves, there was a significant increase from 1st course to 2nd, but no further increase was observed(1st: 37.4 ± 8.1, 2nd: 61.9 ± 12.4, 3rd: 62.9 ± 10.4, 4th: 55.3 ± 10.6). After six month and one year, the scores were significantly lower compared with the last day of the fourth cycle (4.28 ± 0.50, 2.53 ± 0.25, 2.85 ± 0.39, respectively). CTC, grade 2 or more sensory disturbance was observed in 57.9% after four cycles, but improved to 9.5% and 5.4% after six month and one year respectively. CONCLUSIONS: Patient-reported nPIPN was significantly getting worse without frozen gloves during chemotherapy, however be largely reversible within 1 year of PST or adjuvant treatment. The J-Q findings support that nab-PTX treatment is tolerable. Abstract

BACKGROUND Surgical indication for hepatic resection is controversial in gastric cancer liver metastasis (GLM). The aim of this study is to clarify the effect of hepatic resection for GLM. METHODOLOGY Ten patients who underwent hepatic resection for GLM between 2001 and 2013 were enrolled in this study. Six patients underwent synchronous hepatic resection and gastrectomy, and the remaining four patients underwent metachronous hepatic resection. Six patients had solitary liver metastasis, and 4 patients had multiple liver metastasis. The median follow-up period was 12.4 months (the range being 0.5 months to 50 months). RESULT The actual 1- year and 3-year overall survival rates for the patients who underwent hepatic resection are 88.9% and 17.8%, respectively. The median survival time was 21.5 months. And the 1-year recurrence free survival time was 20.0%. The median recurrence free survival rate was 4.7 months. Regarding post-operative recurrence, synchronous hepatic resection tended to be a recurrence factor (p=0.08). CONCLUSION Hepatic resection for GLM has an acceptable outcome. Metachronous hepatic resection tends to have a better outcome than synchronous hepatic resection for the treatment of GLM. J. Med. Invest. 65:27-31, February, 2018.

Abstract

Background Although some retrospective studies have suggested the value of adjuvant therapy, no recommended standard exists in bile duct cancer. The aim of this study was to test the hypothesis that adjuvant gemcitabine chemotherapy would improve survival probability in resected bile duct cancer. Methods This was a randomized phase III trial. Patients with resected bile duct cancer were assigned randomly to gemcitabine and observation groups, which were balanced with respect to lymph node status, residual tumour status and tumour location. Gemcitabine was given intravenously at a dose of 1000 mg/m2, administered on days 1, 8 and 15 every 4 weeks for six cycles. The primary endpoint was overall survival, and secondary endpoints were relapse-free survival, subgroup analysis and toxicity. Results Some 225 patients were included (117 gemcitabine, 108 observation). Baseline characteristics were well balanced between the gemcitabine and observation groups. There were no significant differences in overall survival (median 62·3 versus 63·8 months respectively; hazard ratio 1·01, 95 per cent c.i. 0·70 to 1·45; P = 0·964) and relapse-free survival (median 36·0 versus 39·9 months; hazard ratio 0·93, 0·66 to 1·32; P = 0·693). There were no survival differences between the two groups in subsets stratified by lymph node status and margin status. Although haematological toxicity occurred frequently in the gemcitabine group, most toxicities were transient, and grade 3/4 non-haematological toxicity was rare. Conclusion The survival probability in patients with resected bile duct cancer was not significantly different between the gemcitabine adjuvant chemotherapy group and the observation group. Abstract

Co content (x) dependences of cationic distribution and local fields at the tetrahedral A-site and octahedral B-site nuclei in Co ferrites, CoxFe3−xO4 (x = 0–1.0), were investigated by means of hyperfine interaction techniques. A combined investigation by 57Fe transmission and 57Co emission Mossbauer spectroscopies revealed that about 6% of Co2+ ions occupy the A sites for the sample with Co contents of x = 0 and 1.0. This observation evidently suggests that the site selectivity of Co2+ ions is independent of x for samples prepared on the same heat treatment condition. For hyperfine interaction parameters, obvious x dependence was observed for the B-site Fe nuclei, reflecting variation of the atomic ratio of Fe3+/Fe2+ changing with x, whereas for the A-site Fe nuclei, the parameters hardly change along with x. Contrary to the x-independence for the A-site Fe nuclei, however, it was demonstrated by perturbed angular correlation spectroscopy with the nonmagnetic probe 111Cd(←111In) that the hyperfine field at the A site significantly changes along with x. These contradictory results on the A-site fields verify that the nonmagnetic probe can distinguish a subtle change in the local fields with higher sensitivity.

Abstract

To investigate deuterium retention and the onset of blistering, deuterium was implanted in cold rolled tungsten foils at fluences ranging from 3 ×1020 to 3 ×1022D/m2. Ion energies were 300eV and 2000eV in order to be below and above the tungsten theoretical damage energy threshold. While energy dependent phenomena were observed, blistering occurs regardless of ion energy. Both plastically and elastically deformed blisters were found, as manifest in before and after micrographs. The fraction of plastically deformed blisters did not saturate at the fluences used in these studies. However, the size of the largest blister that relaxed during TDS does saturate at ∼7µm. A simple conceptual model is presented, which proposes that the deuterium released from elastically deformed blisters appears at ∼600K in the thermal desorption spectra, which is consistent with large vacancy clusters. Steady state hydrogen, deuterium and helium plasma interactions with a liquid metal: Ga 67 In 20.5 Sn 12.5 at termperatures between room temperature and ∼250 °C have been investigated with JxB-forced convection, using a laboratory-scale facility: VEHICLE-1. Noticeably reduced recycling has been observed for all these gases, when JxB-forced convection is applied, under respevtive plasma bombardment. Preliminary fluid dynamics modeling has been done to interpret experimental observations. For hydrogenic species, a trend has been found, indicating temperature-dependent retention saturation levels. With thermal desorption spectrometry, the amount of deuterium retention after saturation at ∼250 °C has been evaluated to be of the order of 10 14 D/cm 3 whereas the retention of inert gases is found to be undetectable.

Abstract

The proposed Mitchell Institute Neutrino Experiment at Reactor (MINER) experiment at the Nuclear Science Center at Texas A&M University will search for coherent elastic neutrino-nucleus scattering within close proximity (about 2 m) of a 1 MW TRIGA nuclear reactor core using low threshold, cryogenic germanium and silicon detectors. Given the Standard Model cross section of the scattering process and the proposed experimental proximity to the reactor, as many as 5–20 events/kg/day are expected. We discuss the status of preliminary measurements to characterize the main backgrounds for the proposed experiment. Both in situ measurements at the experimental site and simulations using the MCNP and GEANT4 codes are described. A strategy for monitoring backgrounds during data taking is briefly discussed.

A series of first-of-a-kind laboratory-scale experiments on the JxB -force convected liquid metal divertor concept have been carried out in the temperature range from room temperature to ∼200 °C, employing a eutectic alloy: Ga 67 In 20.5 Sn 12.5 , the melting point of which is 10.5 °C. The electrical current conducted through the alloy is set at about 70A and the magnetic field is set at about 700 G. It has reproducibly been observed that hydrogen as well as helium particle recycling is noticeably reduced under steady state plasma bombardment when the liquid is convected by the JxB force.

Abstract

Understanding and managing D retention in plasma facing components is essential for tritium safety in fusion reactors. Neutron irradiated and virgin low carbon arc cast (LCAC) Mo, as well as Mo foil samples with and without He pre-irradiation, were used to investigate D retention. D and He retention were investigated simultaneously in thermal desorption spectroscopy using a high resolution residual gas analyzer. Results show a significant increase in D retention with He pre-irradiation. Vacancies and vacancy clusters are found to retain D in LCAC samples, but neutron irradiated Mo retains more D in vacancy clusters.

Abstract

We investigated the availability of human hepatoblastoma cell line (Hep G2), compared with human primary hepatocytes (HH) and porcine primary hepatocytes (PH), as a cell source for the hybrid artificial liver support system (HALSS) by using polyurethane foam (PUF). All three kinds of hepatocytes spontaneously formed spherical multicellular aggregates (spheroids) of 100–200 μm diameter in the pores of PUF within 3 days of culture. In a PUF stationary culture, Hep G2 spheroids recovered the ammonia removal activity that was lost in monolayer culture, although the removal for each unit cell number was about one tenth that of HH spheroids and about one eighth of PH spheroids. The synthesis activities of albumin and fibrinogen of each unit cell number of Hep G2 were also upregulated by PUF spheroid culture, and were about twice as high as in monolayer culture. The albumin secretion activity of Hep G2 spheroids was almost the same as that of PH spheroids. HH scarcely secreted these proteins in this experiment, probably because they were cultured in a serum-free medium. In the PUF module in a circulation culture, HH had high ammonia removal and low synthesis activities similar to stationary culture. Hep G2 proliferated to a high cell density, such as about 4.8 × 107 cells/cm3-module at 10 days of culture. Although Hep G2 spheroids had low ammonia removal activity in each cell, the removal rate in the PUF module was almost the same as for PH at 7 days of culture because of the high cell density culture by cell proliferation. The albumin secretion rate by Hep G2 in the PUF module also increased with cell proliferation and was about 10 times higher than the initial rate for PH at 7 days of culture. These results suggest that Hep G2 is a potential cell source for the PUF-HALSS.

Abstract

Background It is critical to discuss with advance cancer patients on their EOL issues to improve the quality for their future care. Japanese nurses, however, are apt to feel uncomfortable to deal with such discussions with these patients because most of them have limited opportunities for the training for EOL communication. We investigated whether ‘EOL care education course’ for nurses may help improving their knowledge and skills on EOL care, including communication ability. Methods We provided 15 nurse participants with the educational course based on the ELNEC Core Curriculum including communication and ethical consideration. They all had more than 2 to 25 years of nursing experience. Efficacy of the course was evaluated by using the End of Life Nursing Education-Japan Core Quiz. Results The overall correct answer rate before the course was 66% which was significantly improved to 86% after the training (p Conclusions The education course we provided improved knowledge and skills for EOL communication in the nurse participants. We learned that in the future the course needs more attention on the ‘ethical consideration’ and ‘spiritual care’.

Abstract

The Tritium Plasma Experiment (TPE) is a unique high-flux linear plasma device that can handle beryllium, tritium, and neutron-irradiated plasma facing materials, and is the only existing device dedicated to evaluate in-vessel tritium inventory in the nuclear environment for fusion safety. The electrical upgrade were recently carried out to enhance operational safety and to improve plasma performance. New DC power supplies and a new control center enable remote plasma operations from outside of the contamination area for tritium, minimizing the possible exposure risk with tritium and beryllium and eliminating heat stress issue. In November 2015, the TPE successfully achieved first deuterium plasma via remote operation after a significant three-year upgrade. Simple linear scaling estimate showed that the TPE is expected to achieve Γimax of >1.0 × 1023 m−2 s−1 and qheat of >1 MW m−2 with new power supplies. This upgrade not only improves operational safety of the worker, but also enhances plasma performance to better simulate extreme plasma-material conditions expected in ITER, FNSF, and DEMO for improving in-vessel tritium inventory assessment in fusion nuclear environment.

Abstract

We study the efficiency limitation affecting laser-induced coherent synchrotron radiation (CSR) at high laser power. Experiments are made on the UVSOR-II storage ring in conditions of narrowband terahertz CSR emission. While, at moderate power, CSR power increases quadratically with laser power, a noticeable decrease in efficiency and eventually a decrease in CSR power is observed experimentally at high power. Details of the underlying process are analyzed numerically. As the saturation effect depends almost instantaneously on the laser intensity, the saturation occurs locally in longitudinal space. This has important consequences on the modulation pattern induced on the electron bunch.

Abstract

Background In this study, we demonstrated our new device for open donor liver surgery with left-sided heptectomy by use of the real-time moving windows (RTMW) method with 8-cm transverse skin incision for living donors from the viewpoints of cosmetic, economic, and safety procedures. Methods After the upper abdominal 8-cm transverse skin incision was made, the subcutaneous area was exfoliated and the reverse T-shaped-abdominal incision was made, as in open surgery. After that, the 2 Kent hooks for the upper region and the 2 surgical arms for the lower region were placed. The operative fields of hepatic vein, hepatic hilus, and common hepatic artery were explored, respectively, by use of the RTMW method with the use of the 4 surgical hooks. Hepatic parenchymal dissection was carried out with the use of CUSA and laparosonic coagulating shears. Manipulations of 3 hepatic vessels and the hepatic duct were done by the usual procedure of open surgery. Results This operative procedure could be performed without laparoscopic techniques. The operative time was 7 hours, without blood transfusion. The operative course was uneventful, and the patient was discharged on postoperative day 11. Conclusions Our RTMW method for donor left-sided hepatectomy is considered to be a useful operative procedure from the viewpoints of donor safety, cosmetic advantage, and cost performance.

Abstract

This paper describes a new experimental system designed to investigate tritium mass transfer properties in materials important to fusion technology. Experimental activities were carried out at the Safety and Tritium Applied Research (STAR) facility located at the Idaho National Laboratory (INL). The tritium permeation measurement system was developed as part of the Japan/US TITAN collaboration to investigate tritium mass transfer properties in liquid lead lithium eutectic (LLE) alloy. The experimental system is configured to measure tritium mass transfer properties at low tritium partial pressures. Initial tritium permeation scoping tests were conducted on a 1 mm thick α-Fe plate to determine operating parameters and to validate the experimental technique. A second series of permeation tests was then conducted with the α-Fe plate covered with an approximately 8.5 mm layer of liquid lead lithium eutectic alloy (α-Fe/LLE). We present preliminary tritium permeation data for α-Fe and α-Fe/LLE at temperatures between 400 and 600 °C and at tritium partial pressures between 1.7E − 03 and 2.5 Pa in helium. Preliminary results for the α-Fe plate and α-Fe/LLE indicate that the data spans a transition region between the diffusion-limited regime and the surface-limited regime. Additional data is required to determine the existence and range of a surface-limited regime.

Abstract

We have studied the neutrino-less double beta decay(0νββ) of 48Ca by using CaF2(pure) scintillators. Analysis for rejection of backgrounds(212Bi→ 212Po events and 208Tl events) was effective to reduce backgrounds in Qββ-value region. No events are observed in the Qββ-value region for the data of 131 days × 86 kg. It gives a lower limit (90% confidence level) of (math) > 6.2 × 1022 year (preliminary) for the half-life of 0νββ of 48Ca.
