3. [Primary hyperparathyroidism of chance, a case study]

- Abstract
The authors are reporting here a documented case of hyperparathyroidism leading to hypercalcemia diagnosed spontaneously. A 75-year old female patient with has been treated since 3-4 years with metformin and ramipiril fortype 2 diabetes and high blood pressure, respectively. She was seen in our endocrinology outpatient visit for tingling in the arms associated with cervical pain with increasing intensity over the last six months. The patient's mother was diabetic and her father had high blood pressure. The diagnostic of primary hyperparathyroidism, resulting from a diffuse hyperplasia of the parathyroid gland, was retained after medical, biological, chirurgical and anatomo-pathological investigations. The blood level of PTH1-84 was 916.60pg/ml Normal value=15-68.3pg/ml.Diffuse poly-arthralgia in the neck and the lower limbs may not always be due to arthrosis in elderlies. Further investigation such as the dosage of blood level of calcium and PTH are required.Les auteurs rapportent un premier cas d'hyperparathyroïdie primaire de découverte fortuite documentée au Mali et responsable d'hypercalcémie. Il s'agissait d'une patiente âgée de 75 ans, diabétique de type 2 depuis trois ans traitée par metformine et hypertendue depuis 4ans traitée par ramipril ; venue en consultation externe en endocrinologie pour fourmillement brachial et douleur cervicale diffuse augmentant progressivement d'intensité depuis 6 mois. Aux antécédents familiaux de diabète chez la mère et hypertension artérielle chez le père. Apres les investigations médicaux, biologiques, chirurgicaux anatomopathologiques le diagnostic retenu fut une hyperparathyroïdie primaire (PTH1-84= 916,60pg/ml (N=15-68, 3pg/ml) causée par hyperplasie diffuse de la glande parathyroïde.La présence de poly arthralgie diffuse cervicale et des membres inférieurs peut ne pas être seulement due à une arthrose chez le sujet âgé. Cela mérite une investigation plus approfondie telle que le dosage de la calcémie et de la PTH.

- Abstract
Oxidative stress represents an imbalance between the endogenous antioxidant defenses and the production of pro-oxidant molecules. The present study describes oxidative stress markers (oxidant and antioxidant) metabolic disturbances in diabetic and non-diabetic patients at the Internal Medicine and Endocrinology ward of hospital of Mali.We conducted a descriptive case / control study involving 30 diabetic and 30 non-diabetic patients. Studied markers were Glutathione erythrocyte peroxidase (GPX), intra erythrocyte Superoxide dismutase (SOD), plasmatic uric acid, direct and total bilirubins, albumin and markers for diagnosis and monitoring of diabetes.Non-diabetic patients (9%) had higher glutathione peroxidase levels compared diabetics (3%) (p = 0.005). An increase in superoxide dismutase was observed in 73.3% of diabetics versus 40% of nondiabetics (p = 0). The albumin, uric acid and bilirubin levels were identical in both populations. Glycated hemoglobin was significantly correlated with microangiopathies (p = 0.0058) and macro angiopathies( p=0,0007) in diabetics.The study showed an increase in antioxidant defenses in diabetics by the elevation of superoxide dismutase and a relative normalization of glutathione peroxidase.Le stress oxydant est un déséquilibre entre les défenses antioxydantes endogènes et la production de molécules pro-oxydantes. L'objectif principal était d'étudier les différents marqueurs du stress oxydatif (oxydant et antioxydant) chez les sujets diabétiques et non diabétiques au niveau du service de médecine interne et d'endocrinologie de l'hôpital du Mali à Bamako.l'étude était transversale avec comparaison entre 30 sujets diabétiques et 30 sujets non diabétiques. Les marqueurs étudiés : Glutathion peroxydase érythrocytaire (GPX), la Superoxyde dismutase (SOD) intra érythrocytaire, l'acide urique plasmatique, Les bilirubines directes et totales, l'albumine ainsi que quelque marqueur de diagnostic et de suivi du diabète.Trois pour cent de nos diabétiques avaient un taux de glutathion peroxydase élevé contre 9% des non diabétiques (p =0,005) ; augmentation de la Superoxyde dismutase des diabétiques 73,3% contre 40% des non diabétiques (p =0). Taux d'albumine, acide urique et la bilirubine identiques dans les deux populations ; hémoglobine glyquée était corrélée significativement aux complications dégénératives micro angiopathies (p=0,0058) et macro angiopathies (p=0,00017) chez les diabétiques.l'étude a montré une augmentation des défenses antioxydantes chez les trente diabétiques par l'élévation de la Superoxyde dismutase et normalisation relative du glutathion peroxydase.

The diabetic foot wound is a real public health problem, 10% of the reasons for hospitalization. The risk of amputation is 10 to 30 times higher in diabetics than the general population.To study the problem of amputations of the diabetic foot.This was a descriptive and cross-sectional study that took place in the Department of Medicine and Endocrinology of the Mali Hospital from July 1st, 2016 to June 30th, 2017.Twenty-five (25) diabetic patients were enrolled in our study. The sex ratio was 0.66. At admission, 100% of our patients had arterial disease, 96% neuropathy, and mixed foot in 80%. Poor glycemic control in 64% of patients; osteitis in 52% of cases; 92% of the patients had a 100% amputation risk according to the University of Texas classification. Nearly half or 46% of patients had amputations in the leg. We recorded 1 death case that is 4%.The problem of amputation of diabetic feet is a function of the poor equilibrium and progressive neurological and vascular complications of diabetes.La plaie du pied diabétique constitue un réel problème de santé publique, 10% des motifs d'hospitalisation. Le risque d'amputation est de 10 à 30 fois plus élevé chez les diabétiques que la population générale.Etudier la problématique des amputations du pied diabétique.II s'agissait d'une étude descriptive et transversale qui s'est déroulée dans le service de médecine et d'endocrinologie de l'hôpital du Mali du 1er Juillet 2016 au 30 Juin 2017.Vingt-cinq (25) patients diabétiques ont été recrutés dans notre étude. Le sex ratio était de 0,66. A l'admission, 100% de nos patients avaient une artériopathie, 96% une neuropathie, et un pied mixte dans 80%. Un mauvais équilibre glycémique chez 64% des patients ; l'ostéite dans 52% des cas; 92% des patients avaient un risque d'amputation à 100% selon la classification de l'université du Texas. Près de la moitié soit 46% des patients ont été amputé au niveau de la jambe. Nous avons enregistré 1 cas de décès soit 4%.La problématique de l'amputation des pieds diabétiques est fonction du mauvais équilibre et des complications évolutives neurologiques et vasculaires du diabète.

International audience; The "Pont de Pierre" of Bordeaux (France), built between 1810 and 1821, was, until 2014, the only link between the two borders of the Garonne River. Today, it is a strategical axis that is used every day by 100 000 passengers (on foot, by bicycle or by tram). Since its construction, the bridge has been affected by significant settlement. In 1992-1994 and 2002-2003, 16 micropiles drilled into the masonry of each pillar reinforced the first six pillars. A detailed monitoring analysis (before and after reinforcement) is undertaken to expose the effects of micropile reinforcement and the load transfer process. In addition, an analysis of tidal effects is also carried out. Then, a soilstructure interaction model and a soil-pile-soil model are developed and fitted to the previous analysis. The results are compared with the reinforcement design obtained by the safety factor approach. The importance of considering the relative stiffness between micropiles and timber piles to improve the design of reinforcements (micropiles) is highlighted. The analysis sheds light on the issue of strengthening the last 11 pillars projected for 2023-2025.

Introduction: Diabetes is a heterogeneous group of metabolic diseases characterized by chronic hyperglycemia resulting from a defect in the secretion and/or action of insulin, diagnosed by the observation of high levels of glucose in the blood, responsible in the long term for vascular and nervous complications. The diabetic foot is the set of pathological manifestations affecting the foot in relation to the diabetic disease. Approximately 5% of diabetics present a chronic lesion of the foot. Objective: To describe the epidemiological and clinical aspects of the diabetic foot in the medicine/endocrinology department of the Mali hospital. Methodology: This was a retrospective descriptive study from November 2011 to December 2015. It focused on diabetic patients hospitalized in the department with a foot wound and aged 14 years and over. Results: The study involved 94 patients out of 828 hospitalized, a prevalence of 11.35%. Our series included 36 (38.3%) men and 58 (61.7%) women, i.e. a sex ratio of 0.61%. The mean age was 42.66 years with extremes of 14 and 81 years. Type 2 diabetes was present in 95% of the patients with a duration of evolution of more than 5 years in 60.6% of the cases. The mechanism of occurrence of the wounds was minor trauma in 54 cases (57.4%). Self-medication was the primary treatment in 70 patients (74.5%). More than 50% of the patients wore unsuitable footwear. Clinical and para-clinical examinations revealed isolated neuropathy in 37 cases (39.4%), necrotic wound in 37 cases (39.19%), poor glycemic control: HbA1c > 7% (98.9%), absence of osteitis (57.4%), normal Doppler ultrasound 45 cases (45.7%), stenosing arteriopathy 11 cases (22.3%), non-stenosing 3 cases (20.2%), germs present 56 cases (59.6%) including 21 cases (22.4%) of Staphylococcus aureus. Conclusion: Diabetes and diabetic foot constitute a real public health problem. They are responsible for dreadful and sometimes disabling complications. Its management is multidisciplinary and requires significant financial resources.

Background: Pulmonary embolism (PE) is a severe form of venous thromboembolic disease. In Africa, prevalence of PE in hospitalized medical patients varies among studies. Objective: Aim of this work was to study the epidemiological, clinical, therapeutic and evolution aspects of PE in the medicine department of Mali’s Hospital Bamako-Mali. Methodology: This was a retrospective cross-sectional study carried out from January 01, 2017 to December 31, 2020 in the medicine department of Mali’s hospital Bamako, including all inpatients admitted for PE during the study period. Results: Of 1814 hospitalized patients, 54 patients had pulmonary embolism. Hospital frequency of pulmonary embolism was 2.97%. Predominance was female and sex-ratio M/F was 0.38. Mean age in our series was 54.24 ±1.19 years. Predisposing factors to pulmonary embolism were dominated by obesity 37%, high blood pressure 35.2% and history of cardiovascular disease 20.4%. Dominant signs were dyspnoea and chest pain in 83.3% and 70.4% of cases respectively. Probability of pulmonary embolism was high in 40.7% according to Wells score. Patients with right heart failure were 22.2%. EKG showed sinus tachycardia70.4% and S1Q3 aspect 7.4%. Heart right chambers were dilated at transthoracic echography 42.6%. Obstruction was bilateral at chest angio CT for 51.9% and proximal for 42.6%. Deep venous thrombosis was associated at EP in 16.6%. Treatment was low weight molecular heparin followed by vitamin K antagonist or direct oral anticoagulant. One patient was successfully treated by thrombolysis. Hospital mortality was 16.7%. Conclusion: PE is a serious disease probably underdiagnosed. It is responsible of important mortality.

Introduction: The term “diabetic foot” refers to all conditions that affect the foot and are directly related to the impact of diabetes. Objective: Screen the foot at risk in diabetic patients at the hospital of Mali. Methods: It was a cross-sectional study from January 1st, 2016 to June 30, 2016, at the Department of Internal Medicine and endocrinology of the Hospital of Mali. It was focused on all hospitalized diabetic patients. Results: Thirty-two (32) patients had a foot at risk among 76 diabetic patients during the study period representing 42.10%. The sex ratio was 0.52. Type 2 diabetes accounted for 82%. A glycemic imbalance (HBA1C > 7%) was observed in 88.15%. Eighteen percent (18%) of patients had a history of ulceration or amputation; 33% were walking barefoot; 78.9% had tingles in the foot; 31.6% had intermittent claudication; 64.5% had foot cleanliness; 8% claw toes; 42% had abolition or reduction of superficial tenderness to monofilament and 21% had mixed foot (neuropathy + arteriopathy). In our study, 58.9% of patients had no risk of podiatry. Conclusion: Screening of foot at risk is essential in the management of diabetes because it determines the podiatric risk enabling to minimize future functional disabilities.

We compare for the first time two conjugate events showing simultaneous very low frequency (VLF) wave observations between the same ground station and spacecraft, at different geomagnetic conditions and on opposite sides of the magnetosphere. Waves were observed at Kannuslehto (MLAT = 64.4°N, L=5.46), Finland, and on board Arase (Exploration of energization and Radiation in Geospace, ERG) in the inner magnetosphere. Case 1 on 28 March 2017 shows quasiperiodic (QP) emissions and chorus simultaneously observed on the postmidnight side during the recovery phase of a storm, with sustained high solar wind speed and AE index. - Abstract
OBJECTIVES: Further knowledge about typical hand osteoarthritis (OA) characteristics is needed for the development of new classification criteria for hand OA. METHODS: In a cross-sectional multi-centre international study, a convenience sample of patients from primary and secondary/tertiary care with a physician-based hand OA diagnosis (n = 128) were compared with controls with hand complaints due to inflammatory or non-inflammatory conditions (n = 70). We examined whether self-reported, clinical, radiographic and laboratory findings were associated with hand OA using logistic regression analyses. Discrimination between groups was assessed by calculating the area under receiver operating curves (AUC). RESULTS: Strong associations with hand OA were observed for radiographic osteophytes (OR = 1.62, 95% CI 1.40 to 1.88) and joint space narrowing (JSN) (OR = 1.57, 95% CI 1.36 to 1.82) in the distal interphalangeal (DIP) joints with excellent discrimination (AUC = 0.82 for both). Objective: To study the lipid profile among the diabetic and non-diabetic obese patients. Methods: It was a descriptive and prospective study carried out over a period of 9 months from March to December 2010. It concerned all the patients who came for consultation in endocrinology at the internal medicine of Point G University Hospital Center and at the National Center for Diabetes Research. All the patients were included without any gender or age difference, voluntary and with a BMI ≥ 30 kg/m2. Results: Out of the 1543 patients received in consultation, we had identified 178 obese people that is to say 11, 53% of the cases. Among these cases 60 abided by our criteria with 30 diabetic patients and 30 non-diabetic ones. The average age was 45.83 ± 14.68 years old; 73.3% were more than 40 years old. The sex ratio was 0.07. Two third (66.7%) of our patients limited their eating diet to the three main meals/day and 61.7% were sedentary. The notion of family obesity was found in 93.4% of the cases, of family diabetes in 58.3% of the cases, of past personal medical history of hypertension in 43.3% of the cases. It was about an android obesity in 91% and Gynoid in 9% of the cases. The BMI was 55% of the cases comprised between 30 - 34.9 kg/m2. The lipid profile noticed was: A hypertriglyceridemia: 33% of diabetic patients and 10% of non-diabetic patients, a high LDL cholesterol: 37% of diabetic patients and 30% of our diabetic patients, a HDL hypocholesterolemia: 40% of the diabetic patients and 20% of the non-diabetic patients, a total hypercholesterolemia: 37% of the diabetic patients and 23% of non-diabetic patients. A hyperglycemia was noticed in 13.3% of non-diabetic patients. Conclusion: The frequency of metabolic troubles was higher in obese diabetic patients compared to non-diabetic obese patients mainly the hypertriglyceridemia.

The authors are reporting here for the first time a documented case of androgen-secreting luteal cyst responsible for primary sterility in Mali. A 26-year married woman with a history of familial hyper androgenia of diabetes and high blood pressure who consulted for hyper androgen syndrome and primary infertility. Hirsutism with the presence of hairs on her chin, upper lips, thorax, forearms, arms, and her legs under waxing which made a Ferriman and Galloway scoreof10. A pelvic ultrasound coupled with a laparoscopy allowed us to retain the diagnosis of luteal cyst.Hyper androgenicity in women is rare and the luteal cystis one of its multiple etiologies.Les auteurs rapportent le premier cas de kyste lutéal androgéno-sécrétant responsable d'une stérilité primaire documenté au Mali. Il s'agissait d'une femme mariée de 26ans, avec des antécédents d'hyper androgénie familiale de diabète et hypertension artérielle qui a consulté pour syndrome d'hyper androgénie et stérilité primaire. Hirsute avec la présence de poils au niveau du menton, des lèvres supérieurs, du thorax, des avant-bras, des bras, des jambes sous épilation ce qui faisait un score de Ferriman et Galloway à 10. Une échographie pelvienne couplée à une coelioscopie nous a permis de retenir le diagnostic de kyste lutéal.L'hyper androgénie chez la femme est rare et d'étiologie multiple parmi lesquelles le kyste lutéal.

Turner syndrome is a chromosomal aberration linked to the complete or partial absence of an X chromosome. Its prevalence is 1/2500 female newborns. We report a case in the department of internal medicine and endocrinology of the hospital of MALI. This was a 14-year old girl who consulted for stunting and puberty. She presented clinically a dysmorphic syndrome, TANNER stage1 secondary sexual characteristics, weight and height at -3 DS and a bone age estimated to 9½ years old on the X-ray of the hand. The hormonal assessment showed an elevation of FSH and LH and the genetic study showed an iso chromosome Xq. This result was part of a Turner syndrome with Xq iso chromosome.Le syndrome de Turner est une aberration chromosomique liée à l'absence complète ou partielle d'un chromosome X. Sa prévalence est de 1/2500 nouveaux nés de sexe féminin. Nous rapportons un cas dans le service de médecine interne et d'endocrinologie de l'hôpital du MALI. Il s'agissait d'une fille de 14 ans, qui a consulté pour retard de croissance et de puberté. Elle présentait cliniquement un syndrome dysmorphique, les caractères sexuels secondaires au stade1 de TANNER, le poids et la taille à −3 DS et un âge osseux estimé à 9 ans et demi à la radiographie de la main. Le bilan hormonal montrait une élévation de la FSH et la LH et l'étude génétique montrait un iso chromosome Xq. Ce résultat entrait dans le cadre d'un syndrome de Turner avec iso chromosome Xq.

- Abstract
Background: Despite advances in pharmacotherapy of lipid disorders, many dyslipidemic patients do not attain sufficient lipid lowering to mitigate risk of atherosclerotic cardiovascular disease. Several classes of novel lipid-lowering agents are being evaluated to reduce atherosclerotic cardiovascular disease risk. Lipoprotein apheresis (LA) is effective in acutely lowering the plasma concentrations of atherogenic lipoproteins including low-density lipoprotein cholesterol and lipoprotein(a), and novel lipid-lowering drugs may dampen the lipid rebound effect of LA, with the possibility that LA frequency may be decreased, in some cases even be discontinued. Sources of material: This document builds on current American Society for Apheresis guidelines and, for the first time, makes recommendations from summarized data of the emerging lipid-lowering drug classes (inhibitors of proprotein convertase subtilisin/kexin type 9 or microsomal triglyceride transfer protein, high-density lipoprotein mimetic), including the available evidence on combination therapy with LA with respect to the management of patients with dyslipidemia. Abstract of findings: Recommendations for different indications are given based on the latest evidence. However, except for lomitapide in homozygous familial hypercholesterolemia and alirocumab/evolocumab in heterozygous familial hypercholesterolemia subjects, limited data are available on the effectiveness and safety of combination therapy. More studies on combining LA with novel lipid-lowering drugs are needed. Conclusion: Novel lipid-lowering agents have potential to improve the performance of LA, but more evidence is needed. The Multidisciplinary International Group for Hemapheresis TherapY and Metabolic DIsturbances Contrast scientific society aims to establish an international registry of clinical experience on LA combination therapy to expand the evidence on this treatment in individuals at high cardiovascular disease risk.

Patients et methodes Etude transversale, descriptive de 3 ans et 3 mois avec recueil retrospectif des donnees a partir des dossiers de consultation et d’hospitalisation. Test de χ2 utilise avec seuil de significativite p Resultats Prevalence hospitaliere des endocrinopathies : 58,1 %. Âge moyen : 41,32 ans. Predominance feminine sex-ratio : 0,36. Surpoids et obesite (36,4 %) : predominance de l’obesite chez la femme apres 20 ans (p Conclusion Les endocrinopathies sont frequentes a l’hopital de Mali. Cependant, le suivi a long terme reste problematique du fait des ressources limitees.

Introduction L’hyperprolactinemie, secretion supra-physiologique de prolactine est en clinique le desordre hypophysaire anterieur le plus frequemment rencontre. Son incidence et sa prevalence sont peu definies, en Afrique et dans le reste du Monde. Elle demeure une pathologie peu frequente. Objectifs Etudier les aspects cliniques, paracliniques, etiologiques et therapeutiques de l’hyperprolactinemie. Methodologie Etude retro-prospective et descriptive sur 5 ans (juillet 2011 a juillet 2016). Resultats Nous avons collige 37 cas d’hyperprolactinemie. Le sex-ratio etait de 0,85. L’âge moyen etait 37,32 ans avec des extremes allant de 15 a 74 ans. Le tableau clinique etait domine chez les femmes par l’amenorrhee (80 %), galactorrhee (70 %), cephalees (55 %), les troubles de la fertilite (50 %), troubles visuels (25 %) et chez les hommes par la baisse de la libido (64,7 %), gynecomastie (47,1 %), cephalees (47,1 %), troubles visuels (41,2 %) et troubles de l’erection (29,4 %). La prolactinemie basale etait superieure a 100 ng/mL chez 45,9 % des patients. La TDM avait objective : 11 cas de macroadenomes et 5 cas de microadenomes hypophysaires. L’IRM n’etait pas disponible au Mali. Les principales etiologies etaient : l’adenome hypophysaire a prolactine (43,24 %) ; l’hypothyroidie (5,40 %) et prise medicamenteuse (œstroprogestatifs) chez 5,40 % ; 64,9 % etaient mis sous cabergoline ; 27 % sous bromocriptine et 8,10 % sous surveillance clinique et biologique. Conclusion L’hyperprolactinemie est une pathologie qui existe dans nos structures de sante. Son pronostic depend d’un diagnostic precoce et d’une prise en charge efficiente et codifiee.

24. Dysthyroïdie chez les enfants et adolescents à l’hôpital du Mali

Introduction Dysthyroidie (hyperthyroidie ou hyperthyroidie) chez l’enfant et l’adolescent est beaucoup plus rare que chez l’adulte. Objectifs Etudier les aspects epidemiologiques, diagnostiques, etiologiques, therapeutiques et evolutives des dysthyroidies. Methodologie Etude descriptive, retrospective et prospective de 5 ans portant sur enfants âges de moins 18 ans presentant une dysthyroidie. Resultats Au total, 90 patients, 77,8 % (hyperthyroidie) et 22,2 % (hypothyroidie). Hyperthyroidie : âge (13,07 ans), sex-ratio 0,16. 24,3 % un antecedent familial de pathologie thyroidienne. La clinique dominee : tachycardie (88,57 %), amaigrissement (87,14 %), pouls vibrant (87,14 %), palpitation (81,42 %), exophtalmie bilaterale (70 %) avec retraction palpebrale dans 51,42 %. 77,1 % des patients un goitre diffus dans 62,9 % et nodulaire dans 12,9 %. L’etiologie : Basedow (84,3 %), GMNT (12,9 %). 97,1 % d’un traitement medical par ATS. Remission clinique chez 64,3 % ; 54,3 % des patients en euthyroidie biologique au bout 6 mois de suivi. Hypothyroidie : l’âge 10,70 ans, sex-ratio a 0,53. Six cas familiaux d’hypothyroidie et trois cas goitre familial. clinique : hypersomnie (95 %) ; anorexie (75 %), peau seche (60 %), retard staturoponderal (65 %), troubles du comportement (55 %), prise ponderale (40 %) et retard pubertaire (10 %), goitre nodulaire 25 % avec presence d’adenopathies cervicales 15 %, etiologie : la thyroidite d’Hashimoto (45 %) ; troubles dus a la carence iodee (15 %) causes iatrogeniques (thyroidectomie : 2 cas), opotherapies substitutives a vie. Cinq patients volumineux GMN ont beneficie en plus du traitement medical une thyroidectomie totale. L’euthyroidie 55 % des patients au bout 6 mois de suivi. Conclusion La prise en charge des dysthyroidies chez les enfants est difficile a cause du plateau technique et financier. La mise en place d’une strategie de depistage pour faciliter le diagnostic et traitement doit etre une priorite.

Objectifs Decrire les aspects epidemiologiques, cliniques et la prise en charge de l’hyperparathyroidie secondaire a l’insuffisance renale chronique. Methodes Etude descriptive retrospective de 12 mois chez les patients hospitalises dans le service de nephrologie et d’hemodialyse du centre hospitalier universitaire du Point G. Tous les patients avaient realise un dosage de la parathormone. Resultats Quatre-vingt-treize (93) hyperparathyroidies secondaires sur 237 insuffisants renaux chroniques soit une frequence de 39,24 %. La tranche d’âge de 30–59 ans representait 45,2 % des cas ; l’âge moyen des patients etait de 42,23 ± 17,85 ans avec des extremes 11 et 76 ans, et un sex-ratio de 1,21. Au plan clinique : 81,7 % des patients etaient asymptomatiques, 7,5 % avaient une douleur osseuse, 5,3 % des douleurs musculaires et 2,6 % un prurit. L’etiologie de l’insuffisance renale etait la nephropathie hypertensive (97,8 %) et ischemique (2,1 %). Au plan biologique : la PTH1-84 etait superieure a 300 pg/mL (76,3 %) avec une moyenne a 436,11 ± 200 pg/mL et des extremes allant de 106 a 915 pg/mL, la calcemie etait inferieure a 2,1 mmol/L (77,4 %), la phosphoremie superieure a 1,78 mmol/L (91,4 %), la 1–25(OH)2D3 inferieur a 20 nmol/L (58,1 %), les phosphatases alcalines inferieures a 35 UI/L (72 %). Au plan therapeutique les molecules utilisees etaient : carbonate de calcium (17,2 %), vitamine D (12,9 %), association carbonate de calcium et vitamine D (68,8 %). Chez 69,9 % de nos patients, il n’a pas ete realisee de dialyse. Conclusion L’hyperparathyroidie est presente chez nos patients insuffisants renaux chroniques. Sa prise en charge a necessite une supplementation vitamino-calcique.

Introduction Le syndrome de Klinefelter (SK) regroupe l’ensemble des manifestations cliniques et hormonales liees a un caryotype XXY.C’est une cause genetique majeure d’infertilite assez frequente touchant 11 % des hommes atteints d’azoospermie. Observation Patient de 28 ans, militaire des nations unies, celibataire, sans antecedents personnels particuliers, refere pour gynecomastie bilaterale indolore associe a un trouble de l’erection, une baisse de la libido et une voie effeminee. A l’examen physique gynecomastie bilaterale (Tanner S3) souple indolore a la palpation sans nodule ni de galactorrhee ; presence d’une pilosite au niveau des aisselles et du pubis (P1) ; absence de barbe. Les testicules ont une taille de 2 mL a l’orchidometre de Prader ; aspect eunuchoide : taille : 170 cm – envergure : 181 cm. Le diagnostic du SK fut pose devant un hypogonadisme hypergonadotrope FSH : 44,49 mUI/mL ; LH : 29,37 mUI/mL ; testosterone : 1,86 ng/mL ; estradiol : 0,5 Pg/mL ; hypospermie et une azoospermie au spermocytogramme. L’etude cytogenetique a conclu a un caryotype 47, XXY. L’echographie testiculaire : atrophie testiculaire bilaterale, 3 mL a droite et 2 mL a gauche. Sur le plan therapeutique, le malade a beneficie de la reduction mammaire par chirurgie plastique et fut mis sous Androtardyl® 250 mg en injection intramusculaire tous les mois. Conclusion La majorite des SK sont depistes lors de l’enrolement dans l’armee mais l’installation insidieuse des signes peut en retarder le diagnostic.

Introduction Le syndrome du 46, XX mâle est une forme rare d’inversion du sexe chez les hommes. Observation Patient de 15 ans ; frere Sourd-muet, consulte pour gynecomastie bilaterale depuis 3 ans. Examen physique Augmentation bilaterale du volume des seins, souples, indolores, sans masse ni ecoulement mamelonnaire. Presentant une fente sur la face posterieure du penis mesurant 4 sur 1 cm avec un hypospadias ; les testicules sont atrophies 2 mL a gauche et 3 mL a droite a l’orchidometre de Prader. La pilosite pubienne et axillaire sont bien fournies (P4). Bilan hormonal Hypogonadisme hypergonadotrope : LH a 16,4 UI/L FSH a 22,7 UI/L ; testosterone 0,4 ng/mL, testosterone biodisponible : 0,1 ng/mL, testosterone libre : 5 Pg/mL ; estradiol : 40,55 Pg/mL ; AMH : 1,30 ng/mL ; la prolactine : 11 μg/L. Le caryotype : 46,XX isch Yp 11,3 SRY negative. Echographie testiculaire Hypotrophie testiculaire bilaterale : testicule droit 22 × 15 × 11 mm porteur d’un kyste centro-testiculaire de 0,2 mL et le testicule gauche mesurait 17 × 11 × 10 mm. Echographie abdomino-pelvienne Presence de la prostatique et les deux vesicules seminales ; ailleurs presence d’un lambeau tissulaire dans le detrusor de 19 × 10 mm en faveur probablement d’un reliquat uterin. Traitement Apres prise en charge psychologique le patient a beneficie d’une mastectomie totale, chirurgicale reparatrice de son hypospadias et Androtardyl® 250 mg en intramusculaire tous les mois. Conclusion Le diagnostic du syndrome 46,XX mâle est cytogenetique. Le cout et l’accessibilite de cette technique au Mali rendent le diagnostic difficile et une prise en charge inefficiente.

28. Prévalence du diabète de type 1 auto-immun chez les diabétiques de moins de 21ans au Mali

Objectifs Determiner la prevalence du diabete de type 1 auto-immun chez les diabetiques de moins de 21 ans nouvellement diagnostiques. Methodes Etude prospective, descriptive, transversale de janvier a decembre 2014. Variables recueillies : caracteristiques demographiques, cliniques, statuts des auto-anticorps et du C-peptide. Resultats Quatre-vingt-deux (82) diabetiques âges de moins de 21 ans nouvellement diagnostiques. Sex-ratio = 1,05. La cetoacidose a ete le mode de decouverte dans 58,5 % des cas associee ou non a un coma. Il n’y avait pas d’antecedent familial connu de diabete dans 91 % des cas et chez 96 % des patients aucune pathologie auto-immune n’etait associee. Tous nos patients (100 %) avaient une glycemie a jeun de decouverte > 10 mmol/L, et une HbA1c ≥ 10 % dans 60,9 % des cas. Il y avait au moins un auto-anticorps chez 66 patients (80,48 %). Les anticorps anti-GAD etaient presents seuls chez 61 patients (74,4 %) et les anticorps anti-IA-2 seuls positifs chez 14 patients (17,1 %). Chez 9 patients (10,9 %) etaient retrouves les deux anticorps. Le C-peptide etait indetectable chez 13,4 % des patients a la decouverte du diabete. Conclusion Le diabete de type 1 est le plus frequent chez les enfants et adolescents, les caracteristiques cliniques et la recherche des auto-anticorps permettent le typage du diabete de type 1 auto-immun et sa necessaire prise en charge par l’insuline difficile a realiser dans notre contexte.

- Abstract
Epidermal Langerhans cells (LCs) extend dendrites through tight junctions (TJs) to survey the skin surface, but their immunological contribution in vivo remains elusive. We show that LCs were essential for inducing IgG1 responses to patch-immunized ovalbumin in mice that lacked skin dendritic cell subsets. The significance of LC-induced humoral responses was demonstrated in a mouse model of staphylococcal scalded skin syndrome (SSSS), a severe blistering disease in which the desmosomal protein Dsg1 (desmoglein1) is cleaved by Staphylococcus aureus-derived exfoliative toxin (ET). Importantly, ET did not penetrate TJs, and patch immunization did not alter epidermal integrity. Nevertheless, neutralizing anti- ET IgG 1 was induced after patch immunization and abolished upon LC depletion, indicating that antigen capture through TJs by LCs induced humoral immunity. Strikingly, the ET-patched mice were protected from developing SSSS after intraperitoneal ET challenge, whereas LCdepleted mice were susceptible to SSSS, demonstrating a vital role for LC-induced IgG1 in systemic defense against circulating toxin in vivo. Therefore, LCs elicit humoral immunity to antigens that have not yet violated the epidermal barrier, providing preemptive immunity against potentially pathogenic skin microbes. Targeting this immunological process confers protection with minimal invasiveness and should have a marked impact on future strategies for development of percutaneous vaccines.

The movement of N-isopropyl-p[123I] iodoamphetamine (I-123 IMP) in the lungs was studied. The half time of I-123 IMP release (T 1/2) was delayed in the fibrosis and the sarcoidosis. There was a relation between the prolongation of T 1/2 and the radiographic finding. It was considered that T 1/2 was useful for the objective estimation of the change in the lungs. The adsorption of I-123 IMP to the alveolar cells was much higher than to the circulating leukocytes. It was considered that the accumulation and retention of I-123 IMP in the lungs had relation to the alveolar cells.

Background: Non-compliance in the drafting of examination bulletins makes it difficult to perform them and interpret the results. With the aim of continuously improving laboratory services and guaranteeing the quality of urine cytobacteriological examination (ECBU) results, we initiated this study to evaluate non-compliance in the drafting of ECBU reports., Materials and Methods: This was a retrospective descriptive cross-sectional study which focused on non-compliance in the drafting of ECBU reports analysed in the laboratory from January to December 2022., Results: During the study period, we collected 383 non-compliant ECBU reports out of 672, with a frequency of 56.99%. Non-compliances were related to age (2.68%), profession (24.40%), clinical information (6.70%) and residence (52.08%). Aim: The aim of this study was to establish the spermiological profile of patients treated for infertility at Sikasso Hospital., Material and Methods: This was a prospective descriptive cross-sectional study, which took place from January to December 2022 at Sikasso Hospital. Interpretation of spermiological parameters was based on standards established by the World Health Organization in 2010., Results: We enrolled 41 patients, 51.22% of whom had disturbed spermiological parameters. The abnormalities were azoospermia (21.92%), asthenooligozoospermia (12.20%), asthenozoospermia (7.32%), oligozoospermia (7.32%) and asthenonecrozoospermia (2.44%). Introduction: Diabetes is a chronic hyperglycemia which can be discovered with specific organic complications particularly affecting the eyes, kidneys, nerves, heart and vessels. Globally, the prevalence of stroke in people with diabetes is approximately 10%., Objective: Study the epidemiological and clinical aspects of stroke in patients with type 2 diabetes., Methodology: We made a descriptive and analytical study by a retrospective survey over a period of 10 years, in the department of internal medicine at the university Hospital ofPoint G. We focused on all hospitalized patients with type 2 diabetes, in whom the diagnosis of stroke was retained., Results: At the end of the study 24 patients out of 492 diabetes hospitalized in to the period met our inclusion criteria, representing a frequence of 4.9%. The mean age was 64.67 ± 13.409 years with a sex-ratio of 1.4. The reason for hospitalization was muscle weakness in 54.2% of patients. Eighty-seven point five percent (87.5%) of patients were already known to have diabetes before the weakness. Stroke was the mode of discovery of diabetes in 12.5%. Over 70% of our patients had hyperglycemia at the time of admission. Introduction: High blood pressure is a major cardiovascular risk factor. Patients with cardiovascular risk factors are at risk of developing COVID-19. The objective of this study was to determine epidemiology of Covid-19 infected in patients with high blood pressure., Patients and Method: Descriptive cross-sectional study from April 2020 to June 2020 about patients hospitalized for Covid 19 by PCR diagnosis at the Hopital du Mali Bamako and having high blood pressure. Admission registry and patient charts were used to collect data., Results: We collected 78 out of 484 in patients which mean hospital frequency of 16.11%. The mean age was 55.21 +/- 14.61 years. Sex ratio M / F was 1.36. Patients were followed for high blood pressure in 59% of cases. Medical history was ischemic heart disease in 2.6% and dilated cardiomyopathy in 2.6%. Main functional signs were cough in 41.02% and lost of taste in 11.53%. High blood pressure on admission was grade 2 in 37.2% and grade 3 in 3.8%. Treatments received were calcium channel blockers 41.02%, inhibitors of the reninangiotensinaldosterone system 16.66% and combinations 15.38%. Hospital mortality was 10.3%. There was no statistically significant difference in mortality between known hypertensive patients and de novo hypertensive patients. There was also no statistically significant difference in mortality by grade of hypertension., Conclusion: High blood pressure can be associated to Covid 19. Treatment is based on calcium channel blockers and reninangiotensinaldosterone system inhibitors. 38. [Dysthyroidism in children and adolescents at the Mali hospital].

Introduction: Dysthyroidism (hyperthyroidism or hypothyroidism) in children and adolescents is much rarer than in adults. The purpose of our study was to study the epidemiological, clinical, paraclinical, etiological and etiological aspects and to specify the therapeutic and evolutionary modalities of this condition., Method: This was a five-year descriptive, retrospective and prospective study of children and adolescents under 18 years of age with dysthyroidism., Results: We collected 90 children and adolescents with dysthyroidism (hyperthyroidism 77.8% (n=70) and hypothyroidism 22.2% (n=20). For hyperthyroidism (n=70): the average age was 13.07 years and the sex ratio was 0.16. The family history of dysthyroidism was found in 24.3% (n=17). Clinical manifestations were dominated by tachycardia 88.57% (n=62), weight loss 87.14% (n=61), vibrating pulse 87.14% (n=61) and palpitation 81.42% (n=57). Bilateral exophthalmos was present in 70% of patients (n=49) with palpebral retraction in 51.42% (n=56). Goitre was present in 54 patients (77.1%). It was diffuse in 62.9% (n=44) and nodular in 12.9% (n=9). Basedow's disease represented the first etiology with 84.3% (n=59), followed by toxic multinodular goitre12, 9% (n=9). Sixty-eight patients (97.1%) received medical treatment with synthetic antithyroid drugs. Clinical remission was achieved in 64.3% (n=45) and 54.3% of patients were in biological euthyroidism at 6 months follow-up. For hypothyroidism (n=20): the average age was 10.70 years and the sex ratio was 0.53. Six family cases of hypothyroidism and three cases of family goitre were found. The clinical picture was dominated by signs of hypo metabolism at hypersomnia type 95% (n=19) and anorexia 75% (n=15). The skin was dry in 60% (n=12). A staturo-weight delay in 65% (n=13), behavioural disorders in 55% (n=11), weight gain in 40% (n=8) and puberty delay in 10% (n=2). Goitre was nodular in 25% (n=5) with cervical adenopathy present in 15% (n=3). Hashimoto's thyroiditis accounted for 45% (n=9) of etiologies followed by iodine deficiency disorders in 15% (n=3) and iatrogenic causes (2 cases of thyroidectomy). All patients were started on hormone replacement therapy for life. Five patients with large multinodular goiter received total thyroidectomy in addition to medical treatment. Euthyroidism was observed in 55% (n=11) of patients at 6 months follow-up., Conclusion: Dysthyroidism in children and adolescents exists in our health facilities. Its management is deficient because of the technical and financial platform. 39. [Management of Hyperprolactinemia at the Hôpital du Mali].

Introduction: Hyperprolactinemia, which is a supra-physiological secretion of prolactin, is the most common anterior pituitary disorder encountered in clinical practice. Its incidence and prevalence are poorly defined in Africa and the rest of the world. The objectives were to study the clinical, paraclinical, etiological and therapeutic aspects of hyperprolactinemia at the Mali hospital., Methodology: This was a 5-year cross-sectional study. Data collection was retrospective (July 2011 to October 2015) and prospective (December 2015 to July 2016)., Results: We collected 37 cases of hyperprolactinemia. The sex ratio was 0.85. The average age was 37.32 years with extremes ranging from 15 to 74 years. The clinical picture was dominated in women by amenorrhea (80%), galactorrhea (70%), headache (55%), hypofertility (50%), visual disorders (25%) and in men by decreased libido (64.7%), gynecomastia (47.1%), headache (47.1%), visual disorders (41.2%) and erection disorders (29.4%). Basal prolactinemia was greater than 100ng/ml in 45.9% of patients. Cerebral CT had objectified: 11 cases of macroadenomas and 5 cases of pituitary microadenomas. The main causes of hyperprolactinemia were: prolactin pituitary adenoma (43.24%); hypothyroidism (5.40%) and estrogen-progestin use in 5.40%. For treatment, 64.9% of patients were placed on cabergoline; 27% on bromocriptine and 8.10% on simple clinical and biological monitoring., Conclusion: Hyperprolactinemia is a condition that exists in our health care facilities. Clinicians should consider this in the face of galactorrhea amenorrhea or decreased libido. Introduction: Lifestyle changes have played an important role in increasing the prevalence of obesity. The objectives were to describe the epidemiological, clinical and progressive aspects of obesity in patients of the Mali hospital., Patients and Method: this is a descriptive study with retrospective collection of data from the medical records of patients hospitalized in the Medicine and Endocrinology department. Our patients benefited from weight gain, height, waist measurement and hip measurement with calculation of the body mass index., Results: the study concerned 90 obese patients out of 1515 hospitalized (5.94% of frequency). The mean age was 42 ± 5 with extremes 10 and 74. The sex ratio was 0.38. Obesity was mixed: 75.4% of men, 60% of woman; Android in 36% of men, 18% of women and the only child aged 10 had morbid obesity. The reason for hospitalization was complications of obesity in 13.30%. The psychological reactions collected were: anxiety 54.40%, indifference 40% and shame 5.60%. There were 68.90% complications including 37.80% cardio metabolic (hypertension associated in 51.10%). The lipid assessment carried out showed hyper LDLdemia: 70%; HDL hypoemia: 67.80%, hyper triglyceridaemia: 66.70%. Our patients received dietary and dietary advice. No drug or surgical treatment has been initiated despite the indication. We recorded 3.30% of deaths., Conclusion: The epidemiological and clinical description, diagnosis and evaluation of the repercussions of obesity are easy in our daily practice. - Abstract
Malaria, a major cause of child mortality in Africa, is engendered by Plasmodium parasites that are transmitted by anopheline mosquitoes. Fitness of Plasmodium parasites is closely linked to the ecology and evolution of its anopheline vector. However, whether the genetic structure of vector populations impacts malaria transmission remains unknown. Here, we describe a partitioning of the African malaria vectors into generalists and specialists that evolve along ecological boundaries. We next identify the contribution of mosquito species to Plasmodium abundance using Granger causality tests for time-series data collected over two rainy seasons in Mali. We find that mosquito microevolution, defined by changes in the genetic structure of a population over short ecological timescales, drives Plasmodium dynamics in nature, whereas vector abundance, infection prevalence, temperature and rain have low predictive values. Our study demonstrates the power of time-series approaches in vector biology and highlights the importance of focusing local vector control strategies on mosquito species that drive malaria dynamics.

42. [Primary hyperparathyroidism of chance, a case study].

The authors are reporting here a documented case of hyperparathyroidism leading to hypercalcemia diagnosed spontaneously. A 75-year old female patient with has been treated since 3-4 years with metformin and ramipiril fortype 2 diabetes and high blood pressure, respectively. She was seen in our endocrinology outpatient visit for tingling in the arms associated with cervical pain with increasing intensity over the last six months. The patient's mother was diabetic and her father had high blood pressure. The diagnostic of primary hyperparathyroidism, resulting from a diffuse hyperplasia of the parathyroid gland, was retained after medical, biological, chirurgical and anatomo-pathological investigations. The blood level of PTH1-84 was 916.60pg/ml Normal value=15-68.3pg/ml., Conclusion: Diffuse poly-arthralgia in the neck and the lower limbs may not always be due to arthrosis in elderlies. Introduction: The diabetic foot wound is a real public health problem, 10% of the reasons for hospitalization. The risk of amputation is 10 to 30 times higher in diabetics than the general population., Objective: To study the problem of amputations of the diabetic foot., Method: This was a descriptive and cross-sectional study that took place in the Department of Medicine and Endocrinology of the Mali Hospital from July 1st, 2016 to June 30th, 2017., Results: Twenty-five (25) diabetic patients were enrolled in our study. The sex ratio was 0.66. At admission, 100% of our patients had arterial disease, 96% neuropathy, and mixed foot in 80%. Poor glycemic control in 64% of patients; osteitis in 52% of cases; 92% of the patients had a 100% amputation risk according to the University of Texas classification. Nearly half or 46% of patients had amputations in the leg. - Abstract
Oxidative stress represents an imbalance between the endogenous antioxidant defenses and the production of pro-oxidant molecules. The present study describes oxidative stress markers (oxidant and antioxidant) metabolic disturbances in diabetic and non-diabetic patients at the Internal Medicine and Endocrinology ward of hospital of Mali., Materials and Methods: We conducted a descriptive case / control study involving 30 diabetic and 30 non-diabetic patients. Studied markers were Glutathione erythrocyte peroxidase (GPX), intra erythrocyte Superoxide dismutase (SOD), plasmatic uric acid, direct and total bilirubins, albumin and markers for diagnosis and monitoring of diabetes., Results: Non-diabetic patients (9%) had higher glutathione peroxidase levels compared diabetics (3%) (p = 0.005). An increase in superoxide dismutase was observed in 73.3% of diabetics versus 40% of nondiabetics (p = 0). The albumin, uric acid and bilirubin levels were identical in both populations. Glycated hemoglobin was significantly correlated with microangiopathies (p = 0.0058) and macro angiopathies( p=0,0007) in diabetics., Conclusion: The study showed an increase in antioxidant defenses in diabetics by the elevation of superoxide dismutase and a relative normalization of glutathione peroxidase., (Le comitée de rédaction se réserve le droit de revoyer aux auteurs avant toute soumission à l'avis des lecteurs les manuscrits qui ne seraient pas conformes à ces modalités de présentation. The authors are reporting here for the first time a documented case of androgen-secreting luteal cyst responsible for primary sterility in Mali. A 26-year married woman with a history of familial hyper androgenia of diabetes and high blood pressure who consulted for hyper androgen syndrome and primary infertility. Hirsutism with the presence of hairs on her chin, upper lips, thorax, forearms, arms, and her legs under waxing which made a Ferriman and Galloway scoreof10. - Abstract
Turner syndrome is a chromosomal aberration linked to the complete or partial absence of an X chromosome. Its prevalence is 1/2500 female newborns. We report a case in the department of internal medicine and endocrinology of the hospital of MALI. This was a 14-year old girl who consulted for stunting and puberty. She presented clinically a dysmorphic syndrome, TANNER stage1 secondary sexual characteristics, weight and height at -3 DS and a bone age estimated to 9½ years old on the X-ray of the hand. The hormonal assessment showed an elevation of FSH and LH and the genetic study showed an iso chromosome Xq. - Abstract
Numerous studies have shown the influence of some bacteria colonizing the breeding sites on the development of the parasite in Anopheles malaria vectors. The aim of this study was to evaluate the impact of the breeding sites of Plasmodium falciparum on the gametocytes infectivity of Anopheles gambiae sl using the membrane feeding method. We carried out an experimental infestation study from September 2010 to January 2011 in a village located in the southern savanna of Mali. Cross sectional surveys were conducted to collect larvae and to select gametocyte carriers. Female offspring of L3 and L4 stage larvae were used for the experimental infestation. The gametocyte carriers were children aged within 4 to 12 years. Bacteria identified in breeding water of larvae and adult mosquitoes were: Escherichia coli, Salmonella spp, Klebsiella oxytoca, Pseudomonas spp, Staphylococcus spp, Shigella sp. The oocyst load of infected mosquitoes did not significantly vary according to the samples (F = 1.517, P = 0.230). Low infection rates (5.4% and 2.8%) were observed in the larval bedding and in mosquitoes containing K. oxytoca and Shigella. The blood feeding rates (62.4%; 60.9%; 67.7%) and mortality (66.4%; 64.9%; 61.9%) of An. gambiae did not significantly vary depending on the breeding sites [(P = 0.15); (P = 0.22)]., (Le comitée de rédaction se réserve le droit de revoyer aux auteurs avant toute soumission à l'avis des lecteurs les manuscrits qui ne seraient pas conformes à ces modalités de présentation. - Abstract
The public finance and foreign exchange crisis of the 1980s aggravated the unfavourable economic trends in many developing countries and resulted in budget cuts in the health sector. Policymakers, following the suggestions of World Bank experts, introduced user fees. Economic analysis of the demand for health care in these countries focused on the impact of price and income on health service utilisation. But the lesson to date from experiences in cost recovery is that without visible and fairly immediate improvements in the quality of care, the implementation of user fees will cause service utilisation to drop. For this reason, the role of quality of health care has been recently a subject of investigation in a number of health care demand studies. In spite of using the data from both households and facilities, recent studies are quite limited because they measure quality only by structural attributes (availability of drugs, equipment, number and qualifications of staff, and so on). Structural attributes of quality are necessary but not sufficient conditions for demand. A unique feature of this study is that it also considers the processes followed by practitioners and the outcome of care, to determine simultaneously the respective influence of price and quality on decision making. A nested multinomial logit was used to examine the choice between six alternatives (self-treatment, modern treatment at home, public hospital, public dispensary, for-profit facility and non-profit facility). The estimations are based on data from a statistically representative sample of 1104 patients from 1191 households and the data from a stratified random sample of 42 out of 84 facilities identified. The results indicate that omitting the process quality variables from the demand model produces a bias not only in the estimated coefficient of the "price" variable but also in coefficients of some structural attributes of the quality. The simulations suggest that price has a minor effect on utilisation of health services, and that health authorities can simultaneously double user fees and increase utilisation by emphasising improvement of both the structural and process quality of care in public health facilities.
