The article covers a topical problem--study of transitory disablement in oil-processing workers suffering from peptic ulcer. The authors demonstrate data on increased occurrence of peptic ulcer among those workers engaged mostly into the main industrial process and stress the great economic detriment caused by the disease. Application of endoscopy treatment and therapy within the local prophylactic department appeared to dramatically decrease the transitory disablement. The article could be interesting for gastroenterologists, industrial medicine officers.

The article covers the data on high occurrence of complicated peptic ulcer in the workers engaged into oil processing industry, who work at the main technologic process, which made the difference from the control group. The authors describe the clinical signs showing the involvement of the liver into the process. The results were obtained by modern methods applied to analysis of 207 patients of exposed group and 138 patients of reference group.

The article is devoted to morphology of mucus of the upper gastrointestinal tract, which is a topical problem among the workers engaged into the oil processing in the region. The results of endoscopy screening in 718 workers of oil processing enterprise show the following changes: erosive lesions of gastroduodenal region, atrophy of gastric mucus in patients facing the chronic gastric and duodenal peptic ulcer, higher occurrence of gastric carcinoma.

Prolonged retroperitoneal blockades were performed in 20 patients with acute pancreatitis. Positive effects were obtained in 17 patients. The use of prolonged retroperitoneal novocain blockade with the help of a catheter during operation and in the postoperative period facilitated early liquidation of postoperative paresis of the intestine and arrest of acute manifestations of pancreatitis.

Results of a dynamic endoscopic examination of 213 patients after operations of suturing perforating gastric and duodenal ulcers are described. Stable healing of the ulcer was obtained in 20.2% of the patients. In 79.8% of the patients the ulcer failed to heal or recidivated. A comparison of roentgen findings and results of endoscopic examinations has shown that roentgen analysis gave the correct diagnosis but in 44.7% of the patients examined.

Results of the surgical treatment of perforations of asymptomatic ulcers of the stomach and duodenum in 63 patients are analyzed. Closure of the perforation opening is thought by the authors to be operation of choice in the treatment of such ulcers. Good and satisfactory results were obtained in 83% of the observations.

Results of using the curative endoscopy in the early postoperative period in 43 patients following the operation for closure of the perforative opening are described. The authors show high efficiency of the local injection of antiinflammatory drugs and substances stimulating reparation during healing of closed ulcers of the gastroduodenal zone.

Data on using endoscopic laser therapy in patients after operations for ulcer disease of the stomach and duodenum are presented. The method was used in the treatment of 23 patients with postoperative anastomoses. High efficiency of laser therapy was noted in all the patients. In 11 patients the given therapy was used after operation of suturing the perforated ulcer in order to improve results of the treatment of these category of patients. The complete healing was achieved after 8-9 sessions of laser therapy. There were no complications.

We treated 160 patients (141 men and 19 women, the age of patients was from 18 to 67 years) with gastric and duodenal ulcers using a helium-neon laser LG-75 with an output power range from 15 to 35 mW. Laser radiation was injected into a quartz monofilament 4 mm in diameter and 3 m in length using a transient focusing device. The power of radiation at the output of the fiber was 85% of the initial one. In 127 patients the ulcer (size - 0,80,2 cm) was localized in the bulb of duodenum, in 33 patients - in the stomach (0,70,1 cm). The efficacy of treatment was judged by the dynamics of clinical, radiological, endoscopic and morphological parameters. Acid-producing function of the stomach was investigated by intragastric pH-metry, motor function - by electro gastrography. The therapeutic manipulations were performed daily or every other day for 5 min at a power density at the end of the glass fiber of 1.5-2X103 wt-m-2. In all cases laser therapy was started after histological and cytological examination of biopsy material.
