We evaluated bromodeoxyuridine (BrdU) immunohistochemistry of paraffin-, methyl methacrylate (MMA)-, and epon-araldite (epon)-embedded canine bone specimens to establish an optimal technique for studying cell kinetics of fracture healing in a canine tibial gap model. Dogs were sacrificed 4 months after tibial ostectomy and 1 hr after i.v. injection of BrdU (100 mg/kg). BrdU immunohistochemical staining with a peroxidase-labeled streptavidin-biotin (LAB-SA) method was performed on thin sections of tibia fixed in 70% ethanol and embedded in paraffin, MMA, or epon. Thin section of small intestines fixed in 70% ethanol and 10% neutral buffered formalin and embedded in paraffin, MMA, or epon were BrdU-stained and served as a model for proliferating tissue. Good and consistent BrdU immunostaining without detachment of bone sections was obtained for epon-embedded undecalcified bone sections. BrdU-positive cells were easily identifiable, in contrast to negligible background staining. BrdU-labeled osteoprogenitor cells and osteoblasts were observed around and on the surface of woven bone in external and internal callus of the ostectomy gap. Nuclei of osteocytes were not labeled. In contrast to the epon-embedded specimens, BrdU immunostaining of paraffin-embedded decalcified and MMA-embedded undecalcified bone specimens was unsatisfactory. The results of this study suggest that BrdU immunohistochemistry of ethanol-fixed, epon-embedded, undecalcified canine bone sections is a technique suitable for study of fracture healing with the described methodology.

Dual-energy X-ray absorptiometry (DXA) was used to quantitate the localized densitometric changes that occur early (0-16 weeks) in a tibial ostectomy model of three different gap widths in 15 dogs. Dogs were divided into three equal groups. A 5-mm (group 1), 15-mm (group 2), or 25-mm (group 3) unilateral tibial ostectomy was performed and stabilized with a unilateral external skeletal fixator in each dog. DXA of the gap tissue was performed at 0, 14, 30, 60, 90, and 120 days after surgery. Regions of interest (ROIs) included the entire gap (groups 1, 2, 3) and ROIs within the gap a defined distance from the proximal or distal cortical bone ends: 0-2.5 mm (groups 1, 2, 3); 2.5-5.0 mm (groups 2, 3), 5.0-7.5 mm (groups 2, 3), 7.5-10.0 mm (group 3), and 10.0-12.5 mm (group 3). Bone mineral density (BMD) significantly changed over time in all three groups (P0.0001). The BMD of the 5-mm gap increased over the 4-month study period and reached normal middiaphysial tibial BMD by 90 days after surgery. The BMD of the 15-mm gap also increased after surgery but reached a plateau at a BMD of approximately 0.45 g/cm2 (48% of middiaphyseal BMD) at 60 days after surgery. The BMD of the 25-mm gap increased to a small extent during the first 30 days after surgery and then gradually decreased during the study period. Overall, the 5-mm gap had the highest BMD, followed by the 15-mm gap and the 25-mm gap (P0.0001).

SummaryBone and cartilage transplantation have included autogenic, allogenic, and xenogenic tissues. Non-vascular cortical bone grafts are incorporated through a process of resorption, vascular invasion, and new bone deposition. Cancellous grafts are incorporated through a process of bone deposition followed by resorption. A variety of proteins, enzymes and enzyme inhibitors, collectively termed osteogenins, have been identified to have osteo-inductive activity. The incorporation of cartilage grafts differs from that of bone in several ways, largely due to differences in nutrition of the graft and biology of the incorporation process. The completeness of allograft incorporation is restricted in part by the immune response due to genetic disparity between donor and recipient tissues. Effective methods of sterilizing the graft, to prevent transmission of pathogens from donor to recipient, are important. Veterinary applications of graft material are expanding. While autogenous cancellous grafts to augment fracture repair remain the area of greatest use, segmental allografts for repair of large fracture defects in small animals are also utilized. The application of cartilage and osteochondral grafts in horses represent another area of active research.The use of bone and cartilage transplantation in veterinary surgery continues to expand. What follows is a review of the biologic principles relating to graft incorporation, the effects different storage and handling techniques have on graft incorporation, and the clinical relevance of current research to veterinary application of bone and cartilage transplantation.

The histomorphometric, material, and structural properties of the reparative tissue and bone that formed in a two-millimeter tibial defect in dogs were determined at two, four, eight, and twelve weeks after the operation. At two weeks, the tibial defect was filled mainly with undifferentiated connective tissue. After two weeks, the relative proportion of undifferentiated connective tissue decreased and the amount of bone progressively increased throughout the twelve weeks. New bone formed primarily by intramembranous ossification, with a small degree of endochondral ossification. At twelve weeks, bone occupied 62 per cent of the defect. The calcium content of the reparative tissue increased between four and eight weeks and then, at twelve weeks, reached a plateau of 77 per cent that of normal cortical bone. Anisotropy of the new bone that formed in the defect increased from 13.8 per cent to 26 per cent, and the mean width of the trabecular bone increased 27 per cent between the eighth and twelfth weeks. Maximum torque and torsional stiffness increased between two and eight weeks and then, by twelve weeks, the values leveled off at 44 and 29 per cent of the values for intact bone. Indentation stiffness increased between the fourth and twelfth weeks; at twelve weeks, it was 22 per cent of the stiffness of normal cortical bone. Indentation stiffness increased 283 per cent between eight and twelve weeks, despite insignificant changes in calcium content, amount of new bone, non-osseous space, water content, or volume of trabecular bone during this time. This change in indentation stiffness did correlate with increases in anisotropy of the new bone between eight and twelve weeks.

To determine the thermal effects of monopolar radiofrequency energy (mRFE) on bovine articular cartilage when it was moved at different speeds and using varying application forces.Thirty-six fresh osteochondral sections divided into two groups (18 sections/group) were used in this study. The first group was tested at three speed rates of mRFE probe (1 mm/sec, 5 mm/sec and 10 mm/sec) at a constant force (50 g) applied to the probe tip. In the second group, three application forces of the probe tip were tested (25 g, 50 g and 75 g) at a constant speed (5 mm/sec) (n = 6/test). All tests were performed using a custom-built jig to control the mRFE (Vulcan EAS) probe during a 20-mm pass on each section. After treatment, viability of osteochondral sections was determined by confocal laser microscopy (CLM) combined with vital cell staining.There were not any significant differences in cartilage thickness of tested osteochondral sections among the different speeds or forces. During the mRFE probe treatments at different speeds, CLM demonstrated that probe application at the speed of 1 mm/sec caused significantly greater chondrocyte death than at the speeds of 5 and 10 mm/sec, whereas there were no significant differences in chondrocyte death among the variable application forces (p0.05).This in vitro study demonstrated that RFE thermal penetration correlated most closely with probe application speed than application force for this mRFE probe.Improper use of mRFE may cause thermal injury on articular cartilage.

The purpose of this study was to examine the in vitro effects of three radiofrequency energy devices (two bipolar devices and one monopolar device) for the performance of thermal chondroplasty. Thirty-two fresh bovine femoral osteochondral sections (approximately 3 x 4 x 5 cm) from eight cows were divided into four groups (three treatment patterns and one sham-operated group with eight specimens per group). The three treatment patterns consisted of 1) radiofrequency energy delivered by a mechanical jig at 1 mm/sec in a contact mode (50 g of pressure), 2) radiofrequency energy delivered by a mechanical jig at 1 mm/sec in a noncontact mode (1 mm between probe tip and articular cartilage surface), and 3) radiofrequency energy smoothing of abraded cartilage during arthroscopic visualization. Thermal smoothing of the abraded cartilage surface was accomplished with all three devices. Significant chondrocyte death, as determined by confocal laser microscopy and cell viability staining, was observed with each device. The bipolar radiofrequency systems penetrated 78% to 92% deeper than the monopolar system. The bipolar systems penetrated to the level of the subchondral bone in all osteochondral sections during arthroscopically guided paintbrush pattern treatment. Radiofrequency energy should not be used for thermal chondroplasty until further work can establish consistent methods for limiting the depth of chondrocyte death while still achieving a smooth articular for thermal chondroplasty until further work can establish consistent methods for limiting the depth of chondrocyte death while still achieving a smooth articular surface.

Summary The purpose of our study was to determine the types of lesions that cause colic in juvenile Thoroughbreds, factors associated with these lesions and the subsequent survival and athletic performance of the juveniles. The age of juvenile Thoroughbreds requiring surgical exploration for abdominal pain has an influence on the type of lesion causing colic. The short-term survival rate (discharge from the hospital) after colic surgery for foals was 85% and was strongly influenced by the lesion causing colic. Thirteen percent of juveniles recovered from the first surgery experienced another severe colic episode requiring additional surgery or euthanasia. Eight percent of foals recovered from the first celiotomy developed adhesions. Adhesion formation was related to the initial lesion causing colic and the foals' age at the first surgery. Foals being suckled (15 days to 6 months) were at greatest risk for adhesions and more frequently required multiple surgeries. Juvenile Thoroughbreds that had a celiotomy were significantly less able to race (63%) than their unaffected siblings (82%), and age at the initial surgery was associated with the percentage of horses that raced. However, affected foals able to race won as much money, raced as often, and made as many starts as their siblings. Colic and surgical treatment have a negative impact on athletic performance, but the majority of foals discharged from the hospital after colic surgery will perform athletically as adults.

To evaluate chondrocyte viability using confocal laser microscopy (CLM) following exposure to bipolar radiofrequency energy (bRFE) and to contrast CLM with standard light microscopy (LM) techniques.In vitro analysis using chondromalacic human cartilage.Twelve fresh chondral specimens were treated with the ArthroCare 2000 bRFE system (ArthroCare, Sunnyvale, CA) coupled with 1 of 2 types of probes and at 3 energy delivery settings (S2, S4, S6). A sham-operated group was treated with no energy delivered. Specimens were analyzed for chondrocyte viability and chondral morphology with CLM using fluorescent vital cell staining and with LM using HE and safranin-O staining.LM with HE staining showed smoothing of fine fronds of fibrillated cartilage; thickened fronds were minimally modified. Chondrocyte nuclei were present and not morphologically different than nuclei within sham-operated and adjacent untreated regions. LM with safranin-O staining showed a clear demarcation between treated and untreated regions. CLM, however, showed chondrocyte death: the depth and width of chondrocyte death increased with increasing bRFE settings.CLM showed that bRFE delivered through the probes investigated created significant chondrocyte death. These changes were not apparent using LM techniques.

The thermal properties of collagen have been extensively studied in a variety of experimental models since the mechanism of collagen shrinkage was first proposed by Flory et al. [7–9] in the 1950s. Flory et al. [7–9] described how thermal contraction of collagen is brought about by a transition between the crystalline and amorphous phases of collagen. The investigators applied the thermodynamic theory of high-polymeric materials described by Gee [11] to collagenous shrinkage and demonstrated that the transition from a crystalline to an amorphous phase is fully analogous to that involved in the melting of other crystalline polymers. The transition to an amorphous state where polypeptide chains are disordered and randomly coiled was deduced from the disappearance of X-ray diffraction and the loss of optical birefringence of native collagen [9]. The process was described as “degradation to gelatin” or as “denaturation.”

A recent pilot clinical study has demonstrated that the application of nonablative laser energy to the glenohumeral joint capsule in patients with glenohumeral instability stabilized the shoulder joint in the majority of the patients treated [17]. In this multi-institutional clinical trial, a nonablative level of holmium:yttrium-aluminum-garnet (Ho: YAG) laser energy was applied to patients with glenohumeral instability in order to shrink the redundant capsuloligamentous tissues of glenohumeral joint under arthroscopic guidance. At a mean follow-up time after surgery of 6 months (range 2–12 months), for all patients, regardless of arm dominance, age, sex, or direction of instability, postsurgical subjective scores were significantly higher than presurgical scores [presurgical subjective score 17 ± 1.9 (mean ± SEM); mean postsurgical subjective score 68 ± 2.2] (p < 0.0001). Although this pilot study did not have a comparable nonoperated control population or an operated open surgical repair group, the results indicated that nonablative reduction of redundant glenohumeral joint capsule using the Ho:YAG laser is potentially an effective treatment for joint instability in certain subgroups.

The purpose of this study was to compare joint capsular healing after two delivery patterns of monopolar radiofrequency energy: 1) uniform treatment of the joint capsule (paintbrush pattern) and 2) multiple single linear passes (grid pattern). First, an in vitro study was performed to compare the percent shrinkage of these two treatment patterns using the femoropatellar joints (stifles) of six sheep. Monopolar radiofrequency energy (settings, 70 degrees C/15W) was applied to the lateral joint capsule; the treated area was approximately 10 x 10 mm. There was no significant difference in shrinkage between the grid (27% +/- 8.7%) and paintbrush (29% +/- 7.9%) patterns. In the in vivo study, stifles of 24 sheep were randomly assigned to the paintbrush or the grid pattern groups and treatment was performed arthroscopically. Sheep were sacrificed immediately after surgery, or at 2, 6, or 12 weeks after surgery. At 6 weeks after surgery, confocal microscopy demonstrated that treated areas had almost completely repaired in the grid group; some nonviable areas were still present in the paintbrush group. Mechanical testing at 6 weeks indicated that joint capsule in the grid group had better mechanical properties than capsule in the paintbrush group. This study revealed that radiofrequency treatment of joint capsule in a grid pattern allowed faster healing than tissue treated in a paintbrush pattern.

The purpose of this study was to evaluate the thermal effect of monopolar radiofrequency energy, a potential treatment means for joint instability, on the mechanical, morphologic, and biochemical properties of joint capsular tissue in an in vivo ovine model. The energy was applied arthroscopically to the synovial surface of the femoropatellar joint capsule of 24 sheep. The sheep were sacrificed at 0, 2, 6, and 12 weeks after surgery (6 per group). Monopolar radiofrequency energy initially caused a significant decrease in tissue stiffness and an increase in tissue relaxation properties, followed by gradual improvement in the tissue's mechanical properties by 6 weeks after surgery. Microscopic examination illustrated that radiofrequency energy initially caused collagen hyalinization and cell necrosis, followed by active tissue repair. Biochemical analysis revealed that treated collagen was significantly more trypsin-susceptibile than untreated collagen at 0 and 2 weeks after surgery, indicating early collagen denaturation. This study demonstrated that this treatment initially caused a significantly deleterious effect on the mechanical properties of the joint capsule, which was associated with partial denaturation of joint capsular tissue. This was followed by gradual improvement of the mechanical, morphologic, and biochemical properties of the tissue over time.

To determine whether medullary reaming alone, or followed by cemented hemiarthroplasty, influenced porosity, vascularity, and new bone formation in the proximal portion of the femur in dogs.12 adult mixed-breed dogs.Unilateral femoral head and neck excisions were performed, followed by femoral medullary reaming in 6 dogs and femoral medullary reaming and cemented hemiarthroplasty in 6 dogs; the contralateral femur was used as a control. All dogs were euthanatized 28 days after surgery, and femurs were harvested. Vascularity, porosity, and new bone formation were quantified for all femurs of dogs from both groups at 3 proximal-to-distal levels, 3 regions (periosteal, midcortical, and endosteal), and 4 quadrants (cranial, caudal, medial, and lateral) of the femur.Medullary reamed and cemented hemiarthroplasty femurs had significant increases in vascularity and porosity at all levels and in new bone formation at levels 2 and 3. Porosity was increased significantly in the periosteal region of the cemented hemiarthroplasty (9.7+/-0.7%), compared with control (2.3+/-0.2%) and medullary reamed (8.4+/-0.7%) femurs. Porosity was increased in the caudal and medial quadrants in the medullary reamed and cemented hemiarthroplasty femurs; vascularity results were similar.Increased porosity, vascularity, and new bone formation in reamed and cemented hemiarthroplasty-treated femurs supports the theory that surgical trauma associated with medullary reaming is an important factor in early cortical bone loss after hip arthroplasty.Femoral remodeling associated with reaming and broaching is appreciable but may be only a temporary response, whereas other factors may be responsible for chronic cortical bone loss.

The purpose of this in vivo study was to analyze the short-term tissue response of joint capsule to monopolar radiofrequency energy and to compare the effects of five power settings at 65 degrees C on heat distribution in joint capsule. In 12 mature Hampshire sheep, the medial and lateral aspects of both stifles were treated with monopolar radiofrequency energy under arthroscopic control in a single uniform pass to the synovial surface. The radiofrequency generator power settings were 0, 10, 15, 20, 25, and 30 watts (N = 8/group). The electrode tip temperature was 65 degrees C. Histologic analysis at 7 days after surgery revealed thermal damage of capsule at all radiofrequency power settings. The lesion's cross-sectional area, depth, vascularity, and inflammation were commensurate with radiofrequency power. Tissue damage was indicated by variable inflammatory cell infiltration, fusion of collagen, pyknosis of fibroblasts, myonecrosis, and vascular thrombosis, whereas synovial hyperplasia, fibroblast proliferation, and rowing of sarcolemmal nuclei demonstrated regenerative processes. This study revealed that radiofrequency power settings and heat loss through lavage solution play a significant role in heat distribution and morphologic alterations in joint capsule after arthroscopic application of monopolar radiofrequency energy.

A 24-hour-old cria was admitted for evaluation of stranguria evident shortly after birth. On physical examination, the prepuce appeared to be normally formed and small drops of fluid dripped from it as the cria strained to urinate. Testes could not be palpated. Ultrasonographic examination revealed that the bladder was distended. The only abnormality detected on serum biochemical analysis was high creatinine concentration. Tube cystostomy was performed, and the cria was treated with saline (0.9% NaCl) solution administered intravenously and antibiotics. Four days after admission, the cria could not urinate normally when the cystostomy tube was occluded. Five days after admission, normograde cystography and retrograde urethrography were performed. Radiographic findings indicated the cria was an intersex llama with urethral duplication. The llama was euthanatized at the owner's request. Necropsy revealed bilateral ovotestes with a phenotypically female reproductive tract and urethral duplication. Urethral duplication should be considered as a cause of stranguria in male neonatal crias, and hermaphroditism should be included as a cause of urethropathies in llamas.

To summarize the radiographic and clinical findings, treatment, and outcome in cattle with osteochondrosis diagnosed radiographically.Retrospective case series.29 cattle with radiographic evidence of osteochondrosis.Medical records were reviewed, and owners or referring veterinarians were contacted for outcome assessment. Data were analyzed for potential interactions between osteochondrosis classification (osteochondritis dessicans vs subchondral cyst-like lesions), clinical and radiographic findings, treatment, and outcome, using Fisher's exact test and descriptive statistics.Osteochondrosis was associated with young, male, purebred cattle, clinical evidence of lameness, and radiographic evidence of concurrent degenerative joint disease. Osteochondritis dissecans and subchondral cyst-like lesions had similar clinical findings and outcomes but varied significantly in their radiographic distribution among joints. Osteochondrosis often manifests clinically as a unilateral condition, but bilateral lesions were often found (88%) when limbs were radiographically examined. Cattle managed conservatively tended to be culled (within 6 months of diagnosis because of lameness) more often than those managed surgically, despite the lack of treatment bias.Osteochondrosis in cattle is often associated with lameness or degenerative joint disease. Conservative management does not result in a favorable clinical prognosis for long-term, lameness-free survival, and more studies need to be completed to evaluate the efficacy of surgical treatment of osteochondrosis in cattle.

In a canine model, the fixation stability of a prosthesis and proximal bone graft composite were measured relative to the distal femur. One group had the prosthesis graft composite cemented into the distal femur. The second group had the prosthesis graft composite press fit into the distal femur for biologic ingrowth. Displacements of the proximal femoral grafts relative to the host bone in each group were measured after ex vivo (acute with graft) implantation and 4 months after implantation. A third group with no osteotomy (acute intact) simulated perfect graft to host bone union. Relative displacements representing 6 degrees freedom (translation and rotation) were calculated from the displacement values measured by 9 eddy current transducers. Measurements of displacement were used to test the hypothesis that distal press fit fixation equals distal cement fixation at 4 months after implantation. In all cases the measured translations and rotations of the graft to implant construct were small and of a magnitude that should encourage bone ingrowth (0.05 mm and0.1 degree, respectively). The stability of the press fit group at 4 months was not significantly different from the cemented group in axial and transverse displacement during axial and transverse loading, respectively. There was no difference in stabilities at 4 months between distal press fit and cemented fixation in hip replacements requiring a proximal femoral graft.

Urethral obstruction is common in castrated male small ruminants. Perineal urethrostomy is the recommended treatment in animals not intended for breeding; however, stricture formation is a common complication, and few surgical options are available once a stricture has formed. Prepubic urethrostomy has been used successfully in dogs and cats to treat urethral trauma or stricture formation, and in this report, use of prepubic urethrostomy in 2 small ruminants to treat urethral strictures that developed alter perineal urethrostomy is described. Both animals maintained urinary continence and were able to void urine normally. Complications after prepubic urethrostomy included recurrent cystitis in sheep and urethral stricture in goats. Prepubic urethrostomy may be considered in as an alternative for treatment of urethral disorders in small ruminants.

The nonablative application of holmium:yttrium-aluminum-garnet (Ho:YAG) laser energy to the joint capsule of patients with glenohumeral instability has been found to shrink capsular tissue and to help stabilize the joint. The purpose of this study was to evaluate the effect of nonablative laser energy on the short-term histological properties of joint capsular tissue in an in vivo rabbit model.Eighteen mature New Zealand white rabbits were used in this study. One randomly selected stifle was treated with laser energy, and the contralateral stifle was sham-operated. Animals were euthanized immediately after surgery (day 0), at 7 days postsurgery and 30 days postsurgery. Specimens were processed for histology and transmission electron microscopy.Laser-treated samples at day 0 showed diffuse hyalinization of collagen with nuclear karyorrhexis of fibroblasts. Laser-treated tissue at 7 days postsurgery revealed fibroblast proliferation around and into acellular hyalinized regions of collagen. At 30 days postlaser treatment, areas of fused collagen were greatly reduced as large reactive fibroblasts migrated and secreted matrix.This study illustrates the short-term in vivo tissue response to nonablative laser treatment, where acellular hyalinized regions of collagen are infiltrated by fibroblasts that have used the treated collagen as the framework for migration and secretion of new collagen matrix in order for tissue repair to proceed.

To determine cross-sectional geometry and bone mineral density of the Greyhound radius and correlate these values with torsional failure load and torsional stiffness in an ex vivo model.3 regions, proximal, middle, and distal portions of the diaphysis, were studied. 5 levels of cross section were evaluated geometrically within each region.10 paired radiuses from skeletally mature Greyhounds.Cortical area and area moment of inertia were measured from plain radiographic views and from digitized computed tomographic images. Bone mineral density was measured, using dual-energy x-ray absorptiometry. Torsional stiffness and torsional failure load were then determined mechanically.Minimum area moment of inertia and area moment of inertia estimated from plain radiographic views were significantly different in different regions of interest (P0.05). Significant bivariate correlations (P0.05) between torsional mechanical parameters and bone properties of the radius were identified for the distal portion of the diaphysis in particular. When independent variables for the distal portion were analyzed, estimate of area moment of inertia was entered into the model for torsional failure load as a predictor and minimum area moment of inertia was entered into the model for torsional stiffness as a predictor. Bone mineral density of the distal portion of the diaphysis was not a significant predictor.If accurate noninvasive predictors of bone failure load can be identified, further study of fracture risk should be possible.

Multidirectional and unidirectional glenohumeral instability constitute an important clinical problem, particularly in the athlete. Numerous methods have been described for treatment of glenohumeral instability, including closed management, open and arthroscopic procedures, but none have achieved universal success. The purpose of this study was to evaluate the effect of laser energy at non-ablative energy densities on the mechanical properties of joint capsular tissues in a rabbit model. Femoropatellar joint capsular tissues from 12 mature New Zealand white rabbits were harvested and divided using a randomized block design into three treatment groups with laser at 5 W, 10 W and 15 W, and one control group. Mechanical testing was performed before and after application of laser energy. The application of laser energy resulted in 9%, 26% and 38% reduction in capsular tissue length for the 5-W, 10-W and 15-W group, respectively. Laser energy caused a significant decrease in tensile stiffness in the 10-W and 15-W groups only (P0.05). Laser energy did not change the relaxation properties of capsular tissue at any energy density. The loads required to return specimens to their original length were significantly lower for the 5-W group than for the 10-W and 15-W groups. This study demonstrates that significant capsular shrinkage can be achieved with the application of non-ablative Ho:YAG laser energy without detriment to the relaxation properties of the tissue, although at higher energy densities, laser energy did lessen capsular stiffness properties.

Although the majority of patients with immune-mediated inflammatory arthritis are managed conservatively, some patients may require synovectomy if they have persistent pain secondary to chronic unresponsive swelling of a joint. In this study, three methods of synovial ablation using laser energy, electrocautery, and mechanical debridement were compared in an in vivo chronic synovitis rabbit model. STUDY DESIGN/MATERIALS, AND METHODS: In the first phase of this study, the optimal laser energy/pulse frequency combination for synovial ablation in this model was determined. In the study's second phase, 48 mature rabbits were then divided into four equal groups: laser synovectomy, electrocautery synovectomy, mechanical synovectomy, and control. Chronic synovitis was induced in both stifles of all treatment groups and in the right stifle of the control rabbits. Synovectomy was performed on one stifle of each rabbit; the contralateral stifle served as a sham-operated control. Six rabbits per group were euthanized 2 weeks and 3 months after surgery, respectively.There were no differences among groups in synovial fluid parameters, except at 2 weeks, when the electrocautery group had significantly more white blood cells than the laser and mechanical debridement groups. Histologic examination revealed that mechanical debridement resulted in significantly more synovial hemorrhage, capillary dilatation, plasma cell infiltration, lymphocyte infiltration, joint capsular defects, and poorer synovial ablation than ablation achieved with either laser energy or electrocautery.Laser energy and electrocautery achieved similar results when used for ablative purposes, although electrocautery did not achieve the same degree of synovial ablation as laser energy.

By use of wire ropes as the transosseous component, an external skeletal fixator for the repair of long bone fractures in horses and cattle has been designed and tested in axial compression. Theoretical methods were used in the design process to size fixator components; however, our results suggest that conventional methods of analyzing the displacement of the transosseous component may not apply to wire ropes. Large pretensions in the wire ropes are necessary to obtain functional stiffnesses for fracture fixation. Therefore, a method was sought for terminating the ropes so that an appropriate pretension could be introduced into the rope through its interface with the fixator rings. Ropes were terminated by use of 5 methods and were tested in axial tension to failure. These methods included 3 copper sleeve arrangements, welded ends, and drum sockets. The drum sockets (57.6% of rope breaking strength) far exceeded the strengths provided by the copper sleeves (8.5 to 26.6%) and the welded ends (44.3%). Using the drum sockets, 5 rope configurations were assembled to the fixator, using wood blocks to simulate bones with a gap defect. The fixator was loaded in axial compression for each of the rope configurations, and stiffnesses were determined from measured axial displacement and applied load. The 4-ring fixator configuration, with 2 ropes at 60 degrees angular separation/ring, was the stiffest. In a worst case (gap) model, a mean axial compression load of 1,730 N was observed at 2 mm of displacement for a 4-ring fixator configuration.(ABSTRACT TRUNCATED AT 250 WORDS)

We evaluated the single-cycle structural properties for axial compression, torsion, and 4-point bending with a central load applied to the caudal or lateral surface of a diaphyseal segment from the normal adult equine humerus, radius, third metacarpal bone, femur, tibia, and third metatarsal bone. Stiffness values were determined from load-deformation curves for each bone and test mode. Compressive stiffness ranged from a low of 2,690 N/mm for the humerus to a high of 5,670 N/mm for the femur. Torsional stiffness ranged from 558 N.m/rad for the third metacarpal bone to 2,080 N.m/rad for the femur. Nondestructive 4-point bending stiffness ranged from 3,540 N.m/rad for the radius to 11,500 N.m/rad for the third metatarsal bone. For the humerus, radius, and tibia, there was no significant difference in stiffness between having the central load applied to the caudal or lateral surface. For the third metacarpal and metatarsal bones, stiffness was significantly (P0.05) greater with the central load applied to the lateral surface than the palmar or plantar surface. For the femur, bones were significantly (P0.05) stiffer with the central load applied to the caudal surface than the lateral surface. Four-point bending to failure load-deformation curves had a bilinear pattern in some instances, consisting of a linear region at lower bending moments that corresponded to stiffness values from the nondestructive tests and a second linear region at higher bending moments that had greater stiffness values.(ABSTRACT TRUNCATED AT 250 WORDS)

We performed dual-energy x-ray absorptiometry (craniocaudal and lateromedial views) on 10 pairs of humeruses, radiuses, femurs, and tibias from dogs, using an alignment jig, to determine the homotypic bone mineral density variations of long bones. The bones were divided into 3 regions: proximal, middle, and distal parts of the diaphysis. The bone mineral density of cortical bone, medullary bone, and total bone was determined. Of 160 homotypic comparisons, 21 indicated significant (Por = 0.05) differences between right and left bones at either a region or location. These differences were observed most frequently in the craniocaudal view and were probably secondary to positioning errors. Evaluation of elliptical bones, such as the radius, also indicated that, when the thicker dimension, such as the lateromedial view of the radius was measured, the bone mineral density of regions-of-interest in that view was greater than that in the opposite view (ie, craniocaudal view of the radius). This study validates the concept of using the contralateral limb as the control condition in orthopedic studies of dogs, particularly when evaluating the densitometric properties of long bones. This study also emphasizes the importance of accurate positioning to prevent inadvertent alteration of bone mineral density results when using dual-energy x-ray absorptiometry. Dual-energy x-ray absorptiometry is particularly susceptible to positioning errors, because it converts a 3-dimensional structure into a 2-dimensional image.

As more sophisticated research is performed to refine fracture fixation techniques for horses, it is important that normal values for the geometric properties of the bones of the appendicular skeleton be determined and that suitable controls be available. We evaluated the geometric properties of total bone width, cortical bone width, and medullary canal/trabecular bone width measured from 2 radiographic projections of equine long bones (humerus, radius, third metacarpal bone, femur, tibia, and third metatarsal bone) obtained from a general population of horses. Measurements were performed on slices separated by intervals equal to 5% of the bone's length. Slices were then grouped into 5 regions: proximal epiphysis, proximal part of the metaphysis, diaphysis, distal part of the metaphysis, and distal epiphysis. Results validated use of the contralateral bone as a control for assessing experimental models or clinical cases. Of 858 homotypic slice comparisons between left and right bones, significant (Por = 0.05) differences were detected in 31 (3.6%) of the comparisons. Of 168 homotypic region comparisons, significant differences were observed in 3 (1.8%) of the comparisons. The greatest variation between left and right bones was observed in metaphyseal regions, areas with bony protuberances, and regions with prominent bone superimposition. At a power of 0.8 for the statistical tests performed in this study, the mean homotypic variation of bones in each region is5.8% for the proximal epiphysis, 11.3% for the proximal part of the metaphysis, 6.8% for the diaphysis, 12.2% for the distal part of the metaphysis, and 5.2% for the distal epiphysis.

Basic research in canine mechanics must be performed to better understand the forces and moments the appendicular skeleton must withstand. This type of research may allow surgeons to make substantial advances in total joint replacement and fracture fixation design and may enhance our understanding of bone remodeling and fracture occurrence in relation to exercise and trauma. In our study, craniocaudal bending stiffness, mediolateral bending stiffness, axial compressive stiffness, and torsional stiffness of the humerus, femur, radius, and tibia of dogs was determined, using nondestructive bending, compression, and torsional tests. Entire diaphyseal and middiaphyseal properties of these long bones were evaluated. Bones also were tested to failure in torsion to quantify the failure properties of these long bones. Left to right variability was examined to validate the use of contralateral limbs as the control condition for experimental studies. There were no significant right to left differences in entire diaphyseal mechanical properties for any of the long bones, except for compressive stiffness of femurs. Homotypic differences in entire diaphyseal mechanical properties, if present, ranged from 8.0 to 35% for the 4 long bones (power = 0.8). For middiaphyseal mechanical properties, there were no significant right to left differences in the 4 long bones, except for craniocaudal bending stiffness of tibias. The homotypic differences in middiaphyseal mechanical properties, if present, ranged from 7.2 to 62% for the 4 long bones (power = 0.8). In all bones and loading modes, middiaphyseal stiffness was greater than entire diaphyseal stiffness (P0.0001).(ABSTRACT TRUNCATED AT 250 WORDS)

The effect of 5 fracture fixation methods on bone mineral density (BMD) measurement of femurs, using dual-energy X-ray absorptiometry (DXA) was determined in a canine model. Six regions of interest were measured, including the entire femur, the diaphysis of the femur, and small regions centered over the middiaphysis of the bone (lateral middiaphyseal cortex, medial middiaphyseal cortex, middiaphyseal medullary canal, and total middiaphysis). Eight unpaired femurs were collected and scanned by use of DXA before (5 separate scans/femur) and after (5 separate scans/femur) fixation by use of 1 of 5 fixation methods. These fixation methods included: intramedullary (IM) nail; IM nail and cerclage wires; IM nail and external skeletal fixation; locked IM nail; and a dynamic compression plate (DCP). All implants were made of stainless steel. The IM nail fixation devices caused significant decreases in the DXA measurement of BMD in the small regions of interest, compared with femurs without fixation devices (mean decrease, 37.3%; P0.05). The locked nail caused similar, but larger, decreases in the DXA measurement of BMD, compared with the IM nail fixation methods (P0.05). Plate fixation caused a small, but significant (P0.05), decrease (2.8%) in the DXA measurement of BMD in the large regions of interest, but when all regions were averaged, it did not cause significant change in this measurement, compared with femurs without fixation devices.(ABSTRACT TRUNCATED AT 250 WORDS)

Dogs have been used extensively as an experimental model for studying musculoskeletal disorders. Many of these studies incorporated sequential radiography to quantitate a particular treatment's effect, using the contralateral bone as the control condition. The contralateral bone can be used as a control only if there is bilateral symmetry between right and left limbs. We performed radiography (craniocaudal and lateromedial views) on 10 pairs of humeri, radii, femora, and tibiae from dogs, using an alignment jig, to radiographically determine homotypic geometric variations of long bones. The bones were divided into 5 regions: proximal epiphysis, proximal metaphysis, diaphysis, distal metaphysis, and distal epiphysis. The total bone diameter, medullary diameter, and cortical width (total of medial + lateral cortex or total of cranial + caudal cortex) were measured at specified slices throughout each of these regions. Fourteen of 540 homotypic comparisons revealed significant differences between right and left bones at either a slice or region. Although there were only 14 significant differences between right and left bones at any region or slice, measurements were more precise with lower coefficients of variation in the diaphyseal and epiphyseal regions. Homotypic differences in diaphyseal and epiphyseal regions were5.3 and 7.5%, respectively (power = 0.8). In metaphyseal regions, however, homotypic differences were larger; these differences could have been as large as 11.5% for total bone diameter, 15.6% for medullary diameter, and 80.0% for cortical width without achieving significant differences between populations (power = 0.8). This study validated the concept of using the contralateral limb as the control condition in orthopedic studies using dogs, particularly when evaluating the geometric properties of long bones radiographically.

The noninvasive evaluation of fracture healing may allow more precise timing of fixation device removal, quantitative recommendations for weight bearing, and the prediction of abnormal fracture healing patterns. Techniques used for the evaluation of intact bone and of healing fractures include resonance frequency and impedance methods, the measurement of sound speed using ultrasound, computed tomography of ultrasonic energy, roentgenographic photodensitometry, quantitative computed tomography (QCT), single-photon absorptiometry (SPA), and dual-energy x-ray absorptiometry (DEXA). A study comparing QCT, SPA, and DEXA showed that all three techniques had strong correlations with the torsional properties of healing canine tibial osteotomies. Quantitative computed tomography and SPA had stronger correlations with local gap tissue properties than DEXA. The relatively poor performance of DEXA may have been related to the relatively large resolution (2 mm) of the device used in the study. Current DEXA devices have resolutions as low as 0.5 mm, perhaps enhancing its capabilities. Computed tomography of ultrasonic energy, QCT, SPA, and DEXA each appear to have great potential for future use in the evaluation of fracture healing.

Modular, porous-coated, titanium segmental endoprostheses were implanted bilaterally in the femoral diaphysis of 7 adult mixed-breed dogs. Autogenous bone graft in particle form was placed around the implant and bone. In 1 limb, homologous fibrin adhesive was mixed with the graft in situ before soft tissue closure. The contralateral limb was grafted in identical manner, but without fibrin adhesive, and served as a control. Radiography was performed immediately after surgery and 1, 2, 3, 4, 6, 8, 10, and 12 weeks later to assess callus area and bone remodeling. At 12 weeks, dogs were euthanatized and bone/implant fixation strength was tested under torsion and compared with values for 6 in vitro controls. Histomorphometric and microradiographic analyses of transverse sections of the distal portion of the implanted femurs were performed. Radiographic callus area was significantly (P0.05) smaller in the femurs grafted with fibrin adhesive, compared with the contralateral control. New bone formation (21.4 +/- 1.8% vs 19.2 +/- 2.4%), unlabeled bone (64.8 +/- 3.0% vs 67.9 +/- 4.2%), porosity (13.9 +/- 0.7% vs 12.9 +/- 0.8%), and bone ingrowth into the porous coating (10.3 +/- 0.9% vs 10.0 +/- 1.2%) were not significantly different between fibrin- and nonfibrin-grafted implants, respectively. There were no significant differences in torsional strength of implant fixation between the fibrin- and nonfibrin-grafted femurs or between the in vivo implanted femurs and the in vitro controls.(ABSTRACT TRUNCATED AT 250 WORDS)

A 5-year-old Quarter Horse mare was examined because of a 6-month history of quidding and nasal discharge that contained feed material. Physical examination revealed weight loss and dorsal displacement of the soft palate, caused by a soft tissue mass located at the dorsal aspect of the base of the tongue. Surgical resection of the mass was successfully performed through an oral approach. The histopathologic diagnosis was rhabdomyosarcoma.

The incorporation of autogeneic bone graft into a porous coated segmental endoprosthesis after high-dose irradiation was studied in dogs. A mid-diaphyseal defect was surgically created and then reconstructed with a porous-coated segmental endoprosthesis in 16 dogs. Autogeneic bone grafts were placed over the porous-coated regions of the endoprosthesis and at the endoprosthesis-bone junctions to achieve extracortical fixation. In eight dogs, the reconstructed femora were treated with a time-equivalent dose of 5500 cGy, delivered over a five-week period, beginning three weeks after surgery. In eight dogs, which served as the control group, the reconstructed femora were not irradiated. Dogs were killed 12 weeks after surgery, and the reconstructed femora were evaluated clinically, roentgenographically, mechanically, and histologically. Extracortical bone formation and bone ingrowth into the porous-coated segmental endoprosthesis were significantly inhibited by high-dose irradiation. Nonirradiated reconstructed femora had higher maximum torque at the implant-bone junction than irradiated femora. Nonirradiated femora had significantly greater bone ingrowth within the porous space than irradiated femora. Intracortically, irradiated femora had greater unlabeled bone and less porosity as well as more new bone than nonirradiated femora at 12 weeks postsurgery. Extracortically, irradiated femora had greater original cortical bone and less porosity as well as more new bone than nonirradiated femora at 12 weeks postsurgery. Mineral apposition was less in the irradiated femora from the initiation of radiotherapy to the time of killing five weeks after cessation of irradiation.

A modification of the Roux-en-Y anastomosis procedure was used to bypass a pyloroduodenal mass in a 12-year-old Arabian stallion. Clinical signs had consisted of a 4-week progression of ventral and hind limb edema, hypoproteinemia, fecal occult blood, intermittent abdominal pain, weight loss, and gastric reflux. On exploratory celiotomy, an obstructive mass was found in the pylorus and proximal portion of the duodenum. Gastrojejunostomy and duodenojejunostomy were performed by use of stapled side-to-side anastomosis techniques. Inaccessibility of the obstructed pyloric region prevented resection of the affected area.

Incarceration of the large colon through a rent in the gastrosplenic ligament of a horse was surgically corrected via ventral midline celiotomy. Clinical signs were similar to those in other horses with nonstrangulating large colon disorders. Diagnosis of large colon incarceration in the gastrosplenic ligament was determined by surgical abdominal exploration. The findings of medial deviation of the spleen, location of the large colon lateral to the stomach and caudolateral to the spleen, and caudocraniad passage of the large colon through the gastrosplenic ligament are similar to findings in horses with small intestinal incarcerations in the gastrosplenic ligament. Correction was facilitated by performing a pelvic flexure colotomy and evacuating colonic contents prior to returning the large colon through the rent to its normal anatomic position.

A 7-year-old castrated male llama was admitted for evaluation of mild colic. Exploratory celiotomy revealed a diaphragmatic hernia that was entrapping the proximal portion of the ascending colon. The hernia was reduced, and the devitalized ileum, cecum, and 45 cm of the proximal portion of the ascending colon were resected. Surgical repair of the defect in the diaphragm could not be accomplished from the ventral celiotomy, and the owners declined definitive repair via left thoracotomy. The llama recovered and did well until 9 months after surgery, when the hernia recurred. Necropsy revealed a defect, with characteristics consistent with a congenital origin, in the dorsal part of the left hemidiaphragm. On the basis of the findings in this llama, repair of such defects is strongly advocated.

Quantitative computed tomography has been used extensively to measure bone mineral density; particularly in the vertebral column and in the proximal portion of the femur in human beings with osteoporosis. Other potential applications of this technique include evaluation of bone adjacent to metallic endoprostheses and evaluation of fractures as they heal. Unfortunately, metal causes severe image degradation, principally seen as starburst streaking. One method used to decrease these artifacts is by imaging less-attenuating materials, such as titanium alloy. Titanium decreases image degradation sufficiently to allow accurate determination of the geometric properties of cadaveric bone. In our study, the effect of a titanium segmental endoprosthesis on bone mineral density measurement was determined by use of bone specimens from dogs and calibration standards. Titanium decreased the bone mineral density of calibration solutions from 6.8 (500 mg/cm3) to 17.7% (250 mg/cm3), and increased bone mineral density of cortical bone by 5.3%. Titanium did not affect the repeatability of these scans, indicating that the error caused by titanium was systematic and can be corrected. Our data were suggestive that quantitative computed tomography can be used to measure bone mineral density of cortical bone adjacent to titanium endoprostheses, with a predictable increase in density measurement.

The purpose of this study was to determine the precision and accuracy of quantitative computed tomography bone mineral analysis in dogs in coronally reconstructed images. Nonhomogeneous tissues, such as bones with fractures or deformities, may be better analyzed if multiplanar reconstruction of the transaxial data could be performed without degradation of information. Our analysis demonstrated that coronal reconstruction of quantitative-computed tomography data was precise (1.2 to 4.7%) and accurate (1.3 to 7.5%) in vitro. The technique displays high-quality images, which can be analyzed at any location within the volume scanned. Quantitative computed tomography of canine osteotomy healing in vivo accurately determined bone mineral density of selected regions of interest. Bone mineral density correlated highly with calcium content of the tissue (R2 = 0.76, P less than 0.0001).

The histomorphometric, material, and structural properties of the reparative tissue and bone that formed in a two-millimeter tibial defect in dogs were determined at two, four, eight, and twelve weeks after the operation. At two weeks, the tibial defect was filled mainly with undifferentiated connective tissue. After two weeks, the relative proportion of undifferentiated connective tissue decreased and the amount of bone progressively increased throughout the twelve weeks. New bone formed primarily by intramembranous ossification, with a small degree of endochondral ossification. At twelve weeks, bone occupied 62 per cent of the defect. The calcium content of the reparative tissue increased between four and eight weeks and then, at twelve weeks, reached a plateau of 77 per cent that of normal cortical bone. Anisotropy of the new bone that formed in the defect increased from 13.8 per cent to 26 per cent, and the mean width of the trabecular bone increased 27 per cent between the eighth and twelfth weeks. Maximum torque and torsional stiffness increased between two and eight weeks and then, by twelve weeks, the values leveled off at 44 and 29 per cent of the values for intact bone. Indentation stiffness increased between the fourth and twelfth weeks; at twelve weeks, it was 22 per cent of the stiffness of normal cortical bone. Indentation stiffness increased 283 per cent between eight and twelve weeks, despite insignificant changes in calcium content, amount of new bone, non-osseous space, water content, or volume of trabecular bone during this time. This change in indentation stiffness did correlate with increases in anisotropy of the new bone between eight and twelve weeks.

The third carpal bone (C3) was collected from both forelimbs of 27 Thoroughbreds. On the basis of age, training, and history, specimens were assigned to 1 of 5 groups: yearling, untrained horses (group 1, n = 4); 2- to 3-year-old, untrained horses (group 2, n = 7); trained 2-year-old horses (group 3, n = 6); trained 3-year-old horses (group 4, n = 6); and 3-year-old, trained horses with carpal pathologic features (group 5, n = 4). A transverse section of subchondral bone 5-mm thick was cut in a precise fashion 10 mm below the proximal articular surface of all specimens. After high-detail radiography was done, indentation testing was performed on the proximal surface of the section at points 5 mm apart. The stiffness of the subchondral cancellous bone was determined from the slope of the load vs displacement curve. Topographic plots of stiffness measurements were compared with radiographs of each specimen. Point determinations were averaged to derive measures for the radial and intermediate facets, and for regions 5, 10, 15, and 20 mm from the dorsal margin of C3. Area fraction (1-p; p = porosity) was measured for the radial and intermediate facets, using an automated image analysis system. Significant (P less than 0.05) increases in stiffness and area fraction were found in the C3 from trained horses (groups 3 to 5), compared with untrained horses (groups 1 to 2). Stiffness and area fraction of the radial facet of pathologic C3 were significantly higher than the same variables measured in C3 from any other group.(ABSTRACT TRUNCATED AT 250 WORDS)

A randomised, blind prospective study was performed to evaluate effects of low dose heparin therapy on complication and survival rates in 33 horses following exploratory celiotomy. Four horses had non-strangulating lesions and seven had strangulating lesions of the small intestine, 19 had non-strangulating lesions of the large intestine, and three had duodenitis-proximal jejunitis. Eighteen required an enterotomy or intestinal resection and anastomosis. Sixteen received heparin subcutaneously (66 iu/kg every 12 h) post operatively and 17 received a saline placebo for a mean of 3.7 days. There was a significant (P < 0.01) mean decrease in PCV (29.1 per cent) in the heparin group. No significant differences in complication or survival rates were seen between groups. Long term follow-up was obtained for 10 horses in each group. There were no significant differences between groups.

The stiffness, load to failure, and bending moments of adult intact equine humeri and humeri repaired with 3 fixation techniques were determined in vitro. Bones were tested in axial compression (30 pairs), mediolateral 3-point bending (15 pairs), and caudocranial 3-point bending (15 pairs). An oblique osteotomy of 1 humerus of each pair was performed to simulate the long spiral oblique fractures that occur clinically in horses. Bones were repaired in 3 ways: group 1--nylon band cerclage fixation (20 bones); group 2--multiple intramedullary pinning (20 bones); and group 3--nylon band cerclage fixation and multiple intramedullary pinning (20 bones). Intact bones were significantly (P less than 0.05) stronger than repaired bones in each testing mode. Bones repaired with bands only were significantly less stiff in bending than were bones repaired with pins only or with pins and bands. In compression, only specimens repaired with pins and bands were significantly stiffer than were bones repaired with bands only. Bones repaired with bands only required significantly less load to failure in compression and in caudocranial bending than did bones repaired with pins only or with pins and bands. Bones repaired with pins only deformed through the full displacement of the actuator (5 cm), and pins deformed plastically. Bones repaired with pins and bands were stiffer and had higher bending moments than did bones repaired with pins only, but the differences were not significant.

The medical records of 10 horses with invasive neoplasms of the penis, prepuce, and/or superficial inguinal lymph nodes in which treatment involved en bloc resection and penile retroversion were reviewed. All were geldings and ranged in age from 12 to 25 years (mean, 19 years). Evaluation of biopsy specimens obtained before surgery confirmed lymphosarcoma in 1 horse and squamous cell carcinoma in 9 horses. Typical history included swelling, ulceration, and abscessation of the penis and prepuce and large superficial inguinal lymph nodes. Complications after surgery included dehiscence of the urethrostomy site (4 horses), dehiscence of the ventral skin incision (1 horse), urine scalding of 1 hind limb (1 horse), cystitis (1 horse), severe hemorrhage (1 horse), and diarrhea (1 horse). One horse was euthanatized during hospitalization, because of severe dehydration secondary to diarrhea. At necropsy, firm nodules were scattered in the pulmonary parenchyma, myocardium, thyroid gland, parathyroid glands, cranial mediastinum, kidneys, and hilar lymph nodes. Microscopic examination of the nodules revealed undifferentiated carcinoma. Nine horses were discharged from the hospital between 1 and 5 weeks after surgery. The mean follow-up interval was 27 months (range, 6 to 96 months). Eight horses had no evidence of recurrence. One horse had recurrence of neoplasm at 6 months and was euthanatized 12 months later.

Multiple myeloma was diagnosed in a 22-year-old Arabian mare with a history of chronic weight loss. Quantitative immunoglobulin analysis revealed monoclonal gammopathy (IgG(T), 9,800 mg/dl). Due to progressive weight loss, the horse was euthanatized. Microscopy of tissues revealed plasma cell infiltrates in bone marrow, spleen, pituitary gland, adrenal cortex, muscle of the tongue, and bronchial, renal, and mesenteric lymph nodes.

In a retrospective study, we examined the case records for 69 horses with noncomminuted fractures of the proximal phalanx, excluding proximal chip fractures. Forty-nine of the horses sustained the fracture while racing or training for racing. Radiographic examination of all affected limbs was performed, and fractures were classified into 6 noncomminuted types: midsagittal fractures, including short incomplete midsagittal fractures, long incomplete midsagittal (LIMS) fractures, and complete midsagittal (CMS) fractures; dorsal frontal fractures; distal joint fractures; plantar process fractures; physeal fractures; and oblique fractures. Four horses were destroyed before treatment. Sixty-five horses were treated and 63 survived to go home. Long-term follow-up evaluation of the horses that were sent home revealed that 4 were euthanatized after discharge because of persistent lameness of the fractured limb. Of the 59 remaining horses, 34 returned to racing, 7 were used as show or pleasure riding animals, 8 were used for breeding, and 10 were lost to follow-up. More Standardbreds with noncomminuted proximal phalangeal fractures returned to racing than did Thoroughbreds. Of 30 Standardbreds that raced or trained before the fracture, 23 (76.7%) returned to racing--8 to their previous level of performance, 11 to a lower level of performance, and 4 to an unknown level of performance. The Standardbreds that returned to racing were horses with physeal fractures (2/2), LIMS fractures (4/4), CMS fractures (11/16), short incomplete midsagittal fractures (4/7), distal joint fractures (1/2), and plantar process fractures (1/2). Of 21 Thoroughbreds that raced or trained before the injury, 11 (52.4%) returned to racing--7 to their previous level of performance and 4 to a lower level of performance.(ABSTRACT TRUNCATED AT 250 WORDS)

A 3-month-old Quarter Horse filly stepped on a fence staple and developed navicular bone osteomyelitis of the right hindfoot. A 1.5-cm spherical portion of medullary cavity containing purulent material was debrided and flushed with 0.9% NaCl solution. Cancellous bone was collected from a caudal sternebra and placed into the defect. The solar defect had filled with granulation tissue and was epithelialized 6 weeks after surgery. At 6-month follow-up evaluation, the navicular bone defect had healed and the foal was sound on the limb. Cancellous bone grafting may have merit for the treatment of navicular bone osteomyelitis in the horse.

The medical records of 13 horses with strangulated umbilical hernias were reviewed. Typical history included increased swelling, warmth, and firmness of the hernial sac. Enterocutaneous fistulas had developed in 2 horses. Four horses had signs of abdominal pain. Surgery was performed on all horses, and the hernia was reduced by an open reduction technique. Incarcerated tissue included omentum (1 horse), jejunum (5), ileum (4), cecum (1), and ventral colon (2). All horses survived and were discharged from the hospital. Follow-up information on 9 horses (5 to 52 months after discharge) revealed no complications in 6 horses. Of the remaining 3 horses, one horse was euthanatized 5 months after discharge because of laminitis. One horse had persistent drainage from the skin incision requiring removal of nonabsorbable suture material 8 months after discharge. One foal required a second surgery because of signs of abdominal pain 17 days after the initial surgery. The foal was euthanatized during surgery because of severe peritonitis secondary to anastomotic leakage.
