Plant–animal mutualisms such as seed dispersal are key interactions for sustaining plant range shifts. It remains elusive whether the organization of interactions with seed dispersers is reconfigured along the expansion landscape template and, if so, whether its effects accelerate or slow colonization. Here we analyse plant–frugivore interactions in a scenario of rapid population expansion of a Mediterranean juniper. We combined network analyses with field surveys, sampling interactions between individual plants and frugivores by DNA-barcoding and phototrapping over two seasons. We assess the role of intrinsic and extrinsic intraspecific variability in shaping interactions and we estimate the individual plant contributions to the seed rain. The whole interaction network was highly structured, with a distinct set of modules including individual plants and frugivore species arranged concordantly along the expansion gradient. The modular configuration was partially shaped by individual neighbourhood context (density and fecundity) and phenotypic traits (cone size). Interaction reconfiguration resulted in a higher and more uneven propagule contribution, with most effective dispersers having a prominent role at the colonization front stand, where a distinct subset of early arriving plants dominated the seed rain. Our study offers new insights into the key role of mutualistic interactions in colonization scenarios by promoting fast plant expansion processes.

Background Biological drugs used for IBD are detected in breast milk at a concentration below 10% of the maternal serum concentration and have therefore been considered as permitted drugs. However, warnings have recently been published regarding vaccination with live agents in children whose mothers receive infliximab during breastfeeding. Aim To assess the risk of serious adverse events related to the administration of live vaccines in children exposed to biological drugs in utero or whose mothers were receiving biological agents during breastfeeding. Methods Children born to IBD mothers from DUMBO registry of GETECCU were included. DUMBO is a prospective, observational and multicentre registry, which enrolls pregnant women with IBD over 5 years in 70 centres in Spain. Data on treatment during gestation, type of lactation, breastfeeding, end-date of breastfeeding, maternal treatments during breastfeeding and serious adverse events in children from birth are being prospectively included contacting with the mothers every-3-months. Following the Spanish immunization calendar, rotavirus vaccine is (voluntarily) administered at 2, 4 and 6 (3rd dose only with Rotateq®) months; measles, mumps and rubella (MMR) at 12 months and 3-4 years of age; and varicella at 15 months and 3-4 years of age. Results 526 newborns were included in the registry at the time of data analysis. A total of 205 (39%) had been exposed to biologics during pregnancy or breastfeeding (table 1): 80 (42%) during pregnancy, 7 (2%) during breastfeeding, and 109 (57%) during both. Newborn’s demographics and exposure to drugs during breastfeeding are summarized in table 2. Mean follow-up was 12 months; proportion of children breastfed during follow-up is shown in table 3a. The percentage of children who had been vaccinated according to the recommended schedule was above 95% at all visits (table 3b). Live vaccines administered to children exposed in utero to biologics during the 3rd trimester of gestation are shown in table 4a. From birth, 71% of infants were breastfed (52% exclusively breastfed). Live vaccines administered to children breastfed at least until month 6, until month 12 and until month 15 are summarized in table 4b. No serious adverse event related to live vaccine was reported in our cohort. Conclusion Administration of live virus vaccines from 12 months of age in children born to IBD mothers and exposed to biological drugs in utero or during breastfeeding seems safe and should not be recommended against vaccination or breastfeeding. Rotavirus vaccine (under 6 months of age) appears to be equally safe in these children.

Background Main aim: To assess the durability of tofacitinib treatment in patients with ulcerative colitis (UC). Secondary aims: To assess the short and long-term effectiveness; the tolerability of tofacitinib in clinical practice; and to evaluate the evolution of extraintestinal manifestations (EIMs) and immunomediated inflammatory diseases (IMIDs). Methods Retrospective, multicenter study including UC patients who had received the first tofacitinib dose at least 8 weeks before the inclusion. Patients were followed-up from the first tofacitinib dose to treatment discontinuation or last visit, whichever came first. Only patients with active disease [Partial Mayo Score (PMS)>2] at tofacitinib start were considered in the effectiveness analysis. Clinical effectiveness was based on PMS. In patients who stopped tofacitinib before their last visit, the last observation carried forward method was used to impute missing values at subsequent time points. Results 408 patients were included (figure 1). Incidence rate of tofacitinib discontinuation was 41% per patient-year of follow-up. The probability of maintaining tofacitinib is shown in figure 2a. Main reasons for tofacitinib withdrawal were primary non-response (44%) and loss of response (26%). Age at the start of tofacitinib (older) (HR=0.98, 95%CI=0.97-0.99) and the severity of clinical activity were associated with tofacitinib withdrawal (mild vs. remission: HR=1.5, 95%CI=0.5-4; and moderate-severe vs. remission: HR=3.0, 95%CI=1.2-7.4). Short-term effectiveness is shown in figure 3a. To have moderate-severe vs. mild disease activity at baseline (OR=0.2, 95%CI=0.1-0.4) and age at tofacitinib start (older) (OR=1.01, 95%CI=1.002-1.03) were associated with clinical remission at week 8. The probability of maintaining response in shown in figure 2b. Tofacitinib dose was escalated in 55 patients (66%) of those who had lost response, and 82% of them improved (60% regained remission). The proportion of patients on 10 mg b.i.d was over 40% in all timepoints during follow-up. The proportion of patients in clinical remission during follow-up in shown in figure 3b. Adverse events during tofacitinib treatment are summarized in figure 4. There was not any signal of negative impact of tofacitinib on EIM o IMIDs. Conclusion Tofacitinib is effective in inducing remission even in highly refractory UC patients. A relevant proportion of patients discontinue the treatment, mostly due to primary failure. Dose escalation is effective to regain response after loss of efficacy. The safety profile is similar to that previously reported

El cancer musculoesqueletico primario es una patologia muy poco frecuente. Las cifras de incidencia de los sarcomas oseos y de partes blandas, con respecto al total de tumores malignos, son del 0,18 % y del 1 %, respectivamente. La inespecificidad de los sintomas y signos de la enfermedad hace que sea fundamental su sospecha clinica para evitar retrasos diagnosticos y terapeuticos. El medico de Atencion Primaria dispone de diferentes herramientas para solicitar la valoracion de los pacientes por el medico de Atencion Especializada. En la Comunidad Autonoma de Madrid, las derivaciones ordinarias y preferentes se realizan a traves de la aplicacion informatica de ?Solicitud de Cita de Atencion Especializada? (SCAE). Cuando la sospecha es de cancer musculoesqueletico, se utiliza el circuito de ?Sospechas de Malignidad? (SM). El objetivo principal del estudio fue determinar la capacidad del circuito SCAE-SM para identificar las solicitudes susceptibles de adelanto de cita de los pacientes con tumores musculoesqueleticos benignos agresivos y malignos en el area sanitaria del Hospital Universitario 12 de Octubre. De forma secundaria, se planteo analizar la adecuacion del uso de la aplicacion informatica por el medico de Atencion Primaria, determinar la fiabilidad inter-observador entre distintos especialistas e identificar deficits formativos, tanto en los facultativos de Atencion Primaria como en los facultativos traumatologos. Para ello, se ha realizado un estudio observacional descriptivo transversal, con el fin de determinar el efecto y la utilidad del circuito. Los resultados obtenidos demostraron que la gran mayoria de las solicitudes realizadas (91,91 %) no fueron adecuadas al uso de la aplicacion. Solo el 8,23 % de los pacientes remitidos presentaron enfermedad benigna agresiva o maligna. Ademas, el hecho de que solo el 18,81 % de los pacientes llegaran a la Unidad de Tumores Musculoesqueleticos a traves del circuito SCAE-SM confirma la escasa adecuacion al uso de la aplicacion. En cuanto a la informacion aportada por el medico de Atencion Primaria, solo en el 28,87 % de los pacientes que se remitieron por dolor se especifico el tipo de este y hasta el 32 % de los casos fueron derivados sin haberse realizado una radiografia simple en dos proyecciones. Por otro lado, la demora media entre la solicitud de cita desde Atencion Primaria y el comienzo de la Atencion Especializada fue de 18,11 dias. Por lo que respecta a la fiabilidad inter-observador, 11 de los 13 evaluadores adelantaron la cita a mas del 85 % de los pacientes con patologia benigna agresiva o maligna, con mayor concordancia entre los facultativos con formacion especifica moderada. La aplicacion SCAE-SM es una herramienta util, susceptible de mejoras, con una escasa adecuacion tanto en su uso como en la informacion que se aporta. Existe un deficit formativo de los facultativos de Atencion Primaria, tanto en el conocimiento de la enfermedad como en el uso de la aplicacion. Los facultativos con formacion especifica moderada serian los ideales para la evaluacion de las solicitudes, probablemente por conjugar un conocimiento oncologico suficiente, con las dudas propias de una experiencia limitada.

Prostate cancer (PCa) accounts for more than 1 in 5 diagnoses and is the second cause of cancer-related deaths in men. Although PCa may be successfully treated, patients may undergo cancer recurrence and there is a need for new biomarkers to improve the prediction of prostate cancer recurrence and improve treatment. Our laboratory demonstrated that HLA-B-associated transcript 1 (BAT1) was differentially expressed in patients with high Gleason scores when compared to low Gleason scores. BAT1 is an anti-inflammatory gene but its role in PCa has not been identified. The objective of this study is to understand the role of BAT1 in prostate cancer. In vitro studies showed that BAT1 down-regulation increased cell migration and invasion. In contrast, BAT1 overexpression decreased cell migration and invasion. RT-PCR analysis showed differential expression of pro-inflammatory cytokines (TNF-α and IL-6) and cell adhesion and migration genes (MMP10, MMP13, and TIMPs) in BAT1 overexpressed cells when compared to BAT1 siRNA cells. Our in vivo studies demonstrated up-regulation of TNF-α, IL-6, and MMP10 in tumors developed from transfected BAT1 shRNA cells when compared to tumors developed from BAT1 cDNA cells. These findings indicate that BAT1 down-regulation modulates TNF-α and IL-6 expression which may lead to the secretion of MMP-10 and inhibition of TIMP2.

Understanding how fisheries influence seabird distribution is critical in the development of sustainable fisheries management. Species distribution models were applied to analyse the influence of the fishing footprint, discards, and oceanographic factors on seabird attendance patterns to trawlers in the Gulf of Cádiz. More than 30 species of seabirds were attracted to trawlers, although only seven were registered with relatively high frequency and abundance. For these species, fishery footprint was a good predictor for the distribution of four out of seven (yellow-legged gull, lesser black-backed gull, northern gannet, and Cory's shearwater). Yellow-legged and lesser black-backed gull flocked in large numbers to trawlers, mostly in shallow areas where they were also attracted by purse seines. Northern gannets and Cory's shearwaters appeared with relatively high frequency and their distributions correlated with trawler effort and abundance of potential prey discards. Balearic shearwaters were attracted in low numbers and concentrated in shallow areas, where potential prey discards were also more abundant. For these three species, discards as surrogate of natural prey distribution were good predictors of trawler attendance. Our research revealed that, at a local scale, the degree of interaction of seabirds on fishing discards varies by species. Therefore, fine-scale studies are essential in identifying interactions between seabirds and fisheries, and thus assessing conservation issues such as bycatch or the consequences of discard bans.

Fogh Rasmussen, GH, Madeleine, P, Arroyo-Morales, M, Voigt, M, and Kristiansen, M. Resistance training-induced acute hypoalgesia in women with persistent pain after breast cancer treatment. J Strength Cond Res XX(X): 000-000, 2022-The aim of this study was to determine whether a single bout of resistance training (RT) produces acute exercise-induced hypoalgesia (EIH) in breast cancer survivors (BCS) suffering from persistent pain ≥1.5 years after treatment. Twenty individuals with self-reported pain ≥3 on a 0-10 Numerical Rating Scale after treatment for breast cancer completed 3 experimental sessions, (a) familiarization; (b) 1 repetition maximum (1RM) normalization, and (c) training, consisting of 3 sets of 10 repetitions at 60% of 1 repetition maximum. Pressure pain thresholds (PPTs) were measured before and after training for the dorsal and ventral shoulder regions of the affected side. Movement-evoked pain (MEP) and rating of perceived exertion (RPE) were collected immediately after each set. A p-value less than 0.05 was considered statistically significant. The results demonstrated a significant increase in PPTs of the ventral shoulder region after a single bout of RT (p ≤ 0.05), indicating a localized analgesic response for this area. By contrast, no change was detected in PPTs on the dorsal shoulder region. No significant differences were found in MEP between sessions despite a significant increase in load and RPE during 1RM assessment (p ≤ 0.05), indicating that MEP was not affected by increase in absolute and relative intensity. In conclusion, a single bout of submaximal RT reduced PPTs for the ventral shoulder region of BCS with persistent pain after treatment and was well tolerated. Hence, RT may be a useful therapeutic tool for managing persistent pain after breast cancer treatment in clinical practice.

Endozoochory is a mutualistic interaction between plants and animals. Such a relationship has rarely been examined in the Eurasian otter Lutra lutra. This study aimed to assess the use and electivity of fruit by this carnivore, along with the viability of ingested seeds. Otter spraints and fruit were collected from the River Bullaque (Guadiana River basin, central Spain) in June and September 2018-2019. A high occurrence of fruit (> 40%) was found in spraints during September. Otters ingested fruit in June from only one plant species: Iberian bushweed Flueggea tinctoria (Phyllanthaceae); whereas seeds ingested in September belonged to four plant species: apple mint Mentha suaveolens (Lamiaceae), common hawthorn Crataegus monogyna, dog rose Rosa canina and elmleaf blackberry Rubus ulmifolius (three Rosaceae species). According to the environmental availability, otters displayed avoidance for mint and rose, neutral selection for hawthorn and preference for blackberry. Germination was unsuccessful for mint seeds, whereas germination was the highest for blackberry (37%; 49% for blackbird Turdus merula, a well-known frugivorous species). Otters appear to display an ecological role as seed dispersers for riparian vegetation in Iberian fresh waters. Background Data on the outcomes of surgery due to IBD in pregnant patients is scarce, and primarily dates back more than 3 decades ago. Primary aim: to evaluate the evolution of pregnancies and offspring after surgery due to IBD. Secondary aims: to describe the indications for surgery, the surgical techniques used, and the frequency of caesarean section concomitant to surgery for IBD Methods SCAR is a retrospective, multicenter study approved by ECCO COllaborative Network For Exceptionally Rare Case Reports (CONFER). Patients operated on due to IBD during pregnancy after 1998 (first biologic agent approved) were included. Data on patients’ demographics, IBD characteristics, medical treatments, IBD activity, pregnancy outcomes, surgery, delivery, foetal and maternal outcomes, were recorded Results 44 IBD patients were included (figure 1), all singleton pregnancies. Seven patients (16%) were diagnosed with IBD during pregnancy, 75% had Crohn’s disease (51% fistulizing behavior), and 23% had ulcerative colitis. 34% had previously undergone surgery due to IBD. Several complementary examinations were performed during pregnancy without complications (figure 2) Patients were being administered the following medications: corticosteroids (61%), biologicals (57%), thiopurines (23%) with 20% being on biologics and thiopurines. 93% of surgeries were performed in University hospitals, (2nd Trimester: 55%; 3rd trimester: 27%) with 77% of them being urgent surgeries (figure 3) One patient had hemoperitoneum during surgery, and 27% had postsurgical complications. No woman died. 62.5% of deliveries were induced (figure 4), 70% of them by C-section, with 51% of mothers having a prolonged hospitalization. There were 40 newborns alive and 4 miscarriages/stillbirths (1 in the 1st, 2 in the 2nd, and 1 in the 3rd trimester): 2 occurred during surgery and 2 occurred 2 weeks after surgery. 14% of the surgeries during the 2nd trimester and 64% of those in the 3rd trimester ended up with a simultaneous C-section or delivery. Of the 40 newborns alive, there were 9 healthy (24%), 61% premature, and 47% with low birth weight. A total of 42% of newborns needed hospitalization (25% in the intensive care unit, mainly due to respiratory distress) Conclusion The need for surgery for IBD during pregnancy remains an extremely serious situation. Maternal and foetal mortality are lower than previously described, most likely due to better supporting care. However, even in the current years, 30% of the mothers and 80% of newborns have complications, and 25% of the offspring need admission to the intensive care unit. Therefore, surgical management should be performed by a multidisciplinary team, involving gastroenterologists, colorectal surgeons, obstetricians and neonatal specialists.

Background The safety of ustekinumab in pregnant patients with inflammatory bowel disease (IBD) and in their offspring has been barely studied. Aims Primary: To know the risk of serious adverse events (SAEs) in women exposed to ustekinumab during pregnancy and in their offspring. Secondary: To assess the risk of complications of the mothers and their offspring. To describe the patterns of use of ustekinumab during pregnancy in these patients Methods Patients with IBD exposed to ustekinumab during pregnancy from DUMBO registry of GETECCU were included. DUMBO is a prospective, observational and multicentre registry, which enrols pregnant women with IBD over 5 years in 70 centres in Spain. The registry was kicked off in September 2019. SAE definition was based on “Clinical Safety Data Management: Definitions and Standards for Expedited Reporting by European Medicines Agency”. Study protocol is summarized in figure 1. Results 49 pregnant patients have been exposed to ustekinumab during pregnancy so far (table 1). All pregnancies were singleton. Two patients were lost of follow-up (1st and 2nd trimester) but had uneventful pregnancies up-to last visit. There were 2 miscarriages (4%) at 1st trimester of gestation, 34 newborns and 11 pregnancies that were still on-going at the time of this analysis. All patients were on ustekinumab at conception (57% of them 90 mg/8 weeks). A total of 12 patients (24%) withdrawn ustekinumab during pregnancy: 1 (8%) due to disease flare, 1 (8%) underwent surgery due to intestinal obstruction, 2 (17%) due to patient’s choice (at 1st and 2nd trimester), and 8 (67%) due to clinicians’ decision (1 at 1st, 5 at 2nd and 2 at 3rd trimesters). No patient flared up after ustekinumab discontinuation. 10 (20%) patients had SAEs: 2 miscarriages, 1 intestinal infection, 1 subcorionic hematoma, 3 preterm birth, 1 intestinal obstruction and perforation (underwent surgery), 1 preeclapmsia, and 1 stoma obstruction. A total of 34 women gave birth after a median of 39 weeks gestation [interquartile range (IQR)=38–40], 3 (9%) preterm births, 55% by caesarean section (82% obstetric reasons and 18% perianal fistulae). Of the 34 newborns, 53% were female, median birth weight was 3,110 g (IQR=2,820–3,325), 3 (9%) low-birth weight, and 50% were breastfed exclusively. Median babies’ follow-up was 12 months (IQR=7–16). During follow-up, 3 children (9%) had severe infections (2 urinary infections, and 1 bronchiolitis by respiratory syncytial virus). In addition, 4 (13%) children were hospitalized: 1 cardiorespiratory arrest, 1 prematurity, 1 jaundice, and 1 vesicoureteral reflux Conclusion Ustekinumab seems to be safe during pregnancy in patients with IBD and their offspring.

An endoscopist’s adenoma detection rate (ADR) is inversely related to interval colorectal cancer risk and cancer mortality. Previous studies evaluating the impact of gastroenterology fellow participation in colonoscopy on ADR have generated conflicting results. We aimed to determine the impact of fellow participation, duration of fellowship training, and physician sex on ADR and advanced ADR (AADR). We retrospectively analyzed average-risk patients undergoing screening colonoscopy at Veterans Affairs New York Harbor Healthcare System Brooklyn Campus and Kings County Hospital Center. Review of colonoscopy and pathology reports were performed to obtain adenoma-specific details, including the presence of advanced adenoma and adenoma location (right vs. left colon). There were 893 colonoscopies performed by attending only and 502 performed with fellow participation. Fellow participation improved overall ADR (44.6% vs. 35.4%, p

Accurate estimates of soil unit weight are fundamental for correctly post process CPTu data and making use of Soil Behavior Type-based classification systems. Soil-specific and global regressions have been proposed for this purpose. However, soil-specific correlation might pose a problem of pertinence when applied at new sites. On the other hand, global correlations are easy to apply, but generally carry large systematic uncertainties. In this context, this work proposes a data clustering technique applied to geotechnical database aiming to identify hidden linear trends among dimensionless soil unit weight and normalized CPTu parameter according to some unobservable soil classes. Global correlations are then revisited according to such data subdivision aiming to improve accuracy of soil unit weight prediction while reducing transformation uncertainty. A new probabilistic criterion for soil unit weight prediction is also obtained. The potential benefits of the proposed procedure are illustrated with data from a Llobregat delta site (Spain).

Ectopic pregnancy is a quite common and life-threatening pregnancy. The most common site of ectopic implantation of a fertilized embryo is the fallopian tube. In extremely rare situations, the embryo can implant in other locations, which makes the diagnosis and management even more complex. Although close observation of a new pregnancy is key in the diagnosis and treatment of an ectopic pregnancy, there is still a major risk of life-threatening outcomes. This is a case report of a 31-year-old patient with a history of multiple pregnancies who presented to a community hospital in the spring of 2021 with an ectopic caesarean scar pregnancy. A diagnosis of ectopic pregnancy was on a timely basis, and surgical management was advised. Upon refusal of treatment and admission, pharmacological management was initiated, but patient compliance challenged the success of the therapy. Patient non-compliance to close follow-up resulted in a ruptured uterus. Emergency laparotomy with supracervical hysterectomy was performed as a life-saving procedure.

La calidad no solo se evalúa por el ámbito académico sino también por la calidad en los procesos de apoyo y de servicios. Como una solución que permite dar soporte a todos los procesos institucionales en un contexto de mejora continua, aseguramiento de la calidad y pertinencia, se presenta el Sistema de Gestión de la Información llamado “SISCE”. Today, all organizations use information management systems to support all of their processes. Higher-level institutions, in a competitive and globalized environment, are no exception; by the demands of this environment, are subject to constant evaluation, accreditation and certification processes, in order to guarantee to the society, to whom it provides its service, a relevant and quality education, which is also recognized by the different sectors. Quality is not only assessed by the academic field but also by the quality of the support processes and services. As a solution to support all institutional processes in a context of continuous improvement, quality assurance and relevance, the Information Management System called "SISCE" is presented.

Ante el aumento en la oferta de programas computarizados para la rehabilitación neuropsicológica de pacientes con daño cerebral adquirido, es importante conocer la eficacia de éstos y las ventajas de su aplicación sobre diferentes procesos. Este artículo revisa el uso de estos programas en la rehabilitación neuropsicológica de las funciones ejecutivas en pacientes con daño cerebral adquirido. La búsqueda se realizó en fuentes como PUBMED, búsquedas a través de internet y algunas listas de referencias bibliográficas. Se revisaron artículos controlados aleatorizados y por las características del tema, también se incluyeron estudios cuasi experimentales y un estudio de casos. Asimismo, solo se seleccionaron artículos que incluyeran pacientes adultos con daño cerebral adquirido y que tuvieran fallos en funciones ejecutivas sin especificar alguna en particular y que hayan recibido rehabilitación cognitiva a través de programas de ordenador. Los resultados reflejan debilidades en el pareamiento de los grupos controles, el tamaño de sus muestras, y en el uso de instrumentos de medición de las funciones ejecutivas para objetivar la eficacia. En general, los programas revisados no demuestran tener mayor efectividad que aquellas intervenciones que tradicionales a lápiz y papel. Actualmente la pertinencia de los programas educativos está fuertemente ligada al desarrollo de competencias en los estudiantes. Es por ello que los organismos acreditadores, están dirigiendo el enfoque de los criterios de evaluación hacia el aseguramiento de las competencias, haciendo énfasis en la trayectoria escolar y el seguimiento de su avance curricular, y así los estudiantes desarrollan las competencias definidas en el perfil de egreso del programa. Los criterios de evaluación específicos establecen los requerimientos mínimos con los que los programas aseguran contar con las condiciones y los recursos necesarios para abonar al desarrollo de las competencias de los estudiantes. Ante el desafío de recortes presupuestales, un programa integral de desarrollo de competencias que ha dado resultado para satisfacer ampliamente varios criterios de acreditación, en la Universidad Tecnológica del Suroeste de Guanajuato, es el Club de Desarrollo de Aplicaciones, garantizando la pertinencia de los programas en TIC de la institución. Currently the relevance of educational programs is strongly linked to the development of skills in students. That is why the accrediting boards are directing the focus of the evaluation criteria towards the assurance of competences, emphasizing the school trajectory and the follow-up of their curricular advancement, and so the students develop the competences defined in the profile of exit of the program. The specific evaluation criteria establish the minimum requirements with which the programs ensure that they have the necessary conditions and resources to pay for the development of the students' competences. Faced with the challenge of budget cuts, a comprehensive competency development program that has been successful in widely satisfying several accreditation criteria, at the Technological University of the Southwest of Guanajuato, is the Application Development Club, guaranteeing the relevance of the programs in IT of the institution.
