Hypothesis-driven research rests on clearly articulated scientific theories. The building blocks for communicating these theories are scientific terms. Obviously, communication - and thus, scientific progress - is hampered if the meaning of these terms varies idiosyncratically across (sub)fields and even across individual researchers within the same subfield. We have formed an international group of experts representing various theoretical stances with the goal to homogenize the use of the terms that are most relevant to fundamental research on visual distraction in visual search. Our discussions revealed striking heterogeneity and we had to invest much time and effort to increase our mutual understanding of each others use of central terms, which turned out to be strongly related to our respective theoretical positions. We present the outcomes of these discussions in a glossary and provide some context in several essays. Specifically, we explicate how central terms are used in the distraction literature and consensually sharpen their definitions in order to enable communication across theoretical standpoints. Where applicable, we also explain how the respective constructs can be measured. We believe that this novel type of adversarial collaboration can serve as a model for other fields of psychological research that strive to build a solid groundwork for theorizing and communicating by establishing a common language. For the field of visual distraction, the present paper should facilitate communication across theoretical standpoints and may serve as an introduction and reference text for newcomers.

Growth hormone (GH) has been used for over 35 years, and its safety and efficacy has been studied extensively. Experimental studies showing the permissive role of GH/insulin-like growth factor 1 (IGF-I) in carcinogenesis have raised concerns regarding the safety of GH replacement in children and adults who have received treatment for cancer and those with intracranial and pituitary tumours. A consensus statement was produced to guide decision-making on GH replacement in children and adult survivors of cancer, in those treated for intracranial and pituitary tumours and in patients with increased cancer risk. With the support of the European Society of Endocrinology, the Growth Hormone Research Society convened a Workshop, where 55 international key opinion leaders representing 10 professional societies were invited to participate. This consensus statement utilized: (1) a critical review paper produced before the Workshop, (2) five plenary talks, (3) evidence-based comments from four breakout groups, and (4) discussions during report-back sessions. Current evidence reviewed from the proceedings from the Workshop does not support an association between GH replacement and primary tumour or cancer recurrence. The effect of GH replacement on secondary neoplasia risk is minor compared to host- and tumour treatment-related factors. There is no evidence for an association between GH replacement and increased mortality from cancer amongst GH-deficient childhood cancer survivors. Patients with pituitary tumour or craniopharyngioma remnants receiving GH replacement do not need to be treated or monitored differently than those not receiving GH. GH replacement might be considered in GH-deficient adult cancer survivors in remission after careful individual risk/benefit analysis. In children with cancer predisposition syndromes, GH treatment is generally contraindicated but may be considered cautiously in select patients.

In contrast to adults, meningiomas are uncommon tumors in childhood and adolescence. Whether adult and pediatric meningiomas differ on a molecular level is unclear. Here we report detailed genomic analyses of 37 pediatric meningiomas by sequencing and DNA methylation profiling. Histologically, the series was dominated by meningioma subtypes with aggressive behavior, with 70% of patients suffering from WHO grade II or III meningiomas. The most frequent cytogenetic aberrations were loss of chromosomes 22 (23/37 [62%]), 1 (9/37 [24%]), 18 (7/37 [19%]), and 14 (5/37 [14%]). Tumors with NF2 alterations exhibited overall increased chromosomal instability. Unsupervised clustering of DNA methylation profiles revealed separation into three groups: designated group 1 composed of clear cell and papillary meningiomas, whereas group 2A comprised predominantly atypical meningiomas and group 2B enriched for rare high-grade subtypes (rhabdoid, chordoid). Meningiomas from NF2 patients clustered exclusively within groups 1 and 2A. When compared with a dataset of 105 adult meningiomas, the pediatric meningiomas largely grouped separately. Targeted panel DNA sequencing of 34 tumors revealed frequent NF2 alterations, while other typical alterations found in adult non-NF2 tumors were absent. These data demonstrate that pediatric meningiomas are characterized by molecular features distinct from adult tumors.

Individuals with Autism Spectrum Disorder (ASD) are thought to under-rely on prior knowledge in perceptual decision-making. This study examined whether this applies to decisions of attention allocation, of relevance for 'predictive-coding' accounts of ASD. In a visual search task, a salient but task-irrelevant distractor appeared with higher probability in one display half. Individuals with ASD learned to avoid 'attentional capture' by distractors in the probable region as effectively as control participants--indicating typical priors for deploying attention. However, capture by a 'surprising' distractor at an unlikely location led to greatly slowed identification of a subsequent target at that location--indicating that individuals with ASD attempt to control surprise (unexpected attentional capture) by over-regulating parameters in post-selective decision-making.

Contextual cueing refers to the guidance of search by associative learning of the location of task-relevant target items in relation to the consistent arrangement of distractor ("context") items in the search display. The present study investigated whether such target-distractor associations could also be formed in a cross-modal search task in which the invariant distractor context is visual and the target is tactile. Each trial display consisted of 8 vibrotactile stimuli delivered to 4 fingers of each hand, with the target singled out by a vibrotactile feature difference relative to the homogeneous nontarget vibrations. In addition, there were 4 visual (Gabor) distractors and 4 empty circles colocated (on a different depth plane) with the vibrotactile stimuli. Crucially, in half of the trials, the location of the tactile target was associated with an identical (repeated) configuration, or "context," of visual distractors. The results revealed facilitated RTs to the tactile targets when they were presented (250 to) 450 ms prior to--but not simultaneously with or after--a repeated visual distractor configuration, demonstrating a novel visuo-tactile contextual-cueing effect. Additional tests indicated that the optimal tactile preview period is broadly in line with the time required to recode the (initially) somatotopically sensed tactile stimuli into an external format shared with the visual items. We propose that, besides divided attention between the 2 modalities, the acquisition of search-guiding cross-modal context associations critically depends on the spatiotemporal colocation of the visuo-tactile stimuli.

Survivors of sellar/suprasellar tumors involving hypothalamic structures face a risk of impaired quality of life, including tumor- and/or treatment-related hypothalamic obesity (TTR-HO) defined as abnormal weight gain resulting in severe persistent obesity due to physical, tumor- and/or treatment related damage of the hypothalamus. We analyze German claims data to better understand treatment pathways for patients living TTR-HO during the two years following the index surgical treatment. A database algorithm identified patients with TTR-HO in a representative German payer claims database between 2010 and 2021 (n = 5.42 million patients). Claims from 37 patients with TTR-HO were analyzed on a quarterly basis over 2 years. The analysis considered inpatient, outpatient, and prescription data. In the follow-up period, patients with TTR-HO are hospitalized 3.68 times on average; 37% of hospitalizations in year 1 and 31% in year 2 are due to TTR-HO. On average, patients see a general practitioner 12.27 times and various specialists 20.45 times. The need for complex neuroendocrine therapy develops quickly, with most patients having 2–3 neuroendocrine prescriptions in any given quarter. The management of patients with TTR-HO requires frequent inpatient and outpatient visits for tumor follow-up and management of incident comorbidities, and most patients with TTR-HO require intense polytherapy. Hypothalamic obesity (HO) is defined as abnormal weight gain resulting in severe persistent obesity due to physical, tumor‐ and/or treatment‐related damage to the hypothalamus. HO epidemiology is poorly understood. We developed a database algorithm supporting the standardized identification of tumor/treatment‐related HO (TTR‐HO) patients. The algorithm is used to estimate incidence rates of TTR‐HO patients in the German healthcare context from a representative claims database (n = 5.42 million) covering 2010–2020. Patients were identified based on surgery/radiotherapy procedures and HO‐associated tumor diagnoses (n = 3976). HO was defined by incident obesity and validated based on incident diabetes insipidus diagnoses and desmopressin prescription within a 12‐month period after surgery/radiotherapy. Uncertainty due to algorithm definitions is explored in sensitivity analyses. Estimated annual incidence of TTR‐HO in Germany is between 0.7 and 1.7 cases per 1,000,000 persons (2019 prevalence: n = 1262 patients). With observed cases in all age groups, two HO‐incidence peaks are identified: children/young adults aged 10–24 years and adults aged 40–44 years. Most frequent HO‐validated tumor diagnoses are benign sellar/suprasellar tumors (6.1/1,000,000 persons over 9 years), including tumors of the craniopharyngeal duct (1.3/1,000,000), neoplasms of the pituitary gland (4.1/1,000,000), and nonspecific brain tumors of endocrine glands (2.4/1,000,000). This is the first real‐world database analysis of TTR‐HO epidemiology, refining current estimates of HO epidemiology and early patient identification. A more comprehensive characterization of patients with HO as well as a better understanding of clinical implications will be crucial in developing optimal treatment strategies to improve patient outcomes. A crucial function of our goal-directed behavior is to select task-relevant targets among distractor stimuli, some of which may share properties with the target and thus compete for attentional selection. Here, by applying functional magnetic resonance imaging (fMRI) to a visual search task in which a target was embedded in an array of distractors that were homogeneous or heterogeneous along the task-relevant (orientation or form) and/or task-irrelevant (color) dimensions, we demonstrate that for both (orientation) feature search and (form) conjunction search, the fusiform gyrus is involved in processing the task-irrelevant color information, while the bilateral frontal eye fields (FEF), the cortex along the left intraparietal sulcus (IPS), and the left junction of intraparietal and transverse occipital sulci (IPTO) are involved in processing task-relevant distracting information, especially for target-absent trials. Moreover, in conjunction (but not in feature) search, activity in these frontoparietal regions is affected by stimulus heterogeneity along the task-irrelevant dimension: heterogeneity of the task-irrelevant information increases the activity in these regions only when the task-relevant information is homogeneous, not when it is heterogeneous. These findings suggest that differential neural mechanisms are involved in processing task-relevant and task-irrelevant dimensions of the searched-for objects. In addition, they show that the top-down task set plays a dominant role in determining whether or not task-irrelevant information can affect the processing of the task-relevant dimension in the frontoparietal regions.

Accurate pathological diagnosis is crucial for optimal management of patients with cancer. For the approximately 100 known tumour types of the central nervous system, standardization of the diagnostic process has been shown to be particularly challenging-with substantial inter-observer variability in the histopathological diagnosis of many tumour types. Here we present a comprehensive approach for the DNA methylation-based classification of central nervous system tumours across all entities and age groups, and demonstrate its application in a routine diagnostic setting. We show that the availability of this method may have a substantial impact on diagnostic precision compared to standard methods, resulting in a change of diagnosis in up to 12% of prospective cases. For broader accessibility, we have designed a free online classifier tool, the use of which does not require any additional onsite data processing. Our results provide a blueprint for the generation of machine-learning-based tumour classifiers across other cancer entities, with the potential to fundamentally transform tumour pathology.

One of the most firmly established factors determining the speed of human behavioral responses toward action-critical stimuli is the spatial correspondence between the stimulus and response locations. If both locations match, the time taken for response production is markedly reduced relative to when they mismatch, a phenomenon called the Simon effect. While there is a consensus that this stimulus-response (S-R) conflict is associated with brief (4-7 Hz) frontal midline theta (fmθ) complexes generated in medial frontal cortex, it remains controversial (1) whether there are multiple, simultaneously active theta generator areas in the medial frontal cortex that commonly give rise to conflict-related fmθ complexes; and if so, (2) whether they are all related to the resolution of conflicting task information. Here, we combined mental chronometry with high-density electroencephalographic measures during a Simon-type manual reaching task and used independent component analysis and time-frequency domain statistics on source-level activities to model fmθ sources. During target processing, our results revealed two independent fmθ generators simultaneously active in or near anterior cingulate cortex, only one of them reflecting the correspondence between current and previous S-R locations. However, this fmθ response is not exclusively linked to conflict but also to other, conflict-independent processes associated with response slowing. These results paint a detailed picture regarding the oscillatory correlates of conflict processing in Simon tasks, and challenge the prevalent notion that fmθ complexes induced by conflicting task information represent a unitary phenomenon related to cognitive control, which governs conflict processing across various types of response-override tasks.SIGNIFICANCE STATEMENT Humans constantly monitor their environment for and adjust their cognitive control settings in response to conflicts, an ability that arguably paves the way for survival in ever-changing situations. Anterior cingulate-generated frontal midline theta (fmθ) complexes have been hypothesized to play a role in this conflict-monitoring function. However, it remains a point of contention whether fmθ complexes govern conflict processing in a unitary, paradigm-nonspecific manner. Here, we identified two independent fmθ oscillations triggered during a Simon-type task, only one of them reflecting current and previous conflicts. Importantly, this signal differed in various respects (cortical origin, intertrial history) from fmθ phenomena in other response-override tasks, challenging the prevalent notion of conflict-induced fmθ as a unitary phenomenon associated with the resolution of conflict.

Repeated encounter of abstract target-distractor letter arrangements leads to improved visual search for such displays. This contextual-cueing effect is attributed to incidental learning of display configurations. Whether observers can consciously access the memory underlying the cueing effect is still a controversial issue. The current study uses a novel recognition task and eyetracking to tackle this question. Experiment 1 investigated observers' ability to recognize or "generate" the display quadrant of the target in a previous search array when the target was now substituted by distractor element as well as where observers' eye fixations would fall while they freely viewed the recognition display, examining the link between the fixation pattern and explicit recognition judgments. Experiment 2 tested whether eye fixations would serve a critical role for explicit retrieval from context memory. Experiment 3 asked whether eye fixations of the target region are critical for context-based facilitation of search reaction times to manifest. The results revealed longer fixational dwell times in the target quadrant for learned relative to foil displays. Further, explicit recognition was enhanced, and above chance level, when observers were made to fixate the target quadrant as compared to when they were prevented from doing so. However, the manifestation of contextual cueing of visual search did itself not require fixations of the target quadrant. Moreover, contextual-cueing of search reaction times was significantly correlated with both fixational dwell times and observers' explicit generation performance. The results argue in favor of contextual cueing of visual search being the result of a single, explicit, memory system, though it could nevertheless receive support from separable--automatic versus controlled--retrieval processes. Fixational eye movements, that is, the directed overt allocation of visual attention, provide an interface between these processes in context cueing.
