1. Langerhans cell histiocytosis mimicking acute otitis media in childhood—a case presentation

Abstract

Abstract Background Langerhans cell histiocytosis (LCH) is an abnormal polyclonal proliferation of Langerhans cells that affect different parts of the body. Temporal bone involvement is quite common among the involved sites. The etiology is unknown. Diagnosis is based on symptoms, imaging, and histopathology. Especially LCH in temporal bone is confused with acute or subacute otitis media. There are many treatment options in LCH. Case presentation Here, a 2-year-old pediatric patient with pain in the right ear was diagnosed as having LCH as a result of the examinations. MRG revealed multiple lesions in the temporal bone, sphenoid bone, and clivus. She was treated with steroids and vinblastine. Conclusion Possible tumoral formations should be kept in mind when children complain of otalgia and otitis media-like clinical picture for a long time.

2. Kikuchi-Fujimoto Disease as a Cause of Fever of Unknown Origin: A Case Report

Abstract

Histiocytic necrotizing lymphadenitis also known as Kikuchi-Fujimoto disease (KFD) is a benign, rare and self-limiting disease. A thirty-six years old woman had high fever and cervical and axillary lymphadenopathy for three months. Her symptoms prompted antibiotic therapy failure and extensive evaluations of fever of unknown origin (FUO). At admission there were several enlarged cervical and axillary lymph nodes and fever of 40°C. Laboratory findings and imaging studies did not show an infectious, rheumatologic or hematologic cause of her symptoms. Histopathological examination of an excisional lymph node biopsy revealed histiocytic necrotizing lymphadenitis as the underlying disease. After spontaneous resolution without specific therapy the patient is now symptomless 12 months after diagnosis. KFD which presents with fever and lymphadenopathy, should be considered in differential diagnosis of FUO. Physicians should be aware of this uncommon entity since early histologic recognition will minimize unnecessary evaluations and treatments.

Abstract

In this experimental study, the postnatal and fetal membranous bone healing were investigated radiologically, histologically and levels of growth factors. Sixteen sheep fetuses and 16 sheep were included in this study. In the fetal group, 5 mm diameter ostectomy, and a 10 mm osteotomy were created in 90th gestational day. In the postnatal group, similar ostectomy and osteotomies were created. In the early period, radiologically similar radiolucencies in the ostectomy areas were seen in both groups. Histologically, fetal bone healing was decreased in the early postoperative period. However, it was accelerated in further time points. Histomorphometric analyses revealed accelerated fetal bone healing. TGF-beta1 levels were higher and then lower in early and late postoperative periods respectively in the fetal group. In the postnatal group, the levels of TGF-beta1 were lower and the differences between two groups were statistically significant in all time points (p < 0.05). The FGF and PDGF levels in both areas were higher in early postoperative period whereas lower in the late period in both groups. However, the fetal FGF levels were higher compared to the postnatal group (p < 0.05). The fetal PDGF levels were lower compared to the postnatal ones (p < 0.05). In conclusion, the calvarial bone gap model at the end of the second trimester of gestation in the fetal sheep model proved useful in examining the membranous bone healing. Histologically, the process of fetal bone healing seems similar to that of postnatal healing, albeit at an accelerated rate. Histomorphometric evaluation is a valuable tool in the evaluation of bone formation and gives more objective information about the ratio of bone formation.

6. Bilateral cervical chondrocutaneous remnants: a case report and review of the literature

Abstract

Cervical chondrocutaneous remnants are less common lesions, which are encountered at the lateral neck. They are similar in appearance to preauricular tags, which are more frequent. Bilateral appearance of this pathology is quite uncommon. The lesions always present at birth, and are located in the middle or lower third of the lateral neck with a significant prevalence of location anterior to the sternocleidomastoid muscle. The overlying skin is similar to the surrounding neck skin and the lesion is painless, lacking any inflammation or discharge. Surgically there is no connection with deep underlying structures. The therapy of choice should be complete surgical removal. Several associated anomalies may accompany cervical chondrocutaneous remnants. Thus these patients must be evaluated carefully in order to detect any additional anomaly. We herein report a four-year-old patient with bilateral cervical chondrocutaneous remnant located at the inferior third of the lateral neck anterior to the sternocleidomastoid muscle. We also review the literature for patients with bilateral cervical chondrocutaneous remnants and discuss embryologic and diagnostic aspects.

7. Effect of octreotide in the prevention of doxorubicin cardiotoxicity

Abstract

A precise method for prevention from doxorubicin cardiotoxicity is not known. We examined whether octreotide has a protective effect against doxorubicin cardiotoxicity.New Zealand rabbits (n=44) were divided into 4 groups according to drugs given: Group A (n=12) doxorubicin and octreotide, Group B (n=12) only doxorubicin, Group C (n=10) only octreotide and Group D (n=10) only saline. Effects of the drugs were evaluated in terms of histopathological score, fractional shortening (FS) and prolongation of the QTc interval.Mean pathological score for cardiotoxicity (Group A: 3.7+/-0.5, Group B: 3.9+/-0.3), prolongation of QTc (Group A: from 244.5+/-21.2 ms to 282.9+/-25.9 ms, p0.0001; Group B: from 248.5+/-17.7 ms to 298.3+/-13.7 ms, p0.00001) and the rate of decrease in FS (Group A: from 34.4+/-2.0 to 28.0+/-2.0, p0.05; Group B: from 35.1+/-1.9 to 24.8+/-1.3, p0.05) were higher in Group B when compared to Group A, but only difference in the rate of decrease in FS was statistically significant (p0.001). None of these variables changed significantly in groups C and D.In this preliminary study, octreotide seems not to reduce doxorubicin cardiotoxicity. On the other hand, a consistent tendency of decreased cardiotoxicity in octreotide+doxorubicin group was observed, although only the difference in FS decrease was significant. Further investigations are needed to address the issue of the extent and the mechanisms of this effect.

Abstract

In the present study, we report a 65-year old man who had been diagnosed earlier as having chronic lymphocytic leukemia and treated for 2 years with fludarabine. He presented to our hospital 2 years later with clinical features of superior vena cava syndrome. The underlying cause was diffuse large B cell lymphoma obstructing the superior vena cava.

Abstract

Neurofibromatosis type 1 can be severe and associated with progressive course and death. Although it has been rarely reported, a subset of patients with neurofibromatosis type 1 may develop interstitial lung disease. However, no case of neurofibrosarcoma and lung involvement together has been reported so far. We report a case of a 45-year-old male who was previously diagnosed as having neurofibromatosis type 1, 12 years later after the initial diagnosis he was histologically confirmed to have malignant transformation, and 3 years later he had associated diffuse lung damage.

Abstract

Objective: Graft-versus-host disease (GVHD) is a major obstacle to successful allogeneic bone marrow transplantation (allo-BMT). While multipotent mesenchymal stromal cells (MSCs) demonstrate alloresponse in vitro and in vivo, they also have clinical applications toward prevention or treatment of GVHD. The aim of this study was to investigate the ability of MSCs to prevent or treat GVHD in a rat BMT model. Materials and Methods: The GVHD model was established by transplantation of Sprague Dawley rats' bone marrow and spleen cells into lethally irradiated (950 cGy) SDxWistar rat recipients. A total of 49 rats were randomly assigned to 4 study and 3 control groups administered different GVHD prophylactic regimens including MSCs. After transplantation, clinical GVHD scores and survival status were monitored. Results: All irradiated and untreated control mice with GVHD died. MSCs inhibited lethal GVHD as efficiently as the standard GVHD prophylactic regimen. The gross and histopathological findings of GVHD and the ratio of CD4/CD8 expression decreased. The subgroup given MSCs displayed higher in vivo proportions of CD25+ T cells and plasma interleukin-2 levels as compared to conventional GVHD treatment after allo-BMT. Conclusion: Our results suggest that clinical use of MSCs in both prophylaxis against and treatment of established GVHD is effective. This study supports the use of MSCs in the prophylaxis and treatment of GVHD after allo-BMT; however, large scale studies are needed.

Abstract

İntihar yöntemleri arasında sık rastlanan ası sonucu ölümlerde, orijinin belirlenmesine yönelik araştırma büyük önem taşımaktadır. Ası sonucu ölümlerde genellikle orijinin intihar olmasına karşın kaza ve cinayet olgularına da rastlanmaktadır. Ası teknikleri genel olarak aynı ise de, iskelet sisteminde meydana gelen travma ve ölüm nedenleri bireyler arasında farklılık göstermektedir. Bu olgularda orijinin belirlenmesinde, olay yeri incelemesi ve otopsi yapılması bir gereklilik olarak karşımıza çıkmaktadır. Ası yöntemi ile intihar ederek öldüğü bildirilen 22 yaşında erkek olguya otopsi yapıldı. Olay yeri incelemesinde, kişinin el bileklerinin birbirine ön taraftan bağlı olduğu, ölü muayenesinde, boyunda yükselici ve yüzeyelleşici nitelikte telem tespit edildi. Otopside, boyun arkada telem altındaki yumuşak dokuda ekimoz tespit edildi. Hyoid kemik, tiroid kıkırdak ve boyun omurları sağlam bulundu. Postmortem biyokimyasal incelemede; kanda, Triiyodotironin seviyesinin 13,71 pg/mL (Referans değeri: 2,57-4,43 pg/mL) olduğu tespit edildi. Bu olgu, ası sonucu bir ölüm olgusunun otopsisinde telem dışında belirgin makroskopik bulgu saptanmamış olmasının yanı sıra postmortem yapılan incelemede tiroid hormon seviyesinde yükseklik saptanması ve kişinin ellerini bağlayarak ası fiilini gerçekleştirmesi açısından ilginç bulunarak sunulmuştur.
