3. Leraar ben je in een school: voorbereiding op de schoolorganisatorische dimensie van het leraarschap

- Abstract
Leraren maken vanaf de allereerste werkdag deel uit van een schoolorganisatie. Empirische studies over de manier waarop toekomstige leraren tijdens hun opleiding worden voorbereid op hun functioneren in schoolorganisaties zijn echter beperkt. Via een exploratieve gevalsstudie is daarom onderzocht hoe student-leraren worden voorbereid op de schoolorganisatorische dimensie van het leraarschap. In het analytisch kader dat gehanteerd wordt in deze studie staat het concept ‘schoolorganisatorische competentie’ centraal. Hierbij wordt een onderscheid gemaakt tussen rolbewustzijn (attitude), micropolitieke geletterdheid (kennis) en samenwerking (vaardigheid). We hebben gekozen voor een meervoudige gevalsstudie van drie hogescholen met een hbo-opleiding tot leraar Basisonderwijs (Nederland). Dataverzameling gebeurde via curriculumanalyse (n=65) en de afname van semigestructureerde interviews met opleidingsdirecteuren, student-leraren en lerarenopleiders (n=18). Hierbij hadden we aandacht voor de manier waarop (wat, wanneer en hoe) deze voorbereiding gebeurt, alsook hoe deze ervaren wordt. De resultaten laten zien dat de voorbereiding gebeurt via Beroepscompetentie 5, waarin de ontwikkeling van samenwerkingsvaardigheden centraal staat. Deze voorbereiding gebeurt bovendien nauwelijks op een geïntegreerde manier in relatie met andere competenties: studenten moeten eerst competent worden in de klas, voordat ze het ‘schoolniveau’ kunnen betreden. Onze analyse identificeert ook het belang van authentieke leerervaringen in de ontwikkeling van de schoolorganisatorische competentie, zoals intervisie, groepsopdrachten en stage. De analyse heeft tot slot twee spanningen aan het licht gebracht met betrekking tot de schoolorganisatorische dimensie van het leraarschap: ‘autonomie versus samenwerking’ en ‘fitting in versus standing out’. We ronden dit artikel af met een theoretische reflectie op het belang van een geïntegreerde, authentieke en gezamenlijke aanpak.

Le début de carrière d’un enseignant ne se caractérise pas seulement par l’apprentissage de la prise en charge de sa classe et de ses élèves, mais aussi par la façon de devenir membre d’une équipe scolaire. Pourtant, les curricula de formation des enseignants et les recherches sur l’insertion professionnelle accordent peu d’attention à cette intégration organisationnelle. Cet article examine en quoi et comment ces questions d’ordre organisationnel sont abordées dans les programmes de formation à l’enseignement. Plus précisément, nous nous concentrons sur les perceptions des futurs enseignants et de leurs formateurs à propos de la préparation à la prise en charge d’un rôle organsiationnel. Une étude de cas multiples a été menée au sein de trois programmes de formation à l’enseignement primaire au Pays-Bas. Plus spécifiquement, dans chaque établissement, nous avons réalisé une analyse qualitative des programmes-cadres et des entrevues avec des directeurs de programmes, des formateurs d’enseignants et des étudiants. Les résultats ont permis d’identifier différentes tensions dans la façon dont les futurs enseignants sont préparés à ce rôle organisationnel.

Deze studie is vertrokken vanuit de vaststelling dat een belangrijke uitdaging voor scholen niet zozeer het gebrek aan innovatieve praktijken is, maar wel het verduurzamen hiervan in de praktijk.Onderwijskundig onderzoek heeft immers aangetoond dat de implementatie van vernieuwingen tot diverse en vaak niet duurzaam verankerde praktijken kan leiden. Vernieuwingsinitiatieven zijn veelal van korte duur en scholen slagen er niet altijd in de vernieuwingsideeën te verspreiden binnen of buiten de eigen school. Ook wisselende beleidsprioriteiten en vluchtige beleidsfinanciering maken dat heel wat onderwijsvernieuwingen verdwijnen voor ze goed en wel ingevoerd zijn. Enkele vragen die hierbij opduiken zijn: Hoe kunnen we succesvolle innovatieve praktijken verder uitbouwen binnen en buiten de eigen klas?; Hoe kunnen we een succesvolle vernieuwing binnen een school verspreiden naar andere scholen?; Hoe kunnen we garanderen dat de vernieuwing beklijft, ook wanneer de externe ondersteuning wegvalt?; Wat gebeurt er wanneer de betrokken leraren en/of schoolleiders de school verlaten?; Wat gebeurt er wanneer de middelen verdwijnen?; Wat gebeurt er wanneer de beleids-/schoolprioriteiten veranderen?

7. Curriculum flexibility in a blended curriculum

This study offers insights into processes that play a role in realizing curriculum flexibility through the implementation of a blended curriculum. Curriculum flexibility is conceptualized in terms of adaptability and accessibility to students’ needs and capabilities. In order to realize curriculum flexibility, the teacher training institution in this study designed a blended curriculum with face-to-face and online components. This flexible blended curriculum aimed at increasing student enrolment and offering variety in students’ graduation profiles. Based on an analysis of formal curriculum materials, flexibility of the intended and formal curriculum was determined. Through subsequent semi-structured interviews with ten teacher educators, fostering and inhibiting factors regarding realizing flexibility were identified. Results show that the teacher educators perceived the formal curriculum as less flexible than intended. Furthermore, they identified different contextual, teacher- and student-related factors that affected (further) flexibilization. Based on a discussion of these findings, recommendations for research and practice are given.

In dit artikel bestuderen we de manier waarop een DocentOntwerpTeam (DOT) zijn ontwerptaak aangepakt heeft, in het bijzonder de samenwerking binnen het ontwerpteam en het creëren van draagvlak. De casus die in dit artikel centraal staat is gesitueerd binnen een Lerarenopleiding Technisch Beroepsonderwijs (LTB), waar een DOT de ontwerptaak kreeg om het bestaande curriculum zodanig te herontwerpen dat het aansluit bij de toenemende diversiteit in de studentenpopulatie en de verschillen in behoeften van het afnemend werkveld. Het DOT heeft zijn taak gerealiseerd door het ontwerp en de implementatie van een blended curriculum voor de gehele opleiding. De blend bestond in dit geval uit een vaste structuur van afwisselend face-to-face en online onderwijs. Naast het realiseren van het product (i.e. hernieuwd curriculum), moest het DOT ook expliciet aandacht schenken aan de communicatie met de collega’s. Aangezien niet alle docenten uit het team bij het herontwerp betrokken waren maar het ontwerp wel moesten implementeren, was het creëren van draagvlak onder het bredere docententeam en het wegnemen van weerstand ten aanzien van het beoogde curriculumontwerp een belangrijke opdracht voor het DOT.

11. Invoering en afschaffing van de mentoruren. Naar een professionalisering van het mentorschap

Om de praktijkschok voor beginnende leerkrachten op te vangen en hun professionele ontwikkeling te verhogen, heeft de Vlaamse overheid voorzien in mentoruren. Daarbij ondersteunen ervaren leerkrachten al dan niet vrijwillig hun beginnende collega’s. In 2010 werden deze mentoruren weer afgeschaft met het oog op besparingen in het onderwijs. In deze bijdrage wordt verslag gedaan van een onderzoek waarin werd nagegaan wat de impact is van het invoeren en vervolgens afschaffen van de mentoruren op de bestaande praktijken rond aanvangsbegeleiding. Verder werd ook onderzocht hoe het invoeren en afschaffen van deze mentoruren de functie van de mentor hebben veranderd.

13. Intraoperative cartilage analysis of the first carpometacarpal joint - comparison with conventional staging according to Eaton and Littler.

- Abstract
Introduction: Osteoarthritis of the first carpometacarpal joint is a common pathology of the hand, which may show an increasing prevalence in Germany due to the demographic development. In recent years, not only the current gold standard - the resection arthroplasty of the thumb saddle joint - has been used, but also therapeutic thumb saddle joint arthroscopy. In addition to the patient's clinical complaints, radiographic diagnostics have been used to decide on treatment, although it has not been proven whether there is a correlation between imaging and clinical complaints., Materials and Methods: Between 2020 and 2022, 20 articular surfaces of the thumb saddle joint undergoing resection arthroplasty for symptomatic basal thumb osteoarthritis were prospectively examined, mapped and compared with preoperative conventional radiographs., Results: The evaluation of the corresponding articular surfaces showed a higher cartilage destruction at the articular surfaces of the trapezium compared to the first metacarpal. No correlation was found between the stage of osteoarthritis and the Eaton-Littler classification., Conclusions: Overall, there is a patient-specific heterogeneity of the cartilage damage of the articular surface of the trapezium bone, as well in the metacarpal bone I base in relation to the radiographic diagnosis. Furthermore, an inhomogeneity of the radiographic stage of osteoarthritis of the carpometacarpal joint according to Eaton and Littler in relation to the intraoperatively assessed cartilage damage. The statistical significance of the surgically assessed cartilage damage in relation to the conventional radiographs could not be demonstrated. Thus, the treatment of symptomatic osteoarthritis of the carpometacarpal joint should primarily address the patient's individual complaints. The radiographic classification according to Eaton and Littler can be used as an additional factor to decide on the surgical procedure but should not delay the therapeutic treatment., Level of Evidence: III., (© 2024. Background: The occurrence of infections has always been feared in all surgical disciplines. Plastic reconstructive surgery faces the challenge of treating infection-related defects on a patient-specific basis, which requires a multidisciplinary treatment concept. Satisfactory treatment success can only be achieved through radical debridement for infection cleansing, optimization of the perfusion situation paired with targeted anti-infective treatment and, if necessary, with soft tissue reconstruction by plastic surgery., Objective: This article presents the current possibilities of plastic and reconstructive surgery with respect to the reconstruction of infection-related defects., Material and Methods: Proven and reliable strategies are presented and supplemented by promising experimental approaches., Results: Due to the often multilayered defect situation caused by infections, from the epidermis to cancellous bone, the entire armamentarium of plastic and reconstructive surgery is used for reconstruction., Conclusion: The early involvement of plastic surgery in the treatment of infections and the interdisciplinary and multimodal treatment approach have proven their worth in the treatment of complex infection situations., (© 2024. 15. [Retrospective Long-Term Results After Dellon's Nerve Decompression In The Lower Leg For Polyneuropathy].

- Abstract
Background: Patients affected by polyneuropathy often report pain, paraesthesia and numbness and are at risk of having an increased propensity to fall with the corresponding complications. In case of persistent symptoms after all conservative and medical treatment options have been exhausted, a nerve decompression in the lower leg as described by Dellon can lead to an improvement of the discomfort for many patients. This article presents our long-term results with Dellon's operation., Methods: In this retrospective study, we included 33 patients with diabetic or idiopathic polyneuropathy in whom we performed Dellon's nerve decompression in the lower leg in the years 2011-2013. In 29 out of the 33 patients, we investigated the level of pain (according to the numeric rating scale NRS 0-10), paraesthesia, numbness, Hoffman-Tinel sign, and Semmes-Weinstein monofilaments three months postoperatively. 88 months postoperatively (range 78-111 months), we were able to contact 20 of the 33 patients with a questionnaire asking about their present pain level (NRS 0-10), satisfaction with the postoperative result (NRS 0-10, 0=most satisfied, 10=not satisfied at all) and whether the patients would recommend the surgical procedure to their friends or family., Results: 1. Significant pain reduction three months postoperatively, NRS 4.5 to 2.2, (p=0.000). 2. Significant reduction of paraesthesia three months postoperatively from 84.8 to 24.2% (p=0.000) 3. Significant reduction of numbness three months postoperatively from 97% to 39% (p=0.000). 4. Significant improvement of sensitivity three months postoperatively from 91% no sensitivity to 28% no sensitivity (p=0.000). 5. Significant improvement of Hoffmann-Tinel sign three months postoperatively from 76% to 13% (p=0.000). 88 months postoperatively, we saw a significant pain reduction from an average preoperative pain level of NRS 4.5 to a postoperative pain level of 2.7 (p=0.048). 88 months postoperatively, 65% of the patients would recommend the surgical procedure to their family and friends, and patient satisfaction was high (NRS 3,4)., Conclusion: Our long-term results show that Dellon's nerve decompression in the lower extremities leads to a marked, lasting pain reduction and a functional improvement., Competing Interests: Die Autorinnen/Autoren geben an, dass kein Interessenkonflikt besteht., (Thieme. 16. Reconstruction of an Extensive Segmental Radial Shaft Bone Defect by Vascularized 3D-Printed Graft Cage.

- Abstract
We report here a 46-year-old male patient with a 14 cm segmental bone defect of the radial shaft after third degree open infected fracture caused by a shrapnel injury. The patient underwent fixed-angle plate osteosynthesis and bone reconstruction of the radial shaft by a vascularized 3D-printed graft cage, including plastic coverage with a latissimus dorsi flap and an additional central vascular pedicle. Bony reconstruction of segmental defects still represents a major challenge in musculo-skeletal surgery. Thereby, 3D-printed scaffolds or graft cages display a new treatment option for bone restoration. As missing vascularization sets the limits for the treatment of large-volume bone defects by 3D-printed scaffolds, in the present case, we firstly describe the reconstruction of an extensive radial shaft bone defect by using a graft cage with additional vascularization.

17. Nerve Decompression in the Lower Leg Results in an Improvement in Symptoms in Patients With Both Diabetic and Idiopathic Polyneuropathy.

Background: Patients suffering from polyneuropathy often complain of pain, tingling, and numbness sensations, as well as an increased risk of falling with the corresponding subsequent complications. If symptoms persist after conservative treatment options have been exhausted, nerve decompression in the lower extremity, as described by Dellon, can bring about an improvement in symptoms in many patients. Dellon originally reported that this surgery led to very successful outcomes in patients with diabetic polyneuropathy. In this study, we compare our postsurgical results in patients with diabetic versus idiopathic polyneuropathy., Methods: Thirty-three patients with idiopathic or diabetic polyneuropathy who had undergone Dellon nerve decompression in the lower extremity between 2011 and 2013 were included in the retrospective study. Pain (numeric rating scale [NRS] 0-10; 0, no pain; 10, worst imaginable pain), tingling, numbness, Hoffmann-Tinel sign, and Semes-Weinstein monofilament were assessed in 20 patients with diabetic polyneuropathy and in 13 patients with idiopathic polyneuropathy., Results: Three months after surgery, a significant reduction in pain was evident in patients with diabetic polyneuropathy, from a preoperative level of NRS 4.9 (minimum, 0; maximum, 10) to 2 (minimum, 0; maximum, 8; P = 0.005). Ninety percent of patients complained of tingling ( P = 0.000) before surgery and 18% after surgery, whereas 100% complained of numbness before surgery and 41% ( P = 0.000) after surgery. One hundred percent of patients had no measurable surface sensitivity before surgery (measured with the Semes-Weinstein monofilament), whereas 3 months after surgery, only 24% of patients still had no measurable surface sensitivity ( P = 0.000). A positive Hoffmann-Tinel sign was recorded in 85% of patients before surgery and only in 11% 3 months after surgery ( P = 0.000). In the case of patients with idiopathic polyneuropathy, a reduction in pain was evident 3 months after surgery, from a preoperative level of NRS 3.9 (minimum, 0; maximum, 9) to 2.2 (minimum, 0; maximum, 9; P = 0.058). Seventy-seven percent of patients complained of tingling before surgery and 42% after surgery ( P = 0.111), whereas 92% complained of numbness before surgery and 50% after surgery ( P = 0.030). Seventy-seven percent of patients had no measurable surface sensitivity before surgery (measured with the Semes-Weinstein monofilament), whereas 3 months after surgery, only 33% of patients still had no measurable surface sensitivity ( P = 0.047). 18. Mental Health Nurses' Social Representations of People Who Experience Mental Illness: A Story of Paradoxes.

Public representations of people who experience mental illness (PEMI) have been well documented within the stigma literature. However, studies about mental health nurses' representations of PEMI are still scarce and characterized by contradictions. Using the theoretical concept of social representation instead of stigma, the current study aims to explore and understand mental health nurses' social representations of PEMI. Qualitative research was conducted based on 13 semi-structured interviews with nurses from two psychiatric units in general hospitals. Data were analyzed using interpretative phenomenological analysis. Results showed that nurses' social representations were characterized by paradoxes. They explicitly displayed positive social representations, while nuancing them from an implicit point of view. Complex and chronic wounds represent a highly prevalent condition worldwide that requires a multimodal and interdisciplinary treatment approach to achieve good functional and aesthetic outcomes. Due to increasing costs of health care, an aging population and an increase in difficult-to-treat microbial colonization of wounds, complex wounds will become a substantial clinical, social and economic challenge in the upcoming years. In plastic reconstructive surgery, a variety of dermal skin substitutes have been established for clinical use. Since its approval as a dermal skin substitute in Germany, NovoSorb
® Biodegradable Temporising Matrix (BTM) has become a valuable therapeutic option for the treatment of full-thickness wound defects. The clinical data published to date are limited to case reports and small-scale case series with the main focus on single wounds. The aim of this single-center study was a retrospective analysis of our own patient collective that has received treatment with BTM for complex wounds. Overall, BTM showed to be a reliable and versatile reconstructive option, especially for patients with multiple co-morbidities and microbiologically colonized wounds. Although the preliminary findings have produced promising results, further investigation and research are warranted regarding long-term outcomes and additional clinical applications.- Published

20. Radiological changes in shoulder osteoarthritis and pain sensation correlate with patients' age.

- Abstract
Purpose: Osteoarthritis (OA) is one of the most common musculoskeletal disorders in the aging population. The correlation of radiographic OA severity, disability and pain is variable and inconsistent for the different joints. This study aims to elucidate the relationship between histological and radiological signs of shoulder OA with pain sensation and functional impairment to potentially adapt the recommendation for surgical treatment for primary total shoulder arthroplasty (TSA)., Methods: Forty-four patients with shoulder OA undergoing TSA using an anatomic stemless implant were included in this study. The radiological OA severity was scored pre-operatively on true ap X-rays according to the Kellgren-Lawrence score (KL-Score). Acromial types according to Bigliani were defined by pre-operative radiological images. The histological OA severity was determined according to the OARSI-Score using bone-cartilage sections from loaded areas of the humeral head. Pain was quantified using the visual analog scale (VAS). The functional status was assessed by the items "mobility" and "strength" out of the Constant-Murley score (CS Score). Demographic data including BMI, age, gender, diabetes mellitus and smoking were recorded., Results: There was no correlation between radiographic and histological severity in shoulder OA. However, a correlation of age and the severity of radiological changes was observed. Further, pain did not correlate with histological or radiological scores, whereas it correlated with age and the presence of diabetes mellitus. The functional shoulder status (mobility, strength) correlated with the severity of radiological changes, but not with the histologic scoring, which correlated with nicotine abuse., Conclusion: This study shows that increased age is the main determinant of radiological changes in shoulder OA, as well as pain. Therefore, age and pain sensation should be considered as important parameters for the recommendation for TSA., (© 2022. The Author(s).)

- Abstract
Background: This study evaluates bone remodeling processes in the proximal humerus induced by the implantation of a stemless shoulder prosthesis with regard to time of response and type and extent of bone turnover., Methods: Twenty-eight patients with primary osteoarthritis of the shoulder undergoing a stemless shoulder arthroplasty were prospectively evaluated. The local metabolic bone activity in 5 regions of interest (ROIs 1-5) around the implant and in 1 reference region (ROI ref.) in the humeral diaphysis was analyzed after the application of technetium Tc 99m DPD using single-photon emission computed tomography integrated with multidetector computed tomography (SPECT/CT). The study cohort was divided into 4 groups according to the timing of the most recent follow-up appointment to evaluate the primary osseointegrative response after surgery. The bone uptake values were expressed in target (ROIs 1-5) to nontarget (ROI ref.) ratios., Results: No difference within the 4 subgroups with respect to the time of local metabolic bone activity in the ROIs was found at 90 days after surgery. The highest initial metabolic activity and most temporal modifications were found in ROI 1, which was localized in the superior segment of the stem., Conclusions: SPECT/CT data suggest that the primary osseointegration of a stemless shoulder prosthesis is almost completed 3 months after implantation.
