This pre-specified analysis of DAPA-CKD assessed the impact of sodium-glucose cotransporter 2 inhibition on abrupt declines in kidney function in high-risk patients based on having chronic kidney disease (CKD) and substantial albuminuria. DAPA-CKD was a randomized, double-blind, placebo-controlled trial that had a median follow-up of 2.4 years. Adults with CKD (urinary albumin-to-creatinine ratio 200-5000 mg/g and estimated glomerular filtration rate 25-75 mL/min/1.73m2) were randomized to dapagliflozin 10 mg/day matched to placebo (2152 individuals each). An abrupt decline in kidney function was defined as a pre-specified endpoint of doubling of serum creatinine between two subsequent study visits. We also assessed a post-hoc analysis of investigator-reported acute kidney injury-related serious adverse events. Doubling of serum creatinine between two subsequent visits (median time-interval 100 days) occurred in 63 (2.9%) and 91 (4.2%) participants in the dapagliflozin and placebo groups, respectively (hazard ratio 0.68 [95% confidence interval 0.49, 0.94]). Accounting for the competing risk of mortality did not alter our findings. There was no heterogeneity in the effect of dapagliflozin on abrupt declines in kidney function based on baseline subgroups. Acute kidney injury-related serious adverse events were not significantly different and occurred in 52 (2.5%) and 69 (3.2%) participants in the dapagliflozin and placebo groups, respectively (0.77 [0.54, 1.10]). Thus, in patients with CKD and substantial albuminuria, dapagliflozin reduced the risk of abrupt declines in kidney function. HJLH and DC are co-primary authors. In order to provide telecommunication and FTTH services, we, NTT, have installed a large number of facilities such as utility poles and optical cables. The number of poles is about 11.9 million and the total length of all installed cables is now about 2.3 million km. These facilities are inspected and maintained by visual inspection by workers every 5 to 10 years, which imposes great costs on the operator. Therefore, we are researching a novel outside-facility renovation technology that can improve cost-effectiveness by long-term safe use. This technology consists of two techniques: a visualization technique of unbalanced tension and a quantitative analysis technique to determine the relationship between unbalanced tension and structural deterioration. In this paper, we describe the concept of the proposed technology. When a utility pole is newly installed, its design assumes that the maximum load consists of wind pressure on the cables. However, when it is impossible to construct a guy wire for bearing the load applied to the utility pole, an unbalanced load occurs because the load cannot be balanced. In addition, when the number of users of various services increases and cables are newly laid, unbalanced loads are generated. Utility poles carrying these unbalanced loads are at significant risk of collapse due to the presence of deflection, inclination, and cracks. Our proposal focuses on the unbalanced load itself, and by detecting and countering it, we aim to enable the use of outside-facilities for a longer period than at present and to reduce replacement costs without sacrificing safety and security. In addition, we also describe how ensuring the long-term safe use of outside-facilities can improve cost-effectiveness. The proposed technique first acquires 3D point cloud data by using 3D laser scanner. It then creates a 3D facility model and calculates the tension in utility poles and cables. In addition, we introduce a novel method to estimate the loads and tension of a whole span from part of the span. Experiments are conducted to compare the estimated and measured values. The results confirm the good agreement of the values (within 10%) which validates the proposal. We aim to realize a tension visualization scheme with improved accuracy.

Therefore, we are researching a novel outside-facility renovation technology that increases cost-effectiveness by ensuring the long-term safe use of facilities. When a utility pole is newly constructed, it is designed under the assumption of a maximum load such as wind pressure being applied to the pole via cables and auxiliary attachments. However, the loads can become imbalanced when the guy wire cannot be tensioned as they cross private land or when there are obstacles, etc. We call this condition that unbalanced loads occur. Unbalanced loads are one of the key reasons for the structural deterioration of utility poles. After the deterioration occurs, the risk of pole collapse increases, because deflection and inclination are generated and finally the cracks propagate until collapse. The conventional solution is to replace the damaged pole. However, in many cases, the loads are created by other contiguous poles, so unbalanced loads occur again. Therefore, we are developing a method that can identify the reason by regarding sets of utility poles as "facility systems" and analyzing unbalanced loads quantitatively by FEM. In our research, simulation accuracy is improved by determining the quantitative differences obtained by comparing the calculated results obtained by FEM with measured values obtained by using a reaction wall. The simulations of structural degradation due to unbalanced loads taking into account the deformation of cables due to wind velocity and air temperature in addition to various material properties such as hysteresis and the sectional model of the utility poles. In addition to the above, the effect of soil is considered as a spring element, and the analysis involves the elements of horizontal spring (Kxi, Kzi) and vertical spring (Kv). While the calculated and the measured values show good agreement, there is some small error. This error is considered to be due to the influence of parameters such as tensile and compressive strength, and we aim to improve the simulation accuracy by considering these effects in the future.

Power generation using dielectric elastomer (DE) artificial muscle has attracted attention because it is light-weight, low-cost and high-efficiency. This method generates carbon dioxide-free electric power without exhausting rare earth materials or contributing to global warming, earning it the status of an eco-friendly system. This paper considers the opportunities for a DE generator system, namely using them to create the foundations of a Recycling Energy Society. If these opportunities are to be commercially successful, they will have to leverage the DE's advantages over conventional technologies. In this paper, we discuss two ways to use DEs more practically in applications: 1) point power generation, in which a single DE is used alone, and 2) distributed power generation, in which a large number of DEs are gathered as one cluster and distributed. We will also discuss the current status and future of DE generators.

The utility of repetitive nonhydrolytic base cleavage of alpha-amino protective groups in solid phase peptide synthesis is shown by a preparation of the model tetrapeptide leucyl-alanyl-glycyl-valine on a p-benzyloxybenzyl ester polystyrene--1% divinylbenzene resin support. Nalpha-9-Fluorenylmethyloxycarbonyl (Fmoc: Carpino & Han, 1970, 1972) amino acids were coupled by the symmetrical anhydride procedure, followed by Fmoc group cleavage using 50% piperidine in methylene chloride. Quantitative removal of the Fmoc-tetrapeptide from the solid support was effected by treatment with 55% trifluoroacetic acid in methylene chloride. Homogeneous free tetrapeptide was obtained in 87% overall yield. The procedure is proposed to offer advantages over present solid phase methods which use acidolysis for repetitive alpha-amino group deblocking.

When methane (CH 4 ) and O 2 are present, nitrogen can be removed from wastewater that does not contain other organic carbon sources. In this study, microbial activities during methane-dependent denitrification (MDD) were investigated by adding inhibitors of methane-oxidation and denitrification. Sludge susceptible to MDD showed methane oxidation activity in the presence of CH 4 and O 2 , and denitrification activity with methanol and acetate under anoxic conditions. Methyl fluoride (CH 3 F) is known to inhibit methane oxidation. When CH 3 F was present, MDD did not occur, perhaps because methane oxidation was inhibited. Tungstate (WO 4 2− ), a known inhibitor of nitrate reduction, also lowered denitrification activity in the sludge, and partly inhibited methane oxidation. When WO 4 2− was added to the medium, MDD almost ceased, perhaps because of a synergic inhibitory effect on denitrification and methane oxidation. These results show that both methane oxidation and denitrification contribute to MDD.

In this work we have studied systematically the effect of fluctuations in thickness and depth of the well, as well as in the thickness of the barrier in semiconductor superlattices. Our results show that the main effect of fluctuations is to break down the whole superlattice into subsets. Localization occurs when there are no degenerate states among the subsets, regardless of symmetry considerations.

We have successfully developed and industrialized a solar-powered air conditioner that operates through a combination of photovoltaic (PV) arrays and commercial power supplies. In order to achieve the most efficient operation possible from a one-horsepower inverter-type household air conditioner, we selected a PV array output of 540 W under standard test conditions, based on single-day power use patterns for air conditioners as well as on solar cell output patterns. When using a 540-W PV array, we have shown that the system can supply at least 90% of the power consumed by an air conditioner during peak power periods in summer. We have confirmed that on average the power generated by the PV array can satisfy at least 50% of the power consumed by an air conditioner in summer. We also confirmed that the system functions according to system design specifications. There is little doubt that the introduction and widespread use of this system will play an important role in reducing peak power energy requirements in summer.

Etching rates of borosilicate, borophosphosilicate, phosphosilicate, and arsenosilicate glass films in various buffered hydrogen fluoride solution (BHF) have been studied. Etching rates were found to depend on the glass composition and dopant concentration as well as the concentration of ammonium fluoride in the BHF. This phenomenon is clearly different from the etching rate of thermal SiO 2 oxide which is hardly affected by the concentration of ammonium fluoride in excess of equimolar to the HF concentration

There is a conflict in previous studies with regard to the relation between adipose tissue mass and total body fluid distribution. This study tested the hypothesis that obesity is accompanied by an increase in the extracellular-to-intracellular fluid ratio above that observed in nonobese subjects. Extracellular fluid was evaluated in obese (n = 39) and nonobese (n = 26) healthy women, using two different dilution volumes, 35SO4 [extracellular water (ECW)] and 24NaCl [exchangeable sodium (Nae)]. Intracellular water (ICW = 3H2O dilution volume-ECW) and total body potassium (TBK; 40K whole body counting) were assumed to represent intracellular fluid. Two independent markers of relative fluid distribution were formulated as ECW/ICW and Nae/TBK. Obese and nonobese women were of similar age and height but differed in body weight and TBW by 67.7 kg and 12.9 liters, respectively. The obese women had significantly larger absolute ECW, Nae, ICW, and TBK compared with the nonobese women (all P less than 0.001). The ratios ECW/ICW and Nae/TBK were significantly higher in obese vs. nonobese women and were highly correlated with each other (r = 0.54, P less than 0.001) in the pooled group of subjects. Fluid volumes are thus increased in obese women, and the expansion is relatively greater for the extracellular compartment. These results have implications in the study of human body composition and may also account in part for the fluid-overload states that often accompany severe obesity.

In this work, as a novel process for use of renewable energy, the authors would like to discuss the possibility of an artificial muscle actuator based on a dielectric electroactive polymer (elastomer). This highly efficient actuator can transform electric energy into mechanical energy (theoretical transformation efficiency of 80 to 90%) with a high energy density of 1.0 W/g. Using the reverse operation for this actuator, it is possible, with current materials, to obtain a maximum output of 0.4 J/g. With this new material, it is not impossible to dream of having an output on the order of 2.0 J/g.

A significant positive correlation was found between serum cholesterol level and BMI, and the total energy intake was significantly higher in hypercholesterolemic group than in normocholesterolemic group. 3. 3. Hypercholesterolemia seems to occur in the background of accumulation of body fat due to relative excess of energy intake in Japan.

A 16b processor chip with sorter and 32Kb dictionary RAM has been developed for hand-printed Kanji character recognition. This paper will describe the design techniques used to achieve defect tolerance in the memory and logic blocks. A 18.5×20.5mm2chip contains 460,000 transistors using a 3μm double metal CMOS technology.

Methane (CH4) can be used as an alternative carbon source for denitrification with added oxygen (O2). However, the off-gas of denitrification reactors using a CH4–O2 mixture contains unused CH4 and O2 in proportions that make it unusable for fuel, carry explosion risks, and, if released into the atmosphere, contribute to the greenhouse effect. This study tested a novel reactor with a partition dividing the headspace completely and extending partly into the liquid layer. When CH4 and O2 were supplied separately to the liquid layer on opposite sides of the partition, the methane-dependent denitrification (MDD) activity was similar to that when the two gases were supplied as a mixture. In reactors with separate gas supplies, the off-gas from the CH4 supply side was high in CH4 and low in O2, and was usable for fuel, and that from the O2 supply side was very low in CH4, and might be released into the atmosphere. MDD activity increased with the O2 supply rate, and separate discharge of CH4 and O2 was maintained. The concentration of dissolved methane in the effluent was decreased by lowering the CH4/O2 supply ratio to 1.0 and drawing the effluent from the O2 supply side. This novel reactor enhances the safety of MDD, allows reuse of methane as fuel, and reduces methane leakage to the atmosphere.

Many studies have reported the cholesterol-lowering, anti-lipogenic, anti-obesity and anti-hypertensive effects of soy protein. Adipose tissue-specific plasma protein, adiponectin, has anti-atherogenic and anti-insulin-resistance properties. Here, we investigated the effects of soy protein diet on body fat composition, plasma glucose, lipid and adiponectin levels and expression of genes involved in glucose and fatty acid metabolism in obese KK-A y mice. Body weights and adipose tissue weights of mesenteric, epididymal, and brown fat were lower in mice on calorie-restricted diet containing soy protein isolate. Plasma cholesterol, triglyceride, free fatty acid, and glucose levels were also decreased by this diet. Body fat content and plasma glucose levels in mice on a soy protein isolate diet were still lower than those treated with an isocaloric casein-protein-diet. Among the genes related to glucose and fatty acid metabolism, adiponectin mRNA levels in adipose tissue and adiponectin plasma concentrations were elevated in mice on a calorie-restricted diet, although there were no significant differences between soy protein and casein protein groups. Our results indicate that that soy protein diet decreased body fat content and plasma glucose levels more effectively than isocaloric casein-protein diet in obese mice.

For effective application of methane-dependent denitrification (MDD) in the treatment of wastewater containing NO2 – or NH4 +, the effect of these inorganic nitrogen compounds on MDD activity needs to be clarified. The MDD activity of sludge acclimatized with CH4 and O2 was determined with mineral media of different nitrogen-compound compositions in the presence of 0.21 atm CH4 and 0.20 atm O2. Incubations with media containing only NO2 – or two of the three inorganic nitrogen compounds (NO3 –+NO2 –, NO2 –+NH4 + or NH4 ++NO3 –) resulted in MDD activity equal to or higher than that with media containing only NO3 –. However, there was no MDD activity in media containing NO2 – at 10 °C, probably because of serious inhibition of NO2 – on methane oxidation. MDD occurred in media containing only NH4 +, although the total nitrogen removal efficiency was very low. These results show that NO2 – and NH4 +, in the presence of NOx –, do not inhibit but rather promote MDD. Consequently, NH4 + does not need to be completely oxidized to NO3 – in the nitrification reactor before MDD. However, under psychrophilic conditions, NO2 – seriously inhibited MDD. Therefore, the nitrification reactor must not discharge effluent containing NO2 – under psychrophilic conditions.

To investigate whether homogenous analysis of serum low-density lipoprotein cholesterol(LDL-C) was applicable in type III hyperlipoproteinemic subjects, 3 reagents were used to estimate LDL-C in 2 cases with apoE 2/2 phenotype. Each measurements were compared to LDL-C levels by ultracentrifugation(density: 1.019-1.063). LDL-C levels by homogenous analysis with any of the reagents were higher than those by ultracentrifugation, and the difference was varied between the reagents: +14-24% by LDL-EX, +29-55% by Cholestest LDL, and +89-115% by Determiner LDL-C. The cross-reaction by the reagents to intermediate density lipoprotein(IDL) and very low density lipoprotein(VLDL) cholesterol was studied in a case; 22% of cholesterol in these fractions was measured by LDL-EX, 32% by Cholestest LDL, and 110% by Determiner LDL-C. A part of the sample serum was stored at -60 degrees C for several days and melted for re-analysis of homogenous LDL-C levels. After the freezing process, IDL fraction pattern on electrophoresis had been changed and homogenous LDL-C levels by all reagents were different from the measurements of native serum. These results indicated that using reagents with low cross-reaction to IDL and VLDL fractions and careful sample handling are important in homogenous LDL-C analysis in type III hyperlipoproteinemia.

T22 ([Tyr5,12,Lys7]-polyphemusin II) has been shown to have strong anti-human immunodeficiency virus (HIV) activity comparable to that of 3'-azido-2',3'-dideoxythymidine (AZT). T22, an 18-residue peptide amide, takes an antiparallel beta-sheet structure that is maintained by two disulfide bridges. Herein we synthesized several shortened analogs of T22 in order to search for a more suitable lead compound. A 14-residue analog having one disulfide bridge, TW70 (des-[Cys8,13, Tyr9,12]-[D-Lys10, Pro11]-T22), was found to have highly potent activity comparable to that of T22, and to take an antiparallel beta-sheet structure similar to that of T22. This indicates that the molecular size of T22 can be reduced without loss of activity or significant change in the secondary structure, and that TW70 may represent a novel lead compound. Furthermore, modifying the N-terminal alpha-amino group of TW70 with a fluoresceinthiocarbamoyl group, and the epsilon-amino group of D-Lys8 at the turn portion with a 5-aminopentanoyl group remarkably increased the selectivity index (50% cytotoxic concentration/50% effective concentration).

The segmented polyether urethanes (PEUs) have been used in implantable medical devices due to excellent mechanical properties, acceptable blood compatibility, and good biostability. However, recent studies demonstrate that the polyether soft segment of PEU is susceptible to oxidative degradation in vivo due to scission of the polyether group. Recently, polycarbonate urethanes (PCUs) having no ether linkage in the soft segment have been developed, and show improved stability against oxidative degradation over PEUs. The current study evaluates blood compatibility of these PCUs in comparison with PEUs using epifluorescent video microscopy (EVM) combined with a parallel plate flow cell. The authors selected two PCUs, Corethane 80A (Corvita Corporation, Miami, FL) and PCU(1560), and two PEUs, Pellethene 2363-80AE (Dow Chemical Japan, Tokyo, Japan) and Tecoflex EG80A (Thermedics, Inc., Woburn, MA), all of which have similar hard segment compositions (MDI or HMDI:1,4-butanediol(BD)) and the same hardness of 80A. The EVM measured the amount of platelet coverage on the surfaces using human whole blood perfused at a wall shear rate of 100/sec for 20 min. Complement activation (C3a) also was measured. Both PEUs, especially Pellethane, showed significantly higher platelet adhesion than the PCUs (p0.05). There were no significant differences in platelet adhesion between the two PCUs. As for C3a measurements, Tecoflex showed higher complement activation than the others. Based on these results, it is recommended that PEUs should be replaced by ether free PCUs for use in implantable blood contacting devices such as artificial hearts and pacemaker lead insulators.

We describe a patient with gastric cancer and membranous glomerulonephritis (MGN). The patient, a 61-year-old male, was admitted to our Hospital in May, 1996, because of proteinuria and hyperlipidemia persisting for a year. Laboratory examination filled the criteria of nephrotic syndrome and renal biopsy revealed MGN of stage II. Prednisolone therapy (40 mg/day p.o.) was started, followed by a gradual decrease in proteinuria from 4.5 g/day to 0.1 g/day. Endoscopic examination was performed because of stomach-ache revealed advanced gastric cancer of Borrmann 4. Desiring for a conservative therapy, he was discharged and moved to a hospice. In literature review, MGN is the most frequent lesion among various glomerular diseases associated with malignancy, such as the lung, stomach, and colon, particularly at an elderly ages, and sometimes antedates the detection of malignancy, as in the present case. In several cases with MGN, immune-complexes composed of tumor antigens, such as carcino-embryonic antigen, and antibodies have been reported to deposit in basement membrane of glomeruli, causing MGN. In the renal and gastric cancer tissues of the present case, the presence of three novel tumor-associated antigens, Span-1, Thomsen-Friedenreich antigen (T antigen) and F1 alpha antigen, was examined, using a immuno-peroxidase method. Although none of these three antigens were immuno-stained in the renal tissue, clinical course and literature review suggest that MGN in this patient seems to be associated with gastric cancer, which may have produced MGN-causing tumor antigens other than the three antigens. It should be emphasized that malignancy should be carefully and routinely examined in patients with MGN, particularly at elderly ages.

In a 59-year-old woman suffering from the syndrome of inappropriate antidiuretic hormone secretion, a small cell carcinoma of the uterine cervix was detected. The tumor was immunoreactive for antidiuretic hormone as well as for neuron specific enolase, chromogranin A, and Leu-7, but not vimentin. Electron microscopic examination of the tumor revealed neurosecretory granules. To our knowledge, this is only the second report of the syndrome of inappropriate antidiuretic hormone secretion with small cell carcinoma of the uterine cervix and the first one confirmed immunohistopathologically.

Six chemical compartments [water, protein, mineral (osseus and cellular), glycogen, and fat] consisting of 11 elements (N, C, Ca, Na, Cl, K, H, P, O, S, and Mg) comprise greater than or equal to 99% of body weight in living humans. The combination of three neutron-activation systems, whole body 40K counting, and 3H2O dilution at Brookhaven National Laboratory now potentially makes it possible to quantify greater than or equal to 96% of the chemical and elemental determinants of body weight in vivo. The aims of the present study were 1) to develop 6- and 11-compartment chemical and elemental models, respectively, and 2) to evaluate these models in a group of 20 healthy adults. Results demonstrated that body weight estimated from either chemical or elemental components was highly correlated with (both r = 0.97, P less than 0.001) and on average differed by less than 4% from actual body weight. The compartmental results obtained using the chemical model were also evaluated by comparing calculated and actual body density (Db) estimated by underwater weighing. Calculated Db [1.041 +/- 0.017 (SD) g/ml] agreed closely and was highly correlated with actual Db (1.039 +/- 0.018 g/ml; r = 0.82; P less than 0.001). Hence a near-complete chemical and elemental analysis of living human subjects is now possible and, with potential future refinements, represents an important opportunity to quantify the effects of gender, aging, and ethnic status on body composition.

A coagulant enzyme, named okinaxobin I, has been purified to homogeneity from the venom of Trimeresurus okinavensis (Himehabu) by chromatographies on Sephadex G-100 and CM-Toyopearl 650M columns. The enzyme was a monomer with a molecular weight of 37,000 and its isoelectric point was 5.4. The enzyme acted on fibrinogen to form fibrin clots with a specific activity of 77 NIH units/mg. Fibrinopeptide B was released at a rate much faster than fibrinopeptide A. The enzyme exhibited 2 to 3 times higher activity toward tosyl-L-arginine methyl ester and benzoyl-L-arginine p-nitroanilide than bovine thrombin. The esterase activity was strongly inhibited by diisopropylfluorophosphate and phenylmethanesulfonyl fluoride, and to a lesser extent by tosyl-L-lysine chloromethyl ketone, indicating that the enzyme is a serine protease like thrombin. The N-terminal sequence was highly homologous to those of coagulant enzymes from T. flavoviridis and Bothrops atrox, moojeni venoms which preferentially release fibrinopeptide A. In order to remove most, if not all, of the bonded carbohydrates, the enzyme was treated with anhydrous hydrogen fluoride (HF), thereby reducing the molecular weight to 30,000. The protein contained approximately 260 amino acid residues when computation was based on this value. The HF-treated enzyme retained about 50% of the clotting and esterolytic (TAME) activities and preferentially released fibrinopeptide B from fibrinogen. The carbohydrate moiety is not crucial for enzyme activity but might be necessary for eliciting full activity.

The Tyr-Pro-Phe peptide sequence, an N-terminal tripeptide fragment of opioid peptide morphiceptin, was searched on the database SEQDB (Peptide Institute, Osaka). More than 30 proteins were drawn in a list with 15 amino acid varieties at the position adjacent to Phe. Seven morphiceptin-like peptides with the H-Tyr-Pro-Phe-Xxx-NH2 sequence, where Xxx denotes the selected amino acids (Ala, Asp, Gly, Gln, Lys, Thr and Tyr), have been synthesized. Together with the side-chain protected analogs [Asp(OBzl), Lys(Z), and Thr(Bzl)], they have been evaluated for opioid activities. For the mu opioid receptors to which morphiceptin binds specifically and selectively, analogs with Xxx = Asp, Lys, Ala, Thr, Gln, Tyr and Gly showed a considerably weaker binding affinity than morphiceptin. In contrast, the Thr(Bzl) derivative demonstrated an affinity ten times greater than morphiceptin. Because of an extremely weak affinity for the delta receptors, its mu-selectivity became very high (260-fold). The present results and the increased activity of [Val4]morphiceptin (Sakaguchi et al., 36) indicate that the mu receptors tolerate the hydrophobic or aromatic residue at the site corresponding to position 4 of morphiceptin. When the circular dichroism (CD) spectra are compared between active and inactive analogs, no significant difference is found in both water and methanol. This suggests that the activity of morphiceptin analogs with the Tyr-Pro-Phe sequence is mainly determined by the structural characters of the amino acid residue in position 4 rather than by the molecular conformation. The results suggested a possible formation of morphiceptin-like peptides in various protein digests.

Sultamicillin, a new semisynthetic oral beta-lactam antibiotic, was evaluated for its antibacteria susceptibility and clinical efficacy against urinary tract infection (UTI), and the following results were obtained. The sensitivity of sultamicillin (SBTPC) on 518 strains of clinical isolates from the urine were tested and compared to ampicillin (ABPC). S. aureus, S. epidermidis, Enterococcus sp., Streptococcus sp., E. coli, K. pneumoniae, K. oxytoca, P. mirabilis, M. morganii and Acinetobacter sp. showed high sensitivity to SBTPC. The antibacterial activity of SBTPC was superior to that of ABPC in most strains and especially more superior in beta-lactamase producing strains. The clinical effectiveness rate on a total of 15 patients with acute uncomplicated cystitis was 93.3% and the eradication rate of causative organisms was 93.3%. On 15 patients with chronic complicated UTI, the clinical effectiveness rate was 73.3% and eradication rate was 76.5%. Side effects (diarrhea) were observed in 3 cases, but this symptom was not severe and soon disappeared. Abnormal laboratory data due to the drug were not observed.

Fifty six patients with transitional cell carcinoma of the urinary bladder were treated by total cystectomy from January, 1979 through December, 1988. A retrospective study was conducted on 52 patients on whom prognosis could be followed up. There were 40 men and 12 women (male to female ratio: 3.3: 1), between 32 and 79 years old (mean age, 64.1 years old). The overall 1-, 3- and 5-year survival rates were 65.8%, 54.1% and 43.0%, respectively. The 5-year survival rates for patients with low grade (G, I, II) and high grade (G. III) were 41.9% and 42.7%, respectively: with no significant difference in the survival curves between the two groups. The 5-year survival rates according to histopathological stage were 78.9% for patients in low stage, 24.7% in high stage, and the difference in the survival curves between the two groups was statistically significant (P less than 0.001). The mortality in the 21 cases of the 33 high stage cases in which cisplatin was used was 42.9%, with 3- and 5-year survival rates were 53.1% and 36.8%, respectively. In the remaining 12 cases in which cisplatin was not used for post-operative chemotherapy, the mortality in the 12 cases was 83.3%, with 3- and 5-year survival rates of 25.1% and 12.3%, respectively. Thus, the patients who received post-operative chemotherapy showed a better survival rate than those who did not with a significant difference (P less than 0.05). The results of the present study suggest the usefulness of post-operative adjuvant chemotherapy in the high stage case.

A total of 518 bacterial strains isolated from the urine of patients with various urological diseases in our Urological Department between November, 1987 and February, 1989 were studied for their beta-lactamase production and susceptibility to various antimicrobial agents was determined. beta-lactamase activity was determined by the acidometry disc method. There were 241 gram-positive cocci, 276 of gram-negative rods and 1 Neisseria gonorrhoeae. Thirty-four percent of the gram positive and 76.3% of gram negative rods produced beta-lactamase. S. aureus (81.3%), S. epidermidis (65.1%) in gram-positive cocci, E. cloacae (100%), S. marcescens (100%), C. freundii (100%), P. aeruginosa (97.2%), P. Rettgeri (88.9%), E. gergoviae (85.7%), K. oxytoca (84.6%), M. morganii (81.8%) and E. coli (69.0%) in gram-negative rods produced beta-lactamase at a higher rate. beta-lactamase produced by gram-positive cocci was entirely penicillinase, and that produced by gram-negative rods only penicillinase in 4.0%, only cephalosporinase in 44.2% and both in 25.4%. In S. aureus and S. epidermidis, the isolated rate of strains resistant to ampicillin (p less than 0.01) and piperacillin (p less than 0.05) in the beta-lactamase producing strains was significantly higher than that in the beta-lactamase non-producing strains. In E. coli, the isolation rate of strains resistant to ampicillin and piperacillin in the penicillinase-producing strains was significantly higher than in the penicillinase non-producing strains (p less than 0.01). But both cephalosporinase-producing strains and beta-lactamase non-producing strains showed high susceptibility to cephalothin. These results suggest that the penicillinase might present a clinical problem in the treatment of urinary tract infections by S. aureus, S. epidermidis and E. coli.

The purpose of this study was to examine the effects of environmental changes (diet and exercise) on coronary heart disease (CHD) risk factors in Japanese exchange students when visiting the United States for six months and two months after their return to Japan. Thirteen subjects completed the study. While visiting the US, the subjects increased their consumption of calories and % calories derived from fat while they decreased their % calories derived from carbohydrates and dietary polyunsaturated to saturated fat ratio (P:S) The subjects' serum fatty acid profiles showed an increased in the saturated fatty acids 15:0 and 24:0 while there was a trend of decreasing omega - 3 fatty acids. While in the US, subjects significantly increased their fitness level as estimated using a sub-maximal cycle ergometer test. Subjects significantly increased body weight and body mass index and showed an increased trend of percent body fat. Blood pressure and blood Hpid concentrations (total cholesterol, HDL-cholesterol, LDL-cholesterol, and triglycerides) remained unchanged. Following their return to Japan, subjects significantly decreased caloric intake and % calories from fat compared to their US diets. Serum concentrations of the saturated fatty acids 15:0 and 24:0 significantly decreased while the omega-3 polyunsaturated fatty acid 22:6 increased. In addition, the ratio of eicosapentaenoic acid to arachadonic acid and total serum omega-3 fatty acids showed trends of increasing. Finally, body weight body fat, and body mass index significantly decreased.

The segmented polyether urethanes (PEUs) have been used in implantable medical devices due to excellent mechanical properties, acceptable blood compatibility, and good biostability. However, recent studies demonstrate that the polyether soft segment of PEU is susceptible to oxidative degradation in vivo due to scission of the polyether group. Recently, polycarbonate urethanes (PCUs) having no ether linkage in the soft segment have been developed, and show improved stability against oxidative degradation over PEUs. The current study evaluates blood compatibility of these PCUs in comparison with PEUs using epifluorescent video microscopy (EVM) combined with a parallel plate flow cell. The authors selected two PCUs, Corethane 80A (Corvita Corporation, Miami, FL) and PCU(1560), and two PEUs, Pellethene 2363-80AE (Dow Chemical Japan, Tokyo, Japan) and Tecoflex EG80A (Thermedics, Inc., Woburn, MA), all of which have similar hard segment compositions (MDI or HMDI:1,4-butanediol(BD)) and the same hardness of 80A. The EVM measured the amount of platelet coverage on the surfaces using human whole blood perfused at a wall shear rate of 100/sec for 20 min. Complement activation (C3a) also was measured. Both PEUs, especially Pellethane, showed significantly higher platelet adhesion than the PCUs (p < 0.05). There were no significant differences in platelet adhesion between the two PCUs. As for C3a measurements, Tecoflex showed higher complement activation than the others. Based on these results, it is recommended that PEUs should be replaced by ether free PCUs for use in implantable blood contacting devices such as artificial hearts and pacemaker lead insulators.

Biosynthesis, processing, and degradation of low density lipoprotein (LDL) receptors were studied in a mouse macrophage-like cell line, J774.1, by immunoprecipitation and immunoblotting with an antibody directed against the COOH-terminal 14 amino acids of the LDL receptor. The molecular weight of the mature LDL receptor of J774.1 cells maintained in RPMI medium was 140,000 under nonreducing condition and 160,000 under reducing condition in sodium dodecyl sulfate-polyacrylamide gels. These sizes are 10,000-15,000 daltons larger than those of the receptor in other mouse fibroblastic cells or P388 leucocyte. However, when J774.1 cells were cultured in Dulbecco's modified Eagle's medium, the molecular weight of the mouse cell lines, 123,000 under nonreducing condition and 153,000 under reducing condition. The larger LDL receptor molecules produced by J774.1 cells cultured in RPMI were insensitive to the treatment with end-alpha-N-acetylgalactosaminidase (O-glycanase), suggesting that aberrant serine/threonine-linked (O-linked) glycosylation might account for the apparent large size. Pulse-chase experiments revealed that the rate of processing of the LDL receptor from precursor to mature form in J774.1 was similar to that in other mouse cell lines, but the rate of degradation was much faster: half-life of the LDL receptor of J774.1 was about 2 h. No significant difference in biological function or lifetime was observed between the normal and the larger LDL receptor. This novel character of molecular size and lifetime of the LDL receptor in J774.1 is discussed in relation to altered maturation and/or modification during receptor biosynthesis.

1. The extent of racemization and the coupling yield in peptide synthesis were studied under high dilution conditions. The azide method yielded the best results. 2. Five linear penta-peptide precursors related to gramicidin S were subjected to cyclization in order to study how the difference in the sequence influences the yield and the ratio of cyclic dimer to monomer. The azide with the sequence of -L-Pro-L-Val-L-Orn(Z)-L-Leu-D-Phe- afforded diZ-gramicidin S in a high yield of 63%. 3. Alternaria mali toxin III, a cyclotetradepsipeptide phytotoxin, was synthesized. The activated linear tetradepsipeptide containing a D-Dap(Z) (N3-Z-D-2,3-diaminopropionic acid) residue at the N-terminus afforded the cyclic precursor (53%). The Dap residue in the precursor was converted into a ΔAla residue by Hofmann degradation to give the desired product.

The effect of end groups on the low-temperature dielectric relaxation in poly(4,4′-oxydiphenylene pyromellitimide) has been studied using three kinds of samples with different content of anhydride end groups. The magnitude of the dielectric loss peak, designated here the V peak, which appears near −40°C at 1 kHz, depends on the content of anhydride end groups. If a sample with few anhydride end groups is allowed to absorb water, a loss peak appears near the temperature region of the V peak, but it disappears after subsequent heating. In contrast, a sample having a larger amount of anhydride end groups exhibits a large V peak which remains even after further heating. We conclude that the Oportion of the anhydride end groups is responsible for the V peak, though there may be an additional contribution from absorbed water.

Hypertensive diabetic patients are particularly prone to renal function impairment. A total of nine out-patients with diabetes and hypertension were, therefore, entered into this single-blind uncontrolled study on the effects of 50 mg/day atenolol on reducing blood pressure and preserving normal kidney functioning. Treatment and evaluations were continued for 12 months. Serum β2-microglobulin concentration was used as the index for measuring renal impairment. Atenolol significantly reduced heart rate, systolic and diastolic blood pressure, and serum β2-microglobulin concentrations compared with baseline. Plasma glucose and glycosylated haemoglobin levels were unchanged, and blood urea nitrogen levels were increased slightly (non-significant). Serum creatinine showed a tendency (non-significant) to reflect the changes in β2-microglobulin concentration. Ways in which atenolol may act to improve kidney functioning are suggested. It is concluded that atenolol is a favourable choice for the treatment of hypertension in diabetic patients with normally functioning kidneys since, even in long-term use, normal renal functioning is preserved.

We have studied function and structure of the low density lipoprotein (LDL) receptors in a monensin-resistant (Monr-31) mutant isolated from Chinese hamster ovary (CHO) cells. To assay the ability of the receptor to bind LDL, we employed three methods, 125I-LDL binding to the cells at 4 degrees C, 125I-LDL binding to the receptor-phospholipid complex (Schneider, W.J., Goldstein, J.L., and Brown, M.S. (1980) J. Biol. Chem. 255, 11442-11447), and ligand blotting (Daniel, T.O., Schneider, W.J., Goldstein, J.L., and Brown, M.S. (1983) J. Biol. Chem. 258, 4606-4611). The LDL receptor number was similar in both CHO and Monr-31, but the binding affinity was reduced in the mutant. The semi-quantitative immunoblotting assay with an antibody directed against the COOH-terminal 14 amino acids and the ligand-blotting assay with LDL also showed that the relative steady-state level of the receptor in Monr-31 was comparable to that in CHO, whereas the binding capacity of the receptor in Monr-31 was lower than that in CHO. The precursor and degradation forms of the LDL receptors produced in the mutant cells were similar in size to those in the parental cells, but the apparent molecular mass of the mature receptor protein in sodium dodecyl sulfate-polyacrylamide gels was reduced about 5000 daltons in the mutant. These results suggest a structural change at the NH2-terminal LDL binding domain. Tests of the effects of tunicamycin, endo-alpha-N-acetylgalactosaminidase (O-glycanase), and sialidase (neuraminidase) on the molecular size of the mature receptors indicated that the reduced size of the receptor in the mutant cells resulted from altered oligosaccharide chain(s) linked to serine/threonine residues in the binding domain. We compared the molecular sizes and binding activity of human LDL receptors in several clones derived from CHO and Monr-31 cells which were transfected with human LDL receptor cDNA. The human LDL receptors produced in the transfected clones of Monr-31 were also smaller in molecular size and lower in binding capacity than those produced in the transfected clones of CHO. These results suggest that both structural and functional alteration of the LDL receptor of Monr-31 is not caused by a mutation in the structural gene of the LDL receptor but by altered processing or maturation of the receptor. The correlation of the decrease in molecular size and reduced binding capacity of the LDL receptor is discussed.

As lasers have an ability to deliver a large amount of energy very rapidly to matter, one can produce a plasma of thermonuclear temperature by laser bombardment of matter. We observed a neutron yield from a solid deuterium target irradiated by the beam of a glass laser, which had a power of 20 GW for 2 nsec. The theoretically estimated threshold laser power for anomalous heating owing to the parametric instability was in agreement with the experimental result. Above this threshold, an increase in the electron temperature, the appearance of a fast-ion group, and an anomaly in the reflection of light from the plasma were observed. These phenomena were closely correlated with the neutron yield. At the high-temperature region above a few hundred electron volts, the anomalous heating plays an essential role in reaching the fusion-reaction temperature.

Time _??_ variations of the reflected laser light from plasma were also investigated. From the time of flight measurements with a Faraday cage and an electrostatic energy analyzer, the different charged ion could be separated and ion temperatures and drift velocities were obtained from their velocity profiles. No influence of laser pulse duration to the temperature was observed in the cases of durations 2 ns, 4 ns and 10 ns. The experimental results for fast rising laser pulse were discussed and compared with those for slow rising one.

The results are given of investigations of the damage to laser glasses by high-power radiation, the use of liquid lasers as light amplifiers, and the application of high-power lasers in plasma heating. A description is given of a new method for the rapid measurement of the temperature and density of a plasma on the basis of the Thomson scattering of light. A complex structure is reported for the central ion peak in the spectrum of the scattered light. It is also reported that 20 neutrons are emitted from solid targets after irradiation with a single laser pulse.

The concentration of Cefoperazone (CPZ) in urological organs and serum was measured after the intravenous administration of 2 g of CPZ. Serum levels on healthy adults attained a maximal value of 122.1 +/- 4.2 micrograms/ml at 30 minutes. Serum levels of patients operated under general anesthesia attained a maximal value of 159.0 +/- 13.9 micrograms/ml at 30 minutes. Serum concentration on operated cases continued to be 1.3-2.2 fold higher than that of the healthy adults. Prostate tissue levels attained a maximal value of 80.9 +/- 3.6 micrograms/g at 30 minutes, vesical tissue levels were 52.1 +/- 1.3 microgram/g at 60 minutes and renal tissue were 94.0 +/- 14.5 micrograms/g at 90 minutes. Judging from the minimal inhibitory concentration of CPZ, CPZ seemed to be clinically useful in the treatment of urological infections.
