1. Prevalence and Comorbidities of Attention Deficit Hyperactivity Disorder Among Adults and Children/Adolescents in Korea

Abstract: Introduction: Objectives This study investigated the prevalence and comorbidities of attention deficit hyperactivity disorder (ADHD) among adults and children/adolescents in Korea. Methods This study used data from the Korea Health Insurance Review and Assessment Service collected from 2008 to 2018. Study participants comprised patients with at least one diagnosis of ADHD (International Statistical Classification of Diseases and Related Health Provisions, 10th revision code F90.0). Prevalence rates and psychiatric comorbidities were also analyzed. Results We identified 878,996 patients diagnosed with ADHD between 2008 and 2018. The overall prevalence rate of diagnosed ADHD increased steeply from 127.1/100,000 in 2008 to 192.9/100,000 in 2018; it increased 1.47 times in children/adolescents (≤ 18 years) and 10.1 times in adults (> 18 years) during this period. Among children/adolescent and adult ADHD patients, 61.84% (95% confidence interval [95% CI] 61.74−61.93) and 78.72% (95% CI 78.53−78.91) had at least one psychiatric comorbidity, respectively. Conclusions Our results showed that the prevalence rate of diagnosed ADHD has increased in Korea; however, it is lower than the global average. Abstract: HIV-1-specific CD8+ T cells are an important component of HIV-1 curative strategies. Viral variants in the HIV-1 reservoir may limit the capacity of T cells to detect and clear virus-infected cells. We investigated the patterns of T cell escape variants in the replication-competent reservoir of 25 persons living with HIV-1 (PLWH) durably suppressed on antiretroviral therapy (ART). We identified all reactive T cell epitopes in the HIV-1 proteome for each participant and sequenced HIV-1 outgrowth viruses from resting CD4+ T cells. All non-synonymous mutations in reactive T cell epitopes were tested for their effect on the size of the T cell response, with a≥50% loss defined as an escape mutation. The majority (68%) of T cell epitopes harbored no detectable escape mutations. These findings suggest that circulating T cells in PLWH on ART could contribute to control of rebound and could be targeted for boosting in curative strategies.

4. Wintertime aerosol optical and radiative properties in the Kathmandu Valley during the SusKat-ABC field campaign

Abstract: Particulate air pollution in the Kathmandu Valley has reached severe levels that are mainly due to uncontrolled emissions and the location of the urban area in a bowl-shaped basin with associated local wind circulations. The AERONET measurements from December 2012 to August 2014 revealed a mean aerosol optical depth (AOD) of approximately 0.30 at 675 nm during winter, which is similar to that of the post-monsoon but half of that of the pre-monsoon AOD (0.63). The distinct seasonal variations are closely related to regional-scale monsoon circulations over South Asia and emissions in the Kathmandu Valley. During the SusKat-ABC campaign (December 2012–February 2013), a noticeable increase in both aerosol scattering (σs; 313 → 577 Mm−1 at 550 nm) and absorption (σa; 98 → 145 Mm−1 at 520 nm) coefficients occurred before and after 4 January 2013. This can be attributed to the increase in wood-burned fires due to a temperature drop and the start of firing at nearby brick kilns. The σs value in the Kathmandu Valley was a factor of 0.5 lower than that in polluted cities in India. The σa value in the Kathmandu Valley was approximately 2 times higher than that at severely polluted urban sites in India. The aerosol mass scattering efficiency of 2.6 m2 g−1 from PM10 measurements in the Kathmandu Valley is similar to that reported in urban areas. However, the aerosol mass absorption efficiency was determined to be 11 m2 g−1 from PM10 measurements, which is higher than that reported in the literature for pure soot particles (7.5 ± 1.2 m2 g−1). This might be due to the fact that most of the carbonaceous aerosols in the Kathmandu Valley were thought to be mostly externally mixed with other aerosols under dry conditions due to a short travel time from their sources. The σs and σa values and the equivalent black carbon (EBC) mass concentration reached up to 757 Mm−1, 224 Mm−1, and 29 µg m−3 at 08:00 LST (local standard time), respectively but decreased dramatically during the daytime (09:00–18:00 LST), to one-quarter of the morning average (06:00–09:00 LST) due to the development of valley winds and an atmospheric bounder layer. The σs and σa values and the EBC concentration remained almost constant during the night at the levels of 410 Mm−1, 130 Mm−1, and 17 µg m−3, respectively. The average aerosol direct radiative forcings over the intensive measurement period were estimated to be −6.9 ± 1.4 W m−2 (top of the atmosphere) and −20.8 ± 4.6 W m−2 (surface). Therefore, the high atmospheric forcing (i.e., 13.9 ± 3.6 W m−2) and forcing efficiency (74.8 ± 24.2 W m−2 τ−1) can be attributed to the high portion of light-absorbing aerosols in the Kathmandu Valley, as indicated by the high black carbon (or elemental carbon) to sulphate ratio (1.5 ± 1.1).

7. Abstract TP203: Annual Population Burden Of Major Adverse Event From Statin Overuse In Acute Cerebral Ischemia Patients Without Indications

Abstract: Background: Statin agents play a major role in secondary prevention after acute cerebral ischemia (ACI) events but are not indicated in all ischemic stroke and TIA patients. National guidelines recommend statins for patients: 1) with ACI of large or small vessel atherosclerotic origin; and 2) without these stroke mechanisms, but co-existing CAD or primary prevention indications. The potential adverse effects of statin overuse in the remaining ACI patients has not been well delineated. Methods: Per PRISMA guidelines, we performed systematic meta-analyses of: 1) statin RCTs to determine absolute risk increases for 6 major adverse events; and 2) large clinical series to determine the proportion of ACI events due to large or small vessel atherosclerotic disease and the proportion of remaining patients with CAD/primary prevention statin indications. Results: For adverse effects, data was available from 63 RCTs enrolling 155,107 patients. Statin therapy increased the risk of occurrence of 6 conditions: diabetes, myalgia, myopathy, liver disease, renal insufficiency, and eye disease. Across 16 large series enrolling 16,161 patients, rate of ACI due to large and small artery atherosclerosis was 45.9% (LAA 19.3%, SVD 17.9%), rate of remaining patients with CAD/primary prevention statin indications 24.0%, and rate of patients without statin indication 30.0%. Data synthesis indicated that, in the US, were all ACI without statin indications treated with statins, a total of 8902 patients would develop needless adverse events each year, most commonly diabetes, myopathy, and eye disease (Table). Conclusion: Nearly one-third of acute cerebral ischemia patients do not have an indication for statins and statin overuse in these patients could annually lead to nearly 9000 needless adverse events, including diabetes, myopathy, and eye disease. These findings emphasize the importance of adhering to guideline indications for the start of statin therapy in acute cerebral ischemia.

Abstract: Traditionally, chemical agents such as formalin (FA) and β-propiolactone (BPL) have long been used for the preparation of inactivated vaccines or toxoids. It has been shown that FA extensively modifies vaccine antigens and thus affects immunogenicity profiles, sometimes compromising the protective efficacy of the vaccines or even exacerbating the disease upon infection. In this study, we show that natural catechins from green tea extracts (GT) can be used as an inactivating agent to prepare inactivated viral vaccines. GT treatment resulted in complete and irreversible inactivation of influenza virus as well as dengue virus. In contrast to FA that reacted extensively with multiple amino acids including lysine, a major anchor residue for epitope binding to MHC molecules, GT catechin epigallocatechin-3-gallate (EGCG) crosslinked primarily with cysteine residues and thus preserved the major epitopes of the influenza hemagglutinin. In a mouse model, vaccination with GT-inactivated influenza virus (GTi virus) elicited higher levels of viral neutralizing antibodies than FA-inactivated virus (FAi virus). The vaccination completely protected the mice from a lethal challenge and restricted the challenge viral replication in the lungs. Of note, the quality of antibody responses of GTi virus was superior to that with FAi virus, in terms of the magnitude of antibody titer, cross-reactivity to hetero-subtypes of influenza viruses, and the avidity to viral antigens. As the first report of using non-toxic natural compounds for the preparation of inactivated viral vaccines, the present results could be translated into a clinically relevant vaccine platform with improved efficacy, safety, productivity, and public acceptance.

9. DYNAMICS OF RENDERING THE COCKNEY ETHNOSOCIOLECT IN THE UKRAINIAN TRANSLATIONS OF G. B. SHAW’S PLAY «PIGMALION»

Abstract: This article reviews the strategies used in three Ukrainian translations of George Bernard Shaw’s play «Pygmalion» focusing upon different approaches to representing the sociolect Cockney. Two of the translations (done by M. Pavlov and O.Mokrovolskiy) resort to surzhyk — a mixed Ukrainian-Russian vernacular, thus employing the strategy of domestication, while the third, and the latest, one (done by T. Nekriach and N. Ferens in collaboration, with the general editing of T. Nekriach) rejects surzhyk in principle, proceeding from the idea that cockney is not a contamination of two languages but a socially and culturally marked set of deviations from the norm within one language. The latter translation unites foreignization in indicating the time and place of action and domestication in consistent using the Ukrainian supradialectal popular parlance, which is termed ad hoc the «harmonizing strategy» in the article. Cockney as a specific ethnosociolect has been researched in the translation perspective in the works of I. Akopyan, V. Komissarov, O. Rebriy, T. Nekriach, A. Hughes, P. Trudgill etc., which form the theoretical foundation of the present article. The aim of the article is to study and systematize the optimal strategies and tactics of reproducing Cockney in the available Ukrainian translations of «Pygmalion». The principal method of research is the comparative translation analysis, which allows to evaluate the gains and losses in employing a particular strategy in order to achieve a faithful translation. The topicality of the research is accounted for by the growing interest on the part of both practical translators and translation scholars in the appropriate handling of translation strategies and tactics within one text in order to reveal the author’s intent to the full with the retaining of the distinguishing features of the form. Special attention is paid to the specific «double» nature of drama works which requires taking into account the “pronounceability” of cues in translation. It is argued that M. Pavlov and T. Nekriach/N. Ferens take this parameter into account, translating «for stage», whereas O.Mokrovolskiy translates «for page» only, which results in his alternatives for Cockney representation being understood visually, not audially. The research prospects are seen in applying the proposed methodology to the study of recent Ukrainian translations of fiction in comparison with the previous ones in order to survey the dynamics and effectiveness of applying various translation strategies and tactics in reproducing a particular language or cultural phenomenon used in the original text.

Abstract: The replacement of conventional protein supply has been studied in the context of continuous population growth and the consequent environmental problems. Among protein substitutes, edible insects have gained relevance due to their high breeding efficiency. This study was conducted to estimate the effect of the partial replacement of porcine myofibril protein (pork protein; PP) with protein extracted from Protaetia brevitarsis (insect protein; IP) in protein-containing solutions and gels. The protein concentration was regulated at 30 and 1 mg/ml for gels and solutions, respectively, and the replacement percentages were increased by 20%. Gels containing IP showed a lower viscosity and thermal stability; the texture and surface of the gels were negatively impacted as the replacement percentage increased. Therefore, heating is not an appropriate method to induce the gelation of IP containing solutions. However, the replacement of PP with IP increased the foam capacity and emulsion capacity in solutions. In conclusion, we show that the partial replacement of PP with IP positively impacts on protein solutions but impairs their gelation.

11. Small bowel obstruction caused by hemorrhagic metastatic melanoma: case report and literature review

Abstract: Melanoma is one of the most common cancers and causes of death from skin cancer. It is also one of the most common malignancies to metastasize to the gastrointestinal tract. However, the incidence of gastrointestinal metastases from unknown primary cutaneous melanoma is reported to be 4–9%. Despite advances in diagnostic techniques, only 1.5–4.4% of metastases to the gastrointestinal tract are detected before death. We report a patient with no prior history of melanoma who presented with both gastrointestinal hemorrhage and small bowel obstruction. Diagnostic laparoscopy, followed by small bowel resection, was performed after a red blood cell radionuclide scan showed active hemorrhage. Pathologic examination confirmed metastatic melanoma of the jejunum. Additional small bowel resections were required for recurrent gastrointestinal bleeding, confirming two additional metastases. A review of the literature on small intestinal melanoma causing hemorrhage and obstruction is discussed.

Abstract: In this study, carbon/epoxy composites were manufactured by coating with a polyamide at different weight percentages (5 wt.%, 10 wt.%, 15 wt.%, and 20 wt.%) to improve their impact resistance and fracture toughness. The chemical reaction between the polyamide and epoxy resin were examined by fourier transform infrared spectroscopy, differential scanning calorimetry and X-ray photoelectron spectroscopy. The mechanical properties and fracture toughness of the carbon/epoxy composites were analyzed. The mechanical properties of the carbon/epoxy composites, such as transverse flexural tests, longitudinal flexural tests, and impact tests, were investigated. After the impact tests, an ultrasonic C-scan was performed to reveal the internal damage area. The interlaminar fracture toughness of the carbon/epoxy composites was measured using a mode I test. The critical energy release rates were increased by 77% compared to the virgin carbon/epoxy composites. The surface morphology of the fractured surface was observed. The toughening mechanism of the carbon/epoxy composites was suggested based on the confirmed experimental data.

17. Serum vitamin D level mitigates fractional exhaled nitric oxide linked to bisphenol-A in school-aged children

Abstract: Previous studies on the relationship of bisphenol-A (BPA) with fractional exhaled nitric oxide (FeNO) had conflicting results, suggesting that other factors may modulate this relationship. Thus, we investigated the modulating effect of vitamin D on the relationship of BPA with FeNO in children.This study recruited 432 children (10 to 12 years old) from the general pediatric population of Korea between June and July 2017. We conducted measurements of urinary BPA, serum vitamin D, specific serum IgE, FeNO, and data from impulse oscillometry (reactance area [AX], airway resistance at 5 Hz [Rrs5] and 20 Hz [Rrs10], and the difference of Rrs5 and Rrs20 [Rrs5-20]).Serum vitamin D (adjusted β =- 0.014, p=0.002) and urinary BPA (β = 0.006, p0.001) level was significantly associated with FeNO. Urinary BPA level was significantly associated with FeNO in children with low vitamin D levels (≤23 ng/mL; αβ = 0.006, p0.001), but not in children with high vitamin D levels (23 ng/mL). The interaction of vitamin D and BPA had a significant effect on FeNO (pint = 0.005). There was no relationship with the airway lung function (Rrs5, AX, and Rrs5-20) to serum vitamin D and urinary BPA level. Vitamin D ameliorated the BPA-mediated increase of FeNO in children.These results suggest that children with low vitamin D levels may be more susceptible to airway inflammation due to BPA.

Abstract: Both morphological characteristics and amplified fragment length polymorphism (AFLP) markers were used to validate the genetic fidelity of 1 080 field-grown Echinacea purpurea plants regenerated from leaf explants of donor T5-9. Morphological diagnosis revealed that 1 067 out of 1 080 regenerants were normal, while 13 regenerants were aberrant. AFLP analysis was further performed to assess DNA variations among donor, 43 sampled normal regenerants and all 13 aberrant regenerants. Seven primer combinations generated 471 fragments among donor and normal regenerants, of which 9 fragments were polymorphic. The same primer pairs generated 484 fragments for aberrant regenerants, of which 417 fragments were polymorphic. UPGMA clustering indicated that 42 normal regenerants and donor fell into same cluster at similarity scale of > 0.99, while all 13 aberrant regenerants and one morphologically normal regenerant comprised the other clusters. AFLP analysis indicated that these 14 regenerants are off-types.

Abstract: Heart failure (HF) with preserved ejection fraction (HFpEF) is increasingly recognized as an important clinical entity. Preclinical studies have shown differences in the pathophysiology between HFpEF and HF with reduced ejection fraction (HFrEF). Therefore, we hypothesized that a systematic metabolomic analysis would reveal a novel metabolomic fingerprint of HFpEF that will help understand its pathophysiology and assist in establishing new biomarkers for its diagnosis.Ambulatory patients with clinical diagnosis of HFpEF (n = 24), HFrEF (n = 20), and age-matched non-HF controls (n = 38) were selected for metabolomic analysis as part of the Alberta HEART (Heart Failure Etiology and Analysis Research Team) project. 181 serum metabolites were quantified by LC-MS/MS and 1H-NMR spectroscopy. Compared to non-HF control, HFpEF patients demonstrated higher serum concentrations of acylcarnitines, carnitine, creatinine, betaine, and amino acids; and lower levels of phosphatidylcholines, lysophosphatidylcholines, and sphingomyelins. Medium and long-chain acylcarnitines and ketone bodies were higher in HFpEF than HFrEF patients. Using logistic regression, two panels of metabolites were identified that can separate HFpEF patients from both non-HF controls and HFrEF patients with area under the receiver operating characteristic (ROC) curves of 0.942 and 0.981, respectively.The metabolomics approach employed in this study identified a unique metabolomic fingerprint of HFpEF that is distinct from that of HFrEF. This metabolomic fingerprint has been utilized to identify two novel panels of metabolites that can separate HFpEF patients from both non-HF controls and HFrEF patients.ClinicalTrials.gov NCT02052804.

Abstract: Smartphone technology has propelled the evolution of health-related mobile technology, referred to as mobile health (mHealth). With the rise of smartphone ownership and the growing popularity of health-related smartphone usage, mHealth offers potential benefits for both patients and health care providers. The objective of this review is to assess the current state of smartphone technology in urology. A literature search of PubMed database was conducted to identify articles reporting on smartphone technology in urology. Publications were included if they focused on smartphone mHealth technology pertinent to the field of urology or included an evaluation of urological applications in digital stores. We identified 50 publications focused on the use of smartphones in urology. Studies were then grouped into the following categories: smartphones employing the built-in camera and light source, applications specific to prostate cancer, urolithiasis, pediatric urology, and as educational tools for urologists. In 23/50 (46%) studies, smartphone technology/intervention was compared to a control group or to standard of care. In this regard, smartphone technology did not demonstrate benefit over standard of care in 13 studies. In contrast, in 10 studies, smartphone interventions were proven beneficial over current practice. Smartphone technology is constantly evolving and has the potential to improve urological care and education. Of concern to consumer and urologist alike is that these downloadable programs are limited due to the accuracy of their content, risk of confidentiality breach, and the lack of central regulation and professional involvement in their development.

Abstract: The California Cancer Registry was analyzed for bladder cancer survival disparities based on race, socioeconomic status (SES), and insurance types. Survival analyses were performed for 72,452 cases to determine the prognostic significance of racial and socioeconomic factors. Black race, low SES, and Medicaid insurance portend poorer outcomes. These findings reflect a multifaceted socioeconomic and public health conundrum. PURPOSE: To examine the California Cancer Registry (CCR) for bladder cancer survival disparities based on race, socioeconomic status (SES), and insurance in California patients. PATIENTS AND METHODS: The CCR was queried for bladder cancer cases in California from 1988 to 2012. The primary outcome was disease-specific survival (DSS), defined as the time interval from date of diagnosis to date of death from bladder cancer. Survival analyses were performed to determine the prognostic significance of racial and socioeconomic factors. RESULTS: A total of 72,452 cases were included (74.5% men, 25.5% women). The median age was 72 years (range, 18–109 years). The racial distribution among the patients was 81% white, 3.8% black, 8.8% Hispanic, 5.2% Asian, and 1.2% from other races. In black patients, tumors presented more frequently with advanced stage and high grade. Medicaid patients tended to be younger and had more advanced-stage, higher-grade tumors compared to patients with Medicare or managed care (P < .0001). Kaplan-Meier analysis demonstrated significantly poorer 5-year DSS in black, low SES, and Medicaid patients (P < .0001). When controlling for stage, grade, age, and gender, multivariate analysis revealed that black race (DSS hazard ratio = 1.295; 95% confidence interval, 1.212–1.384), low SES (DSS hazard ratio = 1.325; 95% confidence interval, 1.259–1.395), and Medicaid insurance (DSS hazard ratio = 1.349; 95% confidence interval, 1.246–1.460) were independent prognostic factors (P < .0001). CONCLUSION: An analysis of the CCR demonstrated that black race, low SES, and Medicaid insurance portend poorer DSS. These findings reflect a multifaceted socioeconomic and public health conundrum, and efforts to reduce inequalities should be pursued.

Abstract: Background One of the greatest challenges presented with RIRS is the potential for movement of the stone within the operative field associated with diaphragm and chest respiratory excursions due to mechanical ventilation. To overcome this challenge, we propose in this pilot study a new general anesthesia technique combining high frequency jet ventilation (HFJV) with small volume mechanical ventilation (SVMV). Data regarding safety, feasibility and surgeons’ impression was assessed. Methods Patients undergoing RIRS for kidney stones from November 2017 to May 2018 were prospectively recruited to participate in the study. In each case after the beginning of general anesthesia (GA) with mechanical ventilation (MV) surgeons were asked to assess the mobility of the operative field and conditions for laser lithotripsy according to the developed questionnaire scale. The questionnaire consisted of 5 degrees of assessment of kidney mobility and each question was scored from 1 to 5, 1 being very mobile (extremely poor conditions for dusting) and 5 completely immobile (Ideal conditions for dusting). After the assessment GA was modified with combined respiratory support (CRS), reducing tidal volume and respiratory rate (small volume mechanical ventilation, SVMV) and applying in the same time transcatheter high frequency jet ventilation (HFJV) inside the closed circuit. After beginning of CRS, surgeons were once again asked to assess the mobility of the operative field and the conditions for laser lithotripsy. Main ventilation parameters were recorded and compared in both regimens. Results A total of 38 patients were included in the study. The mean age was 49 (range 45–53) with a mean stone size of 10 mm (range 10–14) and Hounsfield unit of 1060 (range 930–1190). All patients underwent successful RIRS and no intraoperative complications occurred throughout the duration of the study. A statistically significant difference between ventilation parameters prior to and after CRS institution was detected in all cases, however their clinical impact was negligible. Abstract: Gold flotation concentrate may contain relatively high concentrations of valuable metals such as iron, copper, and zinc, and occasionally, even precious metals such as gold. The major components of the concentrate are SiO2, Fe2O3, and Al2O3, but it also contains reactive sulphide minerals such as pyrite. The sulphides in the flotation concentrate are fully converted into an oxide form during oxidative roasting, therefore, the chemical composition of the roasted concentrate is considered to be a copper slag. High temperature smelting with additives to dissolve Au from the gold concentrate into a molten copper was used in the research. Gold greatly dissolved at 1600?? under a CaO/SiO2 ratio of 1.25, suggesting the increase in the dissolution of gold into molten copper with decreasing viscosity of the molten slag-like concentrate at high temperatures.

Abstract: This work strives to apply the in-slot direct cooling technique in electric machines and optimize the cooling design to improve the machine performance in both cooling and electromagnetic (EM) aspects. The in-slot cooling directly removes the heat from the heating source, but requires extra spaces in the slots that possibly reduce the electromagnetic performance of the machine. It needs an overall evaluation considering both EM and thermal performances to achieve an optimal machine design for in-slot cooling systems. This work utilizes a thermal-network based numerical modeling approach combined with finite element (FE) EM simulations to evaluate the proposed in-slot cooling scheme and compares it with a drip-cooled machine product from multiple aspects, such as torque output, temperature, efficiency and pump power. The thermal network model was verified with thermal FE simulations and is capable of predicting the transient temperature of the machine system. The temperature distribution and corresponding power losses of the electric machines with in-slot cooling designs were calculated on various machine operating points. The final efficiency of the in-slot cooled machine was compared with the drip-cooled machine. Then, an in-slot cooling design was chosen based on the simulation results and comparison for prototyping. Finally, a 3D printed prototype of a partial machine with in-slot cooling channels was designed and constructed to check the manufacturability of the proposed cooling design.

Abstract: Computerized tomographic urography is the diagnostic tool of choice for evaluating hematuria. In keeping with the ALARA (As Low As Reasonably Achievable) principle, we evaluated a triple bolus computerized tomography protocol designed to reduce radiation exposure.Patients with macroscopic or microscopic hematuria were prospectively randomized to conventional computerized tomography (100) or triple bolus computerized tomography (100). The triple bolus computerized tomography protocol entails 2 scans: pre-contrast scan followed by 3 contrast injections at 40 seconds, 60 seconds and 20 minutes prior to the second scan to capture all 3 phases. The conventional computerized tomography protocol requires 4 scans: pre-contrast scan, and 3 post-contrast scans at the corticomedullary, nephrographic and excretory phases. Radiation exposure and the detection of urological pathology were recorded based on radiology reports.There were no differences in patient demographics or body mass index between the 2 groups. Triple bolus computerized tomography exposed patients to 33% less radiation (1,715 vs 1,145 mGy*cm for conventional vs triple bolus computerized tomography; p0.001). For macroscopic hematuria, the pathology detection rates were 70% for triple bolus and 73% for conventional computerized tomography (p=0.72). For microscopic hematuria, the detection rates were 59% for triple bolus and 50% for conventional computerized tomography (p=0.68). In both groups, the rates of detection of urolithiasis, renal cysts, urological masses, bladder pathology and prostate pathology were no different between triple bolus and conventional computerized tomography.In both the settings of macroscopic and microscopic hematuria evaluation, triple bolus computerized tomography significantly reduces radiation exposure while providing equivalent detection of genitourinary pathology compared to conventional computerized tomography. The ability to detect upper tract filling defects was not specifically tested.

Abstract: This protocol is for the Aegea Biotechnologies rapid PCR-based SARS-CoV-2 test. This assay uses patented "Switch-Blocker" technology as well as taqman probes to test for presence of SARS-CoV-2 and simultaneously orthogonally validate. A single amplification reaction is performed, and "Switch Blocker" is used on the forward strand and taqman is used on the reverse strand. The assay design has high sensitivity & specificity--single nucleotide level. Moreover, it is able to detect the SARS-CoV-2 L strain vs the SARS-CoV-2 S strain. The test can be adapted to point of care (Roche LIAT) as well as for different SARS-CoV-2 strains as the virus mutates. A next generation version of the assay could identify the presence of the SARS-CoV-2 L/S strains vs. influenza A/B. Finally, because of the sensitivity and specificity, the Aegea PCR-based SARS-CoV-2 test should be able to use saliva samples, and it is suitable for pooled testing. Abstract: To evaluate the efficacy of interactive virtual reality (iVR) in providing a three-dimensional (3D) experience with the donor's anatomy for surgeons and patients, we present a retrospective, case-controlled study assessing the impact of iVR renal models prior to LDN on both surgical outcomes and patients' understanding of the procedure.Twenty patients undergoing LDN were prospectively recruited; their contrast-enhanced CT scans were transformed into iVR models. An iVR platform allowed the surgeons to rotate and deconstruct the renal anatomy; patients could also view their anatomy as the procedure was explained to them. Questionnaires assessed surgeons' understanding of renal anatomy after CT alone and after CT+iVR. Surgeons also commented on whether iVR impacted their preoperative plan. Patients assessed their anatomical understanding and anxiety level before and after iVR. Surgical outcomes for the iVR cohort were compared to a retrospectively matched, non-iVR cohort of LDN patients.Surgeons altered their preoperative plan in 18 of 20 LDNs after viewing iVR models. Patients reported better understanding of their anatomy (5/5) and noted decreased preoperative anxiety (5/5) after viewing iVR. When compared to the non-iVR group, the iVR group had a 25% reduction in median operative time (P.001). In terms of surgical outcomes, patients in the iVR group had a 40% lower median relative change in postoperative creatinine (P.001).Preoperative viewing of iVR models altered the operative approach, decreased the operative time, and improved donor patient outcomes. iVR models also reduced patients' preoperative anxiety.

Abstract: Oral administration of resveratrol attenuates several symptoms associated with the metabolic syndrome, such as impaired glucose homeostasis and hypertension. Recent work has shown that resveratrol can improve glucose homeostasis in obesity via changes in the gut microbiota. Studies involving fecal microbiome transplants (FMTs) suggest that either live gut microbiota or bacterial-derived metabolites from resveratrol ingestion are responsible for producing the observed benefits in recipients. Herein, we show that obese mice receiving FMTs from healthy resveratrol-fed mice have improved glucose homeostasis within 11 days of the first transplant, and that resveratrol-FMTs is more efficacious than oral supplementation of resveratrol for the same duration. The effects of FMTs from resveratrol-fed mice are also associated with decreased inflammation in the colon of obese recipient mice. Furthermore, we show that sterile fecal filtrates from resveratrol-fed mice are sufficient to improve glucose homeostasis in obese mice, demonstrating that nonliving bacterial, metabolites, or other components within the feces of resveratrol-fed mice are sufficient to reduce intestinal inflammation. These postbiotics may be an integral mechanism by which resveratrol improves hyperglycemia in obesity. Resveratrol-FMTs also reduced the systolic blood pressure of hypertensive mice within 2 wk of the first transplant, indicating that the beneficial effects of resveratrol-FMTs may also assist with improving cardiovascular conditions associated with the metabolic syndrome.

Abstract: This paper presents results of an all-sky search for periodic gravitational waves in the frequency range [50, 1 190] Hzand and with frequency derivative range of approx. [-20,1.1] x 10(exp-10) H(sub 0)z s(exp-1) for the fifth LIGO science run (S5). The search uses a noncoherent Hough-transform method to combine the information from coherent searches on time scales of about one day. Because these searches are very computationally intensive, they have been carried out with the Einstein@Home volunteer distributed computing project. Postprocessing identifies eight candidate signals; deeper follow-up studies rule them out. Hence, since no gravitational wave signals have been found, we report upper limits on the intrinsic gravitational wave strain amplitude ho. For example, in the 0.5 Hz-wide band at 152.5 Hz, we can exclude the presence of signals with h(exp 0) greater than 7.6 x 10(exp-25) at a 90% confidence level. This search is about a factor 3 more sensitive than the previous Einstein@Home search of early S5 LIGO data.

Abstract: We investigated whether treatment of mice with established pressure overload-induced heart failure (HF) with the naturally occurring polyphenol resveratrol could improve functional symptoms of clinical HF such as fatigue and exercise intolerance. C57Bl/6N mice were subjected to either sham or transverse aortic constriction surgery to induce HF. Three weeks postsurgery, a cohort of mice with established HF (%ejection fraction −1·day−1) or vehicle for 2 wk. Although the percent ejection fraction was similar between both groups of HF mice, those mice treated with resveratrol had increased total physical activity levels and exercise capacity. Resveratrol treatment was associated with altered gut microbiota composition, increased skeletal muscle insulin sensitivity, a switch toward greater whole body glucose utilization, and increased basal metabolic rates. Although muscle mass and strength were not different between groups, mice with HF had significant declines in basal and ADP-stimulated O2 consumption in isolated skeletal muscle fibers compared with sham mice, which was completely normalized by resveratrol treatment. Overall, resveratrol treatment of mice with established HF enhances exercise performance, which is associated with alterations in whole body and skeletal muscle energy metabolism. Thus, our preclinical data suggest that resveratrol supplementation may effectively improve fatigue and exercise intolerance in HF patients. NEW & NOTEWORTHY Resveratrol treatment of mice with heart failure leads to enhanced exercise performance that is associated with altered gut microbiota composition, increased whole body glucose utilization, and enhanced skeletal muscle metabolism and function. Together, these preclinical data suggest that resveratrol supplementation may effectively improve fatigue and exercise intolerance in heart failure via these mechanisms. Abstract: Since left ventricular hypertrophy (LVH) increases the susceptibility for the development of other cardiac conditions, pharmacotherapy that mitigates pathological cardiac remodeling may prove to be beneficial in patients with LVH. Previous work has shown that the activation of the energy-sensing kinase AMP-activated protein kinase (AMPK) can inhibit some of the molecular mechanisms that are involved in LVH. Of interest, metformin activates AMPK through its inhibition of mitochondrial complex I in the electron transport chain and can prevent LVH induced by pressure overload. However, metformin has additional cellular effects unrelated to AMPK activation, raising questions about whether mitochondrial complex I inhibition is sufficient to reduce LVH. Herein, we characterize the cardiac effects of a novel compound (R118), which is a more potent complex I inhibitor than metformin and is thus used at a much lower concentration. We show that R118 activates AMPK in the cardiomyocyte, inhibits multiple signaling pathways involved in LVH, and prevents Gq protein-coupled receptor agonist-induced prohypertrophic signaling. We also show that in vivo administration of R118 prevents LVH in a mouse model of hypertension, suggesting that R118 can directly modulate the response of the cardiomyocyte to stress. Of importance, we also show that while R118 treatment prevents adaptive remodelling in response to elevated afterload, it does so without compromising systolic function, improves myocardial energetics, and prevents a decline in diastolic function in hypertensive mice. Taken together, our data suggest that inhibition of mitochondrial complex I may be worthy of future investigation for the treatment of LVH. NEW & NOTEWORTHY Inhibition of mitochondrial complex I by R118 reduces left ventricular hypertrophy (LVH) and improves myocardial energetics as well as diastolic function without compromising systolic function. Together, these effects demonstrate the therapeutic potential of complex I inhibitors in the treatment of LVH, even in the presence of persistent hypertension.
