1. Student Experience and Academic Performance in an Online Asynchronous Community College Course Delivered in Accelerated, Cohort, and Traditional Formats: A Mixed Methods Study

Abstract

This study used an explanatory sequential mixed methods approach to compare student experience and academic performance in an asynchronous online community college course delivered in three formats: accelerated (5-week) cohort, accelerated (5-week) non-cohort, and non-accelerated (16-week) non-cohort. Quantitative results showed no significant difference in final grades, successful course completion, or withdrawals among course formats. No significant differences emerged in course learning outcome (CLO) attainment among course formats, except for the public speaking CLO, with 16-week non-cohort students performing significantly better than 5-week non-cohort students. Quantitative results also revealed that 5-week cohort students rated their course and instructor significantly higher on course evaluation surveys (CES) than 16-week non-cohort students. Qualitative findings supported quantitative results on academic performance; however, qualitative data contradicted differences identified in CES results by revealing similar student experiences across course formats. Overall, this study highlights the nuanced effects of course format on student outcomes and provides insights into online asynchronous course offerings in community college settings. Recommendations include maintaining a range of asynchronous course formats to allow students the flexibility to select the course format that best aligns with their needs, alongside the implementation of support measures such as online orientations to prepare students for asynchronous coursework. Abstract

Renal transplantation is a common treatment option for patients with end-stage renal disease. The goal of transplantation is to restore normal renal function and improve their quality of life. However, some patients may experience complications after transplantation, including the development of calculi or tumors in their native kidneys. In such cases, the question arises whether native nephrectomy should be performed during renal transplantation or not. A 62-year-old patient with a history of renal transplant twenty years ago presented a macroscopic hematuria.

3. Rare association of central pontine myelinolysis with intrasellar arachnoidocele - casual or correlated?

Abstract

Our case report presents a patient with central pontine myelinolysis and arachnoidocele. He was hospitalized twice these 2 last months for a confusional syndrome associated to an alteration of his general health where metabolic disorders where found: a hyponatremia at 125 mmol/l that was quickly corrected and a hypoglycemia at 0.30 g/l. A central pontine myelinolysis was found as an iso-signal on T1-weighted sequences and a hypersignal on T2-weighted and FLAIR sequences on magnetic resonnance imaging. Central pontine myelinolysis lesions did not enhance with contrast. Incidental imaging findings of arachnoidocele was detected. Through this case, we would like to share with the other practitioners these rare images and the consequence of a diagnostic delay. Indeed, hyponatremia in our patient could be the consequence of the intrasellar arachnoidocele and the overly rapid correction of this chronic hyponatremia caused central pontine myelinolysis, or it is an accidental phenomenon where we found both lesions.

Abstract

Resume Introduction Les voies biliaires extra-hepatiques presentent de nombreuses variantes atomiques et des rapports complexes avec les differentes structures anatomiques avoisinantes. Les variantes concernent le nombre, la forme et la disposition, dependant de l’embryogenese de ces elements. Donnees recentes La cholangio-pancreato-IRM represente l’examen de choix non invasif permettant de realiser une cartographie complete de l’arbre biliaire. Elle permet une analyse de la disposition des voies biliaires extra-hepatiques, avec une detection des variantes anatomiques. Certaines variantes exposent a des a des complications chirurgicales (comme les canaux hepatocystiques), d’autres etant associees a un risque de degenerescence maligne. Conclusion La connaissance de la morphologie et des variantes des voies biliaires est indispensable a tous radiologues et ce travail presente les principales qu’il faut connaitre.

Abstract

Cerebral fat embolism is a rare and potentially fatal condition that may occur following a long bone fracture. Its characterized by respiratory, neurological, and mucocutaneous signs. Isolated severe brain syndrome remains exceptional. We report a 21-year-old male patient admitted for the cerebral manifestation of a fat embolism syndrome due to a fracture of long bone after a traffic accident injury. Neurological deterioration after a free interval was seen with generalized tonic-clonic seizures. MRI of the brain was indicated which showed numerous multifocal hyperintensities involving the deep white matter of both hemispheres producing a “starfield” appearance. This pattern of cytotoxic cerebral edema, with lesions in the white matter rather than the grey matter, is indicative of the subacute stage of fat embolism. The patient was treated with comprehensive support in the intensive care unit, he returned to normal neurological function and was discharged after 3 weeks of hospitalization.

Abstract

Penelitian ini bertujuan untuk mengetahui pengaruh bentuk dan interaksi telur puyuh dan bobot telur puyuh terhadap jenis kelamin, bobot tetas dan lama tetas. Materi penelitian menggunakan telur puyuh terseleksi sebanyak 162 butir. Metode penelitian adalah Rancangan Acak Lengkap faktorial dan transformasi akar kuadrat yang terdiri dari dua faktor yaitu bentuk telur dan bobot telur yang meliputi bobot ringan, sedang, dan berat. Hasil penelitian menunjukkan bahwa bentuk telur berpengaruh tidak nyata (P>0,05) terhadap jenis kelamin, sedang faktor bobot telur berpengaruh sangat nyata (P0,05) terhadap bobot tetas, sedang faktor bobot telur berpengaruh nyata (P0,05) terhadap lama tetas. Berdasarkan hasil penelitian dapat disimpulkan bahwa semakin berat bobot telur maka akan mengasilkan DOQ berjenis kelamin jantan begitu pula sebaliknya dan semakin tinggi bobot telur akan menghasilkan DOQ dengan bobot tetas yang tinggi begitu pula sebaliknya. Berdasarkan hasil penelitian disarankan untuk penetasan telur puyuh dengan bobot ringan sehingga berpeluang lebih besar untuk menghasilkan DOQ betina. SHAPE EFFECT OF EGGS AND EGG WEIGHT SEX, WEIGHT AND OLD HATCHING HATCHING QUAIL (Coturnix-coturnix japonica) ABSTRACT The aim of the research was to examine the influence of egg shape and egg weight of Japanese quail egg towards sex, weight, and time of hatching, and the interaction between the shape and weight of Japanese quail eggs with sex, weight, and time of hatching. Materials research using 162 eggs of Japanese quail. The research methodused wasCompletely Randomized Design with factorial design twofactors. The second method is the square root transformation for the variable of sex. The results showed that the egg shape had no significant influence (P>0.05)to variable of sex, and egg weight factor had a very real influence (P0.05) to the weight of hatching, and weight factorhas a significant influence (P0.05)to hatching time. Results of the research can be concluded that eggs with heavy weights greater opportunity to produce DOQ male gender and vice-versa, and the higher weight of the eggs will be producing DOQ with high weight and vice-versa. Abstract

Although lithium is widely used in current practice to treat bipolar disorder (BD) and treatment-resistant major depressive disorder (MDD) among older adults, little is known about its efficacy and tolerability in this population, which is generally excluded from randomized clinical trials. The objective of this study was to evaluate the efficacy and tolerability of long-term use of lithium among older adults with BD and MDD.Data from the Cohort of individuals with Schizophrenia and mood disorders Aged 55 years or more (CSA) were used. Two groups of patients with BD and MDD were compared: those who were currently receiving lithium versus those who were not. The effects of lithium on psychiatric (i.e., depressive symptoms severity, perceived clinical severity, rates of psychiatric admissions in the past-year), geriatric (overall and cognitive functioning) and physical outcomes (i.e., rates of non-psychiatric medical comorbidities and general hospital admissions in the past-year) were evaluated. All analyses were adjusted for age, sex, duration of disorder, diagnosis, smoking status, alcohol use, and use of antipsychotics, antiepileptics or antidepressants.Among the 281 older participants with BD or MDD, 15.7% were taking lithium for a mean duration of 12.5(SD = 11.6) years. Lithium use was associated with lower intensity of depressive symptoms, reduced perceived clinical global severity and lower benzodiazepine use (all p 0.05), without being linked to greater rates of medical comorbidities, except for hypothyroidism.Data were cross-sectional and data on lifetime history of psychotropic medications was not assessed.Our results suggest that long-term lithium use may be efficient and relatively well-tolerated in older adults with BD or treatment-resistant MDD.

Abstract

Introduction COVID-19 is an infectious disease caused by a new coronavirus. The first cases were identified in Wuhan. It rapidly spread causing a pandemic worldwide. The incidence and severity of this disease are likely to be different in children compared with adults. Few publications of COVID-19 in children have been published. Our Moroccan paediatric series is among the first studies on this disease in Africa. Methods We included all children with COVID-19 who were admitted and treated at the hospital from March 25 to April 26, 2020. We have collected information, including demographic data, symptoms, imaging data, laboratory results, treatments and clinical progress from patients with COVID-19. Results Since the outbreak of 2019 novel coronavirus infection (2019-nCoV) in Morocco, a total of 145 COVID-19 confirmed cases have been reported in the Cheikh Khalifa's Hospital. Among this cases, 15 children were registered. The median age of patients was 13 years. There were 7 boys and 8 girls. Five children are asymptomatic, 8 have mild symptoms and 2 have a moderate respiratory difficulty. The RT-PCR test results were positive in all patients. Radiologically, we found in 2 cases, multiple nodules with ground-glass opacities on the chest scan. The treatment was based on the combination of hydroxychloroquine and azithromycin. Evolution under treatment was good for all patients. Conclusion This study describes the profile of COVID-19 in child in a Moroccan hospital and confirms that the severity of illness in children with COVID-19 to be far less than adults.

Le diabete insipide demeure la presentation la plus commune. Dans ce travail, nous rapportons l’observation d’une patiente traitee pour un cancer du sein ayant presente une metastase hypothalamo-hypophysaire apres six ans d’evolution. Le diagnostic a ete confirme par une IRM.

Abstract

Changes of the physicochemical characteristics of sandy soil, with low productivity, treated by the addition of eco and environmentally-friendly composts were evaluated. Two composts were produced from sludge of an urban wastewater treatment plant mixed with two different types of vegetable by-products, wood sawdust and straw. Studies were made on plant pots and two types of wheat were produced inside a greenhouse. Sandy soil tested was characterized before and after wheat production. The obtained results have shown increased levels of available nutrients as nitrogen, phosphorus, calcium and magnesium, an increase of the soil pH and an enhancement of the cationic exchange capacity of the soil as a function of the compost addition. On the other hand, the germination and growth of two types of wheat was improved by a factor of two and three, respectively in the amended soils. The beneficial effect of compost amendment was proportional to the employed dose. All results suggest that application of biocompost to sandy soil is a feasible option as a fertilizer substitute to supplement plant nutrient needs. This alternative material can replace environmentally dangerous and hazardous chemical products traditionally applied in agriculture.

Abstract

Little is known about the respective effects of depression and cognitive impairment on quality of life among older adults with schizophrenia spectrum disorder.We used data from the Cohort of individuals with Schizophrenia Aged 55-years or more (CSA) study, a large multicenter sample of older adults with schizophrenia or schizoaffective disorder (N = 353). Quality of life (QoL), depression and cognitive impairment were assessed using the Quality of Life Scale (QLS), the Center of Epidemiologic Studies Depression scale and the Mini-Mental State Examination, respectively. We used structural equation modeling to examine the shared and specific effects of depression and cognitive impairment on QoL, while adjusting for sociodemographic characteristics, general medical conditions, psychotropic medications and the duration of the disorder.Depression and cognitive impairment were positively associated (r = 0.24, p 0.01) and both independently and negatively impacted on QoL (standardized β = -0.41 and β = -0.32, both p 0.01) and on each QLS quality-of-life domains, except for depression on instrumental role and cognitive impairment on interpersonal relations in the sensitivity analyses excluding respondents with any missing data. Effects of depression and cognitive impairment on QoL were not due to specific depressive symptoms or specific cognitive domains, but rather mediated through two broad dimensions representing the shared effects across all depressive symptoms and all cognitive deficits, respectively.Because of the cross-sectional design of this study, measures of association do not imply causal associations.Mechanisms underlying these two broad dimensions should be considered as important potential targets to improve quality of life of this vulnerable population.

Abstract

The study was based on an external pilot-scale membrane bioreactor (MBR) with a ceramic membrane compared to a conventional activated sludge process (ASP) plant. Both systems received their influent from domestic wastewater. The MBR produced an effluent of much better quality than the ASP in terms of total suspended solids (TSS), 5-day biological oxygen demand (BOD5) and chemical oxygen demand (COD), total phosphorus (TP) and total nitrogen (TN). Other effluent quality parameters also indicated substantial differences between the ASP and the MBR. This study leads to the conclusion that in the case of domestic wastewater, MBR treatment leads to excellent effluent quality. Hence, the replacement of ASP by MBR may be justified on the basis of the improved removal of solids, nutrients, and micropollutants. Furthermore, in terms of reuse the high quality of the treated water allows it to be reused for irrigation.

Abstract

Pericallosal lipomas are the most habitual location for an intracranial lipoma. They are fat-containing lesions arising from the interhemispheric fissure intimately related to the corpus callosum , which is often abnormal. They originate from aberrant differentiation of the persistent primitive meninx. Most Pericallosal lipomas are asymptomatic and come into clinical attention during neuroradiological investigations for other conditions. MRI is the modality of choice to characterize not only the extent of the lipoma but also the frequently associated agenesis/dysgenesis of the corpus callosum . Pericallosal lipomas can be divided into two groups: The Tubulonodular type and The curvilinear type. Curvilinear lipomas are less common than Tubulonodular. We report the clinical and radiological findings of curvilinear Pericallosal lipoma in two patients with corpus callosum abnormalities revealed incidentally during evaluation following trauma.

Abstract

Background Long-term survival with durable response remains possible in the area of targeted therapies. Discontinuation of sunitinib could improve quality of life and reduce treatment costs in metastatic renal cell carcinoma with long-term disease stabilization. We discuss a case of successful interruption of antiangiogenic therapy in a patient with persisting evidence of metastases. The discontinuation of antiangiogenic therapy seems to be an option, even in indolent oligo-metastatic renal cell carcinoma with long disease stabilization before sunitinib. This observation contributes important data to the ongoing discussion on the discontinuation of treatment with kinase inhibitors in selected patients with metastatic renal cell carcinoma. Case presentation We report a case of an 80-year-old Moroccan man treated for renal clear cell carcinoma with multiple pancreatic metastases. He was not on any other medications. He underwent active surveillance with deferred sunitinib at disease progression. He showed significant disease control on sunitinib therapy demonstrating partial response with stable disease after a total of 28 months of therapy. He experienced toxicities which were manageable with supportive care and dose adjustments. Our patient asked for a break of the sunitinib administration, and the treatment was stopped. The disease remained stable after 13 months’ discontinuation of sunitinib therapy. The patient was in excellent overall health. Conclusions All available agents for metastatic renal cell carcinoma have side effects, which may become serious in a minority of patients. Clinicians and patients must therefore carefully balance the goals of maximal efficacy with minimal toxicity. Sunitinib can be discontinued without negatively impacting outcomes in indolent disease. Further research is needed to characterize the molecular determinants of response and resistance to targeted therapy.

Abstract

Physical activity (PA) is a key element in the prevention and management of type 2 diabetes but it is now well established that participation in regular PA improves blood glucose control and can prevent or delay the type 2 diabetes along with affecting lipids, blood pressure, cardiovascular events, mortality and quality of life. Structured interventions combining PA and modest weight loss have been shown to lower type 2 diabetes risks by up to 58% in high-risk populations. Most benefits of PA on diabetes management are realized through acute and chronic improvements in insulin action, gestational diabetes mellitus, and safe and effective practices for PA with diabetes-related complication. Abstract

The Drarga treatment plant was chosen as a pilot site for treatment and reuse of domestic wastewater. It uses reed beds as a tertiary treatment, so this research was conducted to study the performance of the reeds over a period of 3 years (March 2006–March 2009), that is three growing seasons. The reeds were cut after 2 years. Knowing that the Drarga climate is semi arid, we studied the behaviour of purification by reeds over seasonal changes. The reeds were effective in removing pollution over the three growing seasons studied; the average removal of BOD5 over 3 years was 71%. The maximum removal efficiency was in winter; this is associated with high oxygenation of the water mass caused by mixing and stirring induced by the movement of the long stems of the reeds.

Resume La sarcoidose est une affection multisystemique dont la pathogenie implique la formation d’une lesion inflammatoire caracteristique, le granulome epithelioide et giganto-cellulaire. L’atteinte neurologique est l’une des rares manifestations de la sarcoidose, caracterisee par son polymorphisme clinique puisqu’elle peut toucher, a des degres variables, le systeme nerveux au niveau de tous ses etages. Le diagnostic est difficile et repose sur l’association d’un faisceau d’arguments cliniques et paracliniques evocateurs. Il s’agit principalement de l’IRM, de l’etude du liquide cerebrospinal, de l’intradermoreaction a la tuberculine, du bilan phosphocalcique et du dosage de l’enzyme de conversion. A travers 15 cas, les auteurs analysent les aspects scanographiques et en imagerie par resonance magnetique de la neurosarcoidose.

Abstract

Objectifs Decrire et illustrer les differents aspects rencontres au cours de la spondylodiscite dans sa localisation cervicale. Preciser l’apport de l’imagerie (diagnostic et interventionnelle) dans le diagnostic et la prise en charge therapeutique des spondylodiscites cervicales. Connaitre les principaux diagnostics differentiels. Materiels et methodes Etude retrospective portant sur 2 cas de spondylodiscite cervicale confirmes, explores dans notre service par IRM cervicale en sequences ponderees T1, T2 et FAT SAT, avant et apres injection de Gadolinium, en coupes axiales et sagittales. Resultats L’etude portait sur un homme et une femme. L’âge etait compris entre 39 ans et 56 ans. Dans le premier cas il s’agissait d’une spondylodiscite cervicale chez une jeune femme hemodialyse chronique, complique d’un abces des parties molles anterieurs. L’autre cas ne presentait aucun facteur de risque particulier. Il accusait des nevralgies cervicobrachiales trainantes d’exacerbation recente. Les deux cas ont ete explore par IRM cervicale et ayant beneficie d’une confirmation cytologique par ponction biopsie disco-vertebrale guidee par le scanner. Conclusion La spondylodiscite tuberculeuse represente la premiere localisation osseuse de BK. La localisation cervicale reste peu courante. Le diagnostic positif repose sur des arguments cliniques, biologique et surtout radiologiques. La biopsie scanoguidee reste un examen performant pour le diagnostic etiologique et donc une prise en charge adaptee.

Resume Observation. – Nous rapportons un cas de faux anevrisme post-traumatique de l’artere hepatique diagnostique chez un enfant de neuf ans, grâce a l’echographie doppler couleur et a l’angioscanner. L’arteriographie selective a permis un diagnostic de certitude et un traitement par embolisation arterielle. Conclusion. – Complication inhabituelle apres un traumatisme du foie chez l’enfant, le faux anevrisme post-traumatique de l’artere hepatique est actuellement diagnostique par des techniques d’imagerie non invasives. L’embolisation endovasculaire constitue le traitement de choix.

Abstract

Sandhoff disease is a rare and severe lysosomal storage disorder representing 7% of GM2 gangliosidoses. Bilateral thalamic involvement has been suggested as a diagnostic marker of Sandhoff disease. A case of an 18-month-old infant admitted for psychomotor regression and drug resistant myoclonic epilepsy is presented. Cerebral CT scan showed bilateral and symmetrical thalamic hyperdensity. MRI revealed that the thalamus was hyperintense on T1-weighted images and hypointense on T2-weighted images with a hypersignal T2 of the white matter. Enzymatic assays objectified a deficiency of both hexosaminidases A and B confirming the diagnosis of Sandhoff disease.

Abstract

Middle aortic syndrome (MAS) results from a diffuse narrowing of the distal thoracic or abdominal aorta commonly involving both the visceral and renal arteries. Congenital, acquired, inflammatory, and infectious etiologies have been described. Symptoms typically occur within the first three decades of life and include hypertension, lower extremity claudication, and mesenteric ischemia. The condition is considered a life-threatening emergency as a result of the complications associated with severe hypertension. Diagnosis is made with magnetic resonance and computed tomography angiography. Surgical bypass grafting is the optimal method of treatment and must be tailored depending on the distribution of disease. We report one case of MAS treated with thoracic aorta to abdominal aorta bypass and reimplantation of the right renal artery.
