Abstract

Background: The emergence of SARS‐CoV‐2 in late 2019 saw the implementation of public health and social measures (PHSM) by countries across Europe to reduce its transmission and impact on populations. Consequently, countries reported changes in influenza circulation and extensive disruptions to routine surveillance systems.MethodsWe describe the epidemiology of influenza in Europe between Weeks 40/2020 and 39/2022 compared to the 2016/17 to 2019/20 seasons, to assess the impact of the COVID‐19 pandemic and PHSM on surveillance systems and influenza circulation.ResultsLow detections of influenza were observed through primary care sentinel sources during seasonal influenza periods (Week 40 to 20); 56 (of 39,457 specimens tested; < 1% positivity) in 2020/21 and 7261 (of 64,153 specimens tested; 11% positivity) detections in 2021/22 were observed, compared to an average of 18,383 (of 50,544 specimens tested; 36% positivity) detections in 2016/17 to 2019/20. Similarly, 11 (of 19,989 specimens tested; < 1% positivity) and 1488 (of 23,636 specimens tested; 6% positivity) detections were reported through SARI surveillance sources in 2020/21 and 2021/22, respectively, compared to an average of 2850 (of 10,389 specimens tested; 27% positivity) detections in 2016/17 to 2019/20. However, the 2021/22 interseasonal period saw unusual increases in influenza detections across surveillance site types when PHSM were easing.ConclusionIn conclusion, findings suggest that the restriction and easing of PHSM measures were associated with variations in influenza detections. Our observations of out‐of‐season influenza activity highlight the importance of an integrated respiratory surveillance strategy to monitor circulating respiratory viruses throughout the year to inform optimal prevention and control strategies.

Abstract

The COVID-19 pandemic highlighted some potential limitations of transmission-based precautions. The distinction between transmission through large droplets vs aerosols, which have been fundamental concepts guiding infection control measures, has been questioned, leading to considerable variation in expert recommendations on transmission-based precautions for COVID-19. Furthermore, the application of elements of contact precautions, such as the use of gloves and gowns, is based on low-quality and inconclusive evidence and may have unintended consequences, such as increased incidence of healthcare-associated infections and spread of multidrug-resistant organisms. These observations indicate a need for high-quality studies to address the knowledge gaps and a need to revisit the theoretical background regarding various modes of transmission and the definitions of terms related to transmission. Further, we should examine the implications these definitions have on the following components of transmission-based precautions: (i) respiratory protection, (ii) use of gloves and gowns for the prevention of respiratory virus infections, (iii) aerosol-generating procedures and (iv) universal masking in healthcare settings as a control measure especially during seasonal epidemics. Such a review would ensure that transmission-based precautions are consistent and rationally based on available evidence, which would facilitate decision-making, guidance development and training, as well as their application in practice.

Abstract

Objectives: The Puumala virus (PUUV) is a hantavirus that causes hemorrhagic fever with renal syndrome. Studies showing an increased risk of lymphoid malignancies after hantavirus infection, together with the observation that PUUV infects B cells, motivated us to study the risk of lymphoid malignancies after PUUV infection. Methods: We linked data from the Finnish Cancer Registry and National Infectious Diseases Register for 2009-2019. We used a time-dependent Cox regression model to evaluate the hazard of the lymphoid malignancies grouped according to the HAEMACARE classification. Results: We identified 68 cases of lymphoid malignancies after PUUV infection among 16,075 PUUV-infected individuals during 61,114,826 person-years of observation. A total of 10 cases occurred within 3-<12 months and 38 within 1-<5 years after PUUV infection, and the risk of lymphoid malignancies increased with hazard ratios (HRs) of 2.0 (95% confidence interval [CI], 1.1-3.7) and 1.6 (95% CI, 1.2-2.3), respectively. The group of mature B cell neoplasms showed an increased risk 3-<12 months and 1-<5 years after PUUV infection, HR 2.2 (95% CI, 1.2-4.3) and HR 1.8 (95% CI, 1.3-2.5), respectively. Conclusion: PUUV infection is associated with lymphoid malignancies in the Finnish population, supporting the earlier studies. Further research is required to understand the pathophysiological mechanisms behind this association.

Abstract

Hoitoon liittyvien infektioiden torjunta on tärkeä osa potilas- ja asiakasturvallisuutta. Tutkimuksen tarkoituksena oli kuvata ikääntyneiden ympärivuorokautisen pitkäaikaishoidon toimintayksiköissä infektioiden torjuntaan osallistuvan henkilöstön saatavuutta sekä hygieniayhdyshenkilötoiminnan ja infektioiden torjunnan alueellisen yhteistyön toteutumista. Poikkileikkaustutkimus sisälsi kaksi erillistä kyselyaineistoa Manner-Suomessa sijaitsevista pitkäaikaishoidon toimintayksiköistä. Ensimmäinen kysely (n=167 toimintayksikköä) toteutettiin syksyllä 2017 osana Euroopan pitkäaikaishoitolaitosten hoitoon liittyvien infektioiden ja mikrobilääkkeiden käytön prevalenssitutkimusta (HALT-3). Toinen aineisto (n=41) kerättiin syksyllä 2018 ja 2019. Aineistojen analysoinnissa käytettiin kuvailevia tilastollisia menetelmiä. Toimintayksiköistä valtaosalla oli infektioiden torjuntakoulutusta saanut työntekijä käytettävissä, ja kaikilla oli mahdollisuus ulkopuoliseen konsultaatioapuun. Yksiköistä suurimmassa osassa oli vähintään yksi hygieniayhdyshenkilö, joista enemmistö oli osallistunut edeltävän vuoden aikana infektioiden torjuntakoulutukseen. Harvalla oli kirjallinen työnkuva tai varattu erikseen työaikaa infektioiden torjunnan tehtäviin. Lähes kaikki toimintayksiköt tekivät yhteistyötä alueellisten infektioasiantuntijoiden kanssa.
