Abstract

Purpose Patients receiving biologic therapy for inflammatory bowel disease (IBD) require routine laboratory monitoring to ensure the safety and efficacy of therapy. The purpose of this quality improvement project was to evaluate the implementation of a dashboard to prevent treatment gaps by prospectively identifying patients with IBD and outdated laboratory results receiving biologics. Methods We performed a pre/post analysis of dashboard implementation to assess the number of patients with overdue laboratory work resulting in treatment gaps. The dashboard combined data from the electronic health record (EHR) and pharmacy claims database to identify patients on a biologic with laboratory tests (white blood cell count, liver transaminases, C-reactive protein, and erythrocyte sedimentation rate) completed 5 or more months ago and/or a tuberculosis screen completed 11 or more months ago. After implementation, specialty pharmacists reviewed the dashboard and communicated via EHR if a new prescription and laboratory tests were needed. Messages were sent 4 weeks in advance of the next refill-eligible date. Mixed methods were used for analysis of qualitative data, including surveys, and quantitative data, assessing treatment gap length. Results A total of 40 patients who had outdated laboratory values and required a new prescription (15 before dashboard implementation and 25 after implementation) were included in the analysis. The frequency of a treatment gap decreased from 80% (n = 12) in the preimplementation phase to 32% (n = 8) in the postimplementation phase. The median gap length was shorter after dashboard implementation, decreasing from 21 days (range, 3-97 days) to 11 days (range, 2-23 days). Conclusion Utilization of a quality measures dashboard decreased treatment gaps in patients with IBD receiving biologic therapy. Integrated specialty pharmacists are uniquely positioned to monitor adherence to laboratory monitoring parameters for patients on biologics. [ABSTRACT FROM AUTHOR]

10. The response of finely textured and organic soils to lime and phosphorus application: Results from an incubation experiment.

Abstract

A soil's responses to phosphorus (P) input differs based on its chemical composition. Soil acidity and the presence of metallic cations dictate a soil's chemical composition. Currently, soil P application recommendations are universal and do not account for differing soil composition. A targeted soil‐specific approach is required to optimize P efficiency and availability. A pot incubation experiment was established to explore the effects of contrasting lime and P application rates across a range of soils (25), characterized by fine particle size and high levels of soil organic matter. Three contrasting rates of P were applied (0, 50, and 150 kg P ha−1) both with and without ground lime (CaCO3) at 5 tonne ha−1 over a 140‐day incubation period. The addition of lime buffered the soil, increasing nutrient availability and reducing P fixation. The 50 kg P ha−1 treatment rate was required to achieve sufficient plant available P in both mineral soil textural classes. Current legislative recommendations however do not allow the application of such rates, which has an impact on agronomic performance. Loam soils experienced a greater increase in M3 soil P in comparison to clay and organic soils. Organic soils posed a major threat to water quality due to poor P retention. A re‐evaluation of P recommendations is required to account for soil variability as current P allowances are insufficient on these particular soils. [ABSTRACT FROM AUTHOR]

18. A high-fidelity, low-cost curriculum for teaching perineal repair skills to maternity care learners

Abstract

Background: In 2009 the Ryerson Midwifery Education Program identified the need for a standardized sustainable curriculum to teach perineal laceration repair that provided the necessary skills and knowledge to provide learners with basic competencies to refine maximise their opportunities for skill enhancement in the clinical environment. To this end a reusable, composite simulation-program consisting of e-modules, video, instructor demonstration, and hands-on workshop was developed. In future this curriculum could be offered to other maternity care trainees, even interprofessionally.Methods: The tripartite curriculum consisted of: two narrated e-modules (theoretical and practical aspects of management and repair); a narrated video demonstrating a complete repair (added in 2012); and a 3-hour workshop involving live demonstration of repairs followed by a hands-on 4:1 preceptor supervised practice using raw 10 cm sections of incised unsliced pork back bacon (simulation pork) with the muscle section to simulate the vaginal tissue component and the fascial overlay to simulate the perineal skin component. This curriculum has been used with 484 midwifery students since 2010 at Ryerson and McMaster universities. To assess the utility of the curriculum participants completed surveys reporting their comprehension at multiple junctures: prior to the e-modules; after watching the video; and after the workshop.Results: The curriculum was rated as highly effective for teaching suturing skills and perineal laceration management. Students reported significant knowledge acquisition from the tripartite curriculum and the simulation pork perineum.Conclusions: The curriculum appears to provide a high-fidelity, low-cost, standardized method of teaching foundational skills in perineal repair to maternity care learners.

Abstract

The desire to feel positively about the self is frequently studied. However, most work has investigated these self‐enhancing behaviors as a binary choice: to enhance or not to enhance? We propose a novel framework for investigating a more nuanced decision: with a seemingly endless supply of self‐enhancing behaviors, how do people determine engagement in a self‐enhancing behavior among their options? We frame the perceived self‐esteem boost of a strategy is just one antecedent of the probability of acting on a given enhancing behavior. We present a conceptual framework that represents engagement with a strategy based on a weighting of the perceived costs and benefits to other motivations. We then provide examples of how the framework functions regarding commonly studied motivations (e.g., consistency, belonging). Next, we detail how our framework can predict self‐enhancement strategies (e.g., self‐promotion, comparison, association) typically observed in the literature. We also discuss how situations and individual differences may work in the context of this framework. Abstract

Although self-promotion may be the most direct way people self-present, it carries social costs. We propose a novel phenomenon – promotion by others – wherein social networks may afford similar advantages with fewer costs. We utilized egocentric network analysis to examine relationships between social connections and perceived promoter potential (i.e., likelihood a friend will tell others about successes; PPP) and relationship dynamics. Participants enumerated friends and reported perceptions these friends would promote them, were valuable, and the extent to which they wanted these friends to know about successes. PPP was positively related to (a) network connectedness, (b) relational value, and (c) desire to know about success. Abstract

Objective: We disentangled three growth-relevant concepts (redemption, self-improvement, and eudaimonic growth) in personal narratives of high, low, and turning points and tested their relations to well-being.Method: In two studies, participants (Study 1 n = 111, Study 2 n = 206; overall ages 17-83, 56% women, 75% white) wrote narratives of high points, low points, and turning points. Researchers coded each narrative for redemption sequences (i.e., affectively valenced changes in life from bad to good), self-improvement sequences (i.e., affectively valenced changes in oneself for the better), and themes of eudaimonic growth (i.e., values or motives for cultivating meaningful activities or relationships, helping others, or wisdom). Participants also self-reported well-being.Results: Redemption sequences in low points predicted higher well-being but in high points predicted lower well-being. Self-improvement sequences and growth themes each predicted higher well-being in each life event (and interacted in high points). Growth themes consistently mediated predicted relations between both redemption and self-improvement sequences and well-being. Findings held when controlling for global narrative affect, self-reported growth motivation, and big-five traits.Conclusions: Thematic motives for eudaimonic growth were more closely tied to well-being than were affective evaluations of either changes from bad to good (redemption) or one's becoming better (self-improvement). 27. A monastic landscape: The Cistercians in medieval Leinster

Abstract

This study endeavours to discuss the Cistercian monasteries of Leinster with regard to their physical location in the landscape, the agricultural contribution of the monks to the broader social and economic world and the interaction between the cloistered monks and the secular world. The various lands granted to the Leinster houses are identified, discussed, and where possible mapped. This theme is the subject of three different chapters. The lands at foundation are examined then the additional grants and privileges included. The seventh chapter investigates the dissolution of the religious houses. The extents of the monastic possessions that were drawn up at this time permitted a detailed examination of the lands held by the Leinster houses. This chapter also provides a template for a study of the physical, economic and agricultural state of the monasteries. The main overall findings of the thesis relate to the uses and makeup of the Cistercian lands in addition to the situation of these monasteries in the landscape of Leinster. All monasteries of the order in Leinster were found to be situated in close proximity to road and other route ways in addition to territorial and physical boundaries. At dissolution the land of the Cistercians was overwhelmingly under arable cultivation with a figure of 84% arrived at which compares to 72% for lands outside Leinster and 82% for all monasteries recorded in the extents. On the whole the monasteries of Leinster were found to have been in a good state of repair and were still in production in the mid sixteenth century. It is also stated that the total acreage of the Cistercians in medieval Ireland was in excess of 500,000 statute acres. Perhaps significantly the situation with regard to the Cistercian order post dissolution was discussed in the final chapter. In Experiment 1 and Experiment 2, respectively, eighty-five subjects and sixty subjects viewed nine facial caricatures for 1/150 sec per face, nine for 1/100 sec per face, and nine for 1/10 sec per face. Recognition memory for three of the 1/150, three of the 1/100, and three of the 1/10 sec faces was tested before hypnosis; memory for nine more faces was tested during hypnosis; memory for the last nine faces was tested after hypnosis. Before hypnosis, the recognition probabilities for 1/150 sec faces and 1/100 sec faces were at chance levels, and the recognition probability for 1/10 sec faces was better than chance. During hypnosis, recognition memory for 1/150 sec faces was again at the chance level. However, hypnotic recognition of 1/100 sec faces was better than chance, and hypnotic recognition of 1/10 sec faces was no better than chance. Consistent both with cognitive-state monitoring theory and with state-dependent theories of memory, these results indicate that memory for subliminally perceived targets (1/100 sec faces) is facilitated by the hypnotic state.

48. Do Autonomous Individuals Strive for Self Positivity? A Test of the Universality of Self-Enhancement

Abstract

The present research explored the self-enhancement strategies autonomous individuals use and tried to determine which strategies are best for psychological well-being. We examined whether autonomous individuals self-enhance through seeking out positive information about the self, rather than avoiding negative information. The former was hypothesized to be positively associated with well-being while the latter was not. Previous research suggests that individuals high in autonomy and low in controlledness do not self-enhance; however, recent work suggests that self-enhancement is a universal human motivation. In Study 1, to examine patterns of self-enhancement at varying levels of autonomy and controlledness, participants (N = 175) completed the Self-Enhancement and Self-Protection Strategies scale (SSS; Hepper, Gramzow, & Sedikides, 2010) and the General Causality Orientation Scale (GCOS; Deci & Ryan, 1985a). Results suggested autonomy predicted engaging in approach—but not avoidance—oriented self-enhancement strategies, while controlledness was associated with both approach and avoidance oriented strategies. In Study 2, to examine patterns of self-enhancement, autonomy, and well-being, 378 participants completed baseline measures of autonomy, self-enhancement, and well-being at Time 1. At Time 2, participants (N = 105) completed impossible mazes, were given false negative feedback, engaged in a self-enhancement coping strategy either congruent or incongruent with their level of autonomy, and finally completed the same battery of well-being measures as in Time 1. Results suggest there is no support for the hypothesis; while controlling for controlledness and Time 1 well-being, there was no effect of self-enhancement strategy on well-being nor did the effect of self-enhancement strategy on well-being vary by one’s level of autonomy or self-enhancement strategy.
