Abstract

Watershed management and hydrological modeling require data related to the very important matter of precipitation, often measured using raingages or weather stations. Hydrological models often require a preliminary spatial interpolation as part of the modeling process. The success of spatial interpolation varies according to the type of model chosen, its mode of geographical management and the resolution used. The quality of a result is determined by the quality of the continuous spatial rainfall, which ensues from the interpolation method used. The objective of this article is to review the existing methods for interpolation of rainfall data that are usually required in hydrological modeling. We review the basis for the application of certain common methods and geostatistical approaches used in interpolation of rainfall. Previous studies have highlighted the need for new research to investigate ways of improving the quality of rainfall data and ultimately, the quality of hydrological modeling.

Abstract

A heart transplant is considered the gold standard treatment for end-stage heart failure patients, however the operation is reliant on organ donors and poses several clinical and economical risk factors. Tissue engineering has shown promise in providing a future alternative for organ transplants. The potential for mimicking native healthy tissue has been brought closer to realisation with 3D bioprinting. The method allows for precise deposition of cellular material into biomaterials suitable for transplantation. By culturing the cellular material using patient stem cells, the 3D bioprinted transplant can be personalised for each patient. This book chapter presents the current state of 3D bioprinting applied in developing personalised cardiac patches. An overview of cardiac anatomy and the impact of myocardial infarction is followed by a comparison of 3D bioprinting methods, commonly used biomaterials, and the characteristics of cells and cellular spheroids. This is followed by a review of findings from animal studies using 3D bioprinted cardiac patches to date and highlights the challenges of translating the method for clinical studies. Biological and design considerations for developing personalised cardiac patches conclude the review leading to the discussion of the chapter findings.

Abstract

Background: Tuberculosis diagnosis might be delayed or missed in children with severe pneumonia because this diagnosis is usually only considered in cases of prolonged symptoms or antibiotic failure. Systematic tuberculosis detection at hospital admission could increase case detection and reduce mortality. Methods: We did a stepped-wedge cluster-randomised trial in 16 hospitals from six countries (Cambodia, Cameroon, Côte d'Ivoire, Mozambique, Uganda, and Zambia) with high incidence of tuberculosis. Children younger than 5 years with WHO-defined severe pneumonia received either the standard of care (control group) or standard of care plus Xpert MTB/RIF Ultra (Xpert Ultra; Cepheid, Sunnyvale, CA, USA) on nasopharyngeal aspirate and stool samples (intervention group). Clusters (hospitals) were progressively switched from control to intervention at 5-week intervals, using a computer-generated random sequence, stratified on incidence rate of tuberculosis at country level, and masked to teams until 5 weeks before switch. We assessed the effect of the intervention on primary (12-week all-cause mortality) and secondary (including tuberculosis diagnosis) outcomes, using generalised linear mixed models. The primary analysis was by intention to treat. We described outcomes in children with severe acute malnutrition in a post hoc analysis. This study is registered with ClinicalTrials.gov (NCT03831906) and the Pan African Clinical Trial Registry (PACTR202101615120643). Findings: From March 21, 2019, to March 30, 2021, we enrolled 1401 children in the control group and 1169 children in the intervention group. In the intervention group, 1140 (97·5%) children had nasopharyngeal aspirates and 942 (80·6%) had their stool collected; 24 (2·1%) had positive Xpert Ultra. At 12 weeks, 110 (7·9%) children in the control group and 91 (7·8%) children in the intervention group had died (adjusted odds ratio [OR] 0·986, 95% CI 0·597–1·630, p=0·957), and 74 (5·3%) children in the control group

40. Progression to Severe Hypernatremia in Hospitalized General Medicine Inpatients: An Observational Study of Hospital-Acquired Hypernatremia.

Abstract

Background and objectives: Hypernatremia can be community or hospital-acquired, and there may be specific factors unique to the hospital environment, such as intravenous fluid treatment, which contribute to hypernatremia. The aim of this study was to determine the factors associated with the progression from moderate to severe hospital-acquired hypernatremia among patients admitted under general medicine. Material(s) and Method(s): In this retrospective, single-center cohort study (2012 to 2017), we used ICD-10 coding and medical records to identify adult patients who developed moderate hypernatremia and followed them for progression to severe hypernatremia. We profiled the serum biochemistry and the volume and composition of prescribed intravenous fluids. We applied logistic regression to determine the factors associated with the progression to severe hypernatremia, using the patients with moderate hypernatremia as reference. Result(s): Of the 180 medical inpatients (median age of 81 years) with moderate hospital-acquired hypernatremia, 9.4% progressed to severe hypernatremia. Normal saline comprised 76% of intravenous fluid volume administered prior to onset of moderate hypernatremia. After the onset, 38% of fluid volume prescribed remained normal saline. The factors independently associated with progression to severe hypernatremia included chronic kidney disease stage (odds ratio 2.38, 95% CI: 1.26-4.50, P = 0.008) and serum creatinine increase (per 10 micromol/L, OR 1.29, 95% CI: 1.07-1.57, P = 0.009). Conclusion(s): Patients with chronic kidney disease and acute kidney injury may have an increased risk of severe hospital-acquired hypernatremia.

41. Journey mapping as a novel approach to healthcare: a qualitative mixed methods study in palliative care.

Abstract

BACKGROUND: Journey mapping involves the creation of visual narrative timelines depicting the multidimensional relationship between a consumer and a service. The use of journey maps in medical research is a novel and innovative approach to understanding patient healthcare encounters. OBJECTIVE(S): To determine possible applications of journey mapping in medical research and the clinical setting. Specialist palliative care services were selected as the model to evaluate this paradigm, as there are numerous evidence gaps and inconsistencies in the delivery of care that may be addressed using this tool. METHOD(S): A purposive convenience sample of specialist palliative care providers from the Supportive and Palliative Care unit of a major Australian tertiary health service were invited to evaluate journey maps illustrating the final year of life of inpatient palliative care patients. Sixteen maps were purposively selected from a sample of 104 consecutive patients. This study utilised a qualitative mixed-methods approach, incorporating a modified Delphi technique and thematic analysis in an online questionnaire. RESULT(S): Our thematic and Delphi analyses were congruent, with consensus findings consistent with emerging themes. Journey maps provided a holistic patient-centred perspective of care that characterised healthcare interactions within a longitudinal trajectory. Through these journey maps, participants were able to identify barriers to effective palliative care and opportunities to improve care delivery by observing patterns of patient function and healthcare encounters over multiple settings. CONCLUSION(S): This unique qualitative study noted many promising applications of the journey mapping suitable for extrapolation outside of the palliative care setting as a review and audit tool, or a mechanism for providing proactive patient-centred care. This study presents, first of all, an interpretation of a regional Bouguer gravity map (14 degrees W-9 degrees W, 11N-16 degrees N) of the KKI and then introduces a new litho-structural map of the Malian part of the KID using aeromagnetic data (12 degrees 0'W-10 degrees 80'W, 12 degrees 0'N-14 degrees 50'N). The KKI is limited to the west by a negative gravity anomaly forming a 30-100 km wide corridor, oriented N-S to NNE-SSW, and correlated to the Variscan Mauritanides orogenic belt. West of this belt, the Mesozoic to Cenozoic sedimentary deposits are marked by the highest positive anomaly of the region, attributed either to the presence of mafic rocks at intermediate to deep crustal levels or to a shallower Moho depth. Within the KKI, moderately positive to negative anomalies are correlated with exposed plutonic and mafic to intermediate metavolcanic rocks. Gravity data also reveal (i) two north-west trending lineaments attributed to crustal-scale shear zones, (ii) three north-east trending lineaments marked by gravity anomalies sub-parallel to the known Bissau-Kidira-Kayes Fault Zone, and named Kayes, Kedougou-Kenieba, and South Shear Zones, respectively, (iii) and the extension to the south of the known Mauritanides Belt Thrust. The local aeromagnetic map of the Malian part of the KKI discriminates the Mako metavolcanic belt with granitic intrusions in the west characterized by heterogeneous anomalies, from the Kofi metasedimentary series delineated by magnetic lows in the east. ESE-WNW to ENE-WSW-trending alternation of magnetic anomalies in the metavolcanic and metasedimentary rocks are attributed to pervasive structures associated with ductile deformation. The Senegalo-Malian Shear Zone is marked at the regional scale by deflection of the geophysical lineaments but is not expressed by any offset of identified structures or lithological contacts.
