1. Anakinra in Sanfilippo syndrome: a phase 1/2 trial.

Abstract

Sanfilippo syndrome is a fatal childhood neurodegenerative disorder involving neuroinflammation among multiple pathologies. We hypothesized that anakinra, a recombinant interleukin-1 receptor antagonist, could improve neurobehavioral and functional symptoms owing to its capacity to treat neuroinflammation. This phase 1/2 trial aimed to test the safety, tolerability and effects of anakinra on neurobehavioral, functional and quality-of-life outcomes in patients and their caregivers. The primary outcome was the percent of participants requiring a dose increase at week 8 or week 16. Secondary efficacy outcomes included a multi-domain responder index (MDRI). Twenty-three participants (6-26 years of age) were enrolled. Twenty continued treatment to week 8, and 15 (75%) required an increased dose at week 8 or week 16. There was an improvement in at least one domain in the MDRI in 18 of 21 (86%) at week 8 and in 15 of 16 (94%) at week 36. Seven participants withdrew (intolerability of daily injections and lost to follow-up) before week 36. Adverse events occurred in 22 of 23 (96%) participants, most commonly mild injection site reactions. No serious adverse events were related to anakinra. In conclusion, anakinra was safe and associated with improved neurobehavioral and functional outcomes, supporting continued investigation of anakinra in Sanfilippo syndrome and other mucopolysaccharidoses. Abstract

Hemogenic endothelium (HE) undergoes endothelial-to-hematopoietic transition (EHT) to generate blood, a process that requires progressive down-regulation of endothelial genes and induction of hematopoietic ones. Previously, we have shown that the transcription factor HoxA3 prevents blood formation by inhibiting Runx1 expression, maintaining endothelial gene expression and thus blocking EHT. In the present study, we show that HoxA3 also prevents blood formation by inhibiting Notch pathway. HoxA3 induced upregulation of Jag1 ligand in endothelial cells, which led to cis-inhibition of the Notch pathway, rendering the HE nonresponsive to Notch signals. While Notch activation alone was insufficient to promote blood formation in the presence of HoxA3, activation of Notch or downregulation of Jag1 resulted in a loss of the endothelial phenotype which is a prerequisite for EHT. Taken together, these results demonstrate that Notch pathway activation is necessary to downregulate endothelial markers during EHT.

Abstract

The concentrations of high- and low-density lipoprotein cholesterol and triglycerides are influenced by smoking, but it is unknown whether genetic associations with lipids may be modified by smoking. We conducted a multi-ancestry genome-wide gene-smoking interaction study in 133,805 individuals with follow-up in an additional 253,467 individuals. Combined meta-analyses identified 13 novel loci, some of which were detected only because the association differed by smoking status. Additionally, we demonstrated the importance of including diverse populations, particularly in studies of interactions with lifestyle factors, where genomic and lifestyle differences by ancestry may contribute to novel findings., Editorial summary: A multi-ancestry genome-wide gene-smoking interaction study identifies 13 new loci associated with serum lipids.

21. Alcohol-free red wine enhances plasma antioxidant capacity in humans

Abstract

Moderate wine consumption is reputed to exert a protective effect against coronary heart disease (CHD). The nature of the protective compounds is unclear and the mechanisms are incompletely understood. We studied whether the nonalcoholic component of wine increases plasma antioxidant capacity measured as total radical-trapping antioxidant parameter (TRAP), and whether such an effect is associated with the presence of phenolic compounds in plasma. The TRAP and plasma levels of phenolic compounds were measured in 10 healthy subjects after the ingestion of 113 mL of tap water (control) and alcohol-free red and white wine at 1 -wk intervals. Both alcohol-free wines possessed an in vitro dose-dependent peroxyl-radical activity, but red wine, with a polyphenol concentration of 363 [+ or -] 48.0 mg/L quercetin equivalent (QE), was 20 times more active (40.0 [+ or -] 0.1 mmol/L) than white wine (1.9 [+ or -] 0.1 mmol/L), which has a polyphenol concentration of 31 [+ or -] 1 mg QE/L. The ingestion of alcohol-free red wine caused significant increases in plasma TRAP values and polyphenol concentrations 50 min after ingestion. Alcohol-free white wine and water had no effects on either of the plasma values. The parallel and prompt increase of antioxidant status and of circulating levels of polyphenols in fasting subjects after bolus ingestion of a moderate amount of alcohol-free red wine suggests that polyphenols are absorbed in the upper gastrointestinal tract and might be directly involved in the in vivo antioxidant defenses. Abstract

Featured Application: An intelligent active actuation system for intelligent treatment of Helicobacter pylori with light. An endoscopic capsule is a miniaturized ingestible video camera used to acquire images of the gastrointestinal tract wirelessly. Being morphologically equivalent to any ingestible pill, they can be simply swallowed. Endoscopic capsules therefore present an inviting alternative to the traditional endoscope for the examination of the gastrointestinal tract as well for therapeutic purposes. Endoscopic capsules are considered a disruptive technology, as they have revolutionized the examination of the gastrointestinal tract in a relatively short time. The implementation of an active locomotion system can improve the performance of a capsule and, in the solution proposed in this paper, allows providing the capsule the needed power for therapeutic purposes. Alternative therapeutic solutions, based on optical solutions and capsule endoscopy can be applied to patients affected by Helicobacter pylori, a bacterium of the stomach that affects about half of the world population, mainly in developing countries. The infection can be asymptomatic or associated with slight symptomatology. In some cases, it can take to major pathologies or death. The literature reports results deriving from recent applications of photodynamic treatments to H. pylori. Specific wavelengths have been found to exhibit photo-killing capabilities toward the bacterium. Some solutions have been proposed based on the use of endoscopic devices and capsules capable of administering photodynamic therapy inside the stomach. The proposed treatments, however, are invasive and insufficient to achieve long-term eradication. In this work, the administration of photodynamic therapy is proposed, aimed at the eradication of H. pylori by means of an active endoscopic capsule with LED emission. The capsule design, in addition to the therapeutic module aimed at administering an appropriate light intensity at specific wavelengths already demonstrated in the literature, integrates an active locomotion system aimed at maximizing the efficiency of the treatment. Abstract

Recent studies reveal that coronary heart disease is fast becoming a concern not only of well-developed, industrialized countries, but of developing countries as well. Coronary heart disease results from intake of too much fat and cholesterol. As the population grows in the urban areas in most Third World countries, dietary habits of city-dwellers alter. An urbanized diet is usually rich in fat and low in carbohydrates, increasing the risk of a heart disease. Although the rate of coronary heart disease is still low in Third World countries, health risks display a trend similar to that in western countries.

Abstract

The purpose of this study was to investigate whether weight-stable chronically energy-deficient subjects exhibit evidence of metabolic adaptation and to establish whether international predictive equations overestimate the basal metabolic rate (BMR) of tropical populations. BMR, body weight, height, and fat-free mass (FFM) by underwater weighing were measured in healthy, physically active urban dwellers of low socioeconomic status (178 men and women aged 22-38 y) in Bangalore, Southern India. Subjects were selected on the basis of body mass index (BMI; in kg/[m.sup.2]) and classified in three groups: severely undernourished (BMI [is less than] 17.0; n = 30 men, n = 25 women), marginally undernourished (BMI = 17.0-18.5; n = 31 men, n = 30 women), and well nourished (BMI [is greater than] 18.5; n = 27 men, n = 35 women). The BMR of the well-nourished group, expressed in absolute terms (6.20 and 5.18 MJ/d for men and women, respectively), was significantly higher (P [is less than] 0.000) than that of the severely undernourished group (5.72 and 4.64 MJ/d for men and women, respectively). Normalizing BMR for either body weight or FFM by analysis of covariance abolished all differences. The mean BMR of the low-BMI study group was substantially higher (11-14%) than reported previously for undernourished Indian adults. The BMR of both men and women, regardless of their nutritional status, was accurately estimated by age- and sex-specific FAO/WHO/UNU equations. These findings suggest the absence of an enhanced metabolic response in weight-stable chronically undernourished adults. This is in contrast with earlier reports, and supports more recent views. We conducted a multi-ancestry genome-wide gene-smoking interaction study in 133,805 individuals with follow-up in an additional 253,467 individuals. Combined meta-analyses identified 13 new loci associated with lipids, some of which were detected only because association differed by smoking status. Additionally, we demonstrate the importance of including diverse populations, particularly in studies of interactions with lifestyle factors, where genomic and lifestyle differences by ancestry may contribute to novel findings.

Abstract

We present a food pyramid that reflects Mediterranean dietary traditions, which historically have been associated with good health. This Mediterranean diet pyramid is based on food patterns typical of Crete, much of the rest of Greece, and southern Italy in the early 1960s, where adult life expectancy was among the highest in the world and rates of coronary heart disease, certain cancers, and other diet-related chronic diseases were among the lowest. Work in the field or kitchen resulted in a lifestyle that included regular physical activity and was associated with low rates of obesity. The diet is characterized by abundant plant foods (fruit, vegetables, breads, other forms of cereals, potatoes, beans, nuts, and seeds), fresh fruit as the typical daily dessert, olive oil as the principal source of fat, dairy products (principally cheese and yogurt), and fish and poultry consumed in low to moderate amounts, zero to four eggs consumed weekly, red meat consumed in low amounts, and wine consumed in low to moderate amounts, normally with meals. This diet is low in saturated fat ([less than or equal to] 7-8% of energy), with total fat ranging from 35% of energy throughout the region. The pyramid describes a dietary pattern that is attractive for its famous palatability as well as for its health benefits.

Abstract

Nutrition scientists have theorized that people who do not obtain enough energy from the diet may adapt to the reduced calorie intake. In situations of marginal energy intake, metabolism may slow down and the individual may become less active; decreased metabolic rate and decreased activity conserve energy. Throughout the Third World, food supplies are often inadequate and unreliable, particularly in agricultural societies which have a season of harvest and a season of scarcity. A review of the literature concludes that the most effective means of adapting to low energy intake is by reducing physical activity. But this can have deleterious consequences; when the individual becomes less active, food deprivation may worsen. In farming communities, intense physical labor is necessary to generate the food supply, and in urban areas, inactivity may lead to reduced income from employment. Food deprivation causes weight loss and a reduction in the metabolic rate, the rate at which the body burns calories. The latter outcome may be beneficial in that it conserves energy and protects the individual against further weight loss. A critical issue is whether this process represents adaptation, the successful adjustment to less than ideal nutrition, or malnutrition. Adaptation is viewed as having no adverse effect on the individual whereas malnutrition is linked to fatigue, illness and other negative consequences. The policy implications are extensive; if it is concluded that people in developing countries adapt to marginal energy intake, then less economic and food aid can be justified. One expert panel calculated the energy requirement of people in developing countries in two ways, one by the standard method and the other for the adapted state, which made a lower intake acceptable. This is similar to lowering the poverty line, because it shifts large numbers of people out of a risk category and into the mainstream. Au début du XXe siècle, le cinéma investit très largement le champ culturel ; se mêlant à la littérature, il produit des hybrides artistiques, dans une tendance qui se poursuivra tout au long du siècle. La dimension multimédia du cinéma trouve avec Cabiria, film muet de 1914 réalisé par Pastrone et écrit par D’Annunzio, son actualisation la plus réussie. Le film se présente comme une synthèse et une réécriture de topoi littéraires hérités en premier lieu du Flaubert de Salammbô, mais aussi d’auteurs comme Salgari dans Cartagine in fiamme, de peintres comme Sargent ou de musiciens comme Reyer, également influencés par le roman français. Cette étude se propose de retracer les étapes de la réalisation de ce film kolossal, en s’attachant aux motifs récurrents de l’œuvre cinématographique et en mettant en relief les analogies et les différences avec l’œuvre fondatrice de Flaubert. In the early 20th century cinema considerably pervaded the cultural field, and mingling with literature it created artistic hybrids, a trend that would continue throughout the century. With Cabiria, a silent movie directed by Pastrone and written by D’Annunzio, the multimedia dimension of film found its most successful expression. The movie is a synthesis and a rewriting of literary topoi inherited primarily from Flaubert’s Salammbô, but also from other writers such as Salgari and his Cartagine in fiamme, and of painters such as Sargent or musicians like Reyer, themselves influenced by the French novel. This article will go back over the stages of this colossal movie’s making, by studying the film’s recurrent motifs and highlighting the analogies and differences with Flaubert’s founding work. Nei primi anni del Novecento la novità del cinematografo investe prepotentemente il costume sociale e culturale dell’epoca e anche la letteratura irrompe nel grande schermo producendo ibridi artistici destinati a segnare una tendenza che durerà per tutto il secolo. La vocazione intermediale della settima arte trova in Cabiria, il film muto del 1914 frutto della potenza creativa del regista Pastrone e del poeta D’Annunzio, la sua più compiuta formulazione. La produzione si configura come sintesi e rielaborazione di topoi letterari trattati in primis da Flaubert, creatore del personaggio Salammbô, e successivamente da autori come il Salgari della Cartagine in Fiamme, pittori come Sargent o musicisti come Reyer che a lui si sono ispirati. In questo studio si è inteso ripercorrere le vicende che hanno accompagnato la realizzazione del kolossal prendendo in considerazione i motivi ricorrenti nell’opera filmica, eventuali analogie e differenze con l’opera originale di Flaubert, con la quale condivide le caratteristiche atmosfere, il gusto per una certa spettacolarità e alcuni aspetti diegetici.

Abstract

Marked seasonal variability of both production- and consumption is characteristic of virtually all farming systems in the developing world. This study examines the magnitude and significance of seasonal undernutrition in south central Ethiopia, southern Shewa and Zigwa Boto, a peasant association in the Gurage Zone. The study seeks to answer five questions: (1)Does seasonal energy stress affect individuals of various age groups and sexes differently? (2)Do members of the same household show divergent responses to seasonal energy stress? (3)What are the functional consequences of different levels of adult undernutrition? (4)Are the current anthropometric cut-off points for adults appropriate for rural Ethiopia? (5)What household characteristics are associated with vulnerability to seasonal undernutrition? A number of important findings emerge from this research.....The study clarifies some points of contention in the field of adult undernutrition and shows how seasonal undernutrition operates as an intermittent warning signal, reminding us not to miss opportunities to promote good nutrition throughout the life cycle.
