Fortification of condiments or seasonings may be useful for delivering micronutrients if they are consumed consistently by most of the population, as occurs in many countries. The World Health Organization, in collaboration with the Micronutrient Initiative and the Sackler Institute for Nutrition Science at the New York Academy of Sciences, convened a technical consultation on "Fortification of Condiments and Seasonings with Vitamins and Minerals in Public Health: from Proof of Concept to Scaling Up" to review the role of condiments and seasonings in improving micronutrient status, as constituents of regular diets and patterns of production and consumption worldwide. The consultation covered aspects related to implementation, monitoring, evaluation, and legal frameworks of fortification programs, as well as food safety and policy coherence for condiment fortification in the context of other public health strategies. This paper introduces the background and rationale of the technical consultation, synopsizes the presentations, and provides a summary of the main considerations proposed by the working groups.

Condiments and seasonings have been considered as potential vehicles for fortification in place of, or in addition to, fortifiable staple foods. Methodologies for establishing fortification programs focus primarily on use of staple foods, which are consumed in larger portions than condiments and seasonings. Some fortification models assume self-limiting consumption relative to the maximum energy consumed by target populations. However, this assumption may prove incorrect for estimating fortification concentrations of condiments and seasonings because they may only provide negligible energy. Although flavor or color may limit consumption, these limits would vary across each condiment or seasoning vehicle. In addition, the small volume of condiments and seasonings consumed relative to staple foods can lead to proportionally larger potential errors than with staple foods when measuring usual dietary intakes for establishing safe and effective fortification concentrations. This paper reviews available methods for setting fortification levels, whether or how available methods or conceptual frameworks could be adapted to condiments and seasonings, and gaps in knowledge for appropriately using condiments and seasonings as vehicles for fortification in public health.

Objective: To evaluate changes in weight and body fat of Mexican adolescent compared to those of adult lactating women from the Northwest (NM) and Central (CM) regions of Mexico in the first trimester postpartum. Methods: A prospective design was used to evaluate 41 exclusively breastfeeding women (21 adolescents and 20 adults) recruited 1–2 days after delivering a healthy singleton at the Hospital Infantil del Estado de Sonora (Northwest Mexico) and Instituto Nacional de Perinatologia, in Mexico City. Socioeconomic status, height, body weight and composition, dietary intake, physical activity, and milk volume (quantified by deuterium dilution method) were measured at the 1st and 3rd month postpartum. Results: Women did not lose weight throughout the first trimester postpartum. Mean postpartum weight retention at the end of the study was 3.8 kg. No differences in weight and body mass index (BMI) were found between adolescent and adult women in both periods. Energy and macronutrient intakes, physical activity and milk volume were similar between the two groups of women. En el campo de la salud las revisiones sistemáticas y el metaanálisis (MA) han cobrado auge, ya que permiten congregar estudios de características similares y generar indicadores que describan el riesgo o el beneficio de intervenciones clínicas asociadas a la presencia de un problema de salud. Para interpretar el MA y darle su justa dimensión, es necesario tomar en cuenta la racionalidad del marco teórico que lo sustenta, sus criterios metodológicos y la posible relación causal entre exposición y evento, además del contextualizar la información. Actualmente, un gran reto constituye el análisis y la síntesis de la mayor cantidad de información para tomar decisiones de manera rápida y asertiva Este artículo hace un breve recorrido por la racionalidad científica y su aplicación en la teoría causal en el marco de la epidemiología, para sentar los cimientos que permitan evaluar la pertinencia y validez de las decisiones que se tomen con base en estos análisis.Currently, the challenge is to analyze and synthesize as much information as possible in order to make quick, correct decisions. Systematic reviews and meta-analysis have quickly arisen in the health field because they allow researchers to congregate studies of similar characteristics to generate estimators that describe the risk or benefit of practices related to health problems. To understand and attach the appropriate importance to meta-analyses, it is necessary to consider the rationale of the theoretical framework, the methodological criteria, and the possible causal relationship between exposure and outcome, besides contextualizing the information. This paper briefly explores scientific rationality and its application in causal theory within an epidemiological framework, to set the basis that allows decision-makers and health professionals to evaluate the appropriateness and validity of conclusions derived from this type of analyses.

Objective. To describe the prevalence of iron deficiency anemia in the past 66 years among Mexican women on reproductive age, and to analyze the efficacy of interventions implemented for its prevention and control. Material and Methods. Observational and clinical epidemiological studies as well as federal intervention programs published between 1939 and 2005 having original data related to anemia were reviewed using electronic databases (Medline and Artemisa) and manual searches. Keywords were anemia, iron deficiency, women, and Mexico. The quality of clinical trials was evaluated using the Jadad scale. Anemia prevalence tendency was analyzed using a linear regression weighted according to the sample size. Results. Forty-six research studies were included, nine corresponded to clinical trials and four to federal-operative programs. Seventeen papers reported anemia prevalence in non-pregnant women and twenty-three in pregnant women. In the first group, weighted anemia prevalence had decreased from 39.6 to 15.5%, whereas in pregnant women it decreased from 35 to 25%. Of the clinical trials, 55% were considered to be good quality studies. Conclusions. Anemia in women on reproductive age, particularly in pregnant women, is still a public health problem in Mexico. If the current conditons continue, it would take 57 years to eradicate anemia in non-pregnant and 121 years in pregnant women. It is necessary to evaluate intervention policies and conduct consistent studies in order to enable appropriate steps to be taken to control anemia.

The aim of this study was to evaluate the association between the risk of abnormal eating behaviors (AEB) and vitamin and mineral deficiencies among women. Women of childbearing age (n = 282) were systematically sampled with a random start (21.9% adolescents) in 6 suburbs in the west side of Mexico City, they were non pregnant or breastfeeding. Vitamin A, C, E, B12, folic acid, hemoglobin, ferritin, cupper, iron and zinc concentrations were measured. A questionnaire validated in the Mexican population was used for screening AEB. Data were analyzed by descriptive statistics and by using Fisher's test. Approximately 68% of the sample belonged to a mid-low or lower socioeconomic status. 14% had risk of AEB, without statistical differences between adults and teenagers. 10% used diuretics or laxatives to reduce weight within the trimester preceding the survey. Vitamin E, zinc and iron were the most widespread deficiencies affecting 47%, 44% and 27% of the population, respectively. There was no association between the AEB and micronutrient deficiencies neither when AEB were analyzed globally nor individually. Considering these results and the high prevalence of the AEB and overweight in this population, it is important to promote the adoption or healthy behaviors to achieve an adequate weight.

Enzymatically modified soy proteins have the amino acid profile and functional properties required for dietary support. The objective of this study was to evaluate the nutritional and technological properties of an enzymatically modified soy protein ultrafiltered fraction with bound methionine (F(1-10)E) to be used as a protein ingredient for infant enteral formulas. F(1-10)E was chemically characterized and biologically evaluated. Thirty-six weaning Wistar rats were fed during 3 weeks with a 4% casein-containing diet. Rats were divided into three groups and recovered for 3 weeks with 18% protein-containing diets based on: (1) F(1-10)E, (2) casein or (3) soy isolate+methionine. Nutritional indicators were weight gain, protein efficiency ratio, plasma proteins, apparent digestibility and protein in the carcass. Additionally, F(1-10)E was added as a protein ingredient of an enteral formula, and its sensory and rheological properties were compared with a hydrolyzed-whey protein commercial formula. F(1-10)E contained 68% protein and 5% sulphur amino acids, with 60% of peptides/=6 kDa. Casein-based and F(1-10)E-based diets were comparable (P0.05) in weight gain (108 g and 118 g, respectively), protein efficiency ratio (2.7), apparent digestibility (93% and 95%), plasma proteins (5.7 mg/100 ml) and carcass protein (61%), and better than soy isolate-based+methionine diet (P0.05). Viscosity of the commercial formula and our formula was similar during a 24-h period. Sensory acceptability was 8 for our formula and 3.5 for the commercial one, on a scale of 1-10 (P0.05). Due to its nutritional, sensorial and rheological properties, F(1-10)E could be used as a protein source in infant enteral formulas.

Undernutrition and micronutrient deficiencies are still a public health problem in Latin America and the Caribbean (LAC), and overweight and obesity have reached epidemic proportions. To assess the nutrition landscape in LAC countries and guide future nutrition efforts and investments, the Pan American Health Organization and the Micronutrient Initiative joined efforts to 1) identify information gaps and describe the current nutritional situation in the region; 2) map existing policies to address malnutrition in Latin America; 3) describe the impact of conditional cash transfer programs (CCTs) on nutrition and health outcomes; and 4) identify the challenges and opportunities to address malnutrition in the region. This article summarizes the methods and key findings from that research and describes the current challenges and opportunities in addressing malnutrition in the LAC region. LAC countries have advanced in reducing undernutrition and micronutrient deficiencies, but important gaps in information are a major concern. These countries have policies to address undernutrition and micronutrient deficiencies, but comprehensive and intersectoral policies to tackle obesity are lacking. CCTs in Brazil, Colombia, and Mexico have been reported to have a positive impact on child nutrition and health outcomes, providing an opportunity to integrate nutrition actions in intersectoral platforms. The current epidemiological situation and policy options offer an opportunity for countries, technical agencies, donors, and other stakeholders to jointly scale up nutrition actions. This can support the development of comprehensive and intersectoral policies to tackle the double burden of malnutrition, strengthen national nutrition surveillance systems, incorporate monitoring and evaluation as systematic components of policies and programs, document and increase investments in nutrition, and assess the effectiveness of such policies to support political commitment and guarantee sustainability.
