The present study reports the prevalence of Paramphistomum spp. in small and large ruminants and their association with the histopathology of the infected rumens. A total of 384 animals were screened for Paramphistomum spp. The animals found positive for Paramphistomum spp. were divided into three groups according to the worm load/5 cm2 (G1: 10 – 20 worms/5 cm2 = Low, G2: 20 – 40 worms/5 cm2 = Medium, and G3: >41 worms/5 cm2 = High). Tissue slides were prepared from samples of the rumen (1 cm2) taken from animals positive for ruminal fluke to determine the histological parameters, including epithelial length or thickness, length and width of the ruminal papilla, and thickness of tunica submucosa and mucularis externae. The overall prevalence of Paramphistomum spp. in the ruminant population of district Narowal was 56.25 % with a significant (P < 0.05) variation among different species of ruminants. The highest prevalence was in cattle, followed in order by buffalo, goat, and sheep. Epithelium thickness was significantly correlated with parasite load in large ruminants and the most significant (P < 0.05) decrease in epithelium thickness was in Group B (31.12 ± 1.82 μm) and Group C (31.07 ± 1.68 μm) and a same trend was recorded in small ruminants. Histopathological changes due to Paramphistomum spp. are reported for the first time, which explained the histomorphological and physiological changes in Paramphistomum-infected rumens which might be associated with lowered feed efficiency and productivity in ruminants.

Abstract Introduction: Compliance with the laws and regulations on the sale and supply of medicine ensures that it is conducted safely and professionally. Objectives: This study identify the compliance rate of community pharmacists and general practitioners in the state of Sarawak towards the Malaysian Laws on Poisons and Sale of Drugs and review the effect enforcement actions taken by the Sarawak Pharmacy Enforcement Branch. Methods: A cross sectional study where the data were extracted retrospectively from the annual inspection reports on community pharmacists and general practitioners conducted by the Sarawak Pharmacy Enforcement Branch from 2016 to 2020. Data were extracted using a self-developed data collection tool by trained enforcement officers. Overall, 50 criteria were examined but 24 more criteria were also examined for community pharmacists. Results: The compliance rate of community pharmacists has improved slightly from 58.6% in 2016 to 61.1% in 2020. Meanwhile, the compliance rate of general practitioners improved drastically, from 35.9% in 2016 to 71.2% in 2020. The recording provision on the supply of substances containing Codeine, Dextromethorphan, Ephedrine, and Pseudoephedrine (12.3%-24.1%) and of the Prescription Book (7.7%-27.6%) were the most common non-compliance recorded for all the 5 years among community pharmacists and general practitioners, respectively. Enforcement action “issuance of warning letters” found to induced major (79.6%) improvement in the compliance rate. Conclusion: The compliance rate of community pharmacists and general practitioners has improved throughout the years. The highest non-compliance rate was towards the recording provision on the supply of medicine. Constant assessment of compliance rate as well as the effectiveness of enforcement actions must be done regularly. Abstrak: Penelitian ini bertujuan menghasilkan sebuah produk media pembelajaran berupa e-module yang telah melalui proses validasi dan uji coba sehingga layak dan efektif digunakan dalam pembelajaran pada pokok bahasan cahaya dan alat optik. Metode penelitian dan pengembangan yang digunakan oleh peneliti adalah model pengembangan ADDIE yang di adaptasi dari Lee & Owens. Alasan pemilihan model penelitian tersebut karena model tersebut dikhususkan untuk pengembangan multimedia. Selain itu, model penelitian yang digunakan ini dapat dikatakan proseduran karena proses penyusunanya sistematis. Prosedur penelitian dan pengembangan yang dilakukan oleh peneliti adalah: analisis, desain, pengembangan, penerapan, dan evaluasi.Subjek penelitian ini adalah ahli media, ahli materi, siswa dan guru mata pelajaran yang ahli dibidangnya. Data yang didapatkan peneliti berupa data kuantitatif dan data kualitatif. Abstract: This study aims to produce a learning media product in the form of an e-module that has gone through a process of validation and testing so that it is feasible and effective to be used in learning on the subject of light and optical devices. The research and development method used by researchers is the ADDIE development model adapted from Lee & Owens. Research and development procedures carried out by researchers are analysis, design, development, application, and evaluation. The subjects of this research are media experts, material experts, students and subject teachers who are experts in their fields. The data obtained by researchers in the form of quantitative data, and qualitative data. Quantitative data derived from the assessment of media expert questionnaires, material experts, literature studies regarding previous research and student questionnaire data. Qualitative data comes from criticisms and suggestions from material experts, and media experts. Data collection techniques in the form of interviews, observation and questionnaire distribution of media experts, material experts, and the audience or students.

Character education is an important issue in the world of late-adult education. This is related to the phenomenon of moral decadence that occurs in the midst of society as well as in an increasingly diverse government environment. Among others include crime, injustice, corruption, violence in children, to the violation of human rights. This proves that there is indeed a crisis of identity and characteristics of the Indonesian nation. The existence of character education becomes an appropriate answer to the various problems that have been mentioned previously. It is known that character education is defined as education which develops character values in children so that they have values and character as their character, apply those values in their life, as members of society and religious, nationalist, productive and creative citizens. School as a place of education providers is expected to be a place capable of realizing the mission of character education.

This article describes the additional use of incremental atrial burst pacing (A1A1) and double atrial extrastimulation with a predefined fast pathway conducted A2 (A1A2A3), rather than single atrial extrastimulation (A1A2) only, to characterize typical atrioventricular nodal reentrant tachycardia (AVNRT). The authors noted an additional 32% of patients had multiple anterograde AV nodal physiology demonstrated when A1A1 or A1A2A3 protocols were deployed compared to more conventional A1A2 protocols. The A2H2max (449 +/- 147 vs 339 +/- 94 ms) and A3H3max (481 +/- 120 vs 389 +/- 85 ms) were higher in 31 patients where multiple jumps in the AV nodal conduction curve were obtained (group 1) compared to 192 patients where only single jump was obtained (group 2) (both P < 0.01). Postablation, the degree of reduction of A2H2max (49%) and A3H3max (50%) in group 1 was greater than in group 2 (38% and 42%, respectively, P < 0.05). In seven of group 1 patients in whom A1A2A3 stimulation was required to reveal multiple jumps, the A2H2max remained unchanged after ablation (237 +/- 89 vs 214 +/- 59, P > 0.05). A3H3max was the only parameter that shortened significantly after ablation. Generally, successful ablation resulted in loss of multiple discontinuities in A1A1/A1H1 or A2A3/A3H3 curves. In conclusion, a combination of A1A2, A1A1, and A1A2A3 are required to fully elucidate AVNRT. We described a 55-year-old woman with recurrent syncope, complete atrioventricular (AV) block, sparsely scattered idioventricular beats lasting for 56 seconds, and long sinus arrest recorded during the syncopal episode. Paroxysmal atrial flutter-fibrillation was also presented during Holter electrocardiograph (ECG) monitoring without clinical symptom. During tilt test, atrial flutter with variable AV block was induced and the patient suddenly passed out. The vasovagal syncope was successfully treated with a DDD permanent pacemaker with a rate drop response algorithm. Vasovagal syncope with concomitant ventricular asystole and sinus arrest is rare. Aggressive management with permanent pacemaker is strongly advocated in malignant vasovagal syncope. [ABSTRACT FROM AUTHOR]

Islam Nusantara is not a new thing. The word "Islam" combined with "Nusantara" is not only confirmed the name, but also the characters to show the pattern of a heterogeneous entity. The concept of Islam Nusantara face (vis a vis) with Islam in the Middle East (Islam Arab). Diversity as a typology of Islam Nusantara is the result of the struggle that the length between religion and dimensions of life (social, economic, politic, culture and etc.), the text of the context that complement one another so that spawn Islam friendly, peaceful, anti-radical, harmony and rahmatan lil 'alamin. Departing from the historical and epistemological approach, this article tries to provide a long discourse that was raised again in the "Declaration of Nahdlatul Ulama" at the International Conference of Islamic Leaders Moderate (ISOMIL) which took place from 9 to 11 May in Jakarta. The purpose of writing this article is actually also want to answer the issues pointed out that Islam Nusantara models reflect the teachings of Islam are not singular.

Abstract In Indonesia, bitcoin is a form of digital currency (e-money) and means of payment transactions that have been used by some communities. It is necessary to get attention, especially from Bank Indonesia. Other than that, the supervision that was once fully done by the central bank of Bank Indonesia, is now taken over by OJK (Otoritas Jasa Keuangan). Thus, Bank Indonesia only has the authority to regulate and control the circulation of currency only. Since some of the tasks and authorities of Bank Indonesia were taken over by OJK (Otoritas Jasa Keuangan), many things have not been covered, such as the new phenomenon in the field of finance in terms of capital, investment, currency circulation, and others. In addition there is no legal umbrella against bitcoin, which is increasingly widespread transaction using bitcoin done in e-commerce transactions. So in terms of security also need to be questioned, therefore there needs to be regulation and supervision specifically against bitcoin, and society will not feel harmed. When viewed in the perspective of Islamic law, that relating to the issuance of money as a means of transactions in a country, is a matter protected by general rules in Islamic law. Therefore, the issuance of money and the determination of the amount is matters relating to the benefit of the people. Another aspect to consider in the use of bitcoin is whether from the side of madharat is greater or benefits taken if used as currency and tool transactions, even as a commodity though. So the focus in this research is the use of bitcoin as currency and transaction tool by using approach and study of Islamic law.

Changes in global development policy orientation that put the community (society) rather than the state as its main target has been raised various debates about how to create a better society for development processes. This paper critically discuss the discourse of governance as an instrument of regulation in the new development model through some conceptual apparatus such as social capital, participation and deliberative democracy. Through a post-foundationalist approaches and studies of post-Suharto Indonesia, this paper examined the operation of governance discourse. The main thesis is that the absence of the dimension of power in governance has ignored the political configuration and even failed to notice the various interests in society.

Dalam pengoperasian sistem tenaga lisrik perlu dilakukan peminimalan biaya operasi atau fuel cost. Tujuannya agar daya beban tetap terpenuhi, sedangkan biaya operasi dapat ditekan. Faktor yang menjadi pertimbangan dalam meminimalkan biaya operasi adalah harga bahan bakar. Selama ini pemakaian Bahan Bakar Minyak (BBM) dalam pengoperasian pembangkit listrik menimbulkan kerugian dari sisi biaya operasi. Hal tersebut dikarenakan potensi gas alam yang dalam penggunaannya lebih efisien dan murah oleh pemerintah Indonesia lebih dikhususkan untuk kebutuhan ekspor. Maka, kajian didalam tugas akhir ini ialah mengenai seberapa besar dampak kerugian penggunaan BBM pada pembangkit listrik. Alasan PLTG dan PLTGU dipilih yaitu karena pembangkit ini memiliki efisiensi thermal yang tinggi, sehingga dapat menggunakan bahan bakar dari berbagai jenis minyak. Dynamic Optimal Power Flow (DOPF) dipakai untuk menghitung biaya operasi dengan menggunakan metode Quadratic Programming. DOPF dilakukan dengan menggunakan program Matpower dan software matlab. Sistem yang digunakan yaitu PLTG dan PLTGU yang beroperasi pada sistem kelistrikan Jawa Bali 500 kV.

This paper describes the trace and ultra-trace mineral concentration in different forages utilized by sheep (Kajli and Fat tail) in the rangelands of Chakwal district, Punjab, Pakistan. The concentrations of Se, Mo, Zn, Mn, Cu, and Co were assessed using spectrophotometric methods after the digestion of forage samples. The dominant forage species consumed by sheep were Buxus pappillosa, Prosopis juliflora, Ficus palmata, Acacia modesta, Trichodesma indica, Olea permiginea, Lantana camara, Justicia adhatoda, Morus nigra, and Ipomoea carnea. The overall mean concentration of Co (1.27±0.24 mg/Kg of DM), Zn (35.14±1.28 mg/Kg of DM), and Cu (28.06±1.29 mg/Kg of DM) in forages were higher than the requirements for lactating ewes, while the concentration of Se was within the normal range. Molybdenum (0.030±0.004 mg/Kg of DM) and Mn (29.69±1.61 mg/Kg of DM) concentrations were lower than dietary requirements for sheep. In different administrative divisions (tehsils) like Chakwal, Talagang, Choa Saidan Shah, and Kalar Kahar of Chakwal district, the trace minerals Zn, Mn, and Co, showed non-significant variation, whereas Se, Mo, and Cu showed substantial variation. In conclusion, rangeland forages contained a sufficient concentration of most trace minerals (Co, Cu, Se, Zn) to meet the requirements of grazing sheep in the study district. However, Mn and Mo supplementation seems to be essential to maintain the optimal production capabilities of sheep in this area. [ABSTRACT FROM AUTHOR]

In this study, the relationships between meteorological parameters (relative humidity, wind speed, temperature, planetary boundary layer, and rainfall) and air pollutants (particulate matter and gaseous pollutants) have been evaluated during a 3-year period from 2019 to 2021. Diffusion and dispersion of air contaminants were significantly influenced by meteorology over the capital city. The results of correlation matrix and principal component analysis (PCA) suggest a season's specific influence of meteorological parameters on atmospheric pollutants' concentration. Temperature has the strongest negative impact on pollutants' concentration, and all the other studied meteorological parameters negatively (reduced) as well as positively (increased) impacted the air pollutants' concentration. A two-way process was involved during the interaction of pollutants with relative humidity and wind speed. Due to enhanced moisture-holding capacity during non-monsoon summers, particles get larger and settle down on the ground via dry deposition processes. Winter's decreased moisture-holding capacity causes water vapour coupled with air contaminants to remain suspended and further deteriorate the quality of the air. High wind speed helps in the dispersion and dilution but a high wind speed associated with dust particles may increase the pollutants' level downwind side. The PM 2.5 /PM 10 variation revealed that the accumulation effect of relative humidity on PM 2.5 was more intense than PM 10 . The Aldh1l1-Cre/ER T2 mouse is a widely used transgenic mouse model to conditionally express Cre recombinase in astrocytes of the central nervous system. Currently, no reports show whether the Cre recombinase activity, driven by the Aldh1l1 promoter, acts in cells outside of its intended astrocyte population. We crossed the Aldh1l1-Cre/ER T2 mouse with a TdTomato reporter mouse line, ROSA26:CAG-LSL-TdTomato, to generate a fluorescent reporter for Aldh1l1 promoter activity. Gross anatomical observations reveal strong TdTomato expression in the spleen and exocrine glands-the salivary gland and the pancreas. We find TdTomato expression, a reporter of Cre activity, specifically targets serous cells in the parotid, submandibular, sublingual glands, and pancreas along with fibroblast-like cells within the submandibular lymph nodes and spleen. Our data indicate that the Aldh1l1-Cre/ER T2 mouse model has unintended Cre recombinase activity in exocrine glands, which may influence biological and behavioral data., Competing Interests: The authors declare that there are no conflicts of interest present., (Copyright: © 2023 by the authors.)

BACKGROUND Thrombotic microangiopathy (TMA) is a life-threatening condition caused by small-vessel platelet microthrombi. While various disease triggering factors, including infections, have been well described, there have been few reports of an association between TMA and varicella zoster virus (VZV) infection. VZV infection is rare among people age 20 and older, and infection-induced TMA is mostly reported in the pediatric age group. We report a case of TMA induced by a disseminated VZV infection in an adult. CASE REPORT A 43-year-old man presented with a 3-day duration of fever, headache, vomiting, and bloody diarrhea. He also reported body rash after a recent contact with a few roommates with chickenpox. On presentation, the patient developed convulsive seizures. His laboratory test results were significant for acute kidney injury (AKI) and thrombocytopenia. Atypical hemolytic uremic syndrome (aHUS) and thrombotic thrombocytopenic purpura (TTP) were suspected but further diagnostic testing was negative. The treatment plan included acyclovir, therapeutic plasma exchange, and high-dose oral prednisolone (1.5 mg/kg). The clinical and biochemical profile significantly improved, and the patient was discharged home. CONCLUSIONS TMA is a life-threatening hematological emergency with a high mortality rate. Compared to the pediatric population, VZV infection tends to be more severe in the adult age group. This case demonstrates that a high index of suspicion for TMA in adult patients with VZV who present with thrombocytopenia, even when there is no definitive diagnosis, can result in early management with favorable outcome.

The first-principles approach has been used while employing the Perdew-Burke-Ernzerhof exchange-correlation functional of generalized gradient approximation (PBE-GGA) along with the Hubbard parameter to study the structural, optoelectronic, mechanical and magnetic properties of titanium-based MAX materials Ti 3 AC 2 (A = P, As, Cd) for the first time. As there is no band gap found between the valence and conduction bands in the considered materials, these compounds belong to the conductor family of materials. A mechanical analysis carried out at pressures of 0 GPa to 20 GPa and the calculated elastic constants C ij reveal the stability of these materials. Elastic parameters, i.e. , Young's, shear and bulk moduli, anisotropy factor and Poisson's ratio, have been investigated in the framework of the Voigt-Reuss-Hill approximation. The calculated values of relative stiffness are found to be greater than ½ for Ti 3 PC 2 and Ti 3 AsC 2 , which indicates that these compounds are closer to typical ceramics, which possess low damage tolerance and fracture toughness. Optical parameters, i.e. , dielectric complex function, refractive index, extinction coefficient, absorption coefficient, loss function, conductivity and reflectivity, have also been investigated. These dynamically stable antiferromagnetic materials might have potential applications in advanced electronic and magnetic devices. The present study involves the chemical and bacteriological analysis of water from different sources i.e., bore, wells, bottle, and tap, from Peshawar, Mardan, Swat and Kohat districts of Khyber Pakhtunkhwa (KP) province, Pakistan. From each district, 50 water samples (10 samples from each source), regardless of urban and rural status, were collected from these sources and analysed for sulphates, nitrates, nitrites, chlorides, total soluble solids and coliforms (E. coli). Results indicated that majority of the water sources had unacceptable E. coli count i.e.> 34 CFU/100mL. E. coli positive samples were high in Mardan District, followed by Kohat, Swat and Peshawar district. Besides this, the some water sources were also chemically contaminated by different inorganic fertilizers (nitrates/nitrites of sodium, potassium) but under safe levels whereas agricultural and industrial wastes (chloride and sulphate compounds) were in unsafe range. Among all districts, the water quality was found comparatively more deteriorated in Kohat and Mardan districts than Peshawar and Swat districts. Such chemically and bacteriologically unfit water sources for drinking and can cause human health problems.

Atherosclerosis is the underlying cause of most myocardial infarction (MI) and ischaemic stroke episodes. An early sign of atherosclerosis is hypertrophy of the arterial wall. It is known that increased intima media thickness (IMT) is a non-invasive marker of arterial wall alteration, which can easily be assessed in the carotid arteries by high-resolution B-mode ultrasound. Similarly, the other key element of MI and ischaemic strokes is the N-methyl-D-aspartate (NMDA) receptor which is an ionotropic glutamate receptor that mediates the vast majority of excitatory neurotransmission in the brain. NMDA activation requires the binding of both glutamate and a coagonist like D-serine to its glycine site. A special enzyme, serine racemase (SR), is required for the conversion of L-serine into D-serine, and alterations in SR activities lead to a variety of physiological and pathological conditions ranging from synaptic plasticity to ischemia, MI, and stroke. The amount of D-serine available for the activation of glutamatergic signalling is largely determined by SR and we have developed ways to estimate its levels in human blood samples and correlate it with the IMT. This research based short communication describes our pilot study, which clearly suggests that there is a direct relationship between the SR, D-serine, and IMT. Background Existing evidence, predominantly from Western countries, has demonstrated that athletes' hearts undergo structural, physiological, and electrical changes, leading to abnormal electrocardiogram readings that are said to be training-related. Athletes with non-training-related electrocardiographic abnormalities risk developing sudden cardiac death. The lack of studies on this issue in the Asian population warrants further exploration. Therefore, the aim of this study was to estimate the prevalence and predictive factors contributing to electrocardiogram abnormalities among athletes in Brunei. Methods A descriptive cross-sectional study was conducted on 100 athletes (median age 25.2 years) in 10 sporting disciplines, whose electrocardiogram readings and essential information was obtained. Results The prevalence of an abnormal electrocardiogram was 52% (95% confidence interval: 42.0%-62.0%), comprising training-related changes in 49% (95% confidence interval: 39.0%-59.0%) and non-training-related changes in 3% (95% confidence interval: 0.4%-6.4%). Athletes with a higher body mass index were 3.3-times (95% confidence interval: 1.47-9.58) more likely to have abnormal electrocardiogram readings. Athletes <25-years old (odds ratio = 0.25, 95% confidence interval: 0.07-0.81) and those who trained with low dynamic intensity (odds ratio = 0.33, 95% confidence interval: 0.12-0.93) were significantly less likely to have electrocardiogram abnormalities. Conclusions This is the first study reporting abnormal electrocardiograms among athletes in Brunei, which provides important information to relevant agencies involved in the preparation of Asian athletes for domestic or international competitions, particularly those with a higher body mass index and low dynamic training intensity.

Aim: The Micra™ Transcatheter Pacing System is a leadless pacemaker that has been introduced recently. We share our experience in a low volume center and the use of right ventricular angiography (RVA) during implantation. Materials & methods: Patients underwent Micra implantation and RVA was performed to predetermine the implant site. Results: Nine patients underwent Micra implantation. The most common indication was atrial fibrillation with bradycardia. The device was implanted at apical-septum in seven and mid-septum in two. The procedure time ranged from 30 to 100 min and fluoroscopic time 4-18 min. Pacing parameters remained stable after 1-month follow-up. Conclusion: The Micra implantation technique can be easily learnt. RVA was helpful in selecting an appropriate site for the Micra implant.

A left atrial appendage occluder device (Watchman) and leadless pacemaker (Micra) was implanted from a single right femoral vein access in a 73-year-old female patient with persistent atrial fibrillation and symptomatic tachy-brady syndrome and unable to take oral anticoagulants. Standard methods of implantation were followed for both procedures. The Watchman device was implanted first followed by dilatation of the same venous access site in order to implant Micra transcatheter pacing system. The patient tolerated the procedures well and there were no complications. Sheikh Khalifa Medical City (SKMC) in Abu Dhabi is the main tertiary care referral hospital in the United Arab Emirates (UAE) with 560 bed capacity that is fully occupied most of the time. SKMC senior management has made a commitment to make quality and patient safety a top priority. Our governing body Abu Dhabi Health Services Company has identified medication reconciliation as a critical patient safety measure and key performance indicator (KPI). The medication reconciliation electronic form a computerized decision support tool was introduced to improve medication reconciliation compliance on transition of care at admission, transfer and discharge of patients both in the inpatient and outpatient settings. In order to improve medication reconciliation compliance a multidisciplinary task force team was formed and led this quality improvement project. The purpose of this publication is to indicate the quality improvement interventions implemented to enhance compliance with admission medication reconciliation and the outcomes of those interventions. We chose to conduct the pilot study in general medicine as it is the busiest department in the hospital, with an average of 390 patients admitted per month during the study period. The study period was from April 2014 till October 2015 and a total of 8576 patients were evaluated. The lessons learned were disseminated throughout the hospital. Our aim was to improve admission medication reconciliation compliance using the electronic form in order to ensure patient safety and reduce preventable harm in terms of medication errors. Admission medication reconciliation compliance improved in general medicine from 40% to above 85%, and this improvement was sustained for the last four months of the study period.

Background: Appropriate left ventricle (LV) lead placement is integral to successful cardiac resynchronization therapy (CRT). Lead dislodgement and phrenic nerve stimulation (PNS) are major obstacles. A recent trial of an active fixation LV lead (Attain Stability 20066, Medtronic Inc., Tilburg, the Netherlands) has shown promising results. We share our initial experience with this novel active fixation LV lead., Methods: A Medtronic active fixation lead 20066 was used in eight consecutive patients for CRT. An optimal site was chosen and recommended maneuvers were applied for lead fixation. Push and pull maneuvers were used to test stability., Results: There were two initial dislodgements after which we used a transvalvular insertion (TVI) tool that was used in the hemostatic valve during rotation of the lead so that the torque was easily transmitted to the tip. It also allowed better tactile feedback during push-pull tests. There were no further dislodgements in the subsequent six patients. However, in one patient the lead could not be unscrewed due to the tip getting wedged at a distal smaller vein. Repositioning of the LV lead was done in three patients due to PNS or pacing issues. The median time for LV lead placement was 16.5 minutes (interquartile range 9-25 minutes)., Conclusion: The Medtronic Attain Stability 20066 active fixation LV lead can potentially be implanted at any pacing site avoiding PNS and providing better stability. The learning curve is short and additional tricks can be learnt to improve success. Use of TVI while the lead is rotated is beneficial., (© 2014 Wiley Periodicals, Inc.)

Background: Catecholaminergic polymorphic ventricular tachycardia (CPVT) is a serious disease with a high mortality but its management is limited. The aim of this study was to investigate specific target sites for therapy in order to find potential management strategies for CPVT., Methods and Results: The mutant Ryanodine receptor 2 (RyR2) with reduced stored-overloaded-induced Ca²⁺ release (SOICR) threshold was incorporated into the Luo-Rudy dynamic (LRd) cell model to elucidate the underlying pathologies of CPVT. The simulations reveal that β-adrenergic stimulation increased the Ca²⁺ load in cardiac myocyte, which facilitates spontaneous SR Ca²⁺ leakage, resulting in triggered arrhythmias. Varied blockade (from 0% to 90%) in specific ion channels, including the Na⁺/Ca²⁺ exchanger (I(NaCa)), fast Na⁺ channel (I(Na)), RyR2 receptor (I(rel)), Ca²⁺-ATPase (SERCA) (I(up)) or L-type Ca²⁺channel (I(Ca(L))),was performed to simulate the action of specific drugs on target sites. Blockade of the I(NaCa) (≤ 10% blockade), in contrast to the I(up) (≤ 30% blockade), I(Ca(L)) and I(Na) (≤40% blockade), and followed by I(rel) (≤ 80% blockade), was most effective in suppressing the triggered arrhythmias in CPVT. Specifically, dual blockade of I(Ca(L))/I(up), I(Na)/I(rel) or I(Ca(L))/I(rel) had a synergistic effect in CPVT management., Conclusions: Blockade of I(NaCa) appears to be the most efficacious target for CPVT management. Dual blockade of I(Ca(L))/I(up), I(Na)/I(rel) or I(Ca(L))/I(rel) has a synergistic effect in CPVT treatment.

Background: Mechanical discoordination as studied by magnetic resonance imaging has been shown to be a better predictor of left ventricular (LV) reverse remodeling after cardiac resynchronization therapy (CRT) compared with mechanical dyssynchrony., Materials and Methods: This study assessed the value of acute recoordination derived from speckle-tracking echocardiography for predicting response to CRT compared with acute resynchronization. Thirty patients with heart failure scheduled for CRT were studied at baseline, immediately after CRT, and after 6 months of CRT. Acute recoordination after CRT was indexed by an acute reduction in radial discoordination index (RDI), defined as the ratio of average myocardial thinning to thickening during the ejection phase., Results: CRT responders were defined as those patients whose LV end-systolic volume decreased by >or= 15% at the 6-month follow-up. Immediately after CRT, the responders (n = 18) demonstrated a significant reduction in RDI (P < .001), which was sustained at the 6-month follow-up (P < .001). The nonresponders, however, did not show a significant change in RDI after CRT. LV reverse remodeling at the 6-month follow-up was significantly correlated with acute recoordination (r = 0.75, P < .001) but weakly correlated with acute resynchronization (r = 0.43; P = .02)., Conclusions: Receiver operating characteristic analysis revealed that acute recoordination provided the best separation for prediction of CRT responders compared with acute resynchronization, baseline dyssynchrony, or baseline discoordination. LV recoordination after CRT is an acute phenomenon and predicts response to CRT at 6-month follow-up better than resynchronization., (Copyright 2010 American Society of Echocardiography. Ventricular fibrillation (VF) and myocardial ischemia are inseparable. The first clinical manifestation of myocardial ischemia or infarction may be sudden cardiac death in 20-25% of patients. The occurrence of potentially lethal arrhythmia is the end result of a cascade of pathophysiological abnormalities that result from complex interactions between coronary vascular events, myocardial injury, and changes in autonomic tone, metabolic conditions and ionic state of the myocardium. It is also related to the time from the onset of ischemia. Within the first few minutes there is abundant ventricular arrhythmogenesis usually lasting for 30 min. Triggers for ischemic VF occur at the border zone or regionally ischemic heart. The border zone of ischemia is the predominant site of fragmentation. Acute ischemia opens K(ATP) channels and causes acidosis and hypoxia of myocardial cells leading to a large dispersion in repolarization across the border zone. Abnormalities of intracellular Ca2+ handling also occur in the first few minutes of acute myocardial ischemia and may be an important cause of arrhythmias in human coronary artery disease. Substrate on the other hand transforms triggers into VF and serves to maintain it through fragmentation of waves in the ischemic zone. Thrombin levels, stretch, catecholamine, genetic predisposition, etc. are some of these factors. Reentry models described are spiral wave reentry, 3 dimensional rotors, reentry around 'M' cells and figure-of-eight reentry. Continuing efforts to better understand these arrhythmias will help identify patients of myocardial ischemia prone to arrhythmias.

Sick sinus syndrome with symptomatic bradycardia is an indication for a permanent pacemaker. Either a single (AAIR) or dual-chamber (DDDR) pacemaker can be implanted in these patients with normal atrioventricular nodal function. This report presents a 92-year-old male with right ventricular apical pacing related recurrent acute pulmonary edema and mechanical asynchrony demonstrated by three-dimensional echocardiogram. Although three-dimensional echocardiography has been available for many years, it has seldom been applied to evaluate pacing-related intraventricular asynchrony. The systolic asynchrony index for this patient was 6.7% during AAIR pacing mode and 22% during DDDR pacing mode.

There are several causes for ST segment abnormalities in leads V1 to V3. Hypercalcaemia and Brugada syndrome are among them. Both are known to produce ventricular arrhythmia, albeit only rare cases have been reported with documented evidence of ventricular arrhythmias in association with a hypercalcaemic crisis but none when hypercalcaemic coexists with Brugada syndrome. We describe a patient with primary hyperparathyroidism who presented with ventricular fibrillation, and the ECG showed changes similar to Brugada syndrome. The provocation test with flecainide was conducted twice. This was positive, both before and after parathyroidectomy when serum calcium and parathormone levels had normalized. The patient was treated for hypercalcaemia and underwent parathyroidectomy. This is the first report of oral flecainide test unmasking the diagnostic coved Brugada ECG pattern in a patient with primary hyperparathyroidism and raising attention to hypercalcaemia as a potential trigger for life-threatening arrhythmia in Brugada syndrome.

We report a case of recurrent postcardiac injury syndrome (PCIS) after pacemaker lead insertion. Each episode was attended by hemorrhagic pleuro-pericardial effusion with drop in hemoglobin levels leading us to consider cardiac perforation and subject the patient to surgical pericardiotomy. However, no perforation or active bleeding was detected on exploration. This unusual case illustrates the occurrence of PCIS following pacemaker lead insertion, mimicking cardiac perforation. This entity should be considered in patients who, after pacemaker lead insertion, develop pericardial and pleural effusion associated with markers of inflammation.

Background: Prior study has demonstrated that the biplane single-beat method could be used to assess left ventricular function during atrial fibrillation at a beat with equal subsequent cycles. The study was to test whether we could improve the method by measuring a few beats with equal subsequent cycles and cycle-length limits., Methods: In 75 patients with atrial fibrillation, stroke volume and ejection fraction were determined from simultaneous biplane views of left ventricle for 20 beats using a matrix-array transducer and a biplane Simpson's rule. The influence of cycle lengths on the values of systolic parameters at beats with equal subsequent cycles was examined from the plot of normalized parameters (measured values/average values) against cycle lengths. The values of 1 to 3 beats with equal subsequent cycles and cycle-length limits were averaged and compared with the average values over 20 beats by Bland-Altman and mean percentage difference analysis. The variability of repeat measurements was evaluated in 10 patients., Results: The systolic parameters measured at beats with cycle lengths shorter than 500 ms were usually far below the average values. Agreement and mean percentage difference analysis revealed improved accuracy when 2 or 3 beats with cycle-length limits (>500 ms) were used for assessment. As the variability of averaging 2 or 3 beats is no greater than that of repeat measurements, both methods are equally good., Conclusions: Accurate assessment of left ventricular systolic function in atrial fibrillation can be obtained by averaging 2 beats with equal subsequent cycles and cycle-length limits (>500 ms).

Idiopathic left ventricular tachycardia (ILVT) is a distinct entity that arises in the left ventricle, may have reentrant mechanism and is verapamil-sensitive. Pleomorphism as defined by multiple ventricular tachycardia morphologies is usually associated with either coronary artery disease or cardiomyopathy but very rare in cases of ILVT. In this case report, we describe an unusual case of ILVT with two ECG morphologies of the opposite axis that were successfully eliminated with radiofrequency ablation. The successful ablation sites were closely located to each other in the left lower ventricular septum.
