Allergic rhinitis (AR) is the most common chronic inflammatory disease, affecting an estimated 100 million Europeans (1). Despite a substantial burden on individuals, society and health economies (2), AR remains under-diagnosed, under-estimated (in terms of severity), and under-treated (3). Although effective and safe treatments exist, patients wait too long to seek medical advice, often preferring to self-manage at drug stores and at the pharmacy (4).

- Abstract
The European Position Paper on Rhinosinusitis and Nasal Polyps 2020 is the update of similar evidence based position papers published in 2005 and 2007 and 2012. The core objective of the EPOS2020 guideline is to provide revised, up-to-date and clear evidence-based recommendations and integrated care pathways in ARS and CRS. EPOS2020 provides an update on the literature published and studies undertaken in the eight years since the EPOS2012 position paper was published and addresses areas not extensively covered in EPOS2012 such as paediatric CRS and sinus surgery. EPOS2020 also involves new stakeholders, including pharmacists and patients, and addresses new target users who have become more involved in the management and treatment of rhinosinusitis since the publication of the last EPOS document, including pharmacists, nurses, specialised care givers and indeed patients themselves, who employ increasing self-management of their condition using over the counter treatments. The document provides suggestions for future research in this area and offers updated guidance for definitions and outcome measurements in research in different settings. EPOS2020 contains chapters on definitions and classification where we have defined a large number of terms and indicated preferred terms. A new classification of CRS into primary and secondary CRS and further division into localized and diffuse disease, based on anatomic distribution is proposed. There are extensive chapters on epidemiology and predisposing factors, inflammatory mechanisms, (differential) diagnosis of facial pain, allergic rhinitis, genetics, cystic fibrosis, aspirin exacerbated respiratory disease, immunodeficiencies, allergic fungal rhinosinusitis and the relationship between upper and lower airways. Precision medicine (PM) is increasingly recognized as the way forward for optimizing patient care. Introduced in the field of oncology, it is now considered of major interest in other medical domains like allergy and chronic airway diseases, which face an urgent need to improve the level of disease control, enhance patient satisfaction and increase effectiveness of preventive interventions. The combination of personalized care, prediction of treatment success, prevention of disease and patient participation in the elaboration of the treatment plan is expected to substantially improve the therapeutic approach for individuals suffering from chronic disabling conditions. Given the emerging data on the impact of patient stratification on treatment outcomes, European and American regulatory bodies support the principles of PM and its potential advantage over current treatment strategies. The aim of the current document was to propose a consensus on the position and gradual implementation of the principles of PM within existing adult treatment algorithms for allergic rhinitis (AR) and chronic rhinosinusitis (CRS). At the time of diagnosis, prediction of success of the initiated treatment and patient participation in the decision of the treatment plan can be implemented. The second-level approach ideally involves strategies to prevent progression of disease, in addition to prediction of success of therapy, and patient participation in the long-term therapeutic strategy. Endotype-driven treatment is part of a personalized approach and should be positioned at the tertiary level of care, given the efforts needed for its implementation and the high cost of molecular diagnosis and biological treatment. © 2017 EAACI and John Wiley and Sons A/S. The first European Rhinology Research Forum organized by the European Forum for Research and Education in Allergy and Airway Diseases (EUFOREA) was held in the Royal Academy of Medicine in Brussels on 17th and 18th November 2016, in collaboration with the European Rhinologic Society (ERS) and the Global Allergy and Asthma European Network (GA2LEN). One hundred and thirty participants (medical doctors from different specialties, researchers, as well as patients and industry representatives) from 27 countries took part in the multiple perspective discussions including brainstorming sessions on care pathways and research needs in rhinitis and rhinosinusitis. The debates started with an overview of the current state of the art, including weaknesses and strengths of the current practices, followed by the identification of essential research needs, thoroughly integrated in the context of Precision Medicine (PM), with personalized care, prediction of success of treatment, participation of the patient and prevention of disease as key principles for improving current clinical practices. This report provides a concise summary of the outcomes of the brainstorming sessions of the European Rhinology Research Forum 2016.

The European Academy of Allergy and Clinical Immunology (EAACI), the European Rhinologic Society (ERS), and the European Medical Association (EMA) organized, on October 14, 2015, a symposium in the European Parliament in Brussels on Precision Medicine in Allergy and Airways Diseases, hosted by MEP David Borrelli, and with active participation of the EU Commissioner for Health and Food Safety Vytenis Andriukaitis, MEP Sirpa Pietikainen, Chair of the European Parliament Interest Group on Allergy and Asthma, the European Respiratory Society (ERS), the European Federations of Allergy and Airways Diseases Patients Associations (EFA), the Global Allergy and Asthma European Network (Ga2len), Allergic Rhinitis and Its Impact on Asthma (ARIA), and the Respiratory Effectiveness Group (REG). The socioeconomic impact of allergies and chronic airways diseases cannot be underestimated, as they represent the most frequently diagnosed chronic noncommunicable diseases in the EU; 30% of the total European population is suffering from allergies and asthma, and more than half are deprived from adequate diagnosis and treatment. Precision medicine represents a novel approach, embracing four key features: personalized care based on molecular, immunologic, and functional endotyping of the disease, with participation of the patient in the decision-making process of therapeutic actions, and considering predictive and preventive aspects of the treatment. Implementation of precision medicine into clinical practice may help to achieve the arrest of the epidemic of allergies and chronic airways diseases. Participants underscored the need for optimal patient care in Europe, supporting joint action plans for disease prevention, patient empowerment, and cost-effective treatment strategies.

- Abstract
Tumours affecting the nose, paranasal sinuses and adjacent skull base are fortunately rare. However, they pose significant problems of management due their late presentation and juxtaposition to important anatomical structures such eye and brain. The increasing application of endonasal endoscopic techniques to their excision offers potentially similar scales of resection but with reduced morbidity. The present document is intended to be a state-of-the art review for any specialist with an interest in this area 1. to update their knowledge of neoplasia affecting the nose, paranasal sinuses and adjacent skull base; 2. to provide an evidence-based review of the diagnostic methods; 3. to provide an evidence-based review of endoscopic techniques in the context of other available treatments; 4. to propose algorithms for the management of the disease; 5. to propose guidance for outcome measurements for research and encourage prospective collection of data. The importance of a multidisciplinary approach, adherence to oncologic principles with intent to cure and need for long-term follow-up is emphasised.

- Abstract
From June 22 to 26th, 2014 we celebrated the 50th anniversary of the European Rhinologic Society with the 25th Congress of the European Rhinologic Society and the 32nd International Symposium of Infection and Allergy of the Nose. The congress was held in the Beurs van Berlage, an historic building, which was the former Stock Exchange of the Netherlands. Now, the Beurs van Berlage is one of the defining monuments of the Dutch capital and is a unique and inspiring congress venue in the heart of Amsterdam. The European Rhinologic Society (ERS) commemorated 50 years of expertise and experience among specialists in rhinology, skull base surgery, allergology and facial plastic surgery. It was no accident that the 25th Congress of the European Rhinologic Society was held in Amsterdam, the beautiful and historic capital of the Netherlands. After all, the inaugural meeting of the society was organised in the Netherlands by Professor Van Dishoeck in 1963 and the 25th anniversary was celebrated in Amsterdam with Professor Bert Huizing as President. We were very fortunate that Professor Huizing was willing to give an historical overview of our Society during the opening ceremony. At the opening ceremony honorary memberships of the ERS were awarded to Prof. David Kennedy and Prof. Carel Verwoerd for their outstanding contributions to rhinology

MEP Sirpa Pietikainen, Chair of the European Parliament Interest Group on Allergy and Asthma, underlined the importance of the need for a better diagnostic and therapeutic approach for patients with Allergies and Chronic Airways Diseases, and encouraged a joint initiative to control the epidemic of Allergy and Asthma in Europe. 41. [Rhinology Journal: where are we going?]

In the past 5 years Rhinology has evolved to a high impact Journal in Otorhinolaryngology with a citation index of . 2.182. The result is an enormous increase in the number of papers send to the journal for review. In 2010 we expect to reach over 300 manuscripts that are send to the Journal. This forces us to some important decisions about the future of the Journal. One of those decision has been taken. Last year we published 10 case reports with subjects ranging from Nasal reconstruction in advanced sinunasal sarcoidosis, to Bilateral allergic fungal rhinosinusitis caused by Schizophillum commune and Aspergillus niger and A Pott`s puffy tumour as a late complication of a frontal sinus reconstruction (1-3). This year we published the last case reports we had in stock and in this issue of the Journal we publish the last case report (4). But that will not be enough. More fundamental choices have to be made. To guide the editorial team in the choices we have to make, we ask our readers to help us. On the website of Rhinology you can find a questionnaire. All readers of Rhinology are asked to fill in this questionnaire. We hope many of you will make the effort to go to the website and fill it in: www.rhinologyjournal.com/ .The questionnaire contains questions on the sort of papers we want to read. The last 5 years we increased the number of papers with 50% from around 60 papers I 2005 to around 90 papers in 2009. The increase in papers has mainly been in the clinical papers. The top cited papers in the last five years were mainly guidelines and position papers and reviews (5-9). In the last two years we did not publish this sort of papers and the case mix was totally different with subjects ranging from olfaction to foreign bodies and CRS with or without post nasal drip (10-14). We will ask you what sort of papers you are interested to read in the next years, clinical papers, surgical papers and/or basic research and on what subjects. The journal has become much thicker from 64 pages a few years ago to 80-128 pages now. This has resulted in a significant increase in the printing costs and next year we have to increase the subscription price. Of course the price of the journal has always been very low (74 euro) compared to other journals so we have some room for increase but how far do we want to go? Do you want a lot of space to publish your manuscripts which would mean that we have to increase the size of our Journal even further or are we aiming and even stricter selection and do we only offer you the very high quality papers? We would love to go to 6 issues to have more space and to reduce the time from acceptance to publishing but that would further increase the subscription rate. We will ask your opinion. Many of us read the Journal on the net. We can imagine that a number of people are interested in having an online subscription only. Also that question can be anwered in the questionnaire. A number of societies in Europe have a shared membership of the ERS and in some countries the Journal is send to all members of the otorhinolarynogolgy society with a grant from the pharmaceutical industry. If your rhinologic society is interested in a shared membership please let us know. In summary, Rhinology is a rising star. We are at a point that we have to decide how to proceed. We hope that you are willing to fill in the questionaire on the Rhinology website to help us to make the right decision

- Abstract
Tissue remodelling reported in upper airways include epithelial hyperplasia, increased matrix deposition in the nasal or paranasal lining, matrix degradation and accumulation of plasma proteins. Genetic influences, foetal exposures and early life events may contribute to structural changes such as subepithelial fibrosis from an early age. Other structural alterations are related to duration of the disease and long-term uncontrolled inflammation. Structural changes may increase alteration of the protective functions of the upper airways namely by affecting mucociliary clearance and conditioning of inspired air. The sequences of tissue changes during wound repair of upper airway mucosa after surgery are illustrative of the complexicity of tissue modelling and remodelling and could be considered as an important source of information for a better understanding of the complex relationship between inflammatory reaction, of the subsequent tissue damages and fibroblast metabolism of upper airways.
