Background: Cytologic examination of specimens obtained from the respiratory tract is a lung cancer diagnostic procedure with high specificity but moderate sensitivity. The use of molecular biomarkers may enhance the sensitivity of cytologic examination in the detection of lung cancer.Methods: Complement factor H, a protein secreted by lung cancer cells, was quantified in a series of bronchoalveolar lavage supernatants from lung cancer patients and patients with nonmalignant respiratory diseases. Albumin, total protein content, and hemoglobin were also analyzed. Results were validated in independent sets of bronchoalveolar lavage and sputum supernatants.Results: There was a significantly higher concentration of factor H in bronchoalveolar lavage samples from lung cancer patients. The sensitivity and specificity of the factor H test was 82% and 77%, respectively. These results were validated in an independent set of patients with nearly identical results. Furthermore, 70% and 45% of bronchoalveolar lavage fluids from central and peripheral tumors, respectively, reported as cytologically negative, were classified as positive using this marker. Finally, the test was evaluated in a series of sputum supernatants from lung cancer patients and controls. The sensitivity and specificity of the factor H test in this series was 80% and 88%, respectively.Conclusion: Factor H is elevated in bronchoalveolar lavage and sputum from lung cancer patients.Impact: Measurement of molecular biomarkers, such as complement factor H, may be used in the future as an adjunct to cytology in the diagnosis of malignant pulmonary diseases. Background Lung cancer (LC) is a leading cause of mortality claiming more than 1.8 million deaths per year worldwide. Surgery is one of the most effective treatments when the disease is in early stages. The study of metabolic alterations after surgical intervention with curative intent could be used to assess the response to treatment or the detection of cancer recurrence. Methods In this study, we have evaluated the changes in the metabolomic profile in serum samples (n = 110) from preoperative (PRE) and postoperative (POST) LC patients collected at two different timepoints (1 month - A; 3–6 months - B) with respect to a group of healthy people. An untargeted metabolomic platform based on reversed phase (RP) and hydrophilic interaction chromatography (HILIC), both mounted into an ultra-high performance liquid chromatography (UHPLC) coupled to mass spectrometry (MS) was applied. Results A total of 33 altered metabolites belonging to carboxylic acids, organooxygen compounds, prenol lipids, fatty acyls, glycerolipids and glycerophospholipids classes were annotated comparing all the different studied groups. We found a group of metabolites altered in PRE (gabaculine, DG(14:0/22:1), stearamide, proline and E,e-carotene-3,3'-dione) whose levels returned to those of a baseline control group 3–6 months after surgery. Furthermore, humulinic acid A and 3-galactosyllactose levels may predict an unsuccessful surgery because they remained altered after the intervention in some patients. In addition, tranexamic acid could be studied more exhaustively to be proposed for monitoring the postoperative recurrence of the LC because its levels seem to be close to controls after one month of the intervention, but may differ significantly after 3–6 months. Conclusions This study provides unique insights into the metabolic profiles of LC patients after surgery at two different time points with a wide metabolic coverage combining complementary analytical methods.

ObjectivesWe aimed to assess the learning curves and the influence of the pathologist’s performance on the endobronchial ultrasound transbronchial needle aspiration’s (EBUS-TBNA’s) diagnostic accuracy in a real-world study.Design/settingCohort study conducted in a tertiary care university hospital (single centre) with patients referred for EBUS-TBNA.Participants/interventionWe initially evaluated 376 patients (673 lymph nodes), 368 (660 lymph nodes) of whom were recruited. The inclusion criterion was EBUS-TBNA indicated for the study of mediastinal or hilar lesions. The exclusion criteria were the absence of mediastinal and hilar lesions during EBUS confirmed by a normal mediastinum and hilum on chest CT (except in cases of mediastinal staging of cancer) and lost to follow-up.Primary and secondary outcome measuresDiagnostic accuracy and related outcomes.MethodsWe included patients from a prospectively constructed database. We performed a logistic regression multivariate analysis to adjust for potential confounders of the association between pathologist performance and EBUS-TBNA accuracy. The Cumulative Summation (CUSUM) analysis was used to assess pathologists’ performance and learning curves.ResultsMost indications for EBUS were suspicion of malignancy, including intrathoracic tumours (68.3%), extrathoracic tumours (9.8%) and cancer staging (7.0%). The patients’ mean age was 63.7 years, and 71.5% were male. Overall EBUS-TBNA accuracy was 80.8%. In the multivariate logistic regression model, the factors independently associated with EBUS-TBNA accuracy included certain pathologists (ORs ranging from 0.16 to 0.41; pConclusionsPathologists’ proficiency conditioned EBUS-TBNA accuracy. This human factor is a potential source of error independent of factors conditioning tissue sample adequacy.

Fine-tuning of the Cr3+ zero-phonon R1-line fluorescence (2E-4A2) has recently been achieved by varying the composition of Cr-doped solid solution garnets. Chemical substitutions of Y3+ with Lu3+ and Tb3+ produce R1-line shifts and multiplets that cannot be associated with direct or indirect changes in the Cr-O covalency alone and raise important questions on the nature of the interactions between the CrO6 active center and its environment, hence on the impact of local disorder. In this Letter we show that Pauli antisymmetry interactions with next-nearest neighbor ions, which are commonly omitted from theoretical analyses, induce a nephelauxetic effect on the CrO6 active center that shifts the R1-line. The strength of these interactions increases with the ionic radius and number of codopant ions, all of it resulting in controlled shifts and multiplets of R1-lines. The Pauli antisymmetry induced nephelauxetic effect, combined with the R1-line shift with lattice constant, allows for a very fine-tuning of the Cr3+ R1-line.

Chronic obstructive pulmonary disease (COPD) is an entity with a heterogeneous presentation. For this reason, attempts have been made to characterize different phenotypes and endotypes to enable a more individualized approach. The aim of the Biomarkers in COPD (BIOMEPOC) project is to identify useful biomarkers in blood to improve the characterization of patients. Clinical data and blood samples from a group of patients and healthy controls will be analyzed. The project will consist of an exploration phase and a validation phase. Analytical parameters in blood will be determined using standard techniques and certain 'omics' (transcriptomics, proteomics, and metabolomics). The former will be hypothesis-driven, whereas the latter will be exploratory. Finally, a multilevel analysis will be conducted. Currently, 269 patients and 83 controls have been recruited, and sample processing is beginning. Our hope is to use the results to identify new biomarkers that, alone or combined, will allow a better characterization of patients.

SARS-CoV-2, the COVID-19 causative agent, has infected millions of people and killed over 1.6 million worldwide. A small percentage of cases persist with prolonged positive RT-PCR on nasopharyngeal swabs. The aim of this study was to determine risk factors for prolonged viral shedding amongst patient's basal clinical conditions.We have evaluated all 513 patients attended in our hospital between 1 March and 1 July. We have selected all 18 patients with prolonged viral shedding and compared them with 36 sex-matched randomly selected controls. Demographic, treatment and clinical data were systematically collected.Global median duration of viral clearance was 25.5 days (n = 54; IQR, 22-39.3 days), 48.5 days in cases (IQR 38.7-54.9 days) and 23 days in controls (IQR 20.2-25.7), respectively. There were not observed differences in demographic, symptoms or treatment data between groups. Chronic rhinosinusitis and atopy were more common in patients with prolonged viral shedding (67%) compared with controls (11% and 25% respectively) (P 0.001 and P = 0.003). The use of inhaled corticosteroids was also more frequent in case group (P = 0.007). Multivariate analysis indicated that CRS (odds ratio [OR], 18.78; 95% confidence interval [95%CI], 3.89-90.59; P 0.001) was independently associated with prolonged SARS-CoV-2 RNA shedding in URT samples, after adjusting for initial PCR Ct values.We found that chronic rhinosinusitis and atopy might be associated with increased risk of prolonged viral shedding. If confirmed in prospective trials, this finding might have clinical implications for quarantine duration due to increased risk of pandemic spread.

Simple Summary The increasing number of data supporting use of a personalized approach in cancer treatment, is changing the path of patient's management. In the same time, the availability of technologies should allow patients to receive the best test for the specific individual condition. This is theoretically true, when a specific test is designed for the specific disease condition, while it is difficult to implement in the setting of agnostic therapies. Financial sources availability related to the non homogeneous health systems working in the different countries do not allow for an immediate implementation of the technologies and test commercially available. Future perspectives for targeted oncology include tumor-agnostic drugs, which target a given mutation and could be used in treating cancers from multiple organ types. Therefore, the present paper is aimed to both underline a how much important is this new view and also to sensitize the international bodies that supervise health policies at the decision-making level, with the aim of harmonizing cancer treatment pathways in at least all European countries. Rapid and continuing advances in biomarker testing are not being matched by uptake in health systems, and this is hampering both patient care and innovation. It also risks costing health systems the opportunity to make their services more efficient and, over time, more economical. The potential that genomics has brought to biomarker testing in diagnosis, prediction and research is being realised, pre-eminently in many cancers, but also in an ever-wider range of conditions-notably BRCA1/2 testing in ovarian, breast, pancreatic and prostate cancers. Nevertheless, the implementation of genetic testing in clinical routine setting is still challenging. Development is impeded by country-related heterogeneity, data deficiencies, and lack of policy alignment on standards, approval-and the role of real-world evidence in the process-and reimbursement. The acute nature of the problem is compellingly illustrated by the particular challenges facing the development and use of tumour agnostic therapies, where the gaps in preparedness for taking advantage of this innovative approach to cancer therapy are sharply exposed. Europe should already have in place a guarantee of universal access to a minimum suite of biomarker tests and should be planning for an optimum testing scenario with a wider range of biomarker tests integrated into a more sophisticated health system articulated around personalised medicine. Improving healthcare and winning advantages for Europe's industrial competitiveness and innovation require an appropriate policy framework-starting with an update to outdated recommendations. We show herein the main issues and proposals that emerged during the previous advisory boards organised by the European Alliance for Personalized Medicine which mainly focus on possible scenarios of harmonisation of both oncogenetic testing and management of cancer patients.

Rationale Emphysema and osteoporosis are tobacco-related diseases. Many studies have shown that emphysema is a strong and independent predictor of low bone mineral density (BMD) in smokers; however, none of them explored its association with different emphysema subtypes. Objective To explore the association between the different emphysema subtypes and the presence of low bone mineral density in a population of active or former smokers with and without chronic obstructive pulmonary disease (COPD). Methods One hundred and fifty-three active and former smokers from a pulmonary clinic completed clinical questionnaires, pulmonary function tests, a low-dose chest computed tomography (LDCT) and a dual-energy absorptiometry (DXA) scans. Subjects were classified as having normal BMD or low BMD (osteopenia or osteoporosis). Emphysema was classified visually for its subtype and severity. Logistic regression analysis explored the relationship between the different emphysema subtypes and the presence of low BMD adjusting for other important factors. Phys. Chem. Lett. 2019, 10, 1851]. Here we show, by means of multiconfigurational ab initio calculations, that the IVCT absorptions of all 2+/3+ mixed valence lanthanides doped in CaF2 start basically at the same energy. This is rationalized with a simplified model of IVCT absorptions. Emissions above that energy are expected to be partly or totally quenched when 2+/3+ pairs are formed. Rapid and continuing advances in biomarker testing are not being matched by take-up in health systems, and this is hampering both patient care and innovation. It also risks costing health systems the opportunity to make their services more efficient and, over time, more economical. This paper sets out the potential of biomarker testing, the unfolding precision and range of possible diagnosis and prediction, and the many obstacles to adoption. It offers case studies of biomarker testing in breast, ovarian, prostate, lung, thyroid and colon cancers, and derives specific lessons as to the potential and actual use of each of them. It also draws lessons about how to improve access and alignment, and to remedy the data deficiencies that impede development. And it suggests solutions to outstanding issues - notably including funding and the tangled web of obtaining reimbursement or equivalent coverage that Europe's fragmented health system implies. It urges a European evolution towards an initial minimum testing scenario, which would guarantee universal access to a suite of biomarker tests for the currently most common conditions, and, further into the future, to an optimum testing scenario in which a much wider range of biomarker tests would be introduced and become part of a more sophisticated health system articulated around personalised medicine. For exploiting genomics to the full, it argues the need for a new policy framework for Europe. Biomarker testing is not an issue that can be treated in isolation, since the purpose of testing is to improve health. Its use is therefore always closely linked to specific health challenges and needs to be viewed in the broader policy context in the EU and more widely. The paper is the result of extensive engagement with experts and decision makers to develop the framework, and consequently represents a wide consensus of views on how healthcare systems should respond from push and pull factors at local, national and cross-border and EU level. Lung cancer (LC) is still one of the most frequent cancers with a high related mortality. Their prognosis is directly proportional to the stage at the time of diagnosis. Seventy percent are currently diagnosed in advanced or locally advanced stage (higher than stage III), making a cure unlikely for the majority of patients. Developments in LC treatment are significant however they do not seem to be enough to reverse the current situation, at least, in a short period of time. Despite recent advances in treatment, primary prevention and early diagnosis appear to be the key to reduce the incidence and mortality of this disease. Many countries have developed LC screening programs based on the results of clinical trials published in recent years. The aim of this paper is to review the latest results of the NEderlands Leuvens Longkanker Screenings Onderzoek and compare them with the findings of the National Lung Screening Trial. We address the question whether it is necessary to continue discussing the evidence regarding LC screening. In both trials, there is a clear impact on LC mortality but, with a modest reduction in over all mortality. It has always been a spectroscopist's dream to correlate a material's luminescence properties with its microscopic structure, based on reliable structure-property relationships. Electronic structure methods are promising to achieve this goal; yet they are especially challenging in the case of Eu-based materials which are known to feature exceptionally high density of excited states, large spins and severe electron correlation. In this work, state-of-the-art multiconfigurational ab initio embedded-cluster methods are applied to gain a deeper insight into the luminescence mechanisms of Eu2+ and Eu3+-doped phosphors. Regardless of the difficulties, very accurate excitation energies are achieved, reaching 68% prediction intervals of 300 cm(-1), corresponding to an accuracy of 5-10 nm in the visible wavelength range. Complete configurational coordinate curves are obtained, yielding breathing mode vibrational frequencies and equilibrium bond lengths for all excited states. Moreover, electric dipole transition moments and oscillator strengths are used to calculate absorption spectra. Excellent agreement with experiment is found. The ab initio calculations give an unprecedented detailed view of the Eu2+ excited state landscape, allowing for an improved understanding of its structure, including the origin of the so-called 'staircase structure' and the role of ligand covalency in the ligand field and exchange splitting. It is found that more covalent host compounds feature higher exchange splittings due to an increased stabilization of high-spin states by the interaction with virtual LMCT states. It is verified that the equilibrium Eu-ligand bond length contracts upon 4f-5d excitation towards the lowest 5d submanifold and that the bond lengths are directly related to the configurational character of the electronic eigenstate. Moreover, comparing ab initio calculations with crystal field calculations proves that a decoupled model for the Eu2+ excited states is inadequate, making the use of an intermediate coupling scheme compulsory. With this approach, computational design of luminescent materials is getting within reach.

INTRODUCTION: Non-small cell lung cancer (NSCLC) patients diagnosed in early stage and surgically-treated have five-year mortality rate >20%. The identification of biomarkers able to predict progression and death may help to identify patients needing closer follow-up. METHODS: A retrospective cohort of early-stage surgically-treated NSCLC patients enrolled in the International Association for the Study of Lung Cancer (IASLC) Staging Project was created, and tissue Microarrays (TMAs) were constructed with tumor and non-tumor lung tissue. Lanthanides are multivalent chemical systems whose luminescence activates numerous optical devices. Their multivalent nature sets a complex scenario that challenges widespread single-valent-ion models. Here we study the complexity inherent in the 2+/3+ mixed-valence nature of Yb in CaF2. We report X-ray absorption spectroscopy of CaF2:Yb that shows a maximal, huge Yb3+ X-ray-induced valence reduction at 200 K for low Yb concentration (0.01%) samples, which reverts to the initial state upon warming to 300 K. Although reduction doubles the number of Yb2+ ions, the anomalous emission that is normally observed at low temperatures completely disappears and never recovers. Surprisingly, however, after annealing at 900 K, some anomalous emission is again observed below 150 K. Under X-ray exposure electrons are ejected from the F– ions, including charge compensating interstitials, Fi–. Removing an electron from Fi– leaves a neutral atom which appears metastable. Controlling luminescence in phosphors able to produce several emissions from different stable excited states determines their use in optical devices. We investigate the color control mechanism that quenches the greenish-blue emission in favor of the red one in the archetype phosphor CaTiO3:Pr3+. State-of-the-art ab initio calculations indicate that direct host-to-dopant energy transfer (released by electron–hole recombination following the interband excitation and structural reorganization) selectively populates the 1D2 red luminescent state of Pr3+ and bypasses the 3P0 greenish-blue emitter. Local defects can modulate the electron–hole recombination energy and therefore increase the red emission efficiency, as experimentally observed. The selection of red emission does not happen in CaZrO3:Pr3+ because the electron–hole recombination energy is much higher. Materials that luminesce after excitation with ionizing radiation are extensively applied in physics, medicine, security, and industry. Lanthanide dopants are known to trigger crystal scintillation through their fast d–f emissions; the same is true for other important applications as lasers or phosphors for lighting. However, this ability can be seriously compromised by unwanted anomalous emissions often found with the most common lanthanide activators. We report high-resolution X-ray-excited optical (IR to UV) luminescence spectra of CaF2:Yb and SrF2:Yb samples excited at 8949 eV and 80 K. Ionizing radiation excites the known anomalous emission of ytterbium in the CaF2 host but not in the SrF2 host. Wave function-based ab initio calculations of host-to-dopant electron transfer and Yb2+/Yb3+ intervalence charge transfer explain the difference. The model also explains the lack of anomalous emission in Yb-doped SrF2 excited by VUV radiation.

Introduction: Bronchiectasis has been classified as a comorbidity of COPD patients, associated with frequent exacerbations and severity of airflow limitation. In our study, we investigated the prevalence and characteristics of bronchiectasis in the lung cancer screening (LCS) program. Material and methods: A transversal study was performed. From 2014 until September 2018, 1005 patients were included in our LSC program. Of these, 949 had low-dose computed tomography (LDCT). The subjects met the NLST criteria (age between 55 and 75 who have a 30 pack-year smoking history and are currently smoking or have quit within the past 15 years) and had an additional risk factor of pulmonary emphysema and/or CODP. Other variables collected were clinical features, microbiological isolation, blood tests, and previous exacerbations. Statistical analyses were performed using SPSS version 22.0. Results: The prevalence of bronchiectasis was 11.3% (n = 107), mostly cylindrical (39.2%) and unilateral (57.9%). As expected based on inclusion criteria, 76.6% had emphysema on LDCT, and 48.6% had COPD. Most patients denied chronic sputum production (57.9%). Only 3 patients had evidence of colonization by Pseudomonas aeruginosa.
