Background and Purpose: Mechanical thrombectomy (MT) in ischemic stroke patients with poor prestroke conditions remains controversial. We aimed to analyze the frequency of previously disabled patients treated with MT in clinical practice, the safety and clinical response to MT of patients with preexisting disability, and the disabled patient characteristics associated with a better response to MT. Methods: We studied all consecutive patients with anterior circulation occlusion treated with MT from January 2017 to December 2019 included in the Codi Ictus Catalunya registry—a government-mandated, prospective, hospital-based data set. Prestroke disability was defined as modified Rankin Scale score 2 or 3. Functional outcome at 90 days was centrally assessed by a blinded evaluator of the Catalan Stroke Program. Favorable outcome (to return at least to prestroke modified Rankin Scale at 90 days) and safety and secondary outcomes were compared with patients without previous disability. Logistic regression analysis was used to assess the association between prestroke disability and outcomes and to identify a disabled patient profile with favorable outcome after MT. Results: Of 2487 patients included in the study, 409 (17.1%) had prestroke disability (313 modified Rankin Scale score 2 and 96 modified Rankin Scale score 3). After adjustment for covariates, prestroke disability was not associated with a lower chance of achieving favorable outcome at 90 days (24% versus 30%; odds ratio, 0.79 [0.57–1.08]), whereas it was independently associated with a higher risk of symptomatic intracranial hemorrhage (5% versus 3%; odds ratio, 2.04 [1.11–3.72]) and long-term mortality (31% versus 18%; odds ratio, 1.74 [1.27–2.39]) compared with patients without disability. Prestroke disabled patients without diabetes, Alberta Stroke Program Early CT Score >8 and National Institutes of Health Stroke Scale score Conclusions: Despite a higher mortality and risk of symptomatic intracranial hemorrhage, prestroke-disabled patients return as often as independent patients to their prestroke level of function, especially those nondiabetic patients with favorable early ischemic signs profile. These data support a potential benefit of MT in patients with previous mild or moderate disability after large anterior vessel occlusion stroke.

A 500 años de la herida de san Ignacio, las instituciones educativas encomendadas a la Compañía de Jesús inician la conmemoración en un año atípico y complejo. La pandemia surge como una herida colectiva que exige un momento de reposo y curación. Los retos usuales de la educación superior encuentran hoy un panorama incierto que exige una revisión de nuestras tradiciones educativas y habilidades docentes, a la luz de la pedagogía ignaciana que hoy aparece como una oportunidad para situarnos en un momento complicado y evaluarnos sinceramente. Este texto pretende mostrar qué elementos de esa pedagogía van encontrando áreas de oportunidad entre nuestros estudiantes, además de ofrecer perspectivas a los docentes que se sienten limitados al momento de aplicarla a sus respectivos cursos.

A novel water-stable 2D Copper-Imidazolate Framework (2DCIF) exhibits outstanding electrocatalytic activity for the oxygen reduction reaction without the need for calcination. These 2DCIFs have been successfully used as air electrodes in Zn–air batteries.

We report on the preparation and characterization of two dimensional (2D) films of (S,S,P)-1 and (R,R,M)-1 ortho-oligophenylene ethylene (o-OPE) enantiomers presenting high values of circularly polarized luminescence (CPL). The amphiphilic character of these two molecules allows a precise 2D self-assembly at the air/water interface and an efficient transfer onto a glass solid support. The morphological and chiroptical characterization of the solid supports after the transfer of 1, 8, 16 and 32 Langmuir films of (S,S,P)-1 and (R,R,M)-1 has been carried out. The strong chiroptical values of these monomers allow reliable ECD measurements to be obtained after a single transfer, with ECD values increasing as the number of transferred films increases. The semi-liquid behavior of the monomers on the solid substrate allows CPL measurements free of photoselection artifacts that show values similar to those obtained in solution and independent of monomer concentration. Objective To estimate the incidence of falls and to know their characteristics in terms of location, temporality and injuries produced, and to analyse the sociodemographic and clinical characteristics of the patients who suffer falls. Methodology A retrospective observational cohort study was carried out in a level 2 hospital of the Madrid Health Service. Falls in hospitalized patients between July 1, 2018 and June 30, 2019 were studied. The incidence rate of falls per 1000 days of stay was estimated considering a Poisson distribution. The characteristics of the falls are described: temporality, injuries produced, location of the injuries and prescribed drugs. Sociodemographic and clinical characteristics of patients who suffered a fall were registered. A univariate analysis was performed to compare the results by gender. All data were obtained from the electronic medical record. Results One hundred and thirty-two falls were studied, which represent a rate of 1.61 falls per 1,000 days of stay. Men, older age, and admission to a medical specialty showed a significantly higher fall rate. The patients who suffered a fall had a mean age of 77.5 years (SD: 11.7), and had a median of 12.5 drugs prescribed (IQR: 9.25-15). Of the falls, 63.6% did not present any injury. Difference in gender was only found in the situation in which the fall occurred. Conclusions Our data report an incidence of falls similar to other institutions in our environment. The profile of the patient who suffers a fall is an older man, admitted under the charge of a medical specialty, with longer hospital stay, with associated comorbidity and polymedicated, without an obvious temporal feature.

Resumen Objetivo Estimar la incidencia de caidas y conocer sus caracteristicas en cuanto a localizacion, temporalidad y lesiones producidas, y analizar las caracteristicas sociodemograficas y clinicas de los pacientes que sufren caidas. Metodo Se llevo a cabo un estudio observacional de cohortes retrospectivo, en un hospital de nivel 2 del Servicio Madrileno de Salud. Se estudiaron las caidas de pacientes hospitalizados entre el 1 de julio de 2018 y el 30 de junio de 2019. Se estimo la tasa de incidencia de caidas por 1.000 dias de estancia considerando una distribucion de Poisson. Se describen las caracteristicas de las caidas: temporalidad, lesiones producidas, localizacion de las lesiones y pauta de farmacos. Se registraron caracteristicas sociodemograficas y clinicas de los pacientes que sufrieron una caida. Se realizo un analisis univariante para comparar los resultados por sexos. Todos los datos se han obtenido de registros de la historia clinica electronica. Resultados Se estudiaron 132 caidas, que suponen una tasa de 1,61 caidas por 1.000 dias de estancia. Hombres, edad avanzada e ingreso a cargo de una especialidad medica presentaron una tasa de caida significativamente mayor. Los pacientes que sufrieron una caida tienen una edad media de 77,5 anos (DE 11,7), y tuvieron pautados una mediana de 12,5 farmacos (RIC 9,25-15). El 63,6% de las caidas no presentan lesion alguna. Solo se encontro diferencia en cuanto al genero en la situacion en la que se produce la caida. Conclusiones Nuestros datos notifican una incidencia de caidas similar a la de centros de nuestro entorno. El perfil del paciente que sufre una caida es el de un hombre mayor, ingresado a cargo de una especialidad medica, con estancias hospitalarias mas largas, con alguna comorbilidad y polimedicado, sin una asociacion temporal evidente.

Embargado hasta 09/04/2023 Polymersomes and related self-assembled nanostructures displaying Aggregation-Induced Emission (AIE) are highly relevant for plenty of applications in imaging, biology and functional devices. Experimentally simple, scalable and universal strategies for on-demand self-assembly of polymers rendering well-defined nanostructures are highly desirable. A purposefully designed combination of amphiphilic block copolymers including tunable lengths of hydrophilic polyethylene glycol (PEGm) and hydrophobic AIE polymer poly(tetraphenylethylene-trimethylenecarbonate) (P(TPE-TMC)n) has been studied at the air/liquid interface. The unique 2D assembly properties have been analyzed by thermodynamic measurements, UV-vis reflection spectroscopy and photoluminescence in combination with molecular dynamics simulations. The (PEG)m-b-P(TPE-TMC)n monolayers formed tunable 2D nanostructures self-assembled on demand by adjusting the available surface area. Tuning of the PEG length allows to modification of the area per polymer molecule at the air/liquid interface. Molecular detail on the arrangement of the polymer molecules and relevant molecular interactions has been convincingly described. AIE fluorescence at the air/liquid interface has been successfully achieved by the (PEG)m-b-P(TPE-TMC)n nanostructures. An experimentally simple 2D to 3D transition allowed to obtain 3D polymersomes in solution. This work suggests that engineered amphiphilic polymers for AIE may be suitable for selective 2D and 3D self-assembly for imaging and technological applications.

Fluid interfaces provide an advanced platform for directed self-assembly of organic composites and formation of supramolecular polymers (SPs). Intermolecular interactions govern the supramolecular polymerization processes, with hydrogen bonding (H-bonding) as a key interaction in supramolecular chemistry and biology. Two purposefully designed supra-amphiphiles for assessing the role of H-bonding were designed and their supramolecular polymerization (SP) at the air/water interface was compared. H-bonding was confirmed by in situ experimental and computational techniques as the required intermolecular interaction for attaining SPs with well-defined molecular arrangement. Control of H-bonding as opposite to traditionally considered interactions, e.g., π-π stacking is proposed as a successful strategy for SP at fluid interfaces.

A pesar de las leyes existentes, en la práctica el Estado peruano ignora la multiculturalidad y se comporta como una entidad monolingüe y monocultural. Dado que este paradigma equivocado todavía vigente, el Estado no ha invertido lo suficiente para desarrollar las habilidades lingüísticas con el fin de servir a todos los ciudadanos por igual. Las consecuencias de ello son la falta de fomento, la discriminación y finalmente el aislamiento que lleva a la extinción de las lenguas autóctonas. Nuestra iniciativa es cambiar el paradigma equivocado, despertar el orgullo nacional por nuestras raíces nativas y hacerlo en tres frentes: demostrar que nuestras lenguas se pueden usar en el mundo tecnológico moderno al igual que las lenguas bien establecidas, demostrar que nuestras lenguas pueden portar cultura y entretenimiento bajo los cánones contemporáneos y demostrar que nuestras lenguas aportan valor económico a la nación, lo que justifica su preservación más allá del derecho. En este documento se describe una hoja de ruta para el desarrollo de la lingüística computacional de idiomas infrasoportados que todavía son hablados por millones de hablantes. Tal es el caso del quechua, aimara, guaraní, náhuatl, mixteco, otomí, quiché, maya o zapoteco. Debido a la masiva presencia de los hablantes de estas lenguas en el entorno urbano y a su uso habitual de Internet y telefonía móvil, se apuesta por la construcción de corpus de estas lenguas vía crowdsourcing online.

Understanding social behavior and explaining its implications is essential when examining consumer xenocentric behavior. This study evaluated the direct effects of consumer xenoncentrism on product attitude and purchase intention of imported products and analyzed the moderating impact of formal education on xenocentric consumer behavior considering groups with higher and low formal education levels. Structural equation modeling technique and multigroup analysis based on samples collected from Chilean consumers were used to analyze the research model. There were 295 effective online questionnaires (42.4% females and 57.6% males). This study’s findings suggest that consumer xenocentric effects are directly related to imported products’ product attitude (G: R2 = 0.254; L: R2 = 0.121; H: R2 = 0.327) and purchase intention (G: R2 = 0.454; L: R2 = 0.469; H: R2 = 0.365). In addition, findings exhibit that xenocentric consumer behaviors are more significant when associated with formal educational level (G: 0.575; L: 0.640; H: 0.443). Therefore, as education levels increase, the xenocentric effect also increases. An important application of these findings is that education in emerging countries and developing economies should strengthen local production valorization and promote marketing strategies that foster the sustainable consumption of products manufactured in their own countries.

Pyogenic tenosynovitis is caused by hematogenous spread of infection or trauma with direct inoculation of a tendon sheath. Symptoms and clinical examination findings associated with pyogenic tenosynovitis may be confused with superficial soft tissue infections, however management plans between pyogenic tenosynovitis and superficial soft tissue infection vary significantly. In patients with pyogenic tenosynovitis, operative intervention and subsequent irrigation and debridement offer a definitive therapy. Bedside ultrasound helps clinicians inspect the involved tendon sheath and may help assisting diagnosis of pyogenic tenosynovitis. In this case report, we described three cases, where point of care ultrasound was used to assist the diagnosis of pyogenic tenosynovitis, to accelerate consultation, and to expedite operative intervention.
