RIVM rapport:De wereldwijde epidemie van ebola was de meest in het oog springende infectieziekte van dit jaar. In Nederland waren geen ebolapatiënten, maar is veel tijd besteed aan de voorbereiding om patiënten te kunnen verzorgen en verspreiding van het virus te voorkomen. Daarnaast was er de uitbraak van mazelen in Nederland, die in 2013 begon en eindigde in de eerste maanden van 2014. In die periode zijn verspreid over Nederland 2700 zieken gemeld en is 3,9 miljoen euro besteed om de epidemie te bestrijden. De grootste kostenposten waren de werkzaamheden van de betrokken GGD'en en de kosten van ziekenhuisopname van ernstig zieke patiënten. Dit blijkt uit de Staat van Infectieziekten in Nederland 2014, een jaarlijks rapport dat inzicht geeft in ontwikkelingen van infectieziekten bij de Nederlandse bevolking. Daarnaast worden elk jaar de ontwikkelingen op het gebied van de infectieziekten in het buitenland beschreven die voor Nederland relevant zijn. Met deze jaarlijkse uitgave informeert het RIVM beleidsmakers van het ministerie van Volksgezondheid, Welzijn en Sport (VWS). Het thema van dit jaar zijn de kosten voor de behandeling, preventie en bestrijding van de infectieziekten. Om inzicht in de kosten te krijgen, zijn deze berekend voor de mazelenepidemie en voor de Salmonella Thompson-uitbraak door gerookte zalm in 2012. Voor deze twee is gekozen omdat ze omvangrijk waren en verspreid over heel Nederland zieken veroorzaakten. Bij de Salmonella Thompson-uitbraak zijn 1149 ziektegevallen gemeld die met behulp van laboratoriumdiagnostiek werden vastgesteld en is 1,7 miljoen euro aan kosten gemaakt. De grootste kostenposten betroffen de inzet van de Nederlandse Voedsel en Warenautoriteit (NVWA) om de bron van de besmetting op te sporen en de kosten van ziekenhuisopname van ernstig zieke patiënten. In de studie staat ook beschreven hoe de kosten en de opbrengsten van interventies met elkaar kunnen worden vergeleken. The study also describes how the cost and health outcomes of different interventions can be compared. Cost-effectiveness studies can be used to determine the most effective use of health care budgets. In this report, the most common methods of measuring outcomes and cost-effectiveness are explained.

Background: Recent studies have found high prevalences of asymptomatic rectal chlamydia among HIV-infected men who have sex with men (MSM). Chlamydia could increase the infectivity of HIV and the susceptibility to HIV infection. We investigate the role of chlamydia in the spread of HIV among MSM and the possible impact of routine chlamydia screening among HIV-infected MSM at HIV treatment centres on the incidence of chlamydia and HIV in the overall MSM population. Methods: A mathematical model was developed to describe the transmission of HIV and chlamydia among MSM. Parameters relating to sexual behaviour were estimated from data from the Amsterdam Cohort Study among MSM. Uncertainty analysis was carried out for model parameters without confident estimates. The effects of different screening strategies for chlamydia were investigated. Results: Among all new HIV infections in MSM, 15% can be attributed to chlamydia infection. Introduction of routine chlamydia screening every six months among HIV-infected MSM during regular HIV consultations can reduce the incidence of both infections among MSM: after 10 years, the relative percentage reduction in chlamydia incidence would be 15% and in HIV incidence 4%, compared to the current situation. Chlamydia screening is more effective in reducing HIV incidence with more frequent screeni Conclusions: Chlamydia infection could contribute to the transmission of HIV among MSM. Preventive measures reducing chlamydia prevalence, such as routine chlamydia screening of HIV-infected MSM, can result in a decline in the incidence of chlamydia and HIV.
