1. Respective roles of social deprivation, health literacy, and clinical factors for COVID-19: a case-control study in hospitalized patients

Abstract: IntroductionTo investigate the association between social deprivation and COVID-19 among hospitalized patients in an underprivileged department of the greater Paris area.MethodsIndividuals hospitalized for COVID-19 between March 1st and October 31, 2020, were included, matched on age and sex, and compared with patients hospitalized for any other reason with negative RT-PCR for SARS-CoV-2, through a case-control study. Clinical, socio-demographic characteristics, health literacy, and social deprivation, assessed by the EPICES score, were collected. Factors associated with COVID-19 in hospitalized patients were assessed using univariate and multivariate logistic regression models.Results69 cases and 180 controls were included. Participants were mostly men (N = 148: 59.4%) aged 65 or older (N = 109: 44.1%). Median EPICES score was 43.2 (IQR 29.4–62.9). EPICES score > 30.17 (precariousness threshold) was not significantly associated with COVID-19 in hospitalized patients (adjusted odds ratio (aOR) = 0.46; 95% Confidence Interval (CI) [0.21–1.01]). Advanced age, higher BMI, professional activity, home area of less than 25 m2 per person, and low health literacy, were significantly associated with COVID-19 in hospitalized patients.DiscussionThis study highlights probable risk factors for specific exposition in disadvantaged area: maintenance of professional activity, smaller home area, and low health literacy.

Abstract: Introduction Inhalation of crystalline silica (silicon dioxide, SiO2) is associated with a wide range of acute and chronic diseases, including rheumatoid arthritis (RA). The objectives of this work were to identify the main sources of exposure to SiO2 in a series of patients with RA not selected on the basis of their professional activity, compared with a representative sample of the French general population, and to assess the association between silica exposure and disease features.Methods The Dust Exposure Life-Course Questionnaire (DELCQ) is a tool that enables retrospective quantification of both occupational and non-occupational lifetime exposure to SiO2. DELCQ—previously validated in a large representative sample of the French general population—was administered to 97 consecutive RA patients, and exposure scores were compared between cases and age, gender and smoking status-matched controls (1:4). The main sources of SiO2 exposure were identified in cases and controls, and source-specific exposure levels were compared. Abstract: Background: Idiopathic pulmonary fibrosis (IPF) is characterized by a male predominance. The aim of the study was to explore gender differences in a well-designed French multicentre prospective IPF cohort (COhorte FIbrose, COFI) with a 5-year follow-up.Methods: Between 2007 and 2010, 236 patients with incident IPF were included in COFI. Gender characteristics were compared using a t-test, Chi-squared test and ANOVA, as appropriate. Survival analyses were performed.Results: Fifty-one (22%) females and 185 (78%) males with an average age at diagnosis of 70.1 ± 9.20 and 67.4 ± 10.9 years, respectively, were included in the cohort. Women were significantly less exposed to tobacco smoke [never n = 32 (62.7%) vs. n = 39 (21.1%), p < 0.001] and to occupational exposure [n = 7 (13.7%) vs. n = 63 (34.1%), p = 0.012]. Baseline forced vital capacity, % of predicted (FVC%) was significantly better in women compare to men (83.0% ± 25.0 v. 75.4% ± 18.7 p = 0.046). At presentation honeycombing and emphysema on CT scan were less common in women [n = 40 (78.4%) vs. n = 167 (90.3%) p = 0.041] and [n = 6 (11.8%) vs. n = 48 (25.9%) p = 0.029], respectively. During follow-up fewer women were transplanted compared to men [n = 1 (1.96%) vs. n = 20 (10.8%) p = 0.039]. Medians of survival were comparable by gender [31 months (CI 95%: 28–40) vs. 40 months (CI 95%: 33–72) p = 0.2]. After adjusting for age and FVC at inclusion, being a woman was not associated to a better survival.Conclusions: Women appear to have less advanced disease at diagnosis, maybe due to less exposure history compare to men. Disease progression and overall survival remains comparable regardless gender, but women have less access to lung transplantation.

5. The Affinity of Hemoglobin for Oxygen Is Not Altered During COVID-19

Abstract: Background: A computational proteomic analysis suggested that SARS-CoV-2 might bind to hemoglobin (Hb). The authors hypothesized that this phenomenon could result in a decreased oxygen (O2) binding and lead to hemolytic anemia as well. The aim of this work was to investigate whether the affinity of Hb for O2 was altered during COVID-19.Methods: In this retrospective, observational, single-center study, the blood gas analyses of 100 COVID-19 patients were compared to those of 100 non-COVID-19 patients. Fifty-five patients with carboxyhemoglobin (HbCO) ≥8% and 30 with sickle cell disease (SCD) were also included (“positive controls” with abnormal Hb affinity). P50 was corrected for body temperature, pH, and PCO2.Results: Patients did not differ statistically for age or sex ratio in COVID-19 and non-COVID-19 groups. Median P50 at baseline was 26 mmHg [25.2–26.8] vs. 25.9 mmHg [24–27.3], respectively (p = 0.42). As expected, P50 was 22.5 mmHg [21.6–23.8] in the high HbCO group and 29.3 mmHg [27–31.5] in the SCD group (p < 0.0001). Whatever the disease severity, samples from COVID-19 to non-COVID-19 groups were distributed on the standard O2-Hb dissociation curve. When considering the time-course of P50 between days 1 and 18 in both groups, no significant difference was observed. Median Hb concentration at baseline was 14 g.dl–1 [12.6–15.2] in the COVID-19 group vs. 13.2 g.dl–1 [11.4–14.7] in the non-COVID-19 group (p = 0.006). Among the 24 COVID-19 patients displaying anemia, none of them exhibited obvious biological hemolysis.Conclusion: There was no biological argument to support the hypothesis that SARS-CoV-2 could alter O2 binding to Hb.

Nous rapportons l’observation d’une patiente prise en charge initialement pour une pneumopathie interstitielle diffuse aigue medicamenteuse revelant une pneumopathie interstitielle diffuse chronique avec stigmates d’auto-immunite. L’histoire evolue en trois temps avec l’apparition d’une sclerodermie cutanee diffuse rapidement progressive avec anticorps anti-ARNPol3 dans un contexte de remission de cancer de l’ovaire.

Abstract: International audience; Inorganic antigens may contribute to paediatric sarcoidosis. Thirty-six patients matched with 36 healthy controls as well as a group of 21 sickle-cell disease (SCD) controls answered an environmental questionnaire. Patients' indirect exposure to inorganic particles, through coresidents' occupations, was higher than in healthy and SCD controls (median score: 2.5 (0.5-7) vs 0.5 (0-2), p=0.003 and 1 (0-2), p=0.012, respectively), especially for construction, exposures to metal dust, talc, abrasive reagents and scouring products. Wood or fossil energies heating were also linked to paediatric sarcoidosis. This study supports a link between mineral environmental exposure due to adult coresident occupations and paediatric sarcoidosis.

COVID-19 primarily impacts the respiratory system, and can result in mild illness or serious disease leading to critical illness requiring admission to the intensive care unit due to respiratory failure. After hospital discharge, the more commonly described pulmonary function anomalies are alterations in diffusing capacity and the loss of lung volume. Reduction of inspiratory muscle contraction may also be underestimated. This article will focus on the pathophysiology of pulmonary function anomalies in COVID-19 survivors. Abstract: International audience; BackgroundLow income, a known prognostic indicator of various chronic respiratory diseases, has not been properly studied in idiopathic pulmonary fibrosis (IPF). We hypothesize that a low income has an adverse prognostic impact on IPF.MethodsPatients were selected from the French national prospective cohort COFI. Patients’ income was assessed through the median city-level income provided by the French National Institute of Statistics and Economic Studies according to their residential address. Patients were classified in two groups as “low income” vs. “higher income” depending on whether their annual income was estimated to be < or ≥18 170 €/year (the first quartile of the income distribution in the study population). The survival and progression-free survival (PFS) of the groups were compared by a log-rank test and a Cox model in multivariate analysis.Results200 patients were included. The average follow-up was 33.8 ± 22.7 months. Patients in the low income group were significantly more likely to be of non-European origin (p < 0.006), and to have at least one occupational exposure (p < 0.0001), and they tended to have a higher cumulative exposure to fine particles PM2.5 (p = 0.057). After adjusting for age, gender, forced vital capacity at inclusion, geographical origin, and occupational exposure having a low-income level was a factor associated with a worse PFS (HR: 1.81; CI95%: 1.24–2.62, p = 0.001) and overall survival (HR: 1.49; CI95%: 1.0006–2.23, p = 0.049).ConclusionsLow income appears to be a prognostic factor in IPF. IPF patients with low incomes may also be exposed more frequently to occupational exposures.

Abstract: International audience; Background: Accumulating evidence suggests that in utero exposures can influence the development of the immune system and thus contribute to disease development. Studies investigating the association between prenatal exposures to heavy metals and atopic diseases, however, are scarce.Methods: Children from the EDEN birth cohort were prospectively followed up using parental questionnaires with validated questions on asthma, allergic rhinitis, eczema, and food allergy symptoms. The questionnaires were administered every 4 months during the children's first year, and then every year until the age of 5, with a final survey at the age of 8. Serum concentrations of lead (Pb), cadmium (Cd), and manganese (Mn) were assessed in maternal blood samples collected during mid-pregnancy and in cord blood of 651 mother-children pairs. Hazard ratios (HR) for the incidence of each atopic disease in relation to the exposure to metals were calculated using Cox proportional hazard models.Results: Levels of Cd in cord blood were associated with greater risk of asthma (hazard ratio [95% confidence interval] for upper vs lower quartile: 1.81 [1.00-3.29]), eczema (1.60 [1.09-2.35]), and food allergy (3.17 [1.36-7.38]), while Mn levels in maternal serum were associated with eczema (1.55 [1.05-2.28]). These associations were similar in males and females and were confirmed using log concentrations of metals as exposures.Conclusions: Our results support the hypothesis that fetal exposure to heavy metals may affect the development of asthma, eczema, and food allergy in childhood and suggest that timing of exposure in utero may have a role in these associations.

Abstract: Background Few studies analyzed gender-related outcomes and characteristics differences of patients with idiopathic pulmonary fibrosis (IPF). The aim of the study was to explore gender differences in a French IPF cohort. Methods 236 patients with incident IPF were included in the multicentric longitudinal prospective cohort COhorte FIbrose (COFI), and followed for 5 years. Men and women were compared for characteristics atinclusion (t-test, Chi2 or ANOVA) and survival (Log rank, Cox model). Results The population consisted of 51 (22%) women and 185(78%) men. At inclusion women significantly differed from men for an older age (> 65 years, 78 vs. 61%, P = 0.028), a lower proportion of smokers (P Conclusions At diagnosis women appear to have a distinct imaging pattern and a better FVC, which may be due to less exposure history compared to men. However, outcomes remain comparable in both sexes. Less access to lung transplantation in women may be due to age and comorbidities.

Abstract: Few studies have examined the effects of air pollution in diffuse interstitial lung disease and they have focused on small numbers of patients. Most data are available in idiopathic pulmonary fibrosis and studies suggest that the level of exposure to pollutants may influence the development of acute exacerbations (ozone and NO2), their incidence (NO2), decline in respiratory function (PM10) and death (PM10 and PM2.5). Several studies show an increase in the incidence of rheumatoid arthritis in people living near busy roads. In systemic scleroderma, hypersensitivity pneumonitis and sarcoidosis although negative effects of pollution have been reported the data are insufficient to be conclusive. Nevertheless, the observed effects of air pollution are consistent with those described for other chronic respiratory diseases. Exposure to pollution induces oxidative stress, chronic inflammation and shortening of telomeres, which are all mechanisms described in fibrogenesis. New epidemiological studies are needed with individual measurements of exposure to outdoor and indoor pollution, as well as fundamental studies to clarify the effect of pollution on fibrogenesis.

24. Objective structured clinical examinations (OSCEs) for students’ training and assessment in the French respiratory medicine departments in 2021: An overview

Abstract: Medical professional performances can be assessed by objective structured clinical examinations (OSCEs) where medical trainees go through a series of simulated clinical situations. OSCEs are now the gold standard for the assessment of medical students' competence during their training. In France, the first national OSCEs will take place in May 2024 and respiratory teachers will be involved in this reform and will use OSCEs for students' training and assessment in respiratory medicine. Students training regarding this final OSCE may vary across medical faculties and may impact students' results. Therefore, we aimed to provide a national overview of OSCE's training performed by respiratory teachers and their interest in developing a common French databank of OSCEs.We conducted a national anonymous online survey among the members of the French college of respiratory teachers (CEP), from 2021 February the 15th to 2021 June the 15th. The survey consisted of 32 questions.Among 118 French pulmonologists teachers, 52 (45%) responded to the survey. We received a response from at least one of each of the French Medical Universities. Twenty-two (42%) had received specific training on how to conduct an OSCE. Twenty-eight (54%) of respondents used OSCEs for training purposes and 24 (46%) for assessment purposes, for less than 1 year in more than half of the participants. The average satisfaction scores out of 10 about OSCEs was 7.3 ± 1.7 for training and 7.4 ± 1.5 for students' assessment. Respondents were willing (8.9 ± 1.8 out of ten) to develop a common databank to share OSCEs subjects in respiratory medicine in France.This survey confirms heterogeneity in the training and the use of OSCEs among French respiratory teachers. However, a common national databank could be a useful tool to reduce these disparities.

Abstract: Background International guidelines recommend that annual influenza vaccination be performed in patients with idiopathic pulmonary fibrosis (IPF). Viral infections are recognized to be triggers of acute exacerbation (AE). The purpose of this study was to investigate the role of annual influenza vaccination on AE occurrence. Methods 236 patients with incident IPF were included in the multicentric longitudinal prospective cohort COhorte FIbrose (COFI), and followed for 5 years. Influenza vaccination status was requested annually. We have defined aone-year vaccination coverage from September the first to August the 30th of the following year. 638 annual observations were available with an informed vaccination status. To evaluate the association between annual influenza vaccination and AE occurrence, two mixed-effect Cox regression was used. For the second model we divided patients in 3 groups: – patients vaccinated every year; – patients vaccinated at least half of the years of follow-up; – patients vaccinated less than half the years of follow-up. Only the first AE was considered. The definition of AE was that of Akira et al. Results Mean follow-up was 33.2 ± 23.6 months. 36 AEs occurred in 33 patients. Of the 33 patients, only 27 had an informed vaccination status in the year of their AE. Nine (33%) patients who experienced an AE weren’t vaccinated. For 141 (22%) annual observations, the patient didn’t receive the vaccine. In this model, influenza vaccination was associated with a decreased occurrence of AE but this did not reach the level of significance in this small population (HR:0.58 CI95%0.23-1.46, P = 0.25). In the first group patients were vaccinated every year and consisted of hundred and thirty-three individuals. The second group was composed of patients vaccinated at least half of the years of follow-up, with 35 patients. And 36 patients were vaccinated less than half the years of follow-up in the last group. In the second model, patients vaccinated less than half the years of follow-up were significantly more at risk to present AE compared to patients who were vaccinated every year (HR:2.77 CI95%1.11-6.93, P = 0.03). Conclusions In our IPF cohort, only about half of the patients had influenza vaccination coverage throughout the follow-up period. Annual influenza vaccination seems to reduce the risk of AE but due to a lack of power this association was not significant. However patients vaccinated less than half the years of follow-up were significantly more at risk to present AE compared to patients who were vaccinated every year. A study with a larger number of patients is highly desirable to confirm this association.

Abstract: Summary: Human genetic studies of critical COVID-19 pneumonia have revealed the essential role of type I interferon-dependent innate immunity to SARS-CoV-2 infection. Conversely, an association between the HLA-B∗15:01 allele and asymptomatic SARS-CoV-2 infection in unvaccinated individuals was recently reported, suggesting a contribution of pre-existing T cell-dependent adaptive immunity. We report a lack of association of classical HLA alleles, including HLA-B∗15:01, with pre-omicron asymptomatic SARS-CoV-2 infection in unvaccinated participants in a prospective population-based study in the United States (191 asymptomatic vs. 945 symptomatic COVID-19 cases). Moreover, we found no such association in the international COVID Human Genetic Effort cohort (206 asymptomatic vs. 574 mild or moderate COVID-19 cases and 1,625 severe or critical COVID-19 cases). Finally, in the Human Challenge Characterisation study, the three HLA-B∗15:01 individuals infected with SARS-CoV-2 developed symptoms. As with other acute primary infections studied, no classical HLA alleles favoring an asymptomatic course of SARS-CoV-2 infection were identified.

Abstract: The French-language Respiratory Medicine Society (SPLF) proposes a guide for the follow-up of patients who have presented with SARS-CoV-2 pneumonia. The proposals are based on known data from previous epidemics, on acute lesions observed in SARS-CoV-2 patients and on expert opinion. This guide proposes a follow-up based on three categories of patients: (1) patients managed outside hospital for possible or proven SARS-CoV-2 infection, referred by their physician for persistent dyspnoea; (2) patients hospitalized for SARS-CoV-2 pneumonia in a medical unit; (3) patients hospitalized for SARS-CoV-2 pneumonia in an intensive care unit. The subsequent follow-up will have to be adapted to the initial assessment. This guide emphasises the possibility of others causes of dyspnoea (cardiac, thromboembolic, hyperventilation syndrome…). These proposals may evolve over time as more knowledge becomes available.

Abstract: Summary: Background: Although immunomodulators have established benefit against the new coronavirus disease (COVID-19) in general, it is uncertain whether such agents improve outcomes without increasing the risk of secondary infections in the specific subgroup of previously immunocompromised patients. We assessed the effect of immunomodulators on outcomes of immunocompromised patients hospitalized for COVID-19. Methods: The protocol was prospectively registered with PROSPERO (CRD42022335397). MEDLINE, Cochrane Central Register of Controlled Trials and references of relevant articles were searched up to 01-06-2022. Authors of potentially eligible randomized controlled trials were contacted to provide data on immunocompromised patients randomized to immunomodulators vs control (i.e., placebo or standard-of-care). Findings: Eleven randomized controlled trials involving 397 immunocompromised patients hospitalized for COVID-19 were included. Ten trials had low risk of bias. There was no difference between immunocompromised patients randomized to immunomodulators vs control regarding mortality [30/182 (16.5%) vs 41/215 (19.1%); RR 0.93, 95% CI 0.61–1.41; p = 0.74], secondary infections (RR 1.00, 95% CI 0.64–1.58; p = 0.99) and change in World Health Organization ordinal scale from baseline to day 15 (weighed mean difference 0.27, 95% CI -0.09–0.63; p = 0.15). In subgroup analyses including only patients with hematologic malignancy, only trials with low risk of bias, only trials administering IL-6 inhibitors, or only trials administering immunosuppressants, there was no difference between comparators regarding mortality. Interpretation: Immunomodulators, compared to control, were not associated with harmful or beneficial outcomes, including mortality, secondary infections, and change in ordinal scale, when administered to immunocompromised patients hospitalized for COVID-19. Abstract: Introduction Peu d’etudes ont analyse la survie et les differences de caracteristiques initailes des patients atteints de la fibrose pulmonaire idiopathique (FPI) selon le genre. L’objectif de l’etude etait d’explorer les differencesselon le genre dans une cohorte francaise de FPI. Methodes 236 patients atteints d’une FPI incidente ont ete inclus dans la cohorte prospective longitudinale multicentrique COhorte FIbrose (COFI), et a suivi pendant 5 ans. Les hommes et les femmes ont ete compares pour les caracteristiques a l’inclusion (test t, Qui-squared ou ANOVA) et une l’analyse de survie sans transplantation a ete realise (Log rank, modele de Cox). Resultats La population etait composee de 51 (22 %) femmes et 185 (78 %) hommes. A l’inclusion, les femmes avaient un age plus avance que les hommes, (âge > a 65 ans 78 % contre 61 %, p = 0,028), une proportion plus faible de fumeurs (p Conclusion Au moment du diagnostic de FPI, les femmes semblent avoir une meilleure CVF et une presentation radiologique distincte, possiblement du fait qu’elles ont ete moins exposees par rapport aux hommes. Toutefois, la survie sans transplantation reste comparable entre les hommes et les femmes. La diminution de l’acces a la transplantation pulmonaire chez les femmes pourrait etre due a l’âge et aux comorbidites.

Abstract: Mediastinal lymph node (MLN) enlargement on chest computed tomography (CT) is prevalent in patients with interstitial lung disease (ILD) and may reflect immunologic activation and subsequent cytokine-mediated immune cell trafficking.We aimed to determine whether MLN enlargement on chest CT predicts clinical outcomes and circulating cytokine levels in ILD.MLN measurements were obtained from chest CT scans of patients with ILD at baseline evaluation over a 10-year period. Patients with sarcoidosis and drug toxicity-related ILD were excluded. MLN diameter and location were assessed. Plasma cytokine levels were analyzed in a subset of patients. The primary outcome was transplant-free survival (TFS). Secondary outcomes included all-cause and respiratory hospitalizations, lung function, and plasma cytokine concentrations. Cox regression was used to assess mortality risk. Outcomes were assessed in three independent ILD cohorts.Chest CT scans were assessed in 1,094 patients (mean age, 64 yr; 52% male). MLN enlargement (≥10 mm) was present in 66% (n = 726) and strongly predicted TFS (hazard ratio [HR], 1.53; 95% confidence interval [CI], 1.12-2.10; P = 0.008) and risk of all-cause and respiratory hospitalizations (internal rate of return [IRR], 1.52; 95% CI, 1.17-1.98; P = 0.002; and IRR, 1.71; 95% CI, 1.15-2.53; P = 0.008, respectively) when compared with subjects with MLN10 mm. Patients with MLN enlargement had lower lung function and decreased plasma concentrations of soluble CD40L (376 pg/ml vs. 505 pg/ml, P = 0.001) compared with those without MLN enlargement. Plasma IL-10 concentration45 pg/ml predicted mortality (HR, 4.21; 95% CI, 1.21-14.68; P = 0.024). Independent analysis of external datasets confirmed these findings.MLN enlargement predicts TFS and hospitalization risk in ILD and is associated with decreased levels of a key circulating cytokine, soluble CD40L. Incorporating MLN and cytokine findings into current prediction models might improve ILD prognostication.

Abstract: SummaryBackground The diagnosis of HSR to iodinated contrast media (ICM) is challenging based on clinical history and skin tests. Objective This study evaluates the negative predictive value (NPV) of skin tests and intravenous provocation test (IPT) with low-dose ICM in patients with suspected immediate hypersensitivity reaction (HSR) to ICM. Methods Thirty-seven patients with suspected immediate hypersensitivity reaction to ICM were included retrospectively. Skin tests and a single-blind placebo-controlled intravenous provocation test (IPT) with low-dose iodinated contrast media (ICM) were performed. Results Skin tests with ICM were positive in five cases (one skin prick test and five intradermal test). Thirty-six patients were challenged successfully by IPT, and only one patient had a positive challenge result, with a grade I reaction by the Ring and Messmer classification. Ten of 23 patients followed up by telephone were re-exposed to a negative tested ICM during radiologic examination; two experienced a grade I immediate reaction. Conclusions & Clinical Relevance For immediate hypersensitivity reaction to ICM, the NPV for skin tests and IPT with low dose was 80% (95% CI 44–97%).

Abstract: Abstract Objective To investigate the association of air pollution exposure with the severity of interstitial lung disease (ILD) at diagnosis and ILD progression among patients with systemic sclerosis (SSc)-associated ILD. Methods We conducted a retrospective two-center study of patients with SSc-associated ILD diagnosed between 2006 and 2019. Exposure to the air pollutants particulate matter of up to 10 and 2.5 µm in diameter (PM10, PM2.5), nitrogen dioxide (NO2), and ozone (O3) was assessed at the geolocalization coordinates of the patients’ residential address. Logistic regression models were used to evaluate the association between air pollution and severity at diagnosis according to the Goh staging algorithm, and progression at 12 and 24 months. Results We included 181 patients, 80% of whom were women; 44% had diffuse cutaneous scleroderma, and 56% had anti-topoisomerase I antibodies. ILD was extensive, according to the Goh staging algorithm, in 29% of patients. O3 exposure was associated with the presence of extensive ILD at diagnosis (adjusted OR: 1.12, 95% CI 1.05–1.21; p value = 0.002). At 12 and 24 months, progression was noted in 27/105 (26%) and 48/113 (43%) patients, respectively. O3 exposure was associated with progression at 24 months (adjusted OR: 1.10, 95% CI 1.02–1.19; p value = 0.02). We found no association between exposure to other air pollutants and severity at diagnosis and progression. Conclusion Our findings suggest that high levels of O3 exposure are associated with more severe SSc-associated ILD at diagnosis, and progression at 24 months.

Abstract: Introduction: In patients with chronic fibrosing idiopathic interstitial pneumonias (IIPs) and a possible or inconsistent with UIP pattern on HRCT, surgical lung biopsy (SLB) is required to make a definitive diagnosis between IPF and nonspecific interstitial pneumonia (NSIP) after multidisciplinary discussion (MDD). The aim of the study was to determine the predictive factors of IPF in this context. Methods: This monocentric retrospective study was based on a cohort of 289 IIPs patients. We included cases with a possible or inconsistent with UIP pattern on HRCT and a final diagnosis of IPF or NSIP after SLB and MDD. A multivariate logistic regression was used to analyse the predictive factors of IPF diagnosis. Patients with unclassifiable IIPs, mainly due to the absence of SLB, served as a validation cohort. Results: Among patients with a possible or inconsistent with UIP pattern (IPF:n=31, PINS:n=36), age>65 years was the best threshold indicative of IPF by a ROC curve. After adjustment for tobacco and sex, age>65 years (OR=3.35, p=0.046) and FVC decline>5% at 6 months (OR=3.46, p=0.047) were the strongest independent predictors of IPF. The combination of age>65 years + FVC decline>5% had a specificity of 94% and positive predictive value of 75%. In patients with unclassifiable IIPs (n=101), those with age>65 years + FVC decline>5% had a worse survival compared to others (median survival: 3.1 vs 8.8 years, p Conclusion: The combination of age and functional decline may help differentiate IPF and NSIP and assist clinicians in therapeutic decisions for patients who cannot undergo SLB.

We hypothesised that outdoor air pollution might influence the natural history of IPF.MethodsPatients were selected from the French cohort COhorte FIbrose (COFI), a national multicentre longitudinal prospective cohort of IPF (n=192). Air pollutant levels were assigned to each patient from the air quality monitoring station closest to the patient’s geocoded residence. Cox proportional hazards model was used to evaluate the impact of air pollution on AE, disease progression and death.ResultsOnset of AEs was significantly associated with an increased mean level of ozone in the six preceding weeks, with an HR of 1.47 (95% CI 1.13 to 1.92) per 10 µg/m3 (p=0.005). Cumulative levels of exposure to particulate matter PM10 and PM2.5 were above WHO recommendations in 34% and 100% of patients, respectively. Mortality was significantly associated with increased levels of exposure to PM10 (HR=2.01, 95% CI 1.07 to 3.77) per 10 µg/m3 (p=0.03), and PM2.5 (HR=7.93, 95% CI 2.93 to 21.33) per 10 µg/m3 (pConclusionThis study suggests that air pollution has a negative impact on IPF outcomes, corroborating the role of ozone on AEs and establishing, for the first time, the potential role of long-term exposure to PM10 and PM2.5 on overall mortality.

43. Pneumopathies interstitielles diffuses (PID) fibrosantes prédominant aux lobes supérieurs : étiologies, caractéristiques et pronostic

Abstract: Introduction Les causes de PID fibrosantes predominant aux lobes superieurs sont connues pour etre variees dans la litterature, mais elles sont mal decrites. Parmi elles, la fibroelastose pleuroparenchymateuse (FEPP) a ete recemment individualisee comme une entite bien definie. Notre etude avait pour objectif de determiner la prevalence des PID predominant aux lobes superieurs, la repartition de leurs etiologies, leurs caracteristiques et leur pronostic. Methodes Etude retrospective monocentrique basee sur une cohorte de 1132 patients ayant une PID chronique fibrosante adresses entre 2006 et 2016. Etaient exclus les cas de sarcoidose d’evolution fibrosante dont le diagnostic etait deja connu avant la prise en charge dans le service. Resultats Quatre-vingt-sept patients avaient une predominance aux lobes superieurs sur la tomodensitometrie (TDM) thoracique, soit une prevalence de 7,69 %. La gamme diagnostique apres une discussion multidisciplinaire etait la suivante : sarcoidose : n = 21, FEPP : n = 19, pneumopathie d’hypersensibilite (PHS) : n = 15, PID inclassable : n = 11, PID du fumeur : n = 6, silicose : n = 5 ; et autres diagnostics : n = 10. Les quatre groupes les plus prevalents ont ete compares (sarcoidose, FEPP, PHS, et PID inclassable). Les patients avec une FEPP avaient un antecedent familial de PID significativement plus frequent (7/19, 37 %), un IMC plus bas, la survenue plus frequente de pneumothorax et/ou de pneumomediastins (8/19, 42 %) et pas de fixation a la tomographie par emission de positrons. Sur la TDM, les masses de fibrose, les adenopathies et la predominance centrale etaient plus frequentes en cas de sarcoidose. Les FEPP avaient plus souvent une perte de volume, une asymetrie, des condensations sous pleurales et un thorax plus plat. Les lobules clairs distinguaient les PHS. Le declin annuel de la capacite vitale etait moins marque dans la sarcoidose. La survie de la FEPP etait significativement plus mauvaise que celle des autres groupes ( p = 0,02), estimee a 60 % a 5 ans, en particulier en presence d’une PID associee des bases. Conclusion Certaines caracteristiques cliniques et radiologiques aident a differencier les principales causes de PID fibrosantes predominant aux lobes superieurs. La frequence des formes familiales doit inciter a faire une recherche genetique chez les patients ayant une FEPP, dont le pronostic est sombre.

Abstract: Introduction: Use of lung function test (LFT) is essential in respiratory medicine. In France, residents9 training in LFT interpretation is variable. The aim of this survey was to assess the level of French residents in LFT interpretation and to assess the benefits of an internship in a LFT lab. Methods: Prospective anonymous survey including 36 questions. Data were collected during teaching seminars in France. Results: 257 residents have answered the survey; 41 (16%) had followed an internship in a LFT lab. Flow obstruction were identified by 67% (n: 171) of residents, restrictive defects were identified by 35% (n: 90) of residents, emphysemas were identified by 42% (n: 109) of residents, an obstacle on an upper airway was identified by 43% (n: 110) of residents, decrease in the strength of respiratory muscles by 33% (n: 84), diaphragmatic weakness by 26% (n: 69), efficacy of cough was identified by 27% (n: 70). An accurate analysis of arterial blood gas was achieved by 52% (n: 133) of residents. 13% (n: 34) of the residents knew in which population the LFT reference values could be used. 33% (n: 86) of the residents knew that helium dilution shouldn9t be used to measure lung volumes in patients with flow obstruction. Residents who had followed an internship in LFT lab had significantly better results in LFT interpretation than those who had not (p Conclusion: The level of respiratory French residents in LFT interpretation is suboptimal. However an internship in a LFT lab improves the level of respiratory residents with LFT interpretation.

45. Fibrose pulmonaire idiopathique : délai diagnostique et réseau de soin

Abstract: Introduction Les symptomes de la fibrose pulmonaire idiopathique (FPI) sont specifiques et peuvent conduire a un retard diagnostique potentiellement deletere. En effet, la FPI necessite une prise en charge globale et specialisee pour l’introduction precoce des traitements anti-fibrosants (pirfenidone et nintedanib), l’accessibilite optimale a la transplantation pulmonaire (TP), et la gestion des comorbidites et facteurs de risque cardiovasculaires frequents. Le but de cette etude etait de decrire le reseau de soin emprunte depuis l’apparition des symptomes jusqu’au diagnostic, et d’evaluer l’association entre le delai diagnostique et la survie. Methodes Il s’agit d’une analyse retrospective concernant les patients ayant une FPI incidente, suivis a l’hopital Avicenne et inclus dans la cohorte nationale prospective COFI. Resultats Soixante-cinq patients ont ete inclus dont 54 hommes et 76,9 % fumeurs (actifs ou sevres) ; l’âge median etait de 67,1 ans (Q1–Q3 = 60,7–72,4). Au total, 67,7 % des patients avaient consulte spontanement pour des symptomes, alors que la decouverte de la FPI etait fortuite dans 32,3 % des cas. Le delai median entre le debut des symptomes et la premiere consultation etait de 5,6 mois (Q1–Q3 = 1,4–18,0). Avant le diagnostic, la mediane du nombre total de consultation etait de 4 (Q1–Q3 = 2–5), le nombre median de praticiens consultes etait de 2 (Q1–Q3 = 1–2), et 30,9 % des patients avaient beneficie d’au moins trois scanners thoraciques. Le delai diagnostique median (defini comme le delai entre l’apparition des premiers symptomes et l’inclusion dans la cohorte COFI) etait de 1,6 ans (Q1–Q3 = 0,9–2,5). Il etait d’autant plus long que les patients avaient une comorbidite respiratoire ( p = 0,001) et etaient jeunes ( p = 0,044). En revanche, il n’y avait pas de lien entre le delai diagnostique et la fonction respiratoire, le score scanographique de fibrose, ou, le score GAP au moment de l’inclusion. La mediane de survie globale sans TP etait de 2,8 ans. Aucune association significative n’a ete mise en evidence entre le delai diagnostique et la survie globale sans TP ( p = 0,65), la survie sans evenement ( p = 0,48), la survie liee a la FPI ( p = 0,72), ou la survie non liee a la FPI ( p = 0,92). Conclusion Cette etude confirme l’existence d’une errance diagnostique chez les patients ayant une FPI, entrainant un retard de prise en charge en centre specialise. Cependant, il ne semble pas y avoir d’association entre le delai diagnostique et la severite initiale de la maladie ou la survie.

46. Prévalence et caractéristiques des pneumopathies interstitielles idiopathiques inclassables

Abstract: Introduction Dans la classification ATS/ERS revisee des pneumopathies interstitielles idiopathiques (PII), une categorie est dorenavant reservee aux formes dites « inclassables ». Il y a actuellement peu d’etudes decrivant ces PII inclassables. L’objectif du travail est de determiner la prevalence et les caracteristiques des PII inclassables. Methodes Etude retrospective monocentrique incluant les patients consecutifs incidents ou prevalents ayant une PII pris en charge entre 2006 et 2015. Les diagnostics ont ete retenus selon les criteres ATS/ERS, apres discussion multidisciplinaire systematique incluant pneumologues, radiologues et anatomopathologistes. Resultats La population a inclus 281 patients (âge moyen : 71,4 ± 13,5 ans, 69 % d’hommes). La fibrose pulmonaire idiopathique etait la PII la plus representee (55 %), suivie des PII inclassables (26 %), la pneumopathie interstitielle non specifique (14 %) et enfin les autres PII (5 %). Le principal motif d’un diagnostic de PII inclassable etait l’absence de biopsie pulmonaire chirurgicale (BPC) (54 %). La BPC n’a pas ete realisee du fait d’un risque operatoire juge important (57 %) et/ou une atteinte respiratoire discrete et stable (41 %) et/ou un refus du patient (9 %). Les autres motifs d’inclassable etaient l’existence de donnees cliniques, radiologiques et/ou histologiques contradictoires (14 %), des prelevements histologiques insuffisants (15 %), ou des donnees manquantes cliniques ou paracliniques (17 %). La proportion de cas familiaux etait identique entre les PII inclassables ou non (8,6 % vs 5,4 %, p = 0,32). Parmi les patients ayant une PII inclassable, 13,6 % sont restes inclassables malgre la realisation d’une BPC. Conclusion Les PII inclassables constituent un groupe tres heterogene. Il est essentiel de distinguer les PII reellement inclassables au terme d’une evaluation exhaustive, des PII qu’on pourrait plutot nommer « indeterminees », pour lesquelles l’evaluation n’a pas pu, pour diverses raisons, comprendre une BPC.

Abstract: Introduction La formation des internes en physiologie respiratoire n’a jamais ete evaluee. L’objectif de cette etude etait d’evaluer la qualite percue des formations theorique et pratique ainsi que le niveau de connaissances des internes. Methode Enquete prospective conduite durant les seminaires de formation de 6 inter-regions francaises en 2014. Resultats Deux cents cinquante-sept internes ont participe dont 16 % ayant realise un stage en physiologie. La qualite des formations pratique et theorique etait evaluee a 4 [2–5] et 5 [4–7] sur 10. Quatre-vingt-dix pour cent des internes identifiaient correctement un trouble ventilatoire obstructif (TVO). Soixante-neuf pour cent en appreciaient correctement la severite. Quatre-vingt-huit pour cent identifiaient correctement un TV restrictif. Quarante-deux pour cent identifiaient correctement la presence d’une distension thoracique. Quarante-trois pour cent identifiaient la presence d’un obstacle sur les voies aeriennes extra-thoraciques. Trente-sept pour cent identifiaient la presence d’un deficit des muscles respiratoires. Vingt-sept pour cent interpretaient correctement le debit expiratoire a la toux. Trente-trois pour cent savaient que la methode de dilution a l’helium n’etait pas recommandee chez les patients ayant un TVO. Trente pour cent connaissaient les modalites d’entretien d’une plethysmographie. Huit pour cent connaissaient les variables selon lesquelles les normes des EFR ont ete etablies. Les resultats des internes ayant fait un stage de physiologie etaient significativement meilleurs que ceux n’en ayant pas fait ( p Conclusion La formation actuelle doit etre amelioree sur les plans pratique et theorique. Un stage en EFR permet d’ameliorer les capacites des internes a prendre en charge les patients atteints de pathologies respiratoires.

48. Mise en place d’une unité de soins intensifs respiratoire temporaire lors du premier pic épidémique de la COVID-19

Abstract: Introduction Lors du premier pic épidémique de la COVID-19, les capacités d’accueil en réanimation ont été dépassées, conduisant à une augmentation du nombre de lits. À l’hôpital Avicenne, l’augmentation de 16 à 40 lits de réanimation n’a pas permis de répondre complètement à la demande. Cela a conduit à la mise en place d’une unité de soins intensifs respiratoires (USIR) de 10 lits à l’aide de matériel d’assistance respiratoire de domicile. Notre objectif est de décrire les modalités de mise en place de cette unité. Méthodes Les lits d’USIR ont été localisés au sein d’un service d’orthopédie avec une infirmière pour 5 patients. Chaque chambre a été équipée d’un ventilateur Trilogy Evo (Philips-Respironics) mis à disposition par Asten Santé. L’oxygénothérapie à haut débit était délivrée à l’aide de systèmes de domicile Airvo2 (Fisher-Paykel). Chaque chambre a été équipée d’un système de surveillance continue développé spécifiquement pour cette USIR. Le système de surveillance comprenait un oxymètre de pouls (Nonin Xpod) connecté au ventilateur. À l’aide d’un driver co-développé par Philips et Kernel Biomedical, le ventilateur transmettait les données en temps réel à un ordinateur (Raspeberry Pi). L’ordinateur localisé dans la chambre transmettait en temps réel les données d’oxymétrie et de la ventilation (volume courant, fuites, fréquence respiratoire, asynchronismes) à un serveur de surveillance accessible depuis l’ensemble des postes informatiques du service et sur lequel des alarmes sonores et visuelles pouvaient être réglées de manière individuelle (Fig. 1). Résultats De l’identification du besoin à la mise en place de l’USIR, 12 jours se sont écoulés. Un total de 38 patients a été admis au cours du mois d’avril. Les personnels paramédicaux ont reçu une formation dédiée à la prise en charge des patients de cette USIR notamment relative aux modalités techniques. Une permanence des soins sur place était assurée par l’équipe de pneumologie 24 h/24. Le nombre moyen d’alarmes de désaturation ( 40 L/min) était de 6,7 ± 12,9/h, soulignant la sévérité des patients admis. Aucun événement indésirable grave lié à l’utilisation du matériel ou au système de monitoring n’est survenu. Conclusion La mise en place d’une USIR utilisant le matériel d’assistance respiratoire de domicile a permis d’augmenter la capacité d’accueil de patients atteints de COVID-19 très sévère sans s’accompagner d’événement indésirable grave.
