The elderly individual is becoming the most common patient in clinical practice due to the increasing average life span, especially in developed countries. The current elderly patients are different from those of some decades ago. They usually have an active social life, want to be informed, and are actively involved in the current society with consequent high expectations for medical and dental treatment. However, not all the elderly patients are like this. Some of them show limited financial resources, reduced mental and manual skills, and poor motivation. The purpose of this communication is to make a brief characterization of the new elderly population in view of prosthetic and dental management.

A treatment plan based on the use of a preview software can offer the possibility to rapidly communicate with the patient. Fully digital workflow allows for making several objects at the same time in a precise and cost‐efficient manner.

The purpose of this article is to show how to implement an implant-supported prosthetic overdenture using a digital workflow. Esthetic previewing using a specific software, guided-surgery, construction of the prosthesis, and the esthetic finalization are described in this article. Patients suffering from severe loss of bone and soft tissue volume could benefit from the construction of an overdenture prosthesis as a feasible therapeutic choice for functional and esthetic issues of the patient.

This study aimed to evaluate the production of customized composite veneers starting from a two-dimensional (2D) digital preview using the Digital Smile System (DSS). MATERIAL AND METHODS: A photographic examination of 30 patients was performed by taking two digital pictures of the face and a digital preview through the DSS. Moreover, optical scans of the dental arches were obtained and the data were entered into a three-dimensional (3D) software to prepare a virtual preview. The standard tessellation language files were sent for production using CAD-CAM technology. The Friedman test, Bonferroni, and Dunn post hoc tests were used, comparing the linear measurements of the 2D and 3D plans and the final veneers (α = .05).Significant differences emerged between the pictures and digital scans on the mesial-distal widths of the lateral incisors and canine. Linear measurements in the 2D plan were significantly different from those of the 3D plan, except for the height measures of incisors. No significant changes were found on comparing the parameters of the 2D and 3D plans with those of the final pieces.The customized veneers were clinically adequate and similar to 2D and 3D plans, although significant differences emerged between the picture and digital scans as well as between the 2D and 3D plans.

BACKGROUND To compare the accuracy of 3 different devices for facial images acquisition, useful for prosthodontic esthetic preview. METHODS Bidimensional extraoral photographs (Nikon D300), facial scans (Bellus3D) and 3D digital stereophotogrammetry photos (3dMD Face System) were acquired from 15 patients. The intraoral impressions of all subjects were digitally taken through a scanner (i500; Medit). Files obtained from each acquisition method were transferred on Exocad Software, and the measurements of the frontal teeth were performed and compared with those of the intraoral scans, assumed as reference. Background: The prosthetic preparation of the teeth for ceramic laminate veneers has to follow the minimally invasive concept brought by the modern Conservative Dentistry and Prosthodontics. However, during the cementation phase under the rubber dam, the loss of the esthetics landmarks could lead to errors in the future positioning of the laminate veneers. Methods: In this article the authors show an accurate operative prosthetic protocol using different fine intraoperative maneuvers and tricks for the realization of ceramic laminates in order to solve the problems of the cementation phase. Results: The treatment of the anterior sector of the upper maxilla with porcelain laminate veneers was realized in a 30 years old woman with aesthetic issues. Conclusion: Different fine intraoperative maneuvers and tricks during teeth preparation, master impression and rubber dam positioning could reduce errors occurring in the cementation phase and increase the predictability of the results.

Background When used with deproteinized bovine bone mineral (DBBM) delivered as a particulate, the sub-periosteal peri-implant augmented layer (SPAL) technique was effective in completely correcting up to 92% of peri-implant buccal bone dehiscences. The use of a DBBM block (bDBBM), however, may result in an improvement of the peri-implant bone dehiscence as well as a relevant lateral bone augmentation since its mechanical properties may ensure a better dimensional stability at flap manipulation than particulate DBBM. The aim of the present a proof-of-principle case report is to investigate if SPAL may be successfully used to obtain bone augmentation at peri-implant dehiscence sites when used with bDBBM. Case presentation Lateral bone augmentation was performed using the SPAL technique at two implants showing a buccal peri-implant bone dehiscence immediately after their placement. A partial-thickness flap was elevated, leaving the periosteal layer on the buccal cortical bone plate. The periosteal layer was, in turn, elevated to create a pouch, which was used to stabilize a bDBBM graft at the peri-implant buccal bone dehiscences. At re-entry, exposed implant surfaces were completely covered by new thick hard tissue up to their most coronal portion. A free epithelial-connective tissue graft was used to augment the peri-implant soft tissue phenotype. Conclusion When used to accommodate bDBBM over the most coronal portion of an exposed implant, SPAL may successfully lead to an increase in peri-implant buccal tissue thickness.

Background Mock-up based approach allows the preview of the aesthetic rehabilitation, however, it is crucial that the mock-up does not differ from the expected aesthetic outcomes. With CAD-CAM technologies, it is possible to directly create mock-ups from virtual planned smile project, with greater accuracy and efficiency compared to the conventional moulded mock-ups. In this study, we investigated the trueness of mock-ups obtained with milling and 3D printing technology and a full digital work-flow system. Methods Ten adults subjects were included and digital smile design/digital wax-up were performed to enhance the aesthetic of maxillary anterior region. Ten milled mock-ups and 10 prototyped mock-ups were obtained from the original .stl file and a digital analysis of trueness was carried out by superimposing the scanned-milled mock-ups and the scanned-prototyped mock-ups to the digital wax-up, according to the surface-to-surface matching technique. Specific linear measurements were performed to investigate and compare the dimensional characteristics of the physical manufactures, the 3D project and the scanned mock-ups. All data were statistically analyzed. A clinical test was also performed to assess the fitting of the final manufacture. Results The prototyped mock-ups showed a significant increment of the transversal measurements (p p p Conclusion Both prototype and milled mock-ups showed a slight dimensional increment comparing to the original 3D project, with milled-mock-ups showing less fitting after clinical tests. Caution must be taken when assessing the trueness of scanned manufacture since an intrinsic error in the system can underestimate the dimensions of the real object.

Background. Prosthetic rehabilitation of patients with atrophic arches can be very challenging. Purpose. The aim of the present prospective multicenter study was to report the two-year preliminary data on clinical and radiographic performance of implant-supported overdentures with a metal bar and low-profile attachments. Material and Methods. A computer-aided design/computer-aided manufacturing (CAD/CAM) titanium bar or a conventional cast metal bar was fabricated according to the anatomy of the ridge, prosthetic contours (teeth setup), and implant position. Three to six threadable OT Equator attachments (Rhein 83, Bologna, Italy) were placed along the implant bar. A cobalt-chromium alloy metal framework was fabricated and fitted onto the metal bar as a counterpart. Prosthetic survival rate, biologic and technical complications, peri-implant bone loss, changes in oral health impact profile index, bleeding on probing, and plaque index were reported. Results. Overall, 177 implants were placed (range three to six) to support 43 metal bars with 170 OT Equators (Rhein 83, range three to six). Eleven metal bars were fabricated using CAD/CAM technology, while the other 32 were conventionally produced using cast technique. All the participants were followed up for at least two years (mean 42.2 months, range 24–88 months) after prosthesis delivery. Two maxillary implants failed in one nonsmoking patient (1.1%). The 2-year prosthesis survival rate was 97.7%. Only three minor technical complications were reported. Two years after loading, the mean marginal bone loss was 0.22 ± 0.09 mm (95% CI: 0.16 to 0.26). Two years after loading, OHIP was 22.3 ± 7.1 (95% CI from 17.4 to 24.6). Compared to the baseline, the difference was statistically significant ( P ≤ 0.001 ). At the two-year follow-up session, successful periodontal parameters were experienced. Conclusions. Implant overdenture supported by a CAD/CAM titanium bar may be a reliable option for the treatment of the edentulous arch over a 2-year period. Oral health-related quality of life significantly improved in all treated participants.

Purpose: To evaluate implant and prosthetic survival rates, complications, patient satisfaction, and biological outcomes of patients rehabilitated with a ball attachment system for implant retained- and supported-overdentures (IOV), which was in function for 3 to 5 years. Methods: This retrospective study evaluated data collected from patients treated between April 2001 and May 2018 with IOV on splinted and non-splinted implants and a ball attachment system. Patients were followed for 36 to 206 months (mean follow-up was 128.1 &plusmn, 51.9 months). Data were collected at the 3- and 5-year follow-up examination. Outcome measures were implant and prosthetic survival rates, technical complications, marginal bone loss (MBL), oral health impact profile (OHIP), and periodontal parameters (bleeding on probing and plaque index). Results: A total of 46 patients (16 males and 30 females) with 124 implants were included in this study. Twenty-five implant-retained overdentures were delivered on 53 unsplinted implants, while the other 21 patients received an implant-supported overdentures and the implants were splinted. At the five-year follow-up examination, one implant and one prosthesis failed in the unsplinted group, resulting in a cumulative survival rate of 97.8% at the patient level. Two minor technical complications were experienced. Conclusions: Implant overdenture retained or supported by ball attachment systems showed high implant and prosthetic survival and success rates. A low number of complications, high patient satisfaction, and successful biological parameters were experienced in the mid-term follow-up. Data need to be confirmed by further randomized trials.

Purpose: To analyze implant and prosthetic survival rates, complications, patient satisfaction, and biological parameters of patients rehabilitated with implant overdentures (IOV) on splinted and nonsplinted implants and different attachment systems, in function for one to 17 years. Methods: This retrospective study evaluated data collected from patients rehabilitated with implant overdentures between January 2001 and December 2016 in nine different centers. Outcome measures were implant and prosthetic success rates, mechanical complications, marginal bone loss (MBL), oral health impact profile (OHIP), bleeding on probing, and plaque index. Results: A total of 581 implants were installed in 194 patients. Patients were followed for a mean period of 60.6 months (range 6&ndash, 206). Eighty-nine patients received 296 low profile attachment (OT Equator), 62 patients received 124 ball attachments, and 43 patients received 107 Locator attachments. In eighty-three patients the implants were splinted with computer aided design/computer aided manufacturing (CAD/CAM) or casted bar. At the last follow-up, 10 implants failed in eight patients. Statistical significance was found for failed prostheses (P = 0.0723) and complications (P = 0.0165), with better values for splinted implants. No statistically significant differences were found in proportion of implant and prosthetic failure (P &gt, 0.05). At a five-year follow-up, proportion of complications (P = 0.0289) and failed prostheses (P = 0.0069) were statistically higher for IOV on Locator attachments. No difference was founded in MBL at one- and two-year follow-up between different attachment systems (P &gt, 0.05). Statistically significant improvement in all the OHIP categories was reported in all the patients, after one year of function. Conclusions: Implant overdenture showed high implant and prosthetic survival rates, low complications, high patient satisfaction, and good biological parameters in the long-term follow-up. Splinting the implants may reduce number of mechanical complications. Locator attachments showed higher number of complications. Further studies are needed to confirm these preliminary results.

The use of a custom tray is highly desirable for accurate impression making in situations with multiple abutments. This article describes a modified custom tray fabrication method using autopolymerizing acrylic resin. The custom tray is fabricated by intraoral relining with autopolymerizing resin that is polymerized extraorally. The final impression is obtained during the same session after tray polymerization at 100°C for 5 minutes. Relined areas are refined by trimming excess resin with burs of a known diameter to create a 2-mm clearance for the elastomer. For areas with subgingival finish lines, only 0.5 mm of resin is removed to direct the elastomer into the gingival sulcus. The procedure is time-saving because it reduces the need for a retraction cord and minimizes inaccuracies that would necessitate another impression. (J Prosthet Dent 2000;84:237-40.)
