There is a long history of regarding marginalised groups as unfit to parent and of eugenic policies targeting those with 'undesirable' bodily conditions or behaviours. This is part of a broader pattern of stratified reproduction – structural conditions that enable or discourage certain groups from reproducing – that often brings about and exacerbates injustices. This paper critically assesses the US and UK social and medical literature on applying pressure to marginalised groups, or those who have behaved 'irresponsibly', to use long-acting reversible contraception (LARC). Targeting young people for LARC fails to recognise that social inequality is the context for teenage pregnancy, not the result of it. Provider pressure on women of colour to use LARC is linked to institutional racism, whilst policy for those with physical and intellectual disabilities is shaped by disability discrimination. Other groups to be targeted include so-called 'welfare mothers', substance users, those who have had children put into care and offenders. Particularly controversial are cases in which LARC has been ordered by courts. LARC policy incorporating these kind of discriminatory practices needs to stop; future policy should focus on person-centred care that bolsters reproductive justice. [ABSTRACT FROM AUTHOR]

This article examines everyday effects of austerity in Kingshurst – a disadvantaged urban neighbourhood in the West Midlands. It draws on qualitative data gathered from local families with children, and public and third sector professionals working in the area in family support services. While some of the issues raised are common to other disadvantaged communities across the UK, we recognise that austerity is experienced in specific socio-spatial contexts: in this case, Kingshurst's circumstance of deprivation within a local authority borough that (as a whole) is above averagely affluent. This shaped the ways that residents and professionals framed the disadvantage they encountered in their everyday lives and work, in particular strengthening understandings of austerity as unfairly and unevenly experienced on the bases of geography and social class, and highlighting territorial stigma towards the neighbourhood by professionals and decision-makers which impeded residents' engagement with the family support services available to them locally. This novel paper critically addresses a currently popular sex education resource which compares sexual consent to tea drinking. Drawing from a study which considered the meaning of consent through focus groups and interviews with young people and professionals, we argue that the central 'risk avoidance' message of such resources individualises the potential risks of non‐consensual sex and ignores the gendered social structures which shape interpersonal relationships. We suggest that simplifying and extrapolating sexual consent from broader cultural understandings is problematic. Objectives: The internet has changed the organisation of sex work. The risk of sexually transmitted infections has frequently been a research focus, but less is known about sex workers' use of contraception for pregnancy prevention. The aim of this research was to gain a better understanding of contraceptive preferences and provider interactions of online sex workers.Methods: Data were obtained from a multi-methods study of sex workers in the UK who advertise on the internet and have sexual contact with clients, particularly in the Birmingham and Solihull areas. The study comprised an online survey among 67 participants and eight qualitative interviews.Results: Reported high rates of condom use with clients led to sex workers considering pregnancy prevention to be a personal rather than an occupational issue. Disclosure of sex working to health professionals is often seen as unnecessary and/or undesirable due to concerns about stigma. A clear distinction between contraceptive needs for commercial and non-commercial partners was evident.Conclusion: Service providers need to take account of both personal and commercial needs during contraceptive consultations and avoid making assumptions based on homogeneous understandings of sex work. Across the UK, there has been an increase in anti-abortion activism outside abortion clinics. The activism deployed includes explicitly religious activities such as 'prayerful witnessing' and 'pavement counselling', which aim to discourage women from entering clinics. This article stems from a wider ethnographic study of public activism over abortion to determine what claims about motherhood are being made within these debates. Two arguments are presented. First, how women's role as mothers is central and essentialised in anti-abortion discourses, with the body of the mother often disappearing as activists seek to erode the distinction between a foetus and a baby by constructing pregnancy as a foetal environment. Motherhood is constructed as 'natural' and sacred, therefore abortion must be damaging because it destroys women's 'natural' position. Second, the article argues that although the activists' arguments are always religiously framed, their activism takes place in a largely secular context, meaning that they have to find ways of appealing to secular audiences. This leads to a complex interrelationship between secular and religious discourses, where theological viewpoints sit alongside 'scientific' claims to buttress activists' views. This article explores how the presence and absence of mothers within activists' narratives is due to the tensions between religiously based understandings of motherhood, and the need to appeal to a secular audience. In the UK, there is evidence of a recent increase in anti-abortion activism outside clinics. In response, abortion service providers have called for the introduction of 'buffer' zones to protect women from 'harassment' while accessing abortion services. Drawing on two datasets – extensive ethnographic fieldwork, and a content analysis of clinic client comment forms – we deploy Goffman's concept of 'civil inattention' to further our understanding of the material practice of anti-abortion clinic activism. We find that although anti-abortion activists understand their own actions to be supportive, practices of religious observance outside clinics inescapably draw attention to the site and to the act of accessing healthcare, inherently challenging normative expectations of privacy and confidentiality. Our analysis suggests that anti-abortion activism outside clinics consequently violates social rules governing encounters with strangers in specific places and reinforces gendered hierarchies. In the UK, there has been growing concern about young people’s understanding of sexual consent, with the views of young people themselves often lost in academic and educational policy debates. However, the focus on high rates of sexual violence has meant a lack of attention on the everyday negotiation of consensual heterosexual activity, leading to assumptions being made regarding young people’s lack of understanding of sexual consent. This paper emerges from a wider study of over 500 young people which sought to uncover their understanding of the issues. Drawing on data from workshops and the open text responses to an on-line survey the findings presented in this paper show that the majority of heterosexual young people understood the complexity of sexual consent as an embodied process, which can be difficult to define, talk about or practice uniformly. This complex understanding, in which sexual consent is a continuum rather than a dichotomy, has implications for sexual education initiatives. We argue that it is only by providing a closer understanding of how – within consensual sexual activities – young people understand and enact sexual consent through a range of embodied communication strategies that education surrounding sexual assault will become meaningful. In recent years, English welfare and health policy has started to include pregnancy within the foundation stage of child development. The foetus is also increasingly designated as ‘at risk’ from pregnant women. In this article, we draw on an analysis of a purposive sample of English social and welfare policies and closely related advocacy documents to trace the emergence of neuroscientific claims-making in relation to the family. In this article, we show that a specific deterministic understanding of the developing brain that only has a loose relationship with current scientific evidence is an important component in these changes. We examine the ways in which pregnancy is situated in these debates. In these debates, maternal stress is identified as a risk to the foetus; however, the selective concern with women living in disadvantage undermines biological claims. The policy claim of neurological ‘critical windows’ also seems to be influenced by social concerns. Hence, these emerging concerns over the foetus’ developing brain seem to be situated within the gendered history of policing women’s pregnant bodies rather than acting on new insights from scientific discoveries. By situating these developments within the broader framework of risk consciousness, we can link these changes to wider understandings of the ‘at risk’ child and intensified surveillance over family life. Despite growing interest in learning and teaching as emotional activities, there is still very little research on experiences of sensitive issues. Using qualitative data from students from a range of social science disciplines, this study investigates student's experiences. The paper highlights how, although they found it difficult and distressing at times, the students all valued being able to explore sensitive issues during their studies. The paper argues that it is though repeated exposure to sensitive issues within the classroom that the students became more comfortable with the issues. This process of lessening sensitivity is an important part of the emotional journey through higher education. It will argue that good student experiences need not always be positive emotions and that sensitive issues should be seen as an important part of transformational education. Women are actively encouraged to educate themselves about pregnancy from formal sources (e.g., information leaflets, antenatal classes, books). In addition, informal stories of pregnancy and birth are routinely told between women. However, increased prenatal testing means that more fetuses are diagnosed with abnormalities, shifting the information requirements during pregnancy. Traditional sources of information cannot cover all possible outcomes, and the Internet is beginning to fill this gap. In this article, we draw from interviews about experiences of antenatal screening and pregnancy to explore how the Internet provides a unique resource for problematic pregnancies. It allows access to information about rarer conditions beyond standard pregnancy texts, as well as personal narratives about conditions. Learning how others have coped or are coping in similar situations can help alleviate feelings of isolation, and also places women back in a familiar territory of shared pregnancy narratives. This paper argues that sleep disruption is both a strategy and an effect of violence and abuse which profoundly affects the lives of women and children. This paper traces the interconnections between the patterns of sleeping (not sleeping) for women and children living with and recovering from the effects of violence and abuse. It highlights the threat to the emotional and physical well-being of children and women and provides a non-pathologizing route into an exploration of one of the symptoms of trauma. It is based on a pilot study which interviewed 17 women, 14 of whom were mothers to 28 children. Mothers reported that many of their children experienced nightmares, bed-wetting, night panics and disrupted sleep patterns. The management of sleep is embedded within the social context of individuals' lives. This article is based on an exploratory study using focus groups of the sleep problems encountered by 17 women survivors of domestic violence. It argues that fear becomes the organizing framework for the management of sleep and illustrates how this takes place both while living with the perpetrators of violence and after the women have been rehoused. It argues that sleep deprivation is a method used by the perpetrators to exert control over women and that this has long-term implications for women's physical and mental health. This research, based on qualitative interviews and non-participant observation, emerges from a larger study investigating what factors influence the `contraceptive careers' of British women in their 30s. The women informants recognized that contraceptive products often impacted on their health, but viewed them as distinct from `medical matters'. Rather than doctors being seen as having expertise, it was women health professionals, be they nurses, midwives, health visitors or doctors, who were perceived as the ones who `know' about contraception, through an assumption that they are contraception users. This embodied knowledge is valued by the women above their formal medical training. I will also show how general practice surgeries and family planning clinics were viewed as gendered spaces, which altered the expectations and experiences of the women during contraceptive consultations. This study found that as `real' expertise over contraception stems from embodied rather than textual knowledge, the women's choices were grounded by a gendered sense of trust. [ABSTRACT FROM AUTHOR]

The author focuses on the link between contraception and heterosexual practices, while researching the idea that men are an "absent presence" in women's contraceptive decisions. The women in this study view 'having sex' as penile-vaginal intercourse and use contraception primarily to prevent pregnancy. The methods of contraception, coital-related and independently used protection such as the IUD, can produce power relationships in 'normalized' heterosexuality. The author cites research on how sexuality is reconstructed within relationships as well as in the broader social context, including Jackson, 1999 and Petchesky (1986).

This paper is based on qualitative research which found that the contraceptive pill had achieved a “hegemonic status” among some British women in their thirties. In addition, despite the risk of sexually transmitted diseases, the idea of using condoms was very unpopular, and the research suggests that this is linked to a reluctance to rely on male cooperation over contraception. This paper will further argue that the women generally chose methods that they felt would be in their own best interests, and were often exercising considerable agency within the constraints of their relationships. Moreover, by accepting the responsibility for contraception, the women not only gained sole control over their fertility, but contraception may be an area within heterosexuality where women can exercise power. In the UK, narratives about saved women and babies have been a significant feature in anti-abortion campaigns to oppose bufferzones, which seek to prevent anti-abortion activists from being directly outside abortion clinics. Anti-abortion activists argue that without their presence, the 'abortion industry' will fail to seek 'informed' consent and coerce women into abortions. These presumptions are based on positioning abortion, and service providers, as inherently 'evil', whilst saving is rooted in Christian beliefs aimed at ensuring that souls go to heaven. This paper emerges from ethnographic research in Britain on anti-abortion activism and pro-choice counter-demonstrations. Highlighting the importance of saving in many of the anti-abortion campaigns, it will argue that as they have a different conceptualisations of harm to pro-choice activists, this leads to a lack of understanding of each other's saving narrative. We assessed the feasibility of developing a virtual diabetes clinic for young people, using the Internet. The proposed content of the site would be based on self-efficacy theory, aiming to develop confidence in self-management of diabetes. A questionnaire about the proposed Website was delivered to 72 patients who attended a young person's outpatient clinic in a district hospital. Thirty-nine replies were received, a response rate of 54%. The mean age of the respondents was 13 years. A positive attitude to the potential clinic was reported by 95% of respondents. The items rated as most useful were: quick and easy access to up-to-date information about diabetes; the opportunity to ask an expert; good graphics; easy navigation; interactivity. Responses to open questions indicated that 24-hour access and anonymity in asking questions were also valued. This article discusses the findings of a study tracing the incorporation of claims about infant brain development into English family policy as part of the longer term development of a ‘parent training’, early intervention agenda. The main focus is on the ways in which the deployment of neuroscientific discourse in family policy creates the basis for a new governmental oversight of parents. We argue that advocacy of ‘early intervention’, in particular that which deploys the authority of ‘the neuroscience’, places parents at the centre of the policy stage but simultaneously demotes and marginalises them. So we ask, what becomes of the parent when politically and culturally, the child is spoken of as infinitely and permanently neurologically vulnerable to parental influence? In particular, the policy focus on parental emotions and their impact on infant brain development indicates that this represents a biologisation of ‘therapeutic’ governance. [ABSTRACT FROM AUTHOR]

The role of applied theatre in engaging both lay and professional publics with debate on health policy and practice is an emergent field. This paper discusses the development, production performance and discussion of 'Inside View'. The objectives were to produce applied theatre from research findings of a completed study on genetic prenatal screening, exploring the dilemmas for women and health professionals of prenatal genetic screening, and to engage audiences in debate and reflection on the dilemmas of prenatal genetic screening. 'Inside View' was developed from a multidisciplinary research study through identification of emergent themes from qualitative interviews, and development of these by the writer, theatre producer and media technologist with input from the researchers. Inside View was performed in London and the Midlands to varied audiences with a panel discussion and evaluation post performance. The audiences were engaged in debate that was relevant to them professionally and personally. Knowledge translation through applied theatre is an effective tool for engaging the public but the impact subsequently is unclear. There are ethical issues of unexpected disclosure during discussion post performance and the process of transforming research findings into applied theatre requires time and trust within the multidisciplinary team as well as adequate resourcing. Dementia is one of the greatest contemporary health and social care challenges, and novel approaches to the care of its sufferers are needed. New information and communication technologies (ICT) have the potential to assist those caring for people with dementia, through access to networked information and support, tracking and surveillance. This article reports the views about such new technologies of 34 carers of people with dementia. We also held a group discussion with nine carers for respondent validation. The carers' actual use of new ICT was limited, although they thought a gradual increase in the use of networked technology in dementia care was inevitable but would bypass some carers who saw themselves as too old. Carers expressed a general enthusiasm for the benefits of ICT, but usually not for themselves, and they identified several key challenges including: establishing an appropriate balance between, on the one hand, privacy and autonomy and, on the other : maximising safety ; establishing responsibility for and ownership of the equipment and who bears the costs ; the possibility that technological help would mean a loss of valued personal contact ; and the possibility that technology would substitute for existing services rather than be complementary. For carers and dementia sufferers to be supported, the expanding use of these technologies should be accompanied by intensive debate of the associated issues. [ABSTRACT FROM AUTHOR]

Domestic violence is everywhere and nowhere. No statutory organization or health service has work with either perpetrators or survivors of domestic violence (usually women and children) as the primary focus of their service, yet all agencies will have very significant numbers among their clients/service users. It is therefore crucial that the policy framework is developed both within and between agencies to address the need, and scope, of intervention in this area and particularly the impact on children. Currently, significant steps have been taken by some agencies in the UK to address this previously neglected issue, though the developments are patchy. This paper draws on a UK-wide research study which mapped the extent and range of service provision for families where there is domestic violence and also developed a framework of good practice indicators for provision in this area. This article examines one of the indicators of good practice arising from the research—that of policy development—within social service departments and within the multi-agency arena. The article examines how issues relating to sleep, sleep deprivation, and the relationship between sleep and work were treated in British print news media from 1984-2005. A trend is found in which sleep or the lack thereof is often treated as a symbol of work/life imbalances created by social and economic changes created by the capitalist economy and the so-called "24/7" world of commerce.

What role do the media play in the medicalization of sleep problems? This article, based on a British Academy funded project, uses qualitative textual analysis to examine representations of insomnia and snoring in a large representative sample of newspaper articles taken from the UK national press from the mid-1980s to the present day. Constructed as 'common problems' in the population at large, insomnia and snoring we show are differentially located in terms of medicalizing-healthicizing discourses and debates. Our findings also suggest important differences in the gendered construction of these problems and in terms of tabloid and 'broadsheet' newspaper coverage of these issues. Abstract: Medicalisation, healthicisation and ‘personal’ strategies have been identified as the main factors contributing to the socially mediated experience of sleep and sleep disorders in modern societies. Medicalisation and healthicisation are publicly available discourses. But the degree to which apparently ‘personal’ strategies for managing sleep are presented in popular media has been underestimated. This study of the coverage of 5 UK newspapers shows that both medicalised and healthicised discourses are concentrated in the ‘serious’ press. The tabloid press is more likely to constitute sleep as a private realm and tabloid readers are therefore relatively less exposed to officially sanctioned forms of knowledge about sleep. Analysis of Daily Mail coverage shows, though, that women''s ‘personal’ strategies for managing sleep are far from being private solutions. The Mail presents this topic as a component of its social construction of a ‘Middle England’ lifestyle, giving these apparently ‘personal’ solutions a political resonance. [Copyright &y& Elsevier]
