Abstract

Le panorama des sarcomes utérins se complexifie de plus en plus avec la description de nouvelles entités associées à des altérations moléculaires récurrentes. Les léiomyosarcomes et les sarcomes utérins indifférenciés sont des sarcomes à génomique complexe. Les sarcomes du stroma endométrial de bas grade et de haut grade, d’autres sarcomes associés aux transcrits de fusion (tels que NTRK, PDGFB, ALK, RET ROS1) et le sarcome utérin déficient en SMARCA4 sont des sarcomes à génomique simple. Le léiomyosarcome est le sarcome utérin le plus fréquent suivi par les sarcomes du stroma endométrial. Il existe trois différents sous-types histologiques de léiomyosarcome (fusiforme, myxoïde, épithélioïde), les léiomyosarcomes myxoïde et épithélioïde étant plus agressifs que le léiomyosarcome fusiforme. La distinction entre un sarcome du stroma endométrial de bas grade et de haut grade est tout d’abord morphologique et immunohistochimique. La mise en évidence des transcrits de fusions aide au diagnostic. Définitivement reconnu comme entité distincte parmi les tumeurs mésenchymateuses gynécologiques, le PECome utérin fait partie des tumeurs rares. Grâce à des algorithmes d’évaluation du risque, les PEComes utérins sont différentiés en borderlineet en malins. Le rhabdomyosarcome embryonnaire du col utérin survient typiquement chez l’enfant, mais peut s’observer également chez la femme adulte. Le rhabdomyosarcome embryonnaire du col utérin est presque toujours DICER1 muté, à la différence de celui du vagin qui est DICER1 sauvage, et de l’adénosarcome qui peut être DICER1 muté mais en moindre fréquence. Parmi les entités émergentes, les sarcomes associés aux transcrits de fusion impliquant les gènes NTRK, ALK, PDGFBbénéficient d’une thérapeutique ciblée. L’intégration des données moléculaires avec l’histologie et la clinique permet de mieux identifier les sarcomes utérins afin de mieux les traiter.

Abstract

Simple Summary: Since the publication in 2016 of the WHO's classification of primary brain tumors according to their histopathology but also their molecular status (IDH, 1p/19q codeletion), oligodendrogliomas defined by the presence of the 1p/19q codeletion have been clearly identified as having a better prognosis. However, the response to treatment of 1p/19q codeleted gliomas remains heterogeneous. Very few studies have investigated the genetic profiles of these tumors, particularly with regard to their response to treatment (radiotherapy and chemotherapy). Our analyses revealed a gene signature composed of eight genes involved in metabolism, immunity, and extracellular matrix organization pathways that were associated with a poor response to treatment for 1p/19q codeleted tumors. This signature could be used in the future to identify patients who need more intensive treatment, potentially with inhibitors of these pathways. Background. IDH mutant and 1p/19q codeleted oligodendrogliomas are the gliomas associated with the best prognosis. However, despite their sensitivity to treatment, patient survival remains heterogeneous. We aimed to identify gene expressions associated with response to treatment from a national cohort of patients with oligodendrogliomas, all treated with radiotherapy +/− chemotherapy. Methods. We extracted total RNA from frozen tumor samples and investigated enriched pathways using KEGG and Reactome databases. We applied a stability selection approach based on subsampling combined with the lasso-pcvl algorithm to identify genes associated with progression-free survival and calculate a risk score. Results. We included 68 patients with oligodendrogliomas treated with radiotherapy +/− chemotherapy. After filtering, 1697 genes were obtained, including 134 associated with progression-free survival: 35 with a better prognosis and 99 with a poorer one. Eight genes (ST3GAL6, QPCT, NQO1, EPHX1, CST3, S100A8, CHI3L1, and OSBPL3) whose risk score remained statistically significant after adjustment for prognostic factors in multivariate analysis were selected in more than 60% of cases were associated with shorter progression-free survival. Conclusions. We found an eight-gene signature associated with a higher risk of rapid relapse after treatment in patients with oligodendrogliomas. Abstract

The use of radiolabeled non-natural amino acids can provide high contrast SPECT/PET metabolic imaging of solid tumors. Among them, radiohalogenated tyrosine analogs (i.e., [123I]IMT, [18F]FET, [18F]FDOPA, [123I]8-iodo-L-TIC(OH), etc.) are of particular interest. While radioiodinated derivatives, such as [123I]IMT, are easily available via electrophilic aromatic substitutions, the production of radiofluorinated aryl tyrosine analogs was a long-standing challenge for radiochemists before the development of innovative radiofluorination processes using arylboronate, arylstannane or iodoniums salts as precursors. Surprisingly, despite these methodological advances, no radiofluorinated analogs have been reported for [123I]8-iodo-L-TIC(OH), a very promising radiotracer for SPECT imaging of prostatic tumors. This work describes a convenient synthetic pathway to obtain new radioiodinated and radiofluorinated derivatives of TIC(OH), as well as their non-radiolabeled counterparts. Using organotin compounds as key intermediates, [125I]5-iodo-L-TIC(OH), [125I]6-iodo-L-TIC(OH) and [125I]8-iodo-L-TIC(OH) were efficiently prepared with good radiochemical yield (RCY, 51–78%), high radiochemical purity (RCP, >98%), molar activity (Am, >1.5–2.9 GBq/µmol) and enantiomeric excess (e.e. >99%). The corresponding [18F]fluoro-L-TIC(OH) derivatives were also successfully obtained by radiofluorination of the organotin precursors in the presence of tetrakis(pyridine)copper(II) triflate and nucleophilic [18F]F− with 19–28% RCY d.c., high RCP (>98.9%), Am (20–107 GBq/µmol) and e.e. (>99%).
