Introduction Some local protocols suggest using intermediate or therapeutic doses of anticoagulants for thromboprophylaxis in hospitalized patients with coronavirus disease 2019 (COVID‐19). However, the incidence of bleeding, predictors of major bleeding, or the association between bleeding and mortality remain largely unknown. Methods We performed a cohort study of patients hospitalized for COVID‐19 that received intermediate or therapeutic doses of anticoagulants from March 25 to July 22, 2020, to identify those at increased risk for major bleeding. We used bivariate and multivariable logistic regression to explore the risk factors associated with major bleeding. Results During the study period, 1965 patients were enrolled. Of them, 1347 (69%) received intermediate‐ and 618 (31%) therapeutic‐dose anticoagulation, with a median duration of 12 days in both groups. During the hospital stay, 112 patients (5.7%) developed major bleeding and 132 (6.7%) had non‐major bleeding. The 30‐day all‐cause mortality rate for major bleeding was 45% (95% confidence interval [CI]: 36%‐54%) and for non‐major bleeding 32% (95% CI: 24%‐40%). Multivariable analysis showed increased risk for in‐hospital major bleeding associated with D‐dimer levels >10 times the upper normal range (hazard ratio [HR], 2.23; 95% CI, 1.38–3.59), ferritin levels >500 ng/ml (HR, 2.01; 95% CI, 1.02–3.95), critical illness (HR, 1.91; 95% CI, 1.14–3.18), and therapeutic‐intensity anticoagulation (HR, 1.43; 95% CI, 1.01–1.97). Conclusions Among patients hospitalized with COVID‐19 receiving intermediate‐ or therapeutic‐intensity anticoagulation, a major bleeding event occurred in 5.7%. Use of therapeutic‐intensity anticoagulation, critical illness, and elevated D‐dimer or ferritin levels at admission were associated with increased risk for major bleeding.

Background Data monitoring of clinical trials is a tool aimed at reducing the risks of random errors (e.g. clerical errors) and systematic errors, which include misinterpretation, misunderstandings, and fabrication. Traditional ‘good clinical practice data monitoring’ with on-site monitors increases trial costs and is time consuming for the local investigators. This paper aims to outline our approach of time-effective central data monitoring for the SafeBoosC-III multicentre randomised clinical trial and present the results from the first three central data monitoring meetings. Methods The present approach to central data monitoring was implemented for the SafeBoosC-III trial, a large, pragmatic, multicentre, randomised clinical trial evaluating the benefits and harms of treatment based on cerebral oxygenation monitoring in preterm infants during the first days of life versus monitoring and treatment as usual. We aimed to optimise completeness and quality and to minimise deviations, thereby limiting random and systematic errors. We designed an automated report which was blinded to group allocation, to ease the work of data monitoring. The central data monitoring group first reviewed the data using summary plots only, and thereafter included the results of the multivariate Mahalanobis distance of each centre from the common mean. The decisions of the group were manually added to the reports for dissemination, information, correcting errors, preventing furture errors and documentation. Results The first three central monitoring meetings identified 156 entries of interest, decided upon contacting the local investigators for 146 of these, which resulted in correction of 53 entries. Multiple systematic errors and protocol violations were identified, one of these included 103/818 randomised participants. Accordingly, the electronic participant record form (ePRF) was improved to reduce ambiguity. Discussion We present a methodology for central data monitoring to optimise quality control and quality development. The initial results included identification of random errors in data entries leading to correction of the ePRF, systematic protocol violations, and potential protocol adherence issues. Central data monitoring may optimise concurrent data completeness and may help timely detection of data deviations due to misunderstandings or fabricated data.

PONTIFÍCIA UNIVERSIDADE CATÓLICA DO RIO DE JANEIRO COORDENAÇÃO DE APERFEIÇOAMENTO DO PESSOAL DE ENSINO SUPERIOR PROGRAMA DE EXCELENCIA ACADEMICA Os sistemas de múltiplas entradas e múltiplas saídas (MIMO) empregam um número crescente de antenas, o que leva a relevantes consumo de energia e custo de hardware dos front-ends correspondentes. Nesse contexto, o uso de conversores analógico-digitais (ADCs) de baixa resolução é promovido como uma solução promissora para este problema. Neste estudo consideramos um receptor MIMO de baixa resolução que implica que os sinais recebidos são processados simultaneamente pelos 1-bit ADCs e pela rede comparadora. Os sinais de entrada da rede comparadora podem vir de antenas diferentes, de modo que a extensão da rede comparadora pode ser interpretada como canais virtuais com saídas binárias. Com base nesses receptores MIMO de baixa resolução, desenvolvemos um estimador de canal e detector lineares de baixa resolução baseados no critério de mínimo erro médio quadrático (LRA-LMMSE) de acordo com o teorema de Bussgang. Duas redes de comparação são propostas, nomeadas, redes total e parcialmente conectadas. Também desenvolvemos uma rede parcialmente conectada baseada em busca gananciosa que usa muito menos comparadores para obter um desempenho bem próximo ao da rede totalmente conectada. Os resultados numéricos mostram que adicionar canais virtuais pode ser melhor do que adicionar canais físicos extras que correspondem a antenas de recepção adicionais em termos de taxa de erro de bit (BER). Além disso, ao empregar o estimador de canal proposto e seu erro de estimativa correspondente, construímos um limite inferior na taxa de soma ergódica para o receptor LRA-MMSE. Os resultados de simulação mostram que os sistemas com a proposta sistemas MIMO auxiliados por rede com quantização de 1-bit no receptor superam o convencional sistema MIMO de 1-bit em termos de desempenho de BER e erro quadrático médio (MSE). Além disso, as simulações numéricas confirmam uma vantagem significativa em termos de taxa de soma para o sistema proposto. Multiple-input multiple-output (MIMO) systems employs an increasing number of antennas, which leads to relevant energy consumption and hardware cost of the corresponding front ends. In this context, the use of lowresolution analog to digital converters (ADCs) is promoted as a promising solution to this problem. In this study we consider a low-resolution MIMO receiver which implies that the received signals simultaneously are processed by the 1-bit ADCs and the comparator network. The input signals for the comparator network can come from different antennas, such that the comparator network extension can be interpreted as virtual channels with binary outputs. Based on such low-resolution MIMO receivers, we develop low-resolution aware linear minimum mean-squared error (LRA-LMMSE) channel estimator and detector according to the Bussgang theorem. Two comparator networks are proposed, namely, fully and partially connected networks. We also devise a greedy search-based partially connected network that can use much less comparators to approach the performance of the fully connected network. Numerical results shows that adding virtual channels can be better than adding extra physical channels which corresponds to additional receive antennas in terms of bit error rate (BER). Furthermore, by employing the proposed channel estimator and its corresponding estimation error, we build up a lower bound on the ergodic sum rate for the LRA-LMMSE receiver. Simulation results show that the systems with the proposed network-aided MIMO systems with 1-bit quantization at the receiver outperforms the conventional 1-bit MIMO system in terms of BER and mean-square error (MSE) performances. Moreover, numerical simulations confirm a significant advantage in terms of sum rate for the proposed system.

Progesterone (P4) is essential for embryonic development and maintenance of pregnancy when deficiency causes early embryonic loss. In this study, we investigated the ability of hormonal supplementation to improve the fertility of Nellore females subjected to fixed-time artificial insemination (IATF) protocols. Here, we evaluated the effect of long-acting injectable progesterone (iP4) supplementation in the D4 after IATF on pregnancy rate and pregnancy loss in Nellore females (Bos taurus indicus) from different reproductive categories in Western Amazonia. Materials, Methods & Results: Eight hundred thirteen Nellore females from 5 farms were selected and distributed into 2 groups: control [GC; administration of 0.5 mL of 0.9% saline solution, intramuscularly - IM] (n = 407) and a group that received injectable progesterone (iP4) that was long-acting [GiP4; administration of 0.5 mL of iP4, 300 mg, via IM four days after IATF] (n = 406). Each group contained 3 subgroups: heifers, primiparous cows, and multiparous cows. Of the 407 animals in the CG, 103 were heifers, 107 primiparous, and 197 multiparous. Of the 406 animals in the GiP4 group, there were 101 heifers, 107 primiparous, and 198 multiparous. On a random day of the estrous cycle (D0), an intravaginal device containing 1 g of P4 and 2 mg of estradiol benzoate (BE) was inserted by intramuscular injection. On D8, the P4 device was removed and 150 μg of D-cloprostenol (PGF2α), 300 IU eCG, and 1 mg BE were administered IM. Cows were inseminated at D10, 48-52 h after procedure on D8. Pregnancy diagnosis was made between 35 and 40 days after insemination through ultrasound examination. Between 80 and 90 days after insemination, a new ultrasound examination was performed to assess early pregnancy loss. The data were processed using the SAS 9.2. The conception rate, pregnancy loss, and final conception rate were analyzed using PROC GLIMMIX according to groups (CG and GiP4), categories (heifers, primiparous and multiparous), and their interactions as variables. The differences in the means of least squares were adjusted using the Tukey?Kramer method. Statistical significance was defined as P < 0.05. The general conception rate was 46% (375/816). Regardless of the animal class, GiP4 animals (51.97%) had higher conception rates (P < 0.05) than CG (40.29%). In the subgroups (heifers, primiparous and multiparous cows), there was a difference (P < 0.05) between animals treated with iP4 (52.48%, 57.94%, and 48.48%, respectively) and those who were not (39.81%, 41.12%, and 40.10%, respectively). Gestational losses, regardless of the animal class, were higher in females in the CG (7.93%) [P < 0.05] compared to those in the GiP4 group (2.84%). Regardless of treatment with iP4, the percentage of gestational loss in heifers was significantly higher (10.64%) than that in primiparous and multiparous cows (3.77% and 2.86%, respectively). The final conception rates were higher in animals that received long-acting iP4, which increased the final pregnancy in all parity categories. Discussion: In the present study, the use of iP4 increased the pregnancy rate in Nellore females, regardless of the category. Although there has been no consensus on the use of iP4, there is an agreement that increases in the pregnancy rate are related to the moment of exogenous P4 application. In addition to influencing the pregnancy rate, reduction in pregnancy losses is also attributed to iP4 treatment, a fact demonstrated in the present study, where animals treated with iP4 had a lower pregnancy loss rate than normally occurs in beef cattle. Supplementation with long-acting iP4 increased the pregnancy rate at D35-40, reduced pregnancy losses, and increased the conception rate on D80-90 days in Nellore females reared in the Western Amazon, regardless of the animal category. Preconception and prenatal exposure to environmental contaminants may affect future health. Pregnancy and early life are critical sensitive windows of susceptibility. The aim of this review was to summarize current evidence on the toxic effects of environment exposure during pregnancy, the neonatal period, and childhood. Alcohol use is related to foetal alcohol spectrum disorders, foetal alcohol syndrome being its most extreme form. Smoking is associated with placental abnormalities, preterm birth, stillbirth, or impaired growth and development, as well as with intellectual impairment, obesity, and cardiovascular diseases later in life. Negative birth outcomes have been linked to the use of drugs of abuse. Pregnant and lactating women are exposed to endocrine-disrupting chemicals and heavy metals present in foodstuffs, which may alter hormones in the body. Prenatal exposure to these compounds has been associated with pre-eclampsia and intrauterine growth restriction, preterm birth, and thyroid function. Metals can accumulate in the placenta, causing foetal growth restriction. Evidence on the effects of air pollutants on pregnancy is constantly growing, for example, preterm birth, foetal growth restriction, increased uterine vascular resistance, impaired placental vascularization, increased gestational diabetes, and reduced telomere length. The advantages of breastfeeding outweigh any risks from contaminants. However, it is important to assess health outcomes of toxic exposures via breastfeeding. Initial studies suggest an association between pre-eclampsia and environmental noise, particularly with early-onset pre-eclampsia. There is rising evidence of the negative effects of environmental contaminants following exposure during pregnancy and breastfeeding, which should be considered a major public health issue.

Introducción Apenas conocemos cómo es la asistencia de los recién nacidos (RN) con encefalopatía hipóxico-isquémica (EHI) en hipotermia terapéutica (HT), especialmente si existen protocolos asistenciales, la neuromonitorización que se realiza o cómo es la aproximación al pronóstico neurológico. Este conocimiento permite detectar e implementar áreas de mejora asistencial. Método Estudio transversal de los 57 hospitales españoles que realizaban HT en 2015, mediante cuestionario sobre: 1) la disponibilidad de protocolos y de recursos tecnológicos; 2) el uso de herramientas de neuromonitorización; 3) los conocimientos de los profesionales; 4) la información pronóstica que se da los padres, y 5) el informe al alta y del plan de seguimiento. Resultados El 95% utiliza enfriamiento corporal-total servocontrolado y dispone de protocolos específicos de actuación. El 70% utiliza sedación y el 68% deja al paciente a dieta absoluta. La monitorización con electroencefalografía integrada por amplitud se utiliza en más del 80% de los centros, aunque solo en el 50% la enfermera es capaz de interpretarlo. La saturación de oxígeno cerebral es escasamente monitorizada (16%). Entre los estudios diagnóstico-pronósticos, la neuroimagen es universal, pero los neurobiomarcadores apenas se utilizan (29%). Solo el 21% ofrece información pronóstica antes de las 72 h de vida; sin presencia de la enfermera en el 70%. El seguimiento lo realiza el neuropediatra (84%), con una duración desigual entre centros. Conclusiones La asistencia del RN con EHI en España es adecuada, con áreas de mejora en: neuromonitorización, sedación, marco temporal de la información pronóstica, trabajo en equipo y estandarización de la duración del seguimiento.

Little is known about the pathogenesis of cerebral sinovenous thrombosis (CSVT) in the neonate. Although thrombophilia has been described as increasing the risk of CSVT in adults, it remains controversial in pediatric patients, and prospective case-control studies regarding neonatal CSVT are lacking. From 2008 to 2017, all 26 consecutive newborn infants =35 weeks of gestation diagnosed with neonatal CSVT, and their mothers, were tested for factor V Leiden (FV) G1691A, FII G20210A, and methylenetetrahydrofolate reductase C677T (MTHFR C677T) mutations. Eighty-five mother-infant pairs were recruited as controls. All infants except 1 with CSVT were suspected due to clinical symptoms, mainly seizures (22/25). Magnetic resonance imaging was performed in 24/26 infants. Heterozygous FV G1691A, FII G20210A, and homozygous MTHFR C677T mutations were present in 1/26, 3/26, and 3/20 infants with CSVT, respectively. FII (odds ratio: 10.96; 95% confidence interval [CI]: 1.09-110.35) and male sex (3.93; 95% CI: 1.43-10.76) were associated with CSVT. When FII G20210A analysis was adjusted for sex, the OR for FII G20210A was 6.70 (95% CI: 0.65-69.22). No differences were found for FV G1691A or homozygous MTHFR mutations between neonates with CSVT and their mothers, compared to controls.

Background and Objectives: Therapeutic interventions to improve the efficacy of whole-body cooling for hypoxic-ischemic encephalopathy (HIE) are desirable. Topiramate has been effective in reducing brain damage in experimental studies. However, in the clinical setting information is limited to a small number of feasibility trials. We launched a randomized controlled double-blinded topiramate/placebo multicenter trial with the primary objective being to reduce the antiepileptic activity in cooled neonates with HIE and assess if brain damage would be reduced as a consequence. Study Design: Neonates were randomly assigned to topiramate or placebo at the initiation of hypothermia. Topiramate was administered via a nasogastric tube. Brain electric activity was continuously monitored. Topiramate pharmacokinetics, energy-related and Krebs' cycle intermediates, and lipid peroxidation biomarkers were determined using liquid chromatography-mass spectrometry and MRI for assessing brain damage. Results: Out of 180 eligible patients 110 were randomized, 57 (51.8%) to topiramate and 53 (48.2%) to placebo. No differences in the perinatal or postnatal variables were found. The topiramate group exhibited less seizure burden in the first 24 h of hypothermia (topiramate, n = 14 [25.9%] vs. placebo, n = 22 [42%]); needed less additional medication, and had lower mortality (topiramate, n = 5 [9.2%] vs. placebo, n = 10 [19.2%]); however, these results did not achieve statistical significance. Topiramate achieved a therapeutic range in 37.5 and 75.5% of the patients at 24 and 48 h, respectively. A significant association between serum topiramate levels and seizure activity (p < 0.016) was established. No differences for oxidative stress, energy-related metabolites, or MRI were found. Conclusions: Topiramate reduced seizures in patients achieving therapeutic levels in the first hours after treatment initiation; however, they represented only a part of the study population.
