Purpose: In this study, the objectification of the subjective perception of loudness was investigated using electroencephalography (EEG). In particular, the emergence of objective markers in the domain of the acoustic discomfort threshold was examined. Methods: A cohort of 27 adults with normal hearing, aged between 18 and 30, participated in the study. The participants were presented with 500 ms long noise stimuli via in-ear headphones. The acoustic signals were presented with sound levels of [55, 65, 75, 85, 95 dB]. After each stimulus, the subjects provided their subjective assessment of the perceived loudness using a colored scale on a touchscreen. EEG signals were recorded, and afterward, event-related potentials (ERPs) locked to sound onset were analyzed. Results: Our findings reveal a linear dependency between the N100 component and both the sound level and the subjective loudness categorization of the sound. Additionally, the data demonstrated a nonlinear relationship between the P300 potential and the sound level as well as for the subjective loudness rating. The P300 potential was elicited exclusively when the stimuli had been subjectively rated as ”very loud” or ”too loud”. Conclusion: The findings of the present study suggest the possibility of the identification of the subjective uncomfortable loudness level by objective neural parameters.

However, personality trait is one of the most important influencing factors. Therefore, the aim of this study was to investigate the perception of loudness, sensitivity, and annoyance of LFN in relation to personality traits. Background and Objectives: Occupational noise exposure can be associated with various psychological effects. The present study was conducted with the aim of determining the relationship between noise exposure, annoyance, and loudness perception and social intelligence and general decision-making styles in workers. Materials and Methods: This descriptive study was conducted in a mine in Yazd Province in 2022. In this research, 65 workers were selected by convenience sampling and included in the study. In order to determine the level of noise exposure of workers, dosimetry was performed using a TES-1354 device. According to the sound equivalent level, workers were divided into two groups of permitted and unauthorized noise exposure. Worker's annoyance and loudness perception were determined using a questionnaire. Also, in order to evaluate social intelligence, Silvera et al.'s questionnaire was used. Decision-making styles were evaluated by Scott and Bruce's questionnaire. Data analysis was performed using independent sample t-test, Pearson and Spearman’s correlation coefficients, and chi-square and Mann-Whitney tests. Results: In the present study, there was no significant difference between the two studied groups in terms of demographic variables (p>0.05). The median score of noise annoyance (p<0.001) and loudness perception (p=0.003) were significantly higher in the workers with unauthorized noise exposure. Also, the mean score of the avoidant decision-making style was significantly higher in the workers with unauthorized noise exposure (p=0.018). Conclusion: In general, the results of the present study showed that unauthorized noise exposure was related to annoyance, loudness perception, and avoidant decision-making style. Considering the non-auditory effects of noise exposure, it seems necessary to implement a hearing protection program in workplaces with unauthorized noise. [ABSTRACT FROM AUTHOR]

Mechanistic hypotheses about airborne infectious disease transmission have traditionally emphasized the role of coughing and sneezing, which are dramatic expiratory events that yield both easily visible droplets and large quantities of particles too small to see by eye. Nonetheless, it has long been known that normal speech also yields large quantities of particles that are too small to see by eye, but are large enough to carry a variety of communicable respiratory pathogens. Here we show that the rate of particle emission during normal human speech is positively correlated with the loudness (amplitude) of vocalization, ranging from approximately 1 to 50 particles per second (0.06 to 3 particles per cm3) for low to high amplitudes, regardless of the language spoken (English, Spanish, Mandarin, or Arabic). Furthermore, a small fraction of individuals behaves as "speech superemitters," consistently releasing an order of magnitude more particles than their peers. Our data demonstrate that the phenomenon of speech superemission cannot be fully explained either by the phonic structures or the amplitude of the speech. These results suggest that other unknown physiological factors, varying dramatically among individuals, could affect the probability of respiratory infectious disease transmission, and also help explain the existence of superspreaders who are disproportionately responsible for outbreaks of airborne infectious disease.

Objectives The aims of this study were to investigate the test-retest reliability of measurements of loudness discomfort levels (LDLs), to suggest cut-off values for diagnosing patients with hyperacusis, and to evaluate the clinical value of-LDL measurements as a test for monitoring hyperacusis. Methods For the test-retest reliability of LDL measurements (study 1), a total of 68 patients who sought consultations at our clinic were subcategorized into four groups: patients with tinnitus (group 1), tinnitus and hearing loss (group 2), hyperacusis (group 3), and normal controls (group 4). Inter-hour and inter-day test-retest reliability values using different stimuli were investigated. For study 2, the clinical value of LDL measurements using pure tone stimuli was analyzed by comparing changes after sound generator use in patients with hyperacusis. Results In study 1, the group 3 patients showed significantly lower LDLs than the other groups. High test-retest reliability of LDL tests was demonstrated, regardless of the type of stimulus used. The cut-off values for screening patients with hyperacusis were 90 dB HL using pure tone stimuli and 62 dB HL using white-band noise stimuli. In study 2, significantly increased LDLs were correlated with improved symptoms and improved scores on tinnitus questionnaires after sound generator use, indicating that LDL measurement is a reliable test for monitoring hyperacusis during an intervention. Conclusion LDL measurement is a reliable diagnostic tool to reflect the condition of hyperacusis, especially during the course of treatment.

Environmental noise affects significantly our health and quality of life. Simple techniques, such as A-weighted decibels, have been applied commonly to the real issues of environmental noise. More elaborate techniques, considering mechanisms in the central nervous system, have also been developed continuously and approved by the international organization of standardization (e.g., ISO 532-3; hereafter, ISO loudness model). These techniques have advanced our knowledge of perceptual noisiness, but still have some limitations to account for a variety of psychophysical phenomena and our empirical experiences in acoustic engineering. Here, we propose that perceptual noisiness can be explained better by considering auditory attention. Attention driven by sensory input has been modeled originally as “saliency” in vision. This algorithm has also been applied to capture spectral-temporal dynamics of auditory attention (hereafter, spectral saliency). It has been suggested that the central auditory system contains two pathways identifying what and where a sound source is. The above spectral saliency corresponds only to the what-pathway. We therefore created a new auditory spatial saliency model to capture attentional effects along the where-pathway based on an algorithm of horizontal sound localization. We found that our spatial saliency model accounted for perceptual phenomena that cannot be explained by the ISO loudness model. Furthermore, the prediction of perceptual noisiness of environmental sounds (driving sounds of passenger cars) was improved significantly by integrating spatial saliency with ISO loudness. We conclude that spatial saliency can be used to capture sound features affecting perceptual noisiness in everyday life.

Differences in individual listening patterns are reported for a dichotic sample discrimination task. Seven tones were drawn from normal distributions with means of 1000 or 1100 Hz on each trial. Even-numbered tones (2, 4, and 6) and odd-numbered tones (1, 3, 5, and 7) were drawn, respectively, from distributions with a 50-Hz and 200-Hz standard deviation. Task difficulty was manipulated by presenting odd and even tones at different intensities. In easy conditions, high and low informative tones were presented at 70 dB and 50 dB, respectively. In difficult conditions, high informative and low informative tones were presented at 50 dB and 70 dB, respectively. Participants judged whether the sample was from high- or low-mean distribution. Decision weights, efficiency, and sensitivity showed a range of abilities to attend to high informative tones, with d' from 2.4-0.7. Most listeners showed a left-ear advantage, while no listeners showed a right ear advantage. Some listeners, but not all, showed no loudness dominance effect with the ability to selectively attend to quiet tones in difficult conditions. These findings show that the influence of an attentional strategy in dichotic listening can overcome the loudness dominance effect for some listeners.

Purpose: The goal of this study was to determine the relationship between the perceptual measure of speech naturalness and objective measures of pitch, loudness, and rate control as a potential tool for assessment of ataxic dysarthria., Method: Twenty-seven participants with ataxia and 29 age- and sex-matched control participants completed the pitch glide and loudness step tasks drawn from the Frenchay Dysarthria Assessment-Second Edition (FDA-2) in addition to speech diadochokinetic (DDK) tasks. First, group differences were compared for pitch variability in the pitch glide task, loudness variability in the loudness step task, and syllable duration and speech rate in the DDK task. Then, these acoustic measures were compared with previously collected ratings of speech naturalness by speech-language pathology graduate students., Results: Robust group differences were measured for pitch variability and both DDK syllable duration and speech rate, indicating that the ataxia group had greater pitch variability, longer DDK syllable duration, and slower DDK speech rate than the control group. No group differences were measured for loudness variability. There were robust relationships between speech naturalness and pitch variability, DDK syllable duration, and DDK speech rate, but not for loudness variability., Conclusions: Objective acoustic measures of pitch variability in the FDA-2 pitch glide task and syllable duration and speech rate in the DDK task can be used to validate perceptual measures of speech naturalness. Overall, speech-language pathologists can incorporate both perceptual measures of speech naturalness and acoustic measures of pitch variability and DDK performance for a comprehensive evaluation of ataxic dysarthria.

Audible very-high frequency sound (VHFS) and ultrasound (US) have been rated more unpleasant than lower frequency sounds when presented to listeners at similar sensation levels (SLs). In this study, 17 participants rated the sensory unpleasantness of 14-, 16-, and 18-kHz tones and a 1-kHz reference tone. Tones were presented at equal subjective loudness levels for each individual, corresponding to levels of 10, 20, and 30 dB SL measured at 1 kHz. Participants were categorized as either "symptomatic" or "asymptomatic" based on self-reported previous symptoms that they attributed to exposure to VHFS/US. In both groups, subjective loudness increased more rapidly with sound pressure level for VHFS/US than for the 1-kHz reference tone, which is consistent with a reduced dynamic range at the higher frequencies. For loudness-matched tones, participants rated VHFS/US as more unpleasant than that for the 1-kHz reference. These results suggest that increased sensory unpleasantness and reduced dynamic range at high frequencies should be considered when designing or deploying equipment which emits VHFS/US that could be audible to exposed people., (© 2024 Author(s). Objective: The goal of this study was to determine open field stapedius reflex thresholds (oSRTs) in CI patients with fittings based on subjective loudness ratings. A further objective was to compare these oSRTs and those of eSRT-based fittings that are similar to the oSRTs of normal hearing., Design: Impedance measurements of the ear drum were taken while subjects were wearing their audio processors. The stapedius reflex was elicited by electrical stimulation transmitted through the activated CI system in response to an acoustic stimulus presented in the free sound field., Study Sample: Subjects were 50 experienced CI users ( n  = 57 ears) with CI fittings based on subjective loudness scaling., Results: A reference range for the oSRTs was defined that was identified in CI patients with eSRT-based fittings. Sound levels for stapedius reflex detection were inside the reference target range in 70% of the cases, below the reference range (i.e. down to 40 dB HL) in 20% of the cases, and above the reference range in 10% of the cases., Conclusion: Stapedius reflex detection in a free sound field may help detect fittings with too high or too low stimulation levels that might reduce audiological performance.

This paper introduces a ranking and selection approach to psychoacoustic and psychophysical experimentation, with the aim of identifying top-ranking samples in listening experiments with minimal pairwise comparisons. We draw inspiration from sports tournament designs and propose to adopt modified knockout (KO) tournaments. Two variants of modified KO tournaments are described, which adapt the tree selection sorting algorithm and the replacement selection algorithm known from computer science. To validate the proposed method, a listening experiment is conducted, where binaural renderings of seven chamber music halls are compared regarding loudness and reverberance. The rankings obtained by the modified KO tournament method are compared to those obtained from a traditional round-robin (RR) design, where all possible pairs are compared. Moreover, the paper presents simulations to illustrate the method's robustness when choosing different parameters and assuming different underlying data distributions. The study's findings demonstrate that modified KO tournaments are more efficient than full RR designs in terms of the number of comparisons required for identifying the top ranking samples. Thus, they provide a promising alternative for this task. We offer an open-source implementation so that researchers can easily integrate KO designs into their studies., (© 2024 Author(s). Humans are adept at identifying spectral patterns, such as vowels, in different rooms, at different sound levels, or produced by different talkers. How this feat is achieved remains poorly understood. Two psychoacoustic analogs of spectral pattern recognition are spectral profile analysis and spectrotemporal ripple direction discrimination. This study tested whether pattern-recognition abilities observed previously at low frequencies are also observed at extended high frequencies. At low frequencies (center frequency ∼500 Hz), listeners were able to achieve accurate profile-analysis thresholds, consistent with prior literature. However, at extended high frequencies (center frequency ∼10 kHz), listeners' profile-analysis thresholds were either unmeasurable or could not be distinguished from performance based on overall loudness cues. A similar pattern of results was observed with spectral ripple discrimination, where performance was again considerably better at low than at high frequencies. Collectively, these results suggest a severe deficit in listeners' ability to analyze patterns of intensity across frequency in the extended high-frequency region that cannot be accounted for by cochlear frequency selectivity. One interpretation is that the auditory system is not optimized to analyze such fine-grained across-frequency profiles at extended high frequencies, as they are not typically informative for everyday sounds., (© 2024 Acoustical Society of America.)

A series of Bayesian adaptive procedures to estimate loudness growth across a wide frequency range from individual listeners was developed, and these procedures were compared. Simulation experiments were conducted based on multinomial psychometric functions for categorical loudness scaling across ten test frequencies estimated from 61 listeners with normal hearing and 87 listeners with sensorineural hearing loss. Adaptive procedures that optimized the stimulus selection based on the interim estimates of two types of category-boundary models were tested. The first type of model was a phenomenological model of category boundaries adopted from previous research studies, while the other type was a data-driven model derived from a previously collected set of categorical loudness scaling data. An adaptive procedure without Bayesian active learning was also implemented. Results showed that all adaptive procedures provided convergent estimates of the loudness category boundaries and equal-loudness contours between 250 and 8000 Hz. Performing post hoc model fitting, using the data-driven model, on the collected data led to satisfactory accuracies, such that all adaptive procedures tested in the current study, independent of modeling approach and stimulus-selection rules, were able to provide estimates of the equal-loudness-level contours between 20 and 100 phons with root-mean-square errors typically under 6 dB after 100 trials., (© 2024 Acoustical Society of America.)

Background: For hearing-impaired individuals, hearing aids are clinically fit according to subjective measures of threshold and loudness. The goal of this study was to evaluate objective measures of loudness perception that might benefit hearing aid fitting. Method: Seventeen adult hearing aid users and 17 normal-hearing adults participated in the study. Outcome measures including categorical loudness scaling, cortical auditory evoked potentials (CAEPs), and pupillometry. Stimuli were 1-kHz tone bursts presented at 40, 60, and 80 dBA. Results: Categorical loudness scaling showed that loudness significantly increased with intensity for all participants (p < 0.05). For CAEPs, high intensity was associated with greater P1, N1, and P2 peak amplitude for all listeners (p < 0.05); a significant but small effect of hearing aid amplification was observed. For all participants, pupillometry showed significant effects of high intensity on pupil dilation (p < 0.05); there was no significant effect of hearing aid amplification. A Focused Principal Component analysis revealed significant correlations between subjective loudness and some of the objective measures. Conclusion: The present data suggest that intensity had a significant impact on loudness perception, CAEPs, and pupil response. The correlations suggest that pupillometry and/or CAEPs may be useful in determining comfortable amplification for hearing aids. [ABSTRACT FROM AUTHOR]

Mutations in the OTOF gene encoding otoferlin result in a disrupted function of the ribbon synapses with impairment of the multivesicular glutamate release. Most affected subjects present with congenital hearing loss and abnormal auditory brainstem potentials associated with preserved cochlear hair cell activities (otoacoustic emissions, cochlear microphonics [CMs]). Transtympanic electrocochleography (ECochG) has recently been proposed for defining the details of potentials arising in both the cochlea and auditory nerve in this disorder, and with a view to shedding light on the pathophysiological mechanisms underlying auditory dysfunction. We review the audiological and electrophysiological findings in children with congenital profound deafness carrying two mutant alleles of the OTOF gene. We show that cochlear microphonic (CM) amplitude and summating potential (SP) amplitude and latency are normal, consistently with a preserved outer and inner hair cell function. In the majority of OTOF children, the SP component is followed by a markedly prolonged low-amplitude negative potential replacing the compound action potential (CAP) recorded in normally-hearing children. This potential is identified at intensities as low as 90 dB below the behavioral threshold. In some ears, a synchronized CAP is superimposed on the prolonged responses at high intensity. Stimulation at high rates reduces the amplitude and duration of the prolonged potentials, consistently with their neural generation. In some children, however, the ECochG response only consists of the SP, with no prolonged potential. Cochlear implants restore hearing sensitivity, speech perception and neural CAP by electrically stimulating the auditory nerve fibers. These findings indicate that an impaired multivesicular glutamate release in OTOF-related disorders leads to abnormal auditory nerve fiber activation and a consequent impairment of spike generation. The magnitude of these effects seems to vary, ranging from no auditory nerve fiber activation to an abnormal generation of EPSPs that occasionally trigger a synchronized electrical activity, resulting in high-threshold CAPs.

Introduction: Although noise characteristics such as intensity and frequency are the main cause of detrimental effects, it is important to pay attention to the personality traits of individuals as the host of adverse health effects. The aim of this study was to investigate the effect of personality traits on sensitivity, annoyance and loudness perception due to exposure to high frequency sound. Material and method: This interventional and experimental study was carried out among 80 undergraduate and postgraduate students in 2017. First, examinee were exposed to a high frequency noise at 65 dBA for one hour in an acoustic room. Then, to determine amount of annoyance, sensitivity, loudness perception and to investigate personality traits, questionnaire of noise annoyance, noise sensitivity, loudness perception and Eysenck personality inventory was used, respectively. Finally, Chi-square, independent t-test and multivariate analysis of variance (MANOVA) were used to data analyze. Results: The mean±SD of sensitivity, annoyance and loudness perception were 54.08±7.71, 7.0±1.53, and 2.79±1.13, respectively in this study. The mean scores of sensitivity, annoyance and loudness perception were significantly different in terms of personality traits, so that their average was higher in the neurotic and introverted. Based on MANOVA test results, personality traits had a significant effect on sensitivity, annoyance and loudness perception of individuals. In a way, the neuroticism and introversion had the greatest effect on the sensitivity and annoyance, respectively. Conclusion: The results of this study showed that personality traits such as introversion and neuroticism can affect the sensitivity, annoyance and loudness perception of individuals.

Purpose: This report describes a hearing device and corresponding fitting protocol designed for use in a transitional intervention for debilitating loudness-based hyperacusis., Method: The intervention goal is to transition patients with hyperacusis from their typical counterproductive sound avoidance behaviors (i.e., sound attenuation and limited exposure to healthy low-level sounds) into beneficial sound therapy treatment that can expand their dynamic range to the point where they can tolerate everyday sounds and experience an improved quality of life. This requires a combination of counseling and sound therapy, the latter of which is provided via the hearing device technology, signal processing, and precision fitting approach described in this report. The device combines a miniature behind-the-ear sound processor and a custom earpiece designed to maximize the attenuation of external sounds. Output-limiting loudness suppression is used to restrict exposure to offending high-level sounds while unity gain amplification maximizes exposure to healthy and tolerable lower level sounds. The fitting process includes measurement of the real-ear unaided response, the real-ear measurement (REM) system noise floor, the real-ear occluded response, real-ear insertion gain, and the output limit. With these measurements, the device can achieve the prescribed unity gain needed to provide transparent access to comfortable sound levels. It also supports individualized configuration of the therapeutic noise from an on-board sound generator and adaptive output limiting based on treatment-induced increases in dynamic range., Results and Conclusion: The utility of this device and fitting protocol, in combination with structured counseling, is highlighted in the outcomes of a successful 6-month trial of the transitional intervention described in a companion report in this issue.

Purpose: This report provides the experimental, clinical, theoretical, and historical background that motivated a patented transitional intervention and its implementation and evaluation in a field trial for mitigation of debilitating loudness-based hyperacusis (LH)., Background and Rationale: Barriers for ameliorating LH, which is differentiated here from other forms of hyperacusis, are delineated, including counterproductive management and treatment strategies that may exacerbate the condition. Evidence for hyper-gain central auditory processes as the bases for LH and the associated LH-induced distress and stress responses are presented. This presentation is followed by an overview of prior efforts to use counseling and therapeutic sound as interventional tools for recalibrating the hyper-gain LH response. We also consider previous efforts to use output-limiting sound-protection devices in the management of LH. This historical background lays the foundation for our transitional intervention protocol and its implementation and evaluation in a field trial., Conclusions: The successful implementation and evaluation of a transitional intervention, which we document in the outcomes of a companion proof-of-concept field trial in this issue, build on our prior efforts and those of others to understand, manage, and treat hyperacusis. These efforts to overcome significant barriers and vexing long-standing challenges in the management and treatment of LH, as reviewed here, are the pillars of the transitional intervention and its primary components, namely, counseling combined with protective sound management and therapeutic sound, which we detail in separate reports in this issue.

Purpose: So far, there have been no in-depth analyses of the connection between tinnitus sensation-level loudness and sleep quality. Accordingly, the present study was formulated as a mediation analysis focused on exploring this relationship., Method: Overall, 1,255 adults with consecutive subjective tinnitus who had sought outpatient treatment were enrolled in the present study., Results: Direct effects of tinnitus sensation-level loudness on sleep quality were not statistically significant (95% confidence intervals [CI] include zero), as measured by the point estimate, -0.016. However, the 95% CI for indirect effects did not include zero when assessing the Self-Rating Anxiety Scale (SAS) scores, the Self-Rating Depression Scale (SDS) scores, the visual analogue scale (VAS) scores, and self-reported tinnitus annoyance., Conclusions: These results suggest that tinnitus sensation-level loudness does not directly have an effect on sleep quality. However, it indirectly impacts sleep quality, mediated by SAS scores, SDS scores, the impact of tinnitus on life measured using the VAS, and self-reported tinnitus annoyance. As such, alleviating anxiety and depression in patients with tinnitus may result in reductions in their insomnia even if there is no reduction in tinnitus loudness. Importantly, otolaryngologists and other clinicians treating tinnitus should refer patients with tinnitus suffering from insomnia with comorbid depression or anxiety for appropriate psychological and/or psychiatric treatment.

The purpose of the present study was to explore the effectiveness of the Voice Performance Chart (VPCH) as a pedagogical training tool to enhance vocal expressive ability of 1st year acting students. Forty recorded audio samples were perceptually assessed by six blinded raters, using a five-points Likert scale for each of the dependent variables observed. Results showed that loudness, pitch, and speech rate variations significantly differ when comparing the vocal condition before and after a 11 weeks training period. That suggests that VPCH can be an effective pedagogical tool to develop vocal expressive capabilities of acting students, by enhancing their expressive nuances level, according to the text content. How different sensory modalities interact to shape perception is a fundamental question in cognitive neuroscience. Previous studies in audiovisual interaction have focused on abstract levels such as categorical representation (e.g., McGurk effect). It is unclear whether the cross-modal modulation can extend to low-level perceptual attributes. This study used motional manual gestures to test whether and how the loudness perception can be modulated by visual-motion information. Specifically, we implemented a novel paradigm in which participants compared the loudness of two consecutive sounds whose intensity changes around the just noticeable difference (JND), with manual gestures concurrently presented with the second sound. In two behavioral experiments and two EEG experiments, we investigated our hypothesis that the visual-motor information in gestures would modulate loudness perception. Behavioral results showed that the gestural information biased the judgment of loudness. More importantly, the EEG results demonstrated that early auditory responses around 100 ms after sound onset (N100) were modulated by the gestures. These consistent results in four behavioral and EEG experiments suggest that visual-motor processing can integrate with auditory processing at an early perceptual stage to shape the perception of a low-level perceptual attribute such as loudness, at least under challenging listening conditions. Long-term loudness perception of a sound has been presumed to depend on the spatial distribution of activated auditory nerve fibers as well as their temporal firing pattern. The relative contributions of those two factors were investigated by measuring loudness adaptation to sinusoidally amplitude-modulated 12-kHz tones. The tones had a total duration of 180 s and were either unmodulated or 100%-modulated at one of three frequencies (4, 20, or 100 Hz), and additionally varied in modulation depth from 0% to 100% at the 4-Hz frequency only. Every 30 s, normal-hearing subjects estimated the loudness of one of the stimuli played at 15 dB above threshold in random order. Without any amplitude modulation, the loudness of the unmodulated tone after 180 s was only 20% of the loudness at the onset of the stimulus. Amplitude modulation systematically reduced the amount of loudness adaptation, with the 100%-modulated stimuli, regardless of modulation frequency, maintaining on average 55%-80% of the loudness at onset after 180 s. Because the present low-frequency amplitude modulation produced minimal changes in long-term spectral cues affecting the spatial distribution of excitation produced by a 12-kHz pure tone, the present result indicates that neural synchronization is critical to maintaining loudness perception over time.

Although the cochlear implant has been widely acknowledged as the most successful neural prosthesis, only a fraction of hearing-impaired people who can potentially benefit from a cochlear implant have actually received one due to its limited awareness, accessibility, and affordability. To help overcome these limitations, a 26-electrode cochlear implant has been developed to receive China's Food and Drug Administration (CFDA) approval in 2011 and Conformité Européenne (CE) Marking in 2012. The present article describes design philosophy, system specification, and technical verification of the Nurotron device, which includes advanced digital signal processing and 4 current sources with multiple amplitude resolutions that not only are compatible with perceptual capability but also allow interleaved or simultaneous stimulation. The article also presents 3-year longitudinal evaluation data from 60 human subjects who have received the Nurotron device. The objective measures show that electrode impedance decreased within the first month of device use, but was stable until a slight increase at the end of two years. The subjective loudness measures show that electric stimulation threshold was stable while the maximal comfort level increased over the 3 years. Mandarin sentence recognition increased from the pre-surgical 0%-correct score to a plateau of about 80% correct with 6-month use of the device. Both indirect and direct comparisons indicate indistinguishable performance differences between the Nurotron system and other commercially available devices. The present 26-electrode cochlear implant has already helped to lower the price of cochlear implantation in China and will likely contribute to increased cochlear implant access and success in the rest of the world. This paper presents investigations into a comparative assessment of the effects of low- and high-frequency noise in relation to personality traits. The high- and low-frequency noises used are produced in the research laboratory using CoolEdit software. In all, 80 candidates were exposed to equivalent continuous sound pressure level of 65 dBA of low- and high-frequency noise in an acoustic room with a 2-week interval. After 1 hour of exposure to noise, participants were asked to complete noise annoyance scale, Weinstein noise sensitivity questionnaire and loudness perception. The results obtained indicate that there is a significant difference between annoyance and perception of low-frequency noise in comparison to annoyance and perception of high-frequency noise, but no significant difference is noted between sensitivity to low- and high-frequency noise. The multivariate analysis of covariance test is applied, which reveals that personality traits have a significant effect on sensitivity to low- and high-frequency noise, annoyance due to low- and high-frequency noise, loudness perception of low-frequency noise, but no interaction effects are found. It is further shown that personality traits are more effective on sensitivity, annoyance and loudness perception to high-frequency noise than those of low-frequency noise, and such effects are not only influenced by severity of noise but also by personality traits and frequency components. Objective: Current hearing aid prescription rules assume that spectral loudness summation decreases with hearing impairment and that binaural loudness summation is independent of hearing loss and signal bandwidth. Previous studies have shown that these assumptions might be incorrect. Spectral loudness summation was measured and compared for loudness scaling and loudness matching. Design: In this study, the effect of bandwidth on binaural summation was investigated by comparing loudness perception of low-pass filtered, high-pass filtered, and broadband pink noise at 35 Categorical Units for both unilateral and bilateral presentation. Study sample: Sixteen hearing-impaired listeners. Results: The results show that loudness differences between the three signals are different for bilateral presentation than for unilateral presentation. In specific, binaural loudness summation is larger for the low-pass filtered pink noise than for the high-pass filtered pink noise. Finally, individual variability in loudness perception near loudness discomfort level was found to be very large. Conclusions: Loudness matching is offered as a fast and reliable method to measure individual loudness perception. As discomfort with loud sounds is one of the major problems encountered by hearing aid users, measurement of individual loudness perception could improve hearing aid fitting substantially. Direct drive hearing devices, which deliver a signal directly to the middle ear by vibrating the tympanic membrane via a lens placed in contact with the umbo, are designed to provide an extension of audible bandwidth, but there are few studies of the effects of these devices on preference, speech intelligibility, and loudness. The current study is the first to compare aided speech understanding between narrow and extended bandwidth conditions for listeners with hearing loss while fitted with a direct drive hearing aid system. The study also explored the effect of bandwidth on loudness perception and investigated subjective preference for bandwidth. Fifteen adult hearing aid users with symmetrical sensorineural hearing loss participated in a prospective, within-subjects, randomized single-blind repeated-measures study. Participants wore the direct drive hearing aids for 4 to 15 weeks (average 6 weeks) prior to outcome measurement. Outcome measures were completed in various bandwidth conditions achieved by reducing the gain of the device above 5000 Hz or by filtering the stimuli. Aided detection thresholds provided evidence of amplification to 10000 Hz. A significant improvement was found in high-frequency consonant detection and recognition, as well as for speech in noise performance in the full versus narrow bandwidth conditions. Subjective loudness ratings increased with provision of the full bandwidth available; however, real-world trials showed most participants were able to wear the full bandwidth hearing aids with only small adjustments to the prescription method. Single-channel modulation detection thresholds (MDTs) have been shown to predict cochlear implant (CI) users' speech performance. However, little is known about multi-channel modulation sensitivity. Two factors likely contribute to multichannel modulation sensitivity: multichannel loudness summation and the across-site variance in single-channel MDTs. In this study, single- and multi-channel MDTs were measured in 9 CI users at relatively low and high presentation levels and modulation frequencies. Single-channel MDTs were measured at widely spaced electrode locations, and these same channels were used for the multichannel stimuli. Multichannel MDTs were measured twice, with and without adjustment for multichannel loudness summation (i.e., at the same loudness as for the single-channel MDTs or louder). Results showed that the effect of presentation level and modulation frequency were similar for single- and multi-channel MDTs. Multichannel MDTs were significantly poorer than single-channel MDTs when the current levels of the multichannel stimuli were reduced to match the loudness of the single-channel stimuli. This suggests that, at equal loudness, single-channel measures may over-estimate CI users' multichannel modulation sensitivity. At equal loudness, there was no significant correlation between the amount of multichannel loudness summation and the deficit in multichannel MDTs, relative to the average single-channel MDT. With no loudness compensation, multichannel MDTs were significantly better than the best single-channel MDT. The across-site variance in single-channel MDTs varied substantially across subjects. However, the across-site variance was not correlated with the multichannel advantage over the best single channel. This suggests that CI listeners combined envelope information across channels instead of attending to the best channel.

Background: Low-frequency noise as a detrimental occupational and environmental factor which can cause noise annoyance. In addition to noise, factors such as sensitivity and awareness of adverse health effects of noise seem to play an important role in causing annoyance. Therefore, this study aimed to investigate the association of noise annoyance and loudness perception caused by exposure to low-frequency noise with noise sensitivity. Methods: To achieve this goal, 80 students were exposed to low-frequency noise exposure at 65 dBA in an acoustic room. After one hour of exposure, the subjects completed the Weinstein's Noise Sensitivity Scale, Loudness Perception Scale, and Noise Annoyance Scale. Data were analyzed using Mann-Whitney, Kruskal-Wallis test and the Multivariate Analysis of variance (MANOVA). Results: The results showed that the mean score of noise annoyance and loudness perception was significantly higher in the group with high noise sensitivity. Also, increasing the awareness of the harmful effects of noise significantly increased the level of annoyance and loudness perception of individuals. The results of MANOVA showed that noise sensitivity has a significant effect on both the response variables. Also, awareness of the harmful effects of noise only affects loudness perception. The interaction effect of two independent variables was not significant on any of the response variables .Conclusion: In general, it can be concluded that noise sensitivity and awareness of the harmful effects of noise are two important factors that exacerbate the annoyance caused by exposure to low-frequency noise.

Background: Today, the effects of environmental pollution on human life and human needs, which reduces the level of community health caused by traffic noise in large cities, are clearly visible by creating irritation and pathogenic conditions. The aim of this study was to assess the risk of noise pollution resulted from central traffic in Shiraz by analyzing its indicators and related components. Methods: By specifying the high traffic jam in Shiraz, the sound and its frequency in 156 points of the city were analyzed using a sound analyzer (Tes-1358C model). The sound pressure levels (LA and LP) and the one and one-third octave band were continuously measured in A and C networks. Then, data were extracted and analyzed using Excel 2016 and Minitab 18.1. Eventually, phon and sone indexes were calculated for the heavy traffic jam in some of the central streets of Shiraz and the results were statistically calculated and their charts were prepared with comfort disturbance ranges. Results: The highest harmful frequency of traffic in Shiraz was 50 Hz and 83.3 dB. The sound pressure level ranged from 70 to 92.7 dB in network A and 87.8 dB in network C. The maximum sound level was 87 phon and the maximum sone was 26 at 14 stations. Conclusion: According to the results, the sound pressure levels exceeded the environmental standards of Iran and the values of phon and sone were higher than the standard of comfort at night and day.

Objective: This study aims to evaluate the influence of minimum and maximum stimulation levels on auditory thresholds and speech recognition abilities in adult cochlear implant users. Method: Fifteen adults implanted with a Cochlear® device with over 12 months listening experience. Participants underwent routine programming for optimization of minimum (T) and maximum comfort (C) stimulation levels, which was saved in Program 1 (MO). Three further maps were constructed artificially adjusting the measured levels: Program 2 – MO with 10 fewer electrical current units at C level (MC-); Program 3 – MO with 10 fewer electric current units at T level (MT−); and Program 4 – MO with 10 more electric current units at T level (MT+). Sound field thresholds, recorded sentence recognition and monosyllable tests were presented in quiet and in noise. Results: There were significantly better thresholds at 1, 3, 4, and 6 kHz frequencies in MT+ and worse in MC−. A statistically significant difference was observed for sentences in quiet and monosyllables in quiet and noise with changing C levels, with worsening of the results for MC− program. Conclusion: The results suggest that T levels above the behavioural threshold provided an improvement in sound field thresholds but did not influence performance on speech recognition tests in quiet and in noise. In contrast, C levels below the behavioural comfort level worsened sound field thresholds and led to poorer performance in tests of sentence recognition in quiet and monosyllable recognition in quiet and in noise. Categorical loudness scaling (CLS) measures provide useful information about an individual’s loudness perception across the dynamic range of hearing. A probability model of CLS categories has previously been described as a multi-category psychometric function (MCPF). In the study, a representative “catalog” of potential listener MCPFs was used in conjunction with maximum-likelihood estimation to derive CLS functions for participants with normal hearing and with hearing loss. The approach of estimating MCPFs for each listener has the potential to improve the accuracy of the CLS measurements, particularly when a relatively low number of data points are available. The present study extends the MCPF approach by using Bayesian inference to select stimulus parameters that are predicted to yield maximum expected information (MEI) during data collection. The accuracy and reliability of the MCPF-MEI approach were compared to the standardized CLS measurement procedure (ISO 16832:2006, 2006). A non-adaptive, fixed-level, paradigm served as a “gold-standard” for this comparison. The test time required to obtain measurements in the standard procedure is a major barrier to its clinical uptake. Test time was reduced from approximately 15 min to approximately 3 min with the MEI-adaptive procedure. Results indicated that the test–retest reliability and accuracy of the MCPF-MEI adaptive procedures were similar to the standardized CLS procedure. Computer simulations suggest that the reliability and accuracy of the MEI procedure were limited by intrinsic uncertainty of the listeners represented in the MCPF catalog. In other words, the MCPF provided insufficient predictive power to significantly improve adaptive-tracking efficiency under practical conditions. Concurrent optimization of both the MCPF catalog and the MEI-adaptive procedure have the potential to produce better results. Regardless of the adaptive-tracking method used in the CLS procedure, the MCPF catalog remains clinically useful for enabling maximum-likelihood determination of loudness categories.

Background: For hearing-impaired individuals, hearing aids are clinically fit according to subjective measures of threshold and loudness. The goal of this study was to evaluate objective measures of loudness perception that might benefit hearing aid fitting. Method: Seventeen adult hearing aid users and 17 normal-hearing adults participated in the study. Outcome measures including categorical loudness scaling, cortical auditory evoked potentials (CAEPs), and pupillometry. Stimuli were 1-kHz tone bursts presented at 40, 60, and 80 dBA. Results: Categorical loudness scaling showed that loudness significantly increased with intensity for all participants (p < 0.05). For CAEPs, high intensity was associated with greater P1, N1, and P2 peak amplitude for all listeners (p < 0.05); a significant but small effect of hearing aid amplification was observed. For all participants, pupillometry showed significant effects of high intensity on pupil dilation (p < 0.05); there was no significant effect of hearing aid amplification. A Focused Principal Component analysis revealed significant correlations between subjective loudness and some of the objective measures. Conclusion: The present data suggest that intensity had a significant impact on loudness perception, CAEPs, and pupil response. The correlations suggest that pupillometry and/or CAEPs may be useful in determining comfortable amplification for hearing aids.

Abnormal auditory adaptation is a standard clinical tool for diagnosing auditory nerve disorders due to acoustic neuromas. In the present study we investigated auditory adaptation in auditory neuropathy owing to disordered function of inner hair cell ribbon synapses (temperature-sensitive auditory neuropathy) or auditory nerve fibres. Subjects were tested when afebrile for (i) psychophysical loudness adaptation to comfortably-loud sustained tones; and (ii) physiological adaptation of auditory brainstem responses to clicks as a function of their position in brief 20-click stimulus trains (#1, 2, 3 … 20). Results were compared with normal hearing listeners and other forms of hearing impairment. Subjects with ribbon synapse disorder had abnormally increased magnitude of loudness adaptation to both low (250 Hz) and high (8000 Hz) frequency tones. Subjects with auditory nerve disorders had normal loudness adaptation to low frequency tones; all but one had abnormal adaptation to high frequency tones. Adaptation was both more rapid and of greater magnitude in ribbon synapse than in auditory nerve disorders. Auditory brainstem response measures of adaptation in ribbon synapse disorder showed Wave V to the first click in the train to be abnormal both in latency and amplitude, and these abnormalities increased in magnitude or Wave V was absent to subsequent clicks. In contrast, auditory brainstem responses in four of the five subjects with neural disorders were absent to every click in the train. The fifth subject had normal latency and abnormally reduced amplitude of Wave V to the first click and abnormal or absent responses to subsequent clicks. Thus, dysfunction of both synaptic transmission and auditory neural function can be associated with abnormal loudness adaptation and the magnitude of the adaptation is significantly greater with ribbon synapse than neural disorders.

The perception of the distance to a sound source is relevant in many everyday situations, not only in real spaces, but also in virtual reality (VR) environments. Where real rooms often reach their limits, VR offers far-reaching possibilities to simulate a wide range of acoustic scenarios. However, in virtual room acoustics a plausible reproduction of distance-related cues can be challenging. In the present study, we compared the detection of changes of the distance to a sound source and its neurocognitive correlates in a real and a virtual reverberant environment, using an active auditory oddball paradigm and EEG measures. The main goal was to test whether the experiments in the virtual and real environments produced equivalent behavioral and EEG results. Three loudspeakers were placed at ego-centric distances of 2 m (near), 4 m (center), and 8 m (far) in front of the participants (N = 20), each 66 cm below their ear level. Sequences of 500 ms noise stimuli were presented either from the center position (standards, 80 % of trials) or from the near or far position (targets, 10 % each). The participants had to indicate a target position via a joystick response ("near" or "far"). Sounds were emitted either by real loudspeakers in the real environment or rendered and played back for the corresponding positions via headphones in the virtual environment. In addition, within both environments, loudness of the auditory stimuli was either unaltered (natural loudness) or the loudness cue was manipulated, so that all three loudspeakers were perceived equally loud at the listener's position (matched loudness). The EEG analysis focused on the mismatch negativity (MMN), P3a, and P3b as correlates of deviance detection, attentional orientation, and context-updating/stimulus evaluation, respectively. Overall, behavioral data showed that detection of the target positions was reduced within the virtual environment, and especially when loudness was matched. Except for slight latency shifts in the virtual environment, EEG analysis indicated comparable patterns within both environments and independent of loudness settings. Thus, while the neurocognitive processing of changes in distance appears to be similar in virtual and real spaces, a proper representation of loudness appears to be crucial to achieve a good task performance in virtual acoustic environments., (Copyright © 2024 The Author(s). Objective: Distorted loudness perception is one of the main complaints of hearing aid users. Measuring loudness perception in the clinic as experienced in everyday listening situations is important for loudness-based hearing aid fitting. Little research has been done comparing loudness perception in the field and in the laboratory., Design: Participants rated the loudness in the field and in the laboratory of 36 driving actions. The field measurements were recorded with a 360° camera and a tetrahedral microphone. The recorded stimuli, which are openly accessible, were presented in three conditions in the laboratory: 360° video recordings with a head-mounted display, video recordings with a desktop monitor and audio-only., Study Samples: Thirteen normal-hearing participants and 18 hearing-impaired participants with hearing aids., Results: The driving actions were rated as louder in the laboratory than in the field for the condition with a desktop monitor and for the audio-only condition. The less realistic a laboratory condition was, the more likely it was for a participant to rate a driving action as louder. The field-laboratory loudness differences were bigger for louder sounds., Conclusion: The results of this experiment indicate the importance of increasing realism and immersion when measuring loudness in the clinic.

Cochlear synaptopathy, a form of cochlear deafferentation, has been demonstrated in a number of animal species, including non-human primates. Both age and noise exposure contribute to synaptopathy in animal models, indicating that it may be a common type of auditory dysfunction in humans. Temporal bone and auditory physiological data suggest that age and occupational/military noise exposure also lead to synaptopathy in humans. The predicted perceptual consequences of synaptopathy include tinnitus, hyperacusis, and difficulty with speech-in-noise perception. However, confirming the perceptual impacts of this form of cochlear deafferentation presents a particular challenge because synaptopathy can only be confirmed through post-mortem temporal bone analysis and auditory perception is difficult to evaluate in animals. Animal data suggest that deafferentation leads to increased central gain, signs of tinnitus and abnormal loudness perception, and deficits in temporal processing and signal-in-noise detection. If equivalent changes occur in humans following deafferentation, this would be expected to increase the likelihood of developing tinnitus, hyperacusis, and difficulty with speech-in-noise perception. Physiological data from humans is consistent with the hypothesis that deafferentation is associated with increased central gain and a greater likelihood of tinnitus perception, while human data on the relationship between deafferentation and hyperacusis is extremely limited. Many human studies have investigated the relationship between physiological correlates of deafferentation and difficulty with speech-in-noise perception, with mixed findings. A non-linear relationship between deafferentation and speech perception may have contributed to the mixed results. Cochlear implant listeners receive auditory stimulation through amplitude-modulated electric pulse trains. Auditory nerve studies in animals demonstrate qualitatively different patterns of firing elicited by low versus high pulse rates, suggesting that stimulus pulse rate might influence the transmission of temporal information through the auditory pathway. We tested in awake guinea pigs the temporal acuity of auditory cortical neurons for gaps in cochlear implant pulse trains. Consistent with results using anesthetized conditions, temporal acuity improved with increasing pulse rates. Unlike the anesthetized condition, however, cortical neurons responded in the awake state to multiple distinct features of the gap-containing pulse trains, with the dominant features varying with stimulus pulse rate. Responses to the onset of the trailing pulse train (Trail-ON) provided the most sensitive gap detection at 1,017 and 4,069 pulse-per-second (pps) rates, particularly for short (25 ms) leading pulse trains. In contrast, under conditions of 254 pps rate and long (200 ms) leading pulse trains, a sizeable fraction of units demonstrated greater temporal acuity in the form of robust responses to the offsets of the leading pulse train (Lead-OFF). Finally, TONIC responses exhibited decrements in firing rate during gaps, but were rarely the most sensitive feature. Unlike results from anesthetized conditions, temporal acuity of the most sensitive units was nearly as sharp for brief as for long leading bursts. The differences in stimulus coding across pulse rates likely originate from pulse rate-dependent variations in adaptation in the auditory nerve. Introduction: Common audiometric tests can assess if there is any hearing injury or not, but they can not assess the impact of disability due to hearing loss, individual's satisfaction level from rehabilitation services, and the impact of disability on life, due to hearing loss. These items can be assessed using self-assessment questionnaires. This study aimed to use self-assessment Profile of Aided Loudness (PAL) questionnaire to compare satisfaction and loudness levels of sounds in young individuals and elderlies with moderate and severe hearing loss using hearing aid. Materials and Methods: In this cross-sectional study, 20 young individuals with age range of 20 to 40 years and 20 elderlies with age range from 60 to 80 years were enrolled. Satisfaction and sound loudness levels of the hearing aid were assessed using self-assessment PAL questionnaire. Data were collected and analyzed using computer statistical information analysis software. Results: Loudness level had no significant differences among young individuals and elderlies, and women and men. Loudness level had no significant differences for soft sounds, but for average and loud sounds, the differences were significant. Satisfaction levels did not show any significant differences between moderate and severe hearing loss, and between women and men; but the difference between young individuals and elderlies was significant. Conclusion: PAL index is efficient to assess the understood sound loudness in an individual with and without hearing aid. Comparing the outcomes of this questionnaire with normal levels is used to select and adjust the hearing aid. The results of this study show that loudness understanding is affected by the level of hearing loss, and satisfaction level is affected by age factor.

OBJECTIVES: Differential sensitivity of intensity is known to be important for the perception of the relative distance of sounds in the environment, emotions of speakers, and localize sounds. However, a few features in listening devices, such as cochlear implants, used by individuals with hearing loss alter the output intensity heard by them. This makes soft sounds loud and loud sounds soft. Hence, the aim of the present study was to compare the intensity discrimination of children using cochlear implants with that of typically developing children. MATERIALS AND METHODS: Intensity discrimination of 30 children (15 using cochlear implants and 15 typically developing children) was obtained for three warble tones (500 Hz, 1000 Hz, and 4000 Hz) and three vowels (/a/, /i/, and /u/). The responses of the two participant groups, obtained using a 3-alternative forced-choice technique, were compared. RESULTS: Children using cochlear implants performed significantly poorer than typically developing children for the 4000 Hz warble tone and for the vowels /a/ and /u/. However, there was no significant difference for the remaining stimuli. CONCLUSION: The study indicated that the intensity discrimination threshold varies as a function of the frequency of the signals in children using cochlear implants. Intensity discrimination for high-frequency tones was significantly poorer for typically developing children, but not for low-frequency tones. The physical properties of a sound evolve when traveling away from its source. As an example, the sound pressure level at the listener's ears will vary according to their respective distance and azimuth. However, several studies have reported loudness to remain constant when varying the distance between the source and the listener. This loudness constancy has been reported to occur when the listener focused attention on the sound as emitted by the source (namely the distal stimulus). Instead, the listener can focus on the sound as reaching the ears (namely the proximal stimulus). The instructions given to the listener when assessing loudness can drive focus toward the proximal or distal stimulus. However, focusing on the distal stimulus requires to have sufficient information about the sound source, which could be provided by either the environment or by the stimulus itself. The present study gathers three experiments designed to assess loudness when driving listeners' focus toward the proximal or distal stimuli. Listeners were provided with different quality and quantity of information about the source depending on the environment (visible or hidden sources, free field or reverberant rooms) and on the stimulus itself (noise or speech). The results show that listeners reported constant loudness when asked to focus on the distal stimulus only, provided enough information about the source was available. These results highlight that loudness relies on the way the listener focuses on the stimuli and emphasize the importance of the instructions that are given in loudness studies., (© 2023. Objective: Some studies have directly compared the National Acoustic Laboratories' prescription for non-linear hearing aids (HAs) version 2 (NAL-NL2) and Desired Sensation Level for non-linear HAs version 5 (DSLv5), although none were performed in Japan. As the Japanese language is a tonal language that has different linguistic characteristics than those of the studied languages, we compared the outcomes of the NAL-NL2 and DSLv5 in hearing-impaired Japanese participants., Methods: A crossover-controlled trial was conducted on 18 first-time HA users with bilateral moderate sensorineural hearing loss. Participants wore HAs adjusted with each prescriptive method for four weeks. The prescriptions were assessed using speech discrimination testing and the abbreviated profile of hearing aid benefit (APHAB). Consequently, participants were asked to select their preferred prescription and determine which was better for "listening to a conversation" and when "noisy.", Results: The mean DSLv5 real ear insertion gain for an input level of 65 dB sound pressure level (SPL) was higher than that of the NAL-NL2 at 250 and 500 Hz. The average speech discrimination score was 78 ± 14% at a 65-dB SPL and 75 ± 17% at an 80-dB SPL for the NAL-NL2, and 79 ± 11% at a 65-dB SPL and 77 ± 17% at an 80-dB SPL for the DSLv5. These differences were not significant. No significant differences were observed in APHAB subscale scores between the two prescription methods. Ultimately, 11 (61%) and 7 (39%) participants preferred the NAL-NL2 and DSLv5, respectively, with no significant differences., Conclusion: Although the gain of the NAL-NL2 is smaller than that of the DSLv5, both had the same hearing effect.
