There is a tension between state oversight and state intrusion into our personal lives. The analytical powers of artificial intelligence/machine learning and the pervasive data collection of the Internet of Things, the Smart City and all those personal devices we use together permit revelations as to our lives as never seen before. We must consider the impact of this on the relations between citizen and government in this new, ubiquitous world of government cybersurveillance and revelation.

The COVID-19 pandemic has dramatically impacted the delivery of nutrition education through Cooperative Extension. This study aimed to identify a Cooperative Extension audience's attitudes towards virus prevention behaviors, preferred sources of information regarding COVID-19, and nutrition education preferences during the pandemic. We distributed an online survey to 477 potential nutrition education participants including questions about behaviors like mask wearing, preferences for virtual nutrition lesson delivery, and attitudes towards sources of information about COVID-19. Attitudes towards mask wearing and preferred sources of information differed significantly by race, income, and age. These differences have serious implications for future outreach and program delivery.

Português, como muitas outras línguas românicas, é tradicionalmente classificado como uma língua pro-drop com alinhamento nominativo-acusativo em seu sistema pronominal. Isso significa que a função do sujeito é basicamente codificada no verbo, enquanto a função do objeto é marcada no próprio pronome. No entanto, o português falado no Brasil é conhecido por ter simplificado drasticamente seu sistema pronominal devido à introdução de novos pronomes, como você e a gente, que não se flexionam quanto ao caso e ambos requerem o verbo na 3.ª pessoa do singular. Obviamente, mudanças tão radicais podem afetar todo o sistema pronominal. Novas evidências provenientes de várias fontes, como por exemplo a música popular e Twitter, mostram exatamente isso: O português brasileiro popular parece estar em via de abandonar completamente qualquer distinção de caso em seu sistema pronominal. Este artigo tenta acompanhar esse desenvolvimento com o exemplo do pronome de 1.ª pessoa do singular eu, analisando dados encontrados nas redes sociais e comparando-os com fontes escritas que remontam até ao século XV.

La finalidad de este artículo es reflexionar acerca de las implicaciones emocionales y personales en la construcción del objeto de estudio de una tesis de Maestría. Para ello, se recuperarán algunos fragmentos correspondientes a las circunstancias y experiencias de vida y al recorrido disciplinar y académico en el campo de la salud mental respecto a la cronicidad y discapacidad, en los roles de profesional de la salud mental y de investigadora. Entendiendo que lo acontecido en dichas trayectorias no sólo han sido los motivos que subyacen en el interés por investigar acerca de dichas problemáticas, sino también porque han permitido una reflexión y cuestionamiento acerca de los discursos y prácticas hegemónicas desde las cuales se abordan las mismas.

BackgroundNo study to date has analyzed the progression of sinonasal symptoms over time in COVID-19 patients. The purpose of this study is to analyze the progression of sinonasal symptoms and risk factors for olfactory dysfunction in the mild severity COVID-19 patient.MethodsAn internet survey was used to assess sinonasal symptoms in patients with COVID-19. Changes in rhinologic domain and symptom-specific Sinonasal Outcome Test (SNOT-22) scores were compared at five time points: two weeks before diagnosis, at diagnosis, two weeks after diagnosis, four weeks after diagnosis, and six months after diagnosis.Results521 responses were collected. Rhinologic domain SNOT-22 scores increased significantly (p  0.999). Taste-specific SNOT-22 scores also peaked at diagnosis (2.06, p  0.999). There were no significant differences in sense of smell or taste between 1-month and 6-month timepoints.ConclusionSinonasal symptoms, particularly loss of smell and taste, may be important presenting symptoms in the mild severity COVID-19 patient. Our findings support incorporating these symptoms into screening protocols.Level of evidence4.

BackgroundDisorders of consciousness (DOC), i.e., unresponsive wakefulness syndrome (UWS) or vegetative state (VS) and minimally conscious state (MCS), are conditions that can arise from severe brain injury, inducing widespread functional changes. Given the damaging implications resulting from these conditions, there is an increasing need for rehabilitation treatments aimed at enhancing the level of consciousness, the quality of life, and creating new recovery perspectives for the patients. Music may represent an additional rehabilitative tool in contexts where cognition and language are severely compromised, such as among DOC patients. A further type of rehabilitation strategies for DOC patients consists of Non-Invasive Brain Stimulation techniques (NIBS), including transcranial electrical stimulation (tES), affecting neural excitability and promoting brain plasticity.ObjectiveWe here propose a novel rehabilitation protocol for DOC patients that combines music-based intervention and NIBS in neurological patients. The main objectives are (i) to assess the residual neuroplastic processes in DOC patients exposed to music, (ii) to determine the putative neural modulation and the clinical outcome in DOC patients of non-pharmacological strategies, i.e., tES(control condition), and music stimulation, and (iii) to evaluate the putative positive impact of this intervention on caregiver's burden and psychological distress.MethodsThis is a randomised cross-over trial in which a total of 30 participants will be randomly allocated to one of three different combinations of conditions: (i) Music only, (ii) tES only (control condition), (iii) Music + tES. The music intervention will consist of listening to an individually tailored playlist including familiar and self-relevant music together with fixed songs; concerning NIBS, tES will be applied for 20 minutes every day, 5 times a week, for two weeks. After these stimulations two weeks of placebo treatments will follow, with sham stimulation combined with noise for other two weeks. The primary outcomes will be clinical, i.e., based on the differences in the scores obtained on the neuropsychological tests, such as Coma Recovery Scale-Revised, and neurophysiological measures as EEG, collected pre-intervention, post-intervention and post-placebo.DiscussionThis study proposes a novel rehabilitation protocol for patients with DOC including a combined intervention of music and NIBS. Considering the need for rigorous longitudinal randomised controlled trials for people with severe brain injury disease, the results of this study will be highly informative for highlighting and implementing the putative beneficial role of music and NIBS in rehabilitation treatments.Trial registrationClinicalTrials.gov identifier: NCT05706831, registered on January 30, 2023.

Pragmatic communication abilities refer to the capacity to use language in a social context. Despite evidence to the contrary, the left cerebral hemisphere of the majority of right handers has been considered exclusively specialized for control of language phonology, syntax and semantics, whereas the right hemisphere has been specialized for the control of language pragmatics. Many studies have shown the non-exclusivity of the left hemisphere for language skills. Communication deficits observed in these studies for patients with right hemisphere damage confirmed the necessity for integrity of the right hemisphere across a number of language components. The aim of this study is to investigate the specific role of the right and left hemispheres across several aspects of communication deficits, with particular attention given to the influence of these deficits on functional outcome. The second aim is to characterize possible correlations between pragmatic and other cognitive deficits. We evaluated 22 patients, 15 with left- and 7 with right-brain ischemic or hemorrhagic damage, using cognitive, pragmatic and language tests. We deployed the Right Hemisphere Language Battery–Santa Lucia and Montreal d’Evaluation de la Communication to assess pragmatic abilities. The results showed no statistically significant differences between patients with left- and right-brain damage, highlighting the importance of integration between the two hemispheres in the communication process. Multiple significant correlations were found between pragmatic abilities and cognitive tests assessing global cognitive functioning, pantomime expression and comprehension. Pragmatic deficits were also shown to correlate with functional cognitive outcome. It is important to assess pragmatic abilities in patients with cognitive deficits after both left and right stroke for tailoring neuropsychological intervention to mitigate pragmatic disabilities in functional outcomes.

Background: This study was an examination of the puzzling finding that people assessed for symptoms of posttraumatic stress disorder (PTSD) consistently score higher on the self-report PTSD Checklist for DSM-5 (PCL-5) than the Clinician-Administered PTSD Scale for DSM-5 (CAPS-5). Both scales purportedly assess PTSD severity with the same number of items, scaling, and scoring range, but differences in scores between measures make outcomes difficult to decipher. Objective: The purpose of this study was to examine several possible psychometric reasons for the discrepancy in scores between interview and self-report. Method: Data were combined from four clinical trials to examine the baseline and posttreatment assessments of treatment-seeking active duty military personnel and veterans. Results: As in previous studies, total scores were higher on the PCL-5 compared to the CAPS-5 at baseline and posttreatment. At baseline, PCL-5 scores were higher on all 20 items, with small to large differences in effect size. At posttreatment, only three items were not significantly different. Distributions of item responses and wording of scale anchors and items were examined as possible explanations of the difference between measures. Participants were more likely to use the full range of responses on the PCL-5 compared to interviewers. Conclusions: Suggestions for improving the congruence between these two scales are discussed. Administration of interviews by trained assessors can be resource intensive, so it is important that those assessing PTSD severity are afforded confidence in the equivalence of their assessment of PTSD regardless of the assessment method used.

Background: Shame and guilt are key emotions known to amplify trauma-related symptoms in veterans. Maintenance of symptoms is facilitated by avoidance behaviors, such as substance use. However, limited research has examined the associations between shame, guilt, and substance use in daily life. Methods: The current study sought to examine the cross-lagged association between shame, guilt, and substance use. Forty veterans completed 28 days of experience sampling reporting on their current emotional experiences and use of substances. Results: Results suggest a reciprocal relationship among shame and guilt and substance use, such that shame and guilt separately predicted subsequent substance use, and substance use predicted subsequent shame and guilt. Conclusions: These results highlight the dynamic relationship among shame, guilt, and substance use and suggest the potential value of conceptualizing these clinical targets as mutually reinforcing to inform integrative intervention strategies that can interrupt the in-the-moment cascade of negative consequences.

The global outbreak of COVID-19 has required mental health providers to rapidly rethink and adapt how they provide care. Cognitive processing therapy (CPT) is a trauma-focused, evidence-based treatment for posttraumatic stress disorder that is effective when delivered in-person or via telehealth. Given current limitations on the provision of in-person mental health treatment during the COVID-19 pandemic, this article presents guidelines and treatment considerations when implementing CPT via telehealth. Based on lessons learned from prior studies and clinical delivery of CPT via telehealth, recommendations are made with regard to overall strategies for adapting CPT to a telehealth format, including how to conduct routine assessments and ensure treatment fidelity.

BackgroundMany types of diet intervention can achieve negative energy balance and successful weight loss in persons with obesity. However, within any dietary strategy, there is large inter-individual variation in the weight loss response. The aim of this study is to determine factors that predict weight loss success for diet interventions that vary by macronutrient and caloric composition.MethodsParticipants with BMI 30.0 to 49.9 kg/m2 self-selected one of three diet intervention trials for weight loss: low carbohydrate (LOW CHO), low fat (LOW FAT), or low calorie (LOW KCAL). (1) Background: Most daily activities and sport gestures involve asymmetric movement patterns of the upper and lower extremities, transferring asymmetric mechanical loadings to the spine. Therefore, asymmetric lumbar stabilization exercises are frequently prescribed in athletic programs and preventive/rehabilitation interventions. This study analyzed the bilateral activity of the thoracic erector spinae (ES), lumbar multifidus (MF), external oblique (EO), and internal oblique (IO) during asymmetric lumbar stabilization exercises executed in prone, quadruped, and standing-prone positions, rising an upper and/or lower limb in all possible combinations. A limited subset of these data has been previously published in earlier studies. (2) Methods: Surface EMG signals were bilaterally recorded from the selected muscles using wireless EMG sensors. (3) Results: ES, MF, and oblique muscles’ activity was significantly higher in prone, standing-prone, and quadruped positions, respectively, and was maximized by specific limb rise combinations (up to 69%, 64%, 34%, and 24% maximum-voluntary-isometric-contraction for ES, MF, EO, and IO). The bilateral difference in muscle activation was significantly higher in the quadruped position and revealed different strategies used to stabilize the body in response to the different exercise conditions. (4) Conclusions: The study results can provide deeper insights into the stabilizing function of the lumbar and oblique muscles and aid in designing optimal progressions for lumbar stabilization exercises.

Pattern recognition, machine learning and artificial intelligence offer tremendous opportunities for efficient operations, management and governance. They can optimise processes for object, text, graphics, speech and pattern recognition. In doing so the algorithmic processing may be subject to unknown biases that do harm rather than good. We examine how this may happen, what damage may occur and the resulting ethical/legal impact and newly manifest obligations to avoid harm to others from these systems. But what are the risks, given the Human Condition?

Background: Traumatic spinal cord injury (SCI) is a neurological disorder that causes a traumatic anatomical discontinuity of the spinal cord. SCI can lead to paraplegia, spastic, or motor impairments. Go-karting for people with SCI is an adapted sport that is becoming increasingly popular. The purpose of this case report is to shed light on the effects of driving a go-kart on a patient with SCI-related spasticity and to deepen understanding of the possible related role of whole-body vibration (WBV) and neuroendocrine reaction. Methods: The patient was a 50-year-old male with a spastic paraplegia due to traumatic SCI. He regularly practiced go-kart racing, reporting a transient reduction in spasticity. He was evaluated before (T0), immediately after (T1), 2 weeks after (T2), and 4 weeks after (T3) a go-kart driving session. On both sides, long adductor, femoral bicep, and medial and lateral gastrocnemius spasticity was assessed using the Modified Ashworth Scale (MAS), and tone and stiffness were assessed using MyotonPro. Results: It was observed that a go-kart driving session could reduce muscle spasticity, tone, and stiffness. Conclusions: Go-kart driving can be a valid tool to obtain results similar to those of WBV and hormone production in the reduction of spasticity.

(1) Background: The “bird dog” exercise is considered one of the most effective therapeutic exercises for lumbopelvic rehabilitation and the prevention and treatment of low back pain. The “standing bird dog” (SBD) exercise, executed in a single-leg stance, constitutes a natural and challenging variation in the “bird dog”; nevertheless, this exercise has not yet been investigated. This study provides a stabilometric and electromyographic analysis of the SBD performed in static and dynamic conditions and in ipsilateral and contralateral variations; (2) Methods: A time-synchronized motion capture system, wireless electromyography sensors, and triaxial force platform were used to analyze the selected SBD exercises; (3) Results: In dynamic conditions, the gluteus maximum, multifidus, lumbar erector spinae, and gluteus medius reached a mean activation level higher than in the static condition, with peak activation levels of 80%, 60%, 55%, and a 45% maximum voluntary isometric contraction, respectively. In the static condition, balance control was more challenging in the mediolateral compared to the anteroposterior direction. In the dynamic condition, the balance challenge was higher in the anteroposterior direction and higher than the static condition in both directions; (4) Conclusions: The SBD was proved to be effective for strengthening the hip and lumbar extensor muscles and provided a powerful challenge to single-leg balance control in both mediolateral and anteroposterior directions.

Abstract After spinal cord injury (SCI), patients face many physical and psychological issues including intestinal dysfunction and comorbidities, strongly affecting quality of life. The gut microbiota has recently been suggested to influence the course of the disease in these patients. However, to date only two studies have profiled the gut microbiota in SCI patients, months after a traumatic injury. Here we characterized the gut microbiota in a large Italian SCI population, within a short time from a not only traumatic injury. Feces were collected within the first week at the rehabilitation center (no later than 60 days after SCI), and profiled by 16S rRNA gene-based next-generation sequencing. Microbial profiles were compared to those publicly available of healthy age- and gender-matched Italians, and correlated to patient metadata, including type of SCI, spinal unit location, nutrition and concomitant antibiotic therapies. The gut microbiota of SCI patients shows distinct dysbiotic signatures, i.e. increase in potentially pathogenic, pro-inflammatory and mucus-degrading bacteria, and depletion of short-chain fatty acid producers. While robust to most host variables, such dysbiosis varies by lesion level and completeness, with the most neurologically impaired patients showing an even more unbalanced microbial profile. The SCI-related gut microbiome dysbiosis is very likely secondary to injury and closely related to the degree of completeness and severity of the lesion, regardless of etiology and time interval. This microbial layout could variously contribute to increased gut permeability and inflammation, potentially predisposing patients to the onset of severe comorbidities.

Despite widespread resident interest in global health and underserved care, few otolaryngology residency programs offer a formal global health experience. This article is the first to characterize a formal otolaryngology global health and underserved care track with a focus on how this curriculum integrates with and supplements resident education. Components of the track include longitudinal limited-resource field experiences in domestic and abroad settings, a related quality improvement project, and completion of a formalized global health educational curriculum. In addition to delivering humanitarian aid, residents in this track obtain a unique educational experience in all 6 core competencies of the Accreditation Council for Graduate Medical Education. Early barriers to implementation included identifying mentorship, securing funding, and managing busy resident schedules. In this work, we detail track components, schedule by track year, keys to implementation, and potential educational pitfalls.

Abstract Background Handwriting represents one of the major symptom in Parkinson’s Disease (PD) patients. The computer-aided analysis of the handwriting allows for the identification of promising patterns that might be useful in PD detection and rating. In this study, we propose an innovative set of features extracted by geometrical, dynamical and muscle activation signals acquired during handwriting tasks, and evaluate the contribution of such features in detecting and rating PD by means of artificial neural networks. Methods Eleven healthy subjects and twenty-one PD patients were enrolled in this study. Each involved subject was asked to write three different patterns on a graphic tablet while wearing the Myo Armband used to collect the muscle activation signals of the main forearm muscles. We have then extracted several features related to the written pattern, the movement of the pen and the pressure exerted with the pen and the muscle activations. The computed features have been used to classify healthy subjects versus PD patients and to discriminate mild PD patients from moderate PD patients by using an artificial neural network (ANN). Results After the training and evaluation of different ANN topologies, the obtained results showed that the proposed features have high relevance in PD detection and rating. In particular, we found that our approach both detect and rate (mild and moderate PD) with a classification accuracy higher than 90%. Conclusions In this paper we have investigated the representativeness of a set of proposed features related to handwriting tasks in PD detection and rating. In particular, we used an ANN to classify healthy subjects and PD patients (PD detection), and to classify mild and moderate PD patients (PD rating). The implemented and tested methods showed promising results proven by the high level of accuracy, sensitivity and specificity. Such results suggest the usability of the proposed setup in clinical settings to support the medical decision about Parkinson’s Disease.

This study investigates a service-learning program for university students seeking teacher certification examined through the lens of sociocultural theory. Data was collected from teacher candidates (TCs) in the form of surveys and an open-ended writing assignment. Administrators from the partner schools were also interviewed to provide their perspectives of the service learning. Findings show that the service-learning program allowed TCs and their English learners (ELs) to form communities of practice that were mutually beneficial. Teacher candidates reported numerous opportunities to apply the strategies learned in their methods course, as well as an increase in their sense of preparation for teaching ELs. Administrators provided positive feedback about the service-learning program in terms of the relationship between the partner school and the university. Difficulties arose in terms of TCs' accountability and the level of participation afforded to the TCs by the classroom teacher. Implications highlight the importance of negotiating expectations that address the needs of both parties equally in order to support a strong community of practice that prepares TCs to meet the needs of ELs.

• Although cognitive processing therapy (CPT) is recommended for the treatment of posttraumatic stress disorder, it is infrequently delivered. • Many clinicians have doubts or concerns about CPT that may impact its usage. • The research literature indicates that CPT is effective without preparatory treatment across a range of outcomes, settings, and populations, including clients with childhood trauma and comorbid conditions. • Although symptom exacerbations occur, data suggest that they are typically temporary and occur at similar rates as in control conditions. Cognitive processing therapy (CPT) is recommended for the treatment of posttraumatic stress disorder (PTSD) but is infrequently delivered. Many clinicians have concerns about CPT that impact usage. Common concerns include (a) CPT's effectiveness in "real-world" settings, (b) whether CPT can be individualized, (c) the possibility of making clients worse, (d) CPT's effectiveness for clients with comorbidities, (e) CPT's effectiveness for clients with childhood trauma, (f) the appropriateness of CPT for moral injury, (g) delivering CPT to culturally diverse populations, (h) initiating CPT without first stabilizing clients with preparatory treatment, (i) the effects of CPT's structure on rapport, (j) whether and when CPT should be stopped, and (k) whether clients will maintain their gains long term. To inform clinical decision-making, findings from the psychological literature are presented for each concern. The evidence provides support for CPT's safety, tolerability, acceptability, and effectiveness across populations and settings, as well as for delivery of CPT without preparatory treatment and across a range of secondary outcomes. Although symptom exacerbations occur, evidence suggests they are typically temporary and occur at similar rates as in control conditions. Findings highlight the role of fidelity in symptom outcome. (1) Background: The static body-weight wall-squat (SBWS) exercise is often included in the early rehabilitation stages of the lower limb. To establish its effectiveness and optimise its application, it is important to precisely quantify the muscle contribution to different versions of this exercise. (2) Methods: We analysed the electromyographic activity of lower limb and lumbar muscles during the SBWS with knees flexed at 45° by manipulating three different variables: horizontal distance of the ankles from the wall; scapular or pelvic location of the centre of pressure Cwall of the force exerted by the wall on the back; rearfoot or forefoot location of the centre of pressure CGR of the ground reaction force. (3) Results: The forefoot-to-rearfoot CGR shift significantly increased the vastus medialis, vastus lateralis, and tibialis anterior activity up to 23%, 26%, and 44% of the maximum voluntary isometric contraction (MVIC). The lumbar muscle activity was maximised (23% MVIC) shifting CGR at the forefoot, Cwall at the scapular zone, and placing the feet far from the wall. (4) Conclusions: These SBWS methods might be effective for quadriceps, tibialis anterior, and lumbar muscles strengthening in the early phase of rehabilitation intervention as soon as the patient can tolerate partial weight-bearing. The exercise appears suitable for patients with low back pain and limited lumbar muscle endurance, or quadriceps weakness and inhibition secondary to a knee injury.

INTRODUCTION:Brain-Derived Neurotrophic Factor (BDNF) and its most common polymorphism Val66Met are known to have a role in Multiple Sclerosis (MS) pathogenesis. Evidence is accumulating that there is an involvement of DNA methylation in the regulation of BDNF expression. The aim of this study was to assess in blood samples of MS patients the correlation between the methylation status of the CpG site near BDNF-Val66Met polymorphism and the severity of the disease. METHODS:We recruited 209 MS patients that were genotyped for the BDNF Val66Met polymorphism. For each patient we quantitatively measured the methylation level of cytosine included in the exonic CpG site that can be created or abolished by the Val66Met BDNF polymorphism. Furthermore, we analyzed the clinical history of each patient and determined the time elapsed since the onset of the disease and an EDSS score of 6.0. RESULTS:The genetic analysis identified 122 (58.4%) subjects carrying the Val/Val genotype, 81 (38.8%) with Val/Met genotype, and 6 (2.8%) carrying the Met/Met genotype. When the endpoint of an EDSS score of 6 was taken into account by means of a survival analysis, 52 failures (i.e., reaching an EDSS score of 6) were reported. When the sample was stratified according to the percentage of the BDNF methylation, subjects falling below the median (median methylation = 81%) were at higher risk of failure (IRD = 0.016; 95%CI = 0.0050-0.0279; p = 0.004). CONCLUSIONS:In patients with a high disease progression the hypomethylation of the BDNF gene could increase the secretion of the protective neurotrophin, so epigenetic modifications could be the organism response to limit a brain functional reserve loss. Our study suggests that the percentage of methylation of the BDNF gene could be used as a prognostic factor for disease progression toward a high disability in MS patient.

Media and network systems capture and store data about electronic activity in new, sometimes unprecedented ways; computational systems make for new means of analysis and knowledge development. These new forms offer new, powerful tactical tools for investigations of electronic malfeasance under traditional legal regulation of state power, particular that of Fourth Amendment limitations on police searches and seizures under the U.S. Constitution. But autonomy, identity and authenticity concerns with electronic data raise issues of public policy, privacy and proper police oversight of civil society. We examine those issues and their implications for digital and computational forensics.

The world is experiencing an evolution of Smart Cities. These emerge from innovations in information technology that, while they create new economic and social opportunities, pose challenges to our security and expectations of privacy. Humans are already interconnected via smart phones and gadgets. Smart energy meters, security devices and smart appliances are being used in many cities. Homes, cars, public venues and other social systems are now on their path to the full connectivity known as the “Internet of Things.” Standards are evolving for all of these potentially connected systems. They will lead to unprecedented improvements in the quality of life. To benefit from them, city infrastructures and services are changing with new interconnected systems for monitoring, control and automation. Intelligent transportation, public and private, will access a web of interconnected data from GPS location to weather and traffic updates. Integrated systems will aid public safety, emergency responders and in disaster recovery. We examine two important and entangled challenges: security and privacy. Security includes illegal access to information and attacks causing physical disruptions in service availability. As digital citizens are more and more instrumented with data available about their location and activities, privacy seems to disappear. Privacy protecting systems that gather data and trigger emergency response when needed are technological challenges that go hand-in-hand with the continuous security challenges. Their implementation is essential for a Smart City in which we would wish to live. We also present a model representing the interactions between person, servers and things. Those are the major element in the Smart City and their interactions are what we need to protect.

Knowing what event precipitated a client's abnormal behaviors makes the client appear more normal than if the event is not known (Meehl, 1973). Does such knowledge also influence judgments of the need for psychological treatment, and if so, does it matter whether the precipitating event was inside or outside the client's control? We presented undergraduates with cases of hypothetical clients exhibiting abnormal behaviors and manipulated whether they were also told of a precipitating event explaining those behaviors. Knowing the precipitant significantly reduced perceptions of clients’ need for treatment, but only when the precipitating event was outside the client's control. These findings call into question the notion that it need always be beneficial for an outside reasoner to uncover the root cause of a client's psychological problems, particularly when the root cause is still unknown to the client. The rationality of the effect and additional implications for decision-making are discussed.

Las aplicaciones móviles sensibles al contexto presentan características propias que afectan el proceso de desarrollo del software. Estas deben ejecutarse en diferentes plataformas de computación, ajustarse a las capacidades de diversos dispositivos de acceso y adaptarse a diferentes contextos de uso. El contexto de uso facilita la utilización del ambiente en una forma que garantice la calidad del servicio al usuario. Esto es un aspecto determinante en aplicaciones móviles, generalmente inalámbricas, cuyas propiedades imponen como determinante la gestión de la calidad de estos servicios. El nivel de servicio requerido se especifica de acuerdo a un modelo de calidad. La meta de este trabajo es definir un modelo de calidad para el dominio de aplicaciones móviles sensibles al contexto. La importancia de este modelo es la especificación de los requisitos de calidad para el producto final del software y éste puede ser usado para una evaluación cuantitativa de todos los productos de software durante el proceso de desarrollo. Este modelo se define utilizando una taxonomía denominada RECLAMO (Requirements Classification Model) y el nuevo estándar ISO/IEC 25030.

Objective: Serious device-related complications for hypoglossal nerve stimulators are rare, but surgeons should implement a prompt and systematic approach to quickly troubleshoot a non-functioning device. Method: Records were queried at a single academic tertiary referral centre between January 2019 and June 2021. Results: The authors present four cases of non-functioning hypoglossal nerve stimulator devices: one case in which migration of the stimulation lead required a revision implantation, one in which the implantable pulse generator was found to be non-functional intra-operatively, one case of an intramuscular sensory lead tract causing pain and one case of implantable pulse generator failure that was probably triggered by implantable cardiac device discharge. In this study, computed tomography imaging was critical to the diagnosis for the first and third cases. Conclusion: Given the limited complication reporting available for hypoglossal nerve stimulators, these cases highlight management and unique imaging findings. The Requirements Engineering (RE) discipline is where the software system needs or requirements are captured; these are then “translated” into software components. At present, functional requirements are treated, but non-functional requirements (NFR) are neglected, causing problems at later stages of development. In an industrial software production context, product quality must be considered, and the Domain Analysis discipline within RE, proposes different approaches to treat NFR for building a Reference Architecture (RA) from which all products of a domain family can be generated. Consequently, the same process is adapted to different contexts and abstraction levels. This paper proposes a Unified Process for Domain Analysis (UPDA), based on Aspect and Goal orientations to deal with NFR, specified by quality standards to enhance communication. UPDA integrates techniques that are separately used: - the Chung and others extended process of Losavio and others, based on the NFR Framework with treatment of crosscutting concerns, and – the ISO/IEC 25010 quality standard to specify NFR. Three sub-processes constitute UPDA: - Construction of the quality model, - Identification of crosscutting concerns and - RA design. The main artifact obtained is the RA, which can be reused as an asset in the context of software product lines.
