1. Behavioral and EEG evidence for inter-individual variability in late encoding stages of word production

Abstract

Individuals differ in the time it takes to produce words when naming a picture. However, it is unknown whether this inter-individual variability emerges in earlier stages of word production (e.g., lexical selection) or later stages (e.g., articulation). The current study measured participants' (N = 45) naming latencies and continuous EEG in a picture-word-interference task, as well as naming latencies in a delayed naming task. The inter-individual variability in naming latencies in immediate naming was not larger than the variability in the delayed task. Thus, a large part of the variability in immediate naming seems to originate in relatively late stages of word production. This interpretation was complemented by the EEG data: Differences between relatively fast vs. slow speakers were seen in response-aligned analyses in a time window close to the vocal response. Finally, we show that inter-individual variability can influence EEG results at the group level.

2. On the Lexical Representation(s) of Compounds: A Continuous Picture Naming Study

Abstract

The lexical representation of compound words in speech production is still under debate. While most studies with healthy adult speakers suggest that a single lemma representation is active during compound production, data from neuropsychological studies point toward multiple representations, with activation of the compound's constituent lemmas in addition to the compound's lemma. This study exploits the cumulative semantic interference effect to investigate the lexical representation of compounds in speech production. In a continuous picture naming experiment, category membership was established through the compounds' first constituents (category animals: "'zebra' crossing," "'pony' tail," "'cat' litter" …), while the compounds themselves were not semantically related. Moreover, pictures depicting the compounds' first constituents ("zebra," "pony," "cat" …) were presented as a control condition. As expected, naming latencies within categories increased linearly with each additionally named category member when producing monomorphemic words, which is interpreted as increasing interference during lexical selection. Importantly, this cumulative semantic interference effect was also observed for compounds. This indicates that the lemmas of the compounds' first constituents were activated during compound production, causing interference due to their semantic relationship and thereby hampering the production of the whole compound. The results are thus in line with the multiple-lemma representation account (Marelli et al., 2012). We argue that the apparent contradiction between results of previous studies with healthy adult speakers and our current study can be explained by the different experimental paradigms used.

Abstract

Diese Dissertation untersucht einige ungeklärten Aspekte der Sprachproduktion. Das erste Ziel war es zu klären, wie Komposita (z.B. Goldfisch) auf der lexikalisch-syntaktischen Ebene unseres Sprachproduktionssystems repräsentiert sind. Gibt es dort einen einzelnen lexikalischen Eintrag für das gesamte Kompositum (GOLDFISCH) oder mehrere Einträge für jedes seiner Konstituenten (GOLD und FISCH), welche beim Sprechen zusammengesetzt werden? Zur Beantwortung dieser Frage wurde die sogenannte kumulative semantische Interferenz (KSI) verwendet. Dieser semantische Kontexteffekt beschreibt die Beobachtung, dass die Benennlatenzen von Sprechern systematisch länger werden, wenn diese eine Reihe von semantisch verwandten Bildern benennen. Obwohl KSI bereits viel als Instrument in der Sprachproduktionsforschung genutzt wird, sind einige Fragen rund um den Effekt selbst noch offen. Das zweite Ziel dieser Dissertation war es daher einige dieser Fragen mit Hilfe von behavioralen und elektrophysiologischen Maßen zu beantworten, um so unser Verständnis von KSI zu erweitern. Die Ergebnisse deuten darauf hin, dass KSI ihren Ursprung auf der konzeptuellen Ebene des Sprachproduktionssystems hat und dass sie nicht von der morphologischen Komplexität der verwendeten Begriffe moduliert wird, aber davon, wie häufig diese benannt werden. Diese Erkenntnisse ermöglichen es in der Zukunft zielgenauere Vorhersagen zu machen, wenn KSI als Forschungsinstrument verwendet wird. Die Ergebnisse zeigen zudem, dass die Konstituenten von Komposita während deren Produktion aktiviert werden. Dies belegt, dass Komposita in einer komplexen Struktur repräsentiert sind, die aus einem Eintrag für das ganze Kompositum und zusätzlichen Einträgen für die Konstituenten besteht. Somit zeigen diese Ergebnisse, dass die Morphologie bereits die Repräsentationen auf der lexikalisch-syntaktischen Ebene beeinflusst und erweitern somit unser Wissen über den Aufbau unseres Sprachproduktionssystems., This dissertation addresses unresolved issues concerning speech production processes and the cognitive architecture of our speech production system. The first aim was to answer the question how compounds (e.g., goldfish) are represented on the lexical-syntactic level of our speech production system. Is there a single entry for the whole compound (GOLDFISH) or multiple ones for each of its constituents (GOLD and FISH), which are assembled for each use? To investigate this question, we used the cumulative semantic interference (CSI) effect. This semantic context effect describes the observation that speakers’ naming latencies systematically increase when naming a sequence of semantically related pictures. Although CSI has been extensively used as a tool in language production research, several aspects of it are not fully understood. Thus, the second aim of this dissertation was to close some of these knowledge gaps and gain a more comprehensive understanding of CSI. In three studies, we first investigated the CSI effect, before using it as a tool to study the lexical representation of compounds. Behavioural and electrophysiological data from the first two studies point to a purely conceptual origin of CSI. Furthermore, they revealed that CSI is not influenced by the items’ morphological complexity but affected by item repetition. These findings advance our understanding of CSI and thus allow us to make more informed predictions when using CSI as a research tool. The last study showed that the compounds’ constituents are activated during compound production, which provides evidence for a complex lexical-syntactic representation of compounds, consisting of one entry for the holistic compound and additional entries for each of its constituents. This dissertation thus reveals that the morphological complexity of compounds affects the lexical-syntactic level during speech production and thus advances our understanding of the architecture of our speech production system.

Abstract

Background: Collation of aphasia research data across settings, countries and study designs using big data principles will support analyses across different language modalities, levels of impairment, and therapy interventions in this heterogeneous population. Big data approaches in aphasia research may support vital analyses, which are unachievable within individual trial datasets. However, we lack insight into the requirements for a systematically created database, the feasibility and challenges and potential utility of the type of data collated. Aim: To report the development, preparation and establishment of an internationally agreed aphasia after stroke research database of individual participant data (IPD) to facilitate planned aphasia research analyses. Methods: Data were collated by systematically identifying existing, eligible studies in any language ( ≥ 10 IPD, data on time since stroke, and language performance) and included sourcing from relevant aphasia research networks. We invited electronic contributions and also extracted IPD from the public domain. Data were assessed for completeness, validity of value-ranges within variables, and described according to pre-defined categories of demographic data, therapy descriptions, and language domain measurements. We cleaned, clarified, imputed and standardised relevant data in collaboration with the original study investigators. We presented participant, language, stroke, and therapy data characteristics of the final database using summary statistics. Results: From 5256 screened records, 698 datasets were potentially eligible for inclusion; 174 datasets (5928 IPD) from 28 countries were included, 47/174 RCT datasets (1778 IPD) and 91/174 (2834 IPD) included a speech and language therapy (SLT) intervention. Abstract

Background: Stroke rehabilitation interventions are routinely personalized to address individuals' needs, goals, and challenges based on evidence from aggregated randomized controlled trials (RCT) data and meta-syntheses. Individual participant data (IPD) meta-analyses may better inform the development of precision rehabilitation approaches, quantifying treatment responses while adjusting for confounders and reducing ecological bias. Aim: We explored associations between speech and language therapy (SLT) interventions frequency (days/week), intensity (h/week), and dosage (total SLT-hours) and language outcomes for different age, sex, aphasia severity, and chronicity subgroups by undertaking prespecified subgroup network meta-analyses of the RELEASE database. Methods: MEDLINE, EMBASE, and trial registrations were systematically searched (inception-Sept2015) for RCTs, including > 10 IPD on stroke-related aphasia. We extracted demographic, stroke, aphasia, SLT, and risk of bias data. Overall-language ability, auditory comprehension, and functional communication outcomes were standardized. A one-stage, random effects, network meta-analysis approach filtered IPD into a single optimal model, examining SLT regimen and language recovery from baseline to first post-intervention follow-up, adjusting for covariates identified a-priori. Data were dichotomized by age (<=/> 65 years), aphasia severity (mild-moderate/ moderate-severe based on language outcomes' median value), chronicity (<=/> 3 months), and sex subgroups. We reported estimates of means and 95% confidence intervals. Where relative variance was high (> 50%), results were reported for completeness. Results: 959 IPD (25 RCTs) were analyzed. Abstract

BACKGROUND AND PURPOSE: Optimizing speech and language therapy (SLT) regimens for maximal aphasia recovery is a clinical research priority. We examined associations between SLT intensity (hours/week), dosage (total hours), frequency (days/week), duration (weeks), delivery (face to face, computer supported, individual tailoring, and home practice), content, and language outcomes for people with aphasia. METHODS: Databases including MEDLINE and Embase were searched (inception to September 2015). Published, unpublished, and emerging trials including SLT and >= 10 individual participant data on aphasia, language outcomes, and time post-onset were selected. Patient-level data on stroke, language, SLT, and trial risk of bias were independently extracted. Outcome measurement scores were standardized. A statistical inferencing, one-stage, random effects, network meta-analysis approach filtered individual participant data into an optimal model examining SLT regimen for overall language, auditory comprehension, naming, and functional communication pre-post intervention gains, adjusting for a priori-defined covariates (age, sex, time poststroke, and baseline aphasia severity), reporting estimates of mean change scores (95% CI). RESULTS: Data from 959 individual participant data (25 trials) were included. Greatest gains in overall language and comprehension were associated with >20 to 50 hours SLT dosage (18.37 [10.58-26.16] Western Aphasia Battery-Aphasia Quotient; 5.23 [1.51-8.95] Aachen Aphasia Test-Token Test). Greatest clinical overall language, functional communication, and comprehension gains were associated with 2 to 4 and 9+ SLT hours/week. Greatest clinical gains were associated with frequent SLT for overall language, functional communication (3-5+ days/week), and comprehension (4-5 days/week). Abstract

Background: Collation of aphasia research data across settings, countries and study designs using big data principles will support analyses across different language modalities, levels of impairment, and therapy interventions in this heterogeneous population. Big data approaches in aphasia research may support vital analyses, which are unachievable within individual trial datasets. However, we lack insight into the requirements for a systematically created database, the feasibility and challenges and potential utility of the type of data collated. Aim: To report the development, preparation and establishment of an internationally agreed aphasia after stroke research database of individual participant data (IPD) to facilitate planned aphasia research analyses. Methods: Data were collated by systematically identifying existing, eligible studies in any language (≥10 IPD, data on time since stroke, and language performance) and included sourcing from relevant aphasia research networks. We invited electronic contributions and also extracted IPD from the public domain. Data were assessed for completeness, validity of value-ranges within variables, and described according to pre-defined categories of demographic data, therapy descriptions, and language domain measurements. We cleaned, clarified, imputed and standardised relevant data in collaboration with the original study investigators. We presented participant, language, stroke, and therapy data characteristics of the final database using summary statistics. Results: From 5256 screened records, 698 datasets were potentially eligible for inclusion; 174 datasets (5928 IPD) from 28 countries were included, 47/174 RCT datasets (1778 IPD) and 91/174 (2834 IPD) included a speech and language therapy (SLT) intervention. Abstract

Background and Purpose: The factors associated with recovery of language domains after stroke remain uncertain. We described recovery of overall-language-ability, auditory comprehension, naming, and functional-communication across participants' age, sex, and aphasia chronicity in a large, multilingual, international aphasia dataset. Methods: Individual participant data meta-analysis of systematically sourced aphasia datasets described overall-language ability using the Western Aphasia Battery Aphasia-Quotient; auditory comprehension by Aachen Aphasia Test (AAT) Token Test; naming by Boston Naming Test and functional-communication by AAT Spontaneous-Speech Communication subscale. Multivariable analyses regressed absolute score-changes from baseline across language domains onto covariates identified a priori in randomized controlled trials and all study types. Change-from-baseline scores were presented as estimates of means and 95% CIs. Heterogeneity was described using relative variance. Risk of bias was considered at dataset and meta-analysis level. Results: Assessments at baseline (median=43.6 weeks poststroke; interquartile range [4-165.1]) and first-follow-up (median=10 weeks from baseline; interquartile range [3-26]) were available for n=943 on overall-language ability, n=1056 on auditory comprehension, n=791 on naming and n=974 on functional-communication. Abstract

Background: Speech and language therapy (SLT) benefits people with aphasia following stroke. Group level summary statistics from randomised controlled trials hinder exploration of highly complex SLT interventions and a clinically relevant heterogeneous population. Creating a database of individual participant data (IPD) for people with aphasia aims to allow exploration of individual and therapy-related predictors of recovery and prognosis. Aim: To explore the contribution that individual participant characteristics (including stroke and aphasia profiles) and SLT intervention components make to language recovery following stroke. Methods and procedures: We will identify eligible IPD datasets (including randomised controlled trials, non-randomised comparison studies, observational studies and registries) and invite their contribution to the database. Where possible, we will use meta- and network meta-analysis to explore language performance after stroke and predictors of recovery as it relates to participants who had no SLT, historical SLT or SLT in the primary research study. We will also examine the components of effective SLT interventions. Outcomes and results: Outcomes include changes in measures of functional communication, overall severity of language impairment, auditory comprehension, spoken language (including naming), reading and writing from baseline. Data captured on assessment tools will be collated and transformed to a standardised measure for each of the outcome domains. Conclusion: Our planned systematic-review-based IPD meta- and network meta-analysis is a large scale, international, multidisciplinary and methodologically complex endeavour. It will enable hypotheses to be generated and tested to optimise and inform development of interventions for people with aphasia after stroke. Abstract

Purpose: Speech and language pathology (SLP) for aphasia is a complex intervention delivered to a heterogeneous population within diverse settings. Simplistic descriptions of participants and interventions in research hinder replication, interpretation of results, guideline and research developments through secondary data analyses. This study aimed to describe the availability of participant and intervention descriptors in existing aphasia research datasets. Method: We systematically identified aphasia research datasets containing ≥10 participants with information on time since stroke and language ability. We extracted participant and SLP intervention descriptions and considered the availability of data compared to historical and current reporting standards. We developed an extension to the Template for Intervention Description and Replication checklist to support meaningful classification and synthesis of the SLP interventions to support secondary data analysis. Result: Of 11, 314 identified records we screened 1131 full texts and received 75 dataset contributions. We extracted data from 99 additional public domain datasets. Participant age (97.1%) and sex (90.8%) were commonly available. Prior stroke (25.8%), living context (12.1%) and socio-economic status (2.3%) were rarely available. Therapy impairment target, frequency and duration were most commonly available but predominately described at group level. Home practice (46.3%) and tailoring (functional relevance 46.3%) were inconsistently available. Conclusion: Gaps in the availability of participant and intervention details were significant, hampering clinical implementation of evidence into practice and development of our field of research. Improvements in the quality and consistency of participant and intervention data reported in aphasia research are required to maximise clinical implementation, replication in research and the generation of insights from secondary data analysis.
