Abstract

Cochlear implantation is a timeous and cost-effective solution for severe-profound sensorineural hearing loss, and has transformed the lives of many individuals with significant hearing loss. The advent of cochlear implantation has meant that, for the first time, one of the senses (hearing), having been entirely lost, can be restored. This, the first of a two-part series on cochlear implantation in South Africa (SA) puts global (and especially SA) hearing loss in perspective, sets out the principles of cochlear implantation and briefly sketches the history of this world-changing technology.

30. The simple bread tag - a menace to society?

Abstract

Foreign bodies (FBs) are potentially life-threatening when inhaled by a child, depending on where they lodge. Symptoms can range from acute upper airway obstruction to mild, vague respiratory complaints. Between 80% and 90% of inhaled FBs occlude the bronchi, while the larynx is a less common site. The commonest inhaled paediatric FBs are organic, e.g. seeds or nuts. Plastic FBs are less common and more difficult to diagnose. They are generally radiolucent on lateral neck radiographs and are often clear and thin. We report three cases of an unusual plastic laryngeal FB, the bread tag. Plastic bread tags were first reported in the medical literature as an ingested gastrointestinal FB in 1975. Since then, over 20 cases of gastrointestinal complications have been described. We report what is to our knowledge the first paediatric case of an inhaled bread tag, and also the first case series, briefly discuss the symptoms and options for removal of laryngeal FBs, and highlight the dangers of the apparently harmless bread tag. Images of the bread tags in situ and after their removal are included.

Abstract

The North Sea is threatened by eutrophication. Excess supply of nutrients, especially nitrogen compounds, causes proliferation of algae, leading to pernicious oxygen depletion. The nitrogen supply is not only due to rivers, but to the atmosphere as well. Especially in summer, when nutrients are scarce and the river supply is reduced, the atmospheric contribution can become predominant. The relevant atmospheric nitrogen is emitted as nitrogen oxides (by traffic and industry) and as ammonia (by agriculture). Organic nitrogen compounds might be important but their deposition fluxes are unknown.Measurements of the concentrations of the various inorganic and organic nitrogen components and of their deposition, with rain and in the dry phase, will allow evaluating the contribution of the atmosphere to eutrophication processes, using adequate mathematical models.

37. Assessment of potential toxic influence of uranium trial mining in the Karoo uranium province

Abstract

An assessment of uranium trial mining on four mining sites in the Karoo Uranium Province, South Africa revealed localised above-background values for U, Mo, Pb, Cu, As and Fe in surface - and ground water, soils, sediment and crops. Inadequate remedial action on cessation of mining activities in 1980 led to the presence of uranium ore in stockpiles, open pits, mining shafts, mining equipment and waste dumps within featured areas. Heavy metal contamination is suppressed by the lack of run–off and the dry climate experienced within the mining areas. However, the heavy metal content in surface water and sediment within the open pits on Rietkuil and Mooifontein is especially high. These values pose a risk for human ingestion and may cause cancer in the long term or renal damage over the short term. These pits are easily accessed, lack a fence and are used for a drinking medium by fauna and as a growth medium for flora. The easily accessed Cameron Shaft on Ryst Kuil is a matter of concern due to the possible presence of the radioactive inert gas, radon. Farm owners were unaware of the possible toxic effects of uranium and coherent heavy metals. This led to previous usage of mine water for crop irrigation, the moving and feeding of livestock as well as wildlife amongst uranium ore stockpiles, swimming in water-filled open pits and using crushed uranium ore for gravel road maintenance and construction. The presence of uranium ore in stockpiles and the coherent effects on the water, soils, sediment, fauna and flora and possibly man, prioritises the remediation and rehabilitation of the of uranium trial mining sites within the Karoo Uranium Province.
