Abstract

Mechanisms behind the phenomenon of Arctic amplification are widely discussed. To contribute to this debate, the (AC)3 project was established in 2016 (www.ac3-tr.de/). It comprises modeling and data analysis efforts as well as observational elements. The project has assembled a wealth of ground-based, airborne, shipborne, and satellite data of physical, chemical, and meteorological properties of the Arctic atmosphere, cryosphere, and upper ocean that are available for the Arctic climate research community. Short-term changes and indications of long-term trends in Arctic climate parameters have been detected using existing and new data. For example, a distinct atmospheric moistening, an increase of regional storm activities, an amplified winter warming in the Svalbard and North Pole regions, and a decrease of sea ice thickness in the Fram Strait and of snow depth on sea ice have been identified. A positive trend of tropospheric bromine monoxide (BrO) column densities during polar spring was verified. Local marine/biogenic sources for cloud condensation nuclei and ice nucleating particles were found. Atmospheric–ocean and radiative transfer models were advanced by applying new parameterizations of surface albedo, cloud droplet activation, convective plumes and related processes over leads, and turbulent transfer coefficients for stable surface layers. Four modes of the surface radiative energy budget were explored and reproduced by simulations. To advance the future synthesis of the results, cross-cutting activities are being developed aiming to answer key questions in four focus areas: lapse rate feedback, surface processes, Arctic mixed-phase clouds, and airmass transport and transformation.

Abstract

Airborne meteorological and aerosol measurements have been performed in Longyearbyen (Svalbard islands) in the summer of 2018, coupling an instrumental aerosol payload with a meteorological radiosonde deployed on a tethered balloon. More than 70 vertical profiles of aerosol and meteorological properties have been recorded up to a maximum altitude of 1.2 km. As a main result, the present work provides a homogeneous gridded dataset of vertical profiles of equivalent black carbon (eBC) and nanoparticles (NP) concentrations and associated meteorological data (temperature, T, relative humidity, RH, pressure, P) to be employed for future modelling studies of Arctic pollution. Mean values (±SD) of eBC and NP below 500 m were 110 ± 10 ng m−3 and 1400 ± 400 particles cm−3, respectively. Mean values above 500 m were 150 ± 30 ng m−3 and 1000 ± 350 particles cm−3, respectively. Group medians of maximum eBC and NP concentrations in vertical profiles with temperature inversions were significantly higher than for those without inversion. The dataset has been complemented by continuous ground measurements of eBC with an average value of 208 ± 130 ng m−3 (median value 110 ± 70 ng m−3) for the entire campaign; the ground-based background (absence of local emission) eBC value was below 100 ng m−3 while maximum values were in the 1000–2000 ng m−3 range. Median eBC concentration measured at ground for 2 h before the tethered balloon launch was higher when temperature inversion was observed. The ground-based measurements, coupled with aerosol optical depth measurements, allowed for a preliminary discussion of two case studies related to high pollutants concentration events.

Abstract

The tethered balloon system BELUGA (BalloonbornE moduLar Utility for profilinG the lower Atmosphere) was operated in spring 2022 at the AWIPEV research station (Ny-Ålesund, Svalbard). In-situ profiles of thermodynamic parameters, thermalinfrared radiation, aerosol particle concentrations, and turbulence, were measured and analyzed. Additionally, samples of ice-nucleating particles were collected at various heights. In combination with previous BELUGA datasets, measurements from this campaign provide a solid base for studying the vertical profiles of the radiative energy budget and heating rates in different atmospheric states in the Arctic lower troposphere. Here, example thermal-infrared radiation profiles are presented for a period of persisting cloudless conditions related to a series of marine cold air outbreaks in late March/early April. Measurements in clouds are analyzed for a developing cloud observed on 6 May and display the impact of cloudiness on radiation profiles., Das Fesselballonsystem BELUGA (BalloonbornE moduLar Utility for profilinG the lowerAtmosphere) wurde im Frühjahr 2022 an der Forschungsstation AWIPEVStation (Ny-Ålesund, Svalbard) eingesetzt. Zusätzlich wurden in verschiedenen Höhen Proben von eiskeimbildenden Partikeln gesammelt. In Kombination mit früheren BELUGA Messungen zu anderen Jahreszeiten und an anderen arktischen Messstandorten bieten die Messungen in Ny-Ålesund eine Grundlage fürweitereUntersuchungen des Strahlungsenergiehaushalts und des Einflusses vonWolken auf atmosphärische Heizraten. Profile der Strahlungsbilanz werden für eine anhaltende Kälteperiode zwischen Ende März bis Anfang April 2022 vorgestellt. Über diesen Zeitraum herrschten vor allem wolkenlose Bedingungen. Weitere Beobachtungen unter einer sich entwickelnden Wolkendecke am 6. Mai 2022 zeigen den Einfluss der Bewölkung auf die Strahlungsprofile.

Abstract

The HALO-(AC)3 campaign was conducted in March and April 2022 to investigate warm air intrusions into the Arctic and marine cold air outbreaks. In coordinated flights over the Arctic, the High Altitude and Long Range Research Aircraft (HALO), equipped with a remote sensing payload and dropsondes, investigated these air mass transformations together with the research aircraft Polar 5 and Polar 6. In this report, we give an overview about the research flights and preliminary results from projects, which are carried out by employees of the Leipzig Institute for Meteorology (LIM)., Die HALO-(AC)3 Kampagne wurde im März und April 2022 durchgeführt, umWarmlufteinbrüche in die Arktis und marine Kaltluftausbrüche zu untersuchen. Das 'High Altitude and Long Range Research Aircraft' (HALO), ausgestattet mit Instrumenten zur Fernerkundung und Standardmeteorologiesonden, untersuchte zusammen mit den Forschungsflugzeugen Polar 5 und Polar 6, in koordinierten Flügen über der Arktis, diese Veränderungen der Luftmassen. In diesem Bericht wird eine Übersicht über die durchgeführten Forschungsflüge gegeben und Forschungsprojekte werden vorgestellt, welche von Mitarbeitern des Leipziger Instituts für Meteorologie (LIM) durchgeführt werden.

Abstract

With the Arctic rapidly changing, the needs to observe, understand, and model the changes are essential. To support these needs, an annual cycle of observations of atmospheric properties, processes, and interactions were made while drifting with the sea ice across the central Arctic during the Multidisciplinary drifting Observatory for the Study of Arctic Climate (MOSAiC) expedition from October 2019 to September 2020. An international team designed and implemented the comprehensive program to document and characterize all aspects of the Arctic atmospheric system in unprecedented detail, using a variety of approaches, and across multiple scales. These measurements were coordinated with other observational teams to explore crosscutting and coupled interactions with the Arctic Ocean, sea ice, and ecosystem through a variety of physical and biogeochemical processes. This overview outlines the breadth and complexity of the atmospheric research program, which was organized into 4 subgroups: atmospheric state, clouds and precipitation, gases and aerosols, and energy budgets. Atmospheric variability over the annual cycle revealed important influences from a persistent large-scale winter circulation pattern, leading to some storms with pressure and winds that were outside the interquartile range of past conditions suggested by long-term reanalysis. Similarly, the MOSAiC location was warmer and wetter in summer than the reanalysis climatology, in part due to its close proximity to the sea ice edge. The comprehensiveness of the observational program for characterizing and analyzing atmospheric phenomena is demonstrated via a winter case study examining air mass transitions and a summer case study examining vertical atmospheric evolution. Overall, the MOSAiC atmospheric program successfully met its objectives and was the most comprehensive atmospheric measurement program to date conducted over the Arctic sea ice. Abstract

The BalloonbornE moduLar Utility for profilinG the lower Atmosphere (BELUGA) was deployed in autumn 2021 in the Arctic at the AWIPEV research station in Ny-Ålesund (Svalbard). In-situ profiles of thermodynamic parameters, broadband radiation, turbulence, aerosol particle concentrations, and cloud microphysical structure, were performed. Additionally, samples of ice nucleating particles were collected. Thermal infrared radiation profiles are presented for different cloud conditions. The data provides the base for studying the vertical distribution of cloud radiative effects, and extends the common view of the bi-modal distribution of the Arctic surface energy budget., Das Fesselballonsystem BalloonbornE moduLar Utility for profilinG the lower Atmosphere (BELUGA) wurde im Herbst 2021 an der Forschungsstation Ny-Ålesund in der Arktis eingesetzt. Es wurden In-situ Profile von thermodynamischen Parametern, breitbandiger Strahlung, Turbulenz, Aerosolpartikelkonzentrationen und der mikrophysikalischer Wolkenstruktur erstellt. Zusätzlich wurden Proben von eiskeimbildenden Partikeln gesammelt. Strahlungsprofile wurden unter verschiedenen Wolkenbedeckungen gemessen und quantifizieren die vertikale Verteilung der Wolkenstrahlungseffekte. Die Profilmessungen erweitern damit die für bodennahe Messungen bekannte modale Verteilung des Energiehaushalts der Arktis.

Abstract

Background: A growing body of evidence suggests that non-viral hepatocellular carcinoma (HCC) might benefit less from immunotherapy. Materials and methods: We carried out a retrospective analysis of prospectively collected data from consecutive patients with non-viral advanced HCC, treated with atezolizumab plus bevacizumab, lenvatinib, or sorafenib, in 36 centers in 4 countries (Italy, Japan, Republic of Korea, and UK). The primary endpoint was overall survival (OS) with atezolizumab plus bevacizumab versus lenvatinib. Secondary endpoints were progression-free survival (PFS) with atezolizumab plus bevacizumab versus lenvatinib, and OS and PFS with atezolizumab plus bevacizumab versus sorafenib. For the primary and secondary endpoints, we carried out the analysis on the whole population first, and then we divided the cohort into two groups: non-alcoholic fatty liver disease (NAFLD)/non-alcoholic steatohepatitis (NASH) population and non-NAFLD/NASH population. Results: One hundred and ninety patients received atezolizumab plus bevacizumab, 569 patients received lenvatinib, and 210 patients received sorafenib. In the whole population, multivariate analysis showed that treatment with lenvatinib was associated with a longer OS [hazard ratio (HR) 0.65; 95% confidence interval (CI) 0.44-0.95; P = 0.0268] and PFS (HR 0.67; 95% CI 0.51-0.86; P = 0.002) compared to atezolizumab plus bevacizumab. In the NAFLD/NASH population, multivariate analysis confirmed that lenvatinib treatment was associated with a longer OS (HR 0.46; 95% CI 0.26-0.84; P = 0.0110) and PFS (HR 0.55; 95% CI 0.38-0.82; P = 0.031) compared to atezolizumab plus bevacizumab. In the subgroup of non-NAFLD/NASH patients, no difference in OS or PFS was observed between patients treated with lenvatinib and those treated with atezolizumab plus bevacizumab. All these results were confirmed following propensity score matching analysis. By comparing patients receiving atezolizumab plus bevacizumab versus sorafenib, no statistically significant difference in survival was observed. Conclusions: The present analysis conducted on a large number of advanced non-viral HCC patients showed for the first time that treatment with lenvatinib is associated with a significant survival benefit compared to atezolizumab plus bevacizumab, in particular in patients with NAFLD/NASH-related HCC.

Abstract

Airborne meteorological and aerosol measurements have been performed in Longyearbyen (Svalbard islands) in the summer of 2018, coupling an instrumental aerosol payload with a meteorological radiosonde deployed on a tethered balloon. More than 70 vertical profiles of aerosol and meteorological properties have been recorded up to a maximum altitude of 1.2 km. As a main result, the present work provides a homogeneous gridded dataset of vertical profiles of equivalent black carbon (eBC) and nanoparticles (NP) concentrations and associated meteorological data (temperature, T, relative humidity, RH, pressure, P) to be employed for future modelling studies of Arctic pollution. Mean values (±SD) of eBC and NP below 500 m were 110 ± 10 ng m−3 and 1400 ± 400 particles cm−3, respectively. Mean values above 500 m were 150 ± 30 ng m−3 and 1000 ± 350 particles cm−3, respectively. Group medians of maximum eBC and NP concentrations in vertical profiles with temperature inversions were significantly higher than for those without inversion. The dataset has been complemented by continuous ground measurements of eBC with an average value of 208 ± 130 ng m−3 (median value 110 ± 70 ng m−3) for the entire campaign; the ground-based background (absence of local emission) eBC value was below 100 ng m−3 while maximum values were in the 1000–2000 ng m−3 range. Median eBC concentration measured at ground for 2 h before the tethered balloon launch was higher when temperature inversion was observed. The ground-based measurements, coupled with aerosol optical depth measurements, allowed for a preliminary discussion of two case studies related to high pollutants concentration events.

Abstract

During the Multidisciplinary drifting Observatory for the Study of Arctic Climate (MOSAiC) expedition, the Balloon-bornE moduLar Utility for profilinG the lower Atmosphere (BELUGA) was deployed from an ice floe drifting in the Fram Strait from 29 June to 27 July 2020. The BELUGA observations aimed to characterize the cloudy Arctic atmospheric boundary layer above the sea ice using a modular setup of five instrument packages. The in situ measurements included atmospheric thermodynamic and dynamic state parameters (air temperature, humidity, pressure, and three-dimensional wind), broadband solar and terrestrial irradiance, aerosol particle microphysical properties, and cloud particle images. In total, 66 profile observations were collected during 33 balloon flights from the surface to maximum altitudes of 0.3 to 1.5 km. The profiles feature a high vertical resolution of 0.01 m to 1 m, including measurements below, inside, and above frequently occurring low-level clouds. This publication describes the balloon operations, instruments, and the obtained data set. The present study aimed to evaluate if a logistic reorganization (generic outpatient department, OPD vs dedicated clinic, NRU) and a change in management (dedicated vs non dedicated staff) of the follow-up of children between 6 and 24 months of age with acute malnutrition, can reduce the default, relapse and readmission rate and increase the recovery rate., Methods: Retrospective observational study on the impact of quality improvement interventions on rehabilitation outcomes of children (6-24 months) with acute malnutrition, admitted at the Catholic Mission Hospital of Chiulo (Angola) from January 2018 to February 2020. Main outcome measures were recovery rate, the default rate, the relapse rate, and the readmission rate., Results: The intervention was associated with a decrease in the default rate from 89 to 76% (p = 0.02). Recovery rate was 69% in OPD and 88% in NRU (p = 0.25). Relapse rate was nil., Conclusions: The present study supports the hypothesis that an improvement in quality of care can positively influence the rehabilitation outcomes of malnourished children. Further studies are needed to identify children at risk of low adherence to follow-up visits to increase the effectiveness of rehabilitation programs., (© 2022. The Author(s).)

26. Molecular Mechanisms Related to Oxidative Stress in Retinitis Pigmentosa.

Abstract

Retinitis pigmentosa (RP) is an inherited retinopathy. Nevertheless, non-genetic biological factors play a central role in its pathogenesis and progression, including inflammation, autophagy and oxidative stress. The retina is particularly affected by oxidative stress due to its high metabolic rate and oxygen consumption as well as photosensitizer molecules inside the photoreceptors being constantly subjected to light/oxidative stress, which induces accumulation of ROS in RPE, caused by damaged photoreceptor's daily recycling. Oxidative DNA damage is a key regulator of microglial activation and photoreceptor degeneration in RP, as well as mutations in endogenous antioxidant pathways involved in DNA repair, oxidative stress protection and activation of antioxidant enzymes ( MUTYH , CERKL and GLO1 genes, respectively). Moreover, exposure to oxidative stress alters the expression of micro-RNA (miRNAs) and of long non-codingRNA (lncRNAs), which might be implicated in RP etiopathogenesis and progression, modifying gene expression and cellular response to oxidative stress. The upregulation of the P2X7 receptor (P2X7R) also seems to be involved, causing pro-inflammatory cytokines and ROS release by macrophages and microglia, contributing to neuroinflammatory and neurodegenerative progression in RP. The multiple pathways analysed demonstrate that oxidative microglial activation may trigger the vicious cycle of non-resolved neuroinflammation and degeneration, suggesting that microglia may be a key therapy target of oxidative stress in RP.

Abstract

Background: Dietary counseling can play an important role in managing child malnutrition but is often inadequate or absent. Moreover, little emphasis is given to the usefulness of local available foods in the rehabilitation of malnourished children. This study aimed to evaluate the adherence and effectiveness of nutritional education during rehabilitation of children (6-24 months) with acute malnutrition, in a setting of unavailability of therapeutic/supplementary foods., Methods: Retrospective observational study on the adherence to dietary counseling and the impact on growth in children 6-24 months who were referred for acute malnutrition at the Catholic Mission Hospital of Chiulo (Angola) from August 2018 to January 2019. Main outcome measures were change in dietary habits and growth gain., Results: Sixty-four out of 120 children returned at first follow-up visit (default rate 47%). A change in dietary habits was reported in 32/64 (50%) children. Changing dietary habits was associated with an improved change in weight gain (MD 9.3 g/kg/day, 95%CI 4.2 to 14.3; p = 0.0005) and in weight/height ratio (MD 1.1 SD, 95%CI 0.7 to 1.4; p < 0.0001)., Conclusions: A change in dietary habits after discharge was noted in only half of the patients who returned at first follow up visit, but it provided some advantages in term of weight gain and weight/height ratio. Further studies are needed to identify children at risk of low adherence to follow-up visits and low compliance to the nutritional recommendations, in order to increase the effectiveness of rehabilitation programs.

Abstract

Objective: (1) To report the number and type of emotional expressions (cues/concerns) raised by breast cancer patients; (2) to identify the influence of setting, patient characteristics, and doctor-patient interaction on emotional expression., Methods: 308 Italian-speaking female patients were recruited at their first breast cancer consultation. The visits were audio-recorded and analysed for number and type of emotional expressions (VR-CoDES). Oncologists' interaction skills were rated by the VR-COPE. Socio-demographic, clinical and personality variables were gathered before the consultation. Clinical variables and oncologists' evaluations of the patient were collected after., Results: Breast cancer patients raised emotional issues mainly as cues. The setting (centre where the consultations took place), the oncologist's attribution of anxiety, regardless of anxiety test screening (STAI-X1) score before the consultation, and the oncologist's ability to pick up on patient's worries, handle emotional needs or understand psychosocial condition were all positively related with the number of emotional expressions. More tightly structured consultations had fewer emotional expressions., Conclusion: Both contextual and interactional aspects have an impact on patient emotional expressions., Practice Implications: Oncologists need to be trained to manage both the content and the process of medical consultation. 29. Analysis of modal coupling due to birefringence and ellipticity in strongly guiding ring-core OAM fibers.

Abstract

After briefly recalling the issue of OAM mode purity in strongly-guiding ring-core fibers, this paper provides a methodology to calculate the coupling strength between OAM mode groups due to fiber perturbations. The cases of stress birefringence and core ellipticity are theoretically and numerically investigated. It is found that both perturbations produce the same coupling pattern among mode groups, although with different intensities. The consequence is that birefringence causes the highest modal crosstalk because it strongly couples groups with a lower propagation-constant mismatch. The power coupling to parasitic TE and TM modes is also quantified for both perturbations and is found to be non-negligible. Approximate modal crosstalk formulas valid for weakly-guiding multi-core fibers, but whose parameters are adapted to the present case of strongly guiding OAM fibers, are found to provide a reasonable fit to numerical results. Finally, the effect that modal coupling has on OAM transmission is assessed in terms of SNR penalty.

Abstract

Objective: Sub-standard quality in X-ray image acquisition and interpretation is common in low-resource countries, and can ultimately result in higher patient morbidity and mortality. This study aimed at evaluating; 1) feasibility of implementing a digital X-ray device in a second level hospital in Angola; 2) quality of digital X-ray images, when digital radiology was in the hands of local technicians; 3) feasibility of digital teleradiology and its potential impact on case management., Methods: We developed and tested at the Hospital Divina Providencia (HDP) in Luanda, Angola, a digital X-ray device and a telemedicine network to acquire and print digital X-ray images and send them as DICOM files for remote consultation., Results: 20,564 digital X-ray images were made at HDP from November 2010 to December 2012, with no major technical problems and no need for on-site supervision. Digital radiology largely improved the number of X-ray images of good and very good quality (100% of images with digital radiology, compared to 15% of screen-film images, p<0.0001). Teleradiology using digital images was used in 7.6% of paediatric cases, and provided, in these cases, an important contribution to case management., Conclusions: The implementation of a digital X-ray device is feasible in low resource settings with significant improvement in quality of X-ray images compared to standard screen film radiology.

Abstract

Background: Antiretroviral therapy is effective in reducing rates of mother-to child transmission of HIV to low levels in resource-limited contexts but the applicability and efficacy of these programs in the field are scarcely known. In order to explore such issues, we performed a descriptive study on retrospective data from hospital records of HIV-infected pregnant women who accessed in 2007-2010 the Luanda Municipal Hospital service for prevention of mother-to-child transmission (PMTCT). The main outcome measure was infant survival and HIV transmission. Our aim was to evaluate PMTCT programme in a local hospital setting in Africa., Results: Data for 104 pregnancies and 107 infants were analysed. Sixty-eight women (65.4%) had a first visit before or during pregnancy and received combination antiretroviral treatment (ART) in pregnancy. The remaining 36 women (34.6%) presented after delivery and received no ART during pregnancy. Across a median cohort follow-up time of 73 weeks, mortality among women with and without ART in pregnancy was 4.4% and 16.7%, respectively (death hazard ratio: 0.30, 95% CI 0.07-1.20, p = 0.089). The estimated rates of HIV transmission or death in the infants over a median follow up time of 74 weeks were 8.5% with maternal ART during pregnancy and 38.9% without maternal ART during pregnancy. Following adjustment for use of oral zidovudine in the newborn and exposure to maternal milk, no ART in pregnancy remained associated with a 5-fold higher infant risk of HIV transmission or death (adjusted odds ratio: 5.13, 95% CI: 1.31-20.15, p = 0.019)., Conclusions: Among the women and infants adhering to the PMTCT programme, HIV transmission and mortality were low. However, many women presented too late for PMTCT, and about 20% of infants did not complete follow up. This suggests the need of targeted interventions that maintain the access of mothers and infants to prevention and care services for HIV.
