1. Recent progress of the development of a long pulse 140GHz ECRH system on EAST

Abstract

A long pulse ECRH system with a goal of 140GHz 4MW 100~1000s has been developed to meet the requirement of steady-state operation on EAST. Gycom gyrotrons are employed in the No.1 and No.3 systems, CPI gyrotrons are used in the No.2 and No.4 systems. The development of the two Gycom gyrotron systems has been finished. The first short pulse EC wave injection has been demonstrated successfully during the EAST 2015 Spring campaign. In the commissioning and operation towards steady-state operation, 0.4MW 100s has been injected to plasma successfully by using the No.1 system, 4.7keV 102s L-mode and 102s H-mode plasma have been achieved on EAST with the help of ECRH. Recently, a longest pulse of 0.55MW 1000s has been obtained based on calorimetric dummy load measurements on the No.3 gyrotron. The No.2 gyrotron also has been installed and partially tested, 500kW 80s has been demonstrated in the dummy load. The remaining No.4 gyrotron will be ready to test in 2018 or 2019. The whole 4MW system will be completed within two years. The 400s fully non-inductive H-mode operation would be expected in the next four years in the condition of fully tungsten diverter on EAST.

The gyrotron installation on the DIII-D tokamak has been in operation at the second harmonic of the electron cyclotron resonance since the mid-1990s. Prior to that a large installation of ten 60 GHz tubes was operated at the fundamental resonance. The system has been upgraded regularly and is an everyday tool for experiments on DIII-D.

Abstract

The mixed zinc-ferrite spinel magnetic nanoparticles (MNPs) with the general formula ZnxFe3−xO4 are among the most extensively studied families of Fe oxides due to their interesting and diverse chemical, electronic, and magnetic properties. These systems offer the possibility of surface functionalization and possess high biocompatibility, making them highly attractive for applications in biomedicine, such as magnetic fluid hyperthermia (MFH). The efficiency of the MFH process relies on the magnetic, structural and morphological properties of the MNPs. The substitution with the Zn ion and the cationic distribution, as well as the synthesis process employed, have a direct impact on the final properties of these oxides. Therefore, it is essential to have tools that enable a comprehensive characterization of the system to assess its performance in MFH. In this study, we have synthesized four ZnxFe3−xO4 MNP systems using three different methods: two by thermal decomposition at high temperatures, one by co-precipitation, and another by co-precipitation followed by ball milling. We analyze the effect of these various synthesis processes on the magnetic and crystallographic properties, aiming to correlate them with the response of each system in MFH. Neutron diffraction data are employed to determine the cation site occupation and to investigate the correlation with the synthesis method. MFH measurements were conducted in media of diverse viscosities, revealing different values of specific loss power, thus demonstrating a clear dependence on the synthesis process and Zn content. Abstract

The DIII-D ECH complex is being upgraded with three new depressed collector gyrotrons. The performance of the existing system has been very good. As more gyrotrons having higher power are added to the system, diagnostics of gyrotron operation, optimization of the performance and qualification of components for higher power become more important. A new FPGA-based gyrotron control system is being installed, additional capabilities for rapid real time variation of the rf injection angles by the DIII-D Plasma Control System are being tested and infrastructure enhancements are being completed. Longer term plans continue to include ECH as a major component in the DIII-D heating and current drive capabilities.

Abstract

In KSTAR device, a 110 GHz ECH system has been a useful heating device for a stable plasma start-up because a pure ohmic discharge scenario with a limited loop voltage of about 4 V was sometimes not successful for burn-through and plasma current ramp-up due to inconsistent wall conditioning and density control. Even though a pure ohmic discharge also was successful, the application of X2-mode ECH could reduce the flux consumption of poloidal field coils, leading to long pulse discharges. The ECH power was injected at the time of the field null formation after the onset of the toroidal electric field in which the electron temperature significantly increased up to 100 eV so that burn-through is overcome. The ECH heating enabled the formation of close flux surfaces earlier, leading to the reliable plasma current ramp-up, but, it caused outward plasma movement and failure of the plasma control, resulting in loss of the discharge. Moreover, impurities from the plasma facing components caused by not fully absorbed ECH power had a detrimental effect on the H-mode transition. In ECH-assisted start-up in the ramp-up phase, ECH power was mainly used for central electron heating, leading to reduction of the flux consumption of the central coils by increasing the electron temperature along with a density increase in the ramp-up phase. When X2-mode ECH power of 350 kW was injected at the mid-plane with a toroidal angle of 10 degree for 1 sec after the onset of the loop voltage, the flux consumption was reduced by about 30% in comparison with the pure ohmic discharges.

Abstract

Non-rotating (`locked') magnetic islands often lead to complete losses of confinement in tokamak plasmas, called major disruptions. Here locked islands were suppressed for the first time, by a combination of applied three-dimensional magnetic fields and injected millimetre waves. The applied fields were used to control the phase of locking and so align the island O-point with the region where the injected waves generated non-inductive currents. This resulted in stabilization of the locked island, disruption avoidance, recovery of high confinement and high pressure, in accordance with the expected dependencies upon wave power and relative phase between O-point and driven current., Comment: 4 figures

Abstract

Pancreatic exocrine insufficiency (PEI) is defined as a reduction in pancreatic exocrine secretion below the level that allows the normal digestion of nutrients. Pancreatic disease and surgery are the main causes of PEI. However, other conditions and upper gastrointestinal surgery can also affect the digestive function of the pancreas. PEI can cause symptoms of nutritional malabsorption and deficiencies, which affect the quality of life and increase morbidity and mortality. These guidelines were developed following the United European Gastroenterology framework for the development of high-quality clinical guidelines. After a systematic literature review, the evidence was evaluated according to the Oxford Center for Evidence-Based Medicine and the Grading of Recommendations Assessment, Development, and Evaluation methodology, as appropriate. Statements and comments were developed by the working groups and voted on using the Delphi method. The diagnosis of PEI should be based on a global assessment of symptoms, nutritional status, and a pancreatic secretion test. Pancreatic enzyme replacement therapy (PERT), together with dietary advice and support, are the cornerstones of PEI therapy. PERT is indicated in patients with PEI that is secondary to pancreatic disease, pancreatic surgery, or other metabolic or gastroenterological conditions. Specific recommendations concerning the management of PEI under various clinical conditions are provided based on evidence and expert opinions. This evidence-based guideline summarizes the prevalence, clinical impact, and general diagnostic and therapeutic approaches for PEI, as well as the specifics of PEI in different clinical conditions. Finally, the unmet needs for future research are discussed.

The core content for a medical specialty outlines the scope of the discipline as well as the categories of knowledge considered essential to practice in the field. It provides a template for the development of curricula for medical school, graduate, and postgraduate education, as well as for creating certification standards. Venous and Lymphatic Medicine (VLM) is a specialty that has benefitted from contributions from specialists from several medical disciplines. Optimally, the societies, boards, and residency review committees representing these disciplines would uniformly recognize the scope of VLM to develop education and assessment standards to allow training and identification of qualified practitioners. In order to inform the standard setting bodies and other stakeholders of the current scope of VLM, a task force of VLM experts from cardiology, dermatology, emergency medicine, general surgery, interventional radiology, vascular medicine, and vascular surgery was formed to revise a 2014 consensus document defining the core content of the specialty of VLM. 13. Too hard to control: compromised pain anticipation and modulation in mild traumatic brain injury.

Abstract

Mild traumatic brain injury (MTBI) is a vulnerability factor for the development of pain-related conditions above and beyond those related to comorbid traumatic and emotional symptoms. We acquired functional magnetic resonance imaging (fMRI) on a validated pain anticipation task and tested the hypotheses that individuals with a reported history of MTBI, compared with healthy comparison subjects, would show increased brain response to pain anticipation and ineffective pain modulation after controlling for psychiatric symptoms. Eighteen male subjects with a reported history of blast-related MTBI related to combat, and eighteen healthy male subjects with no reported history of MTBI (healthy controls) underwent fMRI during an event-related experimental pain paradigm with cued high or low intensity painful heat stimuli. No subjects in either group met diagnostic criteria for current mood or anxiety disorder. We found that relative to healthy comparison subjects, after controlling for traumatic and depressive symptoms, participants with a reported history of MTBI showed significantly stronger activations within midbrain periaqueductual grey (PAG), right dorsolateral prefrontal cortex and cuneus during pain anticipation. Furthermore, we found that brain injury was a significant moderator of the relationship between anticipatory PAG activation and reported subjective pain. Our results suggest that a potentially disrupted neurocognitive anticipatory network may result from damage to the endogenous pain modulatory system and underlie difficulties with regulatory pain processing following MTBI. In other words, our findings are consistent with a notion that brain injury makes it more difficult to control acute pain. Understanding these mechanisms of dysfunctional acute pain processing following MTBI may help shed light on the underlying causes of increased vulnerability for the development of pain-related conditions in this population.

Abstract

Neutral beam current drive (NBCD) experiments in DIII-D using vertically shifted plasmas to move the current drive away from the axis have clearly demonstrated robust off-axis NBCD. Time-dependent measurements of magnetic field pitch angles by the motional Stark effect diagnostic are used to obtain the evolution of the poloidal magnetic flux, which indicates a broad off-axis NBCD profile with a peak at about half the plasma minor radius. In most cases, the measured off-axis NBCD profile is consistent with calculations using an orbit-following Monte Carlo code for the beam ion slowing down including finite-orbit effects provided there is no large-scale magnetohydrodynamic activity such as Alfv́n eigenmodes modes or sawteeth. An alternative analysis method shows good agreement between the measured pitch angles and those from simulations using transport-equilibrium codes. Two-dimensional image of Doppler-shifted fast ion Dα light emitted by neutralized energetic ions shows clear evidence for a hollow profile of beam ion density, consistent with classical beam ion slowing down. The magnitude of off-axis NBCD is sensitive to the alignment of the beam injection relative to the helical pitch of the magnetic field lines. If the signs of toroidal magnetic field and plasma current yield the proper helicity, both measurement and calculation indicate that the efficiency is as good as on-axis NBCD because the increased fraction of trapped electrons reduces the electron shielding of the injected ion current, in contrast with electron current drive schemes where the trapping of electrons degrades the efficiency. The measured off-axis NBCD increases approximately linearly with the injection power, although a modest amount of fast ion diffusion is needed to explain an observed difference in the NBCD profile between the measurement and the calculation at high injection power. © 2009 American Institute of Physics.

Abstract

Modification of the two existing DIII-D neutral beamlines is planned to allow vertical steering to provide off-axis neutral beam current drive (NBCD) peaked as far off-axis as half the plasma minor radius. New calculations for a downward-steered beam indicate strong current drive with good localization off-axis so long as the toroidal magnetic field, BT, and the plasma current, Ip, point in the same direction. This is due to good alignment of neutral beam injection (NBI) with the local pitch of the magnetic field lines. This model has been tested experimentally on DIII-D by injecting equatorially mounted NBs into reduced size plasmas that are vertically displaced with respect to the vessel midplane. The existence of off-axis NBCD is evident in the changes seen in sawtooth behaviour in the internal inductance. By shifting the plasma upwards or downwards, or by changing the sign of the toroidal field, off-axis NBCD profiles measured with motional Stark effect data and internal loop voltage show a difference in amplitude (40-45%) consistent with differences predicted by the changed NBI alignment with respect to the helicity of the magnetic field lines. The effects of NBI direction relative to field line helicity can be large even in ITER: off-axis NBCD can be increased by more than 30% if the BT direction is reversed. Modification of the DIII-D NB system will strongly support scenario development for ITER and future tokamaks as well as provide flexible scientific tools for understanding transport, energetic particles and heating and current drive. © 2009 IAEA, Vienna.

Abstract

Neutral beam injection into reversed magnetic shear DIII-D plasmas produces a variety of Alfvénic activity including toroidicity and ellipticity induced Alfvén eigenmodes (TAE/EAE, respectively) and reversed shear Alfvén eigenmodes (RSAE) as well as their spatial coupling. These modes are studied during the discharge current ramp phase when incomplete current penetration results in a high central safety factor and strong drive due to multiple higher order resonances. It is found that ideal MHD modelling of eigenmode spectral evolution, coupling and structure are in excellent agreement with experimental measurements. It is also found that higher radial envelope harmonic RSAEs are clearly observed and agree with modelling. Some discrepancies with modelling such as that due to up/down eigenmode asymmetries are also pointed out. Concomitant with the Alfvénic activity, fast ion (FIDA) spectroscopy shows large reductions in the central fast ion profile, the degree of which depends on the Alfvén eigenmode amplitude. Interestingly, localized electron cyclotron heating (ECH) near the mode location stabilizes RSAE activity and results in significantly improved fast ion confinement relative to discharges with ECH deposition on axis. In these discharges, RSAE activity is suppressed when ECH is deposited near the radius of the shear reversal point and enhanced with deposition near the axis. The sensitivity of this effect to deposition power and current drive phasing as well as ECH modulation are presented. © 2009 IAEA, Vienna.

Reversed shear Alfvén eigenmode (RSAE) activity in DIII-D is stabilized by electron cyclotron heating (ECH) applied near the minimum of the magnetic safety factor (qmin) in neutral beam heated discharges with reversed-magnetic shear. The degree of RSAE stabilization, fast ion density and the volume averaged neutron production (Sn) are highly dependent on ECH deposition location relative to qmin. While discharges with ECH stabilization of RSAEs have higher Sn and more peaked fast ion profiles than discharges with significant RSAE activity, neutron production remains strongly reduced (up to 60% relative to TRANSP predictions assuming classical fast ion transport) even when RSAEs are stabilized. © 2008 IOP Publishing Ltd.

Abstract

Advanced Tokamak research in DIII-D seeks to develop a scientific basis for steady-state high-performance tokamak operation. Fully noninductive (fNI ≈ 100%) in-principle steady-state discharges have been maintained for several confinement times. These plasmas have weak negative central shear with qmin ≈ 1.5-2, βN ≈ 3.5, and large, well-aligned bootstrap current. The loop voltage is near zero across the entire profile. The remaining current is provided by neutral beam current drive (NBCD) and electron cyclotron current drive (ECCD). Similar plasmas are stationary with fNI ≈ 90-95% and duration up to 2 s, limited only by hardware. In other experiments, βN ≈ 4 is maintained for 2 s with internal transport barriers, exceeding previously achieved performance under similar conditions. This is allowed by broadened profiles and active magnetohydrodynamic instability control. Modifications now underway on DIII-D are expected to allow extension of these results to higher performance and longer duration. A new pumped divertor will allow density control in high triangularity double-null divertor configurations, facilitating access to similar in-principle steady-state regimes with βN > 4. Additional current drive capabilities, both off-axis ECCD and on-axis fast wave current drive (FWCD), will increase the magnitude, duration, and flexibility of externally driven current. © 2006 Elsevier B.V. Abstract

The DIII-D Advanced Tokamak (AT) program in the DIII-D tokamak [J. L. Luxon, Plasma Physics and Controlled Fusion Research, 1986, Vol. I (International Atomic Energy Agency, Vienna, 1987), p. 159] is aimed at developing a scientific basis for steady-state, high-performance operation in future devices. This requires simultaneously achieving 100% noninductive operation with high self-driven bootstrap current fraction and toroidal beta. Recent progress in this area includes demonstration of 100% noninductive conditions with toroidal beta, ΒT =3.6%, normalized beta, ΒN =3.5, and confinement factor, H89 =2.4 with the plasma current driven completely by bootstrap, neutral beam current drive, and electron cyclotron current drive (ECCD). The equilibrium reconstructions indicate that the noninductive current profile is well aligned, with little inductively driven current remaining anywhere in the plasma. The current balance calculation improved with beam ion redistribution that was supported by recent fast ion diagnostic measurements. The duration of this state is limited by pressure profile evolution, leading to magnetohydrodynamic (MHD) instabilities after about 1 s or half of a current relaxation time (τCR). Stationary conditions are maintained in similar discharges (∼90% noninductive), limited only by the 2 s duration (1 τCR) of the present ECCD systems. By discussing parametric scans in a global parameter and profile databases, the need for low density and high beta are identified to achieve full noninductive operation and good current drive alignment. These experiments achieve the necessary fusion performance and bootstrap fraction to extrapolate to the fusion gain, Q=5 steady-state scenario in the International Thermonuclear Experimental Reactor (ITER) [R. Aymar, Fusion Energy Conference on Controlled Fusion and Plasma Physics, Sorrento, Italy (International Atomic Energy Agency, Vienna, 1987), paper IAEA-CN-77/OV-1]. The modeling tools that have been successfully employed to both plan and interpret the experiment are used to plan future DIII-D experiments with higher power and longer pulse ECCD and fast wave and co- and counterneutral beam injection in a pumped double-null configuration. The models predict our ability to control the current and pressure profiles to reach full noninductivity with increased beta, bootstrap fraction, and duration. The same modeling tools are applied to ITER, predicting favorable prospects for the success of the ITER steady-state scenario. © 2006 American Institute of Physics.

21. A functional magnetic resonance imaging study of cortical asymmetry in bipolar disorder

Abstract

Objectives: Individuals with bipolar disorder (BPD) exhibit motor, perceptual, and cognitive disturbances involving predominantly right hemisphere dysfunction. This asymmetry has been used to advance the hypothesis that the pathogenesis of bipolar disorder may be related to disturbances of the right cerebral hemisphere. We employed functional magnetic resonance imaging to examine hemispheric asymmetries in manic and depressed BPD. A secondary goal of the Study was to examine effects of psychotropic medications on blood Volume changes in the motor cortices. Methods: We studied 18 right-handed BPD and 13 right-handed normal healthy comparison subjects. Blood oxygen level dependent (BOLD) responses in the primary motor area (M1) and Supplementary motor area (SMA) of both hemispheres were elicited during reaction time (RT) tasks. Results: Healthy subjects activated the SMA in a reciprocal fashion with significantly greater activity in the left SMA for right hand trials and the right SMA for left hand trials. Depressed BPD subjects failed to show this normal reciprocity indicating a failure to Suppress unwanted activity in the ipsilateral right SMA, whereas manic BPD subjects failed to suppress unwanted ipsilateral SMA activity in both hemispheres. Manic and depressed BPD subjects exhibited greater activity in the left primary motor area suggesting increased cortical excitability. BPD subjects treated with antipsychotics or mood-stabilizing medications exhibited longer RTs, lower BOLD responses in M1 and SMA, and a loss of normal hemispheric asymmetry in the SMA than untreated subjects. Conclusions: The presence of a right hemisphere disturbance in BPD is consistent with the hypothesis that the right hemisphere may be dominant in mood regulation. The presence of both left and right hemisphere disturbances in mania may explain the coexisting psychotic and affective symptoms observed in this condition.

Abstract

Significant progress in obtaining high performance discharges lasting many energy confinement times in the DIII-D tokamak has been realized in recent experimental campaigns. Normalized performance ∼10 has been sustained for more than 5>E with qmin > 1.5. (The normalized performance is measured by the product βNH89, indicating the proximity to the conventional β limits and energy confinement quality, respectively.) These H mode discharges have an ELMing edge and β < 5%. The limit to increasing β is a resistive wall mode, rather than the tearing modes as previously observed. Confinement remains good despite qmin > 1. The global parameters were chosen to optimize the potential for fully non-inductive current sustainment at high performance, which is a key program goal for the DIII-D facility. Measurement of the current density and loop voltage profiles indicate that ≈75% of the current in the present discharges is sustained non-inductively. The remaining ohmic current is localized near the half-radius. The electron cyclotron heating system is being upgraded to replace this remaining current with ECCD. Density and β control, which are essential for operating advanced tokamak discharges, were demonstrated in ELMing H mode discharges with βNH89 ≈ 7 for up to 6.3 s or ≈34τE. These discharges appear to have stationary current profiles with qmin ≈ 1.05 in agreement with the current profile relaxation time ≈1.8 s.

Abstract

Advanced tokamaks seek to achieve a high bootstrap current fraction without sacrificing fusion power density or fusion gain. Good progress has been made towards the DIII-D research goal of demonstrating a high-β advanced tokamak plasma in steady state with a relaxed, fully non-inductive current profile and a bootstrap current fraction greater than 50%. The limiting factors for transport, stability, and current profile control in advanced operating modes are discussed in this paper.

Abstract

Fusion power has been increased by a factor of 3 in DIII-D by tailoring the pressure profile to avoid the kink instability in H-mode plasmas. The resulting plasmas are found to have neoclassical ion confinement. This reduction in transport losses in beam-heated plasmas with negative central shear is correlated with a dramatic reduction in density fluctuations. Improved magnetohydrodynamic stability is achieved by controlling the plasma pressure profile width. In deuterium plasmas the highest gain Q (the ratio of fusion power to input power), was 0.0015, corresponding to an equivalent Q of 0.32 in a deuterium-tritium plasma. © 1996 The American Physical Society.

Abstract

Introduction: Immunoglobulin G4-related disease (IgG4-RD) is an immune-mediated condition associated with fibroinflammatory lesions that can occur at almost any anatomical site. It often presents as a multiorgan disease that may mimic malignancy, infection, or other immune-mediated conditions. Autoimmune pancreatitis (AIP) type 1 is the most prominent manifestation of IgG4-RD in the digestive tract, with common extra-pancreatic inflammation. We present the first patient with AIP and involvement of the testicles and nasal cavity. Patient and methods: A case of a patient with AIP type 1 and other organ involvement (bile ducts, testicles, nasal polyps, and lungs) is described. Additionally, a systematic review of AIP type 1 with testicular and nasal involvement was conducted. Results: The systematic review found two cases of AIP type 1 with testicular involvement and 143 cases with AIP type 1 with nasal cavity involvement. None of them had both testicular and nasal involvement. Conclusions: This is the first case of AIP type 1 with other organ involvement, including testicular and nasal involvement, to be described. The number of patients with nasal and testicular involvement described in the literature is low. Abstract

Recent results from the DIII-D tokamak have provided significant contributions to the understanding of many of the elements of tokamak physics and the application of this understanding to the design of next generation devices including ITER and CIT. The limitations of magnetohydrodynamic stability on the values of plasma beta (the ratio of kinetic pressure to the containing pressure of the magnetic field) that can be attained has been experimentally demonstrated and found to be described by existing theory. Values of beta (10.7%) well in excess of those required for proposed devices (ITER and CIT) have been demonstrated. Regimes of confinement (H-mode) have been established that scale favorably to proposed next generation devices, and experiments demonstrating the dependence of the energy confinement on plasma size have been completed. Understanding of confinement is rapidly developing especially in the areas of bulk transport and the role of turbulence in the plasma edge. Key experimental results in areas of plasma transport and edge plasma phenomena are in agreement with theories based on short wavelength turbulence. Control of the divertor heat loads and impurity influx has been demonstrated, and new progress has been made in the understanding of plasma edge phenomena. Experiments with ion Bernstein wave heating have not found regimes in which these waves can produce effective central ion heating. Electron cyclotron current drive experiments have demonstrated 70 kA of driven current in 400 kA discharges.

Abstract

DIII-D physics research addresses critical challenges for the operation of ITER and the next generation of fusion energy devices. This is done through a focus on innovations to provide solutions for high performance long pulse operation, coupled with fundamental plasma physics understanding and model validation, to drive scenario development by integrating high performance core and boundary plasmas. Substantial increases in off-axis current drive efficiency from an innovative top launch system for EC power, and in pressure broadening for Alfven eigenmode control from a co-/counter-I p steerable off-axis neutral beam, all improve the prospects for optimization of future long pulse/steady state high performance tokamak operation. Fundamental studies into the modes that drive the evolution of the pedestal pressure profile and electron vs ion heat flux validate predictive models of pedestal recovery after ELMs. Understanding the physics mechanisms of ELM control and density pumpout by 3D magnetic perturbation fields leads to confident predictions for ITER and future devices. Validated modeling of high-Z shattered pellet injection for disruption mitigation, runaway electron dissipation, and techniques for disruption prediction and avoidance including machine learning, give confidence in handling disruptivity for future devices. For the non-nuclear phase of ITER, two actuators are identified to lower the L-H threshold power in hydrogen plasmas. With this physics understanding and suite of capabilities, a high poloidal beta optimized-core scenario with an internal transport barrier that projects nearly to Q = 10 in ITER at ∼8 MA was coupled to a detached divertor, and a near super H-mode optimized-pedestal scenario with co-I p beam injection was coupled to a radiative divertor. The hybrid core scenario was achieved directly, without the need for anomalous current diffusion, using off-axis current drive actuators. Also, a controller to assess proximity to stability limit

Abstract

DIII-D physics research addresses critical challenges for the operation of ITER and the next generation of fusion energy devices. This is done through a focus on innovations to provide solutions for high performance long pulse operation, coupled with fundamental plasma physics understanding and model validation, to drive scenario development by integrating high performance core and boundary plasmas. Substantial increases in off-axis current drive efficiency from an innovative top launch system for EC power, and in pressure broadening for Alfven eigenmode control from a co-/counter-I (p) steerable off-axis neutral beam, all improve the prospects for optimization of future long pulse/steady state high performance tokamak operation. Fundamental studies into the modes that drive the evolution of the pedestal pressure profile and electron vs ion heat flux validate predictive models of pedestal recovery after ELMs. Understanding the physics mechanisms of ELM control and density pumpout by 3D magnetic perturbation fields leads to confident predictions for ITER and future devices. Validated modeling of high-Z shattered pellet injection for disruption mitigation, runaway electron dissipation, and techniques for disruption prediction and avoidance including machine learning, give confidence in handling disruptivity for future devices. For the non-nuclear phase of ITER, two actuators are identified to lower the L-H threshold power in hydrogen plasmas. With this physics understanding and suite of capabilities, a high poloidal beta optimized-core scenario with an internal transport barrier that projects nearly to Q = 10 in ITER at similar to 8 MA was coupled to a detached divertor, and a near super H-mode optimized-pedestal scenario with co-I (p) beam injection was coupled to a radiative divertor. Abstract

DIII-D physics research addresses critical challenges for the operation of ITER and the next generation of fusion energy devices. This is done through a focus on innovations to provide solutions for high performance long pulse operation, coupled with fundamental plasma physics understanding and model validation, to drive scenario development by integrating high performance core and boundary plasmas. Substantial increases in off-axis current drive efficiency from an innovative top launch system for EC power, and in pressure broadening for Alfven eigenmode control from a co-/counter-I p steerable off-axis neutral beam, all improve the prospects for optimization of future long pulse/steady state high performance tokamak operation. Fundamental studies into the modes that drive the evolution of the pedestal pressure profile and electron vs ion heat flux validate predictive models of pedestal recovery after ELMs. Understanding the physics mechanisms of ELM control and density pumpout by 3D magnetic perturbation fields leads to confident predictions for ITER and future devices. Validated modeling of high-Z shattered pellet injection for disruption mitigation, runaway electron dissipation, and techniques for disruption prediction and avoidance including machine learning, give confidence in handling disruptivity for future devices. For the non-nuclear phase of ITER, two actuators are identified to lower the L-H threshold power in hydrogen plasmas. With this physics understanding and suite of capabilities, a high poloidal beta optimized-core scenario with an internal transport barrier that projects nearly to Q = 10 in ITER at ∼8 MA was coupled to a detached divertor, and a near super H-mode optimized-pedestal scenario with co-I p beam injection was coupled to a radiative divertor. Abstract

Background Paraduodenal pancreatitis (PDP) is a particular form of chronic pancreatitis (CP) occurring in and around the duodenal wall. Despite its low prevalence, this rare condition presents a significant challenge in clinical practice. Methods We retrospectively analysed the electronic medical charts of all patients with a diagnosis of chronic pancreatitis and identified those with PDP, between January 1999 and December 2020. Results There were 35 patients diagnosed with PDP (86% males and 14% females); median age of 56 +/- 11 (range 38-80). Alcohol overconsumption was reported in 81% and smoking in 90% of patients. Abdominal pain was the leading symptom (71%), followed by weight loss, nausea and vomiting, jaundice, and diarrhoea. In 23 patients (66%), recurrent acute pancreatitis attacks were noted. Focal duodenal wall thickening was present in 34 patients (97%), cystic lesions in 80%, pancreatic duct dilatation in 54% and common bile duct dilatation in 46%. Endoscopic treatment was performed on nine patients (26%) and five patients (14%) underwent surgery. Complete symptom relief was reported in 12 patients (34%), partial symptom relief in three (9%), there was no improvement in five (14%), data were not available in three (9%) and 12 (34%) patients died before data analysis. Conclusions PDP is a rare form of pancreatitis, most commonly occurring in the 5th or 6th decade of life, with a predominance in males and patients with a history of smoking and high alcohol consumption. Focal thickening and cystic lesions of the duodenal wall are the most common imaging findings, followed by pancreatic duct and common bile duct dilatation. A minority of patients requires surgery.

IgG4-related digestive disease can be diagnosed only with a comprehensive work-up that includes histology, organ morphology at imaging, serology, search for other organ involvement, and response to glucocorticoid treatment. Indications for treatment are symptomatic patients with obstructive jaundice, abdominal pain, posterior pancreatic pain, and involvement of extra-pancreatic digestive organs, including IgG4-related cholangitis. Treatment with glucocorticoids should be weight-based and initiated at a dose of 0.6-0.8 mg/kg body weight/day orally (typical starting dose 30-40 mg/day prednisone equivalent) for 1 month to induce remission and then be tapered within two additional months. Response to initial treatment should be assessed at week 2-4 with clinical, biochemical and morphological markers. Maintenance treatment with glucocorticoids should be considered in multi-organ disease or history of relapse. If there is no change in disease activity and burden within 3 months, the diagnosis should be reconsidered. If the disease relapsed during the 3 months of treatment, immunosuppressive drugs should be added.
